Abstract Introduction: Spinal injuries are common among young adult men. Recognizing kinematics can help reduce the number of late diagnoses, especially if there are consciousness disorders. Vertebral fractures can be single or multiple, and they are often associated with extravertebral injuries. Objectives: To examine the distribution of the lesion based on the mechanism of production, to characterize the neurological condition, to evaluate the lesion pattern and its relationship with extravertebral lesions, and to analyze the treatment. Materials and Methods: This was a multicenter, prospective study of patients admitted with post-trauma vertebral injuries between July 1, 2018 and June 30, 2020. Age, gender, kinematics, neurological condition, affected sector, pattern of injury, and associated extravertebral injuries were all examined. Results: There were 281 patients (60% men) evaluated, with 400 vertebral and 118 extravertebral lesions. The causes were as follows: polytrauma in 62 cases, falls from great heights in 147 patients, and traffic accidents in 98. ASIA E was the most frequently observed neurological picture (8 cases), which was not determined at admission. The T2-L5 sector was the most affected, mostly by compression injuries. Head and chest trauma were the most common extravertebral injuries; there was one case of SCIWORA and one early death. Conclusions: The most commonly affected spinal sectors were thoracolumbar, thoracic, and lumbar; the injuries are typically caused by falls from great heights and, in general, are isolated, with no neurological injuries. The treatment is determined by the stability and neurological condition.

Resumen Introducción: Las lesiones raquídeas son frecuentes en hombres adultos jóvenes. Reconocer la cinemática ayuda a disminuir la tasa de diagnósticos tardíos, principalmente si hay trastornos de la conciencia. Las fracturas vertebrales pueden ser únicas o múltiples, y asociarse con lesiones extravertebrales. Los objetivos de este estudio fueron analizar la distribución de la lesión según el mecanismo de producción, caracterizar el cuadro neurológico, evaluar el patrón de lesión y la asociación con lesiones extravertebrales, y analizar el tratamiento. Materiales y Métodos: Estudio multicéntrico, prospectivo de pacientes con lesiones vertebrales postrauma, que ingresaron entre el 1 de julio de 2018 y el 30 de junio de 2020. Se analizaron los siguientes parámetros: edad, sexo, cinemática, cuadro neurológico, sector afectado, patrón de lesión, lesiones extravertebrales asociadas. Resultados: Se evaluó a 281 pacientes (60% hombres) con 400 lesiones vertebrales y 118 extravertebrales que, en 62 casos, conformaban un cuadro de politraumatismo; 147 con trauma por caída de altura y 98, por accidente de tránsito. El cuadro neurológico más observado fue ASIA E (8 casos), no determinado al ingreso. El sector T2-L5 fue el más afectado, en su mayoría, por lesiones por compresión. Las lesiones extravertebrales más frecuentes fueron el trauma de cráneo y de tórax; hubo un caso de SCIWORA y un óbito temprano. Conclusiones: Los sectores raquídeos más afectados fueron: el toracolumbar, el torácico y el lumbar; las lesiones suelen deberse a caídas de altura y suelen ser únicas, sin cuadro neurológico. El tratamiento se decide según la estabilidad y el cuadro neurológico.

RESUMO Introdução: O abscesso peridural piogênico da coluna (PSEA) é de difícil diagnóstico e pode ter consequências devastadoras. A ressonância magnética (RM) tem alta sensibilidade e especificidade, que são aumentadas ainda mais com contraste. Existem várias classificações dos processos infecciosos vertebrais, com ênfase em espondilodiscite. Objetivo: Analisar os parâmetros morfológicos e sua reprodutibilidade; e analisar as diferentes sequências de imagens de ressonância. Métodos: A partir de um banco de imagens, foi planejada uma classificação morfológica do PSEA, com cinco parâmetros: Região (R) que indica os limites superior e inferior do abscesso; Localização (U), indicando se o abscesso é anterior ou posterior dentro do canal; Comprometimento (C), meníngeo ou de conteúdo das estruturas; Associação (A), discite, osteomielite ou ambas e Perivertebral (P), abscesso extravertebral anterior, lateral ou posterior. Os três primeiros parâmetros dão uma ideia do volume da PSEA, enquanto os dois últimos mostram os focos infecciosos relacionados. Trinta e cinco casos foram analisados usando o coeficiente de Kappa. Resultados: A reprodutibilidade global intra e interobservador foi Kappa 0,81. Os resultados para cada parâmetro foram os seguintes: R = 0,95, U = 0,92, C = 0,66, A = 0,70 e P = 0,80. Os três primeiros dão uma noção de volume e os dois últimos se relacionam com a presença de focos infecciosos vertebrais fora do canal. A RM ponderada em T2 com contraste foi considerada a sequência de imagens mais eficaz. Conclusões: A classificação morfológica é simples de usar e tem excelente reprodutibilidade. Os parâmetros com maior reprodutibilidade foram região e localização com valores > 0,92. A adição do contraste de gadolínio aumentou a sensibilidade do diagnóstico; o uso de imagens sagitais e axiais em RM T2 foi a sequência de imagens mais sensível. Nível de Evidência III; Original.

