Abstract Background: Cardiovascular diseases are the leading cause of mortality worldwide and Mexico is no exception. The epidemiological data obtained in 1990 showed that cardiovascular diseases represented 19.8% of all causes of death in our country. This figure increased significantly to 25.5% for 2015. Some national surveys suggest that more than 60% of the adult population has at least one risk factor for cardiovascular disease (obesity or overweight, hypertension, smoking, diabetes, dyslipidemias). On the other hand, data from the Pan American Health Organization have linked the process of atherosclerosis as the first cause of premature death, significantly reducing life expectancy, which has enormous social repercussions. Objective: This document constitutes the Clinical Practice Guide (CPG) prepared at the initiative of the Mexican Society of Cardiology in collaboration with the Mexican Society of Nutrition and Endocrinology, AC, National Association of Cardiologists of Mexico, AC, Mexican Association for the Prevention of Atherosclerosis and its Complications, AC, National Normative Committee of General Medicine, AC, National College of Geriatric Medicine, AC, College of Internal Medicine of Mexico, AC, Mexican Society of Angiology and Vascular and Endovenous Surgery, AC, Mexican Institute of Research Nephrological, AC and the Mexican Academy of Neurology, A.C.; with the methodological support of the Ibero-American Agency for the Development and Evaluation of Health Technologies, in order to establish recommendations based on the best available evidence and agreed upon by an interdisciplinary group of experts. The objective of this document is to provide evidence-based recommendations to help decision makers in the diagnosis and treatment of dyslipidemias in our country. Material and methods: This document complies with international quality standards, such as those described by the Institute of Medicine of the USA, the Institute of Clinical Excellence of Great Britain, the Scottish Intercollegiate Guideline Network and the Guidelines International Network. A multidisciplinary group of clinical experts and methodologists with experience in systematic reviews of the literature and the development of clinical practice guidelines was formed. A scope document was agreed upon, relevant clinical questions were established, the best available evidence critically evaluated in systematic literature reviews was exhaustively identified, and clinical recommendations were developed. The modified Delphi Panel methodology was used to achieve an adequate level of consensus in each of the recommendations contained in this CPG. Results: 23 clinical questions were agreed upon which gave rise to their respective clinical recommendations. Conclusions: We consider that this document contributes to better clinical decision-making and becomes a point of reference for clinicians and patients in the management of dyslipidemias and this contributes to reducing the morbidity and mortality derived from atherosclerotic cardiovascular events in our country.

Ultraviolet radiation and generally recognized as safe (GRAS) preservatives for inactivation of Aspergillus niger in vitro and corn dough

Abstract Corn is the main cereal produced in the world, it is also used for direct human consumption and for the production of various food products; however, it is prone to being contaminated by fungi, especially by mycotoxin producers. Aspergillus spp. is a contaminant fungus related to postharvest of stored grains, especially in corn. This study evaluated the effect of Ultraviolet radiation (UV) and Generally Recognized As Safe (GRAS) treatments on the inhibition of mycelial growth of A. niger (GIBI_00056) in vitro and in corn (Zea mays L.) dough. For the in vitro study, UV radiation and solutions of citric acid, potassium sorbate, sodium bicarbonate, sodium benzoate, and ascorbic acid were used, which were added to PDA agar in which A. niger was inoculated and evaluated at 24, 48, and 72 h. Subsequently, the best treatment was selected and applied in different concentrations in an A. niger inoculated corn dough, thus evaluating the incidence of contamination at 24, 48, and 72 h. The sodium bicarbonate and sodium benzoate solutions had the best effect on the inhibition of A. niger in vitro compared to the control, whereas the other treatments evaluated did not show differences in the mycelial inhibition. In the corn dough inoculated with A. niger, the effect of sodium bicarbonate depended on the concentration used; the lowest incidence of contamination of the microorganism at 72 h was 0% with 1.8 and 2.7% (w/w) of sodium bicarbonate, whereas the highest was for the control with 100% incidence. The potential of sodium bicarbonate to inactivate A. niger growth in vitro and corn dough was observed.
Resumo O milho é o principal cereal produzido no mundo e é utilizado para o consumo humano direto e para a produção de diversos produtos alimentícios. No entanto, está sujeito a ser contaminado por fungos, especialmente por produtores de micotoxinas. Aspergillus spp. É um fungo contaminante relacionado à pós-colheita de grãos armazenados, principalmente nos grãos de milho. Este estudo avaliou o efeito da radiação ultravioleta (UV) e de tratamentos geralmente reconhecidos como seguros (GRAS) na inibição do crescimento micelial de Aspergillus niger (GIBI_00056) in vitro e em massa de milho (Zea mays). Para o estudo in vitro, foram utilizadas a radiação UV e as soluções de ácido cítrico, sorbato de potássio, bicarbonato de sódio, benzoato de sódio e ácido ascórbico, que foram adicionadas ao meio de cultura BDA, no qual A. niger foi inoculado sendo avaliado nos períodos de 24, 48 e 72 h. Posteriormente, o melhor tratamento foi selecionado e aplicado em diferentes concentrações em uma massa de milho inoculada com A. niger, avaliando a incidência de contaminação nos períodos de 24, 48 e 72 h. As soluções de bicarbonato de sódio e benzoato de sódio tiveram o melhor efeito na inibição de A. niger in vitro, em relação ao controle, enquanto os demais tratamentos avaliados não apresentaram diferenças na inibição micelial. Na massa de milho inoculada com A. niger, o efeito do bicarbonato de sódio dependeu da concentração utilizada: a menor incidência de contaminação do microrganismo em 72 h foi de 0% com 1,8 e 2,7% (p/p) de bicarbonato de sódio, enquanto a maior foi para o controle, com 100% de incidência. Foi observado o potencial do bicarbonato de sódio para inativar o crescimento de A. niger in vitro e na massa de milho.

