Resumo Objetivo Avaliar aspectos emocionais e clínicos observados em mulheres com doença trofoblástica gestacional (DTG) acompanhadas em um centro de referência (CR), por equipe multiprofissional. Método Estudo de coorte retrospectivo nos prontuários clínicos de 186 mulheres comDTG, e dos aspectos emocionais (AE) observados nessas mulheres pela equipe de psicólogas e registrados nos 389 grupos de apoio (GAs), ocorridos de 2014 a 2018. Resultados As pacientes eram jovens (idade média 31,2 anos), 47% sem filhos vivos, 60% tinham desejado ou planejado esta gravidez e 50% delas participaram de dois ou mais GAs. A maioria (n=137-73,6%) apresentou remissão espontânea da gestação molar com mediana de 10 semanas e um tempo total de seguimento de 7 meses. Quarenta e nove mulheres (26,3%) evoluíram para neoplasia trofoblástica gestacional (NTG); amediana para atingir a remissão após tratamento comquimioterapia foi de 19 semanas e o tempo total de seguimento foi de 36 meses. Os AE incluíram variados graus de ansiedade e depressão, mais evidentes em 9,1% das nossas pacientes; tais AE tenderam a ocorrer mais em mulheres com idade acima de 40 anos (p=0,067), com menor escolaridade (p=0,054), com evolução para NTG (p=0,018), e nas que necessitaram de tratamento quimioterápico com regime de múltiplos agentes (p=0,028), ou de histerectomia complementar ao tratamento clínico (p=0,001). Conclusão Este estudo mostrou presença de vários AE associados em todos os tipos de DTG. Destaca tambéma importância de umatendimento psicológico especializado, somente encontrado nos CR, que é essencial para ajudar na recuperação da saúde mental dessas mulheres.

Abstract Objective To evaluate the emotional and clinical aspects observed in women with gestational trophoblastic disease (GTD) followed-up in a reference center (RC) by a multidisciplinary team. Methods Retrospective cohort study of the clinical records of 186 women with GTD and of the emotional aspects (EA) observed in these women by a teamof psychologists and reported by the 389 support groups conducted from 2014 to 2018. Results The women were young (mean age: 31.2 years), 47% had no living child, 60% had planned the pregnancy, and 50% participated in two or more SG. Most women (n=137; 73.6%) reached spontaneous remission ofmolar gestation in a median time of 10 weeks and had a total follow-up time of seven months. In the group of 49 women (26.3%) who progressed to gestational trophoblastic neoplasia (GTN), time to remission after chemotherapy was 18 weeks, and total follow-up time was 36 months. EA included different levels of anxiety and depression,more evident in 9.1% of the women; these symptoms tended to occur more frequently in women older than 40 years (p=0.067), less educated (p=0.054), and whose disease progressed to GTN (p=0.018), as well as in those who had to undergo multi-agent chemotherapy (p=0.028) or hysterectomy (p=0.001) adjuvant to clinical treatment. Conclusion This study found several EA in association with all types of GTD. It also highlights the importance of specialized care only found in a RC, essential to support the recovery of the mental health of these women.

Resumo O envelhecimento da população aponta à necessidade de novas formas de cuidado às demandas que começam a surgir. Em idosos, a depressão se apresenta a partir do isolamento, sentimento de desesperança, irritabilidade e baixa tolerância à frustração. A partir disso, este estudo teve como objetivo avaliar um protocolo de psicoterapia para depressão geriátrica desenvolvido a partir da integração da Terapia de Aceitação e Compromisso com outros modelos teóricos. Trata-se de um ensaio clínico randomizado-controlado, paralelo, aberto, com um braço, sem grupo controle. Contou-se com nove idosas, submetidas a 12 sessões. Foram mensurados os níveis de depressão geriátrica, positividade, satisfação com a vida e ansiedade. As testagens ocorreram na primeira sessão, na sexta, na última e três meses após o fim da intervenção e foram analisadas através do teste de Wilcoxon. Os resultados apontam diminuição nos níveis de depressão (T = 0,09) e ansiedade (T = 0,04) e aumento da Positividade (T = 0,09) e da satisfação com a vida (T = 0,09). Concluiu-se que o protocolo Kairós apresenta evidências de eficácia para tratamento da depressão geriátrica, apesar da não generalização dos dados. Faz-se necessária a realização de novos estudos com populações maiores.

