RESUMEN Introducción En México, esta indagación hasta donde se tiene conocimiento representa el primer trabajo multicéntrico que estudia el grado de desarrollo de la aptitud clínica en cuatro Unidades de medicina familiar del Sector Salud. Por consiguiente, la interrogante de investigación es ¿Cuál es el grado de desarrollo de la aptitud clínica en tres instituciones de salud?. Este estudio proveerá conocimientos que permitirán emprender investigaciones en las cuales se ensayen estrategias educativas que propicien el desarrollo de esta aptitud. Objetivo Determinar el grado de desarrollo de la aptitud clínica en residentes de medicina familiar de cuatro sedes académicas. Método Diseño prospectivo, transversal, observacional y multicéntrico. Se estudió toda la matrícula del ciclo académico 2019-2020 con consentimiento informado. Se construyó un instrumento que evaluó la variable “aptitud clínica” mediante ocho indicadores y 182 ítems; la validez conceptual, de contenido y confiabilidad se valoró por cinco médicos familiares con experiencia docente y en investigación educativa. Resultados De las cuatro sedes, las calificaciones globales más altas en los indicadores fueron alcanzadas por los R1 de la Unidad de Medicina Familiar # 20 del Instituto Mexicano del Seguro Social (UMF20 IMSS), R2 y R3 del Hospital General de Zona # 1 (HGZMF1 IMSS). Los R1 de la UMF20 alcanzaron los más altos puntajes en 3/8 indicadores. Los R2 y R3 del HGZMF1 lograron las más altas puntuaciones en 2/8 indicadores. Se estimaron diferencias entre R1 (n=46), entre R2 (n=47) y entre R3 (n=40) de las cuatro sedes por indicador y no se encontraron diferencias estadísticas significativas en ningún caso. Conclusiones El grado de desarrollo de la aptitud clínica se puede considerar de nivel medio en las cuatro sedes académicas. Esto en parte se puede explicar a que el instrumento exploró situaciones clínicas problematizadas, que exigieron del residente la reflexión crítica de su experiencia clínica antecedente.
ABSTRACT Introduction In Mexico, this inquiry to the best of our knowledge represents the first multicenter work that studies the degree of development of clinical aptitude in four Family Medicine Units of the Health Sector. Therefore, the research question is : What is the degree of development of clinical aptitude in three health institutions? This study will provide knowledge that will allow undertaking researches in which educational strategies that propitiate the development of this aptitude are tested. Objective To determine the degree of development of clinical aptitude in family medicine residents from four academic sites. Method Prospective, cross-sectional, observational and multicenter design. The entire enrollment of the 2019-2020 academic year was studied with informed consent. An instrument was constructed that assessed the variable "clinical aptitude" by means of eight indicators and 182 items; conceptual, content and reliability validity was assessed by five family physicians with teaching and educational research experience. Results Of the four sites, the highest overall scores on the indicators were achieved by the R1 of the Family Medicine Unit # 20 of the Mexican Social Security Institute (UMF20 IMSS), R2 and R3 of the General Zone Hospital # 1 (HGZMF1 IMSS). UMF20 R1s achieved the highest scores on 3/8 indicators. The R2s and R3s of HGZMF1 achieved the highest scores on 2/8 indicators. Differences were estimated between R1s (n=46), between R2s (n=47) and between R3s (n=40) of the four sites by indicator and no significant statistical differences were found in any case. Conclusions The degree of development of clinical aptitude can be considered medium level in the four academic sites. This can be partly explained by the fact that the instrument explored problematic clinical situations, which required the resident to reflect critically on his previous clinical experience.

Immunogenic potential of Rhipicephalus (Boophilus) microplus aquaporin 1 against Rhipicephalus sanguineus in domestic dogs

Resumo Este estudo avaliou a proteína recombinante (aquaporina) do carrapato Rhipicephalus (Boophilus) microplus como antígeno em vacina contra Rhipicephalus sanguineus. Cinco cães foram imunizados com RmAQP1 (10 µg) + adjuvante (G1) e cinco foram inoculados apenas com adjuvante (G2), três vezes. 21 dias após a última imunização todos os animais foram desafiados com larvas, ninfas e adultos de R. sanguineus, e potencial biótico dos carrapatos foi comparado. Amostras de sangue foram coletadas antes de cada imunização e a cada 28 dias após a última imunização, durante 10 semanas. Títulos de anticorpos dos soros dos cães foram avaliados por ELISA. Resultados: o período de ingurgitamento das fêmeas do G1 foi 12% mais curto que o período de ingurgitamento de G2; o período de ingurgitamento das larvas do G1 8,7% foi mais longo e o peso 7,2% menor que no caso de G2; o período de ingurgitamento das ninfas do G1 4,5% foi mais curto e peso 3,6% menor que no caso do G2; aumento dos títulos de anticorpos do G1 após a segunda imunização e declínio após a terceira imunização. Os resultados indicaram baixo potencial de imunoproteção de RmAQP1 contra R. sanguineus adultos, e possível eficácia contra larvas e ninfas, na dose testada. Sugere-se desenvolver novos estudos para melhor avaliação da eficácia de RmAQP1 contra R. sanguineus em cães.
Abstract This study evaluated a recombinant aquaporin 1 protein of Rhipicephalus (Boophilus) microplus (RmAQP1) as antigen in a vaccine against R. sanguineus. Five dogs were immunized with RmAQP1 (10 µg) + adjuvant (Montanide) (G1), and five were inoculated with adjuvant only (G2), three times. Twenty-one days after the last immunization, animals of both groups were challenged with R. sanguineus larvae, nymphs and adults, and their biotic potential was compared. Blood samples were collected before each immunization and every 28 days after the last immunization for 10 weeks. Serum antibody titers (IgG) were assessed by ELISA. We observed that: engorgement period of adult females from G1 was 12% shorter than G2; larvae from G1 had 8.7% longer engorgement period than G2 and weighed 7.2% less; nymphs from G1 had 4.5% shorter engorgement period than G2 and weighed 3.6% less; although the antibody titers increased following the second immunization, they rapidly decreased after the third immunization. Results indicated low immunoprotection of RmAQP1 against adult R. sanguineus ticks, and possible efficacy on larvae and nymphs fed on immunized dogs. Further studies should be performed for a full evaluation of the immunoprotection of RmAQP1 against R. sanguineus infestations in dogs.

