SUMMARY OBJECTIVE: The aim of this study was to evaluate the relationship between the range of motion and lower-limb hemodynamic indices in the tibiotarsal joint of individuals with diabetic neuropathy. METHODS: Twenty volunteers of both sexes, with a mean age of 61.45±7.05 years, were diagnosed with type 2 diabetes mellitus and diabetic peripheral neuropathy. Arterial blood flow was assessed using Doppler ultrasound, and the variables such as average velocity, pulsatility index, and resistivity index were also evaluated. A range of dorsiflexion and plantar flexion joint movements were assessed using digital goniometry before and after exercise. Data distribution was assessed using the Shapiro-Wilk test, followed by Pearson's correlation for normal data and Spearman's correlation for non-normal data, in order to verify the association between variables. RESULTS: A moderate correlation was found between dorsiflexion and pulse rate on two occasions before (rs=0.497) and after initial evaluation (rs=0.511). A low correlation was found between plantar flexion and mean velocity (rs=-0.357), pulsatility index (rs=0.439), and resistivity index (rs=0.328); dorsiflexion and mean velocity (rs=0.374), pulse rate (rs=0.332), and resistance index (rs=0.327) before evaluation, and peak (rs=0.346) was observed after the evaluation of blood circulation. CONCLUSION: There is a correlation between the range of motion of the tibiotarsal joint and the blood circulation of diabetics, ranging from moderate to poor for the different variables evaluated.

SUMMARY OBJECTIVE: This study aimed to compare the effects of therapeutic ultrasound and paraffin with or without vacuum massage on the biomechanical properties of grafted skin after a burn. METHODS: A total of 44 patients with deep second- and third-degree burns, with a mean age of 35.89 (±11.53) years, who visited the Hospital Burn Unity, were included in the study. The therapeutic interventions were randomly defined by drawing lots, with a crossover design (crossover), and a minimum interval of 7 days (washout) between interventions. Skin biomechanical parameters such as distensibility (R0) and viscoelasticity (R6) were noninvasively evaluated by Cutometer before and after 0, 10, 20, and 30 min of intervention with therapeutic ultrasound and paraffin alone, as well as associated with negative pressure therapy of the skin (vacuum therapy). In this study, all groups showed increased distensibility (R0) in the period immediately after the application of the resources and a progressive reduction in the effects in the consecutive tests. Participants with skin grafts showed a decrease in viscoelasticity (R6) in all groups, except therapeutic paraffin and therapeutic ultrasound and vacuum massage. CONCLUSION: The biomechanical properties of grafted skin after a burn are altered after therapeutic intervention with ultrasound alone or associated with vacuum massage, such as intervention with paraffin associated with vacuum massage, for both parameters evaluated, skin distensibility (R0) and skin viscoelasticity (R6). However, the same did not occur for the intervention with isolated paraffin. There was no significant difference between the interventions therapeutic ultrasound and therapeutic paraffin.

Resumo Introdução: A obesidade é considerada um fator contribuinte para o desenvolvimento da incontinência urinária (IU). Objetivo: O objetivo deste estudo foi avaliar a prevalência dos sintomas da IU em mulheres obesas mórbidas, seu impacto sobre a qualidade de vida (QV) e correlacionar a influência da idade e do índice de massa corporal (IMC) sobre a presença de IU. Método: Foram avaliadas, 65 mulheres, em pré-operatório de cirurgia bariátrica, com idade de 35,8±9,97 anos e IMC de 45,04±6,79 kg/m 2 sendo questionadas sobre sintomas de IU e, se confirmados, responderam ao King’s Health Questionnaire, validado para o português. O questionário foi auto administrado e foram atribuídos valores numéricos às respostas, esses escores foram avaliados em quartis e correlacionados com a idade e o IMC. O nível de rejeição da hipótese foi 5% e as análises foram realizadas com o software BioEstat, versão 5.3. Resultados: Das 65 mulheres que foram entrevistadas, dezenove delas (29,23%) apresentaram os sintomas da IU e responderam ao questionário. Os domínios que apresentaram maior pontuação foram “impacto da IU” (36,8) e “percepção geral da saúde” (32,9). Não foi encontrada correlação significativa entre os escores do questionário e o IMC. Já na associação com a idade, o domínio “impacto da IU” apresentou correlação positiva, significativa e moderada (r = 0,52; p = 0,02). Conclusão: Diante dos achados, com a prevalência de sintomas de IU na amostra estudada, pode-se inferir que a IU compromete pouco a QV, entretanto o aumento da idade quando associada à IU pode afetar significativamente a QV de mulheres obesas classe II e III.

