RESUMO Introdução Poucos estudos realizaram, concomitantemente, o potencial evocado auditivo de tronco encefálico (PEATE) e o P300 na Síndrome de Down (SD), em indivíduos audiologicamente normais, para a avaliação da via auditiva central, principalmente pela dificuldade de realizar estes procedimentos nesta população. Estudos anteriores sugeriram que indivíduos com SD podem apresentar padrões de respostas diferentes das encontradas em indivíduos com desenvolvimento típico, sendo que a identificação destes seria fundamental para o estabelecimento de um diagnóstico audiológico preciso. Objetivo Caracterizar o PEATE e o P300 em indivíduos com SD audiologicamente normais. Método Foram analisados o PEATE e o P300 de 17 indivíduos com SD e 21 com desenvolvimento típico de sete a 15 anos. A análise foi quantitativa e qualitativa, utilizando medidas descritivas e os testes de hipótese. Resultados Os valores de latência foram menores no PEATE para o grupo SD, com diferença estatisticamente significante para a onda V, interpicos III-V e I-V; não foram encontradas diferenças significantes nos valores de latência do P300. Observou-se maior número de indivíduos com valores precoces para as latências do PEATE e com latências atrasadas para o P300 no grupo SD; ambas as comparações mostraram diferenças significantes. Conclusão Crianças e adolescentes com SD podem apresentar respostas precoces para os componentes do PEATE, sugerindo que a via auditiva destes necessita de menor tempo para a transmissão neural do estímulo acústico até o tronco encefálico. Quanto ao P300, indivíduos com SD podem apresentar latências aumentadas, sugerindo comprometimento na via auditiva central quanto ao processamento cortical da informação auditiva.

ABSTRACT Introduction Few studies have performed Brainstem (BAEP) and P300 Auditory Evoked Potentials simultaneously to assess central auditory pathways in normal hearing individuals with Down syndrome (DS), mainly because of the difficulty in applying these procedures to this population. Previous studies have suggested that individuals with DS might present different patterns of response compared with those of individuals with typical development; nevertheless, the identification of these potentials would be crucial for the establishment of an accurate audiological diagnosis. Purpose To characterize BAEP and P300 in normal-hearing individuals with DS. Methods BAEP and P300 were analyzed in 17 individuals with DS and in 21 individuals with typical development aged 7 to 15 years. The results were quantitatively and qualitatively analyzed using descriptive measures and hypothesis tests. Results In the quantitative analysis, latency values were lower in the BAEP for the DS group, with statistically significant difference for wave V and interpeaks III-V and I-V; there were no significant differences in the P300 latency values. In the qualitative analysis, there were a larger number of individuals with early values for BAEP latencies and late latencies for P300 in the DS group; both comparisons showed statistically significant differences. Conclusion Children and adolescents with DS can present early responses to the components of BAEP, suggesting that their auditory pathway requires less time for the neural transmission of acoustic stimuli to the brainstem. Concerning P300, individuals with DS may present increased latencies, suggesting impairment in the central auditory pathway for the cortical processing of auditory information.

RESUMO OBJETIVO: Analisar a literatura científica sobre alterações alimentares em crianças prematuras durante os primeiros anos de vida. FONTE DOS DADOS: A base de dados do Pubmed foi a precursora para a seleção dos artigos. Os textos foram analisados quanto aos seus objetivos, desenhos da pesquisa e características do grupo pesquisa e foram selecionados os seguintes temas para compor os critérios: (1) publicações de 1996 a 2014; (2) participação de bebês e crianças, do nascimento até os 10 anos; (3) desenvolvimento das habilidades motoras orais necessárias para a alimentação; (4) desenvolvimento do processo de alimentação e (5) dificuldades alimentares durante a infância. SÍNTESE DOS DADOS: Foram identificados 282 estudos, 17 foram usados e cinco artigos foram identificados por meio da lista de referência bibliográfica dos artigos selecionados, totalizando 22 referências bibliográficas. CONCLUSÃO: A revisão permitiu concluir que recém-nascidos pré-termo muito baixo peso (RNPTMBP) são mais propensos a apresentar problemas de alimentação nos estágios iniciais pós-natal e durante a infância quando comparados com crianças a termo. O monitoramento da alimentação após a alta hospitalar é estritamente recomendado em um programa de intervenção precoce com vistas ao melhor desenvolvimento das habilidades alimentares.

ABSTRACT OBJECTIVE: To analyze the scientific literature on dietary changes in preterm children during the first years of life. DATA SOURCE: The PubMed database was used for article selection. The texts were analyzed according to their objectives, research design, and research group characteristics. The following were selected to comprise the criteria: (1) publications in the period from 1996 to 2014; (2) participation of infants and children from birth to 10 years of age; (3) development of oral motor skills necessary for feeding; (4) development of the feeding process; and (5) feeding difficulties during childhood. SUMMARY OF THE FINDINGS: There were 282 studies identified, of which 17 were used in the review, and five more articles were identified through the reference list of selected articles, totaling 22 references. CONCLUSION: Very low birth weight preterm newborns are more likely to have feeding problems in early postnatal stages and during childhood when compared with full-term infants. Monitoring the feeding of these infants after hospital discharge is strictly recommended in an early intervention program aiming at better development of feeding skills.

