The objective of this research was to understand the students' view on various aspects of training at IFBA. By means of a structured questionnaire applied at the Camaçari, Santo Amaro and Simões Filho campuses, data were collected from students regarding their expectations and perceptions during their professional training trajectory; integration into the labour market; the positive and negative points of the training received at IFBA and the internship in the area of the technical course. As a result, it was possible to observe that most of the participating students were from the first years, they reported that the stoppages/strikes during the course delay the training and demotivate. Even so, they consider the training offered by the IFBA and the preparation of the teaching staff to be good. The students pointed out the need to integrate the curriculum, reduce course time and provide learning opportunities in an easier way. For future studies, it is proposed that the research be applied to other IFBA campuses and thus have a perception of the.

El objetivo de esta investigación fue conocer la visión de los estudiantes sobre diversos aspectos de la formación en el IFBA. Por medio de un cuestionario estructurado aplicado en los campus de Camaçari, Santo Amaro y Simões Filho, se recogieron datos de los estudiantes sobre sus expectativas y percepciones durante su trayectoria formativa profesional; la integración en el mercado laboral; los puntos positivos y negativos de la formación recibida en IFBA y la pasantía en el área del curso técnico. Como resultado, se pudo observar que la mayoría de los estudiantes participantes eran de los primeros años, informaron que los paros/huelgas durante el curso retrasan la formación y desmotivan. Aun así, consideran que la formación ofrecida por el IFBA y la preparación del profesorado son buenas. Los estudiantes señalaron la necesidad de integrar el currículo, reducir el tiempo de curso y brindar oportunidades de aprendizaje de una manera más fácil. Para futuros estudios, se propone que la investigación se aplique a otros campus del IFBA y así tener una percepción de la.  

O objetivo desta pesquisa foi compreender a visão dos alunos sobre diversos aspectos da formação no IFBA. Por meio de um questionário estruturado aplicado nos campi de Camaçari, Santo Amaro e Simões Filho, foram coletados dados dos alunos em relação às suas expectativas e percepções durante a trajetória de formação profissional; a inserção no mercado de trabalho; os pontos positivos e negativos da formação recebida no IFBA e o estágio na área do curso técnico. Como resultado foi possível observar que a maioria dos alunos participantes foram dos primeiros anos, eles relataram que as paralisações/greves durante o curso atrasam a formação e desmotivam. Mesmo assim, consideram a formação ofertada pelo IFBA e a preparação do corpo docente boas. Os alunos apontaram a necessidade de integração do currículo, redução do tempo do curso e oportunização do aprendizado de forma mais fácil. Para estudos futuros propõe-se que a pesquisa seja aplicada para outros Campi do IFBA e assim tenha uma percepção mais abrangente dos alunos sobre a formação.

Abstract Polymeric composite materials, presenting a practical solution for sealing non-metallic expansion joints under extreme conditions such as high temperatures and harsh chemical and physical abrasion, were investigated in this scientific study to discern the impact of welding parameters on their degradation and properties. The study entailed the bonding of polymeric composite blankets through hot plate pressing with a PTFE film, encompassing variations in temperature, duration, and load application. The findings elucidated that lower temperatures and shorter processing times failed to achieve optimal blanket adhesion, while higher temperatures led to material degradation, subsequently diminishing the mechanical strength of the welded joint. In contrast, extended processing times and the application of load during welding demonstrated a positive correlation, enhancing the mechanical strength of the joint by ameliorating interfacial adhesion. This research underscores the critical significance of carefully selecting welding parameters to ensure the peak performance and durability of polymeric composite structures.

