Resumo Introdução: O acolhimento é uma prática presente nas relações de cuidado, entre trabalhadores de saúde e usuários, nos atos de receber e escutar as pessoas, sendo um mecanismo de ampliação e facilitação do acesso. Objetivos: Compreender, na percepção dos enfermeiros que atuam na Atenção Primária à Saúde, como ocorre o processo de acolhimento nas Unidades Básicas de Saúde da Região Norte do Distrito Federal. Métodos: Trata-se de um estudo descritivo, exploratório de cunho qualitativo, realizado com 33 enfermeiros. A coleta de dados ocorreu no período de 10 de outubro a 30 de novembro de 2022. Os dados foram analisados por meio da análise de conteúdo de Bardin: pré-análise, exploração do material e tratamento dos resultados, inferência e interpretação. Resultados: Foi possível encontrar dois núcleos de sentido que foram divididos em duas categorias: 1- processo de trabalho do enfermeiro no atendimento à demanda espontânea; e 2-necessidades de aprendizagem na percepção do enfermeiro. Conclusões: Verificou-se a necessidade de ampla divulgação dos documentos norteadores com as atividades a serem desenvolvidas pelo enfermeiro do acolhimento, com clareza dos fluxos e dos papéis de cada profissional, além de ampla capacitação de todos os profissionais, definição de matriz de alinhamento da equipe ou equipes ESF em cada UBS com o enfermeiro do acolhimento, a fim de que sejam estabelecidas a organização e a condução do processo de acolhimento com base nas necessidades do território e na organização interna da equipe.

Abstract Introduction: Welcoming is a practice present in care relationships, between health workers and users, in the acts of receiving and listening to people, being a mechanism for expanding and facilitating access. Objectives: Understand, in the perception of nurses who work in Primary Health Care, how the reception process occurs in Basic Health Units in the North Region of the Federal District. Methods: This is a descriptive, exploratory qualitative study, carried out with 33 nurses. Data collection took place from October 10 to November 30, 2022. Data were analyzed using Bardin's content analysis: pre-analysis, material exploration and treatment of results, inference and interpretation. Results: It was possible to find two core meanings that were divided into two categories: 1- The nurse's work process in meeting spontaneous demand and 2- Learning needs in the nurse's perception. Conclusions: There was a need for wide dissemination of guiding documents with the activities to be developed by the reception nurse, clearly of the flows and roles of each professional, besides wide training of all professionals, Definition of team alignment or teams in each UBS with the host nurse, in order to establish the organization and conduct of the reception process based on the needs of the territory and the internal organization of the team.

Abstract Background Cardiovascular diseases (CVD) are the major cause of mortality worldwide, whose most prominent risk factor is unhealthy eating habits, such as high fructose intake. Biogenic amines (BAs) perform important functions in the human body. However, the effect of fructose consumption on BA levels is still unclear, as is the association between these and CVD risk factors. Objective This study aimed to establish the association between BA levels and CVD risk factors in animals that consumed fructose. Methods Male Wistar rats received standard chow (n=8) or standard chow + fructose in drinking water (30%) (n=8) over a 24-week period. At the end of this period, the nutritional and metabolic syndrome (MS) parameters and plasmatic BA levels were analyzed. A 5% level of significance was adopted. Results Fructose consumption led to MS, reduced the levels of tryptophan and 5-hydroxitryptophan, and increased histamine. Tryptophan, histamine, and dopamine showed a correlation with metabolic syndrome parameters. Conclusion Fructose consumption alters BAs associated with CVD risk factors.

Resumo Fundamento As doenças cardiovasculares (DCV) são a principal causa de mortalidade do mundo, e um de seus fatores de risco são os hábitos alimentares não saudáveis, tais como, o alto consumo de frutose. As aminas biogênicas (ABs) realizam funções importantes no corpo humano. Entretanto, o efeito do consumo de frutose nos níveis das ABs ainda não está claro, bem como a associação entre estes e os fatores de risco da DCV. Objetivo Este estudo teve o objetivo de estabelecer a associação entre os níveis de ABs e os fatores de risco de DCV em animais que consumiram frutose. Métodos Ratos Wistar machos receberam ração convencional (n=8) ou ração convencional + frutose na água de beber (30%) (n=8) durante 24 semanas. Ao final, foram analisados os parâmetros nutricionais e da síndrome metabólica (SM) e os níveis plasmáticos das ABs. Foi adotado um nível de significância de 5%. Resultados O consumo de frutose levou à SM, reduziu os níveis de triptofano e 5-hidroxitriptofano e aumentou a histamina. Os níveis de triptofano, histamina e dopamina apresentaram correlação com parâmetros de síndrome metabólica. Conclusão O consumo de frutose altera as ABs associadas a fatores de risco de doenças cardiovasculares.