<sec><title>Objetivos:</title><p> Verificar a acurácia da avaliação clínica da deglutição comparada à videofluoroscopia na detecção de penetração laríngea isolada e aspiração laríngea em crianças com suspeita de disfagia; identificar os sinais e sintomas clínicos associados à presença de penetração laríngea isolada e aspiração laríngea; e determinar a sensibilidade e a especificidade dos sinais e sintomas clínicos identificados.</p></sec><sec><title>Métodos:</title><p> Análise retrospectiva de dados de avaliações clínicas e videofluoroscópicas realizadas em 55 crianças de 1 mês a 7 anos e 11 meses de idade. Na avaliação clínica foi utilizado o Protocolo de Avaliação Clínica da Disfagia Pediátrica. Foram analisados: sensibilidade, especificidade e valores preditivos positivo e negativo da avaliação clínica. Também foram utilizados os testes exato de Fisher e do χ²na análise estatística.</p></sec><sec><title>Resultados:</title><p>A avaliação clínica apresentou, no geral, sensibilidade de 86% e especificidade de 32%. Para penetração laríngea isolada, a avaliação clínica apresentou sensibilidade de 88%. Para aspiração laríngea, a avaliação clínica apresentou sensibilidade de 86%. Contudo, os valores da especificidade foram baixos para ambas as alterações. Não houve associação significativa entre a avaliação clínica e os achados videofluoroscópicos. O engasgo foi o único sinal clínico associado à penetração laríngea isolada com líquido fino e apresentou sensibilidade de 53% e especificidade de 77%.</p></sec><sec><title>Conclusões:</title><p>A avaliação clínica foi sensível para detectar penetração laríngea isolada e aspiração laríngea em crianças com suspeita de disfagia, porém, apresentou baixa especificidade. O engasgo foi o único sinal clínico associado à penetração laríngea isolada com líquido fino. São necessários mais estudos prospectivos que confirmem esses achados nessa população.</p></sec>

<sec><title>Purpose:</title><p> To verify the accuracy of clinical evaluation compared with videofluoroscopic swallowing studies in the detection of isolated laryngeal penetration and laryngeal aspiration in children with suspected dysphagia; to identify clinical signs and symptoms associated with isolated laryngeal penetration and laryngeal aspiration; and to determine the sensitivity and specificity of the clinical signs and symptoms identified.</p></sec><sec><title>Methods:</title><p>Retrospective analysis of data from clinical and videofluoroscopic evaluations carried out in 55 children from 1 month to 7 years and 11 months old. For clinical assessment, the Protocol for Clinical Assessment of Pediatric Dysphagia was used. The sensitivity, specificity, and positive and negative predictive values of clinical evaluation were analyzed. For statistical analysis, the Fisher's exact and χ² tests were used.</p></sec><sec><title>Results:</title><p>Clinical evaluation showed, in general, a sensitivity of 86% and a specificity of 32%. For isolated laryngeal penetration, clinical evaluation showed a sensitivity of 88%. For laryngeal aspiration, clinical evaluation showed a sensitivity of 86%. However, the specificity values were low for both alterations. There was no association between clinical evaluation and videofluoroscopic findings. Choking was the only clinical sign associated with isolated laryngeal penetration thin fluid and showed a sensitivity of 53% and a specificity of 77%.</p></sec><sec><title>Conclusions:</title><p> Clinical evaluation was sensible to detect isolated laryngeal penetration and laryngeal aspiration in children with suspected dysphagia. However, it showed a low specificity. Choking was the only clinical sign associated with isolated laryngeal penetration of thin fluid. More prospective studies are needed to confirm these findings in this population.</p></sec>

Objetivo Identificar como a função da deglutição tem sido avaliada por meio da ultrassonografia (USG). Estratégia de pesquisa Esta revisão da literatura levantou publicações científicas internacionais sobre a USG e seu uso na avaliação da deglutição, por meio da base de dados PubMed. Foi realizada a localização e seleção dos estudos através de levantamento de textos publicados sobre o assunto, no período de janeiro de 2002 a agosto de 2013, limitando-se a estudos em seres humanos, no idioma inglês. Critérios de seleção Foram excluídos aqueles repetidos por sobreposição das palavras-chave, estudos de caso, revisões de literatura, cartas ao editor e os não relacionados diretamente à temática. Resultados Foram identificados 17 estudos que corresponderam aos critérios de inclusão. Observou-se que mais da metade dos estudos avaliou a deglutição de indivíduos adultos saudáveis, sem preferência por nenhum dos gêneros. Os parâmetros adotados para a análise das imagens não foram unânimes, havendo variação considerável entre os estudos. Conclusão A USG da deglutição demonstrou ser um método rápido, não invasivo, de baixo custo, que fornece parâmetros objetivos sobre a deglutição e que pode ser realizado em beira de leito, uma vez que o equipamento costuma ser de fácil manuseio e transporte.