Resumo Introdução: O bullying escolar é uma problemática social que tem preocupado educadores e pesquisadores. Apesar de existir uma noção tradicional sobre bullying, verificam-se, atualmente, outras compreensões que ampliam ou problematizam a visão hegemônica. Objetivo: compreender os sentidos de bullying presentes nas pesquisas científicas brasileiras que envolvem diferentes áreas de estudo, publicadas nos últimos 5 anos, observando aproximações e distanciamentos com a temática do preconceito. Método: pesquisa qualitativa, de natureza exploratória, por meio de revisão integrativa da literatura científica. Buscou-se artigos disponíveis no SciELO e PubMed usando os descritores Bullying AND Escola. Seguindo critérios de inclusão e exclusão, o corpus da pesquisa foi composto por 21 artigos de língua portuguesa. Resultados: Os estudos, em sua maioria, amparam-se no conceito tradicional de bullying e apenas um problematiza essa compreensão. No que diz respeito à aproximação ou distanciamento entre os conceitos de bullying e preconceito, apareceram três posicionamentos. Em sete artigos não foram observadas aproximações entre os conceitos de bullying e preconceito; em outros sete, a relação entre os conceitos aparece de forma indireta, sem problematização acerca dessa aproximação; por fim, em sete artigos, essa aproximação é explicitada e problematizada. Esses últimos discutem, por exemplo, a maior exposição à vivência de bullying, por parte de estudantes negros, da comunidade LGBT+ e mulheres. A recorrência desses perfis como alvo do bullying expõe a aproximação desse tipo de violência ao preconceito, intolerância e negação da diversidade. Conclusões: O sentido tradicional de bullying se faz presente nas pesquisas, contudo tensionamentos foram observados, como os estudos que aproximam bullying a diferentes expressões de preconceito. A abordagem qualitativa do estudo contribui para uma perspectiva crítica que coloca em evidência o bullying como um problema social e político e abre possibilidades para outras questões de pesquisa e intervenções no contexto do bullying escolar no caminho da transformação social.

Abstract Introduction: School bullying is a social problem that has worried educators and researchers. Although there is a traditional notion about bullying, currently there are other understandings that expand or problematize the hegemonic view. Objective: to understand the meanings of bullying in Brazilian scientific research involving different areas of study, published in the last 5 years, observing similarities and differences within the prejudice theme. Method: qualitative research of exploratory nature, through an integrative review of the scientific literature. Articles available in SciELO and PubMed were searched using the descriptors Bullying AND Escola. Following inclusion and exclusion criteria, the research consisted of 21 articles in Portuguese. Results: The majority of studies are based on the traditional concept of bullying and only one problematizes this understanding. Regarding the approximation or distance between the concepts of bullying and prejudice, three positions appeared. In seven articles, approximations between the concepts of bullying and prejudice were not observed; in another seven, the relationship between the concepts appears indirectly, without problematizing this approximation; finally, in seven articles, this approximation is explicit and problematized. The last one discusses, for example, the greater exposure to the experience of bullying by black students, the LGBT+ community and women. The recurrence of these profiles as a target of bullying exposes the approximation of this type of violence to prejudice, intolerance and denial of diversity. Conclusions: The traditional meaning of bullying is present in research, however tensions were observed, such as studies that bring bullying closer to different expressions of prejudice. The studies qualitative approach contributes to a critical perspective that highlights bullying as a social and political problem and opens up to possibilities for other research questions and interventions in the context of school bullying in the path of social transformation.

ABSTRACT BACKGROUND AND OBJECTIVES: The goal of the present study was to assess the frequency of temporomandibular disorders (TMD) subtypes in individuals that search for specialized care, identifying the subtypes of TMD, muscular and/or articular, in addition to relating each subtype with variables such as gender and age. METHODS: In this context, after a screening, 270 individuals, aged between 18 and 70 years, were selected. All data were acquired using the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) instrument. RESULTS: Among the 267 patients, 88.7% (n = 237) were female and 11.3% (n = 30) were male, with a mean age of 42±11.8 years. In this study, the predominance of joint/muscle TMD was evidenced (51.7%; n = 138), followed by only muscle TMD (47.5%; n = 127) and lastly, joint TMD (0.8%; n = 2). Important correlations (p>0.05) were not identified when comparing TMD subtypes with the variables gender and age, according to chi-square test. By relating the diagnosis to the TMD subtype, it was evidenced that bilateral myalgia was the most prevalent in muscle TMD (n = 100; 37.4%) and articular/muscular (n = 112; 41.9%). The other variables did not show significant statistics, neither moderate nor strong correlation. CONCLUSION: In this research, all TMD subtypes were found, with a clear predominance of joint/muscle type of TMD, followed by muscle only, especially in females aged between 39 and 48 years.

