Abstract Background: Maternal mortality rates in Brazil remain above the goals established by the United Nations Sustainable Development Goals. Heart disease is estimated to affect 4% of all pregnancies and remains by itself the main indirect obstetric cause of maternal death. In the last decades, a significant improvement in the prognosis of heart diseases has made pregnancy possible in women with heart disease and provided better maternal and fetal outcomes. Objective: To establish a multicenter Brazilian Registry of pregnant women with heart disease; to study the causes of immediate and late maternal mortality; and to assess the prevalence of heart disease in the country’s macro-regions. Method: This is an observational study, with retrospective and prospective stages, of the clinical and obstetric progression of pregnant women with heart disease. These women consecutively received care during pregnancy and will be followed up for up to a year after delivery at public and private hospitals with infrastructure for the execution of this project, a principal investigator, and approval by Ethics and Research Committees. Results: Our results will be presented after data collection and statistical analysis, aiming to demonstrate immediate and late maternal mortality rates, as well as the prevalence of heart disease in the country and its cardiovascular and obstetric complications during pregnancy. Conclusion: REBECGA will be the Brazilian Registry of heart disease and pregnancy and it will contribute to planning preventive measures, raising financial resources for the improvement of high-risk prenatal care, and reducing immediate and late maternal mortality due to heart disease.

Resumo A síndrome compartimental crônica é uma condição comum e frequentemente subdiagnosticada, induzida pelo exercício, que corresponde em média a um quarto dos casos de dor crônica na perna relacionada ao exercício, e que perde em frequência apenas para o espectro fratura/reação ao estresse tibial. Tradicionalmente ocorre em jovens atletas corredores, embora estudos mais recentes tenham demonstrado uma prevalência considerável em praticantes de atividade física de baixo rendimento, mesmo em pacientes de meia-idade ou idosos. A lista de diagnósticos diferenciais é extensa, e por vezes é difícil fazer a distinção apenas pelos dados clínicos, sendo necessários exames subsidiários. Classicamente, o diagnóstico é feito pelo quadro clínico, pela exclusão dos diferenciais, e pela medida pressórica intracompartimental. Embora a manometria por agulha seja considerada o padrão-ouro no diagnóstico, seu uso não é universalmente aceito, visto que existem algumas limitações importantes, além da disponibilidade restrita do equipamento com agulha no Brasil. Recentemente, novos protocolos de manometria têm sido propostos a fim suplantar a deficiência dos tradicionais, com algum deles inclusive recomendando o uso sistemático da ressonância magnética (RM) na exclusão dos diagnósticos diferenciais. O uso de sequências de RM sensíveis a líquido pós-esforço é uma ótima opção não invasiva à manometria por agulha no diagnóstico da síndrome compartimental crônica, uma vez que o aumento da intensidade de sinal pós-exercício é estatisticamente relevante quando comparados os valores pressóricos de manometria em pacientes com a síndrome e assintomáticos; portanto, o exame pode ser usado no critério diagnóstico. O tratamento definitivo é a fasciotomia, embora existam alternativas menos eficazes.
Abstract Chronic compartment syndrome is a common and often underdiagnosed exercise-induced condition, accounting on average for a quarter of cases of chronic exertional pain in the leg, second only to the fracture/tibial stress syndrome spectrum. It traditionally occurs in young runner athletes, although more recent studies have demonstrated a considerable prevalence in low-performance practitioners of physical activity, even in middle-aged or elderly patients. The list of differential diagnoses is extensive, and sometimes it is difficult to distinguish them only by the clinical data, and subsidiary examinations are required. The diagnosis is classically made by the clinical picture, by exclusion of the differential diagnoses, and through the measurement of the intracompartmental pressure. Although needle manometry is considered the gold standard in the diagnosis, its use is not universally accepted, since there are some important limitations, apart from the restricted availability of the needle equipment in Brazil. New protocols of manometry have recently been proposed to overcome the deficiency of the traditional ones, and some of them recommend the systematic use of magnetic resonance imaging (MRI) in the exclusion of differential diagnoses. The use of post-effort liquid-sensitive MRI sequences is a good noninvasive option instead of needle manometry in the diagnosis of chronic compartment syndrome, since the increase in post-exercise signal intensity is statistically significant when compared with manometry pressure values in asymptomatic patients and in those with the syndrome; hence, the test can be used in the diagnostic criteria. The definitive treatment is fasciotomy, although there are less effective alternatives.

