Early discharge predictors among inpatient crack cocaine users

Abstract Introduction High rates of early hospital discharge are often observed in crack cocaine users and are related to adverse outcomes and increased public spending. This study evaluated clinical and sociodemographic factors associated with early treatment discharge among crack users. Methods The sample comprised 308 men diagnosed with crack cocaine use disorder (crack only), aged 18 to 65 years, admitted between 2013 and 2017 to a male-only hospital unit to treat substance use disorders. Sociodemographic and clinical data were obtained using the Addiction Severity Index, 6th version, and a Sociodemographic Questionnaire. Results Early discharge (within 7 days) was significantly associated with lack of own income, insufficient family support, being single, and recent homelessness. Regarding drug use, lower treatment retention was related to younger age of crack use onset, recent alcohol use, and nicotine use. Factors such as age, skin color, and educational level showed no relation to the outcome. Conclusion Our findings suggest that presence of characteristics verifiable at the time of admission may be related to crack users’ treatment retention. Identification of these factors can contribute to target interventions in order to improve treatment adherence in crack cocaine users.

O severance pay no Brasil: uma análise do Caso HRT

Resumo Os severance packages (“pacotes de indenização”) são pagamentos concedidos a executivos de alto escalão para mitigar problemas de gestão, como a aversão ao risco. Quando o plano de incentivo estipula a aquisição do controle como hipótese de disparo da indenização, o benefício passa a assumir a forma de um golden parachute e serve também como medida defensiva contra aquisições hostis pelo aumento do custo da operação para o adquirente. Compreende-se que a avaliação da atuação do conselho de administração na aprovação do pagamento executivo deve se fundamentar na conveniência do instrumento para o interesse social. Tanto a estrutura de propriedade como o mercado de controle acionário são elementos relevantes para que se possa apurar se o incentivo busca cumprir o fim para o qual está sendo previsto, em especial quando a ameaça de aquisições hostis é utilizada como justificativa para a implementação. Assim, com base no estudo do Caso HRT, analisa-se o severance pay a partir das características do mercado de capitais brasileiro. Ainda que não represente toda a complexidade do tema, o caso escolhido retrata uma realidade específica e oferece reflexões importantes para a regulação eficiente do pagamento executivo no Brasil. O presente artigo pretende demonstrar que a Comissão de Valores Mobiliários (CVM) levou em conta benefícios meramente teóricos do severance pay e não incorporou as especificidades do mercado acionário brasileiro na análise, decisão que trouxe prejuízos aos acionistas da HRT Participações em Petróleo S/A.
Abstract Severance packages are granted to senior executives to mitigate management problems, such as risk aversion. When the incentive plan stipulates the acquisition of control as a hypothesis for triggering the indemnity, the benefit takes the form of a golden parachute and works as a defensive measure against hostile acquisitions by increasing the cost of the operation for the acquirer. It is understood that the evaluation of the board of directors performance in the executive pay approval must be based on the convenience of the instrument for social interest, which requires an institutional analysis. Both the ownership structure and the market for corporate control are relevant elements to determine if the incentive seeks to fulfill the purpose for which it is being planned, especially when the threat of hostile acquisitions is used as a justification for implementation. Therefore, based on the HRT Case, the severance pay is analyzed in observance of the Brazilian capital market characteristics. Although it does not represent all the complexity of the issue, the case chosen is a portrait of a specific reality and offers important thoughts for an efficient regulation of executive pay in Brazil. This work intends to demonstrate that Comissão de Valores Mobiliários [CVM] took into account purely theoretical benefits of the severance pay and did not incorporate the brazilian stock market specificities in the analysis, a decision that brought losses to HRT Participações em Petróleo S/A shareholders.

