The aims of this study were to analyze whether age, tooth type and sex are related to radiographically visible extrusion of filling material as an unintended outcome in teeth undergoing endodontic treatment with a thermoplastic obturation technique (Calamus® or GuttaCore®) and to determine whether the presence of such overfilling is associated to postoperative pain. We selected 120 teeth with diagnoses involving vital pulp and indication for endodontic treatment. Biomechanical preparation was performed using the Protaper Next® system with X2 or X3 master apical file. Teeth were randomly divided into 2 groups (n=60). Teeth in Group 1 were filled with the Guttacore® system and teeth in Group 2 were filled with single cone technique and Calamus® obturation system. Thermoplastic obturation techniques were found to cause overfilling in 53. 33% of the total cases (64 teeth) (56. 66% in Guttacore® group and 50% in the Calamus® group). Anterior teeth were found to be associated to presence of overfilling (p= 0. 024) (OR = 4. 35). Of the 120 teeth treated, 10 (8. 33%) presented postoperative pain and radiographically visible overfilling. The association between presence of extruded filling material and mild/moderate pain was statistically significant withp = 0. 002. To conclude, endodontic thermoplastic obturation with Guttacore® and Calamus® systems are significantly associated with the probability of filling material extrusion, and overfilling is associated to postoperative pain. Anterior teeth are 4 times more likely to be overfilled with the obturation techniques tested.

Los objetivos de esta investigación fueron: Analizar si factores como la edad, el tipo de diente y el género están asociados con la extrusión de material obturador radiográficamente visible como desenlace imprevisto en dientes sometidos a tratamiento endodóntico con una técnica de obturación termoplástica (Calamus® o GuttaCore®) y determinar si la presencia de esta sobreobturación está asociada con dolor posoperatorio. Se seleccionaron 120 dientes con diagnósticos que implicaban pulpa vital y con indicación de tratamiento de endodoncia. La preparación biomecánica se llevó a cabo con el sistema ProtaperNext® con lima apical principal X2 o X3. Los dientes se dividieron aleatoriamente de la siguiente forma (n=60): El grupo 1 se obturó con el sistema Guttacore® y el grupo 2 se obturó con una técnica de obturación de cono único y el sistema de obturación Calamus®. Se observó quelas técnicas de obturación termoplástica generaron sobreobturación en 53. 33% del total (64 dientes), 56. 66% se presentaron en el grupo de obturación con Guttacore® y 50% en el grupo de la técnica con el sistema Calamus®. Se encontró que los dientes anteriores presentaron asociación con la presencia de sobreobturación (p= 0. 024) (OR=4. 35). De los 120 dientes tratados, 10 (8. 33 %) presentaron dolor posoperatorio y sobreobturación radiográficamente visible. La asociación entre la presencia de extrusión de material de obturación y dolor leve-moderado fue estadísticamente significativa con un valorp= 0. 002. Como conclusión las técnicas de obturación termoplástica en endodoncia Guttacore® y el sistema Calamus® están asociadas significativamente con la probabilidad de extrusión de material obturador y dicha sobreobturación está asociada con dolor posoperatorio, los dientes anteriores presentan 4 veces más riesgo de sobreobturación con las técnicas de obturación utilizadas.

La literatura empírica sobre el efecto de los conflictos violentos en variables educativas se ha enfocado en la acumulación de capital humano y ha dejado a un lado otras variables relevantes como la equidad educativa. Usando la implementación empírica de Ferreira y Gignoux (2011) del marco conceptual de igualdad de oportunidades de Roemer (1998), este artículo estima el efecto de la intensidad del conflicto armado sobre la equidad educativa en Colombia, medida a través del desempeño escolar en las pruebas Saber 11º para el período 1997-2010. Para resolver sesgos potenciales por simultaneidad o por variable omitida se utilizan las capturas de homicidas por parte de la fuerza pública como fuente de variación exógena de la violencia en una estimación de variables instrumentales. Los resultados apuntan a que el conflicto aumenta la equidad de oportunidades en los logros educativos. Al aumentar las tasas de deserción escolar, el conflicto genera una selección positiva en cuanto a habilidad y esfuerzo para la fracción de estudiantes que el sistema retiene. Esto sugiere que se debe tener cautela al interpretar los patrones de equidad en la educación basada en resultados, en lugar de procesos.

The empirical literature on the impact of violent conflicts on educational outcomes has focused on the impact of violence on human capital accumulation, neglecting other relevant outcomes such as educational equity. Using the empirical implementation of Ferreira and Gignoux (2011) of Roemer (1998)'s conceptual framework of equality of opportunities, this paper estimates the impact of conflict intensity on educational equity in Colombia, as measured by the school performance in the Saber 11º test, for the period 1997-2010. To deal with potential biases due to reverse causality or omitted variables, we use the apprehension of murderers by the public force as a source of exogenous variation of violence in an instrumental variables estimation. The results suggest that conflict increases equality of opportunities in educational attainment. By increasing school dropout, conflict induces a positive selection in terms of ability and effort for the proportion of students who remain in the system. This suggests that caution is needed when interpreting patterns of educational equity based on outcomes rather than processes.

