ABSTRACT The novel coronavirus disease 2019 (COVID-19) has spread rapidly around the world after the first cases were reported in December 2019 in China. Despite the prevention of the symptoms presented, extrapulmonary manifestations were identified. In particular, there was an increase in cases of Acute Mesenteric Ischemia (AMI), raising its incidence to 1.9%-3.8% in infected patients. The aim of this study was to investigate the existence of an association between IMA and COVID-19 through the literature. An Integrative Literature Review was carried out. The research question was “mesenteric ischemia in patients with COVID-19: coincidence or association?”. After searching the database and applying the inclusion and exclusion criteria, 44 were selected for analysis. COVID-19 was confirmed by RT-PCR and imaging tests, gastrointestinal manifestations, alterations and primarily tomographic imaging findings were identified. Most patients were accelerated to laparotomy. As explanations include direct endothelial and injury by the binding of the ACE-2 virus, between hyperinflammation and hypercoagulability, dysregulation of the renin-angiotensin-aldosterone system and factors associated with the severity of the virus. IMA is an emergency with high associated morbidity and mortality, these cases may be a consequence mainly of the thromboinflammatory mechanism associated with SARS-CoV-2. An early diagnosis, diagnosis and diagnoses are crucial to clinical treatment; an assessment regime should be considered in accordance with current evidence and guidelines.

RESUMO A doença do novo coronavírus 2019 (COVID-19) disseminou-se rapidamente pelo mundo após os primeiros casos serem relatados em dezembro de 2019 na China. Apesar da prevalência dos sintomas respiratórios, manifestações extrapulmonares foram sendo identificadas. Particularmente, houve um aumento de casos de Isquemia Mesentérica Aguda (IMA), elevando sua incidência para 1,9%-3,8% em pacientes infectados. O objetivo deste estudo foi investigar a existência de uma associação entre IMA e a COVID-19 através da literatura. Realizou-se uma Revisão Integrativa da Literatura. A pergunta de pesquisa foi “isquemia mesentérica em pacientes com COVID-19: coincidência ou associação?”. Após a busca na base de dados e aplicação dos critérios de inclusão e exclusão, elegeu-se 44 estudos para análise. A COVID-19 foi confirmada por RT-PCR e exames de imagem, foram identificadas manifestações gastrointestinais, alterações laboratoriais e achados de imagem primordialmente tomográfica. A maioria dos pacientes foi submetida à laparotomia. As explicações incluem lesão endotelial direta pela ligação do vírus ao receptor ECA-2, correlação entre hiperinflamação e hipercoagulabilidade, desregulação do sistema renina-angiotensina-aldosterona e fatores associados à gravidade do quadro. A IMA é uma emergência de alta morbimortalidade associada, nesses casos pode ser consequência principalmente do mecanismo tromboinflamatório associado ao SARS-CoV-2. Um alto nível de suspeita clínica, diagnóstico e tratamento precoces são cruciais diante dessa complicação; um regime de anticoagulação deve ser considerado conforme as evidências e diretrizes vigentes..

ABSTRACT The first cases of COVID-19 were diagnosed in China, rapidly evolving with worldwide spread, turning into a pandemic. A percentage of these patients develop the severe form of the disease and progress to respiratory distress syndrome, requiring support in Intensive Care Units. Intra-abdominal Hypertension and Abdominal Compartment Syndrome are characterized by increased intra-abdominal pressure, and are subject to several predisposing factors, such as mechanical ventilation assistance, extracorporeal membrane oxygenation, elevated PEEP, intestinal obstructions, excessive fluid replacement, major burns and coagulopathies. Hence, for the management of patients with severe COVID-19, there are numerous risk factors for the development of intra-abdominal hypertension and abdominal compartment syndrome. Therefore, this study proposes to analyze the variables that directly interfere with the increase in intra-abdominal pressure in patients with COVID-19, as well as the changes in the organic systems caused, through an integrative literature review.

