O objetivo deste estudo foi identificar defeitos genéticos em paciente com hipotireoidismo congênito (HC) por disormonogênese e defeito total de incorporação de iodeto (DIIT). Neonato do sexo masculino com HC diagnosticado pelo rastreamento neonatal. Exames clínicos e radiológicos confirmaram que o paciente apresentava HC severo e permanente com DIIT (teste de perclorato: 99%). A região codificadora dos genes TPO, DUOX2, DUOXA2 e 2957 pares de bases (pb) do promotor de TPO foram sequenciados. No paciente foi identificada a duplicação em heterozigose GGCC no éxon 8 do gene TPO (c.1186_1187insGGCC). Nenhuma outra mutação foi localizada nos genes TPO, incluindo o promotor, DUOX2 ou DUOXA2. Descrevemos paciente com grave defeito de organificação de iodeto, provocando HC severo com bócio, em consequência de uma única mutação monoalélica no gene TPO. A expressão monoalélica no tecido tireoideano explicaria a associação de uma doença autossômica recessiva com uma única mutação monoalélica.

The aim of this study was to identify the genetic defect of a patient with dyshormonogenetic congenital hypothyroidisms (CH) with total iodide organification defect (TIOD). A male child diagnosed with CH during neonatal screening. Laboratory tests confirmed the permanent and severe CH with TIOD (99% perchlorate release). The coding sequence of TPO, DUOX2, and DUOXA2 genes and 2957 base pairs (bp) of the TPO promoter were sequenced. Molecular analysis of patient's DNA identified the heterozygous duplication GGCC (c.1186_1187insGGCC) in exon 8 of the TPO gene. No additional mutation was detected either in the TPO gene, TPO promoter, DUOX2 or DUOXA2 genes. We have described a patient with a clear TIOD causing severe goitrous CH due to a monoallelic TPO mutation. A plausible explanation for the association between an autosomal recessive disorder with a single TPO-mutated allele is the presence of monoallelic TPO expression.

OBJECTIVES: To evaluate the prevalence of thyroid dysfunction in elderly cardiac patients in an outpatient setting. SUBJECTS AND METHODS: A total of 399 consecutive patients (268 women, age range 60-92 years) who were followed at Heart Institute were evaluated for thyroid dysfunction with serum free T4, TSH, anti-Peroxidase antibodies, urinary iodine excretion measurements and thyroid ultrasound. RESULTS: Hyperthyroidism (overt and subclinical) was present in 29 patients (6.5%), whereas hypothyroidism (overt and subclinical) was found in 32 individuals (8.1%). Cysts were detected in 11 patients (2.8%), single nodules were detected in 102 (25.6%), and multinodular goiters were detected in 34 (8.5%). Hashimoto's thyroiditis was present in 16.8% patients, most of whom were women (83.6%). The serum TSH increased with age and was significantly higher (p= <0.01) in patients, compared to the normal control group. No significant differences in serum TSH and free T4 values were observed when patients with atrial fibrillation (AF) where compared with those without arrhythmia. The median urinary iodine levels were 210 µg/L (40-856 µg/L), and iodine levels were higher in men than in women (p<0.01). Excessive iodine intake (urinary iodine >300 µg/L) was observed in one-third of patients (30.8%). CONCLUSIONS: Elderly patients have a higher prevalence of both hypo- and hyperthyroidism as well as thyroid nodules when compared with the general population. About one-third of the older patients had elevated urinary secretion of iodine and a higher prevalence of chronic Hashimoto's thyroiditis. It is recommended that ultrasonographic studies, tests for thyroid function and autoimmunity should be evaluated in elderly patients.

OBJETIVO: Aprofundar a análise molecular da mutação intrônica IVS30+1G>T do gene tireoglobulina (TG) e relatar a clínica de pacientes portadores da mutação, acompanhados por 11 anos. MÉTODOS: Foram estudados dois irmãos com hipotireoidismo congênito grave com bócio fetal e bócio neonatal, portadores da mutação IVS30+1G>T. Foram coletadas amostras de tecido nodular e não-nodular. Avaliou-se a expressão de genes específicos da tireóide por PCR em tempo real e imunohistoquímica. RESULTADOS: A expressão de genes específicos da tireóide foi menor no tecido não-nodular que no tecido normal controle. Expressões de TPO e NIS foram extremamente baixas no tecido nodular. Verificou-se lúmen folicular aumentado com grandes vesículas de retículo endoplasmático, e detectou-se forte marcação de TG no citoplasma e fraca no lúmen folicular. No tecido não-nodular observou-se forte positividade de NIS intracelular e, TPO e TSHR na membrana plasmática. O acompanhamento em longo prazo dos pacientes mostrou adequado desenvolvimento, apesar de um deles ter recebido tratamento tardio. CONCLUSÕES: A mutação IVS30+1G>T não só promove alterações no retículo endoplasmático, como alterações na expressão de genes específicos da tireóide. A evolução clínica destes pacientes reforça o conceito da influência do meio ambiente, como o aporte nutricional de iodo, no fenótipo final.