RESUMO JUSTIFICATIVA E OBJETIVOS: O objetivo do presente estudo foi avaliar a frequência dos subtipos de disfunção temporomandibular (DTM) em indivíduos que buscaram atendimento especializado, identificando os subtipos da DTM, se muscular e/ou articular, além de relacionar cada subtipo com variáveis como sexo e faixa etária. MÉTODOS: Neste contexto, após uma triagem, foram selecionados 270 indivíduos na cidade de Fortaleza/CE, com faixa etária entre 18 anos e 70 anos de idade. Todos os indivíduos foram avaliados por meio do instrumento Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). RESULTADOS: Entre os 267 pacientes, 88,7% (n = 237) foram do sexo feminino e 11,3% (n = 30) do masculino, com média de idade de 42 ± 11,8 anos. Neste estudo evidenciou-se a predominância de DTM articular/muscular (51,7%; n = 138), seguida de indivíduos com DTM somente muscular (47,5%; n = 127) e por último a articular (0,8%; n = 2). Não foram identificadas correlações importantes (p > 0,05), segundo o teste qui-quadrado, quando comparados os subtipos de DTM com as variáveis gênero e idade. Ao relacionar o diagnóstico com o subtipo de DTM, evidenciou-se que a mialgia bilateral foi a mais prevalente na DTM muscular (n = 100; 37,4%) e articular/muscular (n = 112; 41,9%). As demais variáveis não apontaram significância estatística nem correlação moderada ou forte. CONCLUSÃO: Nesta pesquisa, foram encontrados todos os tipos de DTM, havendo um claro predomínio da DTM do tipo articular/muscular, seguida da somente muscular, especialmente em indivíduos do gênero feminino e de idade entre 39 e 48 anos.

RESUMO OBJETIVOS Comparar a cobertura do rastreamento do câncer de colo do útero no Brasil em 2013 e 2019, investigar os fatores associados à realização do exame e os motivos informados para não ter realizado, além de comparar o tempo do recebimento do resultado do exame no SUS e na rede privada. MÉTODOS A partir de dados da Pesquisa Nacional de Saúde (PNS) foram calculadas as prevalências e os respectivos intervalos de confiança de realização do exame preventivo do câncer do colo do útero há menos de três anos, em mulheres de 25 a 64 anos, em 2013 e 2019. Modelos de regressão de Poisson foram utilizados para comparar as prevalências do desfecho segundo características sociodemográficas. Também foram analisados os motivos para não ter feito o exame e o tempo entre a realização e o recebimento do laudo. RESULTADOS Houve aumento na cobertura do exame preventivo no Brasil entre 2013 (78,7%) e 2019 (81,3%) e redução na proporção de mulheres que nunca fizeram o exame de 9,7% para 6,1%. A prevalência de realização do exame foi maior em mulheres brancas, melhor escolaridade e renda mais alta, residentes nas regiões Sul e Sudeste. Os motivos mais frequentes para não realizar o exame foram achar desnecessário (45% em 2013 e em 2019) e nunca ter sido orientada a fazê-lo (20,6% em 2013 e 14,8% em 2019). CONCLUSÕES Apesar das elevadas coberturas de rastreamento alcançadas pelo país, há grande desigualdade no acesso ao exame, e uma parcela não desprezível de mulheres está sob maior risco de morrer por uma doença que pode ser evitada. Esforços devem ser feitos para a estruturação de um programa de rastreamento organizado que identifique e capte as mulheres mais vulneráveis.

