ABSTRACT Introduction: The strengths and potentialities of the Cuban family medicine model have had an indisputable impact on our population; facing today, the challenge of renewing itself and at the same time maintaining and strengthening its foundational ideas. Objectives: To identify the foundational ideas of the Cuban Family Medicine Model and to assess its current situation. Methods: A qualitative research was carried out. An initial report was elaborated by a group of founders and specialists of the Department of Primary Health Care of the Ministry of Public Health, on the foundational ideas of the Family Medicine Program in Cuba. It was discussed in a focal group of experts, in three working sessions, providing criteria on the current situation of these ideas. Results: The main foundational ideas of the program were identified. Conclusions: The Cuban Family Medicine model responds essentially to the initial conceptualization of the Family Doctor and Nurse Practitioner Program. Some of its foundational ideas need revitalization, others need improvement or renovation. There are situations that can be addressed immediately, others that need reflection and contextualization to the current situation and to be carried out over a longer period of time. However, the quality of care and above all the satisfaction of the population demand action.

RESUMEN Introducción: Las fortalezas y potencialidades del programa de medicina familiar cubano han tenido un indiscutible impacto en nuestra población; se enfrenta hoy al desafío de renovarse y al mismo tiempo mantener y fortalecer sus ideas fundacionales. Objetivos: Identificar las ideas fundacionales del modelo de medicina familiar cubano y valorar su situación actual. Métodos: Se realizó una investigación cualitativa. Se elaboró informe inicial, por grupo de fundadores y de especialistas del Departamento de Atención Primaria de Salud del Ministerio de Salud Pública, sobre ideas fundacionales del programa de medicina familiar de Cuba. Se discutió en grupo focal de expertos, en tres sesiones de trabajo, y se aportaron criterios sobre situación actual de estas ideas. Resultados: Se identificaron las principales ideas fundacionales del programa. Conclusiones: El programa de medicina familiar cubano responde esencialmente a la conceptualización inicial del Programa del Médico y Enfermera de la Familia. Algunas de sus ideas fundacionales necesitan de una revitalización, otras de un perfeccionamiento o renovación. Hay situaciones que pueden ser abordadas de inmediato, otras que necesitan de reflexión y contextualización en la situación actual y llevarse a cabo en un mayor plazo. No obstante, la calidad de la atención y sobre todo la satisfacción de la población reclaman actuar.

ABSTRACT Introduction: Since the beginning of the sixties of the last century, the Mother and Child Care Program has been a priority of the Ministry of Public Health in Cuba. In order to improve the training of its managers, a training system was organized through different training modalities. Objectives: To characterize managers of the Maternal and Child Care Program who participated in different modalities provided in the 2013-2020 stage. Methods: A descriptive and retrospective study was carried out, as well as a documentary review of the contents of the academic modalities taught. A questionnaire was applied to the 181 students who participated in the 2013-2020 courses that explored personal data, preparation for the position, usefulness and importance of the topics contained in courses received and learning needs. Results: The managers surveyed were aged 45 years or older. Most of them were women, family physicians and held some teaching category. Not all were previously prepared for the position. They considered the time spent in the course received to be beneficial. Conclusions: Managers of the Maternal and Child Care Program in this research are mostly women, physicians. They have little management experience, not all of them were previously trained and considered the contents of the courses given to be useful and interesting. However, they suggested the need to include other related topics.

RESUMEN Introducción: El Programa de Atención Materno Infantil ha sido una prioridad del Ministerio de Salud Pública de Cuba desde inicios de la década del sesenta del pasado siglo. Con el propósito de elevar la preparación de sus directivos se organizó un sistema de preparación a través de diversas modalidades de capacitación. Objetivos: Caracterizar a directivos del Programa de Atención Materno Infantil participantes en las diferentes modalidades impartidas en la etapa 2013-2020. Métodos: Se realizó un estudio descriptivo y retrospectivo así como revisión documental de los contenidos en las modalidades académicas impartidas. Se aplicó un cuestionario a los 181 estudiantes que participaron en los cursos 2013-2020 que exploró datos personales, preparación para el cargo, utilidad e importancia de los temas contenidos en los cursos recibidos y necesidades de aprendizaje. Resultados: Los directivos encuestados tuvieron una edad de 45 años o más. La mayoría fueron mujeres, médicos de familia y ostentaron alguna categoría docente. No todos fueron preparados previamente para el cargo. Consideraron provechoso el tiempo empleado en el curso recibido. Conclusiones: Los directivos del Programa de Atención Materno Infantil en esta investigación en su mayoría son mujeres, médicos. Tienen poca experiencia de dirección, no todos fueron previamente capacitados y consideraron útil e interesante los contenidos de los cursos impartidos. No obstante, plantearon necesidad de incluir otras temáticas vinculadas.

