Abstract Objective The study aims to demonstrate an evaluation method to predict the functional success of total wrist arthrodesis (TWA) and assist its indication. Methods A prospective study including ten patients submitted to (TWA) posttraumatic arthritis. Exclusion criteria were patients who lost postoperative follow-up or incomplete information in the medical record. The objective functional assessment (handgrip strength, three-point pinch, lateral pinch and pulp-pulp pinch) and the subjective functional assessment (DASH, PRWE, EVA) were evaluated in 3 different periods: (1) Before surgery without articular anesthesia, (2) Before surgery under articular anesthesia and (3) 12 weeks after the surgical procedure. Results There was an increase in handgrip strength in all three pinches measurements after pain relief, both after joint anesthesia and after the consolidation of the arthrodesis (p < 0.05). In the comparisons between the subjective evaluations (DASH, PRWE and VAS), the patients had better scores in the postoperative evaluation after 12 weeks (p < 0.05). There was no statistical difference when comparing the mean strength values found after anesthesia and after 12 weeks of TWA. Conclusion the outcomes could propose an assessment protocol for patients with indication for TWA, in which patients with good response to intra-articular anesthetic infiltration would benefit from the effects of the surgical procedure.

Resumo Objetivo O estudo tem como objetivo demonstrar um método de avaliação para predizer o sucesso funcional da artrodese total do punho (ATP) e auxiliar na sua indicação. Métodos Um estudo prospectivo incluindo dez pacientes submetidos a ATP por artrite pós-traumática. Os critérios de exclusão foram pacientes que perderam o acompanhamento pós-operatório ou informações incompletas no prontuário. A avaliação funcional objetiva (força de preensão manual, pinça de três pontos, pinça lateral e pinça polpa-a-polpa) e a avaliação funcional subjetiva (DASH, PRWE, EVA) foram avaliadas em 3 momentos diferentes: (1) Antes da cirurgia sem anestesia articular, (2) Antes da cirurgia sob anestesia articular e (3) 12 semanas após o procedimento cirúrgico. Resultados Houve aumento da força de preensão palmar nas três medidas de pinça após o alívio da dor, tanto após a anestesia articular quanto após a consolidação da artrodese (p < 0,05). Nas comparações entre as avaliações subjetivas (DASH, PRWE e VAS), os pacientes tiveram melhores escores na avaliação pós-operatória após 12 semanas (p < 0,05). Não houve diferença estatística ao comparar os valores médios de força encontrados após a anestesia e após 12 semanas de ATP.

ABSTRACT The piramutaba, Brachyplatystoma vaillantii is a freshwater catfish that is the most abundant fishery resource in the Amazon estuary. Piramutaba trawling is done on industrial fishing scale and is characterized by the presence of many freshwater and marine bycatch species, with and without commercial value. Here we describe the bycatch of the industrial fishery of piramutaba in the Amazon estuary and evaluate the relationship of two important environmental factors, depth and salinity, with the accidental capture of freshwater and marine fishes in the Amazon estuary in the rainy and dry seasons. We identified 21 cartilaginous fish species (19.1% freshwater and 80.9% marine) and 125 bony fish species (25.6% freshwater and 74.4% marine). The bycatch included 64 species without commercial value (43% of all bycatch species), which are always discarded. Freshwater and estuarine fishes exhibited significantly higher abundances in shallower environments, while marine fishes were similarly abundant along the entire depth gradient. On the contrary, the abundance of freshwater fishes significantly decreased, and that of estuarine and marine fishes significantly increased with increasing salinity. Regarding the conservation status of the bycatch species, one is classified as vulnerable (VU), and seven as critically endangered (CR). The information on the bycatch of piramutaba fishery in the Amazon estuary is important to subsidize regional fisheries policies and the management of protected areas.