ABSTRACT OBJECTIVES To compare the coverage of cervical cancer screening in Brazil in 2013 and 2019, investigating the factors associated with having the test performed and the reasons given for not doing it. Additionally, a comparison is made concerning the time taken to receive the test result in SUS (Sistema Único de Saúde) and in the private health services. METHODS Using data from the National Health Survey (Pesquisa Nacional de Saúde - PNS), prevalence rates and corresponding confidence intervals were calculated to determine the frequency of recent cervical cancer screenings among women aged between 25 and 64 years old in Brazil, for both 2013 and 2019. Poisson regression models were employed to compare the prevalence of the outcome according to sociodemographic characteristics. The reasons for not having the test and the time between performing and receiving the result were also analyzed. RESULTS The findings revealed an increase in the coverage of preventive cervical cancer exams in Brazil from 78.7% in 2013 to 81.3% in 2019. Additionally, there was a decline in the proportion of women who had never undergone the exam, from 9.7% to 6.1%. Prevalence of test uptake was higher among white women, those with higher levels of education and income, and those residing in the South and Southeast regions of the country. The most commonly cited reasons for not taking the test were the impression it was unnecessary (45% in both 2013 and 2019) and never having been asked to undergo the test (20.6% in 2013 and 14.8% in 2019). CONCLUSIONS Despite the high coverage of screening achieved in the country, there is great inequality in access to the test, and a non-negligible number of women are at greater risk of dying from a preventable disease. Efforts must be made to structure an organized screening program that identifies and captures the most vulnerable women.

Abstract During a survey of mucoralean fungi in soil from an upland forest area located in Pernambuco, Brazil, a strain of Backusella (URM 8637) was isolated. Based on morphological, physiological, and molecular data [internal transcribed spacer (ITS) and large subunit (LSU) ribosomal DNA regions], it was recognized that this Backusella differed from all other species in the genus. Morphologically, the new species is characterized as forming varied-shaped columellae, including elongated, basally constricted, unisporate (rare) and multisporate sporangiola, and ellipsoidal sporangiospores. The maximum temperature growth of URM 8637 on malt extract agar and potato dextrose agar was 36 °C. In the phylogram, it was closely related to B. constricta. Based on the evidence from the analyzed datasets, a new species of Backusella is proposed. An updated identification key for Backusella from the Americas is provided.

BACKGROUND Few studies have focused on microbial diversity in indoor environments of ships, as well as the role of the microbiome and its ecological interconnections. In this study, we investigated the microbiome and virome present on the internal surfaces of a polar ship in different stages (beginning, during, and at the end) of the Brazilian Antarctic expedition in order to evaluate abundance of microorganisms in different periods. OBJECTIVES AND METHODS We used shotgun metagenomic analysis on pooled samples from sampling surfaces in the ship’s interior to track the microbial diversity. FINDINGS Considering the total fraction of the microbiome, the relative abundance of bacteria, eukaryotes, viruses, and archaea was 83.7%, 16.2%, 0.04%, and 0.002%, respectively. Proteobacteria was the most abundant bacterial phyla, followed by Firmicutes, Actinobacteria, and Bacteroidetes. Concerning the virome, the greatest richness of viral species was identified during the middle of the trip, including ten viral families after de novo assembly: Autographiviridae, Chrysoviridae, Genomoviridae, Herelleviridae, Myoviridae, Partitiviridae, Podoviridae, Potyviridae, Siphoviridae, and Virgaviridae. MAIN CONCLUSIONS This study contributed to the knowledge of microbial diversity in naval transportation facilities, and variations in the abundance of microorganisms probably occurred due to factors such as the number of passengers and activities on the ship.

ABSTRACT Objective: to create and validate an app that establishes the diabetic foot risk degree by associating the patients’ health conditions. Methodology: a methodological study conducted between March 2019 and December 2021 in Crato - CE - Brazil, and divided into three phases: 1) Review of the national and international guidelines; 2) Creation of the educational technology; and 3) Face and content validation of the technology. This study followed the guidelines set forth in the Revised Standards for Quality Improvement Reporting Excellence guide. Results: in the review phase, similar content was identified between the guidelines for designing and organizing the content. The technology that was created enabled data collection, as well as diabetic foot risk assessment and classification. A total of 18 judges evaluated the instrument in the validation stage, obtaining a CVI value of 0.96. Conclusion: the study may contribute to improving the indicators referring to hospitalizations, amputations, reduced mobility, dependence, frailty and mortality resulting from diabetic foot.

