The background of the Maʿlot article is the method of Talmud instruction at the Volozyhn Yeshiva and Yeshivat Maʿalot and investigating the claim of the management of Yeshivat Maʿalot, the head of the yeshiva, and the teaching staff that the yeshiva is a direct continuation of the famed Volozhyn Yeshiva that operated in 19th-century Europe. This claim can be examined from many angles, but the aim of the current article is to focus on one major angle common to the entire yeshiva world over the generations in Israel and abroad, that is, the method of Talmud instruction. The research setting is based on historical sources for teaching Talmud in yeshivot, physical attendance during lessons, interviews with teachers and students, and criticism brought by them and by the authors. The research methods include describing, comparing and criticising the method of instruction at Yeshivat Maʿalot. The research results led to a conclusion regarding the question of whether the above claim is justified. The article fits the scope of the journal because it reveals the teaching methods at Yeshivat Maʿalot. CONTRIBUTION: The contribution of the article lies in its being the first to offer a critique of the Talmud instruction method employed at Yeshivat Maʿalot, with the aim of rethinking the current instruction method and creating a possible avenue for changing it and adapting it as much as possible to the students' needs and abilities.

RESUMO O objetivo do presente estudo foi avaliar o grau de conversão, citotoxicidade, solubilidade e pH de cimentos à base de cálcio fotopolimerizáveis submetidos a pré-aquecimento. O grau de conversão foi analisado por espectroscopia no infravermelho com transformada de Fourier, a citotoxicidade pelo teste de MTT e a solubilidade através da perda de massa. Os dados foram submetidos a testes estatísticos (ANOVA/Tukey, p<0,05). Os materiais fotopolimerizáveis apresentaram baixo grau de conversão, independente do pré-aquecimento. Todos os materiais causaram redução da viabilidade celular nas análises de 24 horas e 7 dias, sendo que o Dycal (controle) apresentouse mais citotóxico e o calor apresentou efeito positivo sobre o Biocal na análise de 7 dias. O Dycal é o material mais solúvel e o calor não causou efeito na solubilidade e pH dos materiais polimerizáveis. Assim, conclui-se que os cimentos à base de cálcio fotopolimerizáveis apresentam baixo grau de conversão e são solúveis, que resulta em citotoxicidade suave e moderada.

ABSTRACT The aim of this study was to evaluate the degree of conversion, cytotoxicity, solubility and pH of photopolymerizable calciumbased cements submitted to preheating. The degree of conversion was analyzed by Fourier transform infrared, cytotoxicity by the MTT test and solubility through loss of mass. The data were subjected to statistical tests (ANOVA / Tukey’s, p<0.05). The photopolymerizable materials showed a low degree of conversion, regardless of preheating. All materials caused a reduction in cell viability at 24 hours and 7 days, with the Dycal (control) being more cytotoxic. Heat had a positive effect on Biocal at 7 days. Dycal is the most soluble material. Heat had no effect on the solubility or pH of the polymerizable materials. It is concluded that photopolymerizable calcium-based cements have a low degree of conversion and are soluble, which results in mild to moderate cytotoxicity.

Introduction and objectives: There are no data on outcome of patients with pulmonary hypertension (PH) in Argentina. The objective of this registry was to assess the characteristics of patients with PH belonging to the Group I Dana Point's 2008 classification, pulmonary arterial hypertension (PAH), and to identify variables associated with outcomes. Methods: One hundred and thirty four patients were analyzed in a consecutive sequence. After excluding nine patients, 125 patients were analyzed. Results: The patients mean age was 34 years (SD±15,7) and the female/male ratio was 3.8/1. The most prevalent aetiologies were: idiopathic PAH (IHAP) (61, 48.8%), PH associated to congenital heart disease (35, 28%) and PH associated to connective tissue disease (18, 14.4%). Overall survival, excluding transplantation, was 63.2%; at 12, 24 and 36 months survival was 94% (CI 95% 88-97), 90% (CI 95% 83-94) and 83% (CI 75-89), respectively. Baseline variables associated to increased mortality or need for transplantation were functional class III/IV (NYHA) (OR 3.6 CI 95% 1.5-8.9, p <0.01), 6 minute-walk test distance shorter than 380 m (OR 2.7 CI 95% 1.1-6.5 p = 0.023) and pericardial effusion (OR 4.2 CI 95% 1.3-14.6 p = 0.021). Conclusion: This series shows characteristics and survival of patients with pulmonary arterial hypertension at a reference centre in Argentina. Some readily available variables allowed us to stratify subgroups at a higher risk of death and/or transplantation during the follow up.