RESUMO A piramutaba, Brachyplatystoma vaillantii é um bagre de água doce que representa o recurso pesqueiro mais abundante no estuário amazônico. O arrasto da piramutaba é feito em escala industrial, caracterizado pela presença de muitas espécies de água doce e marinha capturadas de forma incidental, com e sem valor comercial. Aqui descrevemos a captura incidental da pesca industrial de piramutaba no estuário amazônico e avaliamos a relação de dois fatores ambientais, profundidade e salinidade, com a captura incidental de espécies de água doce e marinhas nas estações chuvosa e seca. Identificamos 21 espécies de peixes cartilaginosos (19,1% de água doce e 80,9% marinhos) e 125 espécies de peixes ósseos (25,6% de água doce e 74,4% marinhos). A captura incidental incluiu 64 espécies sem valor comercial (43% de todas as espécies capturadas) que sempre são descartadas. Os peixes de água doce e estuarinos exibiram abundâncias significativamente maiores em ambientes mais rasos, enquanto os peixes marinhos foram igualmente abundantes ao longo de todo o gradiente de profundidade. Inversamente, a abundância de peixes de água doce diminuiu significativamente, e a de peixes estuarinos e marinhos aumentou significativamente em salinidades maiores. Em relação ao estado de conservação das espécies capturadas, uma é reconhecida como vulnerável (VU) e sete como criticamente ameaçadas (CR). As informações sobre a captura incidental da pesca da piramutaba no estuário amazônico são fundaentais para subsidiar políticas pesqueiras regionais e gestão de áreas protegidas.

Resumo O tétano é uma doença grave e rara em pequenos animais, de caráter agudo e mediada pela ação da neurotoxina tetanospasmina, proveniente do bacilo Clostridium tetani. Neste relato descreve-se um caso de tétano em um canino, com ênfase em seus aspectos clínicos e terapêuticos, além da sua recuperação clínica. Foi atendido no Hospital Veterinário da Universidade de Passo Fundo, um canino, fêmea, da raça Pitbull, com cinco meses, em status epilepticus. Após a estabilização da paciente, constataram-se animal em posição de cavalete, com paralisia espástica generalizada, risus sardonicus, trismo, cauda e orelhas eretas e dificuldade em expandir o tórax. O diagnóstico presuntivo de tétano foi firmado a partir da anamnese, sinais clínicos e achados laboratoriais. O tratamento suporte estabelecido incluiu reposição hidroeletrolítica, soro antitetânico, antibioticoterapia, analgesia, fármacos miorrelaxantes e anticonvulsivantes. O manejo intensivo da paciente foi realizado com sondagem uretral e nasoesofágica, trocas de decúbito a cada duas horas e diminuição de estímulos ambientais. A fisioterapia e a acupuntura também foram utilizadas para complementar o tratamento e acelerar a recuperação. A paciente recebeu alta médica um mês após o início do tratamento, apresentando evolução satisfatória. No presente caso, o exame clínico em associação aos sinais clínicos característicos da doença, acrescidos da anamnese minuciosa foram fundamentais ao diagnóstico presuntivo de tétano. Além disso, o manejo intensivo, o tratamento medicamentoso, assim como a realização de fisioterapia e acupuntura, possibilitaram a evolução do quadro clínico à cura.

Abstract Tetanus is a serious and rare disease in small animals, of an acute nature and mediated by the action of the neurotoxin tetanospasmin, from the bacillus Clostridium tetani. This report describes a case of tetanus in a canine, with emphasis on its clinical and therapeutic aspects, in addition to its clinical recovery. A canine, female, Pitbull breed, five months old, in status epilepticus, was attended at the Veterinary Hospital of the University of Passo Fundo. After stabilizing the patient, the animal was found in a trestle position, with generalized spastic paralysis, risus sardonicus, trismus, erect tail and ears, and difficulty in expanding the thorax. The presumptive diagnosis of tetanus was established based on the anamnesis, clinical signs, and laboratory findings. The established supportive treatment included hydro electrolytic replacement, antitetanus serum, antibiotic therapy, analgesia, myorelaxant and anticonvulsant drugs. The patient's intensive management was carried out with urethral and gastroesophageal probing, changes of position every two hours, and reduction of environmental stimuli. Physiotherapy and acupuncture were also used to complement the treatment and accelerate recovery. The patient was discharged one month after the beginning of the treatment, presenting satisfactory evolution. In the present case, the clinical examination in association with the characteristic clinical signs of the disease, added to the detailed anamnesis, was essential for the presumptive diagnosis of tetanus. In addition, intensive management, drug treatment, physiotherapy, and acupuncture enabled the evolution of the clinical condition to cure.