Purpose To identify how the deglutition function has been evaluated using ultrasound (US). Research strategy This literature review used the PubMed database to survey international scientific publications about US and its use to evaluate deglutition. Studies were located and selected by surveying relevant articles published between January 2002 and August 2013. The survey was limited to studies on humans published in English. Selection criteria Repeated studies (determined by overlapping keywords), case studies, literature reviews, letters to the editor and studies not directly related to the topic were excluded. Results A total of 17 studies matching the inclusion criteria were identified. More than half of the studies evaluated the deglutition of healthy adults with no preference for gender. The parameters adopted for image analysis were not unanimous, and there was considerable variation among studies. Conclusion US proved to be a fast, non-invasive, low-cost method for evaluating objective parameters of deglutition. As a further advantage, US may be performed at bedside because the equipment is typically easy to handle and transport.

O cleft laríngeo (CL) é uma malformação congênita que resulta em uma incomum comunicação entre esôfago e o complexo laringotraqueal. É uma doença rara, de maior prevalência no gênero masculino. O objetivo deste estudo foi relatar a atuação fonoaudiológica em um caso de um paciente neonato com diagnóstico de CL tipo I, admitido no Berçário do Hospital Universitário da Universidade de São Paulo, em sua segunda internação por baixo ganho ponderal e quadro de pneumonia. Foi realizada a avaliação clínica da disfagia infantil em beira de leito, cuja principal ocorrência foi a presença de engasgos frequentes. O exame de videofluoroscopia da deglutição evidenciou presença de aspiração laringotraqueal escore 8 para líquido fino e escore 1 para líquido engrossado, segundo a Escala de Penetração-Aspiração. O neonato foi submetido a microlaringoscopia, na qual foi constatada a presença de CL tipo I. Após o diagnóstico, a conduta fonoaudiológica foi de engrossar a fórmula láctea a 6% e, em oito dias, o neonato recebeu alta com dieta exclusiva por via oral, com ausência de engasgos. Foram realizadas oito consultas ambulatoriais durante 11 meses de acompanhamento, com ênfase na reintrodução de líquidos finos. A paciente recebeu alta fonoaudiológica com dieta geral para a idade, sem modificações. Durante todo o gerenciamento, a paciente permaneceu assintomática e não apresentou quadros respiratórios.

Laryngeal cleft (LC) is a congenital malformation that leads to the unusual communication between the esophagus and the laryngotracheal complex. It is a rare disease, mostly prevalent among male individuals. The goal of this study was to describe the evaluation and intervention by the speech language pathologist of a female newborn diagnosed with LC type I, admitted on the University Hospital of Universidade de São Paulo, in her second hospitalization due to small weight gain and pneumonia. She was submitted to a bedside clinical evaluation of the swallowing and the most important occurrence was frequent gagging. The videofluoroscopy swallowing study showed laryngotracheal aspiration level 8 for thin liquid and level 1 for thickened liquid, according to the Penetration-Aspiration Scale. The newborn was submitted to a microlaryngoscopy, in which the presence of LC type I was found. After the diagnosis, the speech language pathologist offered thickened liquid at 6% and, in 8 days, the newborn was discharged with exclusive oral diet without gagging. Eight outpatient consultations were carried out for 11 months, with emphasis on reintroduction of thin liquids. The treatment was discontinued and the patient was put on general diet for the age without modifications. Throughout follow-up, the patient remained asymptomatic and showed no respiratory complications.

Objetivo Caracterizar o sistema auditivo periférico de indivíduos com síndrome de Down, por meio da audiometria convencional e de altas frequências. Métodos Estudo do tipo transversal e observacional. Participaram 15 indivíduos com síndrome de Down, de ambos os gêneros, entre 7 e 15 anos de idade. Foram realizados os seguintes procedimentos: Meatoscopia, Timpanometria com pesquisa do reflexo acústico ipsilateral e contralateral, Audiometria Tonal, Audiometria Vocal e Audiometria de Altas Frequências. Resultados Houve predomínio de perda auditiva condutiva de grau leve, em uma ou ambas as orelhas. As médias dos limiares auditivos para a audiometria convencional ficaram abaixo de 20 dBNA e, para a audiometria de altas frequências, ficaram entre 20 e 40 dBNA. O coeficiente de correlação de Pearson revelou correlação moderada positiva, entre os limiares de 9 a 14 kHz e a idade. Conclusão De forma geral, não foram observadas diferenças significativas, quando comparadas as orelhas direita e esquerda de indivíduos com síndrome de Down, na audiometria tonal, imitanciometria e logoaudiometria. A maioria das crianças apresentou alteração de orelha média e perda auditiva condutiva. A audiometria de altas frequências sugere o início de prejuízo da função coclear, que pode estar associado às otites médias frequentes e/ou à degeneração coclear precoce.