ABSTRACT Background: Pyogenic Spinal Epidural Abscess (PSEA) is difficult to diagnose and can have devastating consequences. Magnetic Resonance Imaging (MRI) has high sensitivity and specificity, which are further increased with the use of contrast. There are several classifications of vertebral infectious processes, with emphasis on spondylodiscitis. Objective: To analyze the morphological parameters and their reproducibility; and to analyze different resonance imaging sequences. Methods: Using an image database, a morphological classification of PSEA was planned, with five parameters: Region (R), indicating the upper and lower limits of the abscess; Location (U), indicating whether the abscess is anterior or posterior within the canal; Compromise (C), meningeal or content of the structures; Association (A), discitis, osteomyelitis or both; and Perivertebral (P), anterior, lateral or posterior extravertebral abscess. The first three parameters give an idea of the volume of the PSEA, while the last two give the related infectious foci. Thirty-five cases were analyzed using Kappa’s coefficient. Results: The global intra- and interobserver reproducibility was Kappa 0.81. The results for each parameter were as follows: R=0.95, U=0.92, C=0.66, A=0.70 and P=0.80. The first three give a notion of volume and the last two relate to the presence of vertebral infectious foci outside the canal. T2 weighted MRI with contrast was found to be the most effective imaging sequence. Conclusion: The morphological classification is simple to use, with excellent reproducibility. The parameters with the highest reproducibility were region and location, with values >0.92. The addition of gadolinium contrast increased the sensitivity of the diagnosis; the use of sagittal and axial images in T2-MRI was the most sensitive imaging sequence. Evidence Level III; Original.

RESUMEN Introducción: El absceso epidural espinal piógeno (PSEA) es difícil de diagnosticar y puede tener consecuencias devastadoras. La resonancia magnética (RM) tiene una alta sensibilidad y especificidad, que aumentan aún más con el contraste. Existen varias clasificaciones de los procesos infecciosos vertebrales, con énfasis en la espondilodiscitis. Objetivo: Analizar los parámetros morfológicos y su reproducibilidad; y analizar las diferentes secuencias de imágenes de resonancia. Métodos: Utilizando un banco de imágenes, se planificó una clasificación morfológica del PSEA, con 5 parámetros: Región (R), que indica los límites superior e inferior del absceso; Ubicación (U), que indica si el absceso es anterior o posterior dentro del canal; Compromiso (C), meníngeo o contenido de las estructuras; Asociación (A), discitis, osteomielitis o ambas; y absceso perivertebral (P), anterior, lateral o posterior extravertebral. Los tres primeros parámetros dan una idea del volumen del PSEA y los dos últimos los focos infecciosos relacionados. Se analizaron 35 casos mediante el coeficiente Kappa. Resultados: La reproducibilidad general intra e interobservador fue Kappa 0,81. Los resultados de cada parámetro fueron los siguientes:: R=0,95, U=0,92, C=0,66, A=0,70 y P=0,80. Los tres primeros dan una noción de volumen y los dos últimos se relacionan con la presencia de focos infecciosos vertebrales fuera del canal. La resonancia magnética ponderada en T2 con contraste se consideró la secuencia de imágenes más eficaz. Conclusiones: La clasificación morfológica es sencilla de utilizar y tiene una excelente reproductividad. Los parámetros con mayor reproducibilidad fueron la región y ubicación con valores > 0,92. La adición de contraste de gadolinio aumentó la sensibilidad del diagnóstico; el uso de imágenes sagitales y axiales en RM T2 fue la secuencia de imágenes más sensible. Abstract Introduction: The numbers of SARS-CoV-2 infection in the pediatric population are low so far. There is limited information about the behavior of SARS-CoV-2 in a pediatric patient with chronic kidney disease. Objective: To formulate informed recommendations to the prevention, diagnosis, and management of SARS-CoV-2 infection in pediatric patients with kidney disease or acute kidney injury associated with COVID-19 in Colombia. Methodology: A rapid systematic review was performed in Embase and Pubmed databases and scientific societies, to answer questions prioritized by clinical experts in pediatric nephrology. The quality of the evidence was evaluated with validated tools according to the type of study. The preliminary recommendations were consulted by an expert group. The agreement was defined when approval was obtained from at least 70% of the experts consulted. Results: A response was obtained from ' 9 experts in pediatric nephrology in Colombia, who declared the conflict of interest before the consultation. The range of agreement for the recommendations ranged from 78.9% to '00%. The recommendations did not require a second consultation. Conclusion: The evidence-based recommendations for the management of a patient with kidney disease and COVID-19 in the Colombian context are presented.

