ABSTRACT BACKGROUND AND OBJECTIVES: Fibromyalgia is a clinical condition that presents generalized and chronic musculoskeletal pain. With the COVID-19 pandemic, factors such as social distancing and a reduction in the search for and supply of care for chronic diseases may have contributed to worsening the quality of life (QoL) of people with fibromyalgia. The aim of this study was to investigate the relationship between access to health care and the QoL of women with fibromyalgia during the COVID-19 pandemic. METHODS: This was a descriptive cross-sectional observational study of 30 women diagnosed with fibromyalgia, with a mean age of 56.3±10.4 years, who took part in group therapy promoted by the Fibro Dance (Dança Fibro) extension project at the Trairi Health Science Faculty of the Federal University of Rio Grande do Norte (Faculdade de Ciência da Saúde do Trairi da Universidade Federal do Rio Grande do Norte - FACISA/ UFRN). QoL was evaluated using the Short Form 36 Health Survey (SF-36) and access to health care was investigated using a questionnaire developed by the researchers, which also included sociodemographic questions and questions about health conditions. Descriptive and inferential statistics were used to analyze the data. The Shapiro-Wilk normality test was used to check the distribution of the data, and the t-test was used to compare the means of the variables with normally distributed data, while the Mann-Whitney non-parametric test was used for the variables without normal distribution. A 5% significance level was adopted for the inferential analysis. RESULTS: 66.7% of the participants received care in primary care, while 36.7% and 26.7% received care in specialized care and hospitals, respectively. As for QoL, the domains that showed the greatest impairment, with the lowest mean score, were limitation of physical aspects (9.2; 95%CI: 1.6; 16.7), limitation of emotional aspects (11.1; 95%CI: 0.6; 21.6), pain (29.9; 95%CI: 23.9;35.8), general state of health (35.8; 95%CI: 32.2; 39.4) and vitality (38.7; 95%CI: 31.9; 45.4). In the inferential analysis, there was no statistically significant difference in QoL between the group of women who received primary care or hospital care compared to the group who did not receive care at these levels (p > 0.05). On the other hand, there were statistically significant differences when the comparison was analyzed in specialized care. In the functional capacity domain (p = 0.005), the average score for the group that received specialized care was higher than the score for the group that did not, 38.2 and 24.2, respectively. For the social aspects domain (p = 0.045), the average score was 54.5 for those who received care and 37.5 for the group who did not receive care at the level of care in question. CONCLUSION: This study concluded that access to specialized care during the COVID-19 pandemic contributed to women with fibromyalgia showing better results in QoL domains such as functional capacity and social aspects.

RESUMO JUSTIFICATIVA E OBJETIVOS: A fibromialgia é uma condição clínica que apresenta dor musculoesquelética generalizada e crônica. Com a pandemia da COVID-19, fatores como distanciamento social e redução na busca e oferta por cuidados para as doenças crônicas podem ter contribuído para piorar a qualidade de vida (QV) das pessoas com fibromialgia. O objetivo deste estudo foi investigar a relação entre o acesso aos níveis de atenção à saúde e a QV de mulheres com fibromialgia durante a pandemia da COVID-19. MÉTODOS: Trata-se de um estudo observacional transversal descritivo com 30 mulheres diagnosticadas com fibromialgia, apresentando média de idade de 56,3±10,4 anos, que participaram da terapia em grupo promovida pelo projeto de extensão Dança Fibro, da Faculdade de Ciência da Saúde do Trairi da Universidade Federal do Rio Grande do Norte (FACISA/UFRN). A QV foi avaliada usando o Short Form 36 Health Survey (SF-36) e o acesso aos níveis de atenção à saúde foi investigado por meio de um questionário desenvolvido pelos pesquisadores, que também contemplou questões sociodemográficas e sobre condições de saúde. Para a análise de dados, foram usadas a estatística descritiva e a inferencial, sendo aplicado o teste de normalidade de Shapiro-Wilk para verificação da distribuição dos dados, e o teste t para comparar as médias das variáveis que apresentaram os dados normalmente distribuídos, enquanto que para as variáveis que não apresentaram distribuição normal, foi utilizado o teste não paramétrico de Mann-Whitney. Na análise inferencial, foi adotado um nível de significância de 5%. RESULTADOS: Foi observado que 66,7% das participantes receberam cuidados na atenção básica, enquanto que 36,7% e 26,7% foram assistidas na atenção especializada e hospitalar, respectivamente. No que diz respeito à QV, os domínios que apresentaram maior comprometimento, com menor escore médio, foram limitação dos aspectos físicos (9,2; IC95%: 1,6; 16,7), limitação dos aspectos emocionais (11,1; IC95%: 0,6; 21,6), dor (29,9; IC95%: 23,9;35,8), estado geral de saúde (35,8; IC95%: 32,2; 39,4) e vitalidade (38,7; IC95%: 31,9; 45,4). Na análise inferencial, não foi observada diferença estatisticamente significativa na QV entre o grupo de mulheres que foram assistidas na atenção básica ou na atenção hospitalar em comparação com o grupo que não recebeu assistência nestes níveis de atenção (p > 0,05). Em contrapartida, constatou-se diferenças estatisticamente significativas quando a comparação foi analisada no âmbito da atenção especializada. No domínio capacidade funcional (p = 0,005), o escore médio para o grupo que recebeu cuidados na atenção especializada foi superior ao escore do grupo que não recebeu, 38,2 e 24,2, respectivamente. Para o domínio aspectos sociais (p = 0,045), o escore médio foi de 54,5 para aquelas que foram assistidas e de 37,5 para o grupo que não recebeu assistência no nível de atenção em questão. CONCLUSÃO: Este estudo concluiu que o acesso à atenção especializada durante a pandemia da COVID-19 contribuiu para que as mulheres com fibromialgia apresentassem melhores resultados nos domínios da QV, tais como capacidade funcional e aspectos sociais.