Abstract Introduction: Obesity is a contributing factor to the development of urinary incontinence (UI). Objective: The objective of this study was to evaluate the prevalence of UI symptoms in women with morbid obesity and their impacts on quality of life (QOL), correlating the influence of age and body mass index (BMI) with the presence of UI. Method: Sixty-five women undergoing preoperative bariatric surgery, aged 35.8 ± 9.97 years, with an average BMI of 45.04 ± 6.79 kg/m2 were asked about the presence of UI symptoms; when they answered positively, the King’s Health Questionnaire (validated into Portuguese) was applied. The questionnaire was self-administered and all answers were assigned numerical scores. These scores were distributed according to quartiles and correlated with age and BMI. The level of hypothesis rejection was 5%, and analyses were performed using BioEstat software version 5.3. Results: Among 65 interviewees, 19 (29.23%) had symptoms of UI and went on to answer the questionnaire. The highest-scoring areas were “incontinence impact” (36.8) and “general health perception” (32.9). There was no significant correlation between questionnaire scores and BMI. In the association with age, the “incontinence impact” domain showed a significant, positive and moderate correlation (r = 0.52; p = 0.02). Conclusion: The prevalence of UI symptoms in the sample had a slight negative effect on QOL; however, UI associated with older ages significantly affected the QOL of the studied volunteers.

Resumen Introducción: La obesidad se considera un factor contribuyente para el desarrollo de la incontinencia urinaria (IU). Objetivo: El objetivo de este estudio fue evaluar la prevalencia de los síntomas de IU en las mujeres obesas morbidas, el impacto en la calidad de vida (CV), y la influencia de la edad y el índice de masa corporal (IMC) sobre la presencia de IU. Método: Fueron evaluados 65 mujeres, en preoperatorio de cirugía bariátrica, con edad de 35,8 ± 9,97 años e IMC de 45,04 ± 6,79 kg/m 2 siendo cuestionadas sobre síntomas de IU y, si confirmados, respondieron al King’s Health Questionnaire, validado para el portugués. El cuestionario fue auto administrado y se asignaron valores numéricos a las respuestas, los escores fueron evaluados en cuartiles y correlacionados con la edad y el IMC. El nivel de rechazo del análisis fue del 5% y los análisis se realizaron con el software BioEstat, versión 5.3. Resultados: De las 65 mujeres que fueron entrevistadas, diecinueve de ellas (29,23%) presentaron los síntomas de la IU y respondieron al cuestionario. Los dominios que presentaron mayor puntuación fueron “impacto de la IU” (36,8) y “percepción general de la salud” (32,9). No se encontró correlación significativa entre los puntajes del cuestionario y el IMC. En la asociación con la edad, el dominio “impacto de la IU” presentó correlación positiva, significativa y moderada (r = 0,52; p = 0,02). Conclusión: Ante los hallazgos, con la prevalencia de síntomas de IU en la populacion estudiada, se puede inferir que la IU compromete poco la QV, sin embargo el aumento de la edad cuando está asociada a la IU puede afectar significativamente a la CV de mujeres obesas clase II y III.

Resumo Câncer de mama é a neoplasia mais comum que mais acomete mulheres no mundo e proporciona alterações funcionais no corpo humano. Objetivo: avaliar mulheres submetidas à mastectomia unilateral, por meio da atividade eletromiográfica (repouso mandibular, lateralidade direita e esquerda, protrusão, contração voluntária máxima com e sem parafilme, mastigação habitual e não habitual) e espessura (repouso e contração voluntária máxima) do músculo temporal direito (TD), temporal esquerdo (TE), masseter direito (MD) e masseter esquerdo (ME), além da força de mordida molar (direita e esquerda) e comparar os dados com mulheres saudáveis. 32 mulheres foram distribuídas em dois grupos: grupo mastectomia unilateral (GM), média ± desvio padrão 56,50 ± 14,50 anos (n=16) e grupo sem a doença (GC), média ± desvio padrão 56,56 ± 14,15 anos (n=16). Os dados eletromiográficos normalizados, espessura muscular e força de mordida molar máxima foram tabulados e submetidos à análise estatística (SPSS 21.0; teste t de student, p≤0,05). Foram encontradas diferenças estatísticas significantes entre GM e GC na lateralidade direita, para o MD (p=0,02); lateralidade esquerda, para o TE (p=0,01); mastigação com amendoim, para o MD (p=0,04); mastigação com uva-passa, para o ME (p=0,04) e força de mordida molar direita (p=0,03). Não houve diferença estatística significante entre GM e GC para espessura muscular. Os resultados deste estudo sugerem que mulheres submetidas à mastectomia unilateral podem apresentar alterações funcionais do sistema estomatognático, com destaque para hiperatividade muscular, menor eficiência mastigatória e menor força de mordida molar máxima.