RESUMO Objetivo: Analisar o excesso de óbitos no município do Rio de Janeiro (MRJ), RJ, durante a pandemia de COVID-19 (março de 2020 a janeiro de 2022). Métodos: Foi realizado um estudo ecológico com dados secundários do Sistema de Informação sobre Mortalidade cuja unidade de análise foi o MRJ. O excesso de mortalidade foi calculado pela diferença entre a média de óbitos esperados e a média dos óbitos observados levando-se em conta o período de 2015 a 2019. Foi ajustado um método de regressão quantílica. Calculou-se o valor total dos casos acima do esperado pela série histórica. Foram selecionados os óbitos por causa básica COVID-19 e Influenza. Também foi calculada a razão entre o excesso de óbitos e os óbitos atribuídos à COVID-19. Resultados: Foi identificado excesso de 31.920 óbitos pela média (26,8% de incremento). Pela regressão quantílica, encontrou-se excesso de 31.363 óbitos. Ocorreram 33.401 óbitos por COVID-19 e 176 por Influenza. A razão entre o excesso de óbitos encontrado e os óbitos atribuídos à COVID-19 foi de 0,96 pela média e 0,95 pela regressão quantílica. Conclusão: O estudo apontou grande excesso de óbitos durante a pandemia de COVID-19 no MRJ, distribuído em ondas, incluindo-se o período do surto de Influenza.

ABSTRACT Objective: To evaluate excess mortality in the city of Rio de Janeiro, Brazil, due to the COVID-19 pandemic (March 2020 to January 2022). Methods: Ecological study using secondary data from the Brazilian Mortality Information System, having the city of Rio de Janeiro as the unit of analysis. Excess mortality was estimated by the difference between the mean number of all expected deaths and the mean number of observed deaths, considering the 2015–2019 period. The quantile regression method was adjusted. The total value of cases above that expected by the historical series was estimated. Among all deaths, cases of COVID-19 and Influenza as underlying causes of death were selected. The ratio between excess mortality and deaths due to COVID-19 was calculated. Results: We identified an excess of 31,920 deaths by the mean (increase of 26.8%). The regression pointed to 31,363 excess deaths. We found 33,401 deaths from COVID-19 and 176 deaths from Influenza. The ratio between the verified excess mortality and deaths due to COVID-19 was 0.96 by the mean and 0.95 by the regression. Conclusion: The study pointed to large excess deaths during the COVID-19 pandemic in the city of Rio de Janeiro distributed in waves, including the period of the Influenza outbreak.

ABSTRACT BACKGROUND: Achalasia is an esophageal motility disorder, with clinical presentation of dysphagia and regurgitation. This is a chronic condition with no cure. Current treatment options aim to reduce lower esophageal sphincter tone by pharmacological, endoscopic or surgical means, with the aim of improving patients’ symptoms. Peroral endoscopic myotomy (POEM) is an alternative endoscopic surgery to Heller cardiomyotomy, in which the procedure is performed orally, by endoscopy, offering efficacy comparable to surgical myotomy, with relative ease and minimal invasion, without external incisions. AIMS: To study the safety of POEM by analyzing its results, adverse events and perioperative complications and the main ways to overcome them, in addition to evaluating the effectiveness of the procedure and the short-term postoperative quality of life. METHODS: A qualitative and quantitative, observational and cross-sectional study that analyzed patients who underwent the POEM in a reference center, from December 2016 to December 2022, maintaining the technical standard of pre-, peri- and postoperative protocol. RESULTS: A total of 94 patients were included in the study, and only three had postoperative complications. The average early postoperative Eckardt score was 0.93 and the late 1.40, with a mean improvement of 7.1 in early results and 6.63 in late results (p<0.05). CONCLUSIONS: POEM can be reproduced with an excellent safety profile, significant relief of symptoms and improvement in esophageal emptying, and in quality of life.