Introducción y objetivos: En Argentina, no se conocen datos de pacientes con hipertensión pulmonar (HP) con diagnóstico de certeza. El propósito del presente registro fue conocer las características de nuestra población de pacientes con HP del Grupo I de la clasificación de Dana Point 2008, hipertensión arterial pulmonar (HAP), estudiar la supervivencia y las variables asociadas a mayor mortalidad. Métodos: Estudio de cohorte de 134 pacientes admitidos por primera vez en el consultorio de HP de nuestra institución en el período comprendido entre enero/2004 y marzo/2012. Se excluyeron 9 pacientes; total analizado: 125 pacientes. Resultados: Edad promedio: 34 ± 15,7 años y la relación mujer/hombre: 3,8/1. Las etiologías más prevalentes fueron la HAP idiopática (HAPI) (61, 48.8%), asociadas a cardiopatías congénitas (35, 28%) y a colagenopatías (18, 14.4%). La supervivencia libre de trasplante o muerte fue de 63.2%; a 12, 24 y 36 meses la supervivencia fue del 94% (IC95% 88-97), 90% (IC95% 83-94) y 83% (IC95% 75-89), respectivamente. Las variables basales asociadas a mayor mortalidad y/o trasplante fueron la disnea clase funcional III/IV (OR 3.6 IC95% 1.5-8.9, p < 0.01), la distancia recorrida en la prueba de la caminata de 6 minutos menor de 380 m (OR 2.7 IC95% 1.1-6.5 p = 0.023) y el derrame pericárdico (OR 4.2 IC95% 1.3-14.6 p = 0.021). Conclusiones: Esta serie muestra las características y supervivencia actuales de pacientes con HP en un centro de referencia en Argentina. Variables de fácil obtención y reproducción permitieron estratificar subgrupos de mayor riesgo de muerte y/o trasplante en el seguimiento.

Pulmonary thromboendarterectomy is the treatment of choice for patients with chronic thromboembolic pulmonary hypertension. Objective: To report our 18-years experience in pulmonary thromboendarterectomy. Material and Methods: Forty one patients were analyzed between November/1992 and August/2010. Inclusion criteria: mean pulmonary artery pressure (PAPm) >30 mmHg and pulmonary vascular resistance (PVR) >300 dinas/sec.cm-5. Pulmonary angiography determined the surgical feasibility. Survival was calculated using the Kaplan-Meier method and to compare the 2 groups log rank test. Results: Forty four percent of the patients were classified in class IV. The hemodynamic changes after pulmonary thromboendarterectomy were significant (p < 0.001). PAPm 53 ± 2 vs. 29 ± 2 mm Hg, PVR 857 ± 65 vs. 245 ± 25 dinas/sec. cm-5 and CI 2.3 ± 0.1 vs. 3 ± 0.1. In-hospital mortality was 15%; in classes II - III was 4%, and in class IV 33% (p = 0.01). Late mortality was 12%. Ninety three percent of the patients improved their functional class. Overall survival at 1, 2, 5 and 10 years was 85%, 82%, 75% and 71% respectively. Survival according to functional class showed significant differences (p = 0.02). Conclusion: In our experience pulmonary thromboendarterectomy showed benefits even in this high risk population.