Abstract The aging of the population leads us to think about the need for new forms of care to the demands that start to emerge. Geriatric. In the elderly, depression comes from isolation, feelings of hopelessness, irritability and low tolerance for frustration. From that, this study aimed to evaluate a psychotherapy protocol for geriatric depression developed from the integration of Acceptance and Commitment Therapy with other models. It is a randomized, open, controlled, parallel, one-arm clinical trial with no control group. A total population of nine elderly women who underwent 12 sessions. To assess its effectiveness, levels of geriatric depression, positivity, life satisfaction and anxiety. The tests occurred at four moments: the first session, the sixth, the last, and three months after the end of the intervention and were analyzed using the Wilcoxon test. The results show a decrease in depression (T = 0.09) and anxiety (T = 0.04) and an increase in Positivity (T = 0.09) and satisfaction with life (T = 0.09). It was concluded that the Kairós protocol presents evidence of efficacy, despite the non-generalization of the data. Further studies with larger populations are required.

Resumo Objetivo Comparar o efeito de alta dose de vitamina A (VitA) no seguimento pósmolar de pacientes com gonadotrofina coriônica humana (hCG) sérica apresentando valoresbaixoseem platô(L&P). Métodos Estudo retrospectivo de série de casos comparando duas coortes não simultâneas. Grupo controle (CG): 34 pacientes com títulos de hCG sérico L&P submetidos a manejo expectante por 6 meses após o esvaziamento uterino, de 1992 a 2010; Grupo de Estudo (SG): de 2011 a 2017, 32 pacientes em condições semelhantes de hCG receberam Vit-A na dose de 200.000 IU por dia, do momento da identificação dohCG em platôate o primeirohCG normaloudiagnóstico de progressão para neoplasia trofoblástica gestacional (NTG). O presente estudo foi aprovado pelo Comitê de Ética da Instituição na qual foi desenvolvido. Resultados Em ambososgrupos, aprevalência de hCGL&P foi < 5%. No SG, os níveis de hCGemplatô forammaiores (CG = 85.5 versus SG = 195 IU/L; p = 0,028), e foram significantemente menores tanto a prevalência de NTG pós-molar (CG = 29.4% versus SG = 6.3%, p = 0,034) como o tempo de seguimento (CG = 14 versus SG = 10 meses, p < 0.001). Na evolução para NTG não houve diferença no estadiamento da Interna tional Federation of Gynecology and Obstetrics (FIGO, na sigla em inglês) ou na agressividade do tratamento. Com altas doses de Vit-A não houve qualquer efeito tóxico relevante. Não houve casos de recidiva de NTG ou de óbito. Conclusão O uso limitado de altas doses de Vit-A parace ser seguro e apresenta efeitos significativos na evolução de pacientes em controle pós-molar com títulos de hCG sérico L&P, e pode diminuir o desenvolvimento de NTG pós-molar nessa população.

Abstract Objective To compare the effect of high-dose vitamin A (HD Vit-A) use during postmolar follow-up of patients with low and plateauing (L&P) serum human chorionic gonadotropin (hCG) levels, from the moment serum hCG plateaued (P-hCG) to the first normal serum hCG value (< 5IU/L). Methods The present retrospective series case study compared two nonconcurrent cohorts of patients. Control group (CG): 34 patients with L&P serum hCG levels who underwent expectant management for 6 months after uterine evacuation, from 1992 to 2010; study group (SG): 32 patients in similar conditions who received 200,000 IU of Vit-A daily, from the identification of a P-hCG level to the first normal hCG value or the diagnosis of progression to gestational trophoblastic neoplasia (GTN), from 2011 to 2017. The present study was approved by the Ethics Committee of the institution where it was conducted. Results In both groups, the prevalence of persistent L&P serum hCG levels was < 5%. In the SG, hCG levels at plateau were higher (CG = 85.5 versus SG = 195 IU/L; p = 0.028), the rate of postmolar GTN was lower (CG = 29.4% versus SG = 6.3%, p = 0.034) and follow-up was shorter (CG = 14 versus SG = 10 months, p < 0.001). During GTN follow-up, there were no differences in GTN staging or treatment aggressiveness in both groups. High-dose Vit-A use did not have any relevant toxic effect. There were no GTN relapses or deaths. Conclusion The limited use of HD Vit-A seems to have a safe and significant effect on the treatment of postmolar patients with L&P serum hCG levels and may decrease the development of postmolar GTN in this population.