OBJECTIVE: To extend the molecular analysis of the IVS30+1G>T intronic thyroglobulin (TG) mutation, and to report the eleven year follow-up of the affected patients. METHOSD: Two siblings with severe congenital hypothyroidism with fetal and neonatal goiter, harboring the IVS30+1G>T mutation were included. Nodular and non-nodular thyroid tissue specimens were collected. Specific thyroid genes expression was evaluated by real-timePCR and by immunohistochemistry. RESULTS: In non-nodular tissue specific thyroid genes mRNA were reduced when compared to normal thyroid sample. In the nodule, TPO and NIS expression was very low. Microscopic examinations showed very large follicular-lumina and swollen vesicles of endoplasmatic-reticulum. Strong cytoplasmatic and low follicular-lumen TG immunostaining were detected. Intracellular NIS, membrane TPO and TSHR immunostaining had higher positivity in non-nodular sample. Both patients had a long-term adequate developmental outcome, besides one patient have been lately-treated. CONCLUSIONS: IVS30+1G>T mutation not only lead to very enlarge endoplasmatic-reticulum, but also to alterations of specific thyroid genes expression. The clinical evolution of patients harboring these mutations strengthen the concept of the influence of environment, like iodine nutrition, to determine the final phenotypic appearance.

O iodo é essencial para a síntese de hormônios tireóideos e tanto a deficiência crônica deste halogeno como o excesso nutricional de iodo levam a hiperplasia e hipertrofia dos elementos foliculares (por excesso de TSH). Esse fenômeno pode se associar a maior risco de câncer de tireóide, especialmente no sexo feminino. Estudos experimentais documentam indução de câncer de tireóide após prolongado excesso circulante de TSH, o qual induz aumento da proliferação celular medida por fator de crescimento epidermal (EGF), decréscimo de síntese de fator de transformação do crescimento (TGFbeta 1) e aumento da angiogenese. Estudos epidemiológicos entre nutrição de iodo e câncer de tireóide são conflitantes. É, todavia, aceito que a correção de prévia deficiência de iodo com aporte nutricional adequado deste halogeno leva à maior prevalência de carcinoma papilífero (e decréscimo de carcinoma folicular). Em alguns países, o excesso de iodo foi apontado como causa aparente de maior prevalência de câncer de tireóide. Em conclusão: não existe uma relação causa-efeito entre iodo nutricional e prevalência de câncer de tireóide, e outros fatores intervenientes ambientais devem ser considerados.

Iodine is a trace element that is essential for the synthesis of thyroid hormone. Both chronic iodine deficiency and iodine excess have been associated with hypertrophy and hyperplasia of follicular cells, attributed to excessive secretion of TSH. This may be associated to thyroid cancer risk, particularly in women. Experimental studies have documented thyroid cancer induction by elevation of endogenous TSH, although in a small number of animals. Iodine deficiency associated with carcinogenic agents and chemical mutagens will result in a higher incidence of thyroid malignancy. Inadequate low iodine intake will result in increased TSH stimulation, increased thyroid cell responsiveness to TSH, increased thyroid cell EGF-induced proliferation, decreased TGFbeta 1 production and increased angiogenesis, all phenomena related to promotion of tumor growth. Epidemiological studies associating iodine intake and thyroid cancer led to controversial and conflicting results. There is no doubt that introduction of universal iodine prophylaxis in population previously in chronic iodine-deficiency leads to a changing pattern of more prevalent papillary thyroid cancer and declining of follicular thyroid cancer. Also anaplastic thyroid cancer is practically not seen after years of iodine supplementation. Iodine excess has also been indicated as a possible nutritional factor in the prevalence of differentiated thyroid cancer in Iceland, Hawaii and, more recently, in China. In conclusion: available evidence from animal experiments, epidemiological studies and iodine prophylaxis has demonstrated a shift towards a rise in papillary carcinoma, but no clear relationship between overall thyroid cancer incidence and iodine intake.

