Abstract Objectives: To review and provide evidence-based recommendations for the diagnosis and treatment of otosclerosis. Methods: Task force members were educated on knowledge synthesis methods, including electronic database search, review and selection of relevant citations, and critical appraisal of selected studies. Articles written in English or Portuguese on otosclerosis were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: 1) Diagnosis – audiologic and radiologic; 2) Treatment – hearing AIDS, pharmacological therapy, stapes surgery, and implantable devices - bone-anchored devices, active middle ear implants, and Cochlear Implants (CI). Conclusions: The pathophysiology of otosclerosis has not yet been fully elucidated, but environmental factors and unidentified genes are likely to play a significant role in it. Women with otosclerosis are not at increased risk of worsening clinical condition due to the use of contraceptives or during pregnancy. Drug treatment has shown little benefit. If the patient does not want to undergo stapedotomy, the use of hearing aids is well indicated. Implantable systems should be indicated only in rare cases, and the CI should be indicated in cases of profound deafness.

SUMMARY Forest Management with Integrated Livestock (MBGI, for its acronym in Spanish) is a technical proposal based on the management of forestry, livestock, forage and environmental components of a property articulated as a feasible farm plan. It requires a monitoring process of these different components within an adaptive management framework, and verification that planning meets the objectives of all dimensions of sustainability. It is necessary for the baseline characterization of the property, the determination of "Ecological Sites" (landscape units with similar types of soil, topography, hydrology, climate and vegetation, which are expected to respond in a similar way to disturbances and management) and their corresponding “Reference States” (condition of greater ecological integrity). The objective of this work is to generate a proposal for "expeditious sampling" of environmental/productive aspects of properties under MBGI. It aims at a holistic visualization of the key dynamics of Northern Patagonian forests and surrounding areas, allowing for their description, categorization and planning in less time compared to the currently available methodology.

RESUMEN El Manejo del Bosque con Ganadería Integrada (MBGI) es una propuesta técnica que se basa en el manejo de los componentes forestal, ganadero, forrajero y ambiental, articulada como un plan predial factible de implementar. Requiere un proceso de monitoreo de los distintos componentes en un marco de manejo adaptativo, verificando que la planificación cumpla con los objetivos en todas las dimensiones de la sustentabilidad. Es necesario para la caracterización de la línea de base del predio, la determinación de “Sitios Ecológicos” (unidades de paisaje con similares tipos de suelo, topografía, hidrología, clima y vegetación, que se espera que respondan de manera similar a disturbios y manejo) y sus correspondientes “Estados de Referencia” (condición de mayor integridad ecológica). El objetivo del presente trabajo es generar una propuesta de “muestreo expeditivo” de los aspectos ambientales/productivos para predios bajo MBGI. Apunta a una visualización holística de procesos clave de dinámicas de los bosques norpatagónicos y zonas aledañas, permitiendo en menor tiempo (en comparación con la metodología actualmente disponible) su descripción, categorización y planificación.

ABSTRACT: Glanders is a disease caused by the bacterium Burkholderia mallei that primarily affects horses, mules and donkeys. The disease can cause lesions in the skin, lungs and several other organs. However, it often manifests as an asymptomatic disease. In Brazil, serological tests of high sensitivity and specificity are used to assist in the detection of antibodies against B. mallei and to contribute to the control of the disease. However, due to the mandatory euthanasia of seroreactive animals, equids with positive serology for B. mallei and asymptomatic generated great conflicts between breeders, veterinarians and diagnostic laboratories. This study clarifies the limitations of complementary diagnostic tests for detecting B. mallei. It describes the clinical, morphological and laboratory findings in 24 equines from different municipalities in the Mato Grosso State, Brazil, which reacted to the complement fixation test and were positive in the western blotting test for glanders. Data and tissue samples were collected from 24 horses for histological, microbiological and molecular analysis. In 23 horses, no clinical signs, morphological alterations, microbiological isolation, or molecular detection would characterize B. mallei infection. On the other hand, samples from an asymptomatic horse without lesional alterations showed sequence amplification compatible with B. mallei in the PCR. Considering that the infection by B. mallei is subject to the application of animal sanitary defense measures and that, by international requirement and national legislation, the serological results are tools that should support the sanitation procedures for the error of the bacteria in the Mato Grosso State, Brazil.