RESUMO RACIONAL: A acalasia é uma desordem da motilidade esofágica, com apresentação clínica de disfagia e regurgitação, uma condição crônica sem cura. As opções de tratamentos atuais visam reduzir o tônus do esfíncter esofagiano inferior (EEI) por meios farmacológicos, endoscópicos ou cirúrgicos, com o objetivo de aliviar os sintomas dos pacientes. A miotomia endoscópica peroral, sigla do inglês (POEM) é uma cirurgia endoscópica alternativa à cardiomiotomia a Heller (CH), em que a miotomia é realizada por via oral, por endoscopia, oferecendo eficácia comparável a miotomia cirúrgica, com relativa facilidade e mínima invasão, sem incisões externas. OBJETIVOS: Analisar a segurança do POEM analisando seus resultados, eventos adversos, complicações perioperatórias e suas principais formas de contorná-los, além de avaliar a eficácia do procedimento e a qualidade de vida pós-operatória, em curto prazo. MÉTODOS: Estudo qualitativo e quantitativo, observacional e transversal avaliando pacientes que realizaram o POEM em um centro de referência, entre dezembro de 2016 até dezembro de 2022, mantendo padrão técnico de preparo pré, peri e pós-operatório. RESULTADOS: Foram incluídos no estudo 94 pacientes e apenas 3 apresentaram complicações pós-operátorias. O escore de Eckardt médio precoce pós-operatório foi de 0,93 e o tardio de 1,40, com melhora média de 7,1 em resultados precoces e de 6,63 em resultados tardios (p<0,05). CONCLUSÕES: O POEM pode ser reproduzido com excelente perfil de segurança, alívio significativo dos sintomas e melhora do esvaziamento do esôfago, oferecendo melhora na qualidade de vida.

Abstract The Amazon River basins present distinct natural and anthropogenic characteristics that influence the structure of stream habitats and their associated biota. The influence of these characteristics can be evaluated through different spatial scales. We aimed to assess the influence (with and without the effect of spatial-geographical factors) of local, macroscale, and land-use variables in the structure of stream fish assemblages of Amazonian catchments with different deforestation levels. A partial redundancy analysis and a reduced metrics model were used to assess these influences. With geographic-spatial effects, we verified that the macroscale and local variables explained the variation in fish composition, and, without the effects, land use also explained the variation in this composition. In the forested catchments, the biota was associated with streams with natural characteristics (e.g., leaf banks). In the deforested catchments, it was associated with land use, sandy catchments with higher soil density (higher capacity of degradation), and less complex streams (fewer leaf banks, more sand). The associated fish have life features linked to these characteristics (e.g., Gymnorhamphichthys rondoni associated with sand). This configuration seems to be a result of both the impact of land use in the catchment (i.e., increased erosion, increased sedimentation) and the naturally sandy constitution of the catchment as well, reflecting the sandy substrate.

Resumo A Amazônia apresenta bacias hidrográficas com características naturais e antrópicas que estão presentes em distintas escalas espaciais da paisagem e que influenciam a estrutura do habitat de riachos e sua biota associada. Nosso objetivo foi verificar a influência (com e sem o efeito de fatores espaciais e geográficos) de variáveis locais, de macroescala e uso da terra na estrutura de assembleias de peixes de riachos de microbacias amazônicas com diferentes níveis de desmatamento. A análise de redundância parcial e modelo reduzido de métricas foram realizadas para verificar essas influências. Com os efeitos geográfico-espaciais, verificou-se que as variáveis de macroescala e locais explicavam a variação da composição dos peixes, e sem os efeitos, os usos da terra também explicaram. Nas microbacias florestadas, a biota foi associada a riachos com características naturais (e.g., bancos de folhas). Nas microbacias desmatadas, foi associada a usos da terra, a microbacias arenosas com maior densidade do solo (maior capacidade de degradação) e a riachos menos complexos (menos bancos de folhas e mais areia). Os peixes associados a essas microbacias desmatadas possuem aspectos de vida atrelados a essas características (e.g., Gymnorhamphichthys rondoni associado a areia). Essa configuração parece ser um reflexo tanto do impacto do uso da terra na microbacia (i.e., maior erosão, maior sedimentação) quanto da própria constituição natural arenosa da microbacia, refletindo o substrato arenoso.