OBJETIVO: Avaliar a possibilidade de diagnóstico pré-operativo de nódulos da tireóide (de diâmetro superior a 10mm) usando ultra-sonografia da glândula tireóide e citologia de punção aspirativa por agulha fina guiada pela ultra-sonografia. CASUÍSTICA E MÉTODOS: Nódulos tireóideos (maiores que 10mm) foram classificados ultra-sonograficamente em graus de I a IV e escores numéricos de 1 a 4, de acordo com crescente possibilidade de malignidade. O exame citológico, subseqüentemente, classificou os nódulos como benigno (escore 1) indeterminado (escore 2) suspeito (escore 3) e maligno (escore 6). Somando-se os escores obtidos nas duas metodologias obtém-se um índice considerado benigno (índice combinado 2-4), duvidoso (índice combinado 5) suspeito para malignidade (índice combinado 6) e elevada probabilidade de malignidade (índice combinado 7 a 10). Cirurgia da Tireóide foi realizada em 274 pacientes, dos quais 64 apresentavam índice de 2-4; destes, apenas 2 pacientes (3,1%) apresentaram comprovação histológica de câncer. Em pacientes com índice 5 (n= 51), 11,8% apresentaram câncer de tireóide e, em 61 pacientes com índice 6, (n= 31), 51% tiveram diagnóstico confirmado de malignidade. O índice combinado de 7-10 (n= 98) apresentou 99% de pacientes com câncer de tireóide. CONCLUSÕES: O índice combinado apresentou sensibilidade de 94,1% e especificidade de 77,5%. A precisão desta metodologia foi de 85,8%. Concluímos que o índice combinado pode ser útil no diagnóstico pré-cirúrgico do nódulo tireóideo, mormente se associado com outras metodologias como a ecografia com Doppler colorido, nível elevado de TSH sérico, análise de expressão de galectina-3 e imagens por FDG/PET.

PURPOSE:To evaluate the preoperative assessment of thyroid nodules using ultrasound studies and cytology of nodular aspirates. SUBJECTS AND METHODS: 2,468 patients with thyroid nodules were examined from 1999 to 2005. All patients were clinically examined and underwent ultrasonography followed by fine-needle aspiration biopsy (FNAB) and cytology. RESULTS:Nodules larger than 10 mm were classified ultrasonographically in a 4-tier system and received a score according to the criterion of possible malignancy. Cytological examinations were conducted independently by 2 cytologists and classified as benign (score 1), indeterminate (score 2), suspicious (score 3), and malignant (score 6). Combining both scores, an index was generated that would indicate a higher probability of malignancy (benign, doubtful, suspicious, and malignant). Thyroid surgery was performed in 274 patients. Of those, 115 patients had a score of 2 to 5 and only 8 had a histological diagnosis of thyroid cancer (6.9%). For patients with a score of 5 (n = 51), 11.5% had a malignant lesion, and 51% of the 61 patients with a score of 6 had confirmed thyroid cancer. Of the 98 patients with a combined score of 7 to 10, 99% had a histological confirmation of malignancy. CONCLUSIONS: The index score had a sensitivity of 94.1% and specificity of 77.5%. The overall accuracy was 85.8%. Therefore, we concluded that this methodology may improve the preoperative diagnosis of thyroid cancer in nodules larger than 10 mm. Association with other methods such as color Doppler echography, serum TSH concentration, galectin-3 expression analysis, and FDG/PET scan would be useful in avoiding the higher costs of thyroid surgical procedures.

OBJETIVO: Analisar a prevalência populacional de tireoidite crônica autoimune (tireoidite de Hashimoto) na área vicinal ao Polo Petroquímico de Capuava comparativamente a área controle em São Bernardo Campo. CASUÍSTICA E MÉTODOS: Em ambas as áreas urbanas foram incluídos, aleatoriamente, indivíduos adultos que, de forma voluntária, concordaram em participar do estudo, estratificado por idade (20 a > 70 anos) e sexo (mulheres 80%, homens 20%). Na área Polo foram incluídos 409 indivíduos e na área controle (São Bernardo Campo) 420 pessoas (sem diferenças significativas quanto a idade e sexo). Na área Polo 15,6% apresentava sinais ecográficos e de positividade para anticorpos anti TPO, confirmando tireoidite crônica autoimune (TCA) comparativamente a 19,5% na área controle (p > 0,05, NS). A presença de hipotiroidismo foi confirmada em 4,9% da população na área Polo e 8,3% na área controle (São Bernardo Campo) (p = 0,046, significativo). No conjunto 6,63% dos pacientes com TCA apresentavam hipofunção tireóidea. A excreção urinária de iodo ultrapassou 300 mcg Iodo/L de urina em 58,5% de ambas populações. O sal coletado nas casas dos examinados apresentava concentração normal de iodo (35,5 + 6,6 mg I/Kg de sal). CONCLUSÕES: A maior prevalência de tireoidite crônica autoimune em ambas as áreas é, provavelmente, decorrente da elevada ingestão nutricional de iodo durante o quinqüênio que precedeu esta pesquisa (1998-2004). A suposta conexão epidemiológica de maior prevalência epidemiológica de TCA com vicinidade com o Polo Petroquímico de Capuava é improvável.

