RESUMO Objetivo: Analisar o fluxo espacial da assistência de pacientes em terapia dialítica nas regionais de saúde do estado de Minas Gerais. Métodos: Estudo ecológico que teve como população pacientes incidentes em terapia dialítica em instituições públicas, filantrópicas ou que tiveram seu tratamento custeado pelo Sistema Único de Saúde em clínicas privadas conveniadas, no estado de Minas Gerais. Os pacientes foram agregados por regional de saúde de residência. Foram calculadas as proporções de pacientes que fizeram diálise, bem como a inscrição na lista de transplante renal em sua própria região de residência ou fora dela. Estimadas as correlações de Person destas proporções com indicadores socioeconômicos e assistenciais das regionais de saúde. Técnicas exploratórias espaciais estimaram coeficientes de correlação espacial geral (I de Moran) e local (LISA). Resultados: Regiões com maior PIB apresentaram maior razão de nefrologistas e maior proporção de inscrições na própria região de residência. Identificou-se um cluster de regiões com PIB baixo mais ao nordeste do estado (também com valores mais baixos de razão de nefrologistas), um cluster de alta proporção de inscritos na lista de transplante no centro do estado, e um cluster de baixa proporção de diálise na mesma região de residência mais ao sudeste. Conclusão: Evidenciou-se disparidades regionais em relação à proporção de inscritos na lista de espera para o transplante renal, proporção de pacientes que realizavam diálise na mesma região de residência e proporção de pacientes inscritos na lista de espera para o transplante renal na mesma região de residência.

ABSTRACT The aim of this study was to evaluate the effectiveness of ozone as an adjuvant factor in the treatment of periodontal diseases. A systematic review was carried out using the PubMed, LILACS/BIREME, CoChrane, and Scielo databases and manual searches. Clinical studies published in English, Spanish, Portuguese, and French, between 1950 and 2020 that evaluated the effects of ozone in the treatment of periodontal diseases were included. The search resulted in 178 articles, of which 24 met the proposed eligibility criteria and were selected for this review. The risk of bias was assessed for each selected study in accordance with the Cochrane risk-of-bias tool - version 5.1.0. The selected studies evaluated the effect of ozone therapy on periodontitis, gingivitis, and peri-implant diseases. It was observed that the ozone in its oily form presented the best clinical results. In cases of periodontitis, it was concluded that ozone therapy in the gaseous form did not bring any additional benefit. With respect to gingivitis, ozone provided faster remission compared to the control group. And in peri-implant disease cases, ozone had the capacity to reduce the development of mucositis. Regarding microbiology, it was observed that ozone can continuously and significantly reduce periodontal microorganisms; however, with no difference from control groups. Further studies with adequate control of biases are suggested, using ozone in the oily or aqueous form, which are the most promising forms, also evaluating the possible effectiveness of ozone by-products.

RESUMO O objetivo deste estudo foi avaliar a eficácia do uso do ozônio como fator adjuvante no tratamento das doenças periodontais. Foi realizada uma revisão sistemática utilizando as bases de dados PubMed, LILACS/BIREME, CoChrane, Scielo e pesquisa manual. Foram incluídos estudos clínicos publicados em inglês, espanhol, português e francês entre 1950 e 2020, que avaliaram os efeitos do ozônio no tratamento de doenças periodontais. A busca resultou em 178 artigos, destes 24 estudos atenderam aos critérios de elegibilidade propostos e foram selecionados para esta revisão. Foi realizada a avaliação do risco de viés em cada estudo selecionado de acordo com a ferramenta de avaliação de risco de viés Cochrane Risk of Bias Tool - versão 5.1.0. Os estudos selecionados avaliaram o efeito da ozonioterapia na periodontite, gengivite e em doenças peri-implantares. Observou-se que a utilização do ozônio na forma oleosa foi a que apresentou os melhores resultados clínicos. Nos casos de periodontite, concluiu-se que a ozonioterapia não trouxe nenhum benefício adicional sob a forma gasosa. Em relação à gengivite, o ozônio proporcionou uma remissão mais rápida do que do grupo controle. E nas doenças peri-implantares, o ozônio foi capaz de reduzir o desenvolvimento da mucosite. Com relação a microbiologia, foi observado que o ozônio pode reduzir contínua e significativamente os microorganismos periodontais, contudo, sem diferença para grupos controle. Sugerem-se mais estudos com controle adequado de vieses, utilizando o ozônio de forma oleosa ou aquosa, formas mais promissoras, além da avaliação de possível eficácia de subprodutos do ozônio.