RESUMO Racional: O diagnóstico tecidual é mandatório nas lesões subepiteliais gástricas da camada muscular própria para o diagnóstico diferencial das neoplasias do estroma gastrointestinal (GISTs). Contudo, ainda não há consenso quanto a melhor agulha para a punção ecoguiada destas lesões. Objetivo: Avaliar o valor da punção aspirativa ecoguiada com agulhas calibrosas de 19 gauge para o diagnóstico diferencial das lesões subepiteliais gástricas da camada muscular própria. Métodos: Foram revisados retrospectivamente os registros de pacientes consecutivos submetidos à punção aspirativa ecoguiada com agulhas de 19 e 22 gauge de lesões subepiteliais gástricas da quarta camada ecográfica em um centro de referência. A punção aspirativa foi realizada sempre pelo mesmo endoscopista, com o emprego da técnica de fanning, sem presença de citopatologista em sala. O material aspirado foi avaliado apenas pela técnica de cell block pelo mesmo patologista. Os resultados foram considerados diagnósticos, na presença de material adequado para coloração pela H&E e imunoistoquímica, ou não-diagnósticos. Resultados: Oitenta e nove pacientes (idade média: 59 anos, 77% do sexo feminino) foram submetidos a 92 punções aspirativas ecoguiadas com agulhas de 19 (75) ou 22 (17) gauges. O tamanho médio das lesões foi de 22,6 mm. O ganho diagnóstico geral foi de 88%. O ganho diagnóstico para as agulhas de 19 gauge foi superior ao das agulhas de 22 gauge (92%x70,6%; p=0,0410), e similar para lesões >2 cm e <2 cm (93,7%x90,7%; p=0,9563). Os melhores resultados com a agulha de 19 gauge foram obtidos com a realização de até três punções. A taxa de complicações foi de 2,8%. Conclusão: O ganho diagnóstico da punção aspirativa ecoguiada de lesões subepiteliais gástricas da camada muscular própria com agulhas de 19 gauge é de 92%. A punção com a agulha mais calibrosa para lesões de qualquer tamanho é procedimento seguro e de grande valor no diagnóstico diferencial dos GISTs e leiomiomas.

ABSTRACT Background: Tissue diagnosis is required for gastric subepithelial lesions for differential diagnosis of GISTs. However, there has not been consensus about the best needle for EUS-guided sampling of these lesions. Aim: To evaluate the diagnostic yield of EUS-FNA for gastric subepithelial lesions of the proper muscle layer with large-bore 19 gauge needles. Methods: A prospectively maintained database was retrospectively reviewed to identify consecutive patients who underwent EUS-FNA with 19 and 22 gauge needles for gastric subepithelial lesions of the fourth endosonographic layer in a tertiary care referral center. EUS-FNA was performed by the same endosonographer, using the fanning technique, without on-site cytopathologist. Specimens were analysed through cell blocks by the same pathologist. Procedure results were categorized into diagnostic, defined as enough material for histopathology and immunohistochemistry, or nondiagnostic. Results: Eighty-nine patients (mean age: 59 years, 77% women) underwent 92 EUS-FNA with 19 (75) or 22 (17) gauge needles. Mean lesion size was 22.6 mm. Overall diagnostic yield was 88%. The diagnostic yield of 19 gauge was higher than that of 22 gauge needle (92%x70.6%; p=0.0410), and similar for lesions >2 cm and <2 cm (93.7%x90.7%; p=0.9563). The best performance for 19 gauge needles was obtained performing <3 needle passes. Complication rate was 2.8%. Conclusions: Diagnostic yield of EUS-FNA with 19 gauge needles is 92% for gastric subepithelial lesions of the proper muscle layer. It is safe and highly valuable for differentiation between GIST and leiomyoma, no matter the size of the lesion.

Abstract: The purpose of this manuscript was to re-discuss apical periodontitis, apical biofilm, and its possible relationship with dendritic cells (DC). DCs are potent regulators of the immune system and their function is divided into three categories that involve the presentation of antigens: the presentation of antigens and activation of T cells; a not well established category suggested that DCs induce and maintain immunological tolerance; and the maintenance of the immune memory in conjunction with B cells. DCs in periapical inflammatory lesions are composed of at least two subpopulations that can be distinguished on the basis of ultrastructure and phenotype. These populations might differ in lineage, state of maturation, differentiation, activation, and/or function. The authors hereby analyzed the root apexes of teeth under SEM, after performing apicoectomy due to the failure of conventional endodontic treatment. Microbial biofilm with multispecies and areas of resorption with the presence of Howship lacunae, and images suggestive of denditric cells could be observed. The presence of DCs in periapical lesion could be an indication of the severity of the lesion, with a constant presence of antigen in the periradicular region.