RESUMO Os primeiros casos de COVID-19 foram diagnosticados na China, evoluindo rapidamente com uma disseminação a nível mundial, transformando-se em uma pandemia. Uma porcentagem desses pacientes desenvolve a forma grave da doença e evolui com Síndrome do Desconforto Respiratório, necessitando de suporte em Unidades de Terapia Intensiva. A Hipertensão Intra-abdominal e a Síndrome Compartimental Abdominal são caracterizadas pelo aumento da pressão intra-abdominal, e estão sujeitas a diversos fatores predisponentes, como assistência por ventilação mecânica, oxigenação por membrana extracorpórea, PEEP elevada, obstruções intestinais, reposição excessiva de fluidos, grandes queimados e coagulopatias. Com isso, para o manejo dos pacientes com COVID-19 grave, numerosos são os fatores de risco para o desenvolvimento da Hipertensão intraabdominal e da Síndrome Compartimental Abdominal. Por isso, esse estudo se propõe a analisar as variáveis que interferem diretamente no aumento da pressão intra-abdominal em pacientes com COVID-19, assim como as alterações nos sistemas orgânicos provocadas, por meio de uma revisão integrativa da literatura.

ABSTRACT The first cases of the COVID-19 disease were identified in late 2019 in China, but it didnt take long for it to become pandemic. At first, it was believed that it was restricted to respiratory symptoms only, until extrapulmonary manifestations were reported worldwide. Acute pancreatitis concomitant with the diagnosis of SARS-CoV-2 infection has been observed in some patients, in the absence of the most common etiologies described in the literature. It is postulated that the presence of the ECA-2 viral receptor in the pancreas is responsible for the direct cellular damage and that the hyperinflammatory state of COVID-19 favors the development of pancreatitis through an immune-mediated mechanism. This study aimed to analyze the correlation between acute pancreatitis and COVID-19 disease as a probable causality factor. An integrative literature review was carried out, including studies published between January 2020 and December 2022 that brought data on patients diagnosed with acute pancreatitis according to the revised Atlanta Classification with a confirmed diagnosis of COVID-19 in the same period. A total of thirty studies were reviewed. Demographic, clinical, laboratory and imaging aspects were analyzed and discussed. It is believed that SARS-CoV-2 was responsible for the development of acute pancreatitis in these patients, due to the absence of other precipitating risk factors, as well as the close temporal relationship between both. Attention should be given to gastrointestinal manifestations in patients affected by COVID-19.

RESUMO Os primeiros casos da doença COVID-19 foram identificados no final de 2019 na China, mas não foi necessário muito tempo para que se tornasse pandêmica. Acreditava-se, a princípio, que ela fosse restrita apenas a sintomas respiratórios, até que manifestações extrapulmonares fossem mundialmente relatadas. Quadros de pancreatite aguda concomitantes ao diagnóstico de infecção por SARS-CoV-2 vêm sendo observados em alguns pacientes, na ausência das etiologias mais comuns descritas na literatura. Postula-se que a presença do receptor viral ECA-2 no pâncreas seja responsável pelo dano celular direto e que o estado hiperinflamatório da COVID-19 favoreça o desenvolvimento da pancreatite por mecanismo imunomediado. Este estudo teve como objetivo analisar a correlação entre pancreatite aguda e a doença COVID-19 como um provável fator de causalidade. Realizou-se uma revisão integrativa da literatura, foram incluídos estudos publicados entre janeiro de 2020 e dezembro de 2022 que trouxessem dados acerca de pacientes diagnosticados com pancreatite aguda conforme a Classificação de Atlanta revisada com diagnóstico confirmado de COVID-19 no mesmo período. Um total de trinta estudos foram revisados. Aspectos demográficos, clínicos, laboratoriais e de imagem foram analisados e discutidos. Acredita-se que o SARS-CoV-2 foi o responsável pelo desenvolvimento de pancreatite aguda nestes pacientes, devido à ausência de demais fatores de risco precipitantes, bem como à estreita relação temporal entre ambos. Uma atenção deve ser dada às manifestações gastrointestinais em pacientes acometidos pela COVID-19.

ABSTRACT Polycystic liver disease, a hereditary pathology, usually manifests as autosomal dominant polycystic kidney disease. The many cysts in the liver cause massive hepatomegaly, majorly affecting the patient’s quality of life. In cases of refractory symptoms, liver transplantation is the only treatment choice. A 43-year-old woman was followed up as a hepatology outpatient in August 2020, with a progressive increase in abdominal volume, lower limb edema, and cachexia. The patient was diagnosed with polycystic renal and liver disease with massive hepatomegaly in March 2021, a combined kidney-liver transplant. Liver size represented 13% of the patient’s corporal composition, weighing 8.6kg. The patient was discharged on the 7th postoperative day with no complications. Only 10-20% of patients with polycystic liver disease have clinical manifestations, most of which result from hepatomegaly. An increase in liver volume deteriorates liver function until the condition becomes end-stage liver disease, as kidney function is already compromised; liver-kidney transplantation remains the only treatment choice. The case described drew significant attention to the massive hepatomegaly presented in the patient, with the liver representing over 10% of the patient’s body weight, approximately five to six times larger than a normal-sized liver.