ABSTRACT Background: Pediatric Multisystemic Inflammatory Syndrome (SIMP) associated with SARS CoV2 seriously affects children. Objective: To describe the symptoms, treatment and evolution of SIMP in patients of a reference Pediatric Intensive Care Unit (PICU). Findings: 18 children were identified with a mean age of 8.76 years, 50% male, all with a positive serological test. The most frequent manifestations were: fever (100%), respiratory distress (94%), abdominal pain (89%), elevated C-reactive protein (average 21.8), thrombocytopenia (50%), pathological chest X-ray (89%) and depressed myocardial contractility (61%). 72% required invasive mechanical ventilation (average 3 days) and catecholamines; 89% used corticosteroids plus immunoglobulins. The average stay in the PICU was 5.5 days. One patient died of intracerebral hemorrhage. Conclusions: SIMP in PICU is characterized by fever, respiratory distress, abdominal pain, elevated inflammatory markers, and myocardial depression. Requiring corticosteroids, immunoglobulin and ventilatory support; presenting intermediate stay and low mortality.
RESUMEN Introducción: El Sindrome Inflamatorio Multisistémico Pediatrico (SIMP) asociado a SARS CoV2 afecta gravemente a niños. Objetivo: Describir clínica, tratamiento y evolución del SIMP en pacientes de una Unidad de Cuidados Intensivos Pediátricos (UCIP) de referencia. Hallazgos: Se identificaron 18 niños con edad promedio de 8.76 años, 50% masculino, todos con prueba serológica positiva. Las manifestaciones más frecuentes fueron: Fiebre (100%), dificultad respiratoria (94%), dolor abdominal (89%), Proteina C reactiva elevada (promedio 21.8), plaquetopenia (50%), Radiografía de tórax patológica (89%) y contractilidad miocárdica deprimida (61%). El 72% requirió ventilación mecánica invasiva (promedio 3 días) y catecolaminas; 89% usó corticoides más inmunoglobulinas. La estancia promedio en UCIP fue 5.5 días. Un paciente falleció por hemorragia intracerebral. Conclusiones: El SIMP en UCIP se caracteriza por fiebre, dificultad respiratoria, dolor abdominal, marcadores inflamatorios elevados y depresión miocárdica. Requiriendo corticoides, inmunoglobulina y soporte ventilatorio; presentando estancia intermedia y baja mortalidad.