Abstract Introduction: The concept of disability is related to the socially-imposed limitations on the disabled because their bodies do not correspond to the model accepted as normal. Access to health care for this population is generally precarious, partly due to the lack of preparation for the care of disabled patients during medical school. This suggests that the school curricula need to be revised. Objective: To analyse the perception of the senior medical students at the Federal University of Ceará (UFC) about the current curriculum, specifically, in relation to the care of people with disabilities. Method: A form previously prepared on the Google Forms platform was made available to collect information from senior medical students at the School of Medicine (FAMED). Afterwards, semi-structured interviews were carried out via Google Meet using questions that aimed to understand the relations between future doctors and people with disabilities, as well as the confidence of the students in their preparation for caring for such patients. For analysis of the empirical material acquired, we used Rueda’s speech analysis. Result: Thirteen interns were interviewed, who reported limitations in learning how to establish a doctor-patient relationship with patients with disabilities during the first two years of medical school, having effects on the future clinical rotations. Politeness, knowledge of the patient’s socioeconomic and cultural conditions, and building an executable therapeutic plan were considered qualities to be developed by the student. The main problems reported was difficulty in performing the physical examination and communication with disabled patients. However, help from the patients’ carers and from the professional team was considered positive by the students. Perceptions regarding the preparation to care for the disabled were contrasting: some reported confidence, due to training and empirical knowledge, while others felt insecure or incapable. It is also important to consider adapting the curriculum on this subject, with interventions in compulsory and elective subjects. Conclusion: Considering the responses and analyses, it was evident that the interviewees experience difficulties in caring for the disabled, suggesting changes to the FAMED-UFC curriculum.

Resumo Introdução: O conceito de deficiência relaciona-se às limitações socialmente impostas aos deficientes por seus corpos não corresponderem ao modelo aceito como normal. Em geral, o acesso à saúde dessa população é precário, em parte, pela falta de preparação voltada ao cuidado dos pacientes deficientes durante a formação médica, demonstrando que os currículos das faculdades necessitam de revisão. Objetivo: Este estudo teve como objetivo analisar a percepção do aluno interno do curso de Medicina da Universidade Federal do Ceará (UFC) acerca do modelo curricular atual no contexto da formação médica, especificamente voltado ao atendimento destinado às pessoas com deficiência. Método: Foi disponibilizado um formulário elaborado no Google Formulários, para coletar informações de internos da Faculdade de Medicina (Famed). Depois, realizaram-se entrevistas semiestruturadas via Google Meet mediante questões que objetivaram entender a relação entre futuros médicos e pessoas com deficiências, além da confiança do preparo para o atendimento a tais pacientes. Para exame do material empírico adquirido, utilizou-se a análise de discurso de Rueda. Resultado: Foram entrevistados 13 internos que relataram limitação no aprendizado do estabelecimento da relação médico-paciente em relação a pessoas com deficiência, durante o ciclo básico (do primeiro ao quarto semestre), evoluindo, principalmente, para o internato. Consideraram-se a educação, o entendimento das condições socioeconômicas e culturais do paciente, e a construção de um plano terapêutico executável as qualidades a serem desenvolvidas pelo interno. Quanto aos principais problemas relatados, destacou-se a dificuldade na realização do exame físico e na comunicação. Por sua vez, a ajuda de acompanhantes e o auxílio da equipe profissional foram apontados como aspectos positivos. Percepções referentes ao preparo para atender deficientes foram contrastantes: alguns relataram segurança por capacitações e conhecimentos empíricos, enquanto outros se sentiram inseguros ou incapazes. Percebe-se, também, importante consideração para haver adaptação curricular acerca desse tema, com intervenções nas disciplinas obrigatórias e optativas. Conclusão: Evidenciou-se que os entrevistados sentem dificuldades no atendimento destinado a deficientes, o que sugere alterações no currículo da Famed-UFC.