Descrevemos neste artigo a construção de um aparato experimental indicado para o ensino de gráficos da Cinemática. O aparato utiliza o software PLX-DAQ e um sensor sonar de ultrassom acoplado a uma plataforma microcontrolada Arduino para realizar leituras de posição e distância de objetos. O instrumento foi desenvolvido durante a realização de um mestrado profissional, com o objetivo de contribuir para a superação de dificuldades de aprendizagem enfrentadas por estudantes da Educação Básica na construção e interpretação de gráficos de posição versus tempo no sistema cartesiano de coordenadas. Na primeira parte do artigo, apresentamos uma descrição detalhada da construção do aparato experimental e, na sequência, um breve relato sobre alguns resultados do seu uso em duas turmas de 1° ano do Ensino Médio, de uma escola estadual do município de Candiota/RS. Durante sua aplicação, pudemos identificar uma evolução no desempenho dos alunos, os quais demonstraram, no início da atividade, alguma dificuldade na análise e interpretação de gráficos de movimento. No entanto, no decorrer das tarefas, eles foram capazes de interpretar e reproduzir gráficos de movimento, em tempo real, a partir do aparato experimental utilizado.

This paper presents an educational tool developed using the Arduino platform with a position sensor and the PLX-DAQ software, which was used in developing a learning experiment suitable for teaching graphics kinematics that was applied to the master's work. The aim was to overcome learning difficulties faced by students of basic education in the construction and interpretation of graphs, like position versus time in the Cartesian coordinate system. We present a detailed description of instrumentation and some results of an implementation carried out with two groups of 1st year of high school, a State school in the city of Candiota / RS. In the implementation, we identified an increase in student performance, which demonstrated certain difficulties in the analysis and interpretation of motion graphics at the beginning of the activity; however, they developed skills in the course of tasks, achieving both qualitative and quantitative results (statistically significant). The students were able to interpret and reproduce motion graphics in real time from the instrumentation proposed in this work.

RACIONAL: O câncer de esôfago é um dos mais comuns e agressivos que existem. Recentemente o AJCC mudou o sistema de estadiamento, considerando, entre outros fatores, a importância da metástase linfonodal sobre o prognóstico. OBJETIVO: Discutir a aplicabilidade de diferentes formas de estadiamento linfonodal em um serviço de cirurgia ocidental. MÉTODOS: Quatrocentos e oitenta e sete pacientes com câncer de esôfago foram arrolados. Três estadiamentos foram avaliados, a 6a e a 7a Edição do AJCC e o Índice de Metástase Linfonodal. RESULTADOS: A maioria foi casos foi composta por carcinoma epidemóide. A média de amostra linfonodal foi de oito. Considerando a sobrevida, não houve diferença significativa entre os pacientes quando foram classificados pela 7a Edição do AJCC. Analisando o Índice de Metástase linfonodal, apenas o grupo de pacientes com 0-25% mostrou diferença significativa (p=0,01). A 6a Edição do AJCC mostrou a maior diferença entre as classificações avaliadas. CONCLUSÃO: Considerando as características da amostra, a nova classificação do AJCC não foi capaz de evidenciar diferenças significativas na sobrevida dos pacientes, quando apenas o estadiamento linfonodal foi avaliado.

BACKGROUND: The esophageal cancer is one of the most common and aggressive worldwide. Recently, the AJCC changed the staging system, considering, among others, the important role of the lymph node metastasis on the prognosis. AIM: To discuss the applicability of different forms of lymph node staging in a western surgical center. METHODS: Four hundred eighty seven patients with esophageal cancer were enrolled. Three staging systems were evaluated, the 6th and the 7th AJCC editions and the Lymph Node Metastatic Ratio. RESULTS: The majority of the cases were squamous cell carcinoma. The mean lymph node sample was eight. Considering the survival, there was no significant difference between the patients when they were classified by the 7th AJCC edition. Analysis of the Lymph Node Metastatic Ratio, just on the group of patients with 0 to 25%, has shown significant difference (p=0,01). The 6th AJCC edition shows the major significant difference between among the classifications evaluated. CONCLUSION: In this specific population, the 7th AJCC edition for esophageal cancer was not able to find differences in survival when just the lymph node analysis was considered.