RESUMEN Objetivo: crear y validar una aplicación que establezca el grado de riesgo del pie diabético asociándolo a sus condiciones de salud. Metodología: estudio metodológico realizado entre marzo de 2019 y diciembre de 2021, en Crato, CE, Brasil, dividido en tres fases: 1) Revisión de directrices nacionales e internacionales; 2) Creación de la tecnología educativa y 3) Validación del contenido y la apariencia de la tecnología. Este estudio siguió los lineamientos de la guía Revised Standards for Quality Improvement Reporting Excellence. Resultados: en la fase de revisión, se identificaron contenidos similares entre las directrices para el diseño y la organización del constructo. La tecnología creada permitió recolectar datos, evaluar y clasificar el riesgo del pie diabético. En la etapa de validación, 18 jueces evaluaron el instrumento, que contó con un IVC de 0,96. Conclusión: el estudio puede contribuir a mejorar los indicadores de hospitalización, amputación, movilidad reducida, dependencia, fragilidad y mortalidad derivados del pie diabético.

Este estudo seguiu as diretrizes do guia Revised Standards for Quality Improvement Reporting Excellence. Resultados: na fase de revisão, identificou-se conteúdo semelhante entre as diretrizes para o delineamento e a organização do constructo. A tecnologia construída possibilitou a coleta de dados, avaliação e classificação de risco do pé diabético. Na etapa de validação, 18 juízes avaliaram o instrumento, que teve o IVC 0,96. Conclusão: o estudo poderá contribuir para melhorar os indicadores referente as hospitalizações, amputações, mobilidade diminuída, dependência, fragilidade e mortalidade advindos do pé diabético.

ABSTRACT This article explores and synthesizes the literature on policy design, a subfield of policy analysis that has received increasing attention over the last few decades. During this period, it contributed to expanding knowledge on the functioning of public policies, implementation instruments, the rationality of decision-making processes, governance systems, social effects, etc. Despite the policy design’s importance, the current literature lacks a systematic analysis. Contributing to this agenda, this review aims to provide a field’s networking linkage, summarizing and classifying existing research through bibliometric and systematic reviews. To do so, data were collected on the Web of Science database covering the period from 1945 to December 31, 2020, capturing 493 articles in the initial search, and publications’ connectedness was analyzed by creating a network map with the frequency of assessments of a simultaneous citation using VOSViewer, and then each cluster was scrutinized applying qualitative methods to understand their relationship. As a result, 184 papers focused explicitly on policy design were selected and then clustered in nine different groups. It should be noticed that implementation instruments represent a very fruitful field of the design’s perspective, influencing five clusters, and other relevant topics concerning governance, means of interaction and representation, institutional analysis, and the policy design theoretical construction also appeared. Regardless of developing countries’ clusters, most studies empirically focused on China, evidencing that there is room for studies concerning Latin American and African contexts.

RESUMEN Este artículo explora y sintetiza la literatura sobre el diseño de políticas, un subcampo del análisis de políticas que ha recibido una atención creciente en las últimas décadas. Durante este período contribuyó a ampliar el conocimiento sobre el funcionamiento de las políticas públicas, los instrumentos de implementación, la racionalidad de los procesos de toma de decisiones, los sistemas de gobernanza, los efectos sociales, etc. A pesar de la importancia de lo diseño de políticas, la literatura actual carece de un análisis sistemático, contribuyendo a esta agenda, esta revisión tiene como objetivo proporcionar un vínculo de red de campo, resumiendo y clasificando la investigación existente a través de revisiones bibliométricas y sistemáticas. Para ello, recopilamos los datos en la base de datos de Web of Science desde 1945 hasta el 31 de diciembre de 2020, capturando 493 artículos a través de la búsqueda inicial, y analizamos la conectividad de las publicaciones creando un mapa de red a través de evaluaciones de frecuencia de una cita simultánea usando VOSViewer, y luego examinamos cada grupo aplicando métodos cualitativos para comprender su relación. Como resultado, seleccionamos solo 184 artículos centrados explícitamente en el diseño de políticas, agrupados en nueve grupos diferentes. Pudimos notar que los instrumentos de implementación representan un campo muy fructífero de la perspectiva del diseño, incidiendo en cinco clústeres, y también aparecieron otros temas relevantes relacionados con la gobernanza, los medios de interacción y representación, el análisis institucional y la construcción teórica del diseño de políticas. A pesar de la existencia del clúster de países en desarrollo, la mayoría de los estudios se han centrado empíricamente en China, lo que demuestra que hay espacio para estudios sobre los contextos latinoamericano y africano.

