ABSTRACT The incidence and clinical characteristics of NTM diseases in Brazil remain relatively unknown. The present study describes the diagnosis of NTM isolates, the clinical presentation and treatment outcomes. We analyzed NTM isolates in patients of a tertiary hospital in the Southeast region of Brazil, from January 2008 to July 2019. The ATS/IDSA criteria for diagnosis and treatment of these patients was applied. Mycobacterium kansasii were identified in 13/113 (11.5%) patients. In 59/113 (52.2%) patients who met the ATS criteria for disease, 29/59 (49.1%) received treatment, and 22/29 (75.8%) were cured. The major species identified was M. kansasii. The most frequent symptoms among the treated patients were dyspnea and cough, and the proportion of cured patients was high.

Objective: The stigma toward individuals with mental disorders is highly prevalent, not only in the general population but among health care providers as well. The aim of this study was to identify subgroups based on stigmatizing beliefs related to psychiatric disorders among Brazilian psychiatrists, as well as to investigate their association with clinical and personality characteristics. Methods: Latent cluster analysis was used to find subgroups of cases in multivariate data according to a psychotic (schizophrenia) and a nonpsychotic disorder (attention-deficit hyperactivity disorder). The clusters for each psychiatric disorder were compared according to sociodemographic, emotional traits, and personality characteristics. Results: A total of 779 psychiatrists answered the questionnaire. Three different subgroups of stigma levels were identified regarding schizophrenia: the highest (n=202 [51.7%]), intermediate (108 [27.6%]), and the lowest (81 [20.7%]). Participants from the highest stigma group had a significantly longer time since graduation, higher anxiety-state scores, and lower positive affect. Two subgroups were identified with respect to attention-deficit hyperactivity disorder, although there were no differences between them in sociodemographic or clinical variables. Conclusion: There were more subgroups of stigmatizing beliefs regarding psychotic disorders. Individual characteristics, such as those related to trait anxiety and affect, can be associated with high stigma toward schizophrenia.

Resumo Investigamos os mecanismos envolvidos na relação entre mudanças de uso da terra e biodiversidade aquática, utilizando a ictiofauna de riachos do Cerrado como modelo de estudo. Testamos a predição de que a degradação da paisagem reduz a heterogeneidade ambiental e muda os tipos predominantes de habitat, por sua vez, diminuindo a diversidade e alterando a identidade funcional de comunidades de peixes. Amostramos 40 riachos da bacia do Alto Rio Paraná, e avaliamos as condições da drenagem e do habitat físico local. Em seguida, conduzimos uma análise ecomorfológica para caracterizar funcionalmente todas as espécies (36) e quantificar diferentes facetas da estrutura funcional das comunidades. Detectamos múltiplos caminhos de impacto das alterações da paisagem sobre a ictiofauna. A degradação das bacias de drenagem reduziu complexidade do leito e heterogeneidade no sombreamento pelo dossel, diminuindo especialização e divergência funcional das comunidades. Alterações na paisagem também reduziram volume de água e quantidade de pedras grandes nos riachos, resultando em diminuição na abundância de espécies de maior porte e com atributos morfológicos que favorecem a natação na coluna d’água. Concluímos que a intensificação dos usos da terra causa alterações significativas para a biodiversidade aquática no Cerrado, reforçando a necessidade de especial atenção a este hotspot global.
Abstract We investigated the mechanisms involved in the relationship between land-use changes and aquatic biodiversity, using stream fish assemblages of the Brazilian Savanna (i.e., Cerrado) as a study model. We tested the prediction that landscape degradation would decrease environmental heterogeneity and change predominant physical-habitat types, which in turn would decrease the functional diversity and alter the functional identity of fish assemblages. We sampled fish from 40 streams in the Upper Paraná River basin, and assessed catchment and instream conditions. We then conducted an ecomorphological analysis to functionally characterize all species (36) and quantify different facets of the functional structure of assemblages. We detected multiple pathways of the impacts from landscape changes on the fish assemblages. Catchment degradation reduced the stream-bed complexity and the heterogeneity of canopy shading, decreasing assemblage functional specialization and divergence. Landscape changes also reduced the water volume and the amount of large rocks in streams, resulting in decreased abundances of species with large bodies and with morphological traits that favor swimming in the water column. We conclude that land-use intensification caused significant changes in aquatic biodiversity in the Cerrado, reinforcing the need to pay special attention to this global hotspot.