RESUMO Objetivo: Relatar a independência funcional e o grau de comprometimento pulmonar em pacientes adultos 3 meses após a alta da unidade de terapia intensiva. Métodos: Este foi um estudo de coorte retrospectiva conduzido em uma unidade de terapia intensiva multiprofissional para pacientes adultos em um único centro. Incluíram-se pacientes admitidos à unidade de terapia intensiva entre janeiro de 2012 e dezembro de 2013 que, 3 meses mais tarde, foram submetidos à espirometria e responderam ao questionário Medida de Independência Funcional. Resultados: Os pacientes foram divididos em grupos segundo sua classificação de independência funcional e espirometria. O estudo incluiu 197 pacientes, que foram divididos entre os grupos maior dependência (n = 4), menor dependência (n = 12) e independente (n = 181). Na comparação dos três grupos com relação à classificação pela Medida de Independência Funcional, pacientes com maior dependência tinham escores Acute Physiology and Chronic Health Evaluation II e Sequential Organ Failure Assessment mais altos quando da admissão à unidade de terapia intensiva, idade mais avançada, mais dias sob ventilação mecânica e tempo mais longo de permanência na unidade de terapia intensiva e no hospital. A maioria dos pacientes apresentava comprometimento pulmonar, sendo o padrão obstrutivo o mais frequentemente observado. Na comparação da independência funcional com a função pulmonar, observou-se que, quanto pior a condição funcional, pior a função pulmonar, observando-se diferenças significantes em relação ao pico de fluxo expiratório (p = 0,030). Conclusão: Em sua maioria, os pacientes que retornaram ao ambulatório 3 meses após a alta tinham boa condição funcional, porém apresentavam comprometimento pulmonar relacionado com o grau de dependência funcional.

ABSTRACT Objective: To relate functional independence to the degree of pulmonary impairment in adult patients 3 months after discharge from the intensive care unit. Methods: This was a retrospective cohort study conducted in one adult intensive care unit and a multi-professional post-intensive care unit outpatient clinic of a single center. Patients admitted to the intensive care unit from January 2012 to December 2013 who underwent (3 months later) spirometry and answered the Functional Independence Measure Questionnaire were included. Results: Patients were divided into groups according to the classification of functional independence and spirometry. The study included 197 patients who were divided into greater dependence (n = 4), lower dependence (n = 12) and independent (n = 181) groups. Comparing the three groups, regarding the classification of the Functional Independence Measure, patients with greater dependence had higher Acute Physiology and Chronic Health Evaluation II and Sequential Organ Failure Assessment values at intensive care unit admission with more advanced age, more days on mechanical ventilation, and longer stay in the intensive care unit and hospital. The majority of patients presented with pulmonary impairment, which was the obstructive pattern observed most frequently. When comparing functional independence with pulmonary function, it was observed that the lower the functional status, the worse the pulmonary function, with a significant difference being observed in peak expiratory flow (p = 0.030). Conclusion: The majority of patients who returned to the outpatient clinic 3 months after discharge had good functional status but did present with pulmonary impairment, which is related to the degree of functional dependence.

RESUMO Introdução Sabe-se que o treino de força traz melhorias para a saúde e o desempenho esportivo, por ocasionar hipertrofia muscular e aumento de força, além de modificar alguns fatores hemodinâmicos e fisiológicos. Foram desenvolvidos vários métodos de treinamento de força, entre eles, a oclusão vascular. Porém, existem poucos estudos com grandes grupamentos musculares, devido à pouca adesão ao estilo de treino e ao fato de a oclusão vascular de grandes grupos musculares ser mais difícil. Objetivo Verificar e comparar os efeitos hemodinâmicos do exercício com e sem oclusão vascular em diferentes grupamentos musculares. Métodos Estudo quantitativo, cruzado, com procedimentos transversais e de campo. A amostra foi composta por 10 indivíduos saudáveis e fisicamente ativos do sexo masculino e feminino, na faixa etária de 18 a 30 anos. Com o desenho cruzado, todos os voluntários participaram de 3 grupos: intervenção com oclusão vascular, intervenção sem oclusão vascular e grupo controle. Resultados De forma geral, entre grupos, o lactato e o colesterol se mantiveram elevados depois de 15 minutos de recuperação, a glicemia e as pressões arteriais não variaram. Conclusão O treinamento com oclusão vascular é um método eficaz para manipular as variáveis hemodinâmicas. Nível de evidência II; Estudo clínico.