ABSTRACT Objective: To identify the lingual foramina in a Brazilian population of 210 individuals through Cone-Beam Computed Tomography images, in order to guide the installation of dental implants in the region of the mental symphysis. Methods: After identifying the lingual foramina on a parasagittal section, four measurements were taken: distance to the alveolar ridge, distance to the mandibular lower border, distance to the limit of the vestibular cortical, and alveolar ridge inclination angle. The measurements were compared in relation to gender by the Mann-Whitney test. The association between gender and the number of foramina was done by the chi-square test. Pearson’s correlation analyzed the linear relationship between age and number of foramina. Results: Lingual foramina are present in 99.6% of the research participants. In the upper-inferior direction, a mean distance to the alveolar ridge of 21.4 mm was noted, while the mean distance to the lower mandibular border was 13.1 mm. In the bucco-lingual direction, a mean distance from the lingual foramina to the vestibular cortical of 15.2 mm is noted. The inclination angle of the alveolar ridge had a mean of 25.4º. Regarding gender, the test was not significant only for the distance to the vestibular cortical. Conclusion: The lingual foramina had a descending way in 100% of cases, being positioned mainly in the middle thirds (66.5%) and lower thirds (32.7%) of the mandibular ridge height. The use of dental implants up to 13 mm in length and up to 4 mm in diameter in the region of the symphysis mentualis is recommended.

RESUMO Objetivos: Identificar as foraminas linguais em uma população brasileira de 210 indivíduos, por meio de imagens de tomografia computadorizada de feixe cônico, a fim de orientar a instalação de implantes dentários em região de sínfise mentual. Métodos: Após identificação das foraminas linguais em corte parasagital, foram realizadas quatro medidas: distância até à crista alveolar, distância até à borda inferior da mandíbula, distância até o limite da cortical vestibular e ângulo de inclinação do rebordo alveolar. As medidas foram comparadas em relação ao sexo pelo teste Mann-Whitney. A comparação entre os sexos e o número de foraminas foi feito pelo teste qui-quadrado. A correlação de Pearson analisou a relação linear entre idade e número de foraminas. Resultados: As foraminas linguais estão presentes em 99,6% dos indivíduos. No sentido súpero-inferior, nota-se uma distância média até à crista alveolar de 21,4 mm, enquanto a distância média até à borda inferior da mandíbula foi de 13,1 mm. No sentido vestíbulo-lingual, nota-se uma distância média da foramina lingual até a cortical vestibular de 15,2 mm. O ângulo de inclinação do rebordo alveolar apresentou uma média de 25,4º. Em relação ao sexo, o teste só não foi significativo para a distância até a cortical vestibular. Conclusão: As foraminas linguais apresentam um trajeto descendente em 100% dos casos, posicionando-se, principalmente, nos terços médios (66,5%) e terços inferiores (32,7%) da altura do rebordo mandibular. Recomenda-se o uso de implantes dentários de até 13 mm de comprimento e de até 4mm de diâmetro em região de sínfise mentual.

Abstract Considering the wide accessibility of population to private community pharmacies, Pharmaceutical Services must be provided comprehensively in such establishments. This research aims to understand how pharmaceutical practice is developed by pharmacists in private community pharmacies of Minas Gerais, Brazil. Qualitative descriptive research was performed. Data were collected through online questionnaires (n=113) and interviews (n=12) with pharmacists working in such institutions and they were analyzed according to Bardin’s Content Analysis, with the contribution of software IRAMUTEQ. Two main categories of analysis were formed: “Professional training of pharmacists and the working conditions in private community pharmacies” and “Pharmaceutical Services in private community pharmacies of Minas Gerais”. Pharmacists understood the population’s healthcare as the main purpose of their professional practice. However, the routine focused on the technical management of medicines and the lack of private rooms hindered the provision of qualified assistance. Furthermore, commercial strategies were identified as motivators for ethical dilemmas and conflicts among the work team. It is suggested that the growth of the pharmaceutical retail market in Minas Gerais should be accompanied by favorable conditions for the production of care, so that pharmaceutical practice in these institutions can be developed in an ethical and responsible way.