ABSTRACT Reinfection by the severe acute respiratory syndrome coronavirus type 2 (SARS-COV-2) has been reported in many countries, suggesting that the virus may continue to circulate among humans despite the possibility of local herd immunity due to massive previous infections. The emergence of variants of concern (VOC) that are more transmissible than the previous circulating ones has raised particular concerns on the vaccines effectiveness and reinfection rates. The P.1 lineage was first identified in December 2020 in Manaus city and is now globally spread. We report the first case of reinfection of SARS-CoV-2 caused by the P.1 variant outside of Manaus. The potential of these new variants to escape naturally and vaccine- induced immunity highlights the need for a global vigilance.

ABSTRACT Hemophagocytic lymphohistiocytosis is a rare complication in Rickettsia typhi infections. We report the case of a 2-year-old boy with sudden night-onset fever, pallor, neck adenopathy and erythematous macular rash on the thorax, thighs and buttocks. During admission, he developed hyponatremia, hypoalbuminemia, anemia, thrombocytopenia, leukopenia, neutropenia, liver damage, hemorrhages and persistent fever. No hematological improvement was observed after the initial management, neoplastic diseases were discarded by bone marrow aspiration and lymph node biopsy; hemophagocytic lymphohistiocytosis was diagnosed. By immunohistochemistry and indirect immunofluorescence, murine typhus was also diagnosed and doxycycline was started with transitory recovery. Later, the child developed kidney failure and distributive shock that evolved to cardiac arrest and death. This is the first case report in Mexico on a fatal murine typhus associated with hemophagocytic lymphohistiocytosis in which the etiology was evidenced by histopathology.

RESUMO Objetivos: Avaliar a mortalidade entre adolescentes e pacientes adultos jovens com doenças crônicas acompanhados em um hospital terciário na América Latina. Métodos: Foi feito um estudo retrospectivo transversal em um hospital terciário/universitário no Estado de São Paulo, Brasil. Houve mortalidade de 529/2.850(18,5%) pacientes adolescentes e adultos jovens com doenças crônicas, porém 25/529(4,7%) foram excluídos devido a prontuários médicos incompletos. Portanto, foram avaliados 504 óbitos. Resultados: Ocorrem 316/504(63%) óbitos entre pacientes no início da adolescência e 188/504(37%) pacientes no fim da adolescência/adultos jovens. As comparações adicionais entre os pacientes no início da adolescência (n = 316) e no fim da adolescência/pacientes jovens (n = 188) com doenças crônicas pediátricas na última internação mostraram que a duração média da doença [22,0 (0-173) em comparação com 43,0 (0-227) meses, p < 0,001], foi significativamente menor nos pacientes no início da adolescência em comparação com os pacientes no fim da adolescência/adultos jovens. O número médio de internação anterior foi significativamente menor no primeiro grupo [4,0 (1-45) em comparação com 6,0 (1-52), p < 0,001], ao passo que a última internação na unidade de terapia intensiva foi significativamente maior (60% em comparação com 47%, p = 0,003). Com relação a medidas de suporte, o cuidado paliativo foi significativamente menor no grupo de pacientes no início da adolescência em comparação com o grupo de pacientes no fim da adolescência (33% em comparação com 43%, p = 0,02). As frequências de terapia de substituição renal (22% em comparação com 13%, p = 0,02), agentes vasoativos (65% em comparação com 54%, p = 0,01) e transfusão de hemoderivados (75% em comparação com 66%, p = 0,03) foram significativamente maiores no primeiro grupo. As cinco etiologias mais importantes de doenças crônicas pediátricas foram: neoplasias (54,2%), doenças hepáticas/transplante (10%), vírus da imunodeficiência humana (5,9%), lúpus eritematoso sistêmico de início na infância e artrite idiopática juvenil (4,9%). Foi feita autópsia em 58/504 (11%) e a discordância entre os diagnósticos clínico e pós-morte foi comprovada em 24/58 (41,3%). Conclusões: Quase 20% dos óbitos ocorreram em adolescentes e adultos jovens com diferentes padrões de cuidados de suporte e doenças graves. A discordância entre o diagnóstico clínico e a necropsia foi frequentemente observada.
ABSTRACT Objectives: To evaluate mortality in adolescents and young adult patients with chronic diseases followed in a Latin American tertiary hospital. Methods: A cross-sectional retrospective study was performed in a tertiary/academic hospital in the state of São Paulo, Brazil. Death occurred in 529/2850 (18.5%) adolescents and young adult patients with chronic diseases, and 25/529 (4.7%) were excluded due to incomplete medical charts. Therefore, 504 deaths were evaluated. Results: Deaths occurred in 316/504 (63%) of early adolescent patients and in 188/504 (37%) of late adolescent/young adult patients. Further comparisons between early adolescents (n = 316) and late adolescent/young adult patients (n = 188) with pediatric chronic diseases at the last hospitalization showed that the median disease duration (22.0 [0-173] vs. 43.0 [0-227] months, p < 0.001) was significantly lower in early adolescents vs. late adolescent/young adult patients. The median number of previous hospitalizations was significantly lower in the former group (4.0 [1-45] vs. 6.0 [1-52], p < 0.001), whereas the last hospitalization in intensive care unit was significantly higher (60% vs. 47%, p = 0.003). Regarding supportive measures, palliative care was significantly lower in the younger group compared to the older group (33% vs. 43%, p = 0.02). The frequencies of renal replacement therapy (22% vs. 13%, p = 0.02), vasoactive agents (65% vs. 54%, p = 0.01), and transfusion of blood products (75% vs. 66%, p = 0.03) were significantly higher in the younger group. The five most important etiologies of pediatric chronic diseases were: neoplasias (54.2%), hepatic diseases/transplantation (10%), human immunodeficiency virus (5.9%), and childhood-onset systemic lupus erythematosus and juvenile idiopathic arthritis (4.9%). Autopsy was performed in 58/504 (11%), and discordance between clinical and postmortem diagnoses was evidenced in 24/58 (41.3%). Conclusions: Almost 20% of deaths occurred in adolescents and young adults with distinct supportive care and severe disease patterns. Discordance between clinical diagnosis and autopsy was frequently observed.