Abstract Breast cancer is the most common cancer affecting women in the world and provides functional changes in the human body. Evaluate women submitted to unilateral mastectomy, through electromyographic activity (mandibular rest, right and left laterality, protrusion, maximum voluntary contraction with and without Parafilm, habitual and non-habitual mastication) and thickness (rest and maximum voluntary contraction) of the right temporal muscle (RT), left temporal (LT), right masseter (RM) and left masseter (LM), also the molar bite force (right and left) and compare the data with healthy women. Material and Methods: 32 women were divided into two groups: unilateral mastectomy group (MG), average ± standard deviation 56.50±14.50 years (n=16) and without the disease group (CG), average ± standard deviation 56.56±14.15 years (n=16). The normalized electromyographic data, muscle thickness and maximal molar bite force were tabulated and submitted to statistical analysis (SPSS 21.0; student t test, p≤0.05). Significant statistical differences between MG and CG were found in right laterality, for RM (p=0.02); left laterality, for LT (p=0.01); chewing with peanuts, for RM (p=0.04); chewing with raisins, for LM (p=0.04) and right molar bite force (p=0.03). There was no statistically significant difference between MG and CG for muscle thickness. The results of this study suggest that women undergoing unilateral mastectomy may present functional changes, with emphasis on muscular hyperactivity, lower masticatory efficiency and lower maxillary bite force.

RESUMO O objetivo deste estudo foi avaliar o efeito da estimulação elétrica de alta voltagem (EEAV) catódica, associada à insulina tópica, em lesão tegumentar de ratos. Para isso, foram utilizados 42 ratos Wistar (240±30 g), submetidos a retirada cirúrgica de 1 cm2 de pele do dorso em seis grupos (n=7), tratados por sete dias consecutivos: controle (C), estimulação elétrica placebo (EP), estimulação elétrica catódica (EE), insulina tópica (IT), insulina placebo (IP) e EEAV associada a insulina tópica (EE+I). A EEAV foi administrada 24 horas após a cirurgia, 30 minutos por dia, com frequência de 100 Hz e voltagem média de 60 V, mantida no limiar motor. Áreas das lesões foram registradas macroscopicamente no primeiro, quarto e oitavo dia, sendo submetidas a tratamento histológico para inclusão em paraplast® e coloração em hematoxilina e eosina. A epitelização e o perfil numérico das células foram obtidos por análises histométricas. Utilizou-se o teste de Shapiro-Wilk e ANOVA one-way seguida de Bonferrone (p<0,05). Observou-se redução significativa na área da lesão no oitavo dia de tratamento, nos grupos EE e EE+I em relação aos demais grupos. A reepitelização não diferiu entre os grupos, mas a distância entre as bordas da lesão foi menor nos grupos EE e EE+I. Nesses grupos houve aumento significativo (p<0,05) no número de fibroblastos e diminuição de leucócitos. Pode-se concluir que a EEAV catódica acelerou o processo de reparação da lesão, não demonstrando efeito adicional com a aplicação da insulina tópica.

ABSTRACT This study aims to evaluate the effect of cathodic high voltage electrical stimulation (HVES), associated with topical insulin, on rat integumentary lesions. For this purpose, 42 Wistar rats (240±30 g) were submitted to surgical removal of 1 cm2 of dorsal skin and divided into six groups (n=7), treated for seven consecutive days: Control (C), placebo electrical stimulation (PES), cathodic electrical stimulation (ES), topical insulin (TI), placebo insulin (PI) and HVES associated with topical insulin (ES+I). HVES was administered 24 hours after surgery, 30 minutes per day, with a frequency of 100 Hz and a mean voltage of 60 V, maintained at the motor threshold. Lesion areas were recorded macroscopically on the first, fourth and eighth day, submitted to histological treatment for inclusion in paraplast® and staining in Hematoxylin and Eosin. Epithelialization and the numerical profile of the cells were obtained by histometric analysis. The Shapiro-Wilk and Anova one-way test was followed by the Bonferroni (p<0.05). There was a significant reduction in the area of the lesion by the eighth day of treatment, both in the ES and ES+I groups, when compared with other groups. Reepithelialization did not differ between groups, but the distance between the edges of the lesion was lower in the ES and ES+I groups. These same groups showed a significant increase (p<0.05) in the number of fibroblasts and a decrease in leukocytes. Thus, we can conclude that cathodic HVES accelerated the lesion repair process, with the topical application of insulin showing no additional effect.