ABSTRACT We carried out an observational, retrospective and descriptive study in order to identify the clinical and epidemiological characteristics of children with SARS-CoV-2 infection admitted to a Peruvian national referral hospital. We included patients from one month old to fourteen years old hospitalized between March and August 2020. A total of 125 patients with SARS-CoV-2 infection were admitted, 18.4% (n = 23) had critical illness and 16.8% (n = 21) had multisystem inflammatory syndrome (MIS-C). The absence of comorbidities and previous history of epidemiological contact were more frequent in patients with MIS-C. Patients in critical condition and patients with MIS-C had lower lymphocyte and platelet counts, and higher C-reactive protein, ferritin and D-dimer values than patients who did not have said conditions. Six (4.8%) out of 125 children died, as well as 3 (13%) children from the group of patients in critical condition. None of the children with MIS-C died.
RESUMEN Con el objetivo de conocer las características clínicas y epidemiológicas de niños con infección por SARS-CoV-2 internados en un hospital peruano de referencia nacional realizamos un estudio observacional, retrospectivo y descriptivo e incluimos pacientes de un mes a catorce años hospitalizados entre marzo a agosto del 2020. Se ingresaron 125 pacientes con infección por SARS-CoV-2, el 18,4% (n = 23) presentaron enfermedad crítica y 16,8% (n = 21) síndrome inflamatorio multisistémico (SIM). En los pacientes con SIM fue más frecuente la ausencia de comorbilidades y el antecedente de contacto epidemiológico. Tanto el grupo en estado crítico como del grupo con SIM, en comparación con los que no tuvieron estas condiciones, presentaron menores recuentos de linfocitos y plaquetas, y mayores valores de proteína C reactiva, ferritina y dímero D. Seis (4,8%) niños de los 125 fallecieron, 3 (13%) del grupo en estado crítico y ninguno del grupo con SIM.

Abstract Recent studies have reported the occurrence of thrombotic phenomena or coagulopathy in patients with COVID-19. There are divergent positions regarding the prevention, diagnosis, and treatment of these phenomena, and current clinical practice is based solely on deductions by extension from retrospective studies, case series, observational studies, and international guidelines developed prior to the pandemic. In this context, the aim was to generate a group of recommendations on the prevention, diagnosis and management of thrombotic complications associated with COVID-19. Methods: A rapid guidance was carried out applying the GRADE Evidence to Decision (EtD) frameworks and an iterative participation system, with statistical and qualitative analysis. Results: 31 clinical recommendations were generated focused on: a) Coagulation tests in symptomatic adults with suspected infection or confirmed SARS CoV-2 infection; b) Thromboprophylaxis in adults diagnosed with COVID-19 (Risk scales, thromboprophylaxis for outpatient, in-hospital management, and duration of thromboprophylaxis after discharge from hospitalization), c) Diagnosis and treatment of thrombotic complications, and d) Management of people with previous indication of anticoagulant agents. Conclusions: Recommendations of this consensus guide clinical decision-making regarding the prevention, diagnosis, and treatment of thrombotic phenomena in patients with COVID-19, and represent an agreement that will help decrease the dispersion in clinical practices according to the challenge imposed by the pandemic.

Abstract Introduction: recent studies have reported the occurrence of thrombotic phenomena or coagulopathy in patients with COVID-19. There are divergent positions regarding the prevention, diagnosis, and treatment of these phenomena, and current clinical practice is based solely on deductions by extension from retrospective studies, case series, observational studies, and international guidelines developed prior to the pandemic. Objective: to generate a group of recommendations on the prevention, diagnosis and management of thrombotic complications associated with COVID-19. Methods: a rapid guidance was carried out applying the GRADE Evidence to Decision (EtD) frameworks and an iterative participation system, with statistical and qualitative analysis. Results: 31 clinical recommendations were generated focused on: a) Coagulation tests in symptomatic adults with suspected infection or confirmed SARS CoV-2 infection; b) Thromboprophylaxis in adults diagnosed with COVID-19 (Risk scales, thromboprophylaxis for outpatient, in-hospital management, and duration of thromboprophylaxis after discharge from hospitalization), c) Diagnosis and treatment of thrombotic complications, and d) Management of people with previous indication of anticoagulant agents. Conclusions: recommendations of this consensus guide clinical decision-making regarding the prevention, diagnosis, and treatment of thrombotic phenomena in patients with COVID-19, and represent an agreement that will help decrease the dispersion in clinical practices according to the challenge imposed by the pandemic.