BMI-1 expression increases in oral leukoplakias and correlates with cell proliferation

Abstract Oral leukoplakia (OL) is a white lesion of an indeterminate risk not related to any excluded (other) known diseases or disorders that carry no increased risk for cancer. Many biological markers have been used in an attempt to predict malignant transformation; however, no reliable markers have been established so far. Objective To evaluate cell proliferation and immortalization in OL, comparing non-dysplastic (Non-dys OL) and dysplastic OL (Dys OL). Methodology This is a cross-sectional observational study. Paraffin-embedded tissue blocks of 28 specimens of Non-dys OL, 33 of Dys OL, 9 of normal oral mucosa (NOM), 17 of inflammatory hyperplasia (IH), and 19 of oral squamous cell carcinomas (OSCC) were stained for Ki-67 and BMI-1 using immunohistochemistry. Results A gradual increase in BMI-1 and K-i67 expression was found in oral carcinogenesis. The immunolabeling for those markers was higher in OSCC when compared with the other groups (Kruskal-Wallis, p<0.05). Ki-67 expression percentage was higher in OL and in IH when compared with NOM (Kruskal-Wallis/Dunn, p<0.05). Increased expression of BMI-1 was also observed in OL when compared with NOM (Kruskal-Wallis/Dunn, p<0.05). No differences were observed in expression of both markers when non-dysplastic and dysplastic leukoplakias were compared. A significant positive correlation between Ki-67 and BMI-1 was found (Spearman correlation coefficient, R=0.26, p=0.01). High-grade epithelial dysplasia was associated with malignant transformation (Chi-squared, p=0.03). Conclusions These findings indicate that BMI-1 expression increases in early oral carcinogenesis and is possibly associated with the occurrence of dysplastic changes. Furthermore, our findings indicate that both Ki-67 and BMI-1 are directly correlated and play a role in initiation and progression of OSCC.

DESIGN OF GEODETIC NETWORKS BASED ON OUTLIER IDENTIFICATIONCRITERIA: AN EXAMPLE APPLIED TO THE LEVELING NETWORK

Resumo: Nós apresentamos um método de simulação numérica para planejamento de redes geodésicas. O critério de qualidade considerado é baseado no poder do teste do procedimento estatístico data snooping. Esse critério expressa a probabilidade do procedimento data snooping de identificar corretamente um outlier. Em geral, o valor do poder do teste é sempre desconhecido na prática (definido na teoria). Porém, com o advento de computadores mais rápidos e sistemas de armazenamento de dados de maior capacidade, o poder do teste pode ser estimado por meio de simulações numéricas. O número de experimentos que o procedimento data snooping identifica corretamente um outlier é quantificado por meio do método Monte Carlo de simulação. Se a configuração inicial da rede não atender o critério de confiabilidade, então uma nova observação é adicionada no projeto da rede. O método não faz uso de observações reais coletadas em campo, dependendo apenas da configuração geométrica da rede, das incertezas das observações e da magnitude dos outliers. A eficiência do método é verificada por meio de um exemplo numérico de uma rede de nivelamento geométrico simulada. Os resultados mostraram a necessidade de cinco observações adicionais entre os pontos adjacentes. A adição destas novas observações melhorou a confiabilidade interna da rede de aproximadamente 18%. Portanto, a rede é projetada de modo que seja capaz de identificar outliers e ainda resistir a outilers não identificados nas observações - de acordo com os níveis de probabilidade.
Abstract: We present a numerical simulation method for designing geodetic networks. The quality criterion considered is based on the power of the test of data snooping testing procedure. This criterion expresses the probability of the data snooping to identify correctly an outlier. In general, the power of the test is defined theoretically. However, with the advent of the fast computers and large data storage systems, it can be estimated using numerical simulation. Here, the number of experiments in which the data snooping procedure identifies the outlier correctly is counted using Monte Carlos simulations. If the network configuration does not meet the reliability criterion at some part, then it can be improved by adding required observation to the surveying plan. The method does not use real observations. Thus, it depends on the geometrical configuration of the network; the uncertainty of the observations; and the size of outlier. The proposed method is demonstrated by practical application of one simulated leveling network. Results showed the needs of five additional observations between adjacent stations. The addition of these new observations improved the internal reliability of approximately 18%. Therefore, the final designed network must be able to identify and resist against the undetectable outliers - according to the probability levels.

