ABSTRACT: Parasitic intestinal infections in dogs represent a problem for human health, because a wide variety of these parasites have zoonotic potential. Therefore, proximity to pets puts us at risk. The objective of this study was to determine the frequency and risk factors (age, sex, size, breed, presence of ectoparasites and gastrointestinal disorders) of intestinal parasites in the feces of dogs attending a Veterinary Hospital in the City of Mexicali, Baja California, Mexico. A total of 148 fecal samples were collected from canine patients and analyzed for parasite identification and parasite load. A 12.2% (18/148) of the samples were positive to parasitic intestinal infections. The frequency of specific infections was an 8.1% of Cryptosporidium sp., followed by a 2.7% of Cystoisospora sp., and 1.4% of Toxascaris leonina. A statistical significance was identified between the presence of intestinal parasites and mongrel breed. The predominance of protozoa shows the importance of diagnosis prior to treatment with anthelminthic drugs, since preventive antiparasitic protocols are commonly used, although these particular parasites are out of the spectrum of those drugs. Cryptosporidium spp. have zoonotic potential, particularly in immunocompromised patients, and there are few or no treatment options.

Abstract Introduction: Spinal injuries are common among young adult men. Recognizing kinematics can help reduce the number of late diagnoses, especially if there are consciousness disorders. Vertebral fractures can be single or multiple, and they are often associated with extravertebral injuries. Objectives: To examine the distribution of the lesion based on the mechanism of production, to characterize the neurological condition, to evaluate the lesion pattern and its relationship with extravertebral lesions, and to analyze the treatment. Materials and Methods: This was a multicenter, prospective study of patients admitted with post-trauma vertebral injuries between July 1, 2018 and June 30, 2020. Age, gender, kinematics, neurological condition, affected sector, pattern of injury, and associated extravertebral injuries were all examined. Results: There were 281 patients (60% men) evaluated, with 400 vertebral and 118 extravertebral lesions. The causes were as follows: polytrauma in 62 cases, falls from great heights in 147 patients, and traffic accidents in 98. ASIA E was the most frequently observed neurological picture (8 cases), which was not determined at admission. The T2-L5 sector was the most affected, mostly by compression injuries. Head and chest trauma were the most common extravertebral injuries; there was one case of SCIWORA and one early death. Conclusions: The most commonly affected spinal sectors were thoracolumbar, thoracic, and lumbar; the injuries are typically caused by falls from great heights and, in general, are isolated, with no neurological injuries. The treatment is determined by the stability and neurological condition.

Resumen Introducción: Las lesiones raquídeas son frecuentes en hombres adultos jóvenes. Reconocer la cinemática ayuda a disminuir la tasa de diagnósticos tardíos, principalmente si hay trastornos de la conciencia. Las fracturas vertebrales pueden ser únicas o múltiples, y asociarse con lesiones extravertebrales. Los objetivos de este estudio fueron analizar la distribución de la lesión según el mecanismo de producción, caracterizar el cuadro neurológico, evaluar el patrón de lesión y la asociación con lesiones extravertebrales, y analizar el tratamiento. Materiales y Métodos: Estudio multicéntrico, prospectivo de pacientes con lesiones vertebrales postrauma, que ingresaron entre el 1 de julio de 2018 y el 30 de junio de 2020. Se analizaron los siguientes parámetros: edad, sexo, cinemática, cuadro neurológico, sector afectado, patrón de lesión, lesiones extravertebrales asociadas. Resultados: Se evaluó a 281 pacientes (60% hombres) con 400 lesiones vertebrales y 118 extravertebrales que, en 62 casos, conformaban un cuadro de politraumatismo; 147 con trauma por caída de altura y 98, por accidente de tránsito. El cuadro neurológico más observado fue ASIA E (8 casos), no determinado al ingreso. El sector T2-L5 fue el más afectado, en su mayoría, por lesiones por compresión. Las lesiones extravertebrales más frecuentes fueron el trauma de cráneo y de tórax; hubo un caso de SCIWORA y un óbito temprano. Conclusiones: Los sectores raquídeos más afectados fueron: el toracolumbar, el torácico y el lumbar; las lesiones suelen deberse a caídas de altura y suelen ser únicas, sin cuadro neurológico. El tratamiento se decide según la estabilidad y el cuadro neurológico.