ABSTRACT Introduction: upon infection with SARS-CoV-2, patients presented with non-classical symptoms, such as gastrointestinal phenomena including loss of appetite, nausea, vomiting, diarrhea, and abdominal pain abdominal pain. These occurrences, typically, were found in severely affected patients with COVID-19. With this, the aim of this paper is to analyze the available knowledge on the development of acute abdomen in SARS-CoV-2 infected patients. Methodology: this is an Integrative Review in PubMed, Web of Science and VHL databases. The following descriptors were used: “Acute abdomen”, “COVID-19”, “Abdominal pain” and “SARS-CoV-2” with the Boolean operator “AND”, and articles relevant to the theme were selected. Initially, 331 articles were selected, all published between 2020 and 2023, in Portuguese and/or English. After analysis, 11 articles matched the proposed objective. Results: the relationship between tenderness in the right upper region or the presence of Murphy’s sign contributed in the association between abdominal pain and the more severe forms of COVID-19 in infected patients. The number of diagnoses for acute conditions such as cholecystitis, appendicitis, diverticulitis and pancreatitis decreased with the pandemic, but at the same time there was an increase in the duration of surgical procedures and in the length of hospital stays. These acute abdominal conditions were the result of delayed demand for hospital care, which also contributed to an increase in the conversion rate to open surgery and in the number of perforative conditions. Conclusion: the development of acute abdomen in SARS-CoV-2 infected patients was predictive of an unfavorable prognosis.

RESUMO Introdução: a partir da infecção com o SARS-CoV-2, os pacientes apresentaram sintomas não clássicos, como fenômenos gastrointestinais que incluem perda de apetite, náuseas, vômitos, diarréia e dores abdominais. Essas ocorrências, normalmente, foram encontradas em pacientes gravemente afetados pelo COVID-19. Com isso, o objetivo deste trabalho é analisar o conhecimento disponível sobre o desenvolvimento do abdome agudo em pacientes infectados pelo SARS-CoV-2. Metodologia: trata-se de uma Revisão Integrativa na base de dados PubMed, Web of Science e BVS. Foi utilizado os descritores: “Acute abdomen”, “COVID-19”, “Abdominal pain” e “SARS-CoV-2” com o operador booleano “AND”, e selecionados artigos de relevância para o tema. Inicialmente, foram selecionados 331 artigos, todos publicados entre 2020 e 2023, em português e/ou inglês. Após análise, 11 artigos corresponderam ao objetivo proposto. Resultados: a relação entre a sensibilidade na região superior direita ou a presença do sinal de Murphy contribuiu na associação entre dor abdominal e as formas mais graves do COVID-19 em pacientes infectados. O número de diagnósticos para quadros agudos como colecistite, apendicite, diverticulite e pancreatite tiveram diminuição com a pandemia, mas ao mesmo tempo houve aumento na duração dos procedimentos cirúrgicos e nos períodos de internações. Esses quadros de abdome agudo foram resultados da procura tardia de assistência hospitalar que colaborou, inclusive no aumento da taxa de conversão para a cirurgia aberta e no número de quadros perfurativos. Conclusão: o desenvolvimento do abdome agudo em pacientes infectados pelo SARS-CoV-2 foi preditor de um prognóstico desfavorável.