Funcionalidad familiar y ansiedad en pacientes adultos con enfermedad Crónica

Objetivo. Describir el grado de relación entre la funcionalidad familiar y el nivel de ansiedad en pacientes adultos con enfermedad crónica de una institución de salud de segundo nivel. Metodología. El diseño del estudio fue descriptivo, transversal, correlacional, se efectuó en una muestra de 97 pacientes y una misma cantidad de familiares; los primeros se seleccionaron mediante un análisis de potencia, cubriendo los siguientes criterios de inclusión: pacientes adultos, orientados, de los servicios de hospitalización y consulta externa de una institución de segundo nivel de atención. Se utilizó una cédula sociodemográfica, la Escala de Efectividad en el Funcionamiento Familiar de Friedemann (E-EFF), y la escala de ansiedad-rasgo de Spielberger, estos últimos con una consistencia interna de ,83 y ,75 respectivamente, determinada por el alpha de Cronbach. Los datos se analizaron a través de estadística descriptiva e inferencial. Resultados. El grupo de edad que predominó en los familiares fue de 33 a 47 años, con una media de 45,73, y en los pacientes predominó el grupo de 60 a 70 años, con una media de 65,05; en ambos grupos se destacó el género femenino. El 56% de los pacientes presenta diabetes mellitus tipo 2. En cuanto a los índices, la ansiedad-rasgo obtuvo una media de 51 (DE 10,7), en la E-EFF fue de 84 (DE 11,5). Por tanto, se detectó nivel moderado de ansiedad en el 92% de los pacientes, y el 91% integra una familia funcional. La correlación de Spearman no arrojó una asociación significativa entre las dos variables del estudio (rho= -,094, p= ,362). Conclusión. Las cuatro subescalas de la E-EFF obtuvieron una media significativa, con valores que fluctúan entre 77,0 a 92,0. Este último valor equivale a la subescala de coherencia que, de acuerdo con Friedemann, guía a la unidad y pertenencia familiar, y permite vínculos emocionales positivos.
Objective. Describe the degree of relationship between family functioning and the level of anxiety in adult patients with chronic disease in a health institution at the second level. Methodology. The study design was descriptive, cross correlation, was performed on a sample of 97 patients and an equal number of families, the first ones were selected with a potential analysis, covering the following inclusion criteria: adult-oriented patients about the services and outpatient consultation of an institution at second level of care. We used a sociodemographic card, the scale of effective family functioning of Friedemann (E-EFF) and the scale of Spielberger trait-anxiety, the latter with an internal consistency of .83 and .75, respectively, as determined by Alpha Cronbach. Data were analyzed using descriptive and inferential statistics. Results. The age group that prevailed in the family was from 33 to 47 years, a mean of 45.73, and in the patients, was the group of 60 to 70 years, an mean of 65.05; in both groups stressed the female gender. 56% of patients had diabetes mellitus type 2. As for the indices, trait-anxiety had a mean of 51 (10.7) in the EFF-E was 84 (11.5). Therefore, there was moderate level of anxiety in 92% of the patients and 91% integrating a functional family. The Spearman correlation showed no significant association between the two variables of the study (rho = -. 094, p = .362). Conclusion. The four subscales of the E-half EFF obtained significant values ranging from 77.0 to 92.0. The latter value is equivalent to the subscale of consistency, according to Friedemann, guide to family unity and membership also allows positive emotional bonds.

Nós usamos um cartão sócio-demográfico, a Escala de Efetividade no Funcionamento Familial, de Friedemann (E-EFF), e a escala de ansiedade- traço de Spielberger; estes últimos com uma consistência interna de ,83 e ,75, respectivamente, determinada pelo alfa de Cronbach. Os dados foram analisados através de estatística descritiva e inferencial. Resultados. A faixa etária predominante nos familiares foi de 33 a 47 anos, com uma média de 45,73; nos os pacientes predominou o grupo de 60 a 70 anos, com uma média de 65,05. Em ambos os grupos sobressaiu o sexo feminino. O 56% dos pacientes têm diabetes mellitus tipo 2. Para índices, a ansiedade-traço marcou uma média de 51 (DE 10,7); na E-EFF foi 84 (DP 11,5). O 92% dos pacientes apresentou nível moderado de ansiedade; o 91% conforma uma família funcional. A correlação de Spearman não mostrou associação significativa entre as duas variáveis do estudo (rho = -, 094, p =, 362). Conclusão. As quatro subescalas da E-EFF apresentam uma media significativa, com valores oscilando de 77,0 a 2,0. Este valor equivale à subescala de coerência que, de acordo com Friedemann, orienta a unidade familiar e a pertencia familiar, e permite vínculos emocionais positivos.

A study was made to determine the mechanisms involved in the beneficial effect of an antioxidant mixture on the treatment of hyperthyroidism. Plasmic and urinary TBARS, levels of long-life circulating hormones and reactive species in urine were determined by applying a novel detecting method. Oxidative stress may be related to symptoms rather than to level of circulating hormones in hyperthyroid patients
Se realizó un estudio para dilucidar los mecanismos involucrados en el efecto beneficioso de una mezcla antioxidante en el tratamiento del hipertiroidismo; se determinaron: TBARS plasmáticas y urinarias, niveles de hormonas circulantes y especies reactivas de larga vida en orina, aplicando un método novel de detección. En los pacientes hipertiroideos, el estrés oxidativo estaría relacionado con los síntomas, pero no con el nivel de hormonas circulante