RESUMEN El propósito de este estudio fue evaluar el resultado de la estimulación eléctrica por alta voltaje (EEAV) catódica, asociada a la insulina tópica, en lesión cutánea de ratas. Para ello, se utilizaron 42 ratas Wistar (240±30 g), les sometieron a cirugía de retirada de 1 cm2 de piel del dorso en 6 grupos (n=7), y les trataron por siete días consecutivos: control (C), estimulación eléctrica placebo (EP), estimulación eléctrica catódica (EE), insulina tópica (IT), insulina placebo (IP) y EEAV asociada a la insulina tópica (EE+I). Se aplicó la EEAV 24 horas después de la cirugía, 30 minutos por día, con frecuencia de 100 Hz y voltaje de media tensión de 60 V, y la mantuvo en el umbral motor. Se registraron las áreas de las lesiones macroscópicamente en el primer, cuarto y octavo día, y las sometieron al tratamiento histológico para inclusión en paraplast® y tinción hematoxilina-eosina. Se obtuvo la epitelización y el perfil numérico de las células por análisis histométricos. Se empleó la prueba Shapiro-Wilk y ANOVA one way de Bonferroni (p<0,05). Se redujo significativamente el área de la lesión en el octavo día del tratamiento en los grupos EE y EE+I comparados a los demás grupos. La reepitalización no fue distinta entre los grupos, sin embargo, la distancia entre los bordes de la lesión fue menor en los grupos EE y EE+I. También en estos grupos aumentó significativamente (p<0,05) el número de fibroblastos y disminuyeron los leucocitos. Se concluye que la EEAV catódica aceleró el proceso de reparación de la lesión, pero no ocurrió resultado con la aplicación de la insulina tópica.

BACKGROUND: Infrared thermography is recognized as a viable method for evaluation of subjects with myofascial pain. OBJECTIVE: The aim of the present study was to assess the intra- and inter-rater reliability of infrared image analysis of myofascial trigger points in the upper trapezius muscle. METHOD: A reliability study was conducted with 24 volunteers of both genders (23 females) between 18 and 30 years of age (22.12±2.54), all having cervical pain and presence of active myofascial trigger point in the upper trapezius muscle. Two trained examiners performed analysis of point, line, and area of the infrared images at two different periods with a 1-week interval. The intra-class correlation coefficient (ICC2,1) was used to assess the intra- and inter-rater reliability. RESULTS: With regard to the intra-rater reliability, ICC values were between 0.591 and 0.993, with temperatures between 0.13 and 1.57 °C for values of standard error of measurement (SEM) and between 0.36 and 4.35 °C for the minimal detectable change (MDC). For the inter-rater reliability, ICC ranged from 0.615 to 0.918, with temperatures between 0.43 and 1.22 °C for the SEM and between 1.19 and 3.38 °C for the MDC. CONCLUSION: The methods of infrared image analyses of myofascial trigger points in the upper trapezius muscle employed in the present study are suitable for clinical and research practices.

OBJECTIVES: The aim of this study was to identify variations in nervous thresholds in different phases of the menstrual cycle in eumenorrheic women and users of oral contraceptives. METHOD: An observational study was performed including 56 volunteers, consisting of 30 eumenorrheic women who were non-users of oral contraceptives and 26 users of oral contraceptives. An electrical stimulator was employed to assess their nervous thresholds, with pulses applied at a fixed frequency of 2,500 Hz, modulated at 50 Hz, with phase variances of 20 μs, 50 μs and 100 μs. Sensitivity, motor and pain thresholds were evaluated during five menstrual cycle phases: phase 1 - menstrual, phase 2 - follicular, phase 3 - ovulatory, phase 4 - luteal and phase 5 - premenstrual. RESULTS: The results indicated low sensitivity thresholds of 100 μs for non-users of oral contraceptives and 50 μs for oral contraceptive users in phase 5. Low motor thresholds of 20 μs, 50 μs and 100 μs were observed for non-users of oral contraceptives in phase 5, while that of oral contraceptive users was 100 μs. Finally, a low pain threshold of 100 μs was observed in phase 5, but only in the oral contraceptive group. CONCLUSION: Nervous thresholds vary systematically across the phases of the menstrual cycle, with or without the use of oral contraceptives. These variations should be taken into account during research performed in women.