ABSTRACT Introduction: the persistence of long-term symptoms of COVID-19 represents a new challenge for the medical-scientific community, it is the condition called long-term COVID-19. Irritable Bowel Syndrome (IBS) is one of the most common Disorders of the Gut-Brain Interaction and its post-infection development is already validated. According to the Rome IV criteria, it is characterized by the presence of recurrent abdominal pain, on average, at least 1 day a week in the last 3 months with onset of symptoms at least 6 months before diagnosis, associated with 2 or more factors: related to defecation and/or associated with change in stool frequency and/or associated with change in stool form. This study aimed to review data on post-COVID-19 IBS. Methods: this is an integrative review of studies published between January 1, 2020 and April 30, 2023, which presented data on IBS with previously diagnosed COVID-19 disease. The PubMed database was used, the descriptors were “Irritable bowel syndrome” AND “COVID-19”; the reference list of the articles was also retrieved. Results: eight studies were reviewed, it was observed that 0.6% to 11.6% of patients had IBS again after a minimum period of 6 months of infection. Risk factors were female gender, severity of COVID-19, presence of acute-phase gastrointestinal symptoms, and depression/anxiety. Conclusion: the results obtained suggest that COVID-19 may be associated with the emergence of de novo IBS. Further studies are needed to investigate its long-term effects and clinical spectra.

RESUMO Introdução: a persistência de sintomas a longo prazo da COVID-19 representa um novo desafio à comunidade médico-científica, é a condição denominada COVID-19 longo. A Síndrome do Intestino Irritável (SII) é um dos Distúrbios da Interação Intestino Cérebro mais comuns e seu desenvolvimento pós-infecção já é validado. Segundo os critérios de Roma IV, caracteriza-se por presença de dor abdominal recorrente, em média, pelo menos 1 dia por semana nos últimos 3 meses com início dos sintomas pelo menos 6 meses antes do diagnóstico, associada a 2 ou mais fatores: relacionada à defecação e/ou associada com mudança na frequência das fezes e/ou associada com mudança na forma das fezes. Este estudo teve como objetivo revisar dados acerca de SII pós-COVID-19. Métodos: trata-se de uma Revisão Integrativa de estudos publicados entre 1º de janeiro de 2020 a 30 de abril de 2023, que apresentassem dados acerca de SII com a doença COVID-19 previamente diagnosticada. Utilizou-se a base de dados PubMed, os descritores foram “Irritable bowel syndrome” AND “COVID-19”; a lista de referência dos artigos também foi recuperada. Resultados: oito estudos foram revisados, observou-se que 0,6% a 11,6% pacientes cursaram com SII de novo após um período mínimo de 6 meses da infecção. Os fatores de risco foram sexo feminino, gravidade da COVID-19, presença de sintomas gastrointestinais de fase aguda e depressão/ansiedade. Conclusão: os resultados obtidos permitem crer que a COVID-19 pode estar associada ao surgimento de SII de novo. São necessários outros estudos para investigar seus efeitos a longo prazo e espectros clínicos..

RESUMO Introdução : O transplante de coração ainda é a melhor alternativa terapêutica para o tratamento da insuficiência cardíaca terminal. A utilização de critérios que considerem as complicações associadas a esse procedimento pode garantir melhor avaliação do receptor e preparar a equipe para possíveis resultados insatisfatórios no pós-transplante. O uso do escore MELD vem sendo expandido para avaliar pacientes cirróticos submetidos a diversos procedimentos, incluindo o transplante cardíaco. Objetivo : Analisar o conhecimento produzido relacionando o escore MELD e os seus derivados com o prognóstico dos pacientes com insuficiência cardíaca terminal considerados para o transplante cardíaco. Método : Foi realizada revisão integrativa das publicações dos últimos dez anos nas bases de dados Pubmed e Lilacs, utilizando os descritores “transplante cardíaco”, “doença hepática” e “prognóstico”. Do total de 111 artigos encontrados, seis foram selecionados e compuseram a amostra. Resultados : O escore MELD-XI (eXcluding INR) foi o mais analisado nos estudos devido à exclusão do INR, já que boa parte dos pacientes com insuficiência cardíaca fazem uso de anticoagulantes, podendo alterar o seu valor. O MELD e derivados esteve associado aos resultados insatisfatórios no transplante cardíaco. Conclusão : O escore MELD pode ser considerado como bom preditor para o transplante cardíaco; porém, ainda são poucos os estudos que fazem essa correlação.

