RACIONAL: A terapêutica do linfoma não-Hodgkin gástrico primário é controversa, com defensores da extirpação cirúrgica, da radioterapia e quimioterapia isoladas ou combinadas, especialmente em relação aos estádios mais iniciais. OBJETIVOS: Analisar as características clínico-patológicas e os resultados do tratamento nos doentes operados no Serviço de Gastroenterologia Cirúrgica do Hospital do Servidor Público Estadual, São Paulo, SP, com linfoma gástrico primário em estádio inicial. Realizar revisão da literatura, destacando os aspectos diagnósticos, fatores prognósticos e o papel das diferentes modalidades de tratamento. MÉTODO: Dezesseis doentes com linfoma gástrico primário no estádio inicial foram submetidos ao tratamento cirúrgico curativo. Idade, sexo, localização, tamanho, tipo de operação, número de lesões, profundidade da invasão, tipo histológico de acordo com a classificação de Kiel, comprometimento linfonodal, estádio pela classificação de Ann Arbor modificada por Musshoff e Schmidt-Vollmer, grau histológico, margens, terapia adjuvante, evolução e sobrevivência. RESULTADOS: Dez (62,5%) doentes foram submetidos a gastrectomia subtotal e seis (37,5%) a gastrectomia total. A maioria (9/56,2%) das lesões estava localizada no antro. Lesões únicas (10/62,5%) foram mais freqüentes que as múltiplas (6/37,5%). Treze doentes (81,2%) foram classificados no estádio IE e três (18,7%) no estádio IIE1. Dez (62,5%) enfermos apresentaram linfoma gástrico primário de baixo grau e seis (37,5%) de alto grau. Os tipos histológicos mais freqüentes foram o linfoplasmocítico citóide (4/25.0%) e o centroblástico (4/25,0%). Dez doentes (62,5%) receberam tratamento adjuvante (quimioterapia e/ou radioterapia). Nove enfermos (56,2%), todos no estádio IE, atingiram sobrevivência maior que 5 anos e oito (50,0%) receberam tratamento adjuvante. Dois (12,5%) doentes no estádio IIE1 tiveram recidiva peritonial e faleceram 3,0 anos e 3,5 anos após suas respectivas operações. A média global de sobrevivência foi de 42,5 meses. CONCLUSÕES: Nos doentes com linfoma gástrico primário em estádio inicial (IE e IIE1), a extirpação gástrica permitiu o estádio clinico-patológico acurado, a obtenção de material para o estudo anatomopatológico, a extirpação da lesão e, para enfermos com doença no estádio IE, a extirpação gástrica combinada com terapia adjuvante foi associada com sobrevivência maior que 5 anos. Até que estudos prospectivos casualizados sejam realizados para avaliar a eficácia dos diferentes tipos de tratamento do linfoma gástrico primário em estádio inicial, os protocolos de tratamento devem ser adaptados individualmente.

BACKGROUND: There is controversy regarding the optimal therapy for primary non-Hodgkin gastric lymphoma with some authors defending surgical extirpation either alone or in association with radiotherapy and or chemotherapy, especially in relation to the earlier stages of the disease. AIM: To analyze the clinical-pathological features and the results of management approaches for patients with primary early-stage non-Hodgkin's lymphoma of the stomach operated in Surgical Gastroenterology Department, "Hospital do Servidor Público Estadual", São Paulo, SP, Brazil. The literature is reviewed to highlight the aspects of diagnosis, prognostic factors and role of the various treatment regimens. METHOD: Sixteen patients with primary early-stage gastric lymphoma underwent curative surgical treatment. The variables analyzed were age, sex, location, size, type of surgery, number of lesions, depth of invasion, histological type in accordance with Kiel's classification, involvement of lymph nodes, Ann Arbor stage classification modified by Musshoff and Schmidt-Vollmer, histological grade, margins, adjuvant therapy, clinical course and survival. RESULTS: Ten patients (62.5%) underwent subtotal gastrectomy and six (37.5%) underwent total gastrectomy. The majority (9/56.2%) of the lesions were located in the antrum. Single lesions (10/62.5%) were more frequent than multiple lesions (6/37.5%). Thirteen patients (81.2%) were classified as stage IE and three (18.7%) as stage IIE1. Primary gastric lymphoma classified histologically as low or high grade was presented by 10 (62.5%) and 6 (37.5%) patients, respectively. The most frequent histological types were the lymphoplasmocytic cytoid (4/25.0%) and centroblastic (4/25.0%). Ten patients (62.5%) received adjuvant treatment (chemotherapy and/or radiotherapy). Nine patients (56.2%), all in stage IE, reached a survival greater than 5 years and of these eight (50.0.%) had received adjuvant therapy. Two (12.5%) patients with stage IIE1 presented peritoneal relapse and died 3.0 years and 3.5 years after their respective operations. The mean overall survival was 42,5 months. CONCLUSIONS: Among the patients with primary early-stage gastric lymphoma (IE and IIE1), the gastric resection enabled an accurate clinicopathological staging, in addition to obtaining sufficient material for histopathological study and extirpation of the lesion. Furthermore, for patients with stage IE disease, the gastric resection combined with adjuvant therapy was associated with a greater than 5-year survival. Until prospective randomized studies are realized in order to evaluate the real efficacy of the different types of treatment for primary early-stage gastric lymphoma, management approaches should be individually tailored.