RESUMO Objetivo: Apresentar caso clínico e atualizar bibliografia. Material e Método: Masculino, 24 anos de idade, consultado devido a lombociatalgia direita com três anos de evolução, com topografia L5 e déficit motor (M4). A radiografia mostra uma lesão radiopaca entre a quarta e a quinta vértebras lombares, com obliteração do pedículo direito de L4. A tomografia identifica uma lesão lítica parcialmente circundada por esclerose, com um ninho central de três centímetros de diâmetro localizado no pedículo direito com acometimento do processo transverso e reação do espaço intertransversário (Enneking 3). É complementado com ressonância magnética e cintilografia óssea, biópsia percutânea guiada por tomografia diagnóstica: osteoblastoma e focos de necrose. A ressecção radical foi realizada em bloco, com margens livres do tumor e estabilização instrumentada. Resultados: Após o tratamento cirúrgico o paciente evoluiu favoravelmente, revertendo o déficit motor. O estudo anatomopatológico da peça confirma o resultado pré-operatório. Discussão: A ressecção intralesional pode ser uma opção nos estágios de Enneking 2. Nos estágios de Enneking 3, uma biópsia diagnóstica percutânea pode ser útil e a ressecção em bloco é preferida como um tratamento definitivo. Conclusão: O manejo do osteoblastoma espinal requer uma análise clínica e imagética exaustiva. A ressecção de bloco com margens livres é preferida em casos avançados para o gerenciamento e para diminuir o risco de recorrência. Nível de Evidência IV; Série de casosh.

ABSTRACT Objective: To present the clinical case and update the bibliography. Methods: A male patient, 24 years of age, sought treatment for right lumbosciatalgia of 3 years of evolution with topography L5 and motor deficit (M4). The radiograph showed a radiopaque lesion between the fourth and fifth lumbar vertebrae, with right pedicle effacement of L4. The tomography identified a lytic lesion, partially surrounded by sclerosis with a central nest of 3 centimeters in diameter located in the right pedicle with involvement of the transverse apophysis and reaction of the intertransverse space (Enneking 3). It was complemented by magnetic resonance and bone scintigraphy. The percutaneous biopsy guided by tomography yielded a diagnosis of osteoblastoma and foci of necrosis. A radical block resection was performed with clear tumor margins and instrumented stabilization. Results: After the surgical treatment, the patient evolved favorably, reversing the motor deficit. The anatomopathological study of the specimen confirms the preoperative diagnosis. Discussion: Intralesional resection may be an option in Enneking stage 2. In Enneking stage 3, a percutaneous diagnostic biopsy may be useful, and block resection is the preferred definitive treatment. Conclusions: The management of spinal osteoblastoma requires an exhaustive clinical-imaging analysis. Block resection with clear margins is preferred in advanced cases for management and to decrease the risk of recurrence. Level of Evidence IV; Case seriesh.