Purpose This study sought to characterize the peripheral auditory system of individuals with Down syndrome (DS) using conventional and high-frequency audiometry. Methods We performed a cross-sectional and observational study. Fifteen individuals with DS, who were of both genders and between 7 and 15 years of age, participated in this study. The following procedures were performed: otoscopy, tympanometry with ipsilateral and contralateral acoustic reflex, pure-tone audiometry, vocal audiometry and high-frequency audiometry. Results There was a predominance of mild conductive hearing loss in one or both ears. The mean hearing thresholds for conventional audiometry were below 20 dB HL and between 20 and 40 dB HL for high-frequency audiometry. The Pearson correlation coefficient indicated a moderate positive correlation between the 9-14 kHz thresholds and age. Conclusion Overall, no significant differences were observed when comparing the right and left ears of individuals with DS, in regards to pure-tone audiometry, immittance testing and speech audiometry. Most children showed middle ear abnormalities and conductive hearing loss. Moreover, high-frequency audiometry suggested the onset of impaired cochlear function, which may be associated with frequent otitis media episodes and/or early cochlear degeneration.

Objetivo apresentar e discutir o uso de comunicação suplementar e/ou alternativa como instrumento terapêutico no trabalho com a morfossintaxe em adolescentes com síndrome de Down. Métodos estudo piloto que acompanhou longitudinalmente (doze meses) quatro adolescentes com idade cronológica entre 13 e 15 anos e idade mental entre cinco e seis anos. Foram consideradas as avaliações inicial e final, filmadas durante 30 minutos em situação de interação livre entre participante e cuidador. As amostras de fala foram analisadas a partir do cálculo da EME, quanto morfemas e palavras, para comparação entre as avaliações. Durante a intervenção terapêutica, todos os participantes foram submetidos ao mesmo procedimento. Foi utilizado o Picture Communication Symbols (PCS), com os símbolos ordenados seguindo uma estrutura sintática simples (substantivo + verbo + complemento), ampliada gradativamente,na seguinte ordem de elementos: artigos, preposições, pronomes (pessoais, possessivos e demonstrativos) e conjunções. Cada sessão terapêutica foi registrada em protocolo específico. Foi realizado o teste de Wilcoxon e o nível de significância adotado foi de 5%. Resultados apesar de não ter sido obtida diferença estatisticamente significante, houve aumento do número de morfemas e de palavras entre as duas avaliações, representados pelos elementos trabalhados, incluindo preposições e conjunções, além de substantivos e verbos. Para pronomes pessoais foi verificada tendência à significância (p=0,068). Conclusão o sistema de comunicação adotado mostrou-se uma ferramenta apropriada ao favorecimento do emprego de estruturas frasais com o uso de elementos com função sintática, como artigos, preposições e conjunções, de difícil utilização por indivíduos com síndrome de Down.

Purpose to present and to discuss the use of augmentative and alternative communication as a therapeutic tool with the morphosyntax with adolescents with Down syndrome. Methods this pilot study monitored during 12 months four adolescents chronologically aged between 13 and 15 years and mentally aged between 5 and 6 years. It were analyzed the initial and the final assessments, situations of free interaction between the participant and the caregiver videotaped during 30 minutes. The speech samples were analyzed as MLU values considering morphemes and words, comparing both the two situations. All of the participants were submitted to the same procedures during the therapeutic interventions. Picture Communication Symbols (PCS) has been used and the symbols were organized according to a simple syntactic structure (substantive + verb + complement) with a gradual extension, and the elements were rated at the following order: articles, prepositions, pronouns (personals, possessives and demonstratives), and conjunctions. Each therapeutic session was recorded in specific protocol. It was used the Wilcoxon test and the p value considered was 5%. Results spites there were no statistical significant differences, increase in number of morphemes and words used between the both assessments was observed. This improve was represented by the worked elements, including prepositions and conjunctions beyond substantives and verbs. For personal pronouns there was tendency to significance (p=0,068). Conclusion the communication system showed to be a therapeutic tool to improve the use of syntactic elements in statement structures, as articles, prepositions and conjunctions, which use for individuals with Down syndrome are difficult.

OBJETIVO: Descrever o desempenho linguístico de crianças com síndrome de Down falantes do Português Brasileiro por meio da análise da Extensão Média do Enunciado, comparar esse desempenho ao de crianças com Distúrbio Específico de Linguagem e com Desenvolvimento Típico e verificar se as crianças com síndrome de Down apresentam atrasos ou desvios do desenvolvimento linguístico. MÉTODO: Participaram do estudo 25 crianças com síndrome de Down (grupo pesquisa), pareadas pela idade mental ao grupo controle de crianças com desenvolvimento típico e ao grupo controle de crianças com Distúrbio Específico de Linguagem. Os participantes foram divididos em subgrupos de acordo com a faixa etária (três, quatro e cinco anos). Foram colhidas amostras de fala do grupo pesquisa e realizadas análises por meio da Extensão Média do Enunciado medida em morfemas e em palavras. RESULTADOS: Observaram-se diferenças de desempenhos entre o grupo pesquisa e os grupos controle, sendo que o primeiro apresentou valores de Extensão Média do Enunciado inferiores em todas as faixas etárias, o que caracteriza o atraso do desenvolvimento gramatical e linguístico geral. CONCLUSÃO: A descrição das habilidades linguísticas de crianças com síndrome de Down falantes do Português Brasileiro apontou para déficits gramaticais importantes, principalmente no que se refere ao uso de palavras funcionais.