O linfedema ainda é uma das principais sequelas decorrentes do tratamento cirúrgico do câncer de mama. O objetivo do estudo foi avaliar a eficácia de um protocolo que inclui a utilização da estimulação elétrica de alta voltagem (EEAV) associada a exercícios terapêuticos, automassagem e autocuidados no tratamento do linfedema de membros superiores em mulheres submetidas a cirurgia para tratamento do câncer de mama. Participaram do estudo 17 voluntárias (60,9_+11,72 anos) submetidas à mastectomia unilateral, portadoras de linfedema de membro superior, homolateral à cirurgia. O tratamento constituiu-se de 14 aplicações da EEAV, duas vezes por semana, complementadas por orientações quanto ao autocuidado, automassagem e exercícios físicos. A evolução do tratamento foi avaliada por perimetria, cálculo da diferença de volume (DV) entre os membros, e percentual de aumento do volume (PAV) do membro afetado em relação ao contralateral. Os dados foram analisados por meio do método estatístico T pareado para variáveis dependentes e revelaram redução significativa de 14,13% (p=0,0067) do PAV e de 13,8% (p=0,0089) da DV, bem como da perimetria em três pontos: sete centímetros acima do cotovelo (p=0,0138), sete centímetros abaixo do cotovelo (p=0,0282) e no punho (p=0,0476). Pôde-se concluir que a utilização da estimulação elétrica de alta voltagem associada a exercícios e orientações foi eficaz na redução do linfedema do grupo avaliado.

Lymphedema is still considered as one of the main sequela resulting from surgical treatment of breast cancer. The aim of this study was to evaluate the efficacy of a protocol that included use of a high-voltage electrical stimulation (HVES) associated with therapeutic exercises, self-massage, and self-care for the treatment of lymphedema of the upper limbs in women who underwent surgery for breast cancer treatment. This study included 17 volunteers (60.9_+11.72 years of age) submitted to unilateral mastectomy, with lymphedema of the upper limb, ipsilateral to surgery. The treatment consisted of application of 14 HVES, to the patients, twice a week, supplemented by guidance on self-care, self-massage, and physical exercises. The evolution of this treatment was assessed by perimetry, calculation of the volume difference (VD) between the limbs, and calculation of the volume increase percentage (VIP) of the affected limb compared to the contralateral limb. Data were analyzed using the statistical method for paired T and dependent variables, which showed a significant reduction of 14.13% (p=0.0067) in VIP and 13.8% (p=0.0089) in VD, as well as perimetry at the following three points: 7 cm above the elbow (p=0.0138), 7 cm below the elbow (p=0.0282), and at the wrist (p=0.0476). It was concluded that the use of HVES associated with the exercises and guidance on self-care and self-massage was effective to reduce the lymphedema in the evaluated group.

El linfedema todavía es una de las principales secuelas derivadas del tratamiento quirúrgico del cáncer de mama. El objetivo del estudio fue evaluar la eficacia de un protocolo que incluye la utilización de estimulación eléctrica de alto voltaje (EEAV) asociada a ejercicios terapéuticos, automasajes y autocuidados en el tratamiento del linfedema de miembros superiores en mujeres sometidas a cirugía para el tratamiento de cáncer de mama. Participaron del estudio 17 voluntarias (60,9±11,72 años) sometidas a mastectomía unilateral, portadoras de linfedema de miembro superior, ipsilateral a la cirugía. El tratamiento consiste en 14 aplicaciones de EEAV, dos veces por semana, complementadas por orientaciones en el autocuidado, automasaje y ejercicios físicos. La evolución del tratamiento fue evaluada por perímetros, cálculo de la diferencia de volumen (DV) entre los miembros y porcentaje de aumento del volumen (PAV) del miembro afectado en relación al contralateral. Los datos fueron analizados por medio del método estadístico T pareado para variables dependientes y revelaron reducción significativa de 14,13% (p=0,0067) del PAV y de 13,8% (p=0,0089) del DV, también en los perímetros en tres puntos: siete centímetros encima del codo (p=0,0138), siete centímetros abajo del codo (p=0,0282) y en la muñeca (p=0,0476). Se puede concluir que la utilización de la estimulación eléctrica de alto-voltaje asociada a ejercicios y orientaciones fue eficaz en la reducción del linfedema del grupo evaluado.

CONTEXTUALIZAÇÃO: A redução da força dos músculos do assoalho pélvico (MAP) é a maior causa da incontinência urinária de estresse (IUE). OBJETIVO: Comparar as forças ativa e passiva e a abertura da cavidade vaginal em mulheres continentes e com IUE. MÉTODOS: O estudo incluiu um total de 32 mulheres, 16 mulheres continentes (grupo 1 - G1) e 16 mulheres com IUE (grupo 2 - G2). Um espéculo dinamométrico de aço inoxidável foi usado para avaliar as forças ativa e passiva dos MAP nas direções ântero-posterior (plano sagital) e látero-lateral (plano frontal). RESULTADOS: A força ativa ântero-posterior nas mulheres continentes (0,3±0,2 N) foi maior que nas mulheres com IUE (0,1±0,1 N). Não houve diferença entre o G1 e o G2 nos valores de força ativa látero-lateral (G1=0,43±0,1 N; G2=0,40±0,1 N), força passiva (G1=1,1±0,2 N; G2=1,1±0,3 N) e abertura da cavidade vaginal (G1=21±3 mm; G2=24±4 mm). CONCLUSÃO: A avaliação da função dos MAP mostrou que mulheres com IUE tiveram uma menor força ativa ântero-posterior quando comparadas com as mulheres continentes.