La tromboendarterectomía pulmonar es la terapéutica de elección para pacientes con hipertensión pulmonar tromboembólica crónica. Objetivos: Reportar la experiencia en tromboendarterectomía pulmonar a 18 años de seguimiento. Material y Métodos: Se analizaron retrospectivamente 41 pacientes entre Noviembre 1992 y Agosto 2010. Criterios de inclusión: presión arterial pulmonar media >30 mmHg y resistencia vascular pulmonar (RVP) >300 dinas/sg. cm-5. La angiografía pulmonar determinó la factibilidad quirúrgica. La supervivencia se calculó utilizando el método de Kaplan-Meier y para comparar 2 grupos log-rank test. Resultados: El 44% de los pacientes estaban en clase funcional IV. Los cambios hemodinámicos posteriores a la tromboendarterectomía fueron significativos p <0.001. La PAPm 53 ± 2 a 29 ± 2 mmHg, la RVP 857 ± 65 a 245 ± 25 dinas/sg. cm-5 y el IC 2,3 ± 0,1 a 3 ± 0,1. La mortalidad hospitalaria fue 15%, en CF II-III 4% y en CF IV 33% (p=0.01). La mortalidad alejada fue 11%. El 93% de los pacientes mejoró su clase funcional. La supervivencia global a 1, 2, 5, 10 años fue del 85%, 82%, 75% y 71% respectivamente. La supervivencia por clase funcional mostró diferencias significativas (p=0.02). Conclusión: En nuestra experiencia la tromboendarterectomía pulmonar mostró beneficios en esta población de alto riesgo.

OBJECTIVE: Trials of environmental risk factors and acute lower respiratory infections (ALRI) face a double challenge: implementing sufficiently sensitive and specific outcome assessments, and blinding. We evaluate methods used in the first randomized exposure study of pollution indoors and respiratory effects (RESPIRE): a controlled trial testing the impact of reduced indoor air pollution on ALRI, conducted among children < 18 months in rural Guatemala. METHODS: Case-finding used weekly home visits by fieldworkers trained in integrated management of childhood illness methods to detect ALRI signs such as fast breathing. Blindness was maintained by referring cases to study physicians working from community centres. Investigations included oxygen saturation (SaO2), respiratory syncytial virus (RSV) antigen test and chest X-ray (CXR). FINDINGS: Fieldworkers referred > 90% of children meeting ALRI criteria, of whom about 70% attended a physician. Referrals for cough without respiratory signs and self-referrals contributed 19.0% and 17.9% of physician-diagnosed ALRI cases respectively. Intervention group attendance following ALRI referral was 7% higher than controls, a trend also seen in compliance with RSV tests and CXR. There was no evidence of bias by intervention status in fieldworker classification or physician diagnosis. Incidence of fieldworker ALRI (1.12 episodes/child/year) is consistent with high sensitivity and low specificity; incidence of physician-diagnosed ALRI (0.44 episodes/child/year) is consistent with comparable studies. CONCLUSION: The combination of case-finding methods achieved good sensitivity and specificity, but intervention cases had greater likelihood of reaching the physician and being investigated. There was no evidence of bias in fieldworkers’ classifications despite lack of concealment at home visits. Pulse oximetry offers practical, objective severity assessment for field studies of ALRI.