PURPOSE: To evaluate the prevalence of chronic autoimmune thyroiditis in 2 urban areas of metropolitan São Paulo (Brazil): Polo Area neighboring a large petrochemical complex and São Bernardo Campo Area (control area). SUBJECTS AND METHODS: Subjects were randomly included from the adult population (20 to 70 years of age) of both genders (women 80%, men 20%) who voluntarily agreed to participate. From the Polo Area, in the vicinity of a large petrochemical industrial complex, 409 subjects were included; from the control area (São Bernardo Campo Area) 420 individuals were included. All subjects were clinically examined, and a detailed record of past thyroid illness and medications was obtained. Ultrasonographic studies were performed using a portable GE Medical Systems apparatus. Blood samples were obtained for free T4, serum TSH, and serum anti-thyroid peroxidase autoantibodies. Urine specimens were collected in Monovette syringes for assaying iodine content. Salt samples were collected at households, and the iodine content was measured. RESULTS: Chronic autoimmune thyroiditis was diagnosed both echographically (marked hypoechogenicity) and immunologically (presence of autoantibodies against thyroid peroxidase). In the Polo Area, 15.6% of the examined population had chronic autoimmune thyroiditis, and in the control area (São Bernardo Campo Area), 19.5% of the population had evidence of chronic autoimmune thyroiditis (P > 0.057, not significant). The prevalence of hypothyroidism was 4.9% in the Polo Area and 8.3% in the São Bernardo Campo Area (P = 0.0461 significant). Taking the 2 populations together, 6.6% had hypothyroidism (about one third of these patients were on L-T4 treatment). The mean thyroid volume was 11.2 mL. Domestic salt had a normal concentration of iodine (35.5 + 6.61 mg/kg). Urinary excretion of iodine was above 300 µg Iodine/L in 58.5% of the total population. CONCLUSION: The high iodine intake (above 300 µg Iodine/L of urine) that was present from 1998 through 2005 may be related to a higher prevalence of chronic autoimmune thyroiditis in both areas that were studied. There was no apparent or documented relationship of chronic autoimmune thyroiditis prevalence to the proximity to the petrochemical complex.

O objetivo do presente estudo foi avaliar esquema terapêutico medicamentoso para aumentar a aderência ao tratamento da moléstia de Graves-Basedow (MGB) em Hospital Público Universitário. Os pacientes foram selecionados segundo critérios rigorosos, que incluíam volume glandular inferior a 60cm³, origem da área urbana de São Paulo e não submetidos a terapia prévia da MGB. Receberam gratuitamente a medicação, eram avisados antecipadamente da data da consulta e acompanhados por um único médico durante todo o tratamento. Foram incluídos 229 pacientes, tratados inicialmente com metimazol (MMI - 60mg/dia) em dose única diária, seguindo-se com combinação de MMI (20mg) com LT4 (100µg) em dose única diária por 24 meses. Apenas 83 pacientes (36,2%) completaram o protocolo quando foram subdivididos em 2 grupos, após a suspensão do MMI+LT4: Grupo 1 (n= 34), que permaneceram em remissão por 3 anos, e Grupo 2 (n= 49), que apresentaram recidiva da doença entre 2 e 36 meses. Os fatores preditivos associados à remissão foram: decréscimo do volume glandular, tireoglobulina sérica < 40ng/ml e valores séricos normais de anticorpos anti-receptor de TSH. Constatamos que apesar do planejamento cuidadoso, mais de 60% dos portadores de MGB não completaram o protocolo e foram encaminhados a tratamento com radioiodo. Admitimos que esse baixo êxito terapêutico poderia ser melhorado mediante identificação dos fatores capazes de indicar quais pacientes estariam propensos a seguir um tratamento de longa duração.