RACIONAL: A síndrome de Mirizzi, é complicação rara da colelitíase de longa duração, caracterizada pelo estreitamento do ducto hepático comum devido a mecanismo de compressão e/ou inflamação por cálculos biliares impactados no infundíbulo da vesícula biliar ou no ducto cístico. OBJETIVO: Descrever série de oito enfermos consecutivos com síndrome de Mirizzi de uma única instituição, submetidos ao tratamento cirúrgico e comentar seus aspectos com ênfase no diagnóstico e tratamento. MÉTODO: Quatro mulheres e quatro homens, com média de idade de 61,6 anos (42 a 82 anos), foram operados com síndrome de Mirizzi entre 1997 e 2003. Avaliaram-se a apresentação clínica, os resultados dos exames laboratoriais e de imagem, os achados operatórios, a presença de coledocolitíase, o tipo de síndrome de Mirizzi de acordo com a classificação de Csendes, a escolha do procedimento operatório e as complicações pós-operatórias. RESULTADOS: Os sintomas mais freqüentes foram dor abdominal (87,5%) e icterícia (87,5%). Todos os doentes apresentaram exames de função hepática alterados. O diagnóstico da síndrome de Mirizzi foi intra-operatório em sete (87,5%) doentes, e pré-operatório em um (12,5%). Observou-se fístula colecistocoledociana associada à coledocolitíase em três (37,5%) casos. A síndrome de Mirizzi foi classificada como tipo I em cinco (71,4%) doentes, tipo II em um (14,3%), tipo III em um (12,5%), e tipo IV em outro (12,5%). A colecistectomia, como procedimento cirúrgico isolado, foi realizada em quatro (50,0%) doentes. Um (12,5%) enfermo foi submetido a colecistectomia parcial e fechamento do orifício fistuloso com a região central do infundíbulo. Dois (25,0%) enfermos foram submetidos a colecistectomia e anastomose coledocoduodenal látero-lateral e outro (12,5%) a anastomose coledocoduodenal látero-lateral, deixando-se a vesícula biliar in situ. Sete (87,5%) doentes evoluíram sem complicações pós-operatórias e obtiveram alta hospitalar em boas condições. Um (12,5%) enfermo apresentou, no pós-operatório, sepse por abscesso sub-hepático, sendo re-operado. Não houve mortalidade operatória. CONCLUSÕES: O diagnóstico pré-operatório da síndrome de Mirizzi é difícil e é necessário elevado índice de suspeita para evitar lesões da árvore biliar. O problema pode se tornar evidente apenas no momento da operação na forma de aderências firmes ao redor do triângulo de Calot. O sucesso do tratamento está relacionado ao reconhecimento precoce da condição, mesmo no intra-operatório, e na individualização da conduta, de acordo com as características de cada caso.

BACKGROUND: Mirizzi syndrome is a rare complication of prolonged cholelithiasis, characterized by narrowing of the common hepatic duct due to mechanical compression and/or inflammation due to biliary calculus impacted in the infundibula of the gallbladder or in the cystic duct. OBJECTIVES: To describe a series of eight consecutive patients with Mirizzi syndrome, at a single institution, submitted to surgical treatment and to comment on their aspects with emphasis on the diagnosis and treatment. METHODS: Four women and four men, with a mean age of 61.6 years (42 to 82 years), presenting Mirizzi syndrome were operated between 1997 and 2003. The following items were evaluated: clinical presentation, laboratory results, preoperative evaluation, operative findings, presence of choledocholithiasis, type of Mirizzi syndrome according to the classification by Csendes, choice of operative procedures, and complications. RESULTS: The most frequent symptoms were abdominal pain (87.5%) and jaundice (87.5%). All the patients presented altered hepatic function tests. The diagnosis of Mirizzi syndrome was intra-operative in seven (87.5%) patients, and preoperative in one (12.5%). Cholecystocholedochal fistula associated with choledocholithiasis was observed in three (37.5%) cases. Mirizzi syndrome was classified as Csendes type I in five (62.5%) patients, type II in one (12.5%), type III in one (12,5%) and type IV in another (12.5%). Cholecystectomy, as an isolated surgical procedure, was performed in four (50.0%) patients. One (12.5%) patient was submitted to partial cholecystectomy and closure of the fistulous orifice with the central part of the infundibula. Two (25.0%) patients were submitted to cholecystectomy and side-to-side choledochoduodenostomy and another (12.5%) to side-to-side choledochoduodenostomy remaining the gallbladder in situ. Seven (87.5%) patients had an uneventful recovery and were discharged in good conditions. One (12.5%) patient presented a postoperative sepsis due to a sub-hepatic abscess, and was reoperated. There was no operative mortality. CONCLUSION: The preoperative diagnosis of Mirizzi syndrome is difficult and an awarded suspicion is necessary to avoid lesions of the biliary tree. The problem may only become evident during the operation due to firm adherences around Calot's triangle. The success of the treatment is related to a precocious recognition of the condition, even at the time of surgery, and adapting the management considering to the individual characteristics of each case.

