RESUMO Objetivo Verificar a associação entre vulnerabilidade clínico-funcional e autopercepção vocal e auditiva de idosos com histórico de hanseníase. Método Estudo transversal desenvolvido com 117 idosos de uma antiga colônia de hanseníase no sudeste do Brasil. Foram analisadas informações sociodemográficas, e os protocolos: Índice de Vulnerabilidade Clínico-funcional (IVCF-20), Hearing Handicap Inventory for the Elderly Screening Version (HHIE-S), Rastreio de Alteração Vocal em Idosos (RAVI) e Índice de Desvantagem Vocal 10 (IDV-10). O IVCF-20 classifica o idoso como robusto, em risco de fragilização e frágil. Para análise dos dados utilizou-se o modelo de regressão logística ordinal de chances proporcionais. Resultados 37,6% dos idosos foram classificados em robustos, 35,0% em risco de fragilização e 27,4% em frágeis. A alteração vocal (RAVI), desvantagem vocal (IDV-10) e restrição à participação auditiva (HHIE-S) foram observadas em 65,8%, 24,8% e 48,7% dos idosos, respectivamente. Na análise multivariada, verificou-se que idosos mais velhos (OR=1,11; IC: 1,05-1,16) e com desvantagem vocal (OR=4,11; IC 95%: 1,77-9,56) tiveram maiores chances de serem classificados como em risco de fragilização ou frágil. A presença simultânea de desvantagem vocal e restrição à participação auditiva (46,9%) foi maior entre os idosos frágeis. Conclusão O aumento da idade dos idosos e a presença de desvantagem vocal tem associação com a maior vulnerabilidade clínico-funcional. As altas prevalências de alterações vocais e auditivas reforçam a necessidade da inclusão do fonoaudiólogo nas políticas públicas voltadas ao cuidado de pessoas com histórico de hanseníase.

ABSTRACT Purpose Verifying the association between clinical-functional vulnerability and vocal and auditory self-perception in seniors presenting a history of leprosy. Methods Cross-sectional study encompassing 117 elderly people from an old leprosy colony in southeastern Brazil. The research analyzed sociodemographic information and the following protocols: Clinical-Functional Vulnerability Index-20 (IVCF-20), Hearing Handicap Inventory for the Elderly Screening Version (HHIE-S), Screening for Vocal Change in the Elderly (RAVI) and Vocal Handicap Index 10 (IDV-10). For data analysis, the ordinal logistic regression model of proportional odds was used. Results 37.6% of the elderly were classified as robust, 35.0% at risk of fragility and 27.4% as frail. Vocal alteration (RAVI), vocal handicap (IDV-10) and restriction to auditory participation (HHIE-S) were observed in 65.8%, 24.8% and 48.7% respectively, in the studied population. In the multivariate analysis, it was found the oldest group (OR = 1.11; CI: 1.05-1.16) and those with vocal handicap (OR = 4.11; 95% CI: 1.77-9.56) were more likely to be classified as at risk of fragility or already fragile. The simultaneous presence of vocal handicap and restriction of auditory participation (46.9%) was larger among frail elderly people. Conclusion The increasing age of the elderly and the presence of vocal handicap is associated with greater clinical-functional vulnerability. The high prevalence of both voice and hearing disorders reinforces the need for the speech therapist to be included in public policies, directed towards caring for people with a history of leprosy.