RESUMEN Objetivo: Presentar un caso clínico y actualizar bibliografía. Métodos: Un paciente de sexo masculino, 24 años de edad, buscó tratamiento para lumbociatalgia derecha de 3 años de evolución, con topografía L5 y déficit motor (M4). La radiografía mostró lesión radiopaca entre la cuarta y quinta vértebra lumbar, con borramiento pedicular derecho de L4. La tomografía identificó lesión lítica rodeada parcialmente de esclerosis, con nido central de 3 centímetros de diámetro ubicada en el pedículo derecho con afectación de la apófisis transversa y reacción del espacio intertransversario (Enneking 3). La tomografía fue complementada con resonancia magnética y gammagrafía ósea. La biopsia percutánea guiada por tomografía produjo diagnóstico de osteoblastoma y focos de necrosis. Se llevó a cabo la resección radical en bloque con márgenes libres del tumor y estabilización instrumentada. Resultados: Luego del tratamiento quirúrgico, el paciente ha evolucionado favorablemente, revirtiendo el déficit motor. El estudio anatomopatológico de la pieza confirma el diagnóstico preoperatorio. Discusión: La resección intralesional puede ser una opción en los estadios Enneking 2. En los estadios Enneking 3, puede ser útil la realización de una biopsia percutánea diagnostica, y como tratamiento definitivo se prefiere la resección en bloque. Conclusiones: El manejo del osteoblastoma espinal requiere un exhaustivo análisis clínico-imagenológico. La resección en bloque con márgenes libres es preferida en los casos avanzados para el manejo y disminuir el riesgo de recidivas. Nivel de Evidencia IV; Serie de casosh.

Em 1978 Christopher T. Russell e Richard C. Elphic detectaram tubos de fluxo magnético na região de fronteira da magnetosfera terrestre por meio de dados observacionais obtidos pelos satélites ISEE-1 e −2, que foram denominados eventos de transferência de fluxo, ou simplesmente FTE (do inglês, flux transfer events). O vento solar, juntamente com o campo magnético interplanetário, oriundos do Sol, atingem a região de campo magnético terrestre, produzindo fenômenos como a reconexão magnética e os tubos de fluxo. Esse artigo apresenta uma introdução sobre a conexão Sol-Terra e o processo de reconexão magnética, e principalmente, os conceitos básicos sobre a geração e a formação dos tubos de fluxo magnético. Os modelos que descrevem o tipo de reconexão e as características dos tubos, como por exemplo Russell-Elphic, Lee-Fu e Southwood-Scholer, e as propriedades dos eventos FTE, são apresentados de forma clara e concisa. Por fim, o artigo trata de um exemplo de identificação dos tubos em simulação computacional, realizado nesse trabalho.

In 1978 Christopher T. Russell and Richard C. Elphic detected magnetic flux tubes in magnetosphere boundary region through observational data obtained from the satellites ISEE-1 and −2, which were named flux transfer events (FTE). The solar wind together with the interplanetary magnetic field, both originated in the Sun, reach the earth magnetic field region, producing the magnetic reconnection and the flux tubes phenomena. This paper presents an introduction on the Sun-Earth connection and the magnetic reconnection process, and mainly the basic concepts on the tubes generation and formation. The models which describe the type of reconnection and the tubes signatures, for example, Russell-Elphic, Lee-Fu, Southwood-Scholer, and the FTE event characteristics, are presented in a clear and concise way. Finally, this paper brings an example of the tubes identification in computing simulation, performed in this work.

Cuba did not constitute exception rule regarding outstanding work of medical personalities. The evidence in each period of national history shows that development always were public health professionals at different times. Medical education was present in our country since the beginning of the colonial period wore on for different periods with an evolutionary character, experiencing essential modifications in the course. You could not talk about Cuban medical education without the presence along the history of major medical - educators responsible for the different systems engineer teachings. Fidel llizástigui was Professor Dupuy, paradigm of clinical sciences, and more than that, the most significant teacher of Cuban medicine. That is why a review of 14 references were made in order to argue the importance of the work of Professor Dupuy Ilizátigui depending on the training of health professionals.