BACKGROUND: The reduction of the pelvic floor muscles (PFM) strength is a major cause of stress urinary incontinence (SUI). OBJECTIVES: To compare active and passive forces, and vaginal cavity aperture in continent and stress urinary incontinent women. METHODS: The study included a total of thirty-two women, sixteen continent women (group 1 - G1) and sixteen women with SUI (group 2 - G2). To evaluate PFM passive and active forces in anteroposterior (sagittal plane) and left-right directions (frontal plane) a stainless steel specular dynamometer was used. RESULTS: The anteroposterior active strength for the continent women (mean±standard deviation) (0.3±0.2 N) was greater compared to the values found in the evaluation of incontinent women (0.1±0.1 N). The left-right active strength (G1=0.43±0.1 N; G2=0.40±0.1 N), the passive force (G1=1.1±0.2 N; G2=1.1±0.3 N) and the vaginal cavity aperture (G1=21±3 mm; G2=24±4 mm) did not differ between groups 1 and 2. CONCLUSION: The function evaluation of PFM showed that women with SUI had a lower anteroposterior active strength compared to continent women.

JUSTIFICATIVA E OBJETIVOS: Limiares reduzidos de dor a pressão (LDP) e presença de pontos de gatilho musculares costumam ser observadas em pacientes com enxaqueca. A fisioterapia costuma ser útil para esses pacientes. O objetivo deste estudo foi demonstrar os benefícios do ultrassom estático no tratamento de pacientes com enxaqueca. RELATO DE CASO: Paciente do sexo feminino, 25 anos, com enxaqueca desde os 15 anos de idade. Foi enviada por especialista em cefaleia devido à refratariedade ao tratamento farmacológico. Tinha aproximadamente 8 crises incapacitantes por mês que duravam 2 a 3 dias. Foram examinados os músculos craniocervicais, medido o LDP e a amplitude de movimento cervical. Participou de 20 sessões, duas vezes por semana com duração de 40 a 50 minutos, de alongamento global e tração cervical, além de liberação miofascial e desativação dos pontos de gatilho musculares. Após a 6ª sessão introduziu-se o ultrassom estático ao protocolo. CONCLUSÃO: Houve redução significativa na frequência e duração dos ataques de enxaqueca, além de aumento do LDP. A fisioterapia com ultrassom estático pode ser útil para pacientes com enxaqueca refratária.

BACKGROUND AND OBJECTIVES: Reduced pressure pain thresholds (PPT) and presence of muscular trigger points are often observed in patients with migraine. Physical therapy is frequently helpful in these patients. The objective of this study was to demonstrate the benefits of static ultrasound in the treatment of patients with migraine. CASE REPORT: Female patient, 25 years-old with migraine since the age of 15 years. She was referred by a headache specialist due to refractoriness to pharmacological treatment. She had around 8 disabling attacks per months, lasting from 2 to 3 days. We examined the craniocervical muscles, measured the PPT and cervical range of motion. She participated in 20 sessions twice a week, lasting from 40 to 50 minutes, of global stretching, stretching and cervical traction, as well as myofascial vibration and deactivation of muscular trigger points. From the 6th section after, static ultrasound was added to the protocol. CONCLUSION: There has been significant reduction in the frequency and duration of migraine attacks, as well as increased PPT. Physical therapy using Static Ultrasound may be of value for patients with refractory migraine.

CONTEXTUALIZAÇÃO: A estimulação elétrica neuromuscular (EENM) é amplamente utilizada no tratamento da incontinência urinária de esforço (IUE), no entanto não há consenso na literatura sobre os parâmetros de tratamento mais eficazes. OBJETIVO: Avaliar os procedimentos de EENM intravaginal no tratamento de mulheres com IUE. MÉTODOS: Participaram do estudo 20 voluntárias com idade média de 55,55±6,51 anos, com diagnóstico clínico de IUE. As voluntárias foram divididas aleatoriamente em dois grupos: grupo 1 (G1), que recebeu EENM com corrente de média frequência, e grupo 2 (G2), com corrente de baixa frequência. A avaliação funcional dos músculos do assoalho pélvico (MAP) foi realizada por meio de perineometria, e a severidade dos sinais e sintomas da IUE foi avaliada, objetivamente, pelo pad test de uma hora e, subjetivamente, pela Escala Visual Analógica (EVA), que mediu o desconforto causado pela incontinência. Para a análise de normalidade dos dados, utilizou-se o teste Shapiro-Wilk, seguido do teste de Friedman para dados não paramétricos. Para a análise dos sintomas relacionados à IUE, usaram-se os testes Exato de Fisher e Mann-Whitney. Em todas as análises, considerou-se o nível de significância de 5%. RESULTADOS: Não houve diferença significativa (p>0,05) entre os grupos em nenhuma variável avaliada. Na comparação intragrupos das avaliações iniciais e finais (após EENM), houve diferença significativa (p<0,05) na quantidade de urina perdida, no desconforto causado pela incontinência urinária e na pressão perineal. CONCLUSÃO: Os procedimentos de EENM utilizados neste estudo foram igualmente eficazes no tratamento da IUE.