PURPOSE: To describe the linguistic performance of Brazilian Portuguese-speaking children with Down syndrome by analyzing their Mean Length Utterance; to compare their performance to that of children with Specific Language Impairment and Typical Development; and to verify whether children with Down syndrome present developmental language delay or disorder. METHOD: Participants were 25 children with Down syndrome (Research Group), matched by mental age to a Control Group of typically developing children, and to a Control Group of children with Specific Language Impairment. Participants were divided into subgroups, according to age range (three, four and five years). Speech samples were collected for the Research Group, and the Mean Length Utterance was analyzed for morphemes and words. RESULTS: Differences were observed between the performance of the Research Group and both Control Groups, and the former presented inferior Mean Length Utterance values for all age ranges, characterizing a delay in grammar and general language development. CONCLUSION: The description of the linguistic abilities of Brazilian Portuguese-speaking children with Down syndrome indicated important grammatical deficits, especially regarding the use of functional words.

Este estudo objetivou pesquisar artigos científicos internacionais publicados sobre a ausculta cervical e sua utilização na prática fonoaudiológica. Trata-se de estudo de revisão qualitativa da literatura, envolvendo os últimos 10 anos. Os artigos foram selecionados da base de dados PubMed, utilizando os descritores: auscultation cervical, deglutition e deglutition disorders. A pesquisa deveria ter sido realizada com seres humanos, adultos (maiores que 18 anos) e escrita no idioma inglês. Cada citação recuperada no banco de dados foi analisada independentemente pelos pesquisadores do estudo, visando à pertinência de sua seleção e de sua inclusão ou não no estudo. A metodologia empregada envolveu a formulação da pergunta, a localização e seleção dos estudos, e a avaliação crítica dos artigos, conforme os preceitos do Cochrane Handbook. Como resultados, foram identificados 35 estudos, dos quais 13 foram analisados, pois permitiram acesso ao texto completo e relacionavam-se diretamente ao tema. Concluiu-se que os estudos foram realizados com grupos de sujeitos saudáveis e sujeitos com diferentes tipos de patologia de base. Alguns estudos compararam os padrões encontrados nos diferentes grupos. Constatou-se que parte dos artigos buscou estudar o padrão dos sons da deglutição, com diferentes interfaces: a experiência dos avaliadores; especificidade e sensibilidade do método; e como aprimorar a técnica da ausculta cervical por meio da utilização de outros instrumentos, diferentes do estetoscópio. Com essa análise crítica, concluiu-se que utilização da ausculta cervical é importante como um instrumento utilizado juntamente de outros, na rotina da avaliação clínica da deglutição.

This study aimed to investigate international scientific papers published on the subject of cervical auscultation and its use in speech therapy. The study involved a qualitative review of the literature spanning the last 10 years. Articles were selected from the PubMed database using the following keywords: cervical auscultation, swallowing and swallowing disorders. Research was included that was conducted on adult humans (over 18 years of age) and was written in English. Each citation retrieved from the database was analyzed independently by each of the study researchers to ascertain its relevance for inclusion in the study. The methodology involved formulating the research question, locating and selecting studies and critically evaluating the articles according to the precepts of the Cochrane Handbook. As a result, 35 studies were identified; 13 articles were analyzed because they allowed access to the full text and were related directly to the subject. We found that the studies were performed with groups of healthy subjects and subjects with different types of base pathology. Some studies compared the patterns found in the different groups. Some of the research sought to study the pattern of swallowing sounds with different factors - evaluator experience, the specificity and sensitivity of the method and how to improve the technique of cervical auscultation through the use of instruments other than the stethoscope. The conclusion of this critical analysis is that cervical auscultation is an important tool to be used in conjunction with other assessment methods in the routine clinical evaluation of swallowing.

OBJETIVO: Verificar a utilização de substantivos e verbos por crianças com síndrome de Down e comparar esse uso em situações de interação com o terapeuta e com a mãe. MÉTODOS: Participaram 21 crianças com idades entre 5 e 11 anos, divididas em 3 grupos igualitários com base nas idades cronológica e mental, a partir da aplicação do Primary Test of Nonverbal Intelligence. A coleta de fala foi feita a partir de situações de interação livre, gravadas em vídeo por período de 30 minutos e transcritas em protocolo específico, sendo utilizados os primeiros 100 enunciados, a partir dos 5 minutos iniciais. O tempo de intervalo entre cada situação foi de 7 a 15 dias. A análise estatística utilizou o teste-t pareado e a análise de variância (ANOVA). Foi adotado o valor de significância de 5%. RESULTADOS: Foram produzidos mais verbos do que substantivos nas duas situações de interação, sendo mais verbos e substantivos com o terapeuta do que com a mãe. A comparação entre os grupos na interação com o terapeuta mostrou diferença tanto para substantivos quanto para verbos e, com a mãe, apenas tendência à diferença para os verbos. CONCLUSÃO: Os dados apontam para crescente desenvolvimento no uso de substantivos e de verbos de acordo com o aumento da idade. Houve maior utilização de verbos que de substantivos, principalmente na situação de interação com o terapeuta.