RESUMO Este artigo explora e sintetiza a literatura sobre desenho de políticas, um subcampo da análise de políticas que tem recebido atenção crescente nas últimas décadas. Nesse período, tal subcampo contribuiu para ampliar o conhecimento sobre o funcionamento das políticas públicas, dos instrumentos de implementação, da racionalidade dos processos decisórios, dos sistemas de governança, dos seus efeitos sociais etc. Apesar da importância do desenho de políticas, a literatura atual carece de uma análise sistemática. Colaborando para essa agenda, esta revisão teve como objetivo fornecer a ligação em rede do campo, resumindo e classificando as pesquisas existentes por meio de revisões bibliométricas e sistemáticas. Para isso, coletamos os dados na base de dados Web of Science de 1945 a 31 de dezembro de 2020, capturando 493 artigos por meio da busca inicial, e analisamos a conectividade das publicações criando um mapa de rede por meio de avaliações de frequência de uma citação simultânea usando o VOSViewer. Em seguida, examinamos cada cluster aplicando métodos qualitativos para entender seu relacionamento. RESUMO As Diretrizes Brasileiras para Reabilitação do AVC são fruto de um esforço conjunto do Departamento Científico de Reabilitação Neurológica da Academia Brasileira de Neurologia com o objetivo de orientar os profissionais envolvidos no processo de reabilitação para a redução da incapacidade funcional e aumento da autonomia dos indivíduos. Membros do grupo acima participaram de fóruns de discussão na web com pré-temas, seguidos de reuniões por videoconferência em que as controvérsias foram discutidas, levando a um consenso. Essas diretrizes, divididas em duas partes, focam as implicações de recentes ensaios clínicos, revisões sistemáticas e metanálises sobre reabilitação do AVC. O objetivo principal é servir de orientação a médicos, fisioterapeutas, fonoaudiólogos, terapeutas ocupacionais, enfermeiros, nutricionistas e demais profissionais envolvidos no cuidado pós-AVC. As recomendações e níveis de evidência foram adaptados de acordo com a literatura disponível atualmente. Aqui é apresentada a Parte I sobre tópicos de reabilitação na fase aguda, prevenção e tratamento de doenças e comorbidades frequentes após o AVC.

ABSTRACT The Guidelines for Stroke Rehabilitation are the result of a joint effort by the Scientific Department of Neurological Rehabilitation of the Brazilian Academy of Neurology aiming to guide professionals involved in the rehabilitation process to reduce functional disability and increase individual autonomy. Members of the group participated in web discussion forums with predefined themes, followed by videoconference meetings in which issues were discussed, leading to a consensus. These guidelines, divided into two parts, focus on the implications of recent clinical trials, systematic reviews, and meta-analyses in stroke rehabilitation literature. The main objective was to guide physicians, physiotherapists, speech therapists, occupational therapists, nurses, nutritionists, and other professionals involved in post-stroke care. Recommendations and levels of evidence were adapted according to the currently available literature. Part I discusses topics on rehabilitation in the acute phase, as well as prevention and management of frequent conditions and comorbidities after stroke.