RESUMO Objetivo Verificar a suspeição de alteração vocal em idosos ativos e a associação com aspectos sociodemográficos, hábitos de vida relacionados à voz e desvantagem vocal. Métodos Estudo observacional transversal realizado com 254 idosos usuários de academias públicas do município de Belo Horizonte (MG). A coleta de dados incluiu uma entrevista, contendo informações sociodemográficas, hábitos de vida relacionados à voz e autorrelato de rouquidão, além da aplicação dos protocolos Índice de Desvantagem Vocal (IDV-10) e Rastreamento de Alteração Vocal em Idosos (RAVI). O resultado do RAVI foi considerado a variável resposta. Os dados foram submetidos à análise descritiva e de associação, por meio dos testes Qui-quadrado de Pearson e Regressão de Poisson, com variância robusta (nível de significância de 5%). Resultados Verificou-se que a maioria dos idosos era do sexo feminino (83,5%), na faixa etária de 60 a 70 anos (65,4%), aposentada (84,9%) e sem companheiros (61,8%). Segundo o protocolo RAVI, 44,5% dos idosos apresentaram suspeição de alteração vocal. Na análise multivariada, as variáveis autorrelato de rouquidão e desvantagem vocal apresentaram associação com a suspeição de alteração vocal mensurada pelo RAVI. Conclusão Foi elevada a suspeição de alteração vocal em idosos ativos, sendo maior entre os idosos com autorrelato de rouquidão e com desvantagem vocal.

ABSTRACT Purpose To verify suspected vocal alterations in active elderly and its association with sociodemographic, voice-related lifestyle habits and vocal handicap aspects. Methods Cross-sectional observational study conducted with 254 elderly users of public health gyms in Belo Horizonte municipality. Data collection included an interview containing sociodemographic information, voice-related life habits and hoarseness self-report, in addition to the application of protocols: vocal handicap index (IDV-10) and screening for voice disorders (RAVI in Portuguese). The result of RAVI was considered the outcome variable. The data were subject to descriptive and association analysis using Pearson's Chi-square and Poisson Regression tests with Robust variance (5% significance level). Results It was found that the majority of the elderly are female (83.5%), aged 60 to 70 years (65.4%), retired (84.9%) and without a partner (61.8%). According to the RAVI protocol, 44.5% of them are suspected of voice alteration. Through multivariate analysis, the self-reported variables of hoarseness and vocal handicap showed association with suspected vocal changes measured by RAVI. Conclusion The vocal alteration suspicion was high in the elderly actives. The suspicion of vocal alteration was higher among the elderly with self-reported hoarseness and with vocal handicap.

Resumo Uma onça parda (Puma concolor) foi encontrada em uma área urbana após atropelamento e, apesar do suporte terapêutico fornecido, o animal morreu três dias depois. No exame post-mortem, múltiplos nódulos firmes foram identificados na mucosa gástrica. Os nódulos eram revestidos por mucosa intacta com um orifício central, do qual se insinuavam nematódeos cilíndricos e avermelhados. Vinte e sete nematódeos foram recuperados para avaliação morfológica e morfométrica. Na avaliação histopatológica do tecido gástrico, os nematódeos adultos apareceram em cortes longitudinais e transversais, circundados por bandas espessas de colágeno, intercaladas por infiltrado inflamatório misto. Os nematódeos eram constituídos por cutícula eosinofílica, com projeções serrilhadas voltadas caudalmente (do tipo bulbar), musculatura celomiariana, pseudoceloma e, nas fêmeas, útero com numerosos ovos larvados cujos achados foram sugestivos do gênero Cylicospirura. Morfologicamente, a cavidade bucal continha seis grandes dentes trífidos, na fêmea, a abertura da vulva era anterior à junção esôfago intestinal; e os machos tinham cinco pares de pequenas papilas próximas à ponta da cauda. Esses achados foram consistentes com Cylicospirura felineus. Este parasita deve ser incluído no diagnóstico diferencial de lesões nodulares da parede gástrica em felinos selvagens.