The aim of the present study was to evaluate a new proposal for increasing compliance to the clinical management of patients with Graves’ disease (GD) in a large and public University Hospital. The patients were carefully selected (no previous GD treatment, goiter volume less than 6mL must be living in the metro area of São Paulo), received medication at no cost, were contacted frequently by the social worker and alerted for the date of consultation and only referred to a single endocrinologist during all phases of treatment. We recruited 229 patients with GD that were initially treated with methimazole (MMI - 60mg q.d) in a single daily dose followed by a combination of MMI (20mg) plus L-T4 (100µg) daily for 24 months. Only 83 patients (36.2%) completed the protocol and were subdivided in: Group 1 (n= 34) that were in remission for 3 years after discontinuation of the MMI and Group 2 (n= 49) that presented recurrence of GD between 2 and 36 months without MMI. Predictive factors associated with remission were: decrease of the glandular volume, serum TG< 40ng/mL and normal TRAb values. We concluded that in spite of a careful protocol planned to increase compliance, more than 60% of patients with GD did not complete the therapeutic trial and were referred for radioiodine treatment. The solution for this low therapeutic success for GD should be the possible identification of factors that would indicate patients that are not inclined to follow a long period of clinical therapy.

Fundamentalmente, a glândula tiróide, face à carência de iodo, se adapta por incremento da captação deste halogênio, aumenta a sua massa glandular (bócio), induz secreção preferencial de T3 e eleva a síntese e liberação de TSH. Os exames laboratoriais e de imagem permitem a comprovação dos mecanismos adaptativos. A iodúria permite avaliação quantitativa do grau de carência iódica. Em algumas populações, coexiste a influência de bocígenos naturais presentes nos alimentos consumidos localmente. O cretinismo endêmico é síndrome neurológica e endócrina que conduz o indivíduo a quadro severo de rebaixamento mental e surdo-mudez. No Brasil, as moléstias associadas à carência de iodo foram sempre avaliadas por inquéritos nacionais em escolares. A correção da deficiência de iodo se faz pela adição de iodato de potássio ao sal de consumo humano. Várias leis e decretos foram elaborados tentando aperfeiçoar o sistema universal de iodação do sal, que, mais recentemente, foi aprimorado e tornou-se mais eficiente em propiciar iodo a toda população brasileira.

The thyroid gland promotes its adaptation to iodine deficiency inducing an increase in the iodine uptake followed by a substantial increase in the thyroid gland mass (goiter). Simultaneously, there is a preferential T3 secretion by the follicular cell and a persistently elevated serum TSH. Laboratory tests, isotopic methods and imaging are routinely used to verify the altered thyroid pathophysiology. In certain populations the presence of goitrogenic natural substances, present in the locally consumed staple food, were found and may add to the pathogenic process. Endemic cretinism is usually present when the iodine deficiency is quite low and may present with the neurologic or myxedematous sub-types, frequently associated with deafmutism. In Brazil, the prevalence of iodine-deficiency disorders has been evaluated, periodically, with schoolchildren examined for goiter. Universal salt iodination was implemented in Brazil in the early fifties but recently the system was considered effective for the supply of iodine to the Brazilian population.

A bioequivalência e biodisponibilidade das preparações comerciais de L-tiroxina podem apresentar diferenças significativas entre si. O objeto do presente trabalho foi comparar variáveis séricas tireóideas (T4 e T3 totais, T4 livre e TSH endógeno) em pacientes previamente tireoidectomizados (há mais de 5 anos) e sem evidência clínica ou ecográfica de tecido glandular cervical remanescente. Os referidos parâmetros foram medidos, por 15 dias, a intervalos de tempo regulares, durante administração de solução oral de L-tiroxina (100µg), presumivelmente de absorção e biodisponibilidade ideais. Comparou-se, em seguida, a oscilação das mesmas variáveis com duas outras preparações comerciais, sob forma de comprimidos de L-tiroxina (LT4A e LT4B) nas doses de 100µg/dia e a seguir 200µg/dia. Não se observou diferença significativa entre a solução oral e as duas preparações comerciais nos vários tempos de medida. Tampouco houve diferença significativa entre os comprimidos de L-T4A e L-T4B em doses de 100µg ou de 200µg/dia. Concluímos que ambas as preparações apresentam biodisponibilidade e bioequivalência adequadas nas condições utilizadas neste estudo.