Resumo Fundamento Alguns pacientes com COVID-19 apresentam injúria miocárdica. Objetivo Detectar a injúria miocárdica em pacientes criticamente doentes, e comparar o envolvimento cardíaco entre crianças com síndrome respiratória aguda grave (SARS) e crianças com síndrome inflamatória multissistêmica (MIS-C). Métodos Todas as crianças acometidas da COVID-19 admitidas em uma unidade de terapia intensiva de referência foram cadastradas de forma prospectiva e fizeram uma ecografia transtorácica bidimensional (ETT-2D) e um teste de troponina I cardíaca (cTnI) nas primeiras 72 horas. Para a análise estatística, um p <0,05 bilateral foi considerado significativo. Resultados 33 pacientes foram incluídos, dos quais 51,5% apresentaram cTnI elevada e/ou ETT-2D anormal e 36,4% precisaram de suporte cardiovascular, que foi mais frequente em pacientes com cTnI elevada e anormalidades em ETT-2D do que em pacientes com exames normais (83,3% e 33,3%, respectivamente; p 0,006, 95% IC = 0,15-0,73). Os achados de ETT-2D mais comuns foram efusão pericárdica (15,2%) e regurgitação tricúspide/mitral (15,2%). Sinais de envolvimento cardíaco foram mais comuns na MIS-C que na SARS. Pacientes com MIS-C também apresentaram um índice mais alto de necessidade de suporte cardiovascular (66,7% X 25%, p 0,03, 95% IC = -0,7 a -0,04) e um índice mais frequente de cTnI elevada (77,8% X 20,8%; p 0,002, 95% IC = 0,19 a 0,79). Os valores preditivos negativos de cTnI para detecção de anormalidades de ETT-2D foram 100% para pacientes com MIS-C, e 73,7% para pacientes com SARS. Conclusão Sinais de injúria cardíaca foram comuns, especialmente em pacientes com MIS-C. As anormalidades na ETT-2D foram sutis. A realização de um teste de cTnI na admissão pode ajudar os prestadores de assistência de saúde a discriminar os pacientes com uma necessidade mais urgente de uma ETT-2D.

Abstract Background Some patients with COVID-19 present myocardial injury. Objective To detect myocardial injury in critically ill paediatric patients, and to compare cardiac involvement between children with severe acute respiratory syndrome (SARS) and children with multisystemic inflammatory syndrome (MIS-C). Methods All COVID-19 children admitted to a referral intensive care unit were prospectively enrolled and had a two-dimensional echocardiogram (2D-TTE) and a cardiac troponin I (cTnI) assay within the first 72 hours. For statistical analysis, two-sided p < 0.05 was considered significant. Results Thirty-three patients were included, of which 51.5% presented elevated cTnI and/or abnormal 2D-TTE and 36.4% needed cardiovascular support, which was more frequent in patients with both raised cTnI and 2D-TTE abnormalities than in patients with normal exams (83.3% and 33.3%, respectively; p 0.006, 95% CI = 0.15-0.73). The most common 2D-TTE findings were pericardial effusion (15.2%) and mitral/tricuspid regurgitation (15.2%). Signs of cardiac involvement were more common in MIS-C than in SARS. MIS-C patients also presented a higher rate of the need for cardiovascular support (66.7% vs 25%, p 0.03, 95% CI = -0.7 to -0.04) and a more frequent rate of raised cTnI (77.8% vs 20.8%; p 0.002, 95% CI = 0.19 to 0.79). The negative predictive values of cTnI for the detection of 2D-TTE abnormalities were 100% for MIS-C patients and 73.7% for SARS patients. Conclusion signs of cardiac injury were common, mainly in MIS-C patients. 2D-TTE abnormalities were subtle. To perform a cTnI assay upon admission might help providers to discriminate those patients with a more urgent need for a 2D-TTE.