OBJETIVO: Los ensayos sobre los factores de riesgo ambientales y las infecciones agudas de las vías respiratorias inferiores (IAVRI) encaran un doble reto: la necesidad de implementar evaluaciones de resultados suficientemente sensibles y específicas, y la aplicación de métodos de enmascaramiento. Evaluamos los métodos utilizados en el primer estudio aleatorizado de la exposición a la contaminación del aire en interiores y sus efectos respiratorios (RESPIRE), un ensayo controlado realizado entre niños < 18 meses en zonas rurales de Guatemala para determinar el impacto de la reducción de ese tipo de contaminación en las IAVRI. MÉTODOS: La localización de casos se hizo mediante visitas domiciliarias semanales realizadas por agentes sobre el terreno adiestrados en los métodos de la atención integrada a las enfermedades prevalentes de la infancia para detectar los signos de IAVRI, como por ejemplo una respiración rápida. El enmascaramiento se logró derivando los casos a médicos que trabajaban en centros comunitarios. Como parte del estudio se determinaron la saturación de oxígeno (SaO2) y la presencia del virus sincitial respiratorio (VSR) y se hizo una radiografía de tórax (RXT). RESULTADOS: Los trabajadores sobre el terreno derivaron a más del 90% de los niños que cumplían los criterios de IAVRI, alrededor del 70% de los cuales fueron llevados al médico. Las derivaciones por tos sin signos respiratorios y las autoderivaciones representaron el 19,0% y el 17,9% de los casos de IAVRI diagnosticados por los médicos, respectivamente. Las visitas al médico en el grupo de intervención fueron un 7% más frecuentes que en el grupo de control, tendencia observada también en la realización de las pruebas del VSR y la RXT. No hubo ningún dato que indicara un posible sesgo por estado de intervención en la clasificación de los trabajadores sobre el terreno o el diagnóstico de los médicos. La incidencia de IAVRI basada en las evaluaciones de los trabajadores sobre el terreno (1,12 episodios/niño/año) refleja una alta sensibilidad y una baja especificidad; la incidencia de IAVRI diagnosticadas por los médicos (0,44 episodios/niño/año) es similar a la de otros estudios comparables. CONCLUSIÓN: La combinación de métodos de búsqueda de casos permitió conseguir una buena sensibilidad y especificidad, pero los casos del grupo de intervención tenían más probabilidades de llegar al médico y ser estudiados. No se observó ningún indicio de sesgo en las clasificaciones de los trabajadores sobre el terreno pese a la falta de ocultación en las visitas domiciliarias. La oximetría de pulso permite evaluar de forma práctica y objetiva la gravedad del estado en los estudios de campo de las IAVRI.

OBJECTIF: Les études de l’influence de facteurs environnementaux sur les infections aigues des voies respiratoires basses (ALRI) se heurtent à une double difficulté, à savoir la mise en œuvre de méthodes d’évaluation des résultats suffisamment sensibles et spécifiques et le masquage des identités. Nous avons donc évalué les méthodes utilisées dans la première étude d’exposition randomisée des effets sur les voies respiratoires de la pollution intérieure RESPIRE (étude cas-témoins randomisée de l’impact d’une réduction de la pollution de l’air intérieur sur les infections aigues des voies respiratoires basses, menée sur des enfants de 18 mois au plus, en milieu rural au Guatemala). MÉTHODES: Pour dépister ces maladies, on a eu recours à des visites à domicile par des agents de terrain formés aux méthodes de prise en charge intégrée des maladies de l’enfance permettant d’en détecter les signes, comme l’accélération du rythme respiratoire par exemple. La confidentialité a été maintenue en orientant les cas vers des médecins de l’étude travaillant dans des centres communautaires. Parmi les examens pratiqués figuraient notamment la détermination de la saturation artérielle en oxygène (SaO2), un test de détection antigénique du virus respiratoire syncytial (VRS) et une radiographie thoracique. RÉSULTATS: Les agents de terrain ont orienté vers un spécialiste plus de 90 % des enfants remplissant les critères de présence d’une ALRI, parmi lesquels 70 % ont consulté un médecin. Les orientations vers un spécialiste pour toux sans signe respiratoire et les consultations de spécialiste à l’initiative des sujets eux-mêmes ont représenté respectivement 19,0 % et 17,9 % des cas d’ALRI médicalement diagnostiqués. Les membres du groupe d’intervention étaient plus nombreux que les témoins (+ 7 %) à suivre la recommandation de consulter un spécialiste pour suspicion d’ALRI, tendance également observée dans la soumission au test de dépistage du VRS et à la radiographie thoracique. On n’a relevé aucun élément indiquant la présence d’un biais lié à la participation du sujet à l’intervention dans la classification par les agents de terrain ou le diagnostic par les médecins. L’incidence des ALRI détectées par des agents de terrain (1,12 épisode/enfant/an) était cohérente avec la forte sensibilité et la faible spécificité de cette méthode de dépistage. Celle des cas d’ALRI diagnostiqués par des médecins (0,44 épisode/enfant/an) était également cohérente avec celle obtenue dans des études comparables. CONCLUSION: L’association des deux méthodes de dépistage a permis d’obtenir une sensibilité et une spécificité satisfaisantes, mais la probabilité d’être vu par un médecin et d’être examiné était plus forte pour les cas ayant bénéficié de l’intervention. Aucun élément n’indiquait la présence d’un biais dans la classification établie par les agents de terrain, en dépit du manque de confidentialité des visites à domicile. La sphygmo-oxymétrie offre un moyen pratique et objectif pour évaluer la gravité de la pathologie dans le cadre des études sur le terrain des ALRI.