ABSTRACT Objective: To analyze the relationship between COVID-19 infodemic and perceived stress in elderly people who use social media. Methods: Cross-sectional study using a web-based survey, 470 aged respondents to sociodemographic questions, exposure to information about COVID-19, and signs and symptoms associated with psychological distress. Outcome variable evaluated by the Perceived Stress Scale and mean score according to independent variables compared by t-test, analysis of variance or Pearson's correlation and final model constructed by multiple linear regression. Results: The significant variables in the final model (p < 0.05) were: TV exposure (β = -0.144; 95%CI= -11.195 - -1.609); exposure through social networks (β = -0.175; CI95% = -8.117 - -0.837); positive screening for psychological distress due to exposure to information about COVID-19 (β = -0.380; 95%CI= -16.033 - -8.975). Conclusion: infodemic can increase stress in the elderly, demanding specific care for this population and the strengthening of their social network.

RESUMEN Objetivo: Analizar la relación entre la infodemia de COVID-19 y el estrés percibido en ancianos usuarios de redes sociales. Métodos: Estudio transversal mediante una encuesta basada en la web; 470 ancianos que respondieron preguntas sociodemográficas, exposición a la información sobre la COVID-19 y los signos y síntomas asociados con la angustia psicológica. La variable se evaluó por la escala de estrés percibido y la puntuación media según las variables independientes, comparadas por t-test, análisis de variación o correlación de Pearson y el modelo final construido por regresión lineal múltiple. Resultados: Las variables significativas en el modelo final (p < 0,05) fueron: exposición a la televisión (β = -0,144; IC95 % = -11,195 - -1,609); exposición a través de redes sociales (β = -0,175; IC95 % = -8,117 - -0,837); cribado positivo de malestar psicológico por exposición a información sobre la COVID-19 (β = -0,380; IC95 %= -16,033 - -8,975). Conclusiones: La infodemia puede aumentar el estrés en los ancianos, lo que exige cuidados específicos para esa población y el fortalecimiento de su red social.

Métodos: Estudo transversal por web-based survey, 470 idosos respondentes a questões sociodemográficas, de exposição a informações sobre COVID-19, e sinais e sintomas associados ao sofrimento psíquico. Variável de desfecho avaliada pela Escala de Estresse Percebido e pontuação média segundo variáveis independentes comparada por teste t, análise de variância ou correlação de Pearson e construído modelo final por regressão linear múltipla. Resultados: As variáveis significativas no modelo final (p < 0,05) foram: exposição pela TV (β = -0,144; IC95 % = -11,195 - -1,609); exposição pelas redes sociais (β = -0,175; IC95 %= -8,117 - -0,837); rastreio positivo para sofrimento psíquico devido à exposição a informações sobre COVID-19 (β = -0,380; IC95 %= -16,033 - -8,975). Conclusão: A infodemia pode potencializar o estresse em idosos, demandando cuidados específicos para essa população e o fortalecimento de sua rede social.

Resumo Sofrimento psíquico e desenvolvimento de transtornos mentais nas prisões são questões de saúde pública reconhecidas mundialmente. Objetivou-se identificar a prevalência destes sintomas e os fatores associados em 99 mulheres com mais de 18 anos de idade, nos regimes provisório, fechado e semiaberto de Juiz de Fora-MG. Trata-se de um estudo transversal, do tipo censo, com dados coletados face a face através de questionário semiestruturado e multidimensional. Os desfechos foram avaliados pelo Patient Health Questionannaire-4 (PHQ-4). Para a análise de associação foi construído um modelo teórico de determinação com três blocos hierarquizados. Foram estimadas razões de prevalência brutas por meio do teste Qui-quadrado e ajustadas entre si dentro de cada bloco (p≤0,20). Para o modelo final de regressão de Poisson com variância robusta foi adotado p≤0,05. A prevalência de sintomas ansiosos e depressivos foi, respectivamente, de 75,8% (IC95% 66,1%-83,8%) e 65,7% (IC95% 55,4%-74,9%). No modelo final, sintomas ansiosos associaram-se à presença de sintomas depressivos. Já sintomas depressivos associaram-se à faixa etária de 20 a 29 anos e à presença de sintomas ansiosos. Verificou-se prevalência dos desfechos em mais da metade das participantes, com destaque para interassociação entre eles.