Lentibulariaceae from Serra dos Pireneus, Goiás, Brazil

Resumo Este estudo apresenta um levantamento florístico das Lentibulariaceae ocorrentes na Serra dos Pireneus (SDP), estado de Goiás, Brasil. O bioma da SDP é o Cerrado, onde diferentes fitofisionomias ocorrem, incluindo campos úmidos, ambientes onde as Lentibulariaceae geralmente habitam. Coletas dos espécimes foram realizadas entre 2007 e 2010 e amostras de herbários também foram consultadas. Nove espécies foram encontradas em Lentibulariaceae: dos gêneros Genlisea (2 spp.) e Utricularia (7 spp.). Descrições, ilustrações, fotografias e chaves de identificação para estas espécies são apresentadas neste estudo.
Abstract This study provides a floristic survey of Lentibulariaceae occurring in Serra dos Pireneus (SDP), state of Goiás, Brazil. The phytogeographic domain in which SDP is located is the Cerrado, with different vegetation, including floodplain grasslands (campos úmidos) where Lentibulariaceae usually occur. Samples were collected from 2007 to 2010 and herbarium samples were consulted as well. We recorded nine species of Lentibulariaceae: the genera Genlisea (2 spp.) and Utricularia (7 spp.). Descriptions, illustrations, photographs and identification keys are presented in this study.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

JUSTIFICATIVA E OBJETIVOS: No âmbito das afecções do aparelho locomotor, a dor é o sintoma mais frequente e, além da alta prevalência, sua importância decorre por estar presente nas doenças que causam maior impacto negativo na qualidade de vida e na produtividade dos indivíduos acometidos. O objetivo deste estudo foi investigar tópicos relevantes sobre a influência da dor osteoarticular crônica na capacidade funcional no idoso.CONTEÚDO: Foi realizado um estudo bibliográfico mediante consulta às principais publicações sobre o tema. O material foi identificado com auxílio das bases eletrônicas de dados da Medline, LILACS, Scielo e Pubmed. Foram empregados os descritores "dor crônica", "doenças osteoarticulares" e "capacidade funcional".CONCLUSÃO: Deve haver maior atenção às políticas públicas de implementação de programas que visem principalmente a prevenção e o controle das doenças crônicas osteoarticulares, promovendo funcionalidade e maior qualidade de vida para essa população.
BACKGROUND AND OBJECTIVES: Among locomotor system diseases, pain is the most frequent symptom and, in addition to its high prevalence, it is important because it is present in diseases causing the highest negative impact on quality of life and productivity of affected individuals. This study aimed at investigating relevant topics on the influence of chronic osteoarticular pain on the functional capacity of the elderly.CONTENTS: A bibliographic study was carried out by querying major publications on the subject. Material was identified with the aid of electronic databases Medline, LILACS, Scielo and Pubmed. Keywords used were "chronic pain", "osteoarticular diseases" and "functional capacity".CONCLUSION: Further attention should be given to public policies implementing programs aiming especially at preventing and controlling chronic osteoarticular diseases, promoting functionality and better quality of life for this population.

O objetivo deste artigo é analisar a influência de intensas tempestades geomagnéticas na ionosfera e no posicionamento por GNSS (GPS). Neste sentido, foram analisados os efeitos da intensa tempestade geomagnética de 20 de novembro de 2003 utilizando dados GPS de algumas estações da RBMC (Rede Brasileira de Monitoramento Contínuo), situadas em diferentes locais na região brasileira, e mapas globais da ionosfera. Analisando os resultados se observa um aumento na densidade de elétrons livres da ionosfera nas regiões mais próximas do equador geomagnético no período da tarde no dia da tempestade. No período após o pôr do Sol do dia da tempestade, verifica-se um aumento na densidade de elétrons livres e de irregularidades ionosféricas em regiões mais afastadas do equador geomagnético, em comparação com dias geomagneticamente calmos. Quando analisado o posicionamento por ponto, se observa maiores discrepâncias em planimetria e altimetria no posicionamento para os mesmos períodos em que ocorreram mudanças na ionosfera, principalmente para as estações GPS mais afastadas do equador geomagnético no período após o pôr do Sol.
The goal of this paper is to analyze the influence of the intense geomagnetic storms in the ionosphere and GNSS (GPS) positioning. It was analyzed the effects of intense geomagnetic storm of November 20th 2003 using GPS data from RBMC (Brazilian Network for Continuous Monitoring) located in different sites in the Brazilian region and ionosphere global maps. While analyzing the results, it can be observed an increase in the electron density of the ionosphere in the regions near to the geomagnetic equator in the afternoon on the day of the storm. In the period after the sunset of the storm day, there is an increase in the density of free electrons and ionospheric irregularities in regions furthest from the geomagnetic equator, when compared to geomagnetically quiet days. When the positioning point is analyzed, it is observed high discrepancies values in planimetry and altimetry at the same position for periods of changes that occurred in the ionosphere, especially for the GPS stations located furthest from the geomagnetic equator in the period after the sunset Sun.