RESUMO: Mormo é uma enfermidade causada pela bactéria Burkholderia mallei que acomete primariamente cavalos, mulas e burros. A doença pode causar lesões na pele, pulmões e em diversos outros órgãos, entretanto frequentemente manifesta-se como uma enfermidade assintomática. No Brasil são utilizados testes sorológicos de elevada sensibilidade e especificidade para auxiliar na detecção de anticorpos contra B. mallei e contribuir para controle da doença. Porém, devido à obrigatoriedade da eutanásia de animais sororeagentes, os equídeos com sorologia positiva para B. mallei e assintomáticos geraram grandes embates entre criadores, médicos-veterinários e laboratórios de diagnóstico. Este trabalho esclarece as limitações dos testes diagnósticos complementares para detecção de B. mallei e descreve os achados clínicos, morfológicos e de exames laboratoriais em 24 equídeos, procedentes de diferentes municípios do estado de Mato Grosso, Brasil, que reagiram ao teste de fixação de complemento e foram positivos no teste de “western blotting” para mormo. Foram colhidos dados e amostras de tecidos de 24 equídeos para análise histológica, microbiológica e molecular. Em 23 equídeos não existiam sinais clínicos, alterações morfológicas, isolamento microbiológico ou detecção molecular que caracterizassem infecção por B. mallei. Por outro lado, amostras de um cavalo assintomático e sem alterações lesionais apresentaram amplificação de sequência compatível com B. mallei na PCR. Considerando que a infecção por B. mallei é passível da aplicação de medidas de defesa sanitária animal e que por exigência internacional e da legislação nacional, os resultados sorológicos são ferramentas que devem amparar os procedimentos de saneamento para erradicação da bactéria no estado de Mato Grosso, Brasil.

ABSTRACT The obstacles in Phakopsora pachyrhizi management result especially from susceptible soybean genotypes and resistant fungal strains. The objective of the current study was to evaluate the applicability of the emission of extremely low and specific frequencies by Effatha technology in the soybean Asian rust control, nutrition, and its impact on yield. The in-vivo test followed the detached leaves method, with six treatments: frequencies 1 and 2 individually and in association; the conventional chemical treatment (fungicide azoxystrobin + benzovindiflupyr); and witnesses in presence and absence of the fungus. Frequency 1 relates to inhibition of the enzyme succinate dehydrogenase and 2 to ubiquinone oxidase. In the field, frequencies 1 and 2 associated (with the same fungicidal action of the in-vivo study); nutritional frequency; application of azoxystrobin + benzovindiflupyr + mancozeb, and control without application were evaluated. In vivo, the fungicide provided 85% control of the disease symptoms, against 65% of frequencies 1 and 2 in association, which showed a higher efficiency compared to the isolated frequencies. In the field, the rate of increase of symptoms were reduced by all treatments compared to the control. At the end of the soybean cycle, the conventional fungicide resulted in 33% severity against 56% of frequencies 1 and 2 associated, and 69.2% of the control. The emission of the frequency for increased nutrient efficiency stood out positively on yield in relation to all the other ones. The conventional application provided the highest weight of 1,000 grains, possibly a direct reflection of the better control of the disease.

Articles written in English or Portuguese on vestibular schwannoma were eligible for inclusion. The American College of Physicians’ guideline grading system and the American Thyroid Association’s guideline criteria were used for critical appraisal of evidence and recommendations for therapeutic interventions. Results: The topics were divided into 2 parts: (1) Diagnosis – audiologic, electrophysiologic tests, and imaging; (2) Treatment – wait and scan protocols, surgery, radiosurgery/radiotherapy, and systemic therapy. Conclusions: Decision making in VS treatment has become more challenging. MRI can diagnose increasingly smaller tumors, which has disastrous consequences for the patients and their families. It is important to develop an individualized approach for each case, which highly depends on the experience of each surgical team.