ABSTRACT Introduction: Heart transplantation is still the best therapeutic alternative for the treatment of end-stage heart failure. The use of criteria that consider the complications associated with this procedure can guarantee a better evaluation of the recipient and prepare the team for possible unsatisfactory post-transplant results. The use of the MELD score has been expanded to evaluate cirrhotic patients undergoing various procedures, including cardiac transplantation. Objective: To analyze the knowledge on MELD score and its derivatives to the prognosis of patients with end-stage heart failure considered for heart transplantation. Method: Was carried out an integrative review of the publications of the last ten years in Pubmed and Lilacs databases, using the descriptors “heart transplantation”, “liver disease” and “prognosis”. From the total of 111 articles found, six were selected and composed the sample. Results: The MELD-XI score (eXcluding INR) was the most analyzed in the studies due to the exclusion of INR, since many patients with heart failure use anticoagulants, which may alter their value. MELD and derivatives were associated with unsatisfactory results in cardiac transplantation. Conclusion: The MELD score can be considered as a good predictor for heart transplantation; however, there are still few studies that make this correlation.

RESUMO Racional: O transplante de fígado (TF) é a única opção eficaz e duradoura para os pacientes com doença hepática em estágio terminal. Inovações e refinamentos nas técnicas cirúrgicas ocorreram com o advento dos transplantes com enxertos parciais e da laparoscopia. Apesar dessas modificações, a incisão abdominal permanece sem grandes mudanças. Objetivo: Demonstrar a experiência com a incisão mediana superior nos receptores de TF com enxertos hepáticos inteiros provenientes de doadores falecidos. Método: É estudo retrospectivo entre os pacientes submetidos ao TF. Foram coletados os dados dos receptores que realizaram o procedimento cirúrgico através da incisão mediana superior . Resultados: Essa incisão foi utilizada em 20 TF, sendo 19 realizados em receptores adultos. A principal causa foi a doença hepática secundária ao álcool. O gênero masculino, IMC>25 kg/m² e o MELD superior a 20 foram prevalentes no estudo. Complicações biliares ocorreram em dois pacientes. Hemoperitônio foi indicação de reoperação em um dos receptores. Complicação da ferida cirúrgica ocorreu em dois pacientes, que apresentaram infecção de sítio cirúrgico superficial e evisceração (omental). Ocorreram dois re-transplantes na primeira semana de pós-operatório devido à disfunção grave do enxerto e à trombose da artéria hepática, sendo realizados com a mesma incisão, sem a necessidade de ampliar o acesso cirúrgico. Ocorreram dois óbitos por disfunção grave do enxerto após o re-transplante em 72 h e sepse respiratória com disfunção de múltiplos órgãos na terceira semana. Conclusão: A incisão mediana superior pode ser utilizada com segurança em receptores de TF com enxertos inteiros provenientes de doadores falecidos. Entretanto, características do receptor e tamanho do enxerto hepático devem ser considerados na opção do acesso cirúrgico abdominal.

ABSTRACT Background: Liver transplant (LT) is the only effective and long-lasting option for patients with end-stage liver disease. Innovations and refinements in surgical techniques occurred with the advent of transplants with partial grafts and laparoscopy. Despite these modifications, the abdominal incision remains with only few changes. Aim: Demonstrate the experience with the upper midline incision in LT recipients with whole liver grafts from deceased donors. Methods: Retrospective study with patients submitted to LT. Data were collected from the recipients who performed the surgical procedure through the upper midline incision. Results: The upper midline incision was used in 20 LT, 19 of which were performed in adult recipients. The main cause was liver disease secondary to alcohol. Male, BMI>25 kg/m² and MELD greater than 20 were prevalent in the study. Biliary complications occurred in two patients. Hemoperitoneum was an indication for reoperation at one of the receptors. Complication of the surgical wound occurred in two patients, who presented superficial surgical site infection and evisceration (omental). Two re-transplant occurred in the first postoperative week due to severe graft dysfunction and hepatic artery thrombosis, which were performed with the same incision, without the need to increase surgical access. There were two deaths due to severe graft dysfunction after re-transplant in 72 h and respiratory sepsis with multiple organ dysfunction in the third week. Conclusion: The upper midline incision can be safely used in LT recipients with whole grafts from deceased donors. However, receptor characteristics and hepatic graft size should be considered in the option of abdominal surgical access.