Productive performance of broiler breeder hens fed 25-hydroxycholecalciferol

Foi realizado um experimento com o objetivo de avaliar a suplementação de 25-hidroxicolecalciferol (25(OH)D3) associado à vitamina D3 em dietas para reprodutoras de frangos de corte. Foram alojadas matrizes Cobb 500 em delineamento completamente casualizado, composto de quatro tratamentos e oito repetições de 20 fêmeas e 2 machos. Os tratamentos experimentais foram constituídos de premixes vitamínicos contendo 2.000 e 3.400 UI/kg ração de vitamina D3 como única fonte ou 2.000 UI D3 associado a 35 ou 69 mg/t de 25(OH)D3. Não houve efeito da suplementação de 25(OH)D3 sobre a produção de ovos durante o período de 32 a 67 semanas. A qualidade da casca dos ovos, mensurada por meio da gravidade específica, melhorou com a suplementação de 25(OH)D3 em reprodutoras com 60 semanas de idade, independentemente da dose. Não foi observada diferença significativa na taxa de eclosão de ovos férteis incubados nas 54 e 64 semanas de idade das aves. Às 64 semanas, houve menor mortalidade de embriões na terceira semana entre as aves recebendo a dieta com 2.000 UI D3, enquanto, às 67 semanas, houve menor mortalidade na primeira semana de embriões provenientes de reprodutoras sob suplementação com 2.000 UI D3 e 2.000 UI D3 + 69 mg 25(OH)D3. A combinação de 25-hidroxicolecalciferol e colecalciferol teve efeito similar ao das dietas com vitamina D3 como fonte exclusiva sobre o desempenho produtivo de reprodutoras pesadas.
An experiment was carried out with the objective of evaluating the addition of 25-hydroxycholecalciferol (25(OH) D3) in diets of broiler breeder hens. The experiment used Cobb 500 broiler breeder hens and was allotted to a complete randomized design with four treatments and eight replications of twenty females and two males each. The treatments consisted of vitamin premixes with 2,000 and 3,400 IU/kg diet vitamin D3 as the only source of vitamin or 2,000 IU D3 plus 35 or 69 mg/t of 25(OH) D3. Results of this experiment indicated that 25(OH) D3 had no significant effect on egg production parameters from 32 to 67 weeks. The supplementation of 25(OH) D3 resulted in better quality egg shells evaluated by the specific gravity at 60 weeks of age, regardless of the dosage. No significant differences were observed for hatchability of broiler breeder fertile eggs at 54 and 64 weeks. At 64 weeks, the hatch residue breakout showed less embryo mortality at the third week for treatments receiving 2,000 UI D3 in the diet and less embryo mortality at the second week of development from hens aged 67 weeks and supplemented with 2,000IU D3 and 2,000IU D3+ 69 mg 25(OH)D3. It was concluded that the supplementation with 25-hydroxycholecalciferol with cholecalciferol had similar effects as the diets with vitamin D3 as the only source on the productive performance of broiler breeder hens.