RESUMO Antecedentes: Poucos estudos brasileiros investigaram fatores de risco para disfagia e suas complicações associadas em uma grande coorte. Objetivo: Investigar frequência, preditores e desfechos associados da disfagia em pacientes até três meses após acidente vascular cerebral (AVC). Métodos: Selecionamos pacientes admitidos consecutivamente em um centro especializado em AVC agudo. Excluímos pacientes com ataque isquêmico transitório, hemorragia subaracnóidea, trombose venosa cerebral, AVC hemorrágico de causa secundária, AVC não agudo ou aqueles que não consentiram em participar. A deglutição foi avaliada por fonoaudiólogos, por meio do teste de deglutição de volume-viscosidade. A função geral foi avaliada usando-se escala de Rankin modificada, índice de Barthel e medida de independência funcional. Resultados: Foram admitidos 831 pacientes e incluídos 305. A idade média foi 63,6±13,3 anos, o tempo médio da avaliação foi 4,2±4,1 dias e 45,2% apresentavam disfagia. Idade (razão de chances [OR] 1,02; intervalo de confiança [IC95%] 1,00-1,04; p=0,017), história médica conhecida de apneia obstrutiva do sono (OR=5,13; IC95% 1,74-15,15; p=0,003) e gravidade do AVC na admissão hospitalar (OR=1,10; IC95% 1,06-1,15; p<0,001) foram independentemente associados à disfagia. Disfagia (OR=3,78; IC95% 2,16-6,61; p<0,001) e gravidade do AVC (OR=1,05; IC95% 1,00-1,09; p=0,024) foram independentemente associadas com morte ou dependência funcional em três meses. Conclusões: A disfagia esteve presente em quase metade dos pacientes com AVC. Idade, apneia obstrutiva do sono e gravidade do AVC foram preditores de disfagia, que esteve independentemente associada com morte ou dependência funcional em três meses.

ABSTRACT Background: Few Brazilian studies investigated risk factors for dysphagia and associated complications in a large cohort. Objective: To investigate frequency, predictors, and associated outcomes of dysphagia in patients up to three months post-stroke. Methods: Prospective cohort study of consecutively admitted patients in a specialized center for acute stroke. Patients with a transient ischemic attack, subarachnoid hemorrhage, cerebral venous thrombosis, hemorrhagic stroke with secondary cause, non-acute stroke, or those who did not consent to participate were excluded. Swallowing was evaluated by speech language pathologists using Volume-Viscosity Swallow Test. General function at three months post-stroke was assessed using the following instruments: Modified Rankin scale, Barthel Index and Functional Independence Measure. Results: A total of 831 patients were admitted and 305 patients were included according to the inclusion and exclusion criteria. The mean age of patients was 63.6±13.3 years, mean time from stroke to swallowing assessment was 4.2±4.1 days, and 45.2% of the patients had dysphagia. Age (OR=1.02; 95%CI 1.00-1.04; p=0.017), known medical history of obstructive sleep apnea (OR=5.13; 95%CI 1.74-15.15; p=0.003), and stroke severity at hospital admission (OR=1.10; 95%CI 1.06-1.15; p<0.001) were independently associated with dysphagia. Dysphagia (OR=3.78; 95%CI 2.16-6.61; p<0.001) and stroke severity (OR=1.05; 95%CI 1.00-1.09; p=0.024) were independently associated with death or functional dependence at three months. Conclusions: Dysphagia was present in almost half of stroke patients. Age, obstructive sleep apnea, and stroke severity were predictors of dysphagia, which was independently associated with death or functional dependence at three months.

RESUMO O objetivo deste artigo é compreender de que modo opera a dinâmica de cooperação intermunicipal no Brasil e de que forma os consórcios se inserem na estrutura da regionalização da saúde. Foram realizadas entrevistas com os secretários executivos dos consórcios. Argumentamos que os consórcios contribuem para a gestão municipal, contudo esforços são necessários para a construção de um sistema coordenado e cooperativo de implementação de polícias públicas.

ABSTRACT The article aims to understand how the dynamics of inter-municipal cooperation in Brazil operate and how consortia are inserted in the structure of health regionalization. Interviews were carried out with the Executive Secretaries of the consortia. We argue that consortia contribute to municipal management, however, efforts are needed to build a coordinated and cooperative system for implementing policies.
