Abstract Objective To describe the clinical results of patients admitted and managed as cases of placenta accreta spectrum (PAS) at a Central American public hospital and the influence of the prenatal diagnosis on the condition. Materials and Methods A retrospective analysis of PAS patients treated at Hospital Bertha Calderón Roque, in Managua, Nicaragua, between June 2017 and September 2021. The diagnostic criteria used were those of the International Federation of Gynecology and Obstetrics (Fédération Internationale de Gynécologie et d'Obstétrique, FIGO, in French). The population was divided into patients with a prenatal ultrasonographic diagnosis of PAS (group 1) and those whose the diagnosis of PAS was established at the time of the caesarean section (group 2). Results During the search, we found 103 cases with a histological and/or clinical diagnosis of PAS; groups 1 and 2 were composed of 51 and 52 patients respectively. Regarding the clinical results of both groups, the patients in group 1 presented a lower frequency of transfusions (56.9% versus 96.1% in group 2), use of a lower number of red blood cell units (RBCUs) among those undergoing transfusions (median: 1; interquartile range: [IQR]: 0–4 versus median: 3; [IQR]: 2–4] in group 2), and lower frequency of 4 or more RBCU transfusions (29.4% versus 46.1% in group 2). Group 1 also exhibited a non-significant trend toward a lower volume of blood loss (1,000 mL [IQR]: 750–2,000 mL versus 1,500 mL [IQR]: 1,200–1,800 mL in group 2), and lower requirement of pelvic packing (1.9% versus 7.7% in group 2). Conclusion Establishing a prenatal diagnosis of PAS is related to a lower frequency of transfusions. We observed a high frequency of prenatal diagnostic failures of PAS. It is a priority to improve prenatal detection of this disease.

ABSTRACT Periodontal disease is an inflammatory condition that irreversibly destroys the tissues that support the tooth, the main characteristics of this disease being probing depth, inflammation, edema and erythema. Objective: Determine the association of periodontal disease with risk factors in 15-year-old adolescents from an educational institution. Material and Methods: The study was observational, descriptive, retrospective and cross-sectional based on the records of the database of the epidemiological oral health survey of 2018, with a sample of 29 15-year-old schoolchildren enrolled in the 2018 academic year. The variables were periodontal disease, depth to probing, gingival bleeding, sex, frequency of brushing, frequency of visits to the dentist, frequency of tobacco consumption, and frequency of alcohol consumption. Excel version 10 was used to manage the data and it was exported to Stata version 15 where the statistical tests were performed. Results: The frequency of brushing less than twice a day and the consumption of tobacco would behave as factors that increase the possibility of the development of periodontal disease with RP: 4,684, IC 95% (0,406 - 54,002) y RP: 1,468, IC 95% (0,489 -4,412), respectively; however, the values obtained are not statistically significant (p> 0.05). Conclusion: There is an association of periodontal disease with both brushing less than twice a day and tobacco use in 15-year-old adolescents from an educational institution.

RESUMEN La enfermedad periodontal es una condición inflamatoria que destruye de forma irreversible a los tejidos que soportan al diente, siendo las principales características la profundidad al sondaje, inflamación, edema y eritema. Objetivo: Determinar la asociación de la enfermedad periodontal con factores de riesgo en adolescentes de 15 años de una institución educativa. Material y Métodos: El estudio fue observacional, descriptivo, retrospectivo y transversal basado en los registros de la base de datos del levantamiento epidemiológico de salud bucal del año 2018, contando con una muestra de 29 escolares de 15 años matriculados en el año académico 2018. Las variables fueron enfermedad periodontal, profundidad al sondaje, sangrado gingival, sexo, frecuencia de cepillado, frecuencia de visita al dentista, frecuencia de consumo de tabaco y frecuencia de consumo de alcohol. Para el manejo de los datos se utilizó Excel versión 10 y se exportó a Stata versión 15 en donde se realizaron las pruebas estadísticas. Resultados: La frecuencia de cepillado menor a dos veces al día y el consumo de tabaco se comportaron como factores que aumentan la posibilidad del desarrollo de la enfermedad periodontal con RP: 4,684, IC 95% (0,406 - 54,002) y RP: 1,468, IC 95% (0,489 - 4,412), respectivamente; sin embargo, los valores obtenidos no son estadísticamente significativos (p>0,05). Conclusión: Existe una asociación de la enfermedad periodontal tanto con el cepillado menor de dos veces al día como con el consumo de tabaco en adolescentes de 15 años de una institución educativa.