ABSTRACT Introduction It is known that strength training brings improvements in health and sports performance by causing muscle hypertrophy and increased strength, as well as modifying some hemodynamic and physiological factors. Several strength training methodologies have been developed, one of which is vascular occlusion. There are few studies with large muscle groups due to poor adherence to the training style and the fact that vascular occlusion of large muscle groups is more difficult. Objective To verify and compare the hemodynamic effects of exercise with and without vascular occlusion in different muscle groups. Methods Quantitative crossover study, with cross-sectional and field procedures. The sample consisted of 10 physically active healthy male and female subjects between 18 and 30 years of age. With the cross-over design, all the volunteers participated in 3 groups: intervention with vascular occlusion, intervention without vascular occlusion and the control group. Results Overall, lactate and cholesterol remained elevated after 15 minutes of recovery and blood glucose and blood pressure did not vary among the groups. Conclusion Vascular occlusion training is an effective method for manipulating hemodynamic variables. Evidence level II; Clinical study.

RESUMEN Introducción Se sabe que el entrenamiento de fuerza trae mejoras para la salud y el desempeño deportivo, por causar hipertrofia muscular y aumento de fuerza, además de modificar algunos factores hemodinámicos y fisiológicos. Fueron desarrollados varios métodos de entrenamiento de fuerza, entre ellos, la oclusión vascular. Sin embargo, existen pocos estudios con grandes grupos musculares, debido a la poca adhesión al estilo de entrenamiento y al hecho de que la oclusión vascular de grandes grupos musculares grandes es más difícil. Objetivo Verificar y comparar los efectos hemodinámicos del ejercicio con y sin oclusión vascular en diferentes grupos musculares. Métodos Estudio cuantitativo, cruzado, con procedimientos transversales y de campo. La muestra fue compuesta por 10 individuos saludables y físicamente activos del sexo masculino y femenino, en el grupo de edad de 18 a 30 años. Con el diseño cruzado, todos los voluntarios participaron en 3 grupos: intervención con oclusión vascular, intervención sin oclusión vascular y grupo control. Resultados De forma general, entre los grupos, el lactato y el colesterol se mantuvieron elevados después de 15 minutos de recuperación, la glucemia y las presiones arteriales no variaron. Conclusión El entrenamiento de oclusión vascular es un método eficaz para manipular las variables hemodinámicas. Nivel de evidencia II; Estudio clínico.

Resumo Ora-pro-nobis (OPN) (Pereskia aculeata Miller) é uma planta alimentícia não convencional rica em proteínas, fibras e minerais. A inovação deste trabalho é baseada na produção sustentável de um extrato aquoso contendo ferro e compostos bioativos utilizando uma planta nativa brasileira. Uma triagem dos componentes fitoquímicos e a determinação dos compostos fenólicos, flavonoides e taninos foram realizadas usando extrato de OPN. A atividade antioxidante do extrato aquoso de OPN foi investigada empregando-se três diferentes métodos: DPPH•, FRAP e ORAC. Folhas secas de OPN apresentaram elevados conteúdos de proteínas e minerais. A análise de FTIR realizada no extrato aquoso de OPN confirmou a presença de grupos funcionais representativos de compostos bioativos. Os resultados sugerem que é possível usar um solvente aquoso simples para produzir um extrato de OPN rico em ferro, compostos bioativos e com atividade antioxidante, o qual poderia ser potencialmente utilizado como ingrediente alimentar funcional.

Abstract Ora-pro-nobis (OPN) (Pereskia aculeata Miller) is a non-conventional edible plant rich in protein, fibres and minerals. The innovation of this work is based on the sustainability production of the aqueous extract (green solvent) containing iron and bioactive compounds as well as employing a native plant from Brazil. A screening of phytochemicals components, phenolic compounds, flavonoids and tannins contents were performed using OPN extract. The antioxidant activity of the OPN aqueous extract was determined by three different assays as following: 2,2-diphenyl-1-picrylhydrazyl (DPPH•); Ferric Reducing Antioxidant Power (FRAP); and Oxygen Radical Absorbance Capacity (ORAC). The OPN dried leaves presented high protein and minerals contents. Indeed, the Fourier-Transform Infrared Spectroscopy (FTIR Spectroscopy) analysis performed in OPN aqueous extract confirmed the appearance of representative functional groups of the bioactive compounds. Overall the results suggested that it is possible to use simple aqueous solvent to produce OPN extract rich in iron, bioactive compounds and within antioxidant activity that could be potentially used as functional food ingredient.