RESUMO Racional: A incidência das variações anatômicas da artéria hepática varia de 20-50% em diferentes casuísticas. Elas são especialmente importantes no contexto do transplante ortotópico hepático, visto que, além de representar oportunidade ideal para seu estudo anatômico cirúrgico, a sua precisa identificação é determinante para o sucesso do procedimento. Objetivo: Identificar as variações anatômicas no sistema arterial hepático em transplantes hepáticos. Método: Foram analisados retrospectivamente, no período de 13 anos, 479 prontuários de pacientes adultos transplantados, sendo coletados dados referentes à anatomia arterial hepática do doador falecido. Resultados: Identificou-se anatomia arterial hepática normal em 416 doadores (86,84%). Os outros 63 indivíduos (13,15%) apresentaram alguma variação. De acordo com a classificação de Michels, as anomalias mais frequentes foram: artéria hepática direita ramo da artéria mesentérica superior (Tipo III, n=27, 5,63%); artéria hepática esquerda ramo da artéria gástrica esquerda (Tipo II, n=13, 2,71%); artéria hepática direita ramo da artéria mesentérica superior associada à artéria hepática esquerda ramo da artéria gástrica esquerda (Tipo IV, n=4, 0,83%). Do mesmo modo, em relação à Classificação de Hiatt, as variações mais prevalentes foram: artéria hepática direita acessória ou substituta da artéria mesentérica superior (Tipo III, n=28, 6,05%), seguida da artéria hepática esquerda acessória ou substituta da artéria gástrica esquerda (Tipo II, n=16, 3,34%). Quatorze pessoas (2,92%) apresentaram alterações anatômicas sem classificação definida, sendo a de maior frequência o tronco hepatomesentérico, identificado em cinco (1,04%). Conclusão: O conhecimento detalhado das variações da anatomia arterial hepática é de grande importância aos cirurgiões que realizam abordagens nessa região, em especial no transplante hepático, visto que sua identificação e correto manejo são fundamentais para o êxito do procedimento.

ABSTRACT Background: The incidence of anatomic variations of hepatic artery ranges from 20-50% in different series. Variations are especially important in the context of liver orthotopic transplantation, since, besides being an ideal opportunity for surgical anatomical study, their precise identification is crucial to the success of the procedure. Aim: To identify the anatomical variations in the hepatic arterial system in hepatic transplantation. Methods: 479 medical records of transplanted adult patients in the 13-year period were retrospectively analyzed, and collected data on hepatic arterial anatomy of the deceased donor. Results: It was identified normal hepatic arterial anatomy in 416 donors (86.84%). The other 63 patients (13.15%) showed some variation. According to the Michels classification, the most frequently observed abnormalities were: right hepatic artery branch of superior mesenteric artery (Type III, n=27, 5.63%); left hepatic artery branch of the left gastric artery (Type II, n=13, 2.71%); right hepatic artery arising from the superior mesenteric artery associated with the left hepatic artery arising from the left gastric artery (Type IV, n=4, 0.83%). Similarly, in relation to Hiatt classification, the most prevalent changes were: right hepatic accessory artery or substitute of the superior mesenteric artery (Type III, n=28, 6.05%)), followed by liver ancillary left artery or replacement of gastric artery left (Type II, n=16, 3.34. Fourteen donors (2.92%) showed no anatomical abnormalities defined in classifications, the highest frequency being hepatomesenteric trunk identified in five (01.04%). Conclusion: Detailed knowledge of the variations of hepatic arterial anatomy is of utmost importance to surgeons who perform approaches in this area, particularly in liver transplantation, since their identification and proper management are critical to the success of the procedure.

RESUMO Introdução: O transplante de fígado tem como finalidade o aumento da sobrevida dos pacientes com doença hepática crônica em fase terminal, além de melhora na qualidade de vida. Desde o primeiro transplante até os dias atuais, muitas mudanças ocorreram no sistema de alocação de órgãos. Objetivo: Analisar o conhecimento produzido sobre o Model for End-stage Liver Disease (MELD) e a sua relação com a sobrevida no pós-transplante de fígado. Método: Realizou-se revisão integrativa nas bases de dados Lilacs, SciELO e Pubmed no mês de outubro de 2015. A amostra contou com oito estudos relacionando o escore MELD e o seu impacto no transplante de fígado. Resultados: Houve predomínio dos transplantes realizados em homens e faixa etária entre 45-55 anos. Como principais indicações tem-se hepatite C, hepatocarcinoma e cirrose por álcool. Os fatores que tiveram maior impacto no pós-operatório estão associados ao alto valor do MELD, idade do receptor, critérios expandidos do doador e hemotransfusão. Conclusão: O sistema MELD reduziu a mortalidade na fila de espera, mas isoladamente não é um bom preditor de sobrevivência no pós-transplante de fígado.

