Abstract Objective: To assess the prevalence and factors associated with smoking in adolescents with cleft lip and/or palate. Methods: This is a cross-sectional study, developed in a Brazilian public and tertiary hospital between November 2018 and August 2019. Adolescents aged between 12 and 19 years old, previously submitted to cheiloplasty and/or palatoplasty surgeries were included. Data collection was carried out through interviews, guided by questionnaires referring to sociodemographic issues, use and factors associated with smoking. For statistical analysis, Fisher’s Exact Test, Chi-square, Student’s t test and bivariate logistic regression were used, all with a significance level of 5% (p≤0.05). Results: 102 adolescents participated. The prevalence of smoking was 20.6% (n=21). Having cleft lip and palate was associated with smoking (p=0.012). The bivariate analysis showed that not dating (p=0.001; OR=0.19) and owning a home (p=0.032; OR=0.33) were configured as protective factors for the act of being a smoker, while believing that smoking facilitates interaction with young people or with the group of friends (p=0.043; OR=2.95), having friends who use alcoholic beverages (p=0.002; OR=8.40), having friends who use drugs, except cigarettes (p=0.002; OR=5.33), use alcoholic beverages (p<0.001; OR=10.67) and sexual initiation (p<0.001; OR=7.00), were associated with being a smoker. Conclusions: From the knowledge of the profile of adolescents most vulnerable to tobacco use, it is possible to plan and implement educational and preventive actions.

RESUMO Objetivo: Avaliar a prevalência e os fatores associados ao tabagismo em adolescentes com fissura de lábio e/ou palato. Métodos: Estudo de corte transversal, desenvolvido em um hospital público e terciário brasileiro entre novembro de 2018 e agosto de 2019. Foram incluídos adolescentes com idade entre 12 e 19 anos, submetidos previamente às cirurgias de queiloplastia e/ou palatoplastia. A coleta de dados foi realizada por meio de entrevista, guiada por questionários referentes a questões sociodemográficas, uso e fatores associados ao tabagismo. Para a análise estatística, utilizaram-se os testes: exato de Fisher, qui-quadrado, t de Student e regressão logística bivariada, todos com nível de significância de 5% (p ≤ 0,05). Resultados: Participaram 102 adolescentes. A prevalência do tabagismo foi de 20,6% (n=21). Possuir fissura de lábio e palato associou-se ao tabagismo (p=0,012). A análise bivariada mostrou que não namorar (p=0,001; OR=0,19) e possuir habitação própria (p=0,032; OR=0,33) configuraram fatores de proteção para o ato de ser tabagista, enquanto acreditar que fumar facilita a interação com os jovens ou com o grupo de amigos (p=0,043; OR=2,95), ter amigos que usam bebida alcoólica (p=0,002; OR=8,40), ter amigos que usam drogas, exceto o cigarro (p=0,002; OR=5,33), usar bebida alcoólica (p<0,001; OR=10,67) e iniciação sexual (p<0,001; OR=7,00) associaram-se a ser tabagista. Conclusões: Pelo conhecimento do perfil dos adolescentes mais vulneráveis ao uso do tabaco, é possível planejar e programar ações educativas e preventivas.

Resumo Para contribuir com a compreensão dos mecanismos relacionados à sintomatologia e aos sinais associados à erupção dentária, investigou-se a presença de mastócitos nos tecidos pericoronários na fase intraóssea (Grupo 1) e submucosa (Grupo 2), comparando-os entre si e com a mucosa bucal (Grupo 3). Em cada grupo, 14 espécimes foram analisados microscopicamente em cortes corados com hematoxilina e eosina, e imunocitoquimicamente marcados com Ckit e Triptase. Pelos resultados obtidos, concluiu-se que a quantidade/densidade dos mastócitos é diferente nos tecidos foliculares de acordo com a fase de erupção, o que permite inferir que: 1) O traumatismo decorrente da mastigação sobre o conjunto “mucosa bucal com o folículo pericoronário na submucosa” pode explicar porque o epitélio reduzido exporia o esmalte às células do tecido conjuntivo; 2) A exposição das proteínas do esmalte com propriedades antigênicas corresponderia à liberação de antígenos sequestrados que levariam à interação de IgE e mastócitos em número aumentado na região; e 3) A consequente degranulação e liberação de mediadores no local, como histamina, leucotrienes, prostaglandinas, proteases, citocinas e fatores de crescimento, contribuem para a compreensão dos sinais e sintomatologia atribuídos à erupção dentária.