RACIONAL: O problema da relação entre os níveis de CEA no sangue e o conteúdo de CEA tissular no carcinoma colorretal e os mecanismos de liberação do CEA das células neoplásicas nos tecidos vizinhos à neoplasia e sua conseqüente entrada dentro do sangue periférico são importantes para o entendimento da biologia do carcinoma colorretal. Ainda não foi convenientemente elucidado se o CEA no sangue é drenado principalmente pelo sangue do sistema portal ou pelos linfáticos para o ducto torácico ou, ainda, por ambos os sistemas. OBJETIVO: Estudar o comportamento dos níveis do CEA no sangue periférico (CEA-p) e no sangue do efluente venoso (CEA-d) de doentes com tumores colorretais operados curativamente. MÉTODO: foram estudados 28 doentes, sendo 12 (42,9%) homens e 16 (57,1%) mulheres. A média de idade foi de 66,1 anos (43 a 84 anos). Imediatamente após a laparotomia, o sangue venoso periférico foi extraído por punção venosa antecubital e o sangue do efluente venoso coletado da veia principal de drenagem das lesões. Os valores de CEA-p, CEA-d e do gradiente entre o CEA-d e CEA-p abaixo de 5,0 ng/mL foram considerados normais. RESULTADOS: Oito (28,6%) doentes foram classificados no estádio A de Dukes, 9 (32,1%) no estádio B e 11 (39,3%) no estádio C. A neoplasia estava localizada no reto em 14 (50,0%), no cólon transverso em 5 (17,9%), no cólon sigmóide em 4 (14,3%), no ceco e/ou cólon ascendente em 3 (10,7%), e no cólon descendente em 2 (7,1%) enfermos. O exame histopatológico revelou adenocarcinoma bem diferenciado em todos os enfermos. Em apenas um (3,6%) doente a neoplasia, estadiada como Dukes C, exibia invasão venosa. O gradiente entre os níveis de CEA-p e de CEA-d estava normal em 25 (89,3%) doentes e elevado em 3 (10,7%). O valor médio do CEA-p foi de 3,8 ± 4,1 ng/mL (0,1 a 21,1 ng/mL) e do CEA-d foi de 4,5 ± 4,3 ng/mL (0,3 a 20,2 ng/mL), sem diferença significativa entre esses valores. Houve diferença significativa entre a média dos valores dos níveis do CEA-p e do CEA-d maiores que 5 ng/mL. CONCLUSÃO: Os níveis de CEA-p e do CEA-d nos doentes com carcinoma colorretal não se mostraram diferentes. Os resultados deste estudo sugerem que, nas neoplasias colorretais sem invasão venosa, o CEA não é drenado expressivamente pelo sangue do efluente venoso portal do tumor.

BACKGROUND: The problem of the relationship between blood carcinoembryonic antigen (CEA) levels and tissue CEA content in colorectal carcinoma, and the mechanisms for CEA release from tumor cells in tissue adjacent to the neoplasm is important to understanding the biology of colorectal carcinoma. It has not been adequately explained whether CEA in the peripheral blood is drained mainly by portal system blood or by the lymphatic system, or indeed by both systems. AIM: To study the behavior of CEA levels in peripheral blood (CEA-p) and venous effluent blood (CEA-d) among patients with colorectal tumors, who underwent curative operation. METHOD: A total of 28 patients were studied (12 male [42.9%] and 16 female [57.1%], mean age 66.1 years [range: 43 - 84]). Immediately after laparotomy, peripheral venous blood was extracted by antecubital venous puncture and venous effluent blood was collected from the main drainage vein of the lesions. Values of CEA-p, CEA-d and the gradient between CEA-d and CEA-p that were less than 5.0 ng/mL were considered normal. RESULTS: Eight (28.6%) patients were stage A in Duke's classification, nine (32.1%) stage B and 11 (39.3%) stage C. The neoplasm was located in the rectum of 14 patients (50.0%), in the transverse colon in five (17.9%), in the sigmoid in four (14.3%), in the cecum and/or ascending colon in three (10.7%), and in the descending colon in two (7.1%). The histopathological examination revealed well-differentiated adenocarcinoma in all the patients. Only one patient (3.6%), Duke's classification stage C, presented neoplasm with venous invasion. The gradient between the CEA-p and CEA-d levels were normal in 25 patients (88.3%) and high in three (10.7%). The mean value for CEA-p was 3.8 ± 4.1 ng/mL (0.1-21.1 ng/mL) and for the drained CEA (CEA-d) it was 4.5 ± 4.3 ng/mL (0.3-20.2 ng/mL), without significant difference between these values. There was a significant difference between the mean value for CEA-p and CEA-d levels greater than 5 ng/mL. CONCLUSION: The CEA-p and CEA-d levels in the colorectal carcinoma patients were not shown to be different. The results from this study suggest that, in colorectal neoplasm without venous invasion, there may not be notable CEA drainage from the tumor by the portal vein effluent blood.