ABSTRACT Objective: To evaluate the prevalence and factors associated with depression in family members of people hospitalized in intensive care units. Methods: A cross-sectional study was conducted with 980 family members of patients admitted to the intensive care units of a large public hospital in the interior of Bahia. Depression was measured using the Patient Health Questionnaire-8. The multivariate model consisted of the following variables: sex and age of the patient, sex and age of the family member, education level, religion, living with the family member, previous mental illness and anxiety. Results: Depression had a prevalence of 43.5%. In the multivariate analysis, the model with the best representativeness indicated that factors associated with a higher prevalence of depression were being female (39%), age younger than 40 years (26%) and previous mental illness (38%). A higher education level was associated with a 19% lower prevalence of depression in family members. Conclusion: The increase in the prevalence of depression was associated with female sex, age younger than 40 years and previous psychological problems. Such elements should be valued in actions aimed at family members of people hospitalized in intensive care.

RESUMO Objetivo: Avaliar prevalência e fatores associados à depressão em familiares de pessoas hospitalizadas em unidades de terapia intensiva. Métodos: Estudo transversal, desenvolvido com 980 familiares de pacientes internados nas unidades de terapia intensiva de um hospital público de grande porte no interior da Bahia. A depressão foi mensurada pelo Patient Health Questionnaire-8. O modelo multivariado foi composto pelas seguintes variáveis: sexo e idade do paciente, sexo e idade do familiar, escolaridade, religião, residir com o familiar, problema psíquico prévio e ansiedade. Resultados: A depressão apresentou prevalência de 43,5%. Na análise multivariada, o modelo com melhor representatividade indicou que fatores associados à maior prevalência de depressão foram ser do sexo feminino (39%), idade menor que 40 anos (26%) e problema psíquico prévio (38%). A maior escolaridade se associou a uma prevalência de depressão 19% menor em familiares. Conclusão: O aumento na prevalência de depressão esteve associado a sexo feminino, idade menor de 40 anos e problemas psíquicos prévios. Tais elementos devem ser valorizados nas ações direcionadas aos familiares de pessoas internadas na terapia intensiva.

Abstract The traumatic event produces intolerable excitations to the psychic apparatus that searches to relief them through the production of symptoms. When established, patients with post-traumatic stress disorders (PTSD) and complex post-traumatic stress disorder (CPTSD) may experience flashbacks, somatizations, negative emotions about themselves, and difficulty in social contact. This work seeks to understand how the psychodynamic functioning of women victims of interpersonal and urban violence, diagnosed with these disorders, is organized, identifying traumatic experiences, ways of interpersonal relationships, conflicts and psychic structures and use of defense mechanisms, and for peculiarities that may differentiate these disorders. The qualitative transversal method was used through the content analysis of clinical interviews based on the Operationalized Psychodynamic Diagnosis (OPD-2). The sample of this study consisted of five women with PTSD and five with CPTSD. The following categories were created: reasons for seeking care, symptoms and desire for treatment, traumatic developmental events, and characteristics of the psychic functioning. Early trauma generates psychic organizations with greater disintegration. A new traumatic event destabilizes the psychic organization and intensifies symptoms. Relationships were marked by dependence and isolation. Participants with CPTSD presented tendency to disintegration related to the object relation regulation and the psychic conflict was of Individuation versus Dependence, with more primitive flaws in object representations, existential need for the other and direct discharge of impulses. Participants with PTSD had moderate to low level of object relation integration and the conflict was need to be care of versus self-sufficiency, with self-representations being fragile and with reduced capacity to manage impulses. Thus, it could be observed that OPD-2 is capable of assessing in a broad and deep way patients with traumatic disorders, in addition to identifying essential peculiarities to guide health professionals towards treatment in the search for better quality of life for patients.