ABSTRACT Due the tolerance in soil degraded, Eucalyptus is widely used in reforestation area. This study aims to evaluate the fungi that use Eucalyptus grandis W. Hill ex Maiden as substrate in reforestation area in southern Brazil. Fungi were identified and macronutrient and micronutrient contents were evaluated in order to understand the relationship between the fungi and the substrate. There were 200 specimens found, categorized into 25 species belonging to 10 families of Agaricales (Basidiomycota, Fungi). Substrates used by fungi were branches, roots, stems, humus, and soil. Macronutrients mean level found in fungi followed the order Ca>K>P>Mg, and micronutrients S>Fe>Mn>Cu/B>Zn. C:N ratio mean was 13:1, associated with substrate degradation potential, since the enzymatic production of fungi is affected by disposition of these nutrients. The data obtained in this study allowed a better understanding of fungi associated with the exotic arboreal substrate, and their nutritional significance in reforestation area.

RESUMO Devido à tolerância em solos degradados, Eucalyptus é amplamente utilizado em áreas de reflorestamento. Este estudo teve como objetivo avaliar os fungos que utilizam Eucalyptus grandis W. Hill ex Maiden como substrato em uma área de reflorestamento no sul do Brasil. Para compreender a relação dos fungos com o substrato, os mesmos foram identificados e os teores de macronutrientes e micronutrientes foram avaliados. Foram encontrados 200 espécimes divididos entre 25 espécies pertencentes a 10 famílias de Agaricales (Basidiomycota, Fungi). Os substratos utilizados pelos fungos foram galhos, raízes, caules, húmus e solo. Os teores médios de macronutrientes encontrados nos fungos seguiram a ordem Ca>K>P>Mg, e os micronutrientes S>Fe>Mn>Cu/B>Zn. A relação C:N foi de 13:1, associada ao potencial de degradação do substrato, uma vez que a produção enzimática dos fungos é afetada pela disposição desses nutrientes. Os dados obtidos neste estudo auxiliam em uma melhor compreensão dos fungos associados ao substrato arbóreo exótico e sua importância nutricional na área de reflorestamento.

RESUMO Objetivo Avaliar o resultado funcional do tratamento das fraturas transversas desviadas do olécrano (Mayo 2A) após o tratamento com parafuso intramedular com banda de tensão (PIBT) em comparação com a banda de tensão clássica (BTC). Métodos Foram coletados prospectivamente todas as fraturas transversas do olécrano desviadas de 2012 a 2016 e randomizá-las em dois grupos: PIBT e BTC. A amplitude de movimento (ADM) foi medida após 2 e 5 semanas, 3 e 6 meses e 1 e 2 anos. As avaliações funcionais (DASH, Oxford Elbow Score e Mayo Elbow Performance Index) foram realizadas após 3 e 6 meses e 1 e 2 anos. As complicações foram coletadas até 2 anos de acompanhamento. Resultados Foram incluídos 22 pacientes, 11 em cada grupo. A idade média foi de 47,9 anos, e o lado esquerdo foi lesado em 13 (59,0%) pacientes. Todos os pacientes completaram o acompanhamento de 2 anos. Não houve diferença na ADM em nenhum momento entre os dois grupos (p> 0,005). O ganho de flexão e extensão foi máximo aos 3 meses e permaneceu inalterado até 2 anos. Nem a flexão nem a extensão voltaram ao normal, faltando em torno de 10º. A pronação e a supinação voltaram ao normal. Todos os três escores funcionais mostraram uma recuperação quase completa da função do cotovelo após 3 meses de pós-operatório, sem diferença entre os grupos. Nenhum grupo apresentou complicações, nem reoperação ou retirada do implante. Conclusão O PIBT teve resultados semelhantes na ADM e pontuação funcional em comparação com o BTC. Ambos tiveram baixas taxas de complicações e não há necessidade de remover implantes. Nível de evidência I; Estudo clínico randomizado.