PURPOSE: To verify the use of nouns and verbs by children with Down syndrome and to compare this use between conditions of interaction with the mother and the Speech-language pathologist (SLP). METHODS: Participants were 21 children aged between 5 and 11 years, divided into three groups, according to chronological and mental age as established by the results of the Primary Tests of Nonverbal Intelligence. The speech sample was obtained through free interaction situations that were videotaped during session of 30 minutes and transcribed in specific protocols. The first 100 utterances from the first five minutes were used. The interval between each situation ranged from 7 to 15 days. T-test and analysis of variance were used for statistical analysis and the significance level adopted was of 5%. RESULTS: More verbs than nouns were used in both conditions; however, a higher number of nouns were observed during the interaction with the SLP. The between-group comparison in the interaction with the SLP shows significant differences for verbs and nouns, but during the interaction with the mother, there was tendency for difference only for the verbs. CONCLUSION: The data indicate the growing development on using of nouns and verbs according to the increase of age. There was a higher use of verbs when compared with nouns mainly in the condition of interaction with the SLP.

O objetivo da presente revisão de literatura foi analisar artigos científicos internacionais publicados sobre a fisiologia da deglutição de alimentos líquidos nas fases oral e faríngea. A metodologia empregada envolveu a formulação da pergunta; localização e seleção dos estudos; avaliação crítica dos artigos; conforme os preceitos do Cochrane Handbook. Foram identificados 185 artigos, dos quais se excluiu 141 por não relacionarem-se diretamente ao tema e analisou-se 29 estudos. As pesquisas estão fortemente relacionadas às formas de identificação de disfagia e não as características proporcionadas pela deglutição de diferentes consistências. Quanto à metodologia empregada nos artigos analisados observa-se que na maioria dos estudos não há grupo-controle. Os grupos estudados são heterogêneos, principalmente quando considerando indivíduos com alterações neurológicas, além disso, não há pareamento de idade na maioria dos estudos. Dessa forma, os achados desta revisão demonstram que há dificuldade na aplicabilidade clínica dos achados científicos, dificultando a prática baseada evidências.

The purpose of the present literature review was to analyze international scientific papers published on the physiology of swallowing fluids in the oral and pharyngeal phases. The employed methodology involved the formulation of the question, location and selection of studies, critical assessment of the manuscripts, according to the concepts of the Cochrane Handbook. We identified 185 articles, out of which 141 were excluded for not being directly related to the theme. Twenty-nine studies were analyzed. The researchers are strongly related to ways of identifying dysphagia and not the features offered by swallowing various consistencies. Regarding the methodology employed in the reviewed articles it is observed that in most studies there are no control groups. The studied groups are heterogeneous, especially when considering individuals with neurological disorders. In addition, subjects are not paired by age. Thus, the findings of this review indicate that there is great difficulty for clinicians to apply in their daily practice scientific findings, which in turn makes evidence-based practice very limited.

OBJETIVO: Levantar artigos científicos internacionais sobre a fisiologia da deglutição de alimentos nas consistências néctar, mel, pudim, pastosa heterogênea, semissólida e sólida, nas fases oral e faríngea. MÉTODOS: Trata-se de estudo de revisão qualitativa da literatura. Para a seleção dos artigos, foi utilizada a base de dados PubMed com emprego dos descritores "Swallowing and consistency", "Swallowing and solid" e "Swallowing and pasty", limitando-se a pesquisas publicadas no idioma inglês, entre os anos de 2005 e 2010, realizadas com seres humanos maiores de 18 anos. A metodologia empregada envolveu formulação da pergunta, localização e seleção dos estudos, e avaliação crítica dos artigos, conforme os preceitos do Cochrane Handbook. RESULTADOS: Foram identificados 211 estudos, dos quais 18 foram analisados, pois permitiam acesso ao texto completo e eram diretamente relacionados ao tema. CONCLUSÃO: Os estudos apresentam metodologia pouco uniforme, não havendo padronização, principalmente quanto aos métodos de avaliação. Em geral, as pesquisas foram realizadas com sujeitos saudáveis ou remeteram a um tipo de patologia, sem utilização de casos-controle. A heterogeneidade dos estudos possibilita que diferentes grupos de patologias sejam avaliados, porém, a variabilidade metodológica dificulta a definição e generalização dos padrões encontrados. Sendo assim, não é possível evidenciar dados que embasem a prática clínica fonoaudiológica no que diz respeito à fisiologia normal ou alterada da deglutição de diferentes consistências alimentares, tanto para sujeitos normais quanto para os acometidos por alguma desordem.