No constituyó Cuba excepción de regla en cuanto a destacada labor de personalidades médicas. La evidencia en cada período de historia nacional demuestra que siempre hubo desarrollo salubrista en las diferentes épocas. La educación médica estuvo presente en nuestro país desde el comienzo de la etapa colonial transcurriendo por diferentes períodos con un carácter evolutivo, experimentando modificaciones esenciales para el perfeccionamiento. No se pudiera hablar de educación médica cubana sin la presencia a lo largo de la historia de grandes médicos - pedagogos encargados de ingeniar los diferentes sistemas de enseñanzas. Fue el profesor Fidel llizástigui Dupuy, paradigma de las ciencias clínicas, y más que ello, el pedagogo más trascendental de la medicina cubana. Es por ello que se realizó una revisión de 14 referencias bibliográficas con el objetivo de argumentar la importancia de la obra del profesor Ilizátigui Dupuy en función de la formación de profesionales en salud.

INTRODUCTION: The South American country Chile now boasts a life expectancy of over 80 years. As a consequence, Chile now faces the increasing social and economic burden of cancer and must implement political policy to deliver equitable cancer care. Hindering the development of a national cancer policy is the lack of comprehensive analysis of cancer infrastructure and economic impact. OBJECTIVES: Evaluate existing cancer policy, the extent of national investigation and the socio-economic impact of cancer to deliver guidelines for the framing of an equitable national cancer policy. METHODS: Burden, research and care-policy systems were assessed by triangulating objective system metrics -epidemiological, economic, etc. - with political and policy analysis. Analysis of the literature and governmental databases was performed. The oncology community was interviewed and surveyed. RESULTS: Chile utilizes 1% of its gross domestic product on cancer care and treatment. We estimate that the economic impact as measured in Disability Adjusted Life Years to be US$ 3.5 billion. Persistent inequalities still occur in cancer distribution and treatment. A high quality cancer research community is expanding, however, insufficient funding is directed towards disproportionally prevalent stomach, lung and gallbladder cancers. CONCLUSIONS: Chile has a rapidly ageing population wherein 40% smoke, 67% are overweight and 18% abuse alcohol, and thus the corresponding burden of cancer will have a negative impact on an affordable health care system. We conclude that the Chilean government must develop a national cancer strategy, which the authors outline herein and believe is essential to permit equitable cancer care for the country.

Introducción: El dolor en el posquirúrgico inmediato ha sido subvalorado y manejado inadecuadamente. Objetivos: Evaluar la percepción del dolor a las 4h del postoperatorio de pacientes del Hospital Universitario San Jorge de Pereira (Colombia). Materiales y métodos: Estudio de corte transversal en pacientes mayores de 18 años entre el 2 de septiembre y el 28 de octubre de 2011. Se valoró la intensidad del dolor postoperatorio mediante escala visual analógica a las 4h del procedimiento. Se consideraron variables sociodemográficas, clínicas y farmacológicas. El análisis se hizo con SPSS 20.0 para Windows. Resultados: Se evaluaron 213 pacientes en postoperatorio, 114 (53,6%) mujeres y 99 (46,4%) hombres, con edad promedio de 47,1 ± 20,0 años. El 51,4% de los pacientes no tenía controlado el dolor a las 4h. Las variables edad, residencia urbana, tipo de cirugía, incumplimiento de la dosis y monoterapia analgésica se asociaron de manera estadísticamente significativa con la falta de control. Discusión: El inadecuado control del dolor obliga a replantear su manejo idealmente con guías de práctica clínica y con el empleo de medicamentos analgésicos a las dosis e intervalos adecuados.

Introduction: Immediate postoperative pain has been underestimated and managed inadequately. Objectives: To assess perceived pain 4 h after surgery in patients at the San Jorge University Hospital in the city of Pereira. Materials and methods: Cross-sectional study in patients over 18 years of age was conducted between September 2 nd and October 28th, 2011. Postoperative pain intensity was assessed using the Visual Analog Scale, 4 h after completing the procedure. Social, demographic clinical and pharmacological variables were considered. The analysis was done using the SPSS 20.0 for Windows. Results: Of the 213 postoperative patients studied, 114 (53.6%) were women and 99 (46.4%) were men, with a mean age of 47.1 ± 20.0 years. At 4 h, 51.4% of patients did not have pain control. There was a statistically significant association between lack of control and age, living in the urban area, type of surgery, non-adherence to the dose, and monotherapy analgesia. Discussion: Inadequate pain control requires revisiting its management, ideally on the basis of clinical practice guidelines and using analgesic drugs at adequate doses and intervals.