Abstract This study contributes to the understanding of the mechanisms associated with signs and symptoms of tooth eruption, by investigating the presence of mast cells in pericoronal tissues during the intraosseous (Group 1) and submucosal (Group 2) phases of eruption. We compared findings for these two groups with each other and with those for the oral mucosa (Group 3). In each group, 14 specimens were analyzed microscopically after hematoxylin and eosin staining and immunohistochemical analysis of c-Kit and tryptase expression. Results revealed that the number and density of mast cells is different in follicular tissues according to the eruption phase, which may mean that: 1) masticatory trauma of the oral mucosa and dental follicles in the submucosa may explain why reduced enamel epithelium exposes enamel to the cells of the connective tissue; 2) exposure of antigenic enamel proteins might correspond to the release of sequestered antigens, which may lead to the interaction of IgE and a greater number of mast cells in the region; and 3) the consequent degranulation and the local release of mediators, such as histamine, leukotrienes, prostaglandins, proteases, cytokines and growth factors, contribute to the understanding of signs and symptoms associated with tooth eruption.

Abstract The present study aimed to investigate the use of platelet-rich plasma (PRP) on tooth extraction sites in rats treated with bisphosphonates. Thirty Albinus Wistar male rats were administered 0.035 mg/kg zoledronic acid intravenously for 8 weeks, divided into four administrations with a 2-week interval between each application, after which their upper right central incisors were extracted to induce the development of bisphosphonate-related osteonecrosis of the jaw (BRONJ). The samples were divided into the following two groups: Group 1 (G1) underwent marginal resection of BRONJ followed by the use of PRP, while Group 2 (G2) underwent resection of BRONJ but without the use of PRP. The treatment groups were evaluated after 14, 28, and 42 days. Clinical, microtomographic, microscopic, and immunohistochemical (IHC) evaluations were performed. Microtomography results revealed no significant difference between the groups (p <0.05) in any time period. Histomorphometric analysis showed increased bone formation over time for both groups (p < 0.001). G1 demonstrated a greater amount of new bone formation than G2 at 28 and 42 days (p < 0.001), with G1 presenting greater vascularization and a slightly higher VEGF expression. For both groups, RANKL/OPG expression levels were sufficient as a parameter for indicating the rate of bone remodeling in a previously treated area of osteonecrosis groups. Taken together, our findings indicated that the use of PRP improves the resolution process of BRONJ.

Abstract The tongue is one of the primary sources of halitosis. The manual or mechanical removal of biofilm is known to decrease oral malodor. Objective: To evaluate a new tongue hygiene technique hereby referred to as “the X technique” and its effects on both halitosis and the number of microorganisms based on microbiological parameters and diagnostic features of the breath. Material and Methods: The study included thirty patients divided into a control group (patients without systematized guidelines of lingual hygiene, but who performed the mechanical cleaning of tongue dorsum, each in its own way), the 3R group (instructed to perform the movements of the X technique for 3 repetitions at each brushing), and the 6R group (instructed to perform 6 repetitions of the technique at each brushing). After two weeks, a new data collection was performed. Results: Patients in the 6R group presented the lowest score on the organoleptic assessment scale at the second consultation, followed by the 3R group and the controls. Regarding the self-perception of breath by the method of Visual Analog Scale (VAS), the control group did not perceive improvements in oral malodor; the results of the 3R group and the 6R group were similar. Conclusion: These results indicate that the X technique improves both measurements and perceptions of halitosis. Microbiological analyses revealed greatest reduction in the 6R group. The findings show that the X technique reduces both organoleptic scores and the number of bacterial colonies, and improves users’ perceptions of their breath.

RESUMO A Síndrome de West é uma rara e severa forma de epilepsia da infância, com início no primeiro ano de vida e etiologia ainda não definida. Mesmo com a ausência de alguns sintomas, a tríade que define a Síndrome de West é a presença de espasmos, retardo no desenvolvimento psicomotor e a presença da hypssarritmia no eletroencefalograma. O objetivo deste estudo foi a obtenção de dados atualizados na literatura sobre a Síndrome de West e relatar um caso clínico de um paciente com diagnóstico médico dessa síndrome, com gastrostomia e padrão clínico de quadriplegia espástica. No exame clínico inicial observou-se retenção prolongada de dentes decíduos, doença periodontal, padrão pobre de higiene bucal, respiração bucal, palato profundo, mordida aberta anterior, interposição de língua entre os arcos dentais e baixa experiência de cárie. Ao longo de 9 anos o paciente apresentou complicações nas suas condições sistêmicas, com necessidade e realizar gastrostomia e com muitos períodos de internação. Apesar disto, atualmente sua condição de saúde bucal é boa e estável. Os cuidados odontológicos para pessoas com deficiência devem ser desenvolvidos, encorajados e estendidos continuamente, de acordo com os princípios constitucionais da dignidade humana e os direitos de saúde e de igualdade.