BACKGROUND: Neuromuscular electrical stimulation (NMES) is widely treatment for stress urinary incontinence (SUI) but there is no consensus in literature regarding the most effective treatment parameters. OBJECTIVE: To compare two NMESintra-vaginal protocols for the treatment of SUI in women. METHODS: The study included 20 volunteers with an average age of 55.55±6.51 years and with the clinical diagnosis of SUI. Volunteers were randomly divided into two groups: group 1 (G1) received NMES with medium-frequency current and group 2 (G2) received NMES with low-frequency current. Functional assessments of pelvic floor muscles (PFM) were performed by perineometry. The severity of signs and symptoms were objectively evaluated using the 1 hour pad test and subjectively evaluated using a visual analog scale that measured the discomfort caused by the SUI. Shapiro-Wilk test was used to analyze data normality, and the Friedman test was used to analyze nonparametric data. For analysis of symptoms related to SUI the Fisher exact test and the Mann-Whitney test were used. Significance level of 5% was set for all data analysis. RESULTS: No significant differences (p>0.05) were found between groups for any of the variable assessed. The within group analysis of initial and final evaluations (after NMES) demonstrated significant differences (p<0.05) in amount of urine lost, the discomfort caused by urinary incontinence and perineal pressure for both treatment groups. CONCLUSION: The two NMES protocols applied were equally effective in the treatment of SUI.

CONTEXTUALIZAÇÃO: Lesões nervosas periféricas provocam limitação funcional prolongada, sendo um desafio para a clínica identificar recursos que acelerem sua recuperação. OBJETIVOS: Investigar a influência da estimulação elétrica de alta voltagem (EEAV) sobre a morfologia e a função do nervo regenerado após esmagamento em ratos. MÉTODOS: Vinte ratos Wistar foram divididos nos grupos: controle (CON) - sem lesão e sem EEAV; desnervado (D) - esmagamento do nervo isquiático; desnervado + EEAV (EEAV) - esmagamento do nervo e EEAV; SHAM - sem lesão, porém submetido à EEAV. Os grupos EEAV e SHAM foram estimulados (100 Hz, tensão mínima de 100 V; 20 μs e 100 μs interpulso) 30 min/dia, 5 dias/semana. O índice funcional do ciático (IFC) foi avaliado antes da lesão, nos 7º, 14º e 21º dias pós-operatório (PO). Componentes neurais, densidade de área de tecido conjuntivo, de vasos sanguíneos e macrófagos foram analisados. RESULTADOS: O diâmetro axonal foi maior no grupo EEAV que no grupo D, atingindo quase 80% dos valores-controle após 21 dias (p<0,05). O diâmetro das fibras e espessura das bainhas de mielina foram maiores no grupo EEAV que no D (p<0,05), alcançando 96,5% e 100% dos valores-controle, respectivamente. A recuperação funcional no 14º dia PO foi melhor no grupo EEAV. A densidade de área de macrófagos e tecido conjuntivo foi menor no grupo EEAV, enquanto o número de vasos sanguíneos não diferiu entre os grupos. CONCLUSÕES: A EEAV acelerou a recuperação funcional, potencializou a maturação das fibras nervosas regeneradas e promoveu diminuição da densidade de área de macrófagos e tecido conjuntivo no nervo, sugerindo aceleração do reparo neural.