Computational prediction and characterisation of miRNAs and their pathway genes in human schistosomiasis caused by Schistosoma haematobium

BACKGROUND Key genes control the infectivity of the Schistosoma haematobium causing schistosomiasis. A method for understanding the regulation of these genes might help in developing new disease strategies to control schistosomiasis, such as the silencing mediated by microRNAs (miRNAs). The miRNAs have been studied in schistosome species and they play important roles in the post-transcriptional regulation of genes, and in parasite-host interactions. However, genome-wide identification and characterisation of novel miRNAs and their pathway genes and their gene expression have not been explored deeply in the genome and transcriptome of S. haematobium. OBJECTIVES Identify and characterise mature and precursor miRNAs and their pathway genes in the S. haematobium genome. METHODS Computational prediction and characterisation of miRNAs and genes involved in miRNA pathway from S. haematobium genome on SchistoDB. Conserved domain analysis was performed using PFAM and CDD databases. A robust algorithm was applied to identify mature miRNAs and their precursors. The characterisation of the precursor miRNAs was performed using RNAfold, RNAalifold and Perl scripts. FINDINGS We identified and characterised 14 putative proteins involved in miRNA pathway including ARGONAUTE and DICER in S. haematobium. Besides that, 149 mature miRNAs and 131 precursor miRNAs were identified in the genome including novel miRNAs. MAIN CONCLUSIONS miRNA pathway occurs in the S. haematobium, including endogenous miRNAs and miRNA pathway components, suggesting a role of this type of non-coding RNAs in gene regulation in the parasite. The results found in this work will open up a new avenue for studying miRNAs in the S. haematobium biology in helping to understand the mechanism of gene silencing in the human parasite Schistosome.

An unusual presentation of urethral duplication presenting with chronic bladder retention, left scrotal transposition and left renal agenesis

ABSTRACT Introduction and objective Urethral duplication is a rare congenital anomaly, with roughly 200 cases reported in the literature (1). It is more frequent in males, with few cases reported in females. The clinical presentation differs according to the anatomical variant present. The duplication most commonly occurs in the sagittal plane with one urethra located ventrally and the other dorsally (2). Usually the ventral urethra is the more functional of both. Duplications occurring in the coronal plane are quite rare and they are usually associated with bladder duplication (3). The purpose of this paper was to present a video of a boy with an unusual urethral duplication form. Materials and Methods Patient was born premature due to oligohydramnios at 7 months-gestational age and he has initial diagnosis of hypospadia. Since then, he presented at least 7 febrile UTI and mother complained of difficult micturition and a presence of a mass at lower abdomen. Patient was referred to our institution and we identified urethral duplication with a glandar and scrotal meatus, palpable bladder and left penile-hemiscrotum transposition. US and CT-scan showed left kidney agenesis and overdistended bladder. VCUG and retrograde urethrography showed duplication, presence of contrast in the seminal vesicles and complete catheterizing of both urethras was not possible. Results The topic urethra was dysplastic and not patent to a 4Fr plastic tube so we were unable to access it endoscopically. We performed initially a Mitrofanoff procedure to allow CIC and treat chronic retention. Six months later, we assessed both urethras surgically and concluded that dorsal urethra was dysplastic after 3cm still in the penile area and scrotal urethra was not possible to be catheterized. We excised the ventral urethra because of dribbling complaints up to bulbar area and reconstructed the scrotal transposition, keeping the topic urethra for cosmetic issues. Patient had excellent outcome, performs CIC every 4 hours and has not presented further UTI episodes. Discussion and conclusion The urethral duplication is an anomaly that has multiple anatomical presentations. There are several theories about the etiology, but none can explain all types of presentations. There is also more than one rating available, and the Effmann classification is the most detailed. The case exemplifies this varied spectrum of anatomic urethral duplication. It resembles the urethral duplication type IIa-Y, however, ventral urethra meatus was located in penoscrotal area and both urethras were at least partially hypoplastic/dysplastic associated with obstruction and bladder retention. In determining how to best manage a patient with Y-type urethral duplication, the caliber and quality of the orthotopic urethra must first be assessed. Published reports suggest that best outcomes are those using the ventral duplicated urethra for the reconstruction (4). In this case, none of urethras were functional and a supravesical outlet channel had to be provided. The treatment of this condition requires an individualized planning and a vast technical knowledge of reconstructive surgery.