OBJETIVO: Em mulheres brasileiras com neoplasia trofoblástica gestacional (NTG) de baixo-risco, de acordo com a Federação Internacional de Ginecologia e Obstetrícia (FIGO) 2002, comparar dois regimes de quimioterapia (Qt) por agente único avaliando resposta e efeitos colaterais no tratamento de primeira linha, e a eficácia no tratamento final por agente único de Qt. MÉTODOS: Análise retrospectiva de duas coortes concorrentes com 194 pacientes com NTG de baixo risco: de 1992 a 2012; como primeira linha, 115 pacientes receberam 4 doses intramusculares de metotrexato alternado com 4 doses orais de ácido folínico (MTX/FA) repetidos a cada 14 dias e, desde 1996, 79 pacientes receberam quinzenalmente dose em bolo de actinomicina D (Act-D) por via endovenosa. No momento do diagnóstico da NTG, a opinião da paciente foi levada em consideração para definir o regime de Qt por agente único inicial e, quando havia resistência ou toxicidade a um regime, o outro fármaco era usado preferentemente. Este estudo foi aprovado pelo Comitê de Ética da Irmandade da Santa Casa de Misericórdia de Porto Alegre. RESULTADOS: Ambos os grupos eram clinicamente semelhantes (p>0,05). Nos tratamentos de primeira linha, a frequência de resposta completa foi semelhante (75,7% com MTX/FA e 67,1% com Act-D em bolo); não houve diferença entre os grupos quanto ao número de séries de Qt - mediana 3 (intervalo: 1-10) com MTX/FA e 2 (intervalo: 1-6) com Act-D em bolo - e ao tempo para remissão - mediana 9 semanas (intervalo: 2-16) com MTX/FA e 10 semanas (intervalo: 2-16) com Act-D em bolo. Em ambos os grupos, foi elevada a frequência de efeitos colaterais no tratamento de primeira linha, mas com intensidade baixa; estomatite foi mais frequente com MTX/FA (p<0.01) e náuseas e vômitos com Act-D (p<0.01). A resposta final à Qt por agente único foi alta nos dois grupos (94,8% com MTX/FA e 83,5% com Act-D em bolo; p<0,01) e 13% maior no grupo inicialmente tratado com MTX/FA. As frequências de histerectomia e de recorrência da NTG foram baixas e semelhantes. Nenhuma paciente morreu devido à NTG. CONCLUSÃO: Os dois regimes tiveram resultados semelhantes no tratamento de primeira linha. A resposta final ao tratamento por agente único foi alta e semelhante, mas a taxa final de remissão foi maior no grupo iniciado por MTX/FA.

PURPOSE: To compare two single-agent chemotherapy (ChT) regimens evaluating, in first-line treatment, response and side effects and, in final single-agent treatment, the outcomes, among Brazilian patients with low-risk gestational trophoblastic neoplasia (GTN), according to International Federation of Gynecology and Obstetrics (FIGO) 2002. METHODS: Retrospective analysis of two concurrent cohorts with 194 low-risk GTN patients: from 1992 to 2012, as first-line treatment, 115 patients received 4 intramuscular doses of methotrexate alternated with 4 oral doses of folinic acid (MTX/FA) repetead every 14 days and, since 1996, 79 patients received an endovenous bolus-dose of actinomycin D (Act-D), biweekly. At GTN diagnosis, patient opinion was taken into consideration when defining the initial single-agent ChT regimen, and when there was resistance or toxicity to one regimen, the other drug was used preferentially. This study was approved by the Irmandade da Santa Casa de Misericórdia de Porto Alegre Ethical Committee. RESULTS: Both groups were clinically similar (p>0.05). In first-line treatments, frequency of complete response was similar (75.7% with MTX/FA and 67.1% with bolus Act-D); the number of ChT courses -median 3 (range: 1-10) with MTX/FA and 2 (range: 1-6) with bolus Act-D - and the time to remission -median 9 weeks (range: 2-16) with MTX/FA and 10 weeks (range: 2-16) with bolus Act-D) - were not different between the groups. In both groups, first-line side effects frequency were high but intensity was low; stomatitis was higher with MTX/FA (p<0.01) and nausea and vomit with Act-D (p<0.01). Final single-agent ChT responses were high in both groups (94.8% with MTX/FA and 83.5% with bolus Act-D; p<0.01) and 13% higher in the group initially treated with MTX/FA. Rates of hysterectomy and of GTN recurrence were low and similar. No patient died due to GTN. CONCLUSION: The two regimens had similar first-line ChT response. Final single-agent response rates were high and similar in both groups but the final single-agent remission rate was higher in the MTX/FA group.