Resumo Objetivo Avaliar os achados dos Potenciais Evocados Auditivos de Longa Latência (PEALL) em crianças com Transtorno dos Sons na Fala (TSF) após terapia fonoaudiológica. Método Estudo clínico longitudinal e prospectivo em um grupo de 14 crianças com TSF, de cinco a sete anos de idade, de ambos os sexos. Foram aplicadas as provas de Nomeação de Figuras e Imitação de palavras, para as quais foi calculado o índice de gravidade Porcentagem de Consoantes Corretas. Foram registrados os PEALL com estímulo de fala e foram analisados os valores de latência e amplitude dos componentes P1, N1, P2, N2 e P3. Cada criança foi avaliada em dois diferentes momentos: avaliação inicial e após 12 sessões de terapia fonoaudiológica. Resultados Os resultados mostraram que após terapia fonoaudiológica, o valor do índice de gravidade Porcentagem de Consoantes Corretas aumentou e um maior número de componentes foi observado nos registros dos PEALL nas crianças com TSF. Também foi observado um aumento estatisticamente significativo na amplitude do componente P3, demostrando que modificações anatomofisiológicas ocorreram no sistema nervoso auditivo central após intervenção, proporcionando melhora nos resultados dos PEALL. Conclusão Após terapia fonoaudiológica, foi observada melhora no desempenho fonológico das crianças, aumento no número de componentes presentes nos PEALL, bem como aumento na amplitude do componente P3, demonstrando que ocorreu plasticidade na via auditiva após um curto período de intervenção fonoaudiológica.

Abstract Purpose To analyze the results of Long-latency Auditory Evoked Potentials (LLAEP) in children with Speech Sounds Disorder (SSD) after speech therapy. Methods Longitudinal and prospective clinical study at 14 children with SSD, with ages ranging from five to seven years, of both genders. Were applied Picture Naming task and Imitation task, and from these tasks it was calculated the Percentage of Consonants Correct index. For an analysis of the LLAEP with speech stimulus and recorded the latency and amplitude values of P1, N1, P2, N2 and P3 components. Each child was evaluated in two different moments: initial evaluation and after 12 sessions of speech therapy. Results It was observed that after twelve sessions of speech therapy the value of Percentage of Consonants Correct index increased, and a greater number of components were observed in the LLAEP records of children with SSD, as well as a statistically significant increase in the amplitude of the P3 component, demonstrating that anatomical and physiological changes occurred in the central auditory nervous system after intervention, resulting in improved of the LLAEP results. Conclusion After speech therapy, improvement in the children's phonology was observed, and there was an increase in the number of components present in the LLAEP, as well as an increase in the amplitude of the P3 component, demonstrating that plasticity occurred in the auditory pathway during these three months of therapeutic intervention.

Resumo Objetivo: Investigar a via auditiva em tronco encefálico de crianças com leucemia linfoide aguda submetidas à quimioterapia (por infusão intravenosa ou por via intratecal). Métodos: Foram avaliadas 14 crianças com idade entre 2 e 12 anos, com diagnóstico de leucemia linfoide aguda. Foram utilizados os seguintes procedimentos: meatoscopia, medidas de imitância acústica, audiometria tonal, audiometria vocal, emissões otoacústicas transientes e potencial evocado auditivo de tronco encefálico. Resultados: Das 14 crianças com limiares auditivos normais, 35,71% demonstraram alteração no Potencial Evocado Auditivo de Tronco Encefálico, com predomínio de comprometimento de via auditiva em tronco encefálico baixo. Verificou-se que 80% das crianças com alteração haviam feito uso do metotrexato via intratecal a menos de 30 dias e que 40% tinham as maiores doses acumulativas de metotrexato por via endovenosa. Conclusão: Crianças com leucemia linfoide aguda submetidas à quimioterapia apresentam comprometimento na via auditiva em tronco encefálico, com predomínio em tronco encefálico baixo.

Abstract Objective: Investigate the auditory pathway in the brainstem of children with acute lymphoblastic leukemia submitted to chemotherapy (by intravenous or intrathecal infusion). Methods: Fourteen children aged between 2 and 12 years with diagnosis of acute lymphoid leukemia were evaluated. The following procedures were used: meatoscopy, acoustic immitance measurements, tonal audiometry, vocal audiometry, transient otoacoustic emissions, and auditory brainstem response. Results: From the 14 children with normal auditory thresholds, 35.71% showed an alteration in auditory brainstem response, with a predominance of hearing impairment in the lower brainstem. It was found that 80% of the children with alteration had used intrathecal methotrexate less than 30 days and that 40% had the highest cumulative intravenous methotrexate doses. Conclusion: Children with acute lymphoblastic leukemia submitted to chemotherapy, present auditory pathway impairment in the brainstem, with a predominance of a low brainstem.