Can a graft be placed over a flap in complex hypospadias surgery? An experimental study in rabbits

ABSTRACT Purpose: To develop a rabbit experimental study to test the hypothesis that surgical repair of hypospadias with severe ventral curvatures might be completed in one stage, if a graft, such as buccal mucosa, could be placed over the tunica vaginalis flap used in corporoplasty for ventral lengthening, with the addition of an onlay preputial island flap to complete the urethroplasty. Materials and methods: The experimental procedure with rabbits included a tunica vaginalis flap for reconstruction of the corpora after corporotomy, simulating a ventral lengthening operation. A buccal mucosa graft was placed directly on top of the flap, and the urethroplasty was completed with an onlay preputial island flap. Eight rabbits were divided into 4 groups, sacrificed at 2, 4, 8 and 12 weeks postoperatively, and submitted to histological evaluation. Results: We observed a large number of complications, such as fistula (75%), urinary retention (50%) and stenosis (50%). There were two deaths related to the procedure. Histological evaluation demonstrated a severe and persistent inflammatory reaction. No viable tunica vaginalis or buccal mucosa was identified. Conclusions: In this animal model, the association of a buccal mucosa graft over the tunica vaginalis flap was not successful, and resulted in complete loss of both tissues.

Reconstrução de mama com expansor de Becker: uma análise de 116 casos

ABSTRACT Introduction: Breast expanders/implants were developed in the 1980s as a breast reconstruction alternative to pre-existing techniques. They were created to provide adequate shape and volume to the breast after one-stage surgery and they do not require replacement. To evaluate the long-term results of breast reconstruction performed with Becker expanders. Methods: This retrospective study evaluated patients who underwent breast reconstruction with implants/expanders between 2004 and 2012. Results: A total of 161 expanders were implanted. Major complications (those that led to the removal or replacement of the prosthesis) comprised 22.98% of the enrolled subjects. The expander was removed and replaced by another type of reconstruction implant in 21.74% of the cases. The main reasons for the replacement were capsular contracture (31.43%), patients’ desire (31.43%), and implant failure (20%). The surgical alternatives for the replaced expanders included prosthesis (91.43%), latissimus dorsi flap (5.71%), and TRAM flap (2.86%). Only few studies have evaluated the long-term effects of Becker expanders. In this study, the replacement rate was 28.57%. The study questions the use of Becker expanders as an adequate, onestage reconstruction technique. In spite of a high rate of optimal outcomes in the new surgical procedures (37%), its use is limited because of the high costs. The expanders are still useful for patients with comorbidities or other factors that limit the performance of other surgical procedures. Conclusion: The use of Becker expanders as an alternative for one-stage breast reconstruction technique is very limited. Therefore, these devices should only be used in selected cases.