BACKGROUND: Peripheral nerve injury causes prolonged functional limitation being a clinical challenge to identify resources that accelerates its recovery. OBJECTIVES: To investigate the effect of high-voltage electrical stimulation (HVES) on the morphometric and functional characteristics of the regenerated nerve after crush injury in rats. METHODS: Twenty Wistar rats were randomly allocated into 4 groups: Control (CON) - without injury and without HVES; Denervated (D) - sciatic nerve crush only; Denervated + HVES - sciatic nerve crush and HVES; SHAM - without injury but HVES. The HVES and SHAM groups were stimulated (100 Hz; minimum voltage of 100 V, 20 μs, 100 μs interpulse interval) for 30 min/day, 5 days/week. The sciatic functional index (SFI) was evaluated before the injury and at the 7th, 14th and 21st postoperatory (PO) days. Neural components and the area density of connective tissue, blood vessels and macrophages were analyzed. RESULTS: Axonal diameter was higher on the HVES than on D group, reaching almost 80% above the control values after 21 days (p<0.05). Fiber diameter and myelin sheath thickness were higher on the HVES than on D group (p<0.05) reaching 96.5% and 100% of the control values, respectively. Functional recovery at the 14th PO day was better on group HVES. The macrophages and connective tissue area density was lower on the HVES group, while blood vessels number did not differ among groups. CONCLUSIONS: The HVES accelerated the functional recovery, potentiated the nerve fibers maturation and decreased macrophages and connective tissue area density, suggesting acceleration of neural repair.

A estimulação elétrica de alta voltagem tem sido indicada para acelerar processos de cicatrização. Os efeitos da estimulação elétrica de alta voltagem, no tratamento de três voluntários com úlceras crônicas de membros inferiores, foram avaliados. Após quinze semanas de tratamento, pôde ser observada a redução da área de todas as úlceras, a qual sugere que a EEAV é um método eficaz no tratamento de úlceras crônicas.

High voltage electrical stimulation has been recommended as a means of accelerating the wound healing process. The effects of high voltage electrical stimulation were evaluated in the treatment of three volunteers with chronic ulcers of the lower limbs. After fifteen weeks of treatment, a reduction was found in the area of all the ulcers, suggesting that high voltage electric stimulation is an effective therapeutic option for chronic ulcers.

OBJETIVO: verificar o comportamento da musculatura abdominal e perineal em face de alterações respiratórias induzidas em 15 nulíparas, sem história de disfunções perineais e/ou respiratórias prévias, com faixa etária de 20 a 26 anos (22,9±1,83). MÉTODOS: a atividade elétrica dos músculos abdominais e perineais foi analisada utilizando a eletromiografia de superfície, e a pressão perineal (PP) obtida mediante biofeedback digital, simultaneamente. As voluntárias foram instruídas a realizar três repetições e a execução de manobras respiratórias: inspiração máxima (PImáx), expiração máxima (PEmáx) e Valsalva (MV), em ordem aleatória. As voluntárias foram posicionadas em mesa ginecológica, com 60º de inclinação de tronco. Os sinais eletromiográficos foram coletados pelo software AqDados® (4.4) para linguagem binária ASCii, sendo posteriormente processados no software Matlab® (6.5.1). A análise estatística da envoltória (EN) do sinal foi realizada por meio da correlação de Spearman e do teste Kruskal-Wallis, com nível de significância de 5% (p<0,05). RESULTADO: observou-se que a PP foi maior na PImáx (2,98±2,38), seguida da MV (29,10±10,68), sendo ambas superadas pela PEmáx (38,22±9,98) (p<0,01). Demonstrou-se ainda correlação positiva entre a PEmáx e a PP (p<0,01), bem como entre a EN da musculatura perineal e abdominal na PEmáx e PImáx (p<0,05 e p=0,03, respectivamente). Os resultados relativos às MVs não foram significativos, quando analisadas a PP e EN. CONCLUSÃO: foi possível identificar a presença de sinergia abdômino-pélvica durante a execução das manobras respiratórias, em especial em relação a PEmáx.

PURPOSE: to verify the behavior of the abdominal and perineal musculature in respiratory changes induced in 15 nulliparous women without previous history of perineal or respiratory failures, with age ranging from 20 to 26 years (22.9±1.83). METHODS: the electrical abdominal and perineal activities were analyzed simultaneously through surface electromyography and perineal pressure (PP) obtained through digital biofeedback. The volunteers were told to accomplish three types of respiratory maneuvers: maximum inspiration (PImax), maximum expiration (PEmax) and Valsalva (VM), at random. The electromyographic signs were collected by the AqDados® (4.4) software for binary language ASCII, being processed later using the Matlab® (6.5.1) software. The statistical analysis of the envoltory (EN) of the signal was accomplished through Spearman correlation and Kruskal-Wallis test, and the level of significance was set at 5% (p<0.05). RESULTS: it was observed that PP was larger in PImax (2.98±2,38), followed by VM (29.10±10.68), both being overcome by PEmax (38.22±9,98) (p<0.01). A positive correlation between PEmax and PP (p<0.01), as well as between EN of the perineal and abdominal musculature in PEmax and PImax (p<0.05 and p=0,03, respectively) could be shown. The results regarding VM were not significant, when PP and EN were analyzed. CONCLUSION: it was possible to identify the presence of abdomino-pelvic synergy during the execution of breathing maneuvers, especially in relation to PEmax.