Resumo Na região de Guaratiba, ao longo de quase cinco séculos de ocupação e consequentes processos de transformação da paisagem, cenários ecológicos tornaram-se áreas produtivas, residenciais e de transição urbana. Essa historicidade constitui elemento-chave para a compreensão das vulnerabilidades locais e riscos associados. Nesse sentido, no âmbito da gestão da paisagem, sob uma abordagem integradora de vulnerabilidade e riscos, temos como objetivo mapear e analisar as vulnerabilidades e riscos socioecológicos na área de estudo. O trabalho se estruturou na investigação da história de uso, ocupação e transformação da paisagem, na identificação de áreas vulneráveis, bem como na classificação dos riscos socioecológicos. Assim, estabeleceram-se três etapas metodológicas para tal compreensão: (1) análise dos processos históricos de uso e ocupação das terras em Guaratiba; (2) delimitação das Unidades Socioecológicas da Paisagem (USEP), com o intuito de compreender os níveis atuais de heterogeneidade da área estudada; e por fim, (3) a modelagem e categorização dos riscos socioecológicos identificados. Essa análise, baseada na técnica de geoprocessamento conhecida como lógica fuzzy, levou ao entendimento de uma classificação socioecológica dos riscos, reconhecendo que tais fenômenos só podem ser compreendidos a partir da presença humana perante a pertinência de eventos que possam ferir os grupos expostos. Como resultado desta investigação, quatro das seis unidades de paisagem delimitadas apresentaram locais com tendência a riscos: USEP 2, USEP 4, USEP 5 e USEP 6. Tais áreas sugerem riscos socioecológicos do tipo físico relacionados a dinâmicas geomorfológicas (deslizamentos e de enchentes) e de saúde ambiental.

Abstract Through almost five centuries of occupation and the resulting processes of landscape transformation, ecological scenarios in the neighborhood of Guaratiba have become areas of production, residence, and urban transition. This historicity is a key element to understand local vulnerabilities and associated risks. Thus, within the scope of landscape management and under an integrative approach of vulnerabilities and risks, our objective in this study is to map and analyze socio-ecological vulnerabilities and risks in the study area. Research was structured on i) investigating the history of landscape use, occupation and transformation, ii) identifying vulnerable areas, and iii) classifying socio-ecological risks. Three methodological steps were used: (1) analysis of historical processes of land use and occupation in Guaratiba; (2) delimitation of Socio-ecological Landscape Units (SELU) in order to understand current levels of heterogeneity in the study area; and (3) modeling and characterization of identified socio-ecological risks. Based on the geoprocessing technique known as fuzzy logic, analysis led to a socio-ecological classification of risks, recognizing that such phenomena can only be understood by examining human presence in face of the relevance of events which can harm exposed groups. The investigation showed that four out of six delimitated landscape units have points on which there is a tendency of risks: SELU 2, SELU 4, SELU 5, and SELU 6. These areas suggest socio-ecological risks of the physical type, related to geomorphologic (landslides and floods) and environmental health dynamics.

RESUMO A exodontia dos terceiros molares inferiores é um procedimento comum na rotina dos Cirurgiões Bucomaxilofaciais. Entretanto, mesmo este procedimento sendo frequente, por vezes, o seu curso se mostra de forma inesperada. Neste cenário, algumas classificações desenvolvidas auxiliam na predição do nível de dificuldade para tais extrações. Objetivo Este trabalho objetivou, por meio de extrações de terceiros molares inferiores, verificar a associação das classificações I e II de Pell e Gregory e a dificuldade cirúrgica. Métodos Pacientes voluntários do Curso de Odontologia da Universidade do Estado do Rio Grande do Norte, entre a faixa etária de 16 a 45 anos, que se encaixaram nos critérios de inclusão, tiveram seus terceiros molares classificados de acordo com Pell e Gregory e extraídos. Resultados Um total de 42 elementos foram extraídos. Não existiram diferenças estatisticamente significantes para a correlação da classificação de Pell e Gregory com a dificuldade cirúrgica levando-se em conta as variáveis tempo de cirurgia (p = 0,419), necessidades de osteotomia (p= 0,428) e quantidade de anestubes (0,939), quanto a necessidade de odontossecção, apenas quando comparados os dentes classificados como IA e IIA, a diferença a favor dos elementos IA foi estatisticamente significativa (p = 0,008), inexistindo acidentes e complicações. Conclusão A classificação de Pell e Gregory não se mostrou como um bom preditor da dificuldade cirúrgica, sendo recomendado que novos estudos sejam realizados sobre essa temática.

ABSTRACT Lower third molar extraction is a common procedure in Oral and Maxillofacial Surgeons routine. However, even though this procedure is frequent, sometimes its course is unexpected. In this scenario, some developed classifications help to predict the difficulty level for such extractions. Objective This study aimed to verify the association of Pell and Gregory classifications I and II and surgical difficulty through lower third molar extractions. Methods Voluntary patients from the Dentistry Course of the State University of Rio Grande do Norte, between 16 and 45 years of age, who met the inclusion criteria, had their third molars classified according to Pell and Gregory. Results A total of 42 elements were extracted. There were no statistically significant differences for the correlation of Pell and Gregory classification with the surgical difficulty considering the variables such as surgery time (p=0.419), osteotomy needs (p=0.428) and number of anesthetic tubes (0.939). As for the need for odontosection, only when comparing the teeth classified as AI and IIA, the difference in favor of AI elements was statistically significant (p=0.008). There were no accidents or complications. Conclusions Pell and Gregory classification was not a good predictor of surgical difficulty, and further studies on this subject are recommended.