O tumor epitelial e estromal misto (TESM) representa uma neoplasia renal bifásica descrita recentemente que afeta predominantemente mulheres na perimenopausa. Os autores relatam o caso de um paciente jovem, do sexo masculino, com TESM exibindo positividade para receptores de estrogênio e progesterona. A tomografia computadorizada/ressonância magnética (TC/RM) mostrou lesão expansiva no rim direito. Ao exame macroscópico, identificou-se tumor sólido-cístico medindo 5,7 × 3,5 × 2,4 cm. À microscopia, foi encontrado tumor bifásico constituído por elementos estromais e epiteliais, sem atipias significativas. O componente estromal era composto por células fusiformes, exibindo imunoexpressão positiva para actina, desmina, vimentina e receptores de estrogênio. Os elementos epiteliais mostraram padrão predominantemente tubular e exibiram imunorreação positiva para citoqueratinas. O diagnóstico de TESM foi então estabelecido.

Mixed epithelial and stromal tumor (MEST) represents a recently described biphasic kidney neoplasm, which predominantly affects perimenopausal females. The authors report the case of a young male patient with a MEST exhibiting positivity for estrogen and progesterone receptors. Computed tomography/magnetic resonance imaging (CT/MRI) showed an expansive lesion affecting the right kidney. Grossly, a solid-cystic tumor was identified, which measured 5.7 × 3.5 × 2.4 cm. On microscopic examination, a biphasic tumor constituted by stromal and epithelial elements, without significant atypias, was identified. The stromal element was composed of spindle cells revealing positive immunoexpression for actin, desmin, vimentin, and estrogen receptors. The epithelial component exhibited a predominantly tubular pattern showing positive immunoreaction for cytokeratins. The diagnosis of MEST was then established.

Introdução: O carcinoma de células renais (CCR) é uma doença de comportamento agressivo em todo o mundo. Objetivo: Estudar os fatores prognósticos tradicionais identificados no exame anatomopatológico e sua correlação com a expressão imunoistoquímica dos novos marcadores survivina e B7-H1. Materiais e métodos: Em um hospital de referência de Porto Alegre, foi realizado um estudo transversal de CCR, com pacientes que realizaram nefrectomia total, no período de 2006 a 2009. Foram selecionados aqueles com os tipos histológicos mais comuns: células claras e papilares. Fatores prognósticos tradicionais foram obtidos por meio da revisão de dados patológicos relevantes dos casos, como tamanho, necrose, tipo histológico, grau nuclear e sistema tumor-linfonodo-metástase (TNM). Também se realizou estudo imunoistoquímico (IMQ) da amostra, com o uso dos marcadores survivina e B7-H1. Resultados: Obtivemos 98 casos, com 90% deles do tipo células claras, 73% classificados como T1 e T2, a maioria com grau nuclear de Fuhrman 2 e 3 e cerca de 70% da amostra positivos para necrose. Já no estudo IMQ foi encontrada positividade em 38 casos para o B7-H1 e em 50 para survivina. Ao considerarmos a associação entre os fatores prognósticos tradicionais e a expressão dos marcadores, encontramos associação somente entre o grupo positivo para os marcadores e necrose (p < 0,001). Conclusão: Tal achado vai ao encontro dos dados da literatura que vêm realçando a importância da necrose no prognóstico dos CCR.

Introduction: Renal cell carcinoma (RCC) is an aggressive disease worldwide. Objective: Study traditional prognostic factors associated with pathological reports and the novel markers survivin and B7-H1 by immunohistochemistry. Methods: In a reference hospital of Porto Alegre, Brazil, we conducted a cross-sectional study of RCC in patients who underwent radical nephrectomy between 2006 and 2009. We selected those who were diagnosed with the most common histologic subtypes: clear cell and papillary RCC. We retrospectively reviewed pathological data to determine traditional prognostic factors, like size, presence of coagulative necrosis, Fuhrman grade and tumor-node metastasis (TNM) system. Besides, we performed an immunohistochemistry (IHC) study with survivin and B7-H1. Results: Our sample had 98 cases, 90% of the cases were composed by clear cell histologic subtype, 73% were tumors classified as T1 and T2 in the TNM system, most were Fuhrman nuclear grade 2 or 3, and 70% were positive for necrosis. In relation to the new prognostic markers, we found 50 cases positive to survivin and 38 to B7-H1. In this investigation of traditional prognostic markers and new markers we observed that only necrosis was associated with positive results of biomarkers. < 0.001). Conclusion: This finding confirms previous studies that necrosis is an important factor to consider in the prognosis of RCC.