PURPOSE: To analyze published international scientific papers on the physiology of deglutition in oral and pharyngeal phases, considering different food consistencies: nectar, honey, pudding, pasty heterogeneous, semi-solid, and solid. METHODS: This is a qualitative literature reviews. The studies considered were selected on PubMed, using the keywords "Swallowing and consistency", "Swallowing and solid", and "Swallowing and pasty", limiting the search to manuscripts published in English in the period between 2005 and 2010, and conducted with human beings over 18 years old. The methodology involved question formulation, location and selection of studies, and critical analyses of the manuscripts, according to the concepts of the Cochrane Handbook. RESULTS: Two hundred and eleven studies were identified, out of which only 18 allowed access to the full text and were directly related to the theme. CONCLUSION: The studies presented very few similarities between the applied methodologies, especially when considering assessment methods. Overall, the studies were conducted with healthy individuals or with a specific pathology, without presenting the comparison with control cases. The heterogeneity of studies allows the investigation of different swallowing disorders. However, methodological variability makes it difficult to define and generalize the identified swallowing patterns. For this reason, it is not possible to identify parameters on which to base the clinical practice of speech-language therapists, especially when considering the normal or altered physiology of swallowing different food consistencies.

OBJETIVO: Avaliar a qualidade de vida (QV) de pais/cuidadores de crianças e adolescentes com síndrome de Down (SD) e a influência de aspectos sócio-demográficos nos resultados obtidos. MÉTODOS: Participaram 31 pais/cuidadores de crianças e adolescentes com SD, que foram divididos em três grupos: G1, cuidadores de dez crianças pré-escolares (idades entre 1 ano e 5 anos e 11 meses); G2, cuidadores de 11 crianças em idade escolar (idades entre 6 anos e 10 anos e 11 meses); e G3, cuidadores de dez pré-adolescentes e adolescentes (idades entre 11 anos a 15 anos e 11 meses). Dados de caracterização dos pais/cuidadores: a maioria estava na faixa etária de 40-49 anos; o grau de escolaridade Ensino Médio Completo foi o mais freqüente, seguido de Ensino Fundamental Incompleto e Ensino Superior Completo; quanto à classe econômica, a concentração foi nas classes C e B2. Foi aplicado o questionário de QV WHOQOL-bref. RESULTADOS: Dos pais/cuidadores pesquisados, 84% avaliaram sua QV como "boa" e 55% afirmaram se sentir "satisfeitos" com sua saúde. O domínio Meio Ambiente apresentou menor média quando comparado aos demais e apresentou correlação com as variáveis sócio-demográficas referentes ao "grau de instrução" e ao "nível socioeconômico". Não houve diferença entre os grupos na comparação das faixas etárias. CONCLUSÃO: Os dados obtidos apontam que a população estudada avalia sua QV como "boa" e está "satisfeita" com sua saúde. O domínio Meio Ambiente e as variáveis sócio-demográficas "grau de instrução" e "nível socioeconômico" são os aspectos que influenciam sua QV.

PURPOSE: To assess the quality of life (LQ) of parents/caregivers of children and adolescents with Down syndrome (DS), as well as the influence of socio-demographic aspects on the results obtained. METHODS: Participants were 31 parents/caregivers of children and adolescents with DS, divided into three groups: G1, caregivers of ten preschoolers (ages between 1 year and 5 years and 11 months); G2, caregivers of 11 school-aged children (ages between 6 years and 10 years and 11 months); and, G3, caregivers of ten pre-teens and adolescents (ages between 11 years and 15 years and 11 months). Parents/caregivers characterization: most of them were between 40 and 49 years old; high-school was the most common education level, followed by incomplete elementary school and college education; concerning the socio-economic class, most of them were from classes C and B2. The QL WHOQOL- bref protocol was administered. RESULTS: Eighty-four percent of the parents/caregivers rated their QL as "good", and 55% reported to be "satisfied" with their health. The lowest average score was found for the Environment domain, which was found to be correlated with socio-demographic variables "education degree" and "socio-economic level". No difference was found between groups when age ranges were compared. CONCLUSION: The results suggest that the studied population consider their QL as "good", and is "satisfied" with their health. The Environment domain and the socio-demographic variables "education degree" and "socio-economic level" are the aspects that influence their quality of life.