A major issue influencing prescribing patterns of physicians has been the relative bioavailability and bioequivalence of commercial levothyroxine preparations. Some studies have suggested similar bioequivalence between various L-T4 tablets while other reports have found differences. Some of these studies have been criticized for methodological defects in patients' selection or study design, which have become appreciated only in retrospect. Moreover the US Pharmacopeia has proposed that commercial tablets of L-T4 should be tested for its dissolution in two different solutions. Each manufacturer should periodically test batches of tablets as indicated. In the present study, we have compared serum thyroid function parameters (total T4, total T3, free T4, and serum TSH) in previously totally thyroidectomized patients, without sonographic evidence for reminiscent thyroid tissue in the cervical area. The above parameters were measured, as function of time, for 15 days of either an oral solution of L-T4 (100µg) or tablets of L-T4A and L-T4B (100µg/tablet). There was no significant difference between the various parameters, in relation to time, for each of the different preparations. Moreover L-T4A tablets did not differ from L-T4B tablets, when those parameters were tested, either at the concentration of 100 or 200µg/day. We conclude that the two oral formulations of L-thyroxine have similar bioavailability and bioequivalence under the present experimental conditions.

O hipotireoidismo congênito (HC), detectado em recém-nascidos rastreados ao nascer, é causado por anomalias na organogênese (agenesia, tireóide ectópica, hipoplasia tireóidea) ou por defeitos específicos na hormonogênese. Mais raramente, o hipotireoidismo congênito tem origem em defeitos genéticos centrais, localizados no eixo hipotálamo-hipófise ou em mutações do gene do TSH-beta ou no receptor de TSH. Defeitos específicos da hormonogênese são causados por mutações no gene codificador para a proteína transportadora de iodeto (NIS), no gene da peroxidase (TPO), no gene traduzindo a pendrina (síndrome de Pendred), no gene da tireoglobulina, além de mutações que afetam o receptor de hormônio tireóideo (resistência genética ao HT) ou aos vários defeitos no transporte de HT na circulação periférica. Na maioria dos casos, os defeitos genéticos indicados criam condições para fenótipo com bócio e grau variável de hipotireoidismo, podendo a mutação genética ter expressão variável durante a vida adulta.

Congenital hypothyroidism (CH), as seen in the neonatal period, is predominantly caused by defects in the organogenesis (athyreosis, ectopic thyroid, thyroid hypoplasia) or by specific defects in hormonogenesis (dyshormonogenesis). Central hypothyroidism is rare, being linked to specific transcription factors involved in the maturation of the hypothalamic-pituitary axis or by mutations in the TSH-beta gene. Dyshormonogenesis may be caused by mutation coding for thyroid proteins such as the TSH receptor, the sodium-iodide symporter (NIS), the pendrin (Pendred's syndrome), thyroid peroxidase (TPO), thyroglobulin (Tg), thyroid dehalogenase, the receptor for thyroid hormone (TR-beta) or thyroid transport proteins (TBG, transthyretin). Usually, most of these defective genes will induce a phenotype with goiter and variable degree of hypothyroidism that may be present in the neonatal period or will gradually develop during post-natal life. Moreover, the genetic expression may be partial or total according to the specific mutation occurring in the involved genes.

Auto-anticorpos anti-receptores de TSH (TSHRAbs) foram avaliados por dois diferentes métodos [TRAb, como % de inibição de ligação do TSH por anticorpos séricos e produção de AMP cíclico em cultura de células CHO expressando o receptor de TSH humano recombinante (CHO-rhTSHR)] em 52 pacientes (36F/16M) com moléstia de Graves-Basedow (DGB), tanto antes do tratamento como aos 6 e 12 meses de terapia contínua com metimazol (40-60mg/dia) e L-tiroxina (100mig/dia); outros 20 pacientes tireotóxicos (12F/8M) foram tratados com doses individualizadas de radioiodo. Os TSHRAbs determinados pelo radioreceptorensaio foram positivos em 47/52 pacientes (90,4%) com valor médio±EPM de 56,7±3,9%, diminuindo significantemente aos 6 (40,5±3,2%) e 12 meses (43,5±4,7%) de terapêutica, bem como após radioiodo (30,7±4,5%). Os TSHRAbs foram discriminados pelo bioensaio em todos os 52 pacientes com DGB ativa (1122±409%). Após 6 e 12 meses de terapêutica houve decréscimo (não significante) dos valores iniciais. Nenhum indivíduo do grupo controle normal (n= 80) apresentou TSHRAbs detectado por qualquer dos métodos. Portanto, a sensibilidade da pesquisa dos anticorpos pelo bioensaio, nos 52 pacientes com DGB ativa, foi maior que nos mesmos indivíduos avaliados pelo radioreceptorensaio. Houve correlação positiva (r= 0,59; p<0,001) entre TRAb e CHO-rhTSHR. Concluímos que a pesquisa dos TSHRAbs, realizada quer pelo radioreceptorensaio como pelo bioensaio, constitui recurso útil para se avaliar a atividade autoimune na DGB.