By the end of 2010, Latin America and the Caribbean (LAC) achieved 63% antiretroviral treatment (ART) coverage. Measures to control HIV drug resistance (HIVDR) at the country level are recommended to maximize the efficacy and sustainability of ART programs. Since 2006, the Pan American Health Organization has supported implementation of the World Health Organization (WHO) strategy for HIVDR prevention and assessment through regional capacity-building activities and direct technical cooperation in 30 LAC countries. By 2010, 85 sites in 19 countries reported early warning indicators, providing information about the extent of potential drivers of drug resistance at the ART site. In 2009, 41.9% of sites did not achieve the WHO target of 100% appropriate first-line prescriptions; 6.3% still experienced high rates (> 20%) of loss to follow-up, and 16.2% had low retention of patients (< 70%) on first-line prescriptions in the first year of treatment. Stock-outs of antiretroviral drugs occurred at 22.7% of sites. Haiti, Guyana, and the Mesoamerican region are planning and implementing WHO HIVDR monitoring surveys or threshold surveys. New HIVDR surveillance tools for concentrated epidemics would promote further scale-up. Extending the WHO HIVDR lab network in Latin America is key to strengthening regional lab capacity to support quality assured HIVDR surveillance. The WHO HIVDR control strategy is feasible and can be rolled out in LAC. Integrating HIVDR activities in national HIV care and treatment plans is key to ensuring the sustainability of this strategy.

Hacia fines del 2010, América Latina y el Caribe lograron una cobertura de tratamiento antirretroviral de 63%. Se recomienda la ejecución de medidas para controlar la farmacorresistencia del VIH a nivel de país para potenciar al máximo la eficacia y la sostenibilidad de los programas de tratamiento antirretroviral. Desde el 2006, la Organización Panamericana de la Salud ha apoyado la aplicación de la estrategia de la Organización Mundial de la Salud (OMS) para la prevención y la evaluación de la farmacorresistencia del VIH mediante actividades regionales de formación de capacidad y de cooperación técnica directa en 30 países de América Latina y el Caribe. En 2010, 85 centros en 19 países notificaron indicadores de alerta temprana y suministraron información acerca del alcance de los posibles impulsores de la farmacorresistencia en los centros de tratamiento antirretroviral. En el 2009, 41,9% de los centros no lograron la meta de la OMS de 100% de prescripción de medicamentos de primera línea apropiados; 6,3% todavía tenían tasas elevadas (> 20%) de pérdida de seguimiento y 16,2% tenían una baja retención de pacientes (< 70%) en tratamiento con antirretrovirales de primera línea en el primer año de tratamiento. Se registraron desabastecimientos de medicamentos antirretrovirales en 22,7% de los centros. Haiti, Guyana y la zona mesoamericana están planificando y ejecutando estudios de vigilancia de la farmacorre-sistencia del VIH o estudios del umbral de la OMS. Las nuevas herramientas para la vigilancia de la farmacorresistencia del VIH en las epidemias concentradas permitirán una mejor vigilancia. La ampliación de la red de laboratorios de farmacorresistencia del VIH acreditados por la OMS en América Latina es fundamental para el fortalecimiento de la capacidad de los laboratorios regionales, a fin de de efectuar una vigilancia de la farmacorresistencia del VIH de calidad garantizada. La estrategia para el control de la farmacorresistencia del VIH de la OMS es factible y puede implantarse en América Latina y el Caribe. La integración de las actividades de vigilancia de la farmacorresistencia del VIH con los planes nacionales de atención y tratamiento del VIH es fundamental para garantizar la sostenibilidad de esta estrategia.