Resumo: O artigo analisa a defesa da liberdade sexual, da diversidade e dos direitos sexuais pela Defensoria Pública do Estado de São Paulo e pelos movimentos sociais no processo de inclusão da Profilaxia Pré-Exposição (PrEP) nos protocolos clínicos e diretrizes terapêuticas do Sistema Único de Saúde - SUS para HIV/Aids, enfatizando o contraponto feito nas atuações contra discursos e práticas institucionalizados heteronormativas e tecnocráticas.
Abstract: This article is a brief analysis on sexual freedom, diversity and sexual rights as sustained arguments by Brazilian Public Defense (Defensoria Pública do Estado de São Paulo) and social movements during the process of public consultation and further approval of Pre-Exposure Prophylaxis (PrEP) in the Brazilian Public Health System, emphasizing a substancial critique on technocracy and heteronormativity.

RESUMO Objetivou-se, nesta pesquisa, avaliar os elementos meteorológicos dos ambientes e os componentes de produção da cultura do tomateiro minitomate, em sistema orgânico de produção em diferentes formas de cultivo, doses de biofertilizante e graus tecnológicos de ambientes protegidos. A pesquisa foi conduzida em delineamento inteiramente casualizado em esquema fatorial 3 x 2 x 5 constituído de três casas de vegetação (A - climatizada, B – tela móvel e C – tela fixa) duas formas de cultivo (vasos e canteiros) e cinco doses de biofertilizante comercial (0, 50, 100, 150 e 200% da dose indicada pelo fabricante) com cinco repetições. Os tratamentos influenciaram as variáveis analisadas, exceto para as doses de biofertilizante e interação dos tratamentos. A temperatura do ar se manteve inalterada entre os ambientes e a umidade relativa média e mínima dentro da faixa de controle na casa de vegetação climatizada. A casa de vegetação com tela móvel apresentou os melhores resultados produtivos na forma de cultivo em canteiros ficando as doses de biofertilizante indiferentes a todos os tratamentos.
ABSTRACT The objective of this research was to evaluate the meteorological elements of the environments and the production components of mini tomato crop in organic system in different forms of cultivation, biofertilizer doses and technological levels of protected environments. The research was conducted in completely randomized design in a 3 x 2 x 5 factorial scheme, corresponding to three greenhouses (A - climatized, B - mobile screen and C - fixed screen), two forms of cultivation (pots and beds) and five doses of a commercial biofertilizer (0, 50, 100, 150 and 200% of dose indicated), with five replicates. The treatments affected the analyzed variables, except for biofertilizer doses and interaction of treatments. Air temperature was unchanged among the environments and the mean and minimum relative humidity were within the control range in the climatized greenhouse. The greenhouse with mobile screen showed the best production results for the cultivation in beds and the biofertilizer doses were indifferent for all treatments.