OBJECTIVES: To identify the most cited articles in general surgery published by Brazilian authors. INTRODUCTION: There are several ways for the international community to recognize the quality of a scientific article. Although controversial, the most widely used and reliable methodology to identify the importance of an article is citation analysis. METHODS: A search using the Institute for Scientific Information citation database (Science Citation Index Expanded) was performed to identify highly cited Brazilian papers published in twenty-six highly cited general surgery journals, selected based on their elevated impact factors, from 1970 to 2009. Further analysis was done on the 65 most-cited papers. RESULTS: We identified 1,713 Brazilian articles, from which nine papers emerged as classics (more than 100 citations received). For the Brazilian contributions, a total increase of about 21-fold was evident between 1970 and 2009. Although several topics were covered, articles covering trauma, oncology and organ transplantation were the most cited. The majority of classic studies were done with international cooperation. CONCLUSIONS: This study identified the most influential Brazilian articles published in internationally renowned general surgery journals.

OBJETIVO: Avaliar os resultados pós-operatórios de um protocolo multimodal de cuidados peri-operatórios sem preparo mecânico de cólon (protocolo ACERTO) em pacientes submetidos a operações colorretais. METODOS: Foram avaliados prospectivamente 53 pacientes (37M e 16F; 57 [18-82] anos) submetidos à diversas operações colorretais com pelo menos uma anastomose divididos em dois grupos. O primeiro grupo (n=25) foi operado entre Janeiro de 2004 e Julho de 2005 com protocolo convencional incluindo preparo mecânico de cólon. O segundo grupo (n=28) foi operado entre agosto de 2005 e junho de 2008, após a implantação do protocolo ACERTO e sem preparo de cólon. Comparou-se estatisticamente a incidência de complicações, a duração da hospitalização e a mortalidade em ambos os grupos. RESULTADOS: Dois (3,8%) pacientes faleceram no pós-operatório, um em cada grupo. Pacientes do grupo ACERTO tiveram jejum pré-operatório abreviado, receberam menos fluido intravenoso e re-alimentaram mais cedo que o grupo convencional (p<0.05). Não houve diferença na morbidade pós-operatória (36% vs. 28,6%; p=0,56) com incidência de fístula anastomótica semelhante (12 vs. 10,7%; p=1,00). O número de complicações por paciente foi menor no grupo ACERTO (p=0.01). O tempo de internação do grupo ACERTO, operado sem preparo de cólon foi abreviado em 4,5 dias (12 [4-43] dias vs 7,5 [3-47] dias, p = 0,04). CONCLUSÃO: As rotinas do protocolo ACERTO são seguras e melhoram resultados em cirurgia colorretal por diminuir gravidade de complicações e o tempo de internação.

OBJECTIVE: To evaluate the outcomes after the implementation of a multimodal protocol (ACERTO protocol) with patients undergoing colorectal operations. METHODS: Fifty-three patients (37 M and 16 F; 57 [18-82] years old) submitted to various colorectal operations were prospectively studied in two different periods of time: from January 2004 through July 2005 (n=25, conventional group) and from August 2005 through June 2008 (n=28; ACERTO group). The patients received either the traditional perioperative management (including mechanical bowel cleansing) or a multidisciplinary protocol of perioperative care (without mechanical bowel cleansing) established by the ACERTO protocol. We looked at morbidity and mortality rates and length of hospital stay for comparisons between the groups. RESULTS: Mortality was 3.8% (2 patients) without difference between groups. Preoperative fasting and postoperative feeding were shortened and intravenous fluids were diminished in patients operated under the ACERTO protocol (p<0.05). Postoperative morbidity (36% vs. 28.6%; p=0.56) and the incidence of anastomotic leak (12 vs. 10.7%; p=1.00) were similar. The number of complications per patient with any complication was lower in the ACERTO group (p=0.01). Changing protocols reduced the length of hospital stay by 4.5 days (12 [4-43] vs. 7.5 [3-47] days, p= 0.04). CONCLUSION: The multidisciplinary routines of the ACERTO protocol are safe and enhanced recovery in colorectal surgery by reducing both hospitalization and the severity of postoperative morbidity.

