ABSTRACT A cross-sectional study was done between October 2014 and June 2015 to estimate the frequency of infection due to intestinal coccidiosis in children admitted to a hospital in Peru, and compare the modified acid-fast staining procedure with the enzyme-linked immunoassay (ELISA) method for the detection of Cryptosporidium spp. Coccidia were detected using the modified acid-fast staining procedure and the Cryptosporidium ELISA method (kit r-Biopharm) on seriated stool samples. Out of a total of 325 children, 5.5% had some type of intestinal coccidiosis: 3.7% involved Cryptosporidium spp. (using both techniques) and 1.8% involved Cyclospora cayetanensis (using the modified acid-fast staining procedure). The modified acid-fast staining procedure and ELISA method revealed a 0.955 consistency in the detection of Cryptosporidium spp. In conclusion, intestinal coccidiosis is frequent in children based on the population studied; similarly, both techniques can be used to detect Cryptosporidium spp., but the modified acid-fast staining procedure has an advantage over the other because it is cost-effective and can detect other coccidia in everyday practice

RESUMEN Con el objetivo de estimar la frecuencia de infección por coccidios intestinales en niños admitidos en un hospital de Perú, y comparar la tinción ácido-resistente modificada (TARM) y el ELISA para la detección de Cryptosporidium spp.; se realizó un estudio transversal entre octubre de 2014 y junio de 2015. Los coccidios se detectaron mediante la TARM y ELISA Cryptosporidium (kit r-Biopharm) en muestras seriadas de heces. De un total de 325 niños, el 5,5% tuvieron algún coccidio intestinal, 3,7% Cryptosporidium spp. (usando ambas técnicas) y 1,8% Cyclospora cayetanensis (TARM). La TARM y ELISA mostraron una concordancia de 0,955 en la detección de Cryptosporidium spp. Se concluye que los coccidios intestinales son frecuentes en niños de la población estudiada; asimismo, ambas técnicas pueden usarse para la detección de Cryptosporidium spp., sin embargo, el menor costo y la capacidad de detectar otros coccidios ofrecen una ventaja a la TARM en la práctica diaria.

Objective. To describe the frequency and antiparasitic in vitro susceptibility of Blastocystis hominis in patients admitted to the Hospital Regional Lambayeque, Peru. Material and methods. A cross-sectional study was conducted from January to August 2015 at 313 patients of all ages. B. hominis detection was performed on serial fecal samples by direct microscopic examination and microculture in modified Locke solution. The in vitro susceptibility testing against the drug metronidazole, nitazoxanide, trimethoprim-sulfamethoxazole and erythromycin was performed in 24 strains of B. hominis, which grew up (microculture method) in 10 double concentrations of each antimicrobial (from 256 ug/ml to 0.5 ug/mL) plus a control. Results. 46.3% (145/313) of the sample had B. hominis, also the age between 12 to 17 years and 60 years was associated with higher frequency of parasites (OR: 2.93 and 2.62). The minimum inhibitory concentration (MIC) 90 of metronidazole and nitazoxanide was 3.19 ug/mL and 11.19 ug/ml, respectively, whereas the MIC 90 of trimethoprim-sulfamethoxazole and erythromycin were above 256 ug/mL. Conclusions. B. hominis occurs in high frequency in patients admitted to the Hospital Regional in Lambayeque, proving to be an important problem of public health in the region. Also B. hominis isolated from these patients were shown to be susceptible in vitro to low concentrations of metronidazole and nitazoxanide so they could be chosen for treatment of this parasite.