ABSTRACT Introduction: The National School of Public Health in its condition of Branch School for the preparation of cadres and reserves of the health sector was considered suitable to teach the Diploma in Public Administration, which is carried out according to the policies of the Communist Party of Cuba, of the State and Government. In the 23 editions carried out in the period between February 2012 and March 2020, 495 managers graduated, 20 of them in the last edition. Objective: To characterize the participants of the edition of the Diploma in Public Administration developed at the National School of Public Health in 2020. Methods: A descriptive and retrospective study was conducted. The universe was constituted by 20 managers who attended the edition of the Diploma in Public Administration at the National School of Public Health in 2020. Results: The highest percentage was female, between 41 and 50 years of age, working in the Ministry of Public Health. They had between 1 and 5 years of permanence in the position and the highest percentage corresponded to cadres. Conclusions: Most of the students were cadres, under 50 years of age, women and expressed satisfaction with the knowledge acquired in the different courses; they are useful for daily work and therefore perceive to be better prepared to exercise the functions of control, planning and execution of the assigned tasks.

RESUMEN Introducción: La Escuela Nacional de Salud Pública en su condición de Escuela Ramal para la preparación de los cuadros y reservas del sector salud se consideró apta para impartir el diplomado de Administración Pública, el que se realiza acorde a las políticas del Partido Comunista de Cuba, del Estado y Gobierno. En las 23 Ediciones realizadas en el período comprendido entre febrero de 2012 y marzo de 2020 se graduaron 495 directivos, 20 de ellos en la última edición. Objetivo: Caracterizar a los participantes de la edición del Diplomado en Administración Pública desarrollada en la Escuela Nacional de Salud Pública en el 2020. Métodos: Se realizó un estudio descriptivo y retrospectivo. El universo estuvo constituido por 20 directivos que cursaron la edición del Diplomado de Administración Pública en la Escuela Nacional de Salud Pública en el año 2020. Resultados: El mayor porciento fue del sexo femenino, con edades entre los 41 y 50 años que laboran en el Ministerio de Salud Pública. Tuvieron entre 1 y 5 años de permanencia en el cargo y el porciento más elevado correspondió a los cuadros. Conclusiones: La mayoría de los estudiantes fueron cuadros, de menos de 50 años, mujeres y expresaron estar satisfechos con los conocimientos adquiridos en los diferentes cursos; son útiles para el quehacer diario y por consiguiente perciben estar mejor preparados para ejercer las funciones de control, planificación y ejecución de las tareas asignadas.

ABSTRACT Introduction: The adolescence is a period of development and growth during which the persons establish its sense of individual identity, adaptation to more mature intellectual capacities, to the requirements of the society as for the conduct, internalization of a system of personal values and the preparation for adults' rolls. In this period there happen biological, psychological and social transformations that need of specializing attention. The results of the scientific investigation that are translated in scientific production can contribute to the improving of the integral attention to and the adolescents. Objective: To characterize the behavior of the scientific production related to the adolescence, bibliométricos realized a descriptive study, defining the scientific productions and investigations that tackle the main problems of health of and the Cuban adolescents. Methods: For the study there took the universe of the scientific production of the Cuban Magazine of Public health, Cuban Magazine of Paediatrics and Cuban Magazine of Familiar Medicine in the period 2011-2015. Results: The present investigation allowed to identify of harmonic form the mail between the main problems of health of and the adolescents and the subject-matters tackled in the chosen magazines. Conclusions: The generalization of the scientific communication favors the solution of the main problems of health identified in and the Cuban adolescents.