ABSTRACT Objective evaluate the functional treatment outcome of deviated transverse olecranon fractures (Mayo 2A) after treatment with tension-banded intramedullary screw (PIBT) compared to classical tension band (BTC). Methods Prospectively collect all deviated transverse olecranon fractures from 2012 to 2016 and randomize them into PIBT and BTC groups. Range of motion (ROM) was measured after 2 and 5 weeks, 3 and 6 months, and 1 and 2 years. Functional assessments (DASH, Oxford Elbow Score, and Mayo Elbow Performance Index) were performed after 3 and 6 months and 1 and 2 years. Complications were collected up to 2 years of follow-up. Results 22 patients were included, 11 in each group. The mean age was 47.9 years, and the left side was injured in 13 (59.0%) patients. All patients completed the 2-year follow-up. There was no ROM difference at any time between the two groups (p> 0.005). Flexion and extension gain was maximum at three months and remained unchanged until two years. Neither flexion nor extension returned to normal, missing around 10°. Pronation and supination returned to normal. All three functional scores showed almost complete recovery of elbow function after three months postoperatively, with no difference between the groups. No group had complications, no reoperation, and no implant removal. Conclusion PIBT had similar results in ROM and functional score compared to BTC. Both had low complication rates and no need for implant removal. Level of evidence I; Randomized Trial.

SUMMARY OBJECTIVE: It is not uncommon for medical residents to deal with critically ill patients who frequently show several ethical and human dilemmas, highlighting the need for a consultation with ethical specialists. The objective of this article is to present a description of a Brazilian Ethics Consultation group designed to attend psychiatry residents. METHODS: This article reports a case of a critically ill patient with Borderline Personality Disorder with multiple intervention failures and several ethical conflicts who was seen by a resident and supported by an ethics consultation group. RESULTS: When medical residents and medical staff face severe and unusual ethical dilemmas, they might feel unprepared and have ones’ mental health impaired. Thus, this article reports a successful ethics consultation and discusses its development in other academic institutions. CONCLUSION: Medical educators and staff from academic hospitals should pay attention to the needs of the medical residents. The development and support of ethics consultation groups must be provided to fulfill the need of those residents who face serious ethical and human dilemmas.

Resumo: Neste artigo, discutimos a emergência da mulher na história das práticas punitivas no Brasil, propondo descortinar a invisibilidade do controle e da violência dirigidos às meninas pobres e racializadas no espaço urbano, classificadas como ‘abandonadas’ ou ‘infratoras’, entre os anos 1930-1960, revelando a centralidade dessas práticas para a manutenção e reprodução do sistema de desigualdades e violência baseado nos marcadores de gênero, raça e classe. Por meio dos prontuários do Serviço Social de Menores de São Paulo, identificaram-se formas de criminalização e punição e os dispositivos de controle sobre o corpo, a sexualidade e a autodeterminação que, apesar de reforçarem iniquidades, não impediram que estratégias fossem engendradas pelas sujeitas para resistir às investidas do poder, através de atos de insurgência cotidianos reativos às tentativas reiteradas de sujeição.

Abstract: This article discusses the emergence of women in the history of punitive practices in the country, proposing to unveil the invisibility of control and violence directed at poor girls in the urban space, abandoned and delinquent, in the years 1930-1960, revealing the centrality of these practices for the maintenance and reproduction of the system of inequalities and violence based on gender, race and class markers. Through the institutional records of the Social Service for Minors of São Paulo, forms of criminalization and punishment were identified, as well as the control devices over the body, sexuality and self-determination that, despite reinforcing inequities, did not prevent strategies from being engendered by the subjects to resist the onslaught of power, through daily insurgency acts reactive to repeated attempts at subjection.

Resumen: Este artículo discute el surgimiento de las mujeres en la historia de las prácticas punitivas en Brasil, proponiendo develar la invisibilidad del control y la violencia dirigida a las niñas pobres en el espacio urbano, abandonadas y delincuentes, en las décadas de 1930-1960, revelando la centralidad de estas prácticas para el mantenimiento y reproducción del sistema de desigualdades y violencia soportados por marcadores de género, raza y clase. A través de los registros del Servicio Social del Menor de São Paulo se identificaron formas de criminalización y sanción, así como los dispositivos de control sobre el cuerpo, la sexualidad y la autodeterminación que, a pesar de reforzar las inequidades, no impidieron que los sujetos diseñaran estrategias para resistir el ataque de poder, a través de actos de insurgencia cotidianos reactivos a repetidos intentos de sometimiento.