RESUMO Introdução: A pandemia causada pelo novo coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2, COVID-19) apresenta novos e importantes desafios à gestão de saúde no Brasil. Além da difícil missão de prestar atendimento aos milhares de pacientes infectados pelo COVID-19, os sistemas de saúde têm que manter a assistência às emergências médicas comuns em períodos sem pandemia, tais como o acidente vascular cerebral (AVC), que continuam ocorrendo e requerem tratamento com presteza e eficiência. A alocação de recursos materiais e humanos para o enfrentamento à pandemia não pode comprometer o atendimento ao AVC agudo, uma emergência cujo tratamento é tempo-dependente e se não realizado implica em importante impacto na mortalidade e incapacitação a longo prazo. Objetivo: Este trabalho resume as recomendações do Departamento Científico de Doenças Cerebrovasculares da Academia Brasileira de Neurologia, da Sociedade Brasileira de Doenças Cerebrovasculares e da Sociedade Brasileira de Neurorradiologia para o tratamento do AVC agudo e para a realização de procedimentos de neurointervenção urgentes durante a pandemia de COVID-19, incluindo o uso adequado de ferramentas de triagem e equipamentos de proteção pessoal (para pacientes e profissionais de saúde), além da alocação apropriada de pacientes.

ABSTRACT Introduction: Although the 2019 severe acute respiratory syndrome coronavirus 2 infection (SARS-CoV-2, COVID-19) pandemic poses new challenges to the healthcare system to provide support for thousands of patients, there is special concern about common medical emergencies, such as stroke, that will continue to occur and will require adequate treatment. The allocation of both material and human resources to fight the pandemic cannot overshadow the care for acute stroke, a time-sensitive emergency that with an inefficient treatment will further increase mortality and long-term disability. Objective: This paper summarizes the recommendations from the Scientific Department on Cerebrovascular Diseases of the Brazilian Academy of Neurology, the Brazilian Society of Cerebrovascular Diseases and the Brazilian Society of Neuroradiology for management of acute stroke and urgent neuro-interventional procedures during the COVID-19 pandemic, including proper use of screening tools, personal protective equipment (for patients and health professionals), and patient allocation.

Resumo A anatomia foliar ecológica do gênero Myrcia DC. ainda é pouco estudada. A espécie Myrcia guianensis (Aubl.) DC., popularmente conhecida como mapirunga, ocorre em região de litoral no estado do Ceará e possui importância ecológica e potencial medicinal. Objetivou-se caracterizar a anatomia foliar ecológica dessa espécie nas estações de chuva e seca, bem como, ao sol e à sombra na Restinga cearense. Para isso, foram realizadas coletas no Jardim Botânico de São Gonçalo de folhas totalmente expandidas e fixadas em FAA70, sendo substituído por etanol 70% após 24 horas. Seguiu-se para os procedimentos anatômicos usuais no Laboratório de Ecologia da Universidade Estadual do Ceará, a fim de analisar qualitativamente e quantitativamente as estruturas da lâmina foliar, pecíolo e paradérmico. Como resultados obteve-se a presença de características que contribuem para sua sobrevivência em ambiente de Restinga com períodos de deficit hídrico e alta taxa de luminosidade, tais como: presença de cutícula levemente espessa, folha hipoestomática, aumento da densidade estomática em folhas de sol, presença de fibras abundantes nos feixes vasculares da nervura central e presença de drusas. Portanto, conclui-se que a planta em estudo possui grande capacidade aclimatativa às condições analisadas (intensa luminosidade e períodos de deficit hídrico).

Abstract The ecological foliar anatomy of the genus Myrcia DC. is still little studied. Myrcia guianensis (Aubl.) DC. species popularly known as mapirunga, occurs in a coastal region in the state of Ceará and has ecological importance and medicinal potential. The objective was to characterize the ecological foliar anatomy of this species in rainy and dry seasons, as well as, in the sun and shade in the Ceará’s Restinga. To do so, samples were collected in São Gonçalo Botanical Garden, with leaves totally expanded and fixed in FAA70, being replaced by 70% ethanol after 24 hours. It was followed to the usual anatomical procedures in the Ecology Laboratory of the Ceará State University, in order to qualitatively and quantitatively analyze the structures of the leaf blade, petiole and paradermal sections. The results showed the presence of characteristics that contribute to its survival in a Restinga environment with periods of water deficit and high luminosity, such as: presence of slightly thick cuticle, hypoestomatic leaf, increased stomatal density in leaves of sun, presence of abundant fibers in the vascular bundles of the central vein and presence of drusen. Therefore, it is concluded that the plant under study has a great acclimative capacity to the analyzed conditions (intense luminosity and periods of water deficit).