Abstract Psychological distress and developing mental disorders in prisons are globally recognized public health issues. This study aimed to identify the prevalence of these symptoms and associated factors in 99 women over 18 years of age in the provisional, closed, and semi-open regimes in Juiz de Fora-MG, Brazil. This cross-sectional census study collected data face-to-face through a semi-structured and multidimensional questionnaire. We assessed outcomes using the Patient Health Questionnaire-4 (PHQ-4). We built a theoretical determination model with three hierarchical blocks for the association analysis. We estimated crude prevalence ratios using the chi-square test and adjusted for each other within each block (p≤0.20). We adopted p≤0.05 for the final Poisson regression model with robust variance. The prevalence of anxiety and depressive symptoms was 75.8% (95%CI 66.1%-83.8%) and 65.7% (95%CI 55.4%-74.9%), respectively. In the final model, anxiety symptoms were associated with depressive symptoms. On the other hand, depressive symptoms were associated with the 20-29 years age group and anxiety symptoms. We identified a prevalence of the outcomes in more than half of the participants, emphasizing the inter-association between them.

Abstract Objectives: to evaluate the temporal trend of hospitalizations for pelvic infammatory disease in Brazil and its regions between 2000 and 2019. Methods: longitudinal ecological study with data from the Hospital Information System. The analysis of temporal trends in hospitalization rates by age group was performed using segmented linear regression (joinpoint regression). Both annual percent change total and by age groups were estimated for Brazil and each region. Results: Brazil had an average reduction of 5.2% per year in the period and the age groups most affected were 20 to 29 and 30 to 39 years. North region had the highest rates and South and Southeast regions, the lowest. Midwest region had the largest annual average reduction (8.1%), followed by the Northeast (5.7%), Southeast (5.0%), North (4.6%) and South (4.3 %). The only age group that showed a significant increase was that of 10 to 19 years in the Southeast in the period from 2008 to 2019 (0.9%) and in the Northeast in the period from 2014 to 2019 (3.3%). Conclusions: hospitalization due to pelvic infammatory disease has significantly decreased in Brazil. The increase observed for adolescents in the Southeast and Northeast in the most recent period points to problems in the prevention and control of sexually transmitted infections in this age group.

Resumo Objetivos: avaliar a tendência temporal de internações por doença infamatória pélvica no Brasil e regiões entre 2000 e 2019. Métodos: estudo ecológico longitudinal com dados do Sistema de Informações Hospitalares. A análise das tendências temporais das taxas de internação hospitalar por faixas etárias foi feita por regressão linear segmentada (joinpoint regression). Foram estimadas variações percentuais anuais gerais e por faixas etárias para o Brasil e cada região. Resultados: o Brasil teve uma redução média de 5,2% ao ano no período e as faixas etárias mais afetadas foram 20 a 29 e 30 a 39 anos. A região Norte apresentou as maiores taxas e as regiões Sul e Sudeste as menores. A região Centro-Oeste teve a maior redução média anual (8,1%), seguida das regiões Nordeste (5,7%), Sudeste (5,0%), Norte (4,6%) e Sul (4,3%). A única faixa etária que apresentou um aumento significativo foi a de 10 a 19 anos nas regiões Sudeste no período de 2008 a 2019 (0,9%) e no Nordeste no período de 2014 a 2019 (3,3%). Conclusões: a internação hospitalar por doença infamatória pélvica reduziu no Brasil de forma importante. O aumento verificado para adolescentes no Sudeste e Nordeste no período mais recente aponta para problemas na prevenção e controle das infecções sexualmente transmissíveis nesta faixa etária.