ABSTRACT Fused deposition modeling (FDM) has gained popularity recently due to its versatility and low cost. Considering that the quality of printed parts depends on many variables, a 3^3 factorial design of experiments was conducted, where three influential parameters in the process were studied: layer height (0.15, 0.2, and 0.25 mm), infill percentage (30%, 60%, and 90%), and shell count (2, 3, and 4), using polylactic acid as the material. Their effects on tensile strength, flexural strength, printing time, and interactions were measured. The maximum tensile and flexural stress obtained were 33.5 MPa and 87.3 MPa, respectively, at a 90% infill percentage. It was found that layer height does not significantly affect mechanical strength, while infill percentage has the most significant influence, followed by the number of shells. The latter two factors show a meaningful interaction. Furthermore, all the studied parameters have a significant impact on printing time.

Abstract Objective: To study the impact of age and the interval between disease diagnosis and death on the organotropism of SARS-CoV-2. Method: Patients underwent post-mortem biopsies from lungs, Waldeyer ring, heart, liver, kidneys and bone marrow between 2020–2021. SARS-CoV-2 organotropism was mapped by using molecular RT-PCR analyses for SARS-CoV2 targeting the Envelope gene (E), the RNA Polymerase Gene (RdRp), and the Nucleocapsid gene (N). Statistical and linear regression analysis was performed to study the impact of age and illness duration in SARS-CoV-2 organotropism. Results: We performed 158 postmortem biopsies in 21 patients, with a mean age of 76 years old. The mean interval between the diagnosis of the infection to the death was 23 days. The RNA of the SARS-CoV-2 was detected in 100% of lung biopsies, 76%–82% of Waldeyer’s ring biopsies, 55% of heart biopsies, 40% of kidney biopsies, 33% of liver and 25% of bone marrow biopsies. Patients who died before the day 9, presented extensive visceral dissemination of SARS-CoV-2 RNA. Most of the patients older than 80 years (90%) presented visceral dissemination of SARS-CoV-2 RNA, while among younger patients, only 3/11 patients presented visceral dissemination of the virus. The relationship between “age” and “illness duration” and multitropism of the virus was statistically significant (p< 0.001). Conclusion: Disease interval and age were factors that were significantly associated with RT-PCR positive results in multiple organs. Critical COVID-19 patients have multiorganic viral dissemination in early stages. In the critical older patients, multiorganic viral dissemination is the rule. Level of evidence: 4. Case Series.

RESUMEN Se desconocen varios aspectos del desarrollo del fruto de duraznero en condiciones tropicales, información que permite realizar labores agronómicas con criterio técnico, por tanto, el objetivo fue determinar los diferentes cambios fisicoquímicos y fenológicos del fruto de durazno, en función del tiempo térmico, de las variedades ‘Dorado’ y ‘Rubidoux’, cultivados en zonas de trópico alto colombiano. Se seleccionaron al azar 51 árboles y 100 flores/planta, en estado de plena floración. Cada 15 días y hasta la cosecha, se hicieron mediciones de la firmeza, el índice de color de epidermis y pulpa, los sólidos solubles, la acidez titulable y la tasa respiratoria. De plena floración a cosecha, las variedades Dorado y Rubidoux tardaron 1081,8 GDC (153 días) y 1667,1 GDC (205 días), respectivamente. La firmeza presentó un incremento en los dos cultivares durante la fase 1 de desarrollo, luego disminuyó hasta la cosecha. El índice de color de epidermis y pulpa aumentó durante la maduración, con tonalidades amarillas, en las dos variedades. Los sólidos solubles incrementaron de forma continua y en los dos cultivares, con valores finales de 15,9 ± 0,9 y 15,5 ± 0,3 °Brix. La acidez mostró un incremento durante la fase 2 y luego disminuyó durante la maduración. La tasa respiratoria decreció entre la fase 1 y 3, con un incremento en la fase 2, relacionado con la lignificación del endocarpio, en los dos cultivares. Estos resultados contribuyen al entendimiento del desarrollo de los dos cultivares de durazno, bajo condiciones tropicales.