ABSTRACT ABS resins have wide application in the market, mainly in the manufacture of household appliances, telephones, automotive industry and others, and need to be processed by extrusion for the manufacture of products. The variables used in the process of extrusion of polymers have a direct influence on their mechanical properties. The objective of this study was to evaluate the different speeds of the extruder screw (15, 30, 45 and 60 rpm) on the mechanical properties of raw ABS after the extrusion process. Izod impact resistance and tensile tests, were also performed. The fractures of the samples tested by Izod impact were observed via optical microscopy, scanning electronic microscopy (SEM), and roughness was measured. Calorimetric tests (DSC) and thermogravimetry (TGA), as well as melt flow index (MFI), were also performed. The most significant results were observed for the extruder screw speed of 60 rpm. For the impact test, a reduction in impact energy of approximately 3% was observed, with the same result for tensile resistance. However, an increase in the value of the elastic modulus and the MFI was observed due to the increase in the degree of crystallinity, which was caused by the higher shear of the polymer mass in the screw of the extrusion equipment. Additionally, an atypical case occurred for the ABS polymer in the highest rotation, since it was the polymer that suffered the greatest shear in the polymer chains due to the degradation of the butadiene phase.

Abstract Microgramma vacciniifolia, an epiphyte fern, hosts of two stem galls. One is induced by Tortrimosaica polypodivora (Lepidoptera), which can also induce galls in M. squamulosa and M. mortoniana. The alterations induced by T. polypodivora on M. vacciniifolia stem were compared to non-galled organs to evaluate the anatomical potentials of host ferns in response to a galling Lepidoptera. Histochemical and histometrical comparisons between galled and non-galled stems were performed to assess the processes leading to gall formation. M. vacciinifolia and M. squamulosa galls were anatomically similar, although their sizes differ, reflecting the growth potential of each host species. Simple structural alterations, such as hyperplasia of cortical and pericycle cells, occur during gall formation, while cell hypertrophy, common in more complex galls, was only detected on the pericycle. Meristele size remained unaltered in galls, but the pericycle appeared hyperplasic. The protective scales were broader in galls. A nutritive tissue with lipids, typical to Lepidoptera galls, was observed around the larval chamber, with small cells and meristematic activity. Starch, proteins, and reducing sugars accumulated in nutritive cells are uncommonly found in Lepidoptera galls. Despite simple structural alterations, T. polypodivora induced a gradient of primary metabolites, similar to angiosperm galls.

Resumo Microgramma vacciniifolia, uma samambaia epifíta, abriga duas galhas caulinares, uma delas induzida por Tortrimosaica polypodivora (Lepidoptera), que também induz galhas em M. squamulosa e em M. mortoniana. Alterações induzidas por T. polypodivora nos caules de M. vacciniifolia foram comparadas aos caules não-galhados, para avaliar os potenciais anatômicos das samambaias hospedeiras em resposta a um Lepidoptera galhador. Comparações histoquímicas e histométricas foram realizadas para avaliar os processos que levam à formação das galhas. As galhas de M. vacciinifolia e M. squamulosa são anatomicamente semelhantes, embora seus tamanhos diferem, refletindo o potencial de crescimento de cada espécie hospedeira. Alterações estruturais simples ocorrem durante a formação da galha, como hiperplasia das células corticais e pericíclicas, enquanto hipertrofia celular, comum em galhas mais complexas, é detectada apenas no periciclo. Os meristelos não têm o tamanho alterado, mas o periciclo apresenta hiperplasia. As escamas são mais largas nas galhas. Ao redor da câmara larval observa-se um tecido nutritivo com lipídios, típico das galhas de Lepidoptera, com células pequenas e atividade meristemática. Amido, proteínas e açúcares redutores também se acumulam nas células nutritivas, raramente relatado em galhas de Lepidoptera. Apesar das alterações estruturais simples, T. polypodivora induz o estabelecimento de um gradiente de metabólitos primários, semelhante ao observado em galhas de angiospermas.

Abstract This study aims to describe a case series of patients who underwent thoracic duct embolization (TDE) to treat traumatic iatrogenic chylothorax (TIC). Three patients were included: Case #1, a 49-year-old woman with follicular lymphoma developed a TIC following video-assisted thoracoscopic surgery to resect a solid right paravertebral mass and was treated with TDE using microcoils and N-butyl cyanoacrylate (NBCA) glue. Case #2, a 68-year-old man with cardiac amyloidosis developed a TIC following heart transplantation and was treated with TDE using microcoils and ethylene vinyl alcohol copolymer. Case#3: A 6-year-old patient with congenital heart disease developed a TIC following a Fontan procedure and was treated with TDE using NBCA glue. All lesions were identified during lymphangiography and TDE was successfully performed in all cases. TDE is a safe and valuable technique that provides minimally invasive treatment for TCI.