ABSTRACT West Syndrome is one of the rare and severe childhood epilepsies, starting in the first year of life and having an uncertain etiology. Even if some of the symptoms are missing, a triad of them defines West Syndrome, including epileptic spasms, arrest or regression of psychomotor development, and hypsarrhythmia on interictal electroencephalography. The objective of this study was to obtain updated data on West Syndrome literature and report a clinical case of a patient with the medical diagnosis of this syndrome, with gastrostomy feed tube, and clinical pattern of spastic quadriplegia. Initial clinical examination showed prolonged retention of deciduous teeth, periodontal disease, poor oral hygiene, mouth breathing, deep palate, anterior open bite, tongue interposition between the dental arches, and low caries experience. Over 9 years the patient presented complications in their sistemicas conditions, with need for gastrostomy and many periods of hospitalization that determined periods of absence for the dental monitoring. Despite this, currently his oral health condition is good and stable. Dental care for people with disabilities should be developed, encouraged and continuously extended, in agreement with the constitutional principles of human dignity and the rights for health and equality.

RESUMO Objetivo: avaliar aspectos topográficos e temporais do osso pré-maxilar e da sutura pré-maxilar/maxilar, por serem elementos anatômicos fundamentais pouco explorados clinicamente. Métodos: foram avaliados 1.138 crânios secos humanos, sendo 116 (10,19%) dos espécimes crianças e 1.022 (89,81%) adultos. Os crânios foram fotografados e determinou-se a porcentagem de abertura da sutura pré-maxilar/maxilar. Posteriormente, os dados foram tabulados e submetidos a análise estatística, adotando-se nível de significância de 5%. Resultados: a progressão de fechamento da sutura pré-maxilar/maxilar do nascimento aos 12 anos de idade foi de 3,72% ao ano. Em 100% dos crânios até 12 anos, observou-se a sutura pré-maxilar/maxilar aberta na região palatina, enquanto 6,16% dos adultos apresentavam diferentes graus. Conclusões: a pré-maxila existe de forma independente dentro do complexo maxilar e a presença da sutura pré-maxilar / maxilar justifica o sucesso de expansões anteroposteriores para estimular o crescimento do terço médio da face, solucionando problemas anatômicos e funcionais.

ABSTRACT Objective: To evaluate topographic and temporal aspects of premaxillary bone and premaxillary-maxillary suture, since they are fundamental anatomical elements little explored clinically. Methods: 1,138 human dry skulls were evaluated, of which 116 (10.19%) of the specimens were children, and 1,022 (89.81%) were adults. The skulls were photographed and the percentage of premaxillary-maxillary suture opening was determined. Subsequently the data were tabulated and submitted to statistical analysis, adopting a level of significance of 5%. Results: The progression of premaxillary suture closure from birth to 12 years of age was 3.72% per year. In 100% of the skulls up to 12 years, the premaxillary-maxillary suture open in the palatal region was observed, while 6.16% of adults presented different degrees of opening. Conclusions: The premaxilla exists in an independent way within the maxillary complex and the presence of the premaxilla-maxillary suture justifies the success of anteroposterior expansions to stimulate the growth of the middle third of the face, solving anatomical and functional problems.