ABSTRACT Several aspects of the development of the peach fruit under tropical conditions are unknown, this information allows agronomic practices to be carried out with technical criteria. Therefore, the objective was to determine the different changes and phenological changes of peach fruit depending on the Growing Degree Days (GDD) in the varieties 'Dorado' and 'Rubidoux', grown in the Colombian high tropics. This study randomly selected 51 trees and 100 flowers per plant that were in full flowering for sampling every 15 days until harvest to determine physical variables such as firmness, color index of the epidermis and pulp, and chemical properties such as soluble solids, titratable acidity, and respiratory rate. From full bloom to harvest, the Dorado and Rubidoux varieties took 1081.8 GDD (153 days) and 1667.1 GDD (205 days) respectively. The firmness increased in the two cultivars during phase 1 of development, then decreased until harvest. The color index of the epidermis and pulp increased during ripening, indicating yellow tones in the two varieties. The soluble solids increased continuously, with final values of 15.9 ± 0.9 and 15.5 ± 0.3 °Brix. The acidity increased during phase 2 and then decreased during ripening. The respiratory rate decreased between phase 1 and 3, with an increase in phase 2, which is related to the lignification of the endocarp in the two cultivars. These results contribute to the understanding of the development of the two peach cultivars under tropical conditions.

Abstract Background: Driver mutations are the genetic components responsible for tumor initiation and progression. These variants, which may be inherited, influence cancer risk and therefore underlie many familial cancers. The present study examines the potential association between SNPs in driver genes SF3B1 (rs4685), TBX3 (rs12366395, rs8853, and rs1061651) and MAP3K1 (rs72758040) and BC in BRCA1/2-negative Chilean families. Methods: The SNPs were genotyped in 486 BC cases and 1258 controls by TaqMan Assay. Results: Our data do not support an association between rs4685:C > T, rs8853:T > C, or rs1061651:T > C and BC risk. However, the rs12366395-G allele (A/G + G/G) was associated with risk in families with a strong history of BC (OR = 1.2 [95% CI 1.0–1.6] p = 0.02 and OR = 1.5 [95% CI 1.0–2.2] p = 0.02, respectively). Moreover, rs72758040-C was associated with increased risk in cases with a moderate-to-strong family history of BC (OR = 1.3 [95% CI 1.0–1.7] p = 0.02 and OR = 1.3 [95% CI 1.0–1.8] p = 0.03 respectively). Finally, risk was significantly higher in homozygous C/C cases from families with a moderate-to-strong BC history (OR = 1.8 [95% CI 1.0–3.1] p = 0.03 and OR = 1.9 [95% CI 1.1–3.4] p = 0.01, respectively). We also evaluated the combined impact of rs12366395-G and rs72758040-C. Familial BC risk increased in a dose-dependent manner with risk allele count, reflecting an additive effect (p-trend = 0.0002). Conclusions: Our study suggests that germline variants in driver genes TBX3 (rs12366395) and MAP3K1 (rs72758040) may influence BC risk in BRCA1/2-negative Chilean families. Moreover, the presence of rs12366395-G and rs72758040-C could increase BC risk in a Chilean population.