RESUMO Introdução: Os implantes-expansores de mama surgiram na década de 80 como uma alternativa de reconstrução de mama às técnicas pré-existentes. Foram criados com a intenção de produzir um bom resultado de forma e volume de mama após uma única operação, não necessitando ser substituído. Avaliar os resultados a longo prazo de reconstruções de mama com uso de expansores de Becker. Métodos: Estudo retrospectivo de pacientes submetidas à reconstrução de mama com implantes-expansores entre 2004 e 2012. Resultados: Foram implantados 161 expansores. As complicações maiores (aquelas que levaram à retirada ou troca da prótese) totalizaram 22,98%. O expansor foi retirado e substituído por outro tipo de reconstrução em 21,74% dos casos. As principais razões que levaram à troca foram: contratura capsular (31,43%), vontade da paciente (31,43%); falhas no implante (20%). As opções cirúrgicas substitutivas para os expansores trocados foram prótese (91,43%), grande dorsal (5,71%) e TRAM (2,86%). Existem poucos trabalhos avaliando os expansores de Becker a longo prazo. Neste trabalho a taxa de substituição foi 28,57%. O estudo levanta questionamentos sobre o uso dessa técnica como reconstrução em tempo único. A taxa de cerca de 37% de novos procedimentos cirúrgicos para atingir o resultado ideal torna muito difícil o seu uso em detrimento do alto custo. Os expansores ainda permanecem úteis em casos de pacientes com comorbidades ou qualquer outro fator que limite a realização de outros tempos cirúrgicos. Conclusão: O uso de expansores de Becker como alternativa técnica de reconstrução de mama em tempo único é muito limitado. Sua indicação deve ser para casos selecionados.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Seleção de isolados de Paecilomyces lilacinus (Thom.) Samson para controle de Meloidogyne paranaensis em tomateiro

Paecilomyces lilacinus é uma espécie fúngica utilizada no controle biológico de nematóides, sendo uma das mais estudadas a campo. O trabalho teve como objetivo selecionar isolados de Paecilomyces lilacinus quanto à capacidade de controlar Meloidogyne paranaensis em tomateiro cv. "Santa Clara", em de casa-de-vegetação. Plantas cultivadas em vasos foram inoculadas com M. paranaensis. Após 45 dias da inoculação, efetuou-se a substituição dos tomateiros e foram aplicados 50 gramas de arroz autoclavado e colonizado por P. lilacinus. Decorridos 45 dias da inoculação do fungo, foram avaliados: número de galhas, massa de ovos, número de ovos por sistema radicular, peso da massa fresca da parte aérea, do sistema radicular e sobrevivência do P. lilacinus no solo. Todos os tratamentos com incorporação de P. lilacinus reduziram a população de M. paranaensis em raízes de tomateiro. Os isolados UEL pae 05, 08, 09, 13, 20, 21, 38, 41, 44, 54, ESALQ 831 e 832 foram os melhores no controle de M. paranaensis, afetando a reprodução dos nematóides e apresentando elevada sobrevivência no solo.
Paecilomyces lilacinus is a fungal species used in biological control of nematodes. It is considered one of the most frequently evaluated fungus under field conditions. The present study aimed to select P. lilacinus isolates with regard to their capacity in controlling Meloidogyne paranaensis in tomato. Santa Clara tomato plants were grown in pots and inoculated with M. paranaensis under greenhouse conditions. The tomato plants were replaced 45 days after inoculation with 50 grams of autoclaved rice colonized with P. lilacinus. Forty-five days after fungus inoculation the number of galls, egg masses, number of eggs in root system, aerial part mass weight, root mass weight and P. lilacinus survival in the soil were evaluated. All treatments involving P. lilacinus incorporation reduced the population of M. paranaensis in tomato roots. UEL pae 05, 08, 09, 13, 20, 21, 38, 41, 44, 54, ESALQ 831 e 832 were the best in the control of M. paranaensis, affecting reproduction of the nematodes and showing a high survival rate in the soil.