RESUMO Objetivo Investigar o efeito da cirurgia para secção completa do retalho faríngeo sobre a hipernasalidade de fala. Métodos Foram avaliados 26 indivíduos com fissura de palato±lábio reparada, submetidos à cirurgia de retalho faríngeo para tratamento da insuficiência velofaríngea e que, em função do aparecimento de queixas respiratórias, necessitaram nova cirurgia para secção do retalho. A hipernasalidade foi determinada por meio das avaliações perceptiva e nasométrica da fala 18 meses, em média, após a secção do retalho. Na avaliação perceptiva, a hipernasalidade foi classificada como: 1 = ausente ou 2 = presente e, na nasometria, foi determinada por meio da medida da nasalância durante a leitura de sentenças contendo, exclusivamente, sons orais, considerando-se, como limite de normalidade, o escore de 27% (p ≤ 0,05). Resultados A avaliação perceptiva mostrou que, antes da secção do retalho, oito (31%) indivíduos apresentavam ressonância equilibrada e 18 (69%) apresentavam hipernasalidade. Após a cirurgia, um (4%) paciente permaneceu com ressonância equilibrada e 25 (96%) apresentaram hipernasalidade. De acordo com a nasometria, antes da cirurgia, 13 (57%) indivíduos apresentaram valores de nasalância inferiores a 27%, indicando ausência de hipernasalidade (média = 15±8%) e dez (43%) pacientes apresentaram valores indicativos de hipernasalidade (média = 41±7%). Após a cirurgia, quatro (17%) pacientes permaneceram com valores indicativos de ausência de hipernasalidade (média = 19±10%) e 19 (83%) apresentaram valores de nasalância indicativos de hipernasalidade (média = 45±7%). Diferença entre as avaliações perceptiva e nasométrica da fala não foi observada. Conclusão A cirurgia para secção completa do retalho faríngeo causou deterioração da ressonância de fala, levando ao reaparecimento da hipernasalidade, na maioria dos pacientes estudados.

ABSTRACT Purpose To investigate the effect of complete section of pharyngeal flap on speech hypernasality. Methods The study analyzed twenty-six individuals with repaired cleft palate±lip underwent pharyngeal flap surgery to treat velopharyngeal insufficiency and posteriorly underwent complete section of the flap due to the occurrence of respiratory symptoms. Hypernasality was determined by auditory-perceptual speech assessments and nasometry at 18 months after surgery, on average. Hypernasality was perceptually classified as: 1 = absent or 2 = present and determined by nasalance measurement during reading of sentences containing exclusively oral sounds, considering a cutoff of 27% (p ≤ 0.05). Results Perceptual assessment before section of the flap revealed that eight (31%) individuals showed normal resonance, while 18 (69%) presented hypernasality. After surgery, one (4%) subject remained with normal resonance and 25 (96%) presented hypernasality. According to nasometry, before surgery, 13 (57%) individuals presented nasalance scores lower than 27%, indicative of absence of hypernasality (mean = 15±8%) and ten (43%) presented nasalance scores indicative of hypernasality (mean = 41±7%). After surgery, four (17%) patients remained with scores indicative of absence of hypernasality (mean = 19±10%) and for 19 (83%) the nasalance scores were indicative of hypernasality (mean = 45±7%). There was no difference between perceptual and nasometric speech evaluations. Conclusion Surgery for complete section of pharyngeal flap caused deterioration of speech resonance, leading to the reappearance of hypernasality in most patients in this study.

Resumo Introdução As respostas eletrofisiológicas obtidas com o Potencial Evocado Auditivo de Tronco Encefálico complexo (Peate-c) fornecem medidas objetivas do processamento subcortical dos estímulos de fala e outros estímulos complexos. O Peate-c também vem sendo utilizado para verificar a plasticidade na via auditiva nas regiões subcorticais. Objetivo Comparar os resultados do Peate-c obtidos em crianças usuárias de Aparelho de Amplificação Sonora Individual (AASI), antes e após 9 meses da adaptação, bem como comparar os resultados destas crianças com os obtidos em crianças com audição normal. Método Foram avaliadas 14 crianças com audição normal (Grupo Controle - GC) e 18 crianças com perda auditiva neurossensorial de grau leve a moderado bilateral (Grupo Estudo - GE), na faixa etária de 7 a 12 anos. As crianças foram submetidas às Audiometrias Tonal e Vocal, Medidas de Imitância Acústica e Peate com estímulo de fala, sendo submetidas às avalições em três momentos diferentes: avaliação inicial (M0), 3 meses após avaliação inicial (M3) e 9 meses após a avaliação inicial (M9), sendo que no M0 as crianças do grupo estudo ainda não faziam uso do AASI. Resultados Na comparação entre GC e GE, observou-se que o GE apresentou menor mediana para a amplitude V-A no M0 e no M3, menor mediana para a latência do componente V no M9 e, maior mediana para a latência do componente O no M3 e no M9. Observou-se no GE, redução na latência do componente A no M9. Conclusão Crianças com perda auditiva de grau leve a moderado apresentaram déficit no processamento do estímulo de fala sendo que o principal prejuízo está relacionado à decodificação da porção transitória do espectro deste estímulo. Evidenciou-se que o uso do AASI promoveu a plasticidade neuronal do Sistema Nervoso Auditivo Central após um tempo prolongado de estimulação sensorial.