ABSTRACT Introduction: Liver transplantation is intended to increase the survival of patients with chronic liver disease in terminal phase, as well as improved quality of life. Since the first transplant until today many changes have occurred in the organ allocation system. Objective: To review the literature on the Model for End-stage Liver Disease (MELD) and analyze its correlation with survival after liver transplantation. Method: An integrative literature review in Lilacs, SciELO, and Pubmed in October 2015, was realized. Were included eight studies related to the MELD score and its impact on liver transplant. Results: There was predominance of transplants in male between 45-55 y. The main indications were hepatitis C, hepatocellular carcinoma and alcoholic cirrhosis. The most important factors post-surgery were related to the MELD score, the recipient age, expanded donor criteria and hemotransfusion. Conclusion: The MELD system reduced the death rate in patients waiting for a liver transplant. However, this score by itself is not a good predictor of survival after liver transplantation.

Racional: Apendicite é causa comum de emergência cirúrgica, que na população de indivíduos submetidos ao transplante de órgãos possui incidência rara e atrasos no diagnóstico são frequentes. Objetivo: Relatar a ocorrência de apendicite aguda em uma coorte de pacientes receptores de transplante hepático. Método: Foram analisados retrospectivamente, no período de 12 anos casuística de 925 transplantes de fígado, onde cinco casos de apendicite aguda foram encontrados. Resultados: O aparecimento da apendicite ocorreu entre 3 e 46 meses após o transplante, a idade variou entre 15 e 58 anos; três eram homens (60%) e duas mulheres (40%). As apresentações clínicas foram variadas, mas não discordantes daquelas encontradas em pacientes não transplantados. Dor foi achado presente em todos os pacientes, sendo em dois bem localizada em fossa ilíaca direita (40%). Dois deles apresentaram sintomatologia característica de irritação peritoneal (40%) e um distensão abdominal (20%). Todos foram abordados por laparotomia. Em 20% não houve complicações e em 80% foram realizadas apendicectomias complicadas por supuração (40%) ou perfuração (40%). Infecção do sítio cirúrgico superficial ocorreu em dois pacientes tratados clinicamente. O tempo de alta hospitalar variou de 48 h a 45 dias. Conclusão: A apendicite aguda após transplante hepático é evento raro. Associa-se com alta taxa de perfuração decorrente aos atrasos no diagnóstico e tratamento. Cursa com mais longo internamento hospitalar.

Background: Appendicitis is a common cause of emergency surgery that in the population undergoing organ transplantation presents a rare incidence due to late diagnosis and treatment. Aim: To report the occurrence of acute appendicitis in a cohort of liver transplant recipients. Methods: Retrospective analysis in a period of 12 years among 925 liver transplants, in witch five cases of acute appendicitis were encountered. Results: Appendicitis occurred between three and 46 months after liver transplantation. The age ranged between 15 and 58 years. There were three men and two women. The clinical presentations varied, but not discordant from those found in non-transplanted patients. Pain was a symptom found in all patients, in two cases well located in the right iliac fossa (40%). Two patients had symptoms characteristic of peritoneal irritation (40%) and one patient had abdominal distention (20%). All patients were submitted to laparotomies. In 20% there were no complications. In 80% was performed appendectomy complicated by suppuration (40%) or perforation (40%). Superficial infection of the surgical site occurred in two patients, requiring clinical management. The hospital stay ranged from 48 h to 45 days. Conclusion: Acute appendicitis after liver transplantation is a rare event being associated with a high rate of drilling, due to delays in diagnosis and therapy, and an increase in hospital stay.