RESUMEN Introducción: La adolescencia es un periodo de desarrollo y crecimiento durante el cual las personas establecen su sentido de identidad individual, adaptación a capacidades intelectuales más maduras, a las exigencias de la sociedad en lo que respecta a la conducta, internalización de un sistema de valores personales y la preparación para roles de adultos. En este periodo acontecen transformaciones biológicas, psicológicas y sociales que requieren de atención especializada. Los resultados de la investigación científica que se traducen en producción científica pueden contribuir al perfeccionamiento de la atención integral a los adolescentes. Objetivo: Caracterizar el comportamiento de la producción científica relacionada con la adolescencia. Métodos: Se realizó un estudio descriptivo bibliométrico, se definieron las producciones científicas e investigaciones que abordan los principales problemas de salud de los adolescentes cubanos. Para el estudio se tomó el universo de la producción científica de la Revista Cubana de Salud Pública, Revista Cubana de Pediatría y Revista Cubana de Medicina Familiar en el periodo 2011-2015. Resultados: Se identificaron de forma armónica la correspondencia entre los principales problemas de salud de los adolescentes y las temáticas abordadas en las revistas seleccionadas. Conclusiones: La generalización de la comunicación científica favorece la solución de los principales problemas de salud identificados en los adolescentes cubanos.

The first 1000 days of life constitute a short and exceptionally important period when the foundation is established for children’s growth, development and lifelong health. Cuba has a comprehensive care system for this population that aims to promote the best start in life so that children can reach their highest development potential. This is carried out through the national public health and education systems and also includes elements of health protection, prevention of harm and disease and social welfare for children. Cuba’s infant mortality rate has remained <5 deaths per 1000 live births for 10 consecutive years, and in 2017 reached 4 per 1000, the lowest rate to date. The mortality rate for children aged <5 years in 2017 was 5.5 per 1000 live births, with a survival rate of 99.5%; low birth weight was 5.1% and vaccination coverage >95%. Among children aged 1 year in Cuba’s Educate Your Child program in 2014, >90% met age-specific indicators in all four developmental domains (intellectual, motor, socioaffective and language). Cuba has universal coverage for antenatal care and, in 2017, 99.9% of births occurred in health institutions. All working mothers receive paid antenatal leave from 34 weeks of gestation, continued through the child’s first year, to facilitate breastfeeding and child care. In 2018, the Cuban government allocated 27% of its national budget to health and social welfare and 21% to education.

ABSTRACT In Cuba, maternal and child health care is based on the priority granted by the State, the implementation of the National Maternal and Child Health Program and the guarantee of equitable access to health services. This article describes the Cuban experience in this field, as well as its main achievements, challenges, and lessons learned. Among the most relevant results up to 2015 are the reduction of infant mortality rate and under-five mortality rate to 4.3 and 5.7 deaths per 1 000 live births, respectively; 5-year survival of 99.4%; more than 10 prenatal check-ups per delivery; 5.3% of low birth weight; 99.9% of institutional births; and being the first country to validate the elimination of mother-to-child transmission of HIV/AIDS and congenital syphilis. The main challenges are to increase the rate of exclusive breastfeeding; to reduce anemia due to iron deficiency in children and pregnant women; to reduce overweight in children; to prevent accidents; and to reduce maternal mortality, adolescent fertility rate, and voluntary abortion. Among the lessons learned are the priority given by the State to health, the programmatic management of maternal and child care, the guarantee of universal coverage, the systematic collection of information for decision-making, the integration of sectors and social participation in health. Sustaining and improving the results achieved will contribute to the fulfillment of the Sustainable Development Agenda for 2030.

RESUMEN En Cuba, la atención a la salud maternoinfantil se sustenta en la prioridad que le otorga el Estado, la ejecución del Programa Nacional de Atención Materno Infantil y la garantía de acceso equitativo a los servicios de salud. Este artículo describe la experiencia cubana en este campo, así como sus principales logros, desafíos y lecciones aprendidas. Entre los resultados más relevantes hasta el 2015 se encuentran la reducción de la mortalidad infantil y del menor de 5 años a 4,3 y 5,7 fallecidos por 1 000 nacidos vivos, respectivamente; supervivencia a los 5 años de 99,4%; más de 10 controles prenatales por parto; 5,3% de peso bajo al nacer; 99,9% de partos institucionales; y ser el primer país en validar la eliminación de la transmisión vertical del VIH y la sífilis congénita. Los principales desafíos son aumentar la tasa de lactancia materna exclusiva; reducir la anemia por déficit de hierro en niños y gestantes y el sobrepeso infantil; prevenir los accidentes; y reducir la mortalidad materna, la tasa de fecundidad en las adolescentes y el aborto voluntario. Entre las lecciones aprendidas se destacan la prioridad que el Estado otorga a la salud, la conducción programática de la atención maternoinfantil, la garantía de cobertura universal, la recolección sistemática de información para la toma de decisiones, la integración de los sectores y la participación social en la salud. Sostener y mejorar los resultados alcanzados contribuirá al cumplimiento de la Agenda de Desarrollo Sostenible para el 2030.