Resumo: Avaliou-se o desempenho dos alunos de programa de pós-graduação na área de cirurgia. Analisou-se a participação de cada aluno em relação ao número de publicações prévias e posteriores à titulação, vinculação do aluno com instituição de ensino superior e publicação da tese em periódico. Dentre os alunos do mestrado e do doutorado não houve diferença significativa em relação ao número de publicações antes e após à titulação. Oito (32%) alunos do mestrado eram vinculados com instituição de ensino superior, assim como cinco (100%) alunos do doutorado. Seis alunos do mestrado (24%) e três alunos do doutorado (60%) publicaram a tese na forma de artigo de revista. No presente estudo, os resultados indicaram falta de vocação para a produção por parte dos alunos do mestrado e doutorado e fraca vinculação dos alunos do mestrado às instituições de ensino superior. Esses resultados nos impulsionam a rever os critérios de admissão à pós-graduação stricto sensu. O desenvolvimento e a utilização de parâmetros para avaliar a qualidade dos egressos dos programas de pós-graduação stricto sensu constituem ferramentas importantes que podem contribuir para a avaliação e aperfeiçoamento dos programas.

Abstract: The authors evaluated the performance of former students from graduate studies program in surgery. Each studen’s output was analysed in relation to the number of publications produced before and after obtaining the graduate diploma, the author’s positions in these publications, the student’s affiilations with instituitions of higher learning, and publication of the thesis or dissertation in a scientif journal. Amon Master’s and Doctoral student, there was no signnificant difference between the number of publications before and after obtaining the diploma. Eight Master’s students (32%) and all five Doctoral students (100%) wereaffiliated with a college. Six Master’s student (24%) and three Doctoral students (60) had their thesis or dissetations published as articles. Results of the presente study suggested a relative lack of vocation for scientific output on the part of Master’s and Doctoral students and weak links with institutions of higher learning amonf the Master’s students, wich leads us to review the admissions criteria for stircto sensu graduate studies programs. The development and application of parameters to evaluate the quality of the alumni of stricto sensu graduate programns are important tools that can contribute to the program’s evaluation and improvement.

OBJETIVO: Comparar duas técnicas cirúrgicas empregadas, no tratamento da doença pilonidal, a de marsupialização e da incisão e curetagem, analisando seus resultados imediatos, e tardios. MÉTODO: A casuística de 52 pacientes que apresentaram doença pilonidal sacrococcígea em suas diferentes fases, divididos em dois grupos. Grupo A, com 23 doentes submetidos à técnica de marsupialização, e grupo B, formado por 29 doentes operados pela técnica de incisão e curetagem. Todos tiveram seguimento pós-operatório de um a três anos, no período de 1981 a 1999 pelo Serviço de Gastroenterologia Cirúrgica do Hospital do Servidor Público Estadual "Francisco Morato de Oliveira" de São Paulo. A média de idade do grupo A foi de 25,39 ± 8,92 anos e a do grupo B foi de 25,52 ± 9,98 anos. A distribuição por sexo foi de 28 mulheres e 24 homens. Segundo a raça, foram 42 brancos e 10 negros. RESULTADOS: A média de tempo de duração de cirurgia, para o grupo A, submetido à marsupialização, foi de 38,91 ± 19,77 minutos, e para o grupo B, doentes submetidos à incisão e curetagem, foi de 47,34 ± 14,34 minutos. A média do tempo de internação para o grupo A foi de 3,26 dias e para o grupo B 1,28 dia. Quanto ao uso de antibiótico, em ambos grupos não foi observada diferença estatística (p>0,05). O tempo médio de cicatrização da ferida operatória nos doentes submetidos à técnica de marsupialização foi de 29,30 ±13,24 dias e para os doentes submetidos a incisão e curetagem foi de 40,76 ± 26,56 dias. A dor pós-operatória incidiu com 72,4% dos doentes do grupo B, e de 17,4% do grupo A, observando-se significância estatística (p=0,0001). O grupo de doentes submetidos à marsupialização não apresentou sangramento, enquanto no grupo da incisão e curetagem 20,7% dos enfermos tiveram sangramento (p=0,2829). A incidência de infecção pós-operatória não diferiu em ambos grupos (4,4% e 3,5%, respectivamente). O grupo B apresentou maiores índices de recidivas (17,6%) em relação ao grupo A (4,3%) com diferença estatisticamente significante (p=0,3364). CONCLUSÕES: Os procedimentos cirúrgicos empregados são de fácil execução, com períodos per-operatórios aceitáveis. A cicatrização parece ser mais eficiente nos doentes operados pela técnica de marsupialização. A técnica de incisão e curetagem apresentou maior número de complicações: dor e sangramento com maior freqüência que na técnica de marsupialização. A técnica de incisão e curetagem apresentou maior recidiva que a técnica de marsupialização.