Uma série de sete complexos alquil substituídos do tipo (RCp)2TiCl2 (R= H, etila, n-propila, iso-propila, n-butila, iso-amila, ciclo-hexila) foi preparada juntamente com o ainda inédito contendo R= sec-butila, e o efeito do substituinte na polimerização do estireno foi investigada à temperaturas de polimerização de 0 e 50 ºC. Foi encontrado que R = n-butila maximiza a produção de poliestireno sindiotático. Ligantes alquilas com R = RCH2- são superiores aos R = RR’CH- e ao precursor Cp2TiCl2 na produção de poliestireno sindiotático de alto peso molecular.

A series of seven alkyl substituted complexes of the type (RCp)2TiCl2 (R= H, ethyl, n-propyl, iso-propyl, n-butyl, iso-amyl, ciclo-hexyl) was prepared, as well as the novel compound with R = sec-butyl, and the effects of the substituent on the polymerisation of styrene was investigated at the temperatures of 0 and 50 ºC. It was found that R = n-butyl maximises the production of syndiotactic polystyrene. Alkyl ligands with R = RCH2- are superior to those with R = RR’CH- and to the precursor Cp2TiCl2 in the production of high molecular weight syndiotactic polystyrene.

The decision in 1987 by the pharmaceutical firm Merck & Co. to provide Mectizan® (ivermectin) free of charge to river blindness control programs has challenged the international public health community to find effective ways to distribute the drug to rural populations most affected by onchocerciasis. In the Americas, PAHO responded to that challenge by calling for the elimination of all morbidity from onchocerciasis from the Region by the year 2007 through mass distribution of ivermectin. Since 1991, a multinational, multiagency partnership (consisting of PAHO, the endemic countries, nongovernmental development organizations, the Centers for Disease Control and Prevention in Atlanta, Georgia, as well as academic institutions and funding agencies) has developed the political, financial, and technical support needed to move toward the realization of that goal. This partnership is embodied in the Onchocerciasis Elimination Program for the Americas (OEPA), which is supported by the River Blindness Foundation (RBF) and now by the Carter Center. OEPA was conceived as a means of maintaining a regional initiative to eliminate what is otherwise a low priority disease. Since its inception in 1993, the OEPA has provided more than US$ 2 million in financial, managerial, and technical assistance to stimulate and/or support programs in Brazil, Colombia, Ecuador, Guatemala, Mexico, and Venezuela, so as to take full advantage of the Merck donation. Now halfway into a five-year, US$ 4 million grant provided through the Inter-American Development Bank, the OEPA's capacity to support the regional initiative is assured through 1999.