RESUMO Objetivo O objetivo do estudo foi caracterizar e comparar o uso de elementos típicos da gramática de história e o nível de coerência global na narrativa oral de crianças com Transtorno de Déficit de Atenção e Hiperatividade à narrativa de crianças sem o transtorno e com desenvolvimento típico. Método Participaram 40 crianças com idade entre 5 e 10 anos, de ambos os sexos, que frequentavam o ensino fundamental, sendo 20 com diagnóstico de Transtorno de Déficit de Atenção e Hiperatividade (Grupo TDAH) e 20 com desenvolvimento típico (Grupo TD). Os participantes de cada grupo eram semelhantes quanto ao sexo, idade cronológica, escolaridade e nível socioeconômico. O livro “Frog Where Are You?” foi utilizado para eliciar a narrativa oral de história, que foi analisada quanto à presença dos principais elementos típicos do esquema de história (personagem, tema/tópico, evento/trama e desfecho) e posteriormente classificada dentre quatro diferentes níveis crescentes de organização, correspondendo ao nível de coerência global da história. Resultados O grupo TDAH apresentou menor pontuação nos elementos estruturais “tema/tópico” e “desfecho” e narrativa com grau de coerência inferior quando comparado ao grupo TD. Conclusão As crianças com TDAH deste estudo apresentaram dificuldades no uso de elementos típicos da gramática de história, principalmente relacionados com a manutenção do tema central e desfecho da história. Tais elementos são considerados fundamentais para a construção do sentido da narrativa, o que justifica os níveis inferiores de coerência encontrados na narrativa oral do grupo TDAH.

ABSTRACT Purpose This study aimed to characterize and compare the use of typical story grammar elements and global coherence level in the oral narrative of children with Attention Deficit Hyperactivity Disorder with the narrative of children without the disorder and with typical development. Methods A total of 40 children of both sexes aged 5 to 10 years who attended elementary school participated in the study, 20 of whom were diagnosed with Attention Deficit Hyperactivity Disorder (ADHD Group), and 20 with typical development (TD Group). Participants from each group were similar in sex, chronological age, schooling and socioeconomic status. The wordless picture book Frog, Where Are You? was used to elicit the oral narrative analyzed for the presence of the main typical elements of the story schema (character, theme/topic, event/plot and outcome), and afterwards their narration was classified according to four different levels of organization corresponding to the global story coherence level. Results The ADHD Group presented lower scores on the structural elements “theme/ topic” and “outcome” and a narrative with lower degree of coherence compared to the TD Group. Conclusion The children with ADHD included in this study presented difficulties to use typical story grammar elements, mainly related to the maintenance of the central theme and outcome of the story. These elements are considered fundamental for construction of narrative coherence, which justifies the lower levels of global coherence found in the oral narrative of the ADHD Group.

Resumo O fator induzível por hipóxia 1 alfa (HIF-1α) e o fator de crescimento endotelial vascular (VEGF) são proteínas que estimulam a proliferação e a migração de células endoteliais. Estas proteínas têm sido descritas em muitas condições patológicas e inflamatórias, mas o envolvimento dessas no desenvolvimento de periodontite não foi completamente investigado. Este estudo comparou a expressão imunohistoquímica destas proteínas, envolvidas na angiogênese e hipóxia, na doença periodontal e em tecidos gengivais saudáveis por meio de contagem de células inflamatórias e endoteliais imunomarcadas. As amostras de tecido gengival foram divididas da seguinte forma: 30 amostras com periodontite crônica, 30 com gengivite crônica e 30 de gengiva saudável. Os resultados foram analisados estatisticamente pelos testes de Kruskal-Wallis e Mann-Whitney (p=0.01). As células inflamatórias e endoteliais foram positivas para estas proteínas. A porcentagem média de células positivas para HIF-1α foi de 39,6% nos casos de periodontite, 22,0% nos casos de gengivite e de 0,9% no grupo de gengiva saudável (p = 0,001). A porcentagem média de células imunopositivas para o VEGF foi de 68,7% nos casos de periodontite, 66,1% nos casos de gengivite, e 19,2% em espécimes gengivais saudáveis (p<0,001). Correlação significativa entre o VEGF e HIF-1α foi observada em gengival. A expressão elevada do HIF-1α e VEGF em periodontite, comparada a gengivite e gengiva saudável, sugere ativação da via do HIF-1α, na doença periodontal avançada. A correlação entre o HIF-1α e expressão de VEGF na gengiva saudável, sugere uma função fisiológica para estas proteínas em condições de homeostase. Na doença periodontal, a expressão de HIF-1α e VEGF pode ser regulada por outros fatores, além da hipóxia, tais como endotoxinas bacterianas e citocinas inflamatórias.