In the present work, the nutritional status, intestinal parasitosis and socio-environmental conditions of children from the city centre and 11 localities of La Plata Department were characterized through a transversal study. Body weight and height were measured in 6.397 children aged 4.0 to 13.9 years old. Body mass index was calculated and the nutritional status estimated using NHANES III. A structured questionnaire was made to evaluate socio-environmental characteristics. Parasitological analyses were carried out in 729 children using fecal samples and implementing the Ritchie technique and anal brushes. The overall results indicated 22.1% of weight excess, 8.7% of undernutrition and 68.5% of parasitosis, being more prevalent Enterobius vermicularis (38.7%), Blastocystis hominis (36.1%), Giardia lamblia (19.1%), and commensal protozoa (26.1%). The worm burden showed a lower value (9.3%). La Plata Department presented great heterogeneity in the nutritional status and parasitic infection of children because they depend on the socio-environmental characteristics. Poor living conditions, low educational levels and lack of formal jobs would explain the co-occurrence of infant parasitic infection and undernutrition, being chronic the most prevalent type of undernutrition. On the other hand, the localities in which parents had a higher educational level and a formalized job -though less access to basic urban servicesshowed a greater prevalence of overweight.

En el presente trabajo se caracterizaron el estado nutricional, las enteroparasitosis y las condiciones socio-ambientales de niños residentes en el Casco urbano y 11 localidades del Partido de La Plata, provincia de Buenos Aires, mediante un estudio transversal. Se midieron peso y talla de 6.397 niños entre 4.0 y 13.9 años. Se calculó el índice de masa corporal y se estimó el estado nutricional utilizando NHANES III. Las variables socio-ambientales se relevaron mediante encuesta estructurada. El análisis parasitológico se realizó en 729 niños sobre muestras seriadas de materia fecal analizadas mediante la técnica de Ritchie y escobillado anal. Los resultados globales indicaron: 22.1% de exceso de peso, 8.7% de desnutrición y 68.5% de parasitosis, siendo más prevalentes Enterobius vermicularis (38.7%), Blastocystis hominis (36.1%), Giardia lamblia (19.1%) y protozoos comensales (26.1%). Los geohelmintos tuvieron valores inferiores (9.3%). El partido de La Plata presenta gran heterogeneidad en el estado nutricional y las infecciones parasitarias de los niños por cuanto resultan dependientes de las características socio-ambientales. Las deficientes condiciones ambientales, el bajo nivel educativo y la falta de trabajo formal de los padres explicarían la coexistencia de parasitosis y desnutrición infantil, prevaleciendo la de tipo crónico. Por otra parte, las localidades en las que hay menor grado de cobertura de servicios urbanos básicos, pero los padres poseen mayor nivel educativo y trabajo formalizado, presentan prevalencias más elevadas de exceso de peso.

Objective Assessing VNTR (variable-number tandem repeat) variability of Mycobacterium leprae from Colombian patients with and without prior treatment to identify potential sources of infection and to understand the patterns of disease transmission. Methodology This was a descriptive cross-sectional study where a convenience sample of biopsies was taken from 161 multibacillary leprosy patients; diagnosis and monitoring of the disease had been requested for these patients. DNA was extracted from M. leprae and standardised using the PCR technique for M. leprae VNTR, genotypes were established and different clusters grouped by unweighted pair group method with arithmetic mean (UPGMA). Results 22 different VNTR genotypes were found from 161 samples, of which 100 samples (62.1%) had a single u-VNTR genotype and the remaining genotypes were VNTR 17 (5.6%), VNTR 20 (4.3%), VNTR 18 (4.3%), VNTR 14 (4.3%) and VNTR 13 (3.7%), namely those forming groups or clusters. Conclusion This study showed that clones can be detected with varying degrees of virulence / aggressiveness by cluster analysis, implying the need for more monitoring programme activities which will result in a real decline in microorganism transmission.

Objetivo Evaluar la variabilidad de VNTR (variable-number tandem repeat) de Mycobacterium leprae de pacientes colombianos con y sin tratamiento previo para identificar posibles fuentes de infección y entender los patrones de transmisión de la enfermedad. Metodología Estudio transversal descriptivo, en donde mediante un muestreo electivo a conveniencia se tomaron 161 biopsias de pacientes multibacilares de lepra, que habían sido solicitadas para diagnóstico y seguimiento de la enfermedad, de las cuales se realizó extracción de ADN de M. leprae y usando la técnica de PCR para VNTRs de M. leprae estandarizada, se establecieron los genotipos y los diferentes clusters mediante el agrupamiento apareado UPGMA. Resultados En las 161 muestras totales se hallaron 22 genotipos VNTRs diferentes, de las cuales 100 muestras (62,1 %) pertenecían al genotipo único VNTRU, y de los genotipos restantes, los mayoritarios, es decir los que dieron lugar a formación de grupos o clusters fueron VNTR17 (5,6 %), VNTR20 (4,3 %), VNTR18 (4,3 %), VNTR14 (4,3 %) y VNTR13 (3,7 %). Conclusión En este estudio se evidencia por análisis de agrupamiento que se pueden detectar clones con diferente grado de virulencia/agresividad, lo cual implica la necesidad de incrementar varias de las actividades del programa de control que darán como resultado la verdadera disminución de la transmisión del microorganismo.