La decisión tomada en 1987 por la Merck & Co., fabricante de productos farmacéuticos, de proveer Mectizan® (ivermectina) gratuitamente a los programas de control de la oncocercosis ha obligado a la comunidad sanitaria internacional a buscar formas de distribuir el medicamento a las poblaciones rurales que se ven más afectadas por la enfermedad. En las Américas, la OPS respondió al reto con un llamado a eliminar de la Región toda morbilidad por oncocercosis para el año 2007 mediante la distribución de ivermectina al público. Desde 1991, una alianza multinacional de diversas entidades (la OPS, países con oncocercosis endémica, agencias de desarrollo no gubernamentales, los Centros para el Control y la Prevención de Enfermedades en Atlanta, Georgia, instituciones académicas y agencias de financiamiento) ha generado el apoyo político, económico y técnico necesario para tratar de alcanzar esa meta. Esta alianza está representada por el Programa de Eliminación de la Oncocercosis en las Américas (OEPA), subvencionado por la Fundación Ceguera de los Ríos y actualmente por el Centro Carter. El OEPA se creó como iniciativa de alcance regional destinada a eliminar una enfermedad que no merece atención prioritaria. Desde su aparición en 1993, el OEPA ha aportado más de US$2 millones en ayuda económica, administrativa y técnica para fomentar y subvencionar programas en Brasil, Colombia, Ecuador, Guatemala, México y Venezuela, logrando así aprovechar al máximo la donación de la Merck & Co. Ahora que hemos llegado a la mitad de una subvención de 5 años y US$ 4 millones aportada por el Banco Interamericano de Desarrollo, se sabe que el OEPA tiene la capacidad para apoyar la iniciativa regional hasta fines de 1999.

Amostras de lã de várias regiões do velo (lombo, paleta, costilhar e quarto) de borregas Ideal e Corriedale foram analisadas com o objetivo de observar dentro de qual metodologia de análise da suarda as características apresentavam melhores coeficientes de correlação com coloração da lã. Os métodos testados foram: extração da cera em primeiro lugar (método Cera + Suor) e extração do suor em primeiro (método Suor + Cera). Os coeficientes de correlação entre as variáveis pH, cor, absorbância e percentagem de suor e porcentagem de cera com brilho e grau de amarelamento da lã mostraram-se superiores no método Cera + Suor (P < 0,01) nas duas raças. Os coeficientes para a raça Corriedale apresentaram valores inferiores aos obtidos com a raça Ideal. As características de pH, cor e absorbância de suor mostraram uma correlação mais estreita com o grau de amarelamento do que a porcentagem do suor em si (r = 0,62; 0,50; 0,61 vs. 0,36, no método Cera + Suor, para a raça Ideal). A porcentagem de cera apresentou um coeficiente de correlação médio com o grau de amarelamento, significativo somente para Ideal, e de valor negativo. Através da análise de regressão múltipla observou-se que a cor e absorbância do suor e a relação cera/suor determinaram 36% das modificações ocorridas em Y-Z.

Wool samples from various fleece regions (back, shoulder, britch and mid-side) of Polwarth and Corriedale hogget ewes were analysed in order to observe under which yolk analysis methodology (Wax extraction before Suint or Suint + Wax) the yolk traits showed the best correlation coefficients with scoured wool colour (Y-Z). The correlations between suint pH, colour, absorbancy and percent and wax percent with brightness and scoured wool colour were higher in Wax + Suint method (P < 0.01) to both breeds. The coefficients to Corriedales showed smaller values from those obtained with Polwarths. The suint pH, colour and absorbancy traits recorded stronger relationship with Y-Z than suint percent itself ( r = 0.62, 0.50, 0.61 vs. 0.36 in the Wax + Suint method to Polwarths). Wax percent had a medium coefficient correlation, significantly only to Polwarths, with Y-Z (negative value). Looking multiple regression analysis, it was possible to understand that suint pH and absorbancy and wax/suint ratio determined 36% of variations ocurred with Y-Z.