Abstract Hypoxia-inducible factor 1 alpha (HIF-1α) and vascular endothelial growth factor (VEGF) are proteins that stimulate the proliferation and migration of endothelial cells. These proteins have been described in many pathologic and inflammatory conditions, but their involvement in the development of periodontitis has not been thoroughly investigated. This study compared the immunohistochemical expression of these proteins, involved in angiogenesis and hypoxia, by imunnostained inflammatory and endothelial cells in periodontal disease and healthy gingival tissues. Gingival tissue samples were divided as follows: 30 samples with chronic periodontitis, 30 with chronic gingivitis, and 30 of healthy gingiva. Results were analyzed statistically by the Kruskal-Wallis, Mann-Whitney and Spearman correlation tests (p=0.01). Inflammatory and endothelial cells were found to express these proteins. Periodontitis showed median percentage of HIF-1α-positive cells of 39.6%, 22.0% in cases of gingivitis and 0.9% in the healthy gingiva group (p=0.001). For VEGF, median percentage of immunopositive cells was 68.7% for periodontitis, 66.1% in cases for gingivitis, and 19.2% for healthy gingival specimens (p<0.001). Significant correlation between VEGF and HIF-1α was also observed in healthy gingiva (p<0.001).The increased expression of HIF-1αα and VEGF in periodontitis, compared to gingivitis and healthy gingiva, suggests possible activation of the HIF-1α pathway in advanced periodontal disease. The correlation between HIF-1α and VEGF expression in healthy gingiva suggests a physiological function for these proteins in conditions of homeostasis. In periodontal disease, HIF-1 and VEGF expression may be regulated by other factors, in addition to hypoxia, such as bacterial endotoxins and inflammatory cytokines.

O texto ora apresentado enfoca a reflexão acerca dos rebatimentos da desigualdade racial nos espaços do sistema educacional, que se configuram fundamentais, tanto para a reprodução, quanto para o enfrentamento da condição desigual. Dessa forma, considera-se o potencial da escola para o processo de construção da igualdade racial e o compromisso ético do Serviço Social na construção de outra ordem societária, buscando-se discutir as contribuições da inserção do assistente social na política pública de educação.

The text presented here focuses on the reflection on the repercussions of racial inequality in the spaces of the educational system that are configured crucial both for reproduction and for coping with the uneven condition. Thus it is considered the potential for the school construction process of racial equality and ethical commitment of social work in building a other social order, seeking to argue the role of the social worker integration in public education policy.

Este artigo analisa como o Centro Regional de Pesquisas Educacionais de São Paulo (CRPE-SP) abordou os temas desempenho e adaptação da criança pobre à escola. O foco incide sobre os anos 1956-1963 e dedica especial atenção às manifestações de Dante Moreira Leite e de Luiz Pereira a respeito do assunto. O argumento central é o de que a transformação da cidade de São Paulo em grande metrópole tornou-se o tema articulador com o qual ambos os autores pesquisaram o desempenho e a adaptação de crianças em idade escolar, especialmente aquelas identificadas como suburbanas. Uma nova cultura urbana desafiava a estrutura da escola e, com base no legado de Antonio Candido e de Florestan Fernandes, tais questões foram investigadas com um novo padrão de pesquisa, que se tornou marca do CRPE-SP naquele momento. Esse novo padrão de pesquisa abriu espaço para que novos recursos analíticos fossem mobilizados para o estudo da inteligência da criança em situação escolar. Foram demonstradas as insuficiências dos parâmetros biológicos para a compreensão do fenômeno da reprovação, que tinha números expressivos. Para além de uma nova compreensão antropológica a respeito da interação entre a cultura escolar e os modos de viver das periferias urbanas, o padrão de pesquisa estabelecido enriqueceu o repertório de análises sociológicas sobre a expansão do número de vagas escolares na cidade de São Paulo. Para aquela sociologia da educação que então se renovava, o caráter fortemente excludente da reprovação escolar foi demonstrado de forma magistral.

This article examines how Centro Regional de Pesquisas Educacionais de São Paulo (CRPE-SP - Sao Paulo Regional Center for Educational Research) addressed the issues of performance and adaptation of poor children to school. It focuses on the years 1956-1963 and devotes special attention to the expressions of Dante Moreira Leite and Luiz Pereira on the subject. The central argument is that the transformation of Sao Paulo city into a great metropolis became the articulating theme with which both authors investigated the performance and adaptation of school-age children, especially those identified as suburbanIII. A new urban culture challenged the school structure and, based on the legacy of Antonio Candido and Florestan Fernandes, such questions were investigated with a new pattern of research, which became the mark of CRPE-SP at that time. This new research pattern paved the way for new analytical resources to be mobilized to study the intelligence of children at school. The shortcomings of biological parameters for understanding the phenomenon of failure and its significant numbers were demonstrated. In addition to a new anthropological understanding about the interaction between the school culture and ways of living of the urban fringes, the research pattern established enriched the repertoire of sociological analyzes of the expansion in the number of school places in Sao Paulo city. For that sociology of education which then renewed, the strongly exclusionary nature of school failure was demonstrated in a masterly manner.