Antibodies directed to the TSH receptor (TSHRAbs), detected by TRAb as TSH-binding inhibitory immunoglobulin, and cyclic AMP production by CHO cells expressing human TSH recombinant receptor (CHO-rhTSHR) were determined in 52 patients (36F/16M) with Graves' disease (DGB), both before and at 6 and 12 months of continuous treatment with metimazol (40-60mg/day) associated with L-thyroxine (100mug/day). Another group of 20 patients (12F/8M) was referred for radioiodine (RAI) treatment. The TSHRAbs were detected by radioreceptor assay in none of the 80 normal control individuals and in 47/52 patients (90.4%) with a mean±SEM value of 56.7±3.9%. There was a significant decrease of mean TRAb values at 6 and 12 months of MMI + LT4 therapy. Six months after RAI, TRAb decreased significantly to 30.7±4.5%. In contrast, TSHRAbs were determined by bioassay in none of the 80 normal control subjects but in all 52 patients with a mean±SEM of 1122±409%. Similarly to TRAb there was a non-significant decrease at 6 and 12 months of MMI + LT4 therapy. Thus, the sensitivity of TSHRAbs detected by bioassay in 52 patients with active DGB seems to be greater than that detected by radioreceptor. In conclusion, the radioreceptor assay values as well as the bioassay for TSHRAbs may be useful for the diagnosis of active DGB.

Avaliamos prospectivamente a função, as imagens ultra-sonográficas e a auto-imunidade tireóidea em grupo racial heterogêneo de 800 mulheres grávidas, primíparas ou multíparas (1 a 7 gestações prévias) atendidas em hospital universitário de São Paulo durante o pré-natal. Quarenta e seis pacientes foram excluídas do estudo por apresentarem diagnóstico confirmado de doença tireóidea anterior; das 754 restantes, 386 abandonaram o seguimento antes ou após o parto. Assim, a coorte avaliada no puerpério constituiu-se de 368 puérperas, examinadas aos 3, 6, 12 meses e no 2º ano pós-parto. Embora 29 pacientes tenham tido aumento transitório daqueles anticorpos antitireóideos, caracterizando reação de auto-imunidade, estes progressivamente declinaram ou tornaram-se negativos durante o puerpério. Entretanto, nenhuma destas evoluiu com disfunção tireóidea durante o acompanhamento. Quarenta e nove pacientes (13,3%) desenvolveram progressivamente alterações funcionais da tireóide (principalmente hipotireoidismo), provavelmente devido à TPP. Além disso, a continuação do seguimento mostrou que em 18 a 24 meses, 42 mostraram níveis séricos de anti-TPO mais elevados, em comparação aos valores do primeiro ano. Os fatores de risco para o desenvolvimento da TPP, presentes durante a gravidez, foram: (1) níveis relativamente baixos de anti-TPO, entre 60 e 100U/mL [risco relativo ou odds ratio de 3,1] e (2) alterações estruturais ultra-sonográficas da tireóide no primeiro trimestre [odds ratio de 6,4]. Concluímos que a prevalência de TPP em hospital público de São Paulo foi de 13,3%, considerando as 368 pacientes seguidas. Em 29 puérperas foi diagnosticada forma transitória de reação tireóidea auto-imune, caracterizada por elevação dos níveis séricos de anti-TPO, que posteriormente regrediram.