Resumo Introdução A doença renal crônica apresenta-se como um problema de saúde pública por causa de sua prevalência, dos custos envolvidos no tratamento e da alta taxa de morbimortalidade. Objetivo Avaliar a não adesão ao regime terapêutico de pacientes em hemodiálise e fatores associados. Método Estudo transversal com pacientes que realizam hemodiálise em um hospital universitário e duas clínicas privadas conveniadas ao Sistema Único de Saúde. Padrões para avaliação da não adesão ao regime terapêutico hemodialítico tiveram como base os indicadores estabelecidos pelo The Dialysis Outcomes and Practice Patterns Study: restrição hídrica, regime dietético, regime medicamentoso e terapia hemodialítica. Não conformidade em pelo menos um dos aspectos do tratamento foi considerada não aderência à terapêutica. Resultados Indivíduos com idade inferior a 60 anos tiveram maior probabilidade de não aderir à restrição hídrica, à terapêutica e aos regimes dietético e medicamentoso. Pacientes anúricos tiveram maior chance de não adesão à restrição hídrica e à terapêutica. Indivíduos com hipoalbuminemia e hipoemoglobinemia tiveram maior probabilidade de não aderir à restrição hídrica, enquanto pacientes que se autodeclararam não brancos apresentaram maior chance de não aderir à terapêutica. Nenhuma variável explicativa da adesão à terapia hemodialítica se manteve significativa no modelo final da regressão logística. Conclusão A inconformidade relacionada aos aspectos do tratamento pode resultar em aumento de hospitalização e gastos com saúde, piora do estado clínico, maiores intercorrências no tratamento e aumento da taxa de mortalidade.

Abstract Background Chronic kidney disease presents itself as a public health problem because of its prevalence, the costs involved in treatment and the high rate of morbidity and mortality. Objective To evaluate non-adherence to the therapeutic regimen of hemodialysis patients and associated factors. Method A cross-sectional study with patients undergoing hemodialysis at a University Hospital and two private clinics agreed to the Unified Health System. Standards for non-adherence to the hemodialysis regimen were based on indicators established by The Dialysis Outcomes and Practice Patterns Study: water restrictions, dietary, drug regimen, and hemodialysis therapy. Non-compliance in at least one aspect of treatment was considered non-adherence to therapy. Results Individuals younger than 60 years were more likely to be non-adherent to water restriction, dietary and drug regimens, and therapy. Anuric patients were more likely to be non-adherent to water restriction and therapy. Individuals with hypoalbuminemia and hypohemoglobinemia were more likely not to adhere to water restriction, while patients who self-declared themselves to be non-whites had a higher chance of not adhering to therapy. No explanatory variables of adherence to hemodialysis therapy remained significant in the final logistic regression model. Conclusion Non-conformity related to treatment aspects can result in increased hospitalization and health expenses, worsening of clinical status, greater intercurrences in treatment and increase in mortality rate.

Resumo Objetivo Determinar a prevalência e as possíveis variáveis associadas à neoplasia intraepitelial anal e ao câncer anal em mulheres imunocompetentes com neoplasia intraepitelial cervical de alto grau. Métodos Estudo transversal em mulheres imunocompetentes com diagnóstico histológico de neoplasia intraepitelial cervical de alto grau e câncer cervical, feito entre janeiro de 2016 e setembro de 2020. Todas as mulheres foram submetidas a citologia anal e responderam a um questionário de caracterização e potenciais fatores de risco. Mulheres com citologia alterada foram submetidas a anuscopia e biópsia. Resultados No total, 69 mulheres foram incluídas no estudo. Destas, 7 (10,1%) tiveram resultados anormais de citologia anal (lesão de alto grau, células escamosas atípicas de significado indeterminado, e células escamosas atípicas, não se pode excluir lesões de alto grau: 28,5% cada; lesão de baixo grau: 14,3%). Das anuscopias, 3 (42,8%) demonstraram alterações. Das 2 biópsias realizadas, apenas 1 apresentou neoplasia intraepitelial anal de baixo grau. O número médio de gestações, partos vaginais e abortos estava associado à citologia anal anormal. No entanto, a maior média de partos cesáreos estava associada à citologia normal. Conclusão A prevalência de neoplasia intraepitelial anal foi compatível com dados de estudos recentes, principalmente daqueles feitos no Brasil. O rastreamento oportunista para neoplasia intraepitelial anal nesta população de alto risco deve ser considerado. A citologia anal é adequada para esse fim, devido ao seu baixo custo e viabilidade nos serviços públicos de saúde.