ABSTRACT The influence of fasting and vaccination against infectious bronchitis (IB) on growth, gastrointestinal development, and morphometric parameters of the small intestine of newborn broiler breeders was investigated. Weight gain (WG), feed conversion ratio (FCR), gastrointestinal visceral growth, residual yolk sac, and morphometric parameters of the small intestine were measured in chicks aged up to 21 d. We showed that WG was lowest in chicks exposed to longer fasting periods (72 h), although compensatory gains with increasing age were also observed. Unvaccinated chicks performed better at seven days; however, at 21 d of age, a compensatory response led to better FCR in vaccinated birds. Allometric growth of the gastrointestinal tract was affected following longer fasting periods, but this effect occurred predominantly in younger birds (three days). Compared with non-fasted, birds fasted for 72 h had lower villus height and crypt depth along the small intestine at three days and lower villus height in the duodenum and jejunum at seven days. However, at 14 and 21 days of age, the previous changes in intestinal morphology imposed by fasting had been offset. The IB vaccine, applied in the first day of life, does not affect the allometric growth of the gastrointestinal organs, but causes momentary loss in the intestinal morphometry, which contributes to the reduction of WG at seven days. Our results suggest that long fasting periods after hatching (48 and 72 h) should be avoided in broiler breeders due to the resulting delay in gastrointestinal organ development and growth impairment up to 21 d of life.

RESUMO: Casos de anemia hemolítica em bovinos de corte em sistema de criação extensiva em Mato Grosso e Rondônia são acompanhados desde 2008. Os animais acometidos apresentam fraqueza, mucosas pálidas ou ictéricas, urina enegrecida, depressão e anorexia. Esse quadro muitas vezes evolui para morte, principalmente quando os animais são movimentados. Durante esse período aproximadamente 429 bovinos morreram em diferentes surtos eem alguns casos a mortalidade chegou a 50%. Foi realizada investigação para as principais causas infecciosas como leptospirose, babesiose, tripanossomíase, anaplasmose e hemoglobinúria bacilar, bem como para as possíveis causas toxicas relatadas como etiologia de anemia hemolítica em bovinos por meio de exames hematológicos, bioquímica sanguínea, moleculares e histopatológicos. Não se observou nas propriedades acometidas, nenhuma das plantas já descritas no Brasil como causadora de anemia hemolítica em animais de interesse pecuário no momento das investigações dos surtos. Apesar da etiologia desses casos ainda não ser conhecida, as evidências epidemiológicas, clínicas e patológicas da doença sugerem que a mesma seja causada por uma planta tóxica de ação hemolítica, presente nas propriedades acometidas, mas que ainda não foi confirmada experimentalmente.

ABSTRACT Cases of hemolytic anemia in beef cattle in na extensive breeding system in Mato Grosso and Rondônia have been monitored since 2008. The affected animals presented weakness, pale or icteric mucosas, blackened urine, prostration and anorexia. The outcome of these cases often is death, principally if the animals are forced to walk. Approximately 429 cattle died in these outbreaks andin some of these, mortality rates ranged from 0.26 to 50%. Research was carried out on the main infectious causes such as leptospirosis, babesiosis, trypanosomiasis, anaplasmosis and bacillary hemoglobinuria, as well as the possible toxic causes reported as etiology of hemolytic anemia in cattle through hematology, blood biochemistry, molecular and histopathological examinations. Any plant ready described in Brazil as cause of hemolytic anemia in animals from livestock interest was observed in thosefarms. Although the etiology of these cases is not yet known, clinical and pathological evidences of the disease suggested that it is caused by a toxic plant of hemolytic action, present in the affected properties, but not yet confirmed experimentally.