OBJECTIVE: Results of two surgical techniques for treatment of pilonidal disease, marsupialization and incision and curettage were compared. METHOD: Fifty-two patients with sacrococcygeal pilonidal disease were divided into two groups: Group A consisted of 23 patients treated with marsupialization technique, and group B was composed of 29 patients submitted to incision and curettage. Postoperative follow-up varied between one and three years, from 1981 to 1999. Mean age for group A was 25.39 ± 8.92 years and for group B 25.52 ± 9.98 years. There were 28 females and 24 males. RESULTS: The average duration of the surgery for group A was 38.91 ± 19.77 minutes and for group B 47.34 ± 14.34 minutes. The mean in-ward duration time was 3,26 days for group A and 1.28 day for group B. Regarding antibiotic use, no statistical difference was observed between groups (p>0.05). Average wound healing time associated with marsupialization was 29.30 ±13.24 days, and for incision and curettage 40.76 ± 26.56 days. Postoperative pain occurred in 72.4% of the patients in group B and 17,4% of group A (p = 0,0001). Wound bleeding was absent in patients of group A but occurred in 29.7% of patients of group B (p=0,02829). The incidence of postoperative infection did not differ between the two groups (4.4% and 3.5% respectively). Group B presented a higher reccurring rate (27.6%) compared to group A (4.3%)(p=0,03364). CONCLUSIONS: Healing appears to be more efficient in patients undergoing marsupialization technique. Incision and curettage technique presents greater number of complications, with pain and bleeding occuring more often than with the marsupialization technique. The incision and curettage technique has a higher recurring rate than the marsupialization technique.

OBJETIVO: Avaliar os efeitos da esplenectomia total e da vagotomia gástrica proximal (VGP) isoladas e associadas sobre a vascularização gástrica. MÉTODOS: Utilizaram-se 28 cães, distribuídos em quatro grupos: grupo A, laparotomia e manipulação gástrica; grupo B, VGP; grupo C, VGP associada à esplenectomia; grupo D, esplenectomia isolada. Todos os animais foram mortos no 7º pós-operatório e imediatamente infundiu-se o corante na aorta torácica. O estômago aberto pela grande curvatura foi fotografado. Na superfície mucosa gástrica, a formação de áreas coradas e desprovidas de coloração foi mensurada por três métodos: decomposição milimétrica, planimétrico e morfometria computadorizada. RESULTADOS: Verificou-se nos grupos B e C formação de área desprovida de coloração na pequena curvatura gástrica, sendo significativamente maior no grupo C (p< 0,05). Nos grupos A e D tais áreas não foram observadas. A análise estatística não revelou diferenças significativas entre os resultados dos três métodos utilizados. CONCLUSÕES: Nas condições desse estudo os resultados permitiram concluir que nos animais com o estômago íntegro e naqueles onde a esplenectomia foi realizada de forma isolada, a vascularização do estômago permitiu a perfusão de toda a superfície do órgão. Nos animais submetidos a VGP isolada e principalmente quando a VGP foi associada ã esplenectomia ocorreu significativa diminuição do suprimento sangüíneo na curvatura gástrica menor, sugerindo a potencialidade isquêmica dessa região.

OBJECTIVE: The aim of the present study was to evaluate the effects of total splenectomy and proximal gastric vagotomy (PGV) isolated or associated to the vascularization of the stomach. METHODS: Twenty-eight dogs were distributed in: group A - animals undergoing laparotomy and gastric manipulation; group B - animals undergoing PGV; group C - animals undergoing PGV and splenectomy; and group D - animals undergoing splenectomy alone. All animals were sacrificed on the 7th postoperative day, and immediately followed by infusion of xanthene dye into the thoracic aorta. On the gastric mucous surface, the formation of stained areas and other areas lacking staining was measured by millimeter squares, planimetry and the computerized morphometry method. RESULTS: The formation of an area lacking in staining along the lesser gastric curvature was seen in all animals of groups B and C, which was significantly increased in group C, although in groups A and D such areas were not observed. CONCLUSION: These results showed that, in animals with splenectomy, the vascularization of the stomach was capable of perfusing the whole organ surface, but in animals with PGV alone or associated with splenectomy there was a notable decrease in blood supply throughout the area of the lesser gastric curvature, which suggests the ischemic impairment of this region.

OBJETIVO: o objetivo deste estudo retrospectivo foi analisar os resultados de 25 doentes com linfoma gástrico primário operados com intenção curativa. MÉTODO: os dados foram obtidos pela revisão dos prontuários e contato com os doentes ou familiares. A doença foi estadiada pelo sistema Ann Arbor modificado por Musshoff e Schmidt-Vollmer e a classificação histológica utilizada foi o sistema de Kiel. O esquema de radioterapia utilizado foi o CHOP e a radioterapia aplicada foi de 2000 a 4000 cGy. RESULTADOS: os sintomas e sinais clínicos assemelhavam-se aos da doença péptica ulcerosa ou do carcinoma gástrico Obteve-se o diagnóstico pré-operatório pela biópsia endoscópica em três casos e a exploração cirúrgica foi necessária para o diagnóstico nos restantes. No pré-operatório, sete doentes (30,4%) foram submetidos ao mielograma, que foi normal. Todos os pacientes foram submetidos à ressecção (12 gastrectomias subtotais e 13 gastrectomias totais) com retirada dos linfonodos regionais. Dez doentes (40%) receberam tratamento complementar (quimioterapia e/ou radioterapia). O estadiamento foi significativamente mais avançado nas lesões fundocárdicas e nos mais idosos e a sobrevida média foi de 31,5 meses. CONCLUSÕES: nesta série, as variáveis que influenciaram significativamente os índices de sobrevida foram a idade e o estádio avançados, o tamanho da lesão maior que 6,0cm e a realização do tratamento adjuvante pós-operatório (p< 0,05). Estes resultados sugerem que a ressecção completa da lesão com linfonodos adjacentes, acompanhada de tratamento adjuvante, constitui a melhor abordagem do linfoma gástrico primário ressecável.