Resumo Este estudo objetiva descrever uma série de casos de pacientes submetidos a embolização do ducto torácico (EDT) para tratamento de quilotórax iatrogênico (QI). Três pacientes foram incluídos. Caso 1: um homem de 49 anos com linfoma folicular apresentou QI após ressecção de uma massa paravertebral por toracoscopia vídeo-assistida e foi submetido a EDT com micromolas e n-butil-cianoacrilato (NBCA). Caso 2: um homem de 68 anos com amiloidose cardíaca apresentou QI após ser submetido a transplante cardíaco e foi submetido a EDT com micromolas e copolímero de etileno e álcool vinílico. Caso 3: um paciente de 6 anos com malformação cardíaca congênita apresentou QI após cirurgia de Fontan e foi submetido a EDT com NBCA. Todas as lesões foram identificadas durante a linfangiografia, e a EDT foi realizada com sucesso. A EDT é uma técnica segura e valiosa, que pode oferecer um tratamento minimamente invasivo em casos de QI.

Abstract A 100-year-old male patient was admitted with a ruptured abdominal aortic aneurysm due to type IA endoleak. Given the proximity of the ruptured site to the superior mesenteric artery (SMA) and renal arteries, a ChEVAR was indicated. Catheterization of the target visceral vessels was a challenging procedural step because of an intensely tortuous thoracic aorta. This hostile aortic anatomy also inhibited exchange for a super stiff guide-wire and selective cannulation with the diagnostic catheter was repeatedly lost when guidewire exchange was attempted. To overcome this issue, a 5 x 40 mm balloon catheter was placed 3cm into the target arteries. The balloon was then inflated below the nominal pressure limit enabling safe exchange for a super stiff guidewire and placement of three 90-cm long 7Fr guiding sheaths. The procedure was thus safely performed with deployment of an aortic extension and the bridging stents.

Resumo Um paciente de 100 anos foi diagnosticado com um aneurisma de aorta abdominal roto por um endoleak 1A. Pela proximidade do ponto de ruptura com a artéria mesentérica superior (AMS) e as artérias renais, um ChEVAR foi indicado. A cateterização das artérias-alvo foi um passo desafiador pela intensa tortuosidade da aorta torácica. Essa anatomia aórtica hostil também impediu a troca por um fio-guia extra-rígido, e a cateterização seletiva foi repetidamente perdida quando a troca de fio-guia foi tentada. Para superar essa dificuldade, um cateter balão 5mm x 40mm foi posicionado nas artérias-alvo. O balão foi, então, insuflado abaixo da pressão nominal, permitindo uma troca segura do fio-guia por um fio-guia extra-rígido e o posicionamento de três bainhas longas 7Frx 90cm. Assim, o procedimento foi executado de forma segura, com o implante de uma extensão aórtica e dos stents recobertos.

ABSTRACT Phosphites can be considered an additional strategy to be included in disease management programs. Therefore, this study aims to evaluate the application of potassium phosphite (KPhi) in common bean seeds, in the reduction of intensity of root rot (Rhizoctonia solani) and in the compatibility with Rhizobium tropici, a nitrogen-fixing bacterium. The emergence speed index (ESI) and the percentage of total emergence were evaluated in seeds inoculated with R. solani and treated with KPhi. The number of nodules per root system, dry nodule mass, quantified the compatibility Phi x R. tropici, dry mass, nitrogen content, and nitrogen accumulation in the shoots. KPhi promoted a higher ESI and increase in plant stand compared to the control treatment. The number of nodules formed and the nitrogen content and accumulation in the KPhi treatments were similar to control (seeds inoculated with R. tropici). Thus, the treatment of seeds with KPhi provides control of R. solani and is compatible with R. tropici.