Abstract Background: Helicobacter pylori is detected by pathogen recognition receptors including toll–like receptors (TLR) and nucleotide–binding oligomerization domain (NOD)–like receptors, eliciting an innate immune response against this bacteria. The aim of this study was to assess if polymorphisms of TLR2, TLR4, TLR5, NOD1 and NOD2 genes are associated with gastric cancer, in particular in individuals infected with H. pylori. Results: A case–control study of 297 gastric cancer patients and 300 controls was performed to assess the association of 17 polymorphisms. Analyses performed under the allele model did not find association with gastric cancer. However, NOD1 rs2075820 (p.E266K) showed association with intestinal–type gastric cancer among H. pylori infected subjects (OR = 2.69, 95% CI 1.41–5.13, p = 0.0026). The association was not statistically significant in diffuse–type gastric cancer cases (OR = 1.26, 95% CI 0.63–2.52, p = 0.51). When the analyses were performed in patients carrying H. pylori strains harboring the cag pathogenicity island (cagPAI), we noticed significant association with NOD1 rs2075820 (OR = 4.90, 95% CI 1.80–3.36, p = 0.0019), in particular for intestinal–type gastric cancer cases (OR = 7.16, 95% CI 2.40–21.33, p = 4.1 × 10−4) but not among diffuse–type gastric cancer cases (OR = 3.39, 95% CI 1.13–0.10, p = 0.03). Conclusions: NOD1 rs2075820 increases the risk of intestinal–type gastric cancer among individuals infected with H. pylori, particularly in those harboring the cagPAI.

Abstract Introduction. According to the World Health Organization, childhood obesity is one of the most serious worldwide public health problems of the 21st century. In Spain, it is the most frequent nutritional disorder among children from 5 to 15 years old, with a prevalence of 34.1%. Objetives. To analyze in a sample of 50 children with obesity, its main characteristics and its possible association with: family history of obesity, feeding with breastfeeding, accompanying pathologies, presence of weekly physical activity, hours of rest per day, time dedicated to video games, mobiles and the like and type of power. Results. The variables associated with obesity were: being an only child (p= 0.011); Older mothers (p = 0.025); Absence of breastfeeding in the first months of life (p = 0.008); Sleep fewer hours (p = 0.045); Do not consume vegetables or greens (p = 0.019), or fruit (p = 0.033); Girls multiply by 2,611 the consumption of vegetables and the older the greater the consumption of fruit (p = 0.011); 74% ate between meals (snacks). Being a woman and having more than 6 years multiplied by 1,934 and by 12, respectively, the presence of pecking. The older the age, the more pecking (p < 0.001). Conclusions. It is evident that family overweight, without breastfeeding in the first months of life, sleeping fewer hours than recommended for the child's age, and a diet poor in vegetables, fruit and vegetables, pecking between meals, play an important role in childhood obesity.

Resumen Introducción. Según la Organización Mundial de la Salud (OMS) la obesidad infantil constituye uno de los problemas de salud pública a nivel mundial más graves del siglo XXI. En España, es el trastorno nutricional más frecuente entre los niños de 5 a 15 años con una prevalencia del 34,1%. Objetivos. Analizar las características principales de una muestra de niños obesos en una zona de Pediatría de Albacete. Método. Estudio transversal observacional en una muestra de 50 niños con obesidad, sus principales características y su posible asociación con: historia familiar de obesidad, alimentación con lactancia materna, patologías acompañantes, actividad física semanal, horas de descanso al día, tiempo que dedican a los videojuegos, móviles y similares y tipo de alimentación. Resultados. Las variables asociadas a la obesidad fueron: ser hijo único (p=0,011); Madres de mayor edad (p=0,025); Ausencia de lactancia materna en los primeros meses de vida (p=0,008); Dormir menos horas (p=0,045); No consumir verduras ni hortalizas (p=0,019), ni fruta (p=0,033); Las niñas multiplican por 2,611 el consumo de verduras y hortalizas y a mayor edad mayor consumo de fruta (p=0,011); El 74% comían entre horas (picoteos). Ser mujer y tener más de 6 años multiplicaba por 1,934 y por 12, respectivamente, la presencia de picoteos. A mayor edad más picoteos (p < 0,001). Conclusiones. Se evidencia que el sobrepeso familiar, la no lactancia materna en los primeros meses de vida, dormir menos horas de las recomendadas para la edad del niño y una alimentación pobre en verduras, hortalizas y frutas, picoteando entre horas, juegan un papel importante en la obesidad infantil.