Abstract Objective To determine the prevalence and possible variables associated with anal intraepithelial neoplasia and anal cancer in immunocompetent women with high-grade cervical intraepithelial neoplasia. Methods A cross-sectional study involving immunocompetent women with a histological diagnosis of high-grade cervical intraepithelial neoplasia and cervical cancer, conducted between January 2016 and September 2020. All women underwent anal cytology and answered a questionnaire on characterization and potential risk factors. Women with altered cytology were submitted to anoscopy and biopsy. Results A total of 69 women were included in the study. Of these, 7 (10.1%) had abnormal anal cytology results: (high-grade lesion, atypical squamous cells of undetermined significance, and atypical squamous cells, cannot exclude high-grade lesions: 28,5% each; low grade lesion: 14,3%). Of the anoscopies, 3 (42.8%) showed alterations. Of the 2 (28,5% of all abnormal cytology results) biopsies performed, only 1 showed low-grade anal intraepithelial neoplasia. The average number of pregnancies, vaginal deliveries, and abortions was associated with abnormal anal cytology. However, the highest mean regarding the cesarean sections was associated with normal cytology. Conclusion The prevalence of anal intraepithelial neoplasia was compatible with data from recent studies, especially those conducted in Brazil. Opportunistic screening for anal intraepithelial neoplasia in this high-risk population should be considered. Anal cytology is suitable for this purpose, due to its low cost and feasibility in public health services.

Resumo Objetivo Verificar a prevalência da autoavaliação ruim do estado de saúde em mulheres encarceradas e analisar os fatores associados. Método Trata-se de estudo transversal, realizado entre os anos de 2019 e 2020, por meio de censo, com participação de 99 mulheres. A análise dos fatores associados ao desfecho foi conduzida a partir de um modelo teórico de determinação com três blocos hierarquizados de variáveis. As variáveis foram ajustadas entre si dentro de cada bloco. Aquelas com nível de significância ≤ 0,20 foram incluídas no modelo de regressão de Poisson e ajustadas ao nível superior ao seu, considerando o nível de 5% de significância. Resultados A prevalência da autoavaliação ruim da saúde foi de 31,3% (IC95% = 22,8%–40,9%). Morbidade referida, presença de sintomas de ansiedade e a pior perspectiva em relação às condições de saúde pós-encarceramento foram as variáveis associadas com o desfecho. Considerações finais e implicações para a prática Os fatores associados à ocorrência do evento investigado poderão direcionar medidas que visem à redução dos impactos à saúde durante o período de encarceramento.

Abstract Objective To verify the prevalence of poor self-rated health status among incarcerated women and to analyze the associated factors. Method This is a cross-sectional study, carried out between 2019 and 2020, by means of a census, with the participation 99 women incarcerated. The analysis of factors associated with the outcome was conducted based on a theoretical model of determination with three hierarchical blocks of variables. Variables were adjusted to each other within each block. Those with significance level ≤ 0.20 were included in the Poisson regression model and adjusted to a level higher than theirs, considering a 5% level of significance. Results The prevalence of poor self-rated health was 31.3% (IC95% = 22.8% - 40.9%). Reported morbidity, presence of anxiety symptoms and the worst perspective regarding post-incarceration health conditions were the variables associated with the outcome. Conclusion and implications for practice The factors associated to the occurrence of the investigated event may direct measures aimed to reduce health impacts during the incarceration period.

Resumen Objetivo Verificar la prevalencia de autoevaluación negativa de la salud en mujeres encarceladas y analizar los factores asociados. Método Se trata de un estudio transversal, realizado entre los años 2019 y 2020, mediante censo, con la participación de 99 mujeres encarceladas. El análisis de factores asociados al resultado se realizó con base en un modelo teórico de determinación con tres bloques jerárquicos de variables. Las variables se ajustaron entre sí dentro de cada bloque. Aquellos con nivel de significancia ≤ 0,20 se incluyeron en el modelo de regresión de Poisson y se ajustaron a un nivel superior al de ellos, considerando un nivel de significancia del 5%. Resultados La prevalencia de autoevaluación negativa de la salud fue 31,3% (IC 95% = 22,8% -40,9%). La morbilidad autoinformada, la presencia de síntomas de ansiedad y la peor perspectiva con respecto a las condiciones de salud después del encarcelamiento fueron las variables asociadas con el resultado. Conclusión e implicaciones para la práctica Los factores asociados a la ocurrencia del evento investigado pueden conducir a medidas destinadas a reducir los impactos en la salud durante el período de encarcelamiento.