Abstract An adult male puma (Puma concolor), hit by a car in an urban area, died three days later despite the therapeutic support provided. At necropsy, multiple firm nodules were identified in the gastric mucosa. The nodules were coated by an intact mucosa with a central opening from which reddish and cylindrical nematodes protruded into the lumen. Twenty-seven nematodes were retrieved for morphological and morphometric evaluations. During histopathological examination of the gastric tissue, the adult nematodes appear in longitudinal and transverse sections, surrounded by thick bands of collagen, interspersed with mixed inflammatory infiltrates. The nematodes had an eosinophilic cuticle with caudal serrated projections (bulbar type), coelomyarian musculature, pseudocoelom, and females with uterus containing numerous larvated eggs, characteristics consistent with the Cylicospirura genus. Morphologically, female nematodes had six large tricuspid teeth in the oral cavity and the vulva had an opening anterior to the esophagus–intestinal junction. Male nematodes had five pairs of small papillae near the tip of the tail. These findings were consistent with Cylicospirura felineus. This parasite should be included in the differential diagnosis of nodular gastric wall lesions in wild felids.

OBJETIVO: Caracterizar o padrão de fechamento velofaríngeo de pacientes operados de fissura palatina e sua relação com gênero, faixa etária, tipo de fissura e diagnóstico da função velofaríngea. MÉTODOS: Estudo retrospectivo realizado a partir de 89 exames de nasofaringoscopia de pacientes operados de fissura palatina, entre 4 e 47 anos (média=17,32±10,50). Os dados foram analisados quanto ao gênero; faixa etária (4 a 8, 9 a 12 anos, 13 a 18 e acima de 18 anos); tipo de fissura (pós-forame e transforame); diagnóstico da função velofaríngea (adequada, marginal e inadequada) e padrão de fechamento velofaríngeo (coronal, circular, circular com prega de Passavant e sagital). Os resultados foram analisados estatisticamente. RESULTADOS: Na amostra estudada, 59,55% eram do gênero feminino; 39,33% tinham mais de 18 anos de idade; 44,94% apresentavam fissura pós-forame e 55,06% transforame; 14,61% fechamento adequado, 53,93% marginal e 31,46% inadequado. Quanto ao padrão de fechamento, 37,08% apresentaram fechamento coronal, 33,71% circular, 15,73% circular com Passavant e 13,48% sagital. Houve relação do padrão de fechamento com o diagnóstico da função velofaríngea (p=0,05). CONCLUSÃO: A maior parte da amostra apresentou fechamento velofaríngeo coronal, seguido pelo circular, circular com Passavant e sagital. Não houve relação entre o padrão de fechamento e as variáveis gênero, faixa etária e tipo de fissura, mas observou-se influência relacionada ao diagnóstico da função velofaríngea.

PURPOSE: To characterize the velopharyngeal closure pattern in patients submitted to cleft palate surgery and its relation with gender, age, cleft type, and diagnosis of velopharyngeal function. METHODS: Retrospective study based on 89 nasopharyngoscopy studies of operated cleft palate patients with ages between 4 and 47 years (mean=17.32±10.50). Data were analyzed regarding gender, age range (4 to 8, 9 to 12, 13 to 18 and >18 years), cleft type (postforamen or transforamen), diagnosis of velopharyngeal function (adequate, borderline, and inadequate), and velopharyngeal closure patterns (coronal, circular, circular with Passavant's ridge, and sagittal). Results were statistically analyzed. RESULTS: In the study sample, 59.55% of the patients were women; 39.33% were above 18 years of age; 44.94% had post-foramen cleft and 55.06%, transforamen cleft; 14.61% showed adequate, 53.93% borderline, and 31.46% inadequate velopharyngeal closure. With regards to the velopharyngeal closure pattern, 37.08% of the subjects presented coronal closure; 33.71%, circular closure, 15.73% circular with Passavant's ridge; and 13.48%, sagittal closure pattern. There was a relation between pattern of closure and diagnosis of velopharyngeal function (p=0.05). CONCLUSION: Most of the sample presented coronal pattern of velopharyngeal closure, followed by circular, circular with Passavant's ridge and sagittal patterns. There was no relationship between closure pattern and the variables gender, age, and cleft type, but there was a related influence to the diagnosis of velopharyngeal function.