Abstract Introduction Our aim was to evaluate the dietary intake and anthropometric indices in medical students with positive family history of type 2 diabetes (FH-T2D)(+) and without FH-T2D(-). Material and Methods 144 students were analyzed in this cross-sectional, observational study, conducted during the 2017-2018 school year using interviews and 7-day food diary. The participants were characterized anthropometrically. Waist-to-hip ratio (WHR) and waist-to-height ratio (WHtR), corrected mid-arm muscle area (MAMA), fat arm index (FAI), and tricipital skinfold (TSF) were calculated. Results We found that 79.2% (95%CI:72.5-85.8) had FH-T2D. BMI was significantly higher in the participants with FH-T2D than without (23.7±3.8 vs. 25.0±3.7, respectively; p<0.05). No significant differences were determined in the indices based on central fat distribution (WHtR and WHR), peripheral distribution (FAI and TSF), or muscle mass (MAMA), when stratified by FH-T2D. Regarding dietary intake, when comparing participants with and without FH-T2D, respectively, we observed low/none legume consumption [30% (95%CI:21.4-38.2) vs. 23% (95%CI:8.2-38.5)], diets high in proteins [38.6% (95%CI:29.7-47.5) vs. 46.7% (95%CI:28.8-64.5)], low in carbohydrates [84.2% (95%CI:77.5-90.9) vs. 83.3% (95%CI:70.0-96.7)], and insufficient energy intake [64% (95%CI:55.2-72.8) vs. 56.7% (95%CI:38.9-74.4)], where the alterations in the dietary pattern were more detrimental for the FH-T2D(+) group. Lastly, the participants with FH-T2D consumed mainly late in the day [60% (95%CI:50.6-68.6) vs. 54% (95%CI:35.5-71.2)]. Conclusions Even though there were minimal significant differences with the consumption by food categories, those students with FH-T2D presented with a poor, little varied and unbalanced dietary pattern with energy consumption mainly at night. These factors, if prolonged, could increase the risk of developing type 2 diabetes.

Resumen Introducción Nuestro objetivo fue evaluar la ingesta dietética y los índices antropométricos en estudiantes de medicina con historia familiar positiva de diabetes tipo 2 (FH-T2D)(+) y sin antecedentes familiares FH-T2D(-). Material y Métodos 144 estudiantes fueron analizados en este estudio transversal y observacional realizado durante el año escolar 2017-2018 mediante entrevistas y un diario de alimentos de 7 días. Los participantes se caracterizaron antropométricamente. Se calculó el ínidce cintura-cadera (WHR) y el índice cintura-altura (WHtR), el área muscular corregida de la mitad del brazo (MAMA), el índice de grasa del brazo (FAI) así como el pliegue cutáneo tricipital (TSF). Resultados El 79,2% (95%CI:72,5-85,8) tenían FH-T2D. El IMC fue significativamente mayor en los participantes con FH-T2D que en aquellos sin FH-T2D (23,7±3,8 vs. 25,0±3,7, respectivamente; p<0,05). No se determinaron diferencias significativas en los índices basados en la distribución de grasa central (WHtR y WHR), la distribución periférica (FAI y TSF) o la masa muscular (MAMA), cuando se estratificó por FH-T2D. Al comparar la ingesta dietética de participantes con y sin FH-T2D, respectivamente, observamos un consumo bajo/ninguno de leguminosas [30% (95%CI:21,4-38,2) frente a 23% (95%CI:8,2-38,5)], dietas altas en proteínas [38,6% (95%CI:29,7-47,5) frente a 46,7% (95%CI:28,8-64,5)], bajas en carbohidratos [84,2% (95%CI:77,5-90,9) frente a 83,3% (95%CI:70,0-96,7)], y la ingesta de energía insuficiente [64% (95%CI:55,2-72,8) frente a 56,7% (95%CI:38,9-74,4)], donde las alteraciones en el patrón de la dieta fueron más perjudiciales para el grupo FH-T2D(+). Los participantes con FH-T2D consumieron al final del día [60% (95%CI:50,6-68,6) frente a 54% (95%CI:35,5-71,2)]. Conclusiones Aunque hubo diferencias mínimas significativas con el consumo por categorías de alimentos, aquellos estudiantes con FH-T2D presentaron un patrón dietético deficiente, poco variado y desequilibrado con un consumo de energía principalmente por la noche. Estos factores, si se prolongan, podrían aumentar el riesgo de desarrollar diabetes tipo 2.