ABSTRACT Background: Social conditions are related to the impact of epidemics on human populations. This study aimed to investigate the spatial distribution of cases, hospitalizations, and deaths from COVID-19 and its association with social vulnerability. Methods: An ecological study was conducted in 81 urban regions (UR) of Juiz de Fora from March to November 2020. Exposure was measured using the Health Vulnerability Index (HVI), a synthetic indicator that combines socioeconomic and environmental variables from the Demographic Census 2010. Regression models were estimated for counting data with overdispersion (negative binomial generalized linear model) using Bayesian methods, with observed frequencies as the outcome, expected frequencies as the offset variable, and HVI as the explanatory variable. Unstructured random-effects (to capture the effect of unmeasured factors) and spatially structured effects (to capture the spatial correlation between observations) were included in the models. The models were estimated for the entire period and quarter. Results: There were 30,071 suspected cases, 8,063 confirmed cases, 1,186 hospitalizations, and 376 COVID-19 deaths. In the second quarter of the epidemic, compared to the low vulnerability URs, the high vulnerability URs had a lower risk of confirmed cases (RR=0.61; CI95% 0.49-0.76) and a higher risk of hospitalizations (RR=1.65; CI95% 1.23-2.22) and deaths (RR=1.73; CI95% 1.08-2.75). Conclusions: The lower risk of confirmed cases in the most vulnerable UR probably reflected lower access to confirmatory tests, while the higher risk of hospitalizations and deaths must have been related to the greater severity of the epidemic in the city’s poorest regions.

ABSTRACT Objective To characterize the epidemiological profile of patients aged 50 years or older diagnosed as HIV/AIDS, in a Specialized Service of the Public Health System. Methods A retrospective cohort study using secondary data from medical records in the period 2014 to 2018. Sociodemographic and clinical characteristics, and features related to treatment adherence were organized in a database. Quantitative variables were expressed as mean (or median) ± standard deviation (or interquartile range), and categorical variables expressed as number and percentage of patients. The Kaplan-Meier method was applied to assess the probability of overall specific survival. Results Of the 241 initially eligible patients, 149 patients were evaluated, registering 19 deaths in the studied period. There was a predominance of males aged 50-59 years, with severe immunodeficiency upon admission (29.7%), and with a CD4+ T lymphocyte count below 200 cells in 62 (46.3%) of patients. Elderly people aged 61 or over were more adherent. There was an increase in the CD4+ T lymphocyte count by an average of 139.63 in the first 6 months, and 50.51 from the first 6 months to 12 months of follow-up, with an average increase in the first 12 months of 157.63 cells. Specific overall survival in the period was 85%. Conclusion Patients older than 50 years had an immune response and no viral load detection in the 12-month period, deserving further studies to improve survival.

Resumo: Introdução: A orientação comunitária apresenta, em estudos de avaliação dos atributos da atenção primária à saúde, baixa performance entre os usuários. Questiona-se, em especial, se a qualificação profissional em Medicina de Família e Comunidade (MFC) contribui para a otimização desse desempenho. Objetivo: Este estudo teve como objetivo verificar a associação entre qualificação profissional em MFC e o grau de orientação comunitária na perspectiva dos profissionais médicos atuantes na Estratégia Saúde da Família de Belo Horizonte. Método: Trata-se de estudo censitário que reuniu médicos de todas as regionais de Belo Horizonte. Variáveis sociodemográficas relativas à formação acadêmica e à situação ocupacional foram obtidas por meio de questionário. Utilizou-se o instrumento PCATool-Brasil para medir o desempenho do atributo. Resultado: A comparação entre desempenho do escore orientação comunitária e variáveis independentes foi feita pelo teste qui-quadrado, com correção de Fischer. O escore médio foi de 7,9 (DP ±1,2). Nenhuma variável independente foi associada com a orientação comunitária satisfatória. Houve correlação entre tempo de conclusão da residência médica em MFC e alto escore para orientação comunitária. Conclusão: A qualificação em MFC não determina, por si só, um alto grau de orientação comunitária. O maior tempo de formação em MFC na modalidade residência médica correlacionou-se com maior desempenho desse atributo derivado.