RESUMO Em Cuba, os cuidados de saúde materno-infantil baseiam-se na prioridade concedida pelo Estado, na implementação do Programa Nacional de Saúde Materno-Infantil e na garantia de acesso equitativo aos serviços de saúde. Este artigo descreve a experiência cubana neste campo, bem como as principais conquistas, desafios e lições aprendidas. Entre os resultados mais relevantes até 2015 estão a redução da mortalidade infantil e mortalidade em menores de 5 anos para 4,3 e 5,7 mortes por 1 000 nascidos vivos, respectivamente; sobrevivência a 5 anos de 99,4%; mais de 10 exames pré-natal por nascimento; 5,3% do baixo peso ao nascer; 99,9% dos partos institucionais; e seja o primeiro país a validar a eliminação da transmissão materno-infantil de HIV/AIDS e sífilis congênita. Os principais desafios são aumentar a taxa de aleitamento materno exclusivo; reduzir a anemia devido a deficiência de ferro em crianças e mulheres grávidas e reduzir o excesso de peso infantil; prevenir acidentes; e reduzir a mortalidade materna, taxa de fertilidade adolescente e aborto voluntário. Entre as lições aprendidas estão a prioridade dada pelo Estado à saúde, a gestão programática dos cuidados materno e infantil, a garantia de cobertura universal, a coleta sistemática de informações para a tomada de decisões, a integração de setores e a participação social em saúde. Sustentar e melhorar os resultados obtidos contribuirá para o cumprimento da Agenda de Desenvolvimento Sustentável para 2030.

This paper was based on statistical data from the existing official statistical information systems. The rate of women in the Cuban population in 2007 was 49.93 %; aging was present since the 60 years and over age group, which represented 9.40% in 1995, rose to 17.11% in 2007. Birth and fecundity rates kept their rising tendency amounting to 10 x 1 000 pop. and 37.3 x 1 000 women aged 15-49 years respectively in 2007. There was a sustained drop in induced abortion rates in the 12-49 years group, from 45.6 x 1 000 women in 1990 to 20.4 x 1 000 women in 2007 as well as high contraception coverage of 77 %. The five first causes of mortality continued to be heart diseases, malignant tumors, cerebrovascular diseases, influenza and pneumonia, and accidents. The incidence of malignant tumors was on the rise and its more frequent location was the breast. Over 80 % of uterine and cervix cancer cases was diagnosed in staging 0, but the number of women diagnosed with this type of cancer in more advanced staging increased. The women accounted for 19.5 % of seropositive cases (HIV/AIDS). The incidence rate of blenorrhagia was higher than that of syphilis (31.8 and 12.7 per 100 000 women respectively) in 2007, being the youngest the most affected. The data provided here allows an approach to the health status of the Cuban women in the last 50 years.

Este trabajo está basado en datos estadísticos provenientes de sistemas de información estadísticos oficiales existentes. La proporción de mujeres en la población en el año 2007 era de 49,93 % y existe un envejecimiento puesto que el grupo de 60 años y más que representaba el 9,40 % en 1995 ascendió a 17,11 % en 2007. La natalidad y fecundidad mantuvieron una tendencia descendente y para el 2007 era de 10,0 x 1 000 habitantes y 37,3 x 1 000 mujeres de la edad entre 15-49 años, respectivamente. Hubo un descenso mantenido de las tasas de abortos inducidos en mujeres de 12-49 años desde 1990 que era de 45,6 a 20,4 x 1 000 mujeres en el 2007, así como una elevada cobertura anticoncepcional de 77 %. Las cinco primeras causas de mortalidad se mantienen: enfermedades del corazón, tumores malignos, enfermedades cerebrovasculares, influenza y neumonía y accidentes. La incidencia de tumores malignos tiene un comportamiento ascendente, su localización más frecuente es la mama. Más de 80 % de los casos de cáncer cérvico uterino se diagnosticaron en etapa 0, pero hubo un incremento de mujeres diagnosticadas en etapas más avanzadas. Las mujeres representaban el 19,5 % de casos seropositivos (VIH/SIDA). La incidencia de blenorragia era mayor que la de sífilis con tasas respectivas de 31,8 y 12,7 por 100 000 mujeres en el año 2007, siendo los jóvenes los más afectados. Los datos expuestos permiten una aproximación al estado de salud de la mujer cubana en los últimos 50 años.