Estas recomendações baseadas na evidência detalham o uso da enteroscopia assistida por dispositivo no manejo clínico das doenças do intestino delgado. Um conjunto de Gastrenterologistas diferenciados em patologia do intestino delgado foi selecionado pelos grupos de estudos Espanhol e Português de intestino delgado para rever a evidência disponível sobre as principais indicações desta técnica, o seu papel nos algoritmos de manejo de cada indicação e sobre o seu rendimento diagnóstico e terapêutico. Foi gerado um conjunto de recomendações pelos autores.

The present evidence-based guidelines are focused on the use of device-assisted enteroscopy in the management of small-bowel diseases. A panel of experts selected by the Spanish and Portuguese small bowel study groups reviewed the available evidence focusing on the main indications of this technique, its role in the management algorithm of each indication and on its diagnostic and therapeutic yields. A set of recommendations were issued accordingly.

Although the incidence is uncertain, some case reports suggest that COVID 19 infection is associated with an increased risk of venous thromboembolism. We suggest starting prophylactic anticoagulant therapy for all patients hospitalized with a symptomatic infection with COVID-19, unless contraindicated, with enoxaparin 40 mg SC daily if creatinine clearance is greater than 30 ml/min.

Si bien la incidencia es incierta, algunos reportes de caso sugieren que la infección por COVID 19 se asocia con un aumento del riesgo de tromboembolismo venoso. Sugerimos iniciar tromboprofilaxis a todos los pacientes hospitalizados por síntomas asociados con una infección por COVID-19, a menos que esté contraindicado, con enoxaparina 40 mg SC diariamente si el clearance de creatinina es mayor a 30 ml/min.

Venous thromboembolic disease (VTE) in hospitalized adults has high morbidity and mortality, is the origin of chronic complications and increased cost for the health system. Since the publication of recommendations for thromboprophylaxis in hospitalized patients in 2013, new alternatives and strategies have emerged, which motivated us to update our recommendations. Although there are different consensus and clinical practice guidelines, adherence to them is suboptimal. The different therapeutic alternatives for hospitalized adult patients (non-surgical, surgical non-orthopedic, with and without cancer, orthopedic an d pregnant) have been updated, paying particular attention to the drugs available in Argentina.

La enfermedad tromboembólica venosa (ETV) en adultos hospitalizados posee elevada morbimortalidad, es origen de complicaciones crónicas y determina incrementos de costos para el sistema de salud. Desde la publicación de recomendaciones de tromboprofilaxis en pacientes internados en 2013, han surgido nuevas alternativas y estrategias, que nos motivaron a actualizar nuestras recomendaciones. A pesar de que existen diferentes consensos y guías de práctica clínica la adherencia a las mismas es subóptima. Se han actualizado las diferentes alternativas terapéuticas para los adultos hospitalizados (clínicos no quirúrgicos, quirúrgicos no ortopédicos, con y sin cáncer, ortopédicos y embarazadas), poniendo particular atención en los fármacos disponibles en Argentina.