Abstract Introduction The electrophysiological responses obtained with the complex auditory brainstem response (cABR) provide objective measures of subcortical processing of speech and other complex stimuli. The cABR has also been used to verify the plasticity in the auditory pathway in the subcortical regions. Objective To compare the results of cABR obtained in children using hearing aids before and after 9 months of adaptation, as well as to compare the results of these children with those obtained in children with normal hearing. Methods Fourteen children with normal hearing (Control Group - CG) and 18 children with mild to moderate bilateral sensorineural hearing loss (Study Group - SG), aged 7-12 years, were evaluated. The children were submitted to pure tone and vocal audiometry, acoustic immittance measurements and ABR with speech stimulus, being submitted to the evaluations at three different moments: initial evaluation (M0), 3 months after the initial evaluation (M3) and 9 months after the evaluation (M9); at M0, the children assessed in the study group did not use hearing aids yet. Results When comparing the CG and the SG, it was observed that the SG had a lower median for the V-A amplitude at M0 and M3, lower median for the latency of the component V at M9 and a higher median for the latency of component O at M3 and M9. A reduction in the latency of component A at M9 was observed in the SG. Conclusion Children with mild to moderate hearing loss showed speech stimulus processing deficits and the main impairment is related to the decoding of the transient portion of this stimulus spectrum. It was demonstrated that the use of hearing aids promoted neuronal plasticity of the Central Auditory Nervous System after an extended time of sensory stimulation.

OBJECTIVE: The objective of this study was to compare long-latency auditory evoked potentials before and after hearing aid fittings in children with sensorineural hearing loss compared with age-matched children with normal hearing. METHODS: Thirty-two subjects of both genders aged 7 to 12 years participated in this study and were divided into two groups as follows: 14 children with normal hearing were assigned to the control group (mean age 9 years and 8 months), and 18 children with mild to moderate symmetrical bilateral sensorineural hearing loss were assigned to the study group (mean age 9 years and 2 months). The children underwent tympanometry, pure tone and speech audiometry and long-latency auditory evoked potential testing with speech and tone burst stimuli. The groups were assessed at three time points. RESULTS: The study group had a lower percentage of positive responses, lower P1-N1 and P2-N2 amplitudes (speech and tone burst), and increased latencies for the P1 and P300 components following the tone burst stimuli. They also showed improvements in long-latency auditory evoked potentials (with regard to both the amplitude and presence of responses) after hearing aid use. CONCLUSIONS: Alterations in the central auditory pathways can be identified using P1-N1 and P2-N2 amplitude components, and the presence of these components increases after a short period of auditory stimulation (hearing aid use). These findings emphasize the importance of using these amplitude components to monitor the neuroplasticity of the central auditory nervous system in hearing aid users.

This in vitro and in vivo study compared different topographies of Ti samples (dense, porosity of 30% and 40%) with or not CaP coating, prepared by powder metallurgy. Osteogenic cells from newborn rat calvaria were plated onto the samples and cell adhesion (24 hours), alkaline phosphatase activity (7 and 10 days) and mineralization nodules (14 days) were assessed. Sixteen rabbits were used for in vivo study. Each animal received three non-treated and three treated implants in the right or left tibia, respectively. Histometric evaluation of bone-implant contact (BIC) were assessed at 1, 2, 4 and 8 weeks. Metallographic analysis revealed porosities of 30% and 40%, with pore size ranging from 250 to 350 µm. Cell adhesion test and ALP revealed similar cell behavior, independently of topography and CaP coating (P > 0.05%). However, CaP coating combined with porosity of 40% influenced positively the mineralized matrix formation (P < 0.05%). CaP-coated implants showed higher BIC than non-CaP implants and BIC was different between the short (1 and 2 weeks) and long (4 and 8 weeks) healing periods (P < 0.05%). The results suggest that CaP coating combined with 40% porosity implants allowed greater osteogenesis in vitro and increased BIC in vivo.