BACKGROUND: we analyzed the results from 25 patients with primary gastric lymphoma operated with curative intention. METHODS: Data were obtained retrospective ly and by contacting patients or theirs relatives. The disease was staged using the Ann Arbor system for non-Hodgkin's lymphoma, as modified by Musshoff and Schmidt-Vollmer, and classified according to Kiel's system. RESULTS: The signs and symptoms were similar to those of peptic ulcer disease or gastric carcinoma. Preoperative diagnosis was obtained by endoscopic biopsy in three cases and by surgical exploration in the remaining. Seven patients (28%) were submitted to myelograms, which were normal in all cases. All patients were submitted to resection (12 subtotal gastrectomies and 13 total gastrectomies) with removal of regional lymph nodes. Ten of them (40%) received complementary treatment (chemotherapy and/or radiotherapy). The staging was significantly more advanced in fundocardiac lesions and in more elderly patients, and the average survival was 31.5 months. CONCLUSIONS: in this series, the variables that influenced significantly survival rate were age, advanced stage, size of the lesion higher than 6,0 cm, and adjuvant postoperative treatment (p< 0,05). These results suggest that complete resection of the lesion with the adjacent lymph nodes, accompanied by adjuvant treatment, constitutes the best approach to resectable primary gastric lymphoma.

São apresentados 18 casos de neoplasias primárias do duodeno, salientando-se sua raridade e as dificuldades diagnósticas, a despeito dos modernos recursos endoscópicos e de identificação por imagem. A literatura revela que de 1% a 10% de todos os tumores do aparelho digestório se situam no intestino delgado. A localização duodenal é, por isso, muito pouco freqüente, possui sintomatologia indefinida e implica conduta terapêutica mais complexa do que quando o tumor se situa em outros segmentos do intestino delgado. Os sintomas mais comuns são dor, náusea, vômito e hemorragia. A presente casuística consiste de 11 neoplasias malignas e sete benignas. Entre as primeiras o adenocarcinoma é o mais freqüente, sendo os lipomas os mais comuns dos neoplasmas benignos. Enquanto nestes a exérese local representa a conduta mais adequada, a duodenopancreatectomia cefálica foi a cirurgia de escolha nos tumores malignos da segunda porção duodenal, exibindo bons resultados. As lesões malignas da terceira porção e principalmente da quarta porção do duodeno foram tratadas pela ressecção duodenojejunal, também com resultados satisfatórios.

Eighteen primary duodenal tumors, including 11 malignant and 8 benign, are presented, stressing their poorly defined natural history and rare frequency. The most common histological diagnosis was adenocarcinoma. There were 6 adenocarcinomas, 2 carcinoids, 1 linfoma, 1 anaplastic and 1 neuroectodermic carcinoma. Benign lesions were occasionally found during a gastroduodenal or a biliary surgery (n = 4) and during the autopsies (n = 3). They consisted of lipomas (n = 4), adenomatous polyps (n = 2) and leiomyoma (n = 1). Concerning the malignant lesions, 6 pancreaticoduodenectomies, 3 duodenojejunal segmentary resection and one paliation consisting of gastrojejunostomy and biopsy were performed. One carcinoid lesion was found at the duodenal bulb during a gastroduodenectomy for a gastric ulcer. Pancreaticoduodenectomy can be done with acceptable risk and it seems that patients with duodenal adenocarcinoma are more likely to survive longer after radical surgery than those with cancer of the head of pancreas. Every patient of this series treated by pancreaticoduodenectomy or duodenojejunal segmentary resection had an uneventful recovery and the follow-up ranged from 16 months to 7 years. As for the duodenal carcinoids, local resection is usually followed by recurrence, so an agressive surgical management is the best approach. Benign tumors must be treated by local excision when disclosed.

O intento do presente estudo foi avaliar em cães os efeitos de dois tipos de procedimentos na prevenção do refluxo gastroesofágico. Foram utilizados 30 animais, divididos em grupos de 10. No Grupo I (controle) foi realizada a esofagogastrostomia à Gröndhal; no Grupo II praticou-se a esofagogastroplastia à Thal-Hatafuku; no III, a esofagogastrostomia foi associada à gastrectomia dois terços e anastomose gastrojejunal em Y. Foram analisados os seguintes parâmetros: peso, endoscopia, exame radiológico e estudo morfológico do esôfago. Para estimular a secreção ácida do estômago foi aplicada diariamente injeção de histamina em cera de abelha. Os resultados foram obtidos em três fases: pré-operatório, entre o 30°.e o 40o.pós-operatório e após aplicação de histamina. No Grupo I, houve queda de peso significante entre as duas primeiras fases, que se acentuou na terceira; no Grupo II, a alteração de peso não foi significativa nas três fases; no III, a queda de peso significante foi verificada entre a 1a e a 2a fase, não havendo registro na fase seguinte devido a morte precoce dos animais com a aplicação da droga. À endoscopia, verificou-se intensidade da esofagite significativamente maior nos animais do Grupo I do que nos do II, após estímulo histamínico. Nos cães do Grupo III, não foi possível obter-se este dado pelo mesmo motivo relatado na análise do parâmetro anterior. O estudo radiológico demonstrou que nos cães do Grupo I o refluxo foi franco em 70% dos cães e, moderado em 30%. No Grupo II ,o refluxo foi ausente na maioria dos cães mas, moderado em 30%. No III, o conteúdo gástrico de bário refluiu em todos os animais e de modo significante na maioria deles (70%). Os dados macro e microscópicos não mostraram diferença significativa entre os três grupos, contudo o Grupo II foi o menos acometido. Os resultados do experimento evidenciaram que a esofagogastrostomia, como se esperava, produz intenso refluxo gastroesofágico; a esofagocardioplastia mostrou ter importante eficácia anti-refluxo e menor morbidade; a cirurgia realizada no grupo III teve morbidade elevada e mortalidade precoce com o estímulo histamínico. Acredita-se que a esofagogastroplastia tenha lugar reservado entre os procedimentos destinados ao tratamento de casos selecionados de acalásia e de estenose péptica do esôfago.