RESUMO Objetivo Determinar o impacto das disfunções temporomandibulares na qualidade de vida. Métodos Foram incluídos 102 pacientes voluntários (68 mulheres) com idades entre 19 e 86 anos, que buscaram atendimento médico nas clínicas de saúde da universidade e foram avaliados no período de setembro a dezembro de 2013. Os pacientes foram examinados segundo os Critérios Diagnósticos para Pesquisa em Disfunções Temporomandibulares, usando um algômetro mecânico (Palpeter®) com pressão padronizada de 0,5 e 1,0kg e o questionário Medical Outcomes Study 36-Item Short Form Health Survey (SF-36), para avaliação da qualidade de vida. Os dados foram tabulados para análise estatística, e as variáveis foram correlacionadas com os achados clínicos das disfunções temporomandibulares e da qualidade de vida. Resultados Metade dos pacientes foi positiva para disfunções temporomandibulares, sendo 39,2% deles classificados como grupo de dor miofascial. O grupo de disfunções temporomandibulares foi significativamente associado ao desconforto ao morder (p=0,0000), à crepitação da articulação temporomandibular (p=0,0001) e ao apertar dos dentes (p=0,0001). O teste de Mann-Whitney usado para analisar o SF-36 revelou que os domínios da dor (pontuação média de 47,80%; p<0,0001) e saúde mental (62,67%; p<0,05) estavam fortemente associados às disfunções temporomandibulares. Conclusão A qualidade de vida de indivíduos com disfunções temporomandibulares foi afetada negativamente pela presença da dor e de transtornos de saúde mental.

ABSTRACT Objective To determine the impact of temporomandibular disorders in quality of life. Methods A total of 102 volunteer patients (68 female) aged 19 to 86 years, who sought medical care in health clinics of the university and were evaluated in the period from September to December 2013. The subjects were examined according to the Research Diagnostic Criteria for Temporomandibular Disorders, using a mechanical algometer (Palpeter®) with standardized pressure of 0.5 and 1.0kg, and Medical Outcomes Study 36-Item Short Form Health Survey (SF-36) questionnaire, to assess quality of life. The data were tabulated for statistical analysis and the variables were correlated with the clinical findings of the temporomandibular disorders and quality of life. Results Fifty percent of patients were positive for temporomandibular disorders and 39.2% classified as myofascial pain group. The temporomandibular disorder group was significantly associated with uncomfortable bite (p=0.0000), temporomandibular joint clicking (p=0.0001) and tooth clenching (p=0.0001). The Mann Whitney test used to analyze the SF-36 revealed that the domains of pain (mean score of 47.80%; p<0.0001) and mental health (62.67%; p<0.05) were strongly associated with temporomandibular disorders. Conclusion The quality of life of individuals with temporomandibular disorders was negatively affected by the presence of pain and mental health disorders.

O objetivo do presente artigo é investigar a relação fluorose/cáries em escolas, de acordo com o nível de fluoreto no abastecimento público de água. A amostra consistiu em 360 alunos de doze anos de idade, de ambos os sexos, que frequentam as escolas próximas da região em que nasceram. As escolas foram classificadas em três grupos de acordo com a presença de fluoreto no abastecimento de água: 1) fluoretação na Estação de Tratamento de Água (ETA); 2) fluoretação direta em poços; e 3) áreas não fluoretadas (ANF). Os testes deχe de Goodman (significância = 5%) foram utilizados para avaliar a associação entre a origem da água e o grau de fluorose. Os resultados mais predominantes foram presença de cáries em toda a amostra (P<0,05); ausência de fluorose em ambos gêneros, para indivíduos brancos e aqueles vivendo em áreas abastecidas com água fluoretada diretamente de poços e não fluoretadas (P<0,05). Não houve diferença no grau de severidade da fluorose entre as fontes de abastecimento de água (P>0,05). As cáries ainda são uma importante questão nessa população, apesar do estabelecimento de medida preventiva. A observação da fluorose em alunos vivendo em áreas não fluoretadas confirma a presença de outras fontes de fluorose.

The sample comprised of twelve-year-old daytime students (360) of both genders, attending schools close to the region where they were born. The schools were classified into three groups according to the presence of fluoride in the water supply: 1) fluoridation in the Water Treatment Station-WTS; 2) Direct Fluoridation in Wells-DFW; and 3) Non Fluoridated Areas-NFA. The χ2 and Goodman tests (significance=5%) were used to evaluate the association between the origin of water and degree of fluorosis; percentage of distribution and descriptive measures were used for investigation of the degree of fluorosis. The following outcomes were predominant: presence of dental caries in the entire sample (P<0.05); absence of fluorosis in both genders, for white individuals and subjects living in areas supplied by both DFW and NFA (P<0.05). There was no difference in the severity of fluorosis between the sources of water supply (P>0.05). Dental caries is still an important problem in this population, despite the establishment of preventive measured. The observation of fluorosis in students living in non-fluoridated areas confirms the presence of other sources of fluoride.