OBJETIVO: Analisar literatura científica sobre fisiologia e efeitos dos exercícios fonoaudiológicos utilizados nos tratamentos de alterações da Motricidade Orofacial. ESTRATÉGIA DE PESQUISA: Foram seguidos os preceitos do Cochrane Handbook que envolveu a formulação da questão a ser investigada, a localização e seleção dos estudos e a avaliação crítica dos artigos. CRITÉRIOS DE SELEÇÃO: Os artigos foram selecionados por meio da base de dados PubMed, utilizando-se os seguintes descritores: "physiology exercise AND speech, language and hearing science", "exercise physiology AND speech therapy", "exercise physiology AND myofunctional therapy", e "physiology exercise AND swallowing therapy". Foram incluídos somente artigos de língua inglesa publicados no período de 2000 a 2010. ANÁLISE DOS DADOS: Os textos foram analisados quanto aos seus objetivos; desenho da pesquisa; características do grupo pesquisa; existência de grupo controle; critérios de avaliação; terapêutica proposta; resultados encontrados; e verificação da existência de discussão sobre a fisiologia muscular dos exercícios. RESULTADOS: Foram identificados 108 estudos e destes, 38 foram analisados, pois permitiam acesso ao texto completo e relacionavam-se ao tema proposto. Os artigos foram classificados em: ensaios clínicos e pesquisas experimentais, estudos de casos, revisões de literatura e textos teóricos, cartas ao editor e críticas aos artigos. CONCLUSÃO: A revisão permite concluir que o conhecimento sobre os efeitos musculares dos exercícios empregados pelos clínicos é pouco aprofundado. Ainda não há evidência cientifica suficiente para determinar a frequência em que estes devem ser realizados. Em geral, tem-se pesquisado a eficácia de programas terapêuticos, sem investigar se os exercícios propostos são efetivos individualmente.

PURPOSE: To analyze the scientific literature about the physiology and effects of exercises used in the treatment of oral myofunctional disorders. RESEARCH STRATEGY: The methodology used followed the concepts of the Cochrane Handbook and involved question formulation related to the topic of investigation, identification and selection of the studies, and a critical evaluation of the selected articles. SELECTION CRITERIA: Papers were selected on PubMed database using the following keyword combinations: "physiology exercise AND speech, language and hearing science", "exercise physiology AND speech therapy", "exercise physiology AND myofunctional therapy", and "physiology exercise AND swallowing therapy". Only papers written in English and published between the years of 2000 and 2010 were included in the analysis. DATA ANALYSIS: Manuscripts were analyzed according to their objectives, research design, participants, inclusion of a control group, assessment criteria, therapeutic proposal, results and existence about physiology of the chosen exercises. RESULTS: One hundred and eight studies were identified, out of which 38 had access to the full text and were directly related to the investigated topic. The articles were classified as clinical trials and experimental research, case studies, literature reviews and theoretical articles, letters to the editor and critical analyses. CONCLUSION: This review concluded that there is a lack of knowledge about the effects of the myofunctional exercises used by clinicians. Also there is a lack of scientific evidence to determine the frequency at which they should be performed. Generally, the articles investigate the efficacy of treatment programs without inquiring whether the included exercises are individually effective.

OBJETIVO: Caracterizar a extensão média de enunciados em morfemas (EME-m) e palavras (EME-p) produzida por crianças com síndrome de Down (SD) e verificar a eficácia da utilização da EME-p como medida do desenvolvimento linguístico geral de crianças com SD. MÉTODOS: Participaram 15 crianças com SD, com idades entre cinco e 12 anos, que foram submetidas à situação de interação livre. As crianças foram divididas em três grupos, com base na idade cronológica e mental, a partir da aplicação do Primary Test of Nonverbal Intelligence. Os 100 primeiros enunciados foram analisados quanto a: número de morfemas gramaticais para artigos, substantivos e verbos (MG-1) e pronomes, preposições e conjunções (MG-2); extensão média dos enunciados considerando-se morfemas (EME-m) e palavras (EME-p). RESULTADOS: A comparação intergrupos mostrou que quanto maior a idade, mais altas foram as médias obtidas, havendo diferença para todas as variáveis, com exceção de MG-2. Os mesmos resultados foram obtidos na comparação intragrupo para todas as variáveis. Houve forte correlação entre EME-m e EME-p. CONCLUSÃO: A EME-p pode ser utilizada como medida de identificação de desenvolvimento linguístico geral. No entanto, ressalta-se que a utilização de todas as variáveis relacionadas à extensão média de enunciados fornece maior eficiência na identificação do desenvolvimento linguístico e na análise de suas alterações.

PURPOSE: To characterize the mean length utterance in morphemes (MLU-m) and words (MLU-w) produced by children with Down syndrome (DS), and to verify the effectiveness of using EME-w as a measure of general language development of children with DS. METHODS: Participants were 15 children with ages between 5 and 12 years, who were submitted to a free interaction situation. They were divided into three groups, according to chronological and mental age, as established by the results of the Primary Test of Nonverbal Intelligence. The first 100 utterances were analyzed considering: number of grammatical morphemes (GM) for articles, nouns and verbs (GM-1), and pronouns, prepositions and conjunctions (GM-2); mean length utterance for morphemes (MLU-m) and words (MLU-w). RESULTS: The between-groups comparison showed that the MLU averages were higher for older groups, and differences were found for all variables, except for GM-2. The same results were obtained in the within-group comparison, for all variables. There was a strong correlation between MLU-m and MLU-w. CONCLUSION: MLU-w can be used as an identification measure of general linguistic development. However, it is emphasized that the use of all MLU variables provides more efficacy in the characterization of linguistic development and the analysis of language impairments.