OBJETIVO: Avaliar o efeito do chá preto sobre a carcinogênese esofágica experimental induzida pela administração oral de dietilnitrosamina (DEN). MÉTODOS: Durante 160 dias foi estudada uma população de 120 camundongos fêmeas, gênero Mus musculus, da cepa CF1, dividida em dois grupos controles e três grupos de tratamento. Os grupos controles receberam água ou chá durante todo o período do estudo. Os três grupos tratados receberam DEN semanalmente, durante três dias consecutivos, e água, chá ou ambos, nos demais dias da semana. Ao completar o período de 160 dias foram efetuadas as eutanásias dos animais e seus esôfagos foram analisados macroscopicamente (a fresco) e, posteriormente, à histopatologia (empregando a técnica da hematoxilina e eosina - HE). RESULTADOS: Na análise comparativa entre os grupos de tratamento, a incidência tumoral (macroscopia) foi significativamente menor naqueles animais que receberam chá preto, além do carcinógeno. No que se refere às alterações histopatológicas, houve maior número de lesões epiteliais de baixo grau nesses mesmos grupos (p < 0,001). CONCLUSÃO: Os animais que receberam chá preto apresentaram menor incidência dos efeitos relacionados à ação do carcinógeno, indicando que, neste modelo, a infusão apresentou efeito quimioprofilático significativo sobre a carcinogênese experimental induzida pela dietilnitrosamina.

PURPOSE: To evaluate the effect of black tea on esophageal carcinogenesis induced by the oral administration of diethylnitrosamine (DEN). METHODS: A population of 120 female mice (Mus musculus, strain CF1) were studied for 160 days. The animals were assigned to two control groups and three treatment groups. The control groups received water or tea throughout the study period, while the three experimental groups received DEN weekly, for three consecutive days, and water, tea, or both, in the other days of the week. On completion of the 160-day period, the animals were killed and their esophagi promptly examined macroscopically and subsequently submitted to histopathology (using the hematoxylin-eosin technique). RESULTS: In the comparative analysis between the treatment groups, tumor incidence (macroscopy) was significantly lower in those animals that received black tea besides the carcinogen. As regards the histopathologic changes, there was a greater number of low grade epithelial lesions in the same groups (p < 0.001). CONCLUSION: The animals that received black tea had a lower incidence of effects related to the carcinogen's action, thus indicating that, in this model, the infusion had a significant chemoprophylactic effect on experimental diethylnitrosamine-induced carcinogenesis.

RACIONAL: O carcinoma de pequenas células do esôfago é rara afecção, caracterizada por uma progressão agressiva, sendo seu aspecto e comportamento semelhantes aquele do pulmão. Detém elevada incidência de doença metastática em sua apresentação inicial, dado que lhe empresta prognóstico reservado na maioria dos casos. Protocolos de tratamento não são bem definidos, devido ao pequeno número de casos relatados na literatura. RELATO DE CASO: Apresenta-se caso de mulher de 66 anos admitida no hospital com queixa de disfagia. Avaliação radiológica e endoscópica revelaram lesão estenosante no terço inferior do esôfago, cuja biópsia mostrou tratar-se de carcinoma de pequenas células do esôfago. A paciente foi submetida à esofagectomia e reconstrução com tubo gástrico. CONCLUSÃO: O papel da cirurgia permance controverso e a melhora na qualidade de vida deve ser considerada. O tratamento terapêutico principal continua sendo a resecção esofágica.

BACKGROUND: Small cell carcinoma of the esophagus is a rare disease, characterized by aggressive progression, which is similar in appearance and behavior to its counterpart of the lung. It has a high incidence of metastatic disease at presentation and a poor prognosis. Treatment protocols are not well established because of the small number of cases and lack of large studies. CASE REPORT: 66 year-old woman, was admitted to the hospital with persistent dysphagia. Radiologic and endoscopic examination revealed an stenosing tumor in the lower third of the esophagus. The biopsy specimen was diagnosed as small cell carcinoma of the esophagus. The patient underwent an esophagectomy with esophagogastric anastomosis. CONCLUSION: The role of surgery remains controversial and the improvement of life quality is to be considered. Therapeutic approach is mainly esophageal resection.