ABSTRACT Objective To describe the radiological characteristics of hepatocellular carcinoma (HCC) lesions that achieved a complete response following drug-eluting bead transarterial chemoembolization (DEB-TACE) preceding liver transplantation. Methods This single-center case-control study enrolled patients with hepatocellular carcinoma who underwent neoadjuvant DEB-TACE therapy, were followed up with contrast-enhanced magnetic resonance imaging or computed tomography, and were successively evaluated according to the modified Response Evaluation Criteria in Solid Tumors. The HCCs were divided into two groups based on their diameter (Group A: ≤3cm; Group B: 3cm). Viability was assessed using the Kaplan-Meier method according to tumor size categories. The relationship between tumor variables was analyzed using bivariate Cox regression. Results Three-hundred and twenty-eight patients with 667 hepatocellular carcinomas who underwent their first DEB-TACE session were enrolled. A total of 105 hepatocellular carcinomas in 59 patients exhibited complete response after the initial DEB-TACE session and were divided into Group A (92 HCCs) and Group B (13 HCCs). The diameter in Group A decreased significantly compared to the pre-procedure size until the second assessment (p<0.001), with no subsequent reduction in diameter, despite maintaining a complete response. In Group B, the reduction in diameter remained significant compared with the initial value until the sixth imaging evaluation (p=0.014). The average reduction was 45.1% for Group B and a maximum of 14.9% in Group A. Conclusion HCCs >3cm exhibited a greater reduction in size and a longer time to recurrence. HCCs ≤3cm had a shorter relapse time. The recurrence rates were similar. These findings may aid in planning for liver transplantation.

Resumo O tétano é uma doença grave e rara em pequenos animais, de caráter agudo e mediada pela ação da neurotoxina tetanospasmina, proveniente do bacilo Clostridium tetani. Neste relato descreve-se um caso de tétano em um canino, com ênfase em seus aspectos clínicos e terapêuticos, além da sua recuperação clínica. Foi atendido no Hospital Veterinário da Universidade de Passo Fundo, um canino, fêmea, da raça Pitbull, com cinco meses, em status epilepticus. Após a estabilização da paciente, constataram-se animal em posição de cavalete, com paralisia espástica generalizada, risus sardonicus, trismo, cauda e orelhas eretas e dificuldade em expandir o tórax. O diagnóstico presuntivo de tétano foi firmado a partir da anamnese, sinais clínicos e achados laboratoriais. O tratamento suporte estabelecido incluiu reposição hidroeletrolítica, soro antitetânico, antibioticoterapia, analgesia, fármacos miorrelaxantes e anticonvulsivantes. O manejo intensivo da paciente foi realizado com sondagem uretral e nasoesofágica, trocas de decúbito a cada duas horas e diminuição de estímulos ambientais. A fisioterapia e a acupuntura também foram utilizadas para complementar o tratamento e acelerar a recuperação. A paciente recebeu alta médica um mês após o início do tratamento, apresentando evolução satisfatória. No presente caso, o exame clínico em associação aos sinais clínicos característicos da doença, acrescidos da anamnese minuciosa foram fundamentais ao diagnóstico presuntivo de tétano. Abstract Tetanus is a serious and rare disease in small animals, of an acute nature and mediated by the action of the neurotoxin tetanospasmin, from the bacillus Clostridium tetani. This report describes a case of tetanus in a canine, with emphasis on its clinical and therapeutic aspects, in addition to its clinical recovery. A canine, female, Pitbull breed, five months old, in status epilepticus, was attended at the Veterinary Hospital of the University of Passo Fundo. After stabilizing the patient, the animal was found in a trestle position, with generalized spastic paralysis, risus sardonicus, trismus, erect tail and ears, and difficulty in expanding the thorax. The presumptive diagnosis of tetanus was established based on the anamnesis, clinical signs, and laboratory findings. The established supportive treatment included hydro electrolytic replacement, antitetanus serum, antibiotic therapy, analgesia, myorelaxant and anticonvulsant drugs. The patient's intensive management was carried out with urethral and gastroesophageal probing, changes of position every two hours, and reduction of environmental stimuli. Physiotherapy and acupuncture were also used to complement the treatment and accelerate recovery. The patient was discharged one month after the beginning of the treatment, presenting satisfactory evolution. In the present case, the clinical examination in association with the characteristic clinical signs of the disease, added to the detailed anamnesis, was essential for the presumptive diagnosis of tetanus.