The aim of this study was to evaluate in dogs the effects of two kinds of procedures in the gastroesophageal reflux prevention. Thirty animals divided in three randomized groups of ten were analysed as follow: group I (control) - esophagastrostomy side-to-side ; group II - esophagogastroplasty; group III -esophagogastrostomy side-to-side, partial gastrectomy and gastrojejunoanastomosis with excluded loop on y, The following parameters were used: body weight, endoscopy, radiological study and macro and microscopy data of the inner surface of the esophagus. The animals received daily histamine-in-beeswax parenterally for the posoperative stimulation of the gastric acid output until death or sacrifice. The research was carried out in three phases: préoperative phase, between the 35° and the 40° postoperative day and after histamine application. Group I showed sgnificant weight loss between the 1st and 2nd phase, which was intense on the 3rd phase. Group II showed no significative weight changes in any phase. Group III revealed significant weight changes even without histaminic stimulus. Endoscopy brought out significant more intensive esophagitis in group I than in II, after histaminic stimulus. In III, it was not possible to obtain these results, because of the precocious death of the animals. Fluoroscopic examination showed that 70% of the animals from group I, exhibited significative reflux, while in 30% this complication was not present. In group II, the reflux ocurred in few dogs and was not seen in 70% of the dogs.. Group III, revealed reflux in all animals and of significant manner in 70% of then. The macro and microscopic data did not show a significant difference among the groups, however the group II was the least committed. The results of this experiment demonstrated that the esophagogastrostomy side-to-side as expected produce intense esophageal reflux, the esophagogastroplasty showed minor morbidity and efficacy to impede the action of esophageal reflux and the surgery performed in group III, exhibited raised morbidity and early mortality after histamine stimulus. It is believed that the esophagogastroplasty takes reserved place in the procedures suggested to the treatment of achalasia and of peptique stenosis of the esophagus.

É apresentada uma conduta cirúrgica para o descolamento do duodeno nas ressecções gastroduodenais por úlcera terebrante na cabeça do pâncreas. Constituem os fundamentos dessa tática a mobilização retrógrada adequada da segunda porção do duodeno por meio da manobra de Kocher, a secção oblíqua do duodeno na altura da borda distal do nicho ulceroso e a introdução, pelo cirurgião, do seu dedo indicador na luz da víscera para palpar a papila duodenal maior. Essa medida permite encontrar o plano de clivagem para separar a parede duodenal da cabeça do pâncreas e afastar o risco de lesão das vias biliares e pancreáticas. O duodeno, assim preparado, possibilita sua utilização para eventual anastomose gastroduodenal ou sua exclusão quando se deseja proceder a gastrojejunostomia.

Beginning in the 1970s, many duodenal ulcers have been adequately managed using H2 receptor blockers or proton pump inhibitors associated to antimicrobial agents. However in the bleeding penetrating duodenal ulcer when the endoscopic treatment is lacking, the severe character of the hemorrhage and the frequence of the recurrence sometimes impose a radical surgery as early as possible, in spite of technical difficulties. Considering these facts, a surgical approach to the duodenal dissection during the gastroduodenectomy for penetrating ulcers is presented. This maneuver basically consist of (1) the adequate retrograde liberation of the descending portion of duodenum, (2) the oblique section of the duodenum at the lower border of the ulcer and (3) the introduction of the surgeon's forefinger into the duodenal lumen in order to facilitate the wall liberation from the pancreas achieved through a blunt dissection with a fine scissor or a Halsted forceps. The duodenal stump is now prepared for a gastroduodenal anastomosis or for closure by suture previously to a gastrojejunostomy. This technique have been used by one of us for many times with fairly good results.