RACIONAL: O carcinoma hepatocelular é um dos tumores malignos mais comuns em todo o mundo. Exames de imagens, especialmente tomografia computadorizada e ultra-sonografia, estão entre as principais técnicas diagnósticas, embora a acurácia destes métodos possa apresentar significativa variabilidade. OBJETIVOS: Determinar a prevalência de carcinoma hepatocelular em pacientes cirróticos submetidos a transplante hepático na Santa Casa de Misericórdia de Porto Alegre, RS; estimar a sensibilidade da tomografia computadorizada e da ultra-sonografia na detecção pré-transplante de carcinoma hepatocelular nesse grupo de pacientes; correlacionar características radiológicas com achados anatomopatológicos. MATERIAIS E MÉTODOS: Estudo de prevalência retrospectivo. População: pacientes adultos, cirróticos, submetidos a transplante hepático de janeiro de 1990 a julho de 2003. Entre os 292 pacientes transplantados, foi diagnosticado 31 casos de carcinoma hepatocelular, dos quais 29 foram incluídos no estudo. As características tomográficas e ecográficas dos tumores diagnosticados pré-transplante foram comparadas com as observadas em exame anatomopatológico. RESULTADOS: A prevalência da infecção pelo vírus da hepatite C nos pacientes com diagnóstico de carcinoma hepatocelular foi de 93,5% e a prevalência deste entre os pacientes transplantados foi de 10,6%. A sensibilidade dos métodos de imagem na detecção de casos de carcinoma hepatocelular foi de 70,3% para tomografia computadorizada e de 72% para ecografia. CONCLUSÃO: A prevalência de carcinoma hepatocelular na instituição onde foi desenvolvido o estudo, bem como a sensibilidade da ultra-sonografia e da tomografia computadorizada para detecção dessa neoplasia na avaliação pré-transplante foi semelhante à relatada na literatura. Em contrapartida, a prevalência de infecção pelo vírus da hepatite C, fator etiológico de hepatopatia mais freqüente nos pacientes desta série, é das maiores já relatadas. Os fatores que influenciaram as taxas de detecção de carcinoma hepatocelular foram: tempo decorrido entre realização do exame e transplante; realização de tomografia computadorizada com fase arterial; tamanho da lesão. A fase arterial provou ser a mais importante no diagnóstico de carcinoma hepatocelular neste estudo.
BACKGROUND: Hepatocellular carcinoma is one of the most common malignant tumors worldwide. Imaging techniques, specially computed tomography and ultrasound, are among the most useful diagnostic tools, although the accuracy of these methods may have a significant variability. AIMS: To determine the prevalence of hepatocellular carcinoma in cirrhotic patients undergoing orthotopic liver transplantation at "Santa Casa de Misericórdia" of Porto Alegre, RS, Brazil; to estimate the sensitivity of computed tomography and ultrasound in pretransplantation detection of hepatocellular carcinoma in this population; to correlate the radiological characteristics with anatomopathological findings. MATERIALS AND METHODS: Retrospective prevalence study. Population: adult, cirrhotic patients undergoing orthotopic liver transplantation from January 1990 to July 2003. Among the 292 transplanted patients, 31 cases of hepatocellular carcinoma were diagnosed, of which 29 were included in the study. Tumor characteristics in both ultrasound and computed tomography were compared to those observed in anatomopathological examination. RESULTS: Prevalence of hepatitis C virus infection among patients with diagnosis of hepatocellular carcinoma was 93.5%, and the prevalence of hepatocellular carcinoma among transplanted patients was 10.6%. The overall sensitivity of the imaging techniques was 70.3% for computed tomography and 72% for ultrasound. CONCLUSION: The prevalence of hepatocellular carcinoma at our institution, as well as the sensitivity of both ultrasound and computed tomography to detect such tumors at pretransplantation screening were similar to those found by other authors, while the prevalence of hepatitis C virus infection, the most common etiological agent for liver disease in our patients, is one of the highest ever reported in literature. Factors influencing hepatocellular carcinoma detection rates were: time from examination to liver transplantation; acquisition of computed tomography images during arterial phase; lesion size. Arterial phase proved to be the most useful part of computed tomography examination in this study.