Objetivo. Describir la frecuencia y susceptibilidad antiparasitaria in vitro de Blastocystis hominis en pacientes admitidos en el Hospital Regional Lambayeque, Perú. Material y métodos. Se realizó un estudio transversal entre enero y agosto del 2015 en 313 pacientes de todas las edades. La detección de B. hominis se realizó en muestras seriadas de heces mediante examen microscópico directo y microcultivo en solución de Locke modificada. La prueba de susceptibilidad in vitro frente a los fármacos metronidazol, nitazoxanida, trimetoprim-sulfametoxazol y eritromicina, se realizó en 24 cepas de B. hominis, a los cuales se los cultivó (método del microcultivo), en 10 concentraciones dobles de cada antimicrobiano (desde 256 ug/ mL hasta 0,5 ug/mL), además de un control. Resultados. El 46,3% (145/313) de la muestra tuvo B. hominis, además se observó que, la edad entre 12 a 17 años y más de 60 años estuvo asociado con la mayor frecuencia del parásito (OR: 2,93 y 2,62 respectivamente). La concentración inhibitoria mínima (CIM) 90 del metronidazol y nitazoxanida fue de 3,19 ug/mL y 11,19 ug/mL respectivamente, mientras que el CIM-90 del trimetoprim-sulfametoxazol y eritromicina fueron superiores a 256 ug/mL. Conclusiones. B. hominis se presenta en alta frecuencia en pacientes admitidos en el Hospital Regional de Lambayeque, mostrando ser importante y un problema de salud pública en la región. Asimismo, los B. hominis aislados de estos pacientes mostraron ser susceptibles in vitro a bajas concentraciones de metronidazol y nitazoxanida por lo que podrían ser de elección para el tratamiento de este parásito.

Indroduction: Leptospirosis is a frequent cause of febrile illness around the world, particularly in tropical and subtropical areas such as Perú. Objective: To estimate the frequency and socio-demographic characteristics associated with leptospirosis in febrile patients in northern Perú. Materials and Methods: A cross sectional study was conducted from January to December 2014 in 143 fever cases admitted to the Hospital Regional Lambayeque, Perú. Structured for socio-demographic information and sanitation questionnaire was used. Serum samples were examined to quantitatively determine the presence of IgM antibodies to leptospira using ELISA IgM. Results: The frequency was 26.6% (95% CI 19.3 to 33.8) and independently associated features were: be between 20-54 years (OR: 3.36, 95% CI 1.17 to 9, 64), work in agriculture-livestock (OR: 6.15, 95% CI 1.99 to 19.05),contact with animals (OR: 4,13, IC 95%: 1,76-9,65) and consumption of unsafe water(OR: 3,33, IC 95%: 1,49-7,43). Conclusions: This study reveals a high frequency of leptospirosis among febrile patients from northern Perú. These results indicate the need to investigate routinely leptospirosis in febrile patients, especially those exhibiting risk characteristics.

Introducción: La leptopsirosis es causa frecuente de enfermedad febril alrededor del mundo, en particular en áreas tropicales y subtropicales como Perú. Objetivo. Estimar la frecuencia de leptospirosis y características socio-demográficas asociadas en pacientes febriles del norte del Perú. Material y Métodos: Se realizó un estudio transversal entre enero y diciembre de 2014 en 143 casos febriles admitidos en el Hospital Regional Lambayeque, Perú. Se usó un cuestionario estructurado para obtener información socio-demográfica y de saneamiento. Se examinaron muestras de suero para determinar cuantitativamente la presencia de anticuerpos IgM contra leptospira usando la técnica de ELISA IgM. Resultados: La frecuencia fue de 26,6% (IC 95%: 19,3-33,8) y las características asociadas independientemente fueron: tener entre 20 y 54 años (OR: 3,36, IC 95%: 1,17-9,64), dedicarse a la agricultura/ganadería (OR: 6,15, IC 95%: 1,99-19,05), el contacto con animales (OR: 4,13, IC 95%: 1,76-9,65) y el consumo de agua no potable (OR: 3,33, IC 95%: 1,49-7,43). Conclusiones: Este estudio revela una alta frecuencia de leptospirosis entre los pacientes febriles procedentes del norte del Perú. Estos resultados indican la necesidad de investigar la leptospirosis rutinariamente como causa de enfermedad febril, especialmente entre aquellos pacientes que presentan características de riesgo.