Uso de tela termorrefletora em ambientes protegidos para cultivo do tomateiro

O emprego de telas termorrefletoras, como forma de limitar o aumento da temperatura do ar em ambientes protegidos, deve ser manejado, a fim de evitar a redução excessiva da radiação solar, dada sua importância sobre o desenvolvimento e a produção de hortaliças como o tomateiro. Nesse contexto, os objetivos do trabalho foram comparar os microclimas entre ambientes protegidos cultivados com tomateiro, sendo um coberto somente com polietileno de baixa densidade (PEBD) reforçado e outro coberto com PEBD reforçado associado a uma tela termorrefletora aluminizada retrátil, sob condições de manejo, e avaliar sua adequação às condições climáticas recomendadas à cultura do tomateiro pela bibliografia consultada. O experimento foi conduzido no município de Rio das Pedras-SP, entre 1º-08-11 e 21-01-12. A tela termorrefletora foi empregada somente em dias de céu límpido, entre as 11h e as 16h. Foram coletados dados de temperatura e umidade relativa do ar, radiação solar global (Qs) e radiação fotossinteticamente ativa (RFA) no interior de ambos os ambientes protegidos, e dados de temperatura do ar no ambiente externo. O emprego da tela termorrefletora aluminizada reduziu significativamente os valores médios de Qs, RFA e a temperatura do ar, ao passo que elevou os valores de umidade relativa do ar. O microclima do ambiente com tela adaptou-se melhor às condições climáticas consideradas adequadas pela bibliografia para o cultivo do tomateiro.
The use of thermoreflective screens in order to limit air temperature rising in greenhouses must be based on appropriate management to avoid excessive reduction of solar radiation, due to its importance for horticultural crops development and production such as tomato. Hence, this study aims to compare the microclimates of two greenhouses cultivated with tomato and evaluate their suitability to climatic recommendations for tomato crop provided by the consulted bibliography. For this purpose two different structures were assessed: a greenhouse covered only with reinforced low-density polyethylene (LDPE) and a greenhouse covered with reinforced LDPE and retractable thermoreflective screen under management conditions. The experiment was carried out in Rio das Pedras-SP during 08/01/11 to 01/21/12. The thermoreflective screen was used only on sunny days, between 11:00 and 16:00. Air temperature and relative humidity, global radiation and photosynthetically active radiation inside both greenhouses and air temperature of external environment data were collected. The thermoreflective screen significantly reduced the mean values of global solar radiation, photosynthetically active radiation and air temperature, while increased air relative humidity. The microclimate of the greenhouse with thermoreflective screen is best suited to the climatic conditions considered appropriated by the bibliography recommendations for tomato cultivation.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Crescimento e biomassa de Melia azedarach L. em diferentes espaçamentos e características tecnológicas da madeira visando à produção de carvão

O objetivo deste trabalho foi avaliar o crescimento, quantificar a biomassa e analisar algumas características tecnológicas que influenciam na produção de carvão vegetal da madeira de cinamomo (Melia azedarach L.), implantado em diferentes espaçamentos, em povoamento misto, em Seropédica-RJ. O experimento foi implantado nos espaçamentos 1,0 × 1,0; 1,5 × 1,5; 2,0 × 2,0 e 3,0 × 2,0 m. Um ano após o plantio, 12 indivíduos de cinamomo foram selecionados em cada espaçamento para mensuração de altura. Estas avaliações ocorreram anualmente, até 5 anos. Na última, também foram avaliados o diâmetro no nível do solo e a área de copa e de cada espaçamento; cinco árvores foram selecionadas, abatidas, separadas em componentes da parte aérea e pesadas. Em seguida, amostras de cada componente foram retiradas, pesadas úmidas e colocadas em estufa para determinação de sua massa seca. Em subamostras da madeira dos espaçamentos 1 × 1 e 3 × 2 m, foi determinada a densidade básica e realizada a pirólise para fins energéticos. Cinamomo apresentou maior crescimento em altura e diâmetro no nível do solo no espaçamento 3 × 2 m. Os valores de área de copa não diferiram entre os espaçamentos. A maior produção de biomassa média por árvore ocorreu no espaçamento 2 × 2 m. A madeira é adequada para a produção de carvão vegetal, não tendo sido verificadas diferenças significativas para as características tecnológicas da madeira entre os dois espaçamentos avaliados.
The objective of this study was to evaluate the growth, quantify the biomass, and analyze some characteristics of the wood of chinaberry (Melia azedarach L.) at different planting spacings in a mixed stand in the municipality of Seropédica, Rio de Janeiro state. The following spacings were used: 1.0 × 1.0; 1.5 × 1.5; 2.0 × 2.0; and 3.0 × 2.0 m. Five years after planting, 12 chinaberry individuals were selected, in each spacing, for measurement of height, diameter at ground level, and canopy area. For analysis of biomass, five trees were selected, felled, separated into components of the aerial part and weighed. Then samples of each component were collected, weighed while humid, and placed in stove for dry weight determination. Subsamples of the components of the aerial part were sent for analysis of carbon content. Basic density was determined and pyrolysis for energetic purposes was performed in the subsamples of wood of spacings 1 × 1 m and 3 × 2 m. Chinaberry presented greater growth in height and diameter at ground level in the 3 × 2 m spacing. Canopy area values did not differ between the spacings. The highest average biomass production occurred four years after planting in the 2 × 2 m spacing. The wood of chinaberry tree is suitable for the production of charcoal. No significant differences were observed in the technological characteristics of wood between the two spacings evaluated.