Abstract: Introduction: In studies evaluating the attributes of Primary Health Care, Community Orientation has shown weak performance among users. In particular, questions remain over whether professional qualification in Family Practice (FP) helps optimize this performance. Purpose: verify the association between professional qualification in FP and the degree of Community Orientation from the perspective of medical professionals working in the Family Health Strategy of Belo Horizonte. Methods: This census study was applied to doctors in Belo Horizonte. Sociodemographic variables related to academic training and occupational status were obtained through a questionnaire. The PCATool-Brasil was used to measure the performance of this attribute. Result: The comparison between the performance of the Community Orientation score and independent variables was made using the chi-square test, with Fischer correction. The average score was 7.9 (SD ± 1.2). No independent variable was associated with a satisfactory level of Community Orientation. There was a correlation between time of completion of medical residency in FP and a high Community Orientation score. Conclusion: Qualification in FP does not itself determine a high degree of Community Orientation. Longer training in FP medical residency was correlated to better performance of this derived attribute.

Resumo Objetivo Analisar o perfil sociodemográfico e repercussões da infodemia de covid-19 nas alterações psicopatológicas em idosos com acesso a mídias digitais. Método Estudo transversal por web-based survey na população acima de 60 anos que utiliza mídias digitais. A análise descritiva contemplou variáveis sociodemográficas e relacionadas à exposição a informações sobre covid-19; analisou-se um conjunto de sinais e sintomas de alterações psicopatológicas devido a essa exposição. Os dados foram analisados pelos testes Mann-Whitney, Kruskal-Wallis e coeficiente de correlação de Spearman. Resultados Participaram da pesquisa 470 pessoas idosas, a maioria na faixa etária de 60 a 69 anos (61,3%), sexo feminino (67,2%), raça/cor branca (71,1%) e com nível superior ou maior escolaridade (41,3%). A exposição às informações sobre covid-19 ocorreu em 89,4% dos respondentes pela televisão e em 71,3%, pelas redes sociais, tendo causado preocupação (76,9%) e medo, tanto da morte de pessoas queridas (76,8%) quanto de adoecer (74%). Esses sinais e sintomas de alterações psicopatológicas estavam presentes em 3,8% das mulheres e 5,9% dos homens, associados à menor nível de escolaridade, diminuição da renda pós-pandemia e ao fato do idoso se sentir afetado pelas notícias sobre covid-19 veiculadas nas redes sociais. Conclusão A infodemia de covid-19 foi associada a alterações psicopatológicas em idosos. Mesmo com o aumento do acesso às redes sociais, a principal fonte de informação dessa população é a televisão. Estratégias de inclusão e alfabetização digitais para a população idosa devem ser pensadas como meio para lidarem com a infodemia de covid-19.

Abstract Objective to analyze the sociodemographic profile and COVID-19 infodemic’s repercussions on psychopathological alterations in older people with access to digital media. Method cross-sectional study by web-based survey in the population over 60 years old who use digital media. Descriptive analysis considered sociodemographic variables and other variables related to exposure to information about COVID-19; a set of signs and symptoms to psychopathological alterations due to this exposure were analyzed. Data analyzed by the Mann-Whitney, Kruskal-Wallis tests and Spearman’s correlation coefficient. Results a total of 470 older people were included in the survey, 61.3% aged 60 to 69 years, 67.2% female, 71.1% white race/color and 41.3% with higher education. 89.4% of respondents be exposed to news and information about COVID-19 on television, and 71.3% on social network. The most cited psychic symptoms were feeling worried (76.9%), fear of death of dear people (76.8%) and fear of getting sick (74%). Psychopathological alterations were observed in 3.8% of women and 5.9% of men. These sign and symptoms were associated with a lower level of education, a decrease in post-pandemic income and felt affected by the news about COVID-19 on social networks. Conclusion COVID-19 infodemic is associated with psychopathological alterations in older people. Even with the increase in access to social networks, the main media used by older people is television. Strategies for digital inclusion and literacy should be developed, offering tools for older people to fight the COVID-19 infodemic.