We evaluated thyroid function, ultrasound images and presence of autoantibodies in a mixed racial group of 800 unselected primiparous or multiparous (1-7 previous pregnancies) pregnant women attended in a public hospital in the city of São Paulo. Forty-six patients were excluded from the study because of a confirmed diagnosis of a thyroid condition. Thus, a total of 754 women were available for further studies in the postpartum period. A relatively large number of those patients (n= 386) were lost or unavailable to follow-up either before or after delivery. A cohort of 386 puerperal healthy women was followed-up regularly at 3, 6, 12 and 24 months during the postpartum period, with regular thyroid function tests, random urine iodine measurements, assays for serum autoantibodies against thyroid antigens and imaging by ultrasound. Twenty-nine patients had a transient rise of anti-TPO-Ab characterizing an autoimmune reaction. These antibodies titres progressively declined or became negative. Moreover none of these patients have evidence for altered thyroid function during the 18-24 months of follow-up. Forty-nine patients (13.3%) changed their thyroid status, mainly to hypothyroidism, indicating the presence of a postpartum thyroid dysfunction (PPTD). Further follow-up studies have evidenced that at 18-24 months of examination 42/49 had serum levels of anti-TPO-Ab that were more elevated as compared with the first year values. Predictive factors found during pregnancy for developing PPTD, were: (1) relatively low levels of anti-TPO-Ab (between 60 and 100U/mL) [odds ratio 3:1] and (2) ultrasonographic thyroid structural changes in the first trimester [odds ratio 3.5:1]. In conclusion, the prevalence of PPTD was 13.3% considering only those postpartum women actually followed-up (n= 368). A transient form of thyroid autoimmune reaction, characterized by elevated serum titers of anti-TPO-Ab that further declined and reached the normal range, was observed in 29 puerperal women.

O hipotireoidismo congênito (HC) ocorre, mundialmente, em 1/3000-4000 neonatos e pode ser classificado em permanente ou transitório. O HC primário é responsável pela maioria dos afetados, enquanto o secundário e terciário são raros. Nos países iodo-suficientes, a disgenesia tireóidea (DT) é a causa mais freqüente de HC. Os defeitos hereditários da síntese hormonal ocorrem em minoria de crianças portadoras de HC. Fatores ambientais, genéticos e auto-imunes concorrem na etiologia do HC, mas na maioria dos casos de DT a causa é obscura. Atribui-se aos genes envolvidos na ontogenia da glândula tireóidea, como os fatores de transcrição TITF1, TITF2, PAX-8 e receptor de TSH (TSHR), função patogenética na DT. Até o momento não foi descrita anormalidade no gene TITF1 como causa de HC, enquanto foram identificadas mutações no PAX-8 em cinco recém-nascidos com DT. Embora não envolvidas na DT, mutações inativadoras do TSHR podem produzir espectro de defeitos congênitos oscilando entre hipertirotropinemia com eutireoidismo e hipotireoidismo com hipoplasia glandular. A clonagem dos genes envolvidos na biossíntese dos hormônios tireóideos, como o da tireoperoxidase (TPO) e tireoglobulina (Tg), permitiu a identificação de mutações responsáveis por alguns casos de bócio e hipotireoidismo decorrente de defeito de incorporação de iodeto ou anormalidades na síntese de Tg. Recentemente, foi demonstrada a base molecular do defeito de transporte ativo de iodeto e da síndrome de Pendred, respectivamente, devidas a mutações no gene NIS (simportador de sódio e iodeto) e no gene PDS (pendrina). Em conclusão, grande parte dos pacientes com HC e DT não tem esclarecida, ainda, a causa molecular desta síndrome.

Congenital hypothyroidism (CH) occurs in 1/3000-4000 neonates worldwide and may be classified as permanent or transient. Primary CH accounts for the majority of affected children, while secondary and tertiary CH are rare. In iodine-sufficient countries, thyroid dysgenesis (TD) is the most frequent cause of CH. Hereditary inborn errors of thyroid hormonogenesis account for about 10-20% CH children. Environmental, genetic and autoimmune factors have been implicated in the etiology of CH, but in the majority of cases the cause of TD remains to be clarified. Candidates for playing a pathogenetic role in TD are genes involved in thyroid gland ontogeny, such as those of thyroid transcription factors TITF1, TITF2, PAX-8 and the TSH-receptor (TSHR). No abnormality in the TITF1 gene has yet been found in CH, while mutations in the PAX-8 gene were identified in at least 5 neonates with TD. Loss of function mutations of the TSHR gene, although not involved in TD, may produce a spectrum of congenital defects ranging from euthyroid hyperthyrotropinemia to overt hypothyroidism with a hypoplastic gland. The cloning of genes implicated in the biosynthesis of thyroid hormones, such as those of thyroperoxidase (TPO) and thyroglobulin (Tg), has led to the identification of mutations responsible for some cases of goitrous hypothyroidism due to iodide organification defect or abnormalities in Tg synthesis. Recently, the molecular basis of the iodide transport defect and Pendred’s syndrome were reported, due to mutations, respectively, on the NIS and PDS genes. In conclusion, we still do not have elucidated the molecular basis of TD, the most common of the defects affecting neonates with CH.