A etapa de produção de mudas é a fase fundamental para obtenção de uniformidade no plantel. Este trabalho avaliou a formação de mudas de bocaiúva (Acrocomia aculeata), com sementes pré-geminadas, em diferentes substratos e ambientes protegidos, na Universidade Estadual de Mato Grosso do Sul, Aquidauana-MS. Como substratos, foram utilizados 100% de esterco bovino, 100% de ramas de mandioca, 100% de vermiculita, 50% de esterco bovino + 50% de ramas de mandioca, 50% de esterco bovino + 50% de vermiculita, 50% de ramas de mandioca + 50% de vermiculita e ⅓ de esterco bovino + ⅓ de ramas de mandioca + ⅓ de vermiculita. Estes substratos foram testados na estufa agrícola coberta com filme de polietileno de baixa densidade (PEBD) de 150 µm, com tela termorrefletora de 50% de sombreamento sob o filme e tela preta de 50% de sombreamento nas laterais, no telado com tela preta de mono filamento de 50% de sombreamento na cobertura e nas laterais, e no telado com tela aluminizada termorrefletora de 50% de sombreamento na cobertura e nas laterais. Foi adotado o delineamento experimental inteiramente casualizado, com 5 repetições de 5 plantas cada. Inicialmente, os dados foram submetidos às análises de variâncias individuais dos substratos, em cada ambiente de cultivo, realizando em seguida a avaliação dos quadrados médios dos resíduos e a análise conjunta dos ambientes, para comparação destes. O melhor ambiente de cultivo é o de tela termorrefletora, comparado à estufa de PEBD e ao telado preto. Todos os substratos que contêm esterco bovino são indicados para a formação de mudas de bocaiúva. A rama de mandioca pura não é indicada para formação de mudas, mesmo utilizando adubação química.
The seedling production stage is the key to achieve uniformity in tree breeding stage. This study evaluated "bocaiúva" (Acrocomia aculeata) seedling formation, with pre-germinated seeds in different substrates and protected environments, in the University of Mato Grosso do Sul State, Aquidauana, MS. As substrates, we used 100% cattle manure (M), 100% cassava branches (CB), 100% vermiculite (V), 50% cattle manure + 50% cassava branches, 50 % cattle manure + 50% vermiculite, 50% cassava branches + 50% vermiculite and ⅓ cattle manure + ⅓ cassava branches + ⅓ vermiculite. These substrates were tested in a greenhouse covered with 150 µm low density polyethylene (LDPE) film under thermo-reflective screen with 50% shading under film; black screen with 50% shading on the sides; black monofilament screen with 50% shading set on roof and sides; and aluminized thermo- reflective screen with 50% shading set on roof and sides. The completely randomized experimental design with 5 replications of 5 plants each was adopted. Initially, data were submitted to analysis of substrate individual variance in each growing environment, then performing the waste mean square evaluation and their environment joint analysis for comparison. The best growing environment is the thermo-reflective screen compared to LDPE greenhouse and black screen set. All substrates containing manure are recommended for bocaiúva seedlings formation. The pure cassava branch is not indicated for seedling, even using chemical fertilizer.

In vitro release of bioidentical hormones in four different liposomal transdermal emulsions (containing testosterone, progesterone, estradiol, or estradiol and estriol) was assessed. For this purpose, novel high-performance liquid chromatography methods were developed and validated in an eco-friendly manner and used to determine the in vitro release of such products. The methods were suitable for our intended goal, and the emulsions employed were found to be effective as transporting candidates for the efficient release of hormones in the transdermal delivery of human sexual steroids.