A resistência aos acaricidas tornou-se amplamente difundida nos países onde os carrapatos bovinos, Rhipicephalus .Boophilus. microplus, são um problema. A resistência surge por meio de alterações genéticas em umapopulação de carrapatos que causam modificações no local de ação, aumento do metabolismo ou sequestro do acaricida, ou ainda redução na capacidade do acaricida em penetrar através das camadas protetoras do corpo do carrapato. Neste artigo, foram revisados os mecanismos moleculares e bioquímicos da resistência aos acaricidas que ocorrem em R. (B.) microplus. A partir de um ponto de vista dos mecanismos envolvidos, a resistência aos piretróides tem sido caracterizada em maior grau do que em qualquer outra classe de acaricida. Embora uma grande quantidade de pesquisas têm sido direcionada para a descoberta de mecanismos que causam resistência aos organofosforados, muito pouco é conhecido ao nível molecular, e essa resistência parece ser mantida por intermédio de um processo multifatorial e complexo. Os mecanismos de resistência para os demais acaricidas são bem menos compreendidos. Os alvos de ação do fipronil e das lactonas macrocíclicas são conhecidos, e os estudos dos mecanismos de ação envolvidos estão ainda em estágios iniciais. O alvo de ação do amitraz ainda não foi definitivamente identificado, e isso é limitante aos estudos dos mecanismos envolvidos na resistência a esse acaricida.
Acaricide resistance has become widespread in countries where cattle ticks, Rhipicephalus (Boophilus) microplus, are a problem. Resistance arises through genetic changes in a cattle tick population that causes modifications to the target site, increased metabolism or sequestration of the acaricide, or reduced ability of the acaricide to penetrate through the outer protective layers of the tick’s body. We review the molecular and biochemical mechanisms of acaricide resistance that have been shown to be functional in R. (B.) microplus. From a mechanistic point of view, resistance to pyrethroids has been characterized to a greater degree than any other acaricide class. Although a great deal of research has gone into discovery of the mechanisms that cause organophosphate resistance, very little is defined at the molecular level and organophosphate resistance seems to be maintained through a complex and multifactorial process. The resistance mechanisms for other acaricides are less well understood. The target sites of fipronil and the macrocyclic lactones are known and resistance mechanism studies are in the early stages. The target site of amitraz has not been definitively identified and this is hampering mechanistic studies on this acaricide.

Este estudo foi realizado com o objetivo de se obter uma visão epidemiológica da resistência aos acaricidas nas populações de Rhipicephalus (Boophilus) microplus no Estado de Mato Grosso do Sul. Vinte e quatro amostras de carrapatos foram coletadas em cidades do estado onde os fazendeiros relataram preocupação com resistência ou falha no controle dos carrapatos. Estes carrapatos foram submetidos a testes de detecção de resistência in vitro usando o teste de imersão de adultos (TIA). A eficácia do tratamento com alfa-cipermetrina, cipermetrina e amitraz foi geralmente pobre nas amostras coletadas em todo o Estado. Obteve-se eficácia de >90% no TIA com o DDVP + chlorfenvinphos (20 das 21 cidades), Diclorvós + Cipermetrina (10 das 16 cidades), e Cipermetrina + Citronela + Clorpirifós + Butóxido de piperonila (20 das 21 cidades). O ensaio da PCR foi utilizado para detectar a presença de mutação do gene do canal de sódio associado à resistência a piretróide. Avaliaram-se larvas de três diferentes populações onde anteriormente foi diagnosticada à resistência por TIA. Os ensaios da PCR mostraram que a mutação no gene associado à resistência ao piretróide estava ausente nessas três populações. Esse estudo confirma que o surgimento da resistência é um constante desafio para a pecuária e o desenvolvimento de resistência continua a ser um importante estímulo para o desenvolvimento de novas drogas antiparasitárias.
This study was conducted to obtain an epidemiological view of acaricide resistance in populations of Rhipicephalus (Boophilus) microplus in the State of Mato Grosso do Sul. Twenty-four tick samples were collected from municipalities in the State where farmers had reported concerns about resistance to or failure of tick control. These ticks were subjected to in vitro resistance detection assays using the adult immersion test (AIT). The efficacy of alpha-cypermethrin, cypermethrin and amitraz treatments on samples collected throughout the State was generally poor. AIT showed efficacy > 90% from the use of DDVP + chlorfenvinphos) (20 out of 21 municipalities), dichlorvos + cypermethrin (10 out of 16 municipalities) and cypermethrin + citronella + chlorpyrifos + piperonyl butoxide (20 out of 21 municipalities). PCR assays were used to detect the presence of pyrethroid resistance-associated sodium channel gene mutation. Larvae from three different populations that had previously been diagnosed as pyrethroid-resistant, through AIT, were evaluated. The PCR assays showed that the pyrethroid resistance-associated gene mutation was absent from these three populations. This study confirms that the emergence of resistance is a constant challenge for the livestock industry, and that development of resistance continues to be a major driver for new antiparasitic drugs to be developed.