RESUMO: Os neoplasmas cutâneos em cães apresentam elevada e relevante prevalência em todo o Brasil. Sendo assim, objetivou-se determinar a frequência e algumas características epidemiológicas dos neoplasmas cutâneos em cães diagnosticados no Laboratório de Patologia da Universidade Federal de Mato Grosso (LPV-UFMT), Cuiabá, entre os anos de 2007 a 2014. Foram revisados os protocolos de necropsias e biopsias do LPV-UFMT, de cães com neoplasmas cutâneos. Dados referentes à idade, sexo, raça dos cães afetados, localização anatômica, tamanho, diagnóstico histomorfológico e comportamento biológico foram coletados e analisados através de uma análise estatística descritiva. Adicionalmente o teste χ2, foi utilizado para associações entre comportamento biológico e tamanho da massa. Dos 3566 exames realizados, 656 (18,4%) foram diagnosticados como tumores cutâneos sendo cães adultos e idosos das raças Pit Bull, Boxer e Poodle os mais acometidos. Dentre os 11 padrões morfológicos mais diagnosticados o mastocitoma, carcinoma de células escamosas (CCE) e os tumores de origem vascular foram os mais frequentemente relatados. Em relação ao sítio anatômico, a cabeça foi a mais acometida. A maioria dos tumores benignos apresentaram menos de 1cm de diâmetro e os malignos de 3 a 5cm. O aumento de neoplasmas em adultos e idosos pode estar relacionado ao somatório de danos causados por agentes carcinogênicos e o comprometimento imunológico.

ABSTRACT: Canine cutaneous neoplasms present high and relevant prevalence throughout Brazil. Here we determine the frequency and epidemiological characteristics of canine cutaneous neoplasms in dogs diagnosed at the Laboratory of Veterinary Pathology of the Federal University of Mato Grosso (LPV-UFMT), Cuiabá, from 2007 to 2014. The necropsy and biopsy protocols from canine cutaneous neoplasms diagnosed at the LPV-UFMT were reviewed. Data regarding age, sex, breed of affected dogs, anatomical location, size, histomorphological diagnosis and biological behavior of the tumors were evaluated through a descriptive statistical analysis. In addition, the χ2 test was used for associations between biological behavior and mass size. Of the 3566 examinations performed, 656 (18.4%) were diagnosed as cutaneous tumors being adult and old dogs of Pit Bull, Boxer and Poodle races most affected. Among the diagnoses, 11 morphological patterns were the most prevalent, being mastocytoma, squamous cell carcinoma (SCC) and tumors of vascular origin most frequently reported. In relation to the anatomical site, the head was the most affected. Most of the benign tumors were less than 1cm in diameter and the malignant were 3 to 5cm. The increase of neoplasms in adults and old age dogs may be associated to the sum of damage caused by carcinogenic agents and the immunological impairment.

The known kavalactones (E)-4-methoxy-6-styryl-2H-pyran-2-one, 4-methoxy-6-(3-phenyloxiran-2-yl)-2H-pyran-2-one, 6-(1,2-dihydroxy-2-phenylethyl)-4-methoxy-2H-pyran-2-one, the three benzoic acid derivatives methyl-4-methoxy-3-(3'-methyl-2'-butenyl)benzoate and methyl 2,2-dimethyl-4-oxochroman-6-carboxylate, and a new methyl 4-methoxy-3-(3-methylbut-2-enoyl)benzoate were isolated from the ethanolic extract of Piper fuligineum. The structures of these compounds were determined by using a combination of spectroscopic methods, including 1D- and 2D-nuclear magnetic resonance spectroscopy and high-resolution mass spectrometry. This is the first report of the chemical study of P. fuligineum, and the methyl 4-methoxy-3-(3-methylbut-2-enoyl)benzoate is described as a new natural product.

ABSTRACT The hemoflagellate protozoan, Trypanosoma cruzi, mainly transmitted by triatomine insects through blood transfusion or from mother-to-child, causes Chagas' disease. This is a serious parasitic disease that occurs in Latin America, with considerable social and economic impact. Nifurtimox and benznidazole, drugs indicated for treating infected persons, are effective in the acute phase, but poorly effective during the chronic phase. Therefore, it is extremely urgent to find innovative chemotherapeutic agents and/or effective vaccines. Since piplartine has several biological activities, including trypanocidal activity, the present study aimed to evaluate it on two T. cruzi strains proteome. Considerable changes in the expression of some important enzymes involved in parasite protection against oxidative stress, such as tryparedoxin peroxidase (TXNPx) and methionine sulfoxide reductase (MSR) was observed in both strains. These findings suggest that blocking the expression of the two enzymes could be potential targets for therapeutic studies.