As alterações de mastigação e de deglutição em crianças com hipertrofia adenoamigdaliana precisam ser melhor caracterizadas. OBJETIVO: Avaliar a frequência das alterações miofuncionais autorreportadas pelos pais, e se há diferenças entre os padrões de alterações entre crianças com hipertrofia adenoamigdaliana e as com hipertrofia apenas adenoideana. MÉTODO: Aplicação de questionário e avaliação clínica fonoaudiológica em crianças com hipertrofia de tonsilas com idade entre 3 e 6 anos. Os dados reportados pelos pais foram comparados com os dados obtidos pela avaliação fonoaudiológica; além disso, os dados das crianças com hipertrofia adenoamigdaliana foram comparados com as com hipertrofia adenoideana. Desenho científico: coorte transversal. RESULTADOS: As alterações miofuncionais observadas pela fonoaudióloga foram muito mais frequentes do que as reportadas pelos pais e não houve concordância entre os dois achados. As crianças com hipertrofia adenoideana e as com hipertrofia adenoamigdaliana apresentaram o mesmo padrão de alterações miofuncionais. CONCLUSÃO: Os pais relacionam pouco a hipertrofia de tonsilas a alterações na mastigação e na deglutição. A causa da obstrução respiratória parece não interferir no padrão de alteração miofuncional.

The changes in mastication and deglutition in children with adenotonsillar hypertrophy need to be better characterized. OBJECTIVE: To evaluate the frequency of parent-reported myofunctional changes and to determine if there are differences in the alteration patterns of children with adenotonsillar hypertrophy and subjects with adenoid hypertrophy. METHOD: Questionnaire and assessment by a speech therapist of children aged between three and six years with tonsillar hypertrophy. The data reported by the parents were compared to the data obtained from the speech therapist's evaluation; additionally, data from children with adenotonsillar hypertrophy were compared to findings from subjects with adenoid hypertrophy. Study Design: cross-sectional cohort. RESULTS: The myofunctional changes observed by the speech therapist were more frequent than the alterations reported by the parents, and there was no correlation between the two findings. The children with adenoid hypertrophy and the individuals with adenotonsillar hypertrophy had the same pattern of myofunctional alteration. CONCLUSION: Parents cannot clearly correlate tonsillar hypertrophy with changes in mastication and deglutition. The cause of the respiratory obstruction does not seem to interfere in the pattern of myofunctional change.

O presente artigo tem como objetivo contribuir com a discussão acerca do trabalho profissional do assistente social na contemporaneidade. Considerando as configurações atuais da sociedade, o assistente social encontra-se inserido em diversos campos de trabalho, sendo a execução de medidas socioeducativas um desses campos. Dessa forma, a reflexão que aqui se apresenta vai abordar e discutir algumas informações sobre esta realidade, enfocando o trabalho do assistente social na medida de internação.

This present article has an objective to contribute to the discussion about the Professional work of the Social Worker in the contemporary. Considering the present configuration of the society, the social worker has being inserted in various camps of work, and the implementation of socio-educational measures these camps. So, the reflection presented here will approach and discuss some information about reality, focusing on the work of social workers in the socio-educational measure of confinement regime.

O conhecimento sobre a fisiologia e regulação dos eletrólitos e do equilíbrio ácido-base é necessário, pois a saúde do animal depende diretamente da composição normal dos fluídos nos diversos compartimentos corporais. Esta regulação está intimamente entrelaçada em um rigoroso e preciso controle, de modo que uma anormalidade em um destes compartimentos reflete na alteração dos demais. A avaliação do equilíbrio eletrolítico e ácido-base pode fornecer informações fundamentais para o diagnóstico, prognóstico e decisão terapêutica de diversas enfermidades nos bovinos, tais como diarreias neonatais, hemoparasitoses (anaplasmose e a babesiose), deslocamento de abomaso e processos de sépticos. As correções destas alterações com medidas terapêuticas adequadas são pontos chave para minimizar as perdas por mortalidade causadas por estas enfermidades.

The knowledge of the physiology and regulation of electrolytes and blood gas parameters is necessary because the health of the animal depends directly on the normal composition of fluid in body compartments. The regulation of these is closely intertwined with a rigorous and precise control so that an abnormality in one of these compartments reflects the change in the other. The evaluation of electrolyte and acid-base balance can provide key information for diagnosis, prognosis and therapeutic decision for several diseases in animals such as neonatal diarrhea, anaplasmosis and babesiosis, displacement of abomasum and septic processes. The corrections of these changes with appropriate treatment are key to minimizing losses due to mortality in these diseases.