RESUMO Objetivo: Caracterizar os Potenciais Evocados Auditivos de Longa Latência (PEALL) (P1, N1, P2, N2, P300) em campo sonoro, em crianças audiologicamente normais, bem como verificar a estabilidade destes potenciais. Métodos: Trata-se de um estudo prospectivo, longitudinal, composto por 17 crianças audiologicamente normais, na faixa etária de 6 a 13 anos de idade, com limiares de audibilidade dentro da normalidade. Foram captados os PEALL P1, N1, P2, N2, P300 com estímulos de fala e tone burst, em três momentos de avaliação: avaliação inicial (M0), três meses após a avaliação inicial (M3) e nove meses após a avaliação inicial (M9). Resultados: Foi observada diminuição dos valores de latência dos componentes P1 (M0xM3 / M0xM9 / M0xM3xM9) e P2 (M0xM9) e aumento no valor de amplitude do P300 (M0xM3), quando obtidos com estímulo de fala, e diminuição no valor de latência do P300 (M0xM9), obtido com estímulo tone burst. Conclusão: Foi possível identificar os componentes do PEALL na maioria dos indivíduos. Os componentes P1, N1, P2, N2 (tone burst) e N1 e N2 (fala) não sofreram modificações em latências e amplitudes entre os diferentes momentos de avaliação, sugerindo estabilidade deste potencial no período de nove meses. O P300 demonstrou ser um componente mais sensível a esse intervalo de tempo entre as avaliações, pois sofreu modificações indicativas de maturação do sistema nervoso auditivo central. As latências de todos os componentes obtidos com estímulo de fala foram maiores do que com tone burst, demonstrando que estímulos diferentes geram respostas corticais distintas.

ABSTRACT Purpose: To characterize the Long-Latency Auditory Evoked Potentials (LLAEP) (P1, N1, P2, N2, P300) with acoustic stimuli presented in sound field system in children with normal hearing, as well as to verify the stability of these potentials. Methods: This prospective, longitudinal study comprised 17 children with ages between 6 and 13 years and hearing thresholds within normal. The LLAEP P1, N1, P2, N2, P300 were recorded with speech stimuli and tone burst presented in sound field system, in three different moments: initial assessment (M0), three months after the initial assessment (M3), and nine months after the initial assessment (M9). Results: When the speech stimuli was used, there was a decrease in the latency values of components P1 (M0xM3/M0xM9/M0xM3xM9) and P2 (M0xM9), as well as an increase in the amplitude of P300 (M0xM3) over time. With the tone bust stimuli, it was verified a decrease in the latency values of P300 (M0xM9) over time. Conclusion: it was possible to identify the components of the LLAEP in most individuals. The P1, N1, P2, N2 (tone burst) and N1 and N2 (speech) did not change their latencies and amplitudes between the different moments of evaluation, suggesting stability of this potential in the period of 9 months. The P300 was the component most sensitive to the time intervals considered, since it presented modifications over time that indicated maturation of the central auditory nervous system. The latencies of all components obtained with speech stimulus were higher than with tone bursts, indicating that different stimuli generate different cortical responses.

OBJETIVO: comparar o efeito do retalho faríngeo e da esfincteroplastia sobre a hipernasalidade da fala e o fechamento velofaríngeo no tratamento de indivíduos com insuficiência velofaríngea residual, por meio de avaliação instrumental. MÉTODOS: foram avaliados 30 pacientes, com fissura de palato±lábio reparada, submetidos à correção cirúrgica da insuficiência velofaríngea (15 com retalho faríngeo e 15 com esfincteroplastia), avaliados antes e, no mínimo, 1 ano após a cirurgia. A hipernasalidade foi estimada a partir dos escores de nasalância (correlato físico da nasalidade) obtidos por meio da nasometria, durante a leitura de 5 sentenças contendo, exclusivamente, sons orais, considerando como limite de normalidade o escore de 27%. O fechamento velofaríngeo foi aferido a partir da medida da área velofaríngea obtida por meio da técnica fluxo-pressão e foi classificado em: 0-4,9mm2=adequado; 5-19,9mm2=marginal e, >20mm2=inadequado. Diferenças entre as duas técnicas foram consideradas estatisticamente significantes ao nível de 5%. RESULTADOS: antes da cirurgia, os valores médios de nasalância foram de 43±8,4% e 45±14,2% e de área velofaríngea foram 51±35,4mm2, e 69±29,2mm2, para os grupos retalho faríngeo e esfincteroplastia, respectivamente. Após a cirurgia, os valores médios de nasalância reduziram para 27±10,1% e 31±14,2% e de área velofaríngea para 3,6±5,5mm2 e 24±32,7mm2 para os grupos retalho faríngeo e esfincteroplastia, respectivamente. A redução dos valores de nasalância e área velofaríngea foi estatisticamente significante nos dois grupos. CONCLUSÃO: estes resultados sugerem que o retalho faríngeo foi mais eficiente do que a esfincteroplastia na eliminação da hipernasalidade e adequação do fechamento velofaríngeo nos pacientes estudados.