Estudo prospectivo da incidência de complicações infecciosas, após operações para câncer colorretal. Sessenta e nove pacientes foram divididos em dois grupos, tendo o primeiro grupo recebido neomicina e metronidazol, por via oral, associados a gentamicina e metronizadol por via parenteral e o segundo grupo somente gentamicina e metronidazol por via parenteral. Foi estabelecido como objetivo principal a avaliação da influência do antimicrobiano administrado no preparo intestinal sobre a incidência de complicações infecciosas pós-operatórias. Os pacientes que receberam antimicrobianos por via oral no preparo intestinal apresentaram menor porcentagem de complicações infecciosas (14,29%) em relação aos pacientes que receberam apenas antimicrobiano por via parenteral (38,24%), sendo esta diferença estatisticamente significante, em nível de 5%. Esses dados apóiam a sugestão de associar antimicrobianos por via oral aos antimicrobianos por via parenteral na tentativa de reduzir as complicações infecciosas na cirurgia colorretal.

According to literature data there is a fifty percent chance of infectious complication after colorectal surgery when antimicrobial agents are not used. Those drugs were first employed orally with fair results. A few years latter the parenteral route was introduced either alone or associated to administrations per os. This work presents a prospective study on infectious complications after coloretal cancer operations in 69 patients divided into two groups. The first group received oral neomycin and metronidazole in addition to parenteral gentamycin and metronidazole; the second group received only parenteral gentamycin and metronidazole. Every patient had a previous bowel cleansing by oral administration of a 1.500ml solution of 10% manitol. The main objective was to evaluate the influence of administering antimicrobials for intestinal preparation on the incidence of postsurgical infectious complications. The abdominal wall soiling, intra-abdominal sepsis and/or fistula were considered as infectious complication. In those cases a bacterial culture was performed. Patients receiving oral antimicrobials during bowel preparation presented lower percentage of infectious complications (14,2%) compared to those receiving only parenteral antimicrobials (38,24%); this difference being statistically significant at the level of 5%. Oral antimicrobials for bowel preparation were efficient in reducing the incidence of infectious complications after surgery for colorectal cancel: These data confirm the recommendation for the association of oral and parenteral antimicrobials for best prevention of infectious complications.

O receio de graves complicações pós-operatórias tem inibido muitas das indicações cirúrgicas da litíase biliar no paciente idoso sintomático. A finalidade principal deste trabalho foi averiguar a extensão real desse problema no Serviço de Gastroenterologia Cirúrgica do HSPE-FMO. Foram estudados 185 idosos portadores de litíase biliar com idade média de 73,0 ± 6,2 anos, no período de seis anos (1990-1995). Os pacientes foram divididos em dois grupos de acordo com a idade: I - com 118 (63,8%) pacientes de 60 a 74 anos e o II - com 67 (36,2%) pacientes entre 75 e 90 anos. O número de doentes do sexo masculino foi proporcionalmente maior no grupo II (p<0,02). Os sintomas foram similares nos dois grupos de enfermos. A maioria dos doentes foi submetida à operação eletiva-163 (88,1 %) sendo 22 (11,9%) operados de urgência por colecistite aguda. Maior número de cirurgias de urgência incidiu no grupo 11 (19,4% contra 7,6%). A colecistectomia foi realizada em todos os doentes. Cirurgia complementar indicada pela presença de coledocolitíase (15,1 %) e estenose papilar (2,7%) foi necessária em 38 (20,5%) deles, sendo maior no grupo II. A coledocolitotomia foi realizada em 28 (15,1%) doentes, a anastomose biliodigestiva em sete (3,8%) e a papilotomia em cinco (2,7%) doentes, não diferindo entre os dois grupos. Complicações pós-operatórias ocorreram em 37 (20%) doentes e foram as mesmas nos dois grupos. Não houve óbitos. Nossos resultados demonstram que a colecistectomia eletiva pode ser realizada com baixa morbidade e sem mortalidade em idosos, quando se impede a demora da indicação operatória em pacientes sintomáticos.

Due to high incidence of postoperative complications in elderly people there is some degree of reluctance in the surgical management of the biliar lithiasis in these patients. The main purpose of this study was to verify the real extension of these problem in the Department of Gastroenterologic Surgery of the HSPE-FMO. Over a 6-year period (1990-1995) 185 elderly patients with biliary lithiasis and mean age of 73.0 ± 6,2 years were evaluated. The patients were divided into two groups according to the age: I-118 (63.8%) patients with 60-74 and II- 67 (36.2%) patients with 75-90 years. The number of mate patients was significantly higher at the group II. Symptoms were similar into two groups of patients. The most part of patients was submitted to elective procedures - (163) 88.1% while (22) 1.9% underwent emergency surgery (acute cholecystitis). The number of emergency cases was more than twice in the group II (19.4% versus 7.6%). Cholecystectomy was performed in all patients. Operative cholangiography was performed in 157 (84,8%) patients. Complementary surgery was necessary for 38 (21.6%)patients (choledocholithiasis in 15.1% and papillar stricture in 2.7%) and was higher in the group II. Choledocholithotomy as a component of the primary biliary operation was performed in 28 (15.1%) patients, biliar enteric anastomosis in seven (3.8%) and papillotomy in five (2.7%) patients. Complications occurred in 37 (20%) patients and were similar in both groups. There was no mortality. Retained stones of the common duct was found in 15 (8.1%) patients. Endoscopic sphincterotomy was performed upon all patients, with success. The results of the present study support the use of elective cholecystectomy with low morbidity and without mortality in geriatric patients.