PURPOSE: to compare the effect of pharyngeal flap surgery and sphincteroplasty on hypernasality and velopharyngeal closure in velopharyngeal insufficiency management, by means of instrumental assessment. METHODS: thirty patients with repaired cleft palate±lip, submitted to surgical treatment for velopharyngeal insufficiency (15 pharyngeal flap and 15 sphincteroplasty) were evaluated before and, at least, 1 year after surgery. Hypernasality was estimated by means of nasalance scores (acoustic correlate of nasality) obtained by nasometry considering a cutoff score of 27%. Velopharyngeal closure was determined by the velopharyngeal area measurement. Nasalance scores were obtained by nasometry, during the reading of a set of 5 sentences containing exclusively oral sounds, considering the cutoff value of 27%. Velopharyngeal area was provided by the measurement of velopharyngeal area by means of pressure-flow technique and was classified as: 0 to 4.9 mm2=adequate; 5 to 19.9 mm2=borderline and ≥20mm2 inadequate. Differences between the two techniques were accepted as significant when p < 0.05. RESULTS: before surgery. nasalance mean scores were 43±8.4% and 45±14.2% and velopharyngeal area mean were 51±35.4mm2 and 69±29.2mm2 for the pharyngeal flap and sphincteroplasty groups, respectively. After surgery, nasalance mean scores were 27±10.1% and 31±14.2% and velopharyngeal area mean were 3.6±5.5mm2 and 24±32.7mm2 for the pharyngeal flap and sphincteroplasty groups, respectively. The reduction of the nasalance scores and velopharyngeal area was statistically significant in both groups. CONCLUSION: these results suggest that pharyngeal flap demonstrated to be more efficient than sphincteroplasty in the elimination of hypernasality and adequacy of velopharyngeal closure in the patients studied.

O objetivo deste trabalho foi identificar a percepção de usuários de um serviço de saúde sobre direito à informação e integralidade do cuidado. Trata-se de pesquisa qualitativa, exploratória, com orientação analítico-descritiva, cujo eixo teórico foi o direito à saúde relacionado ao acesso a informação e integralidade do cuidado. As seguintes categorias foram construídas: dificuldades para concretização do cuidado humanizado à saúde em uma unidade básica; despersonalização da relação profissional/usuário do serviço; lacunas na infraestrutura física e humana do serviço; e relacionamento interpessoal entre os diferentes atores do processo de cuidar. O acesso à informação se constitui como aspecto central para a promoção do cuidado integral à saúde. Para conseguir proporcioná-lo de forma adequada, torna-se importante sensibilizar a equipe com relação à importância do empoderamento do usuário, para torná-lo protagonista do cuidado e tê-lo como um aliado no processo do cuidar.

The aim of this study was to identify the perceptions of patients at a healthcare service regarding the right to information and comprehensive care. This was a qualitative, exploratory investigation with an analytical-descriptive orientation, for which the theoretical axis was the right to health-related information access and comprehensive care. The following categories were constructed: difficulties in achieving humanized healthcare in a primary unit; depersonalization of professional/patient relationships; gaps in physical and human infrastructure of the service; and interpersonal relationships among the different players in the care process. Access to information constitutes a central aspect of promotion of comprehensive healthcare. To be able to provide this adequately, it is important to make professionals aware of the importance of user empowerment, so as to make users the protagonists of care and have them as allies in the care process.

Con la intención de identificar la percepción de usuarios de un servicio de salud sobre el derecho a la información e integralidad del cuidado, fue desenvolvida una investigación cualitativa exploratoria con orientación analítico-descriptiva cuyo eje teórico fue el derecho a la salud y el acceso a la información y a la integralidad del cuidado. Se construyeron las categorías: dificultades para concreción del cuidado humanizado de la salud en una unidad básica; despersonalización de la relación profesional/usuario; brechas en la infraestructura física y humana del servicio y relación interpersonal entre los actores del proceso de cuidar. El acceso a la información se constituye como aspecto central para la promoción del cuidado total de la salud. Para conseguir ese intento es importante sensibilizar al equipo en relación a la importancia del empoderamiento del usuario, para convertirlo en protagonista del cuidado y tenerlo como aliado en ese proceso.