RACIONAL: A dilatação cística da via biliar é uma patologia rara e de origem incerta. É diagnosticada mais frequentemente em crianças, porém sua incidência vem aumentando em adultos, representando 20% dos casos. OBJETIVO: Demonstrar a experiência dos autores no tratamento cirúrgico, e no manejo da evolução, dos pacientes com dilatação cística das vias biliares. MÉTODO: Foram avaliados, retrospectivamente, cinco adultos que tiveram o diagnóstico de cisto de colédoco e que foram submetidos a algum procedimento cirúrgico no Hospital da Restauração, PE, entre março de 2003 e junho de 2004. RESULTADOS: Dor abdominal foi a queixa comum a todos os pacientes. Icterícia esteve presente em 80% dos casos. A ultrassonografia foi realizada em todos os casos como exame inicial. Tomografia computadorizada, ressonância magnética e colangiopancreatografia retrógada endoscópica também foram realizadas em alguns pacientes, porém o diagnóstico foi estabelecido no intra-operatório em todos os casos. O tratamento cirúrgico variou de acordo com a experiência do cirurgião e o quadro clínico do paciente. A ressecção do cisto com reconstrução da via biliar foi realizada em 60%, a cistoduodenostomia em 20% e a drenagem da via biliar em 20% dos casos. CONCLUSÃO: A dilatação cística da via biliar é doença rara, principalmente em nosso meio. Porém, sua incidência vem aumentando na população adulta, devendo estar sempre presente no diagnóstico diferencial das icterícias obstrutivas.

BACKGROUND: The cystic dilatation of the biliary tract is a rare disease and uncertain origin. It is recognized more frequently in children; however, its incidence comes increasing in adults, representing 20% of the cases. AIM: To evaluate morbimortality rates, evolution and handing of patients with cystic dilatation bile ducts in adults. METHODS: Were evaluated, retrospectively, five adults who had the diagnosis of choledochal cyst and that had been submitted to some surgical procedure. RESULTS: Abdominal pain was the commonest complain to all patients. Jaundice was present in 80%. Ultrasound scanning was done in all the cases as initial examination. CT scan, magnetic resonance imaging and endoscopic retrograde cholangiopancreatography were also done in some patients; however, the diagnosis was established intra-operatively in all cases. The cyst resection with reconstruction of the biliary tract was done in 60%; the cystojejunostomy in 20%; and in 20% biliary tract drainage. CONCLUSIONS: Biliary tract cystic dilatation is a rare disease. However, its incidence is increasing in the adult population, so, it must be thought as differential diagnosis when facing obstructive jaundice.

RACIONAL: Os pacientes em unidade de terapia intensiva estão em risco de desenvolver hipertensão intra-abdominal e síndrome compartimental abdominal. OBJETIVOS: Descrever a relação entre o Sequential Organ Failure Assessment (SOFA) com a pressão intra-abdominal e a relação do SOFA com fatores de risco para hipertensão intra-abdominal. MÉTODO: Com base nas recomendações da World Society of Abdominal Compartment Syndrome, foram medidas as pressões intra-abdominais dos pacientes nas 24h e 48h da admissão na UTI e calculado o SOFA ao final das 24h e 48h. O tempo de coleta foi de dois meses. RESULTADOS: Não houve correlação entre o SOFA e a pressão intra-abdominal. Foram 70% de homens com idade média de 44 anos, sendo 10% oriundos da cirurgia geral (pressão intra-abdominal média de 11) e 65% da neurocirurgia (pressão intra-abdominal média de 6,7). Apenas três (7,5%) apresentaram hipertensão intra-abdominal. O SOFA máximo foi de 15 e a falência orgânica mais frequente foi a neurológica com 77%. Houve forte correlação entre o SOFA das 24h e 48h com a pressão de pico respiratória (ρ=0,43/p=0,01; ρ=0,39/p=0,02). CONCLUSÕES: Não houve correlação entre o SOFA e a pressão intra-abdominal nos pacientes aqui estudados; contudo, sinalizou ser possível em pacientes com operação abdominal ou naqueles com sepse abdominal.

BACKGROUND: Patients in the intensive care unit are at risk of developing intra-abdominal hypertension and abdominal compartment syndrome. AIM: To describe the relation between Sequential Organ Failure Assessment (SOFA) vs. intra-abdominal pressure and the relation between SOFA and risk factors for intra-abdominal hypertension. METHOD: In accordance with the recommendations of the World Society of the Abdominal Compartment Syndrome, the present study measured the intra-abdominal pressure of patients 24 h and 48 h after admission to the unit and calculated the SOFA after 24 h and 48 h. Data was collected over two-month period. RESULTS: No correlation was found between SOFA and intra-abdominal pressure. Seventy percent of the patients were men and the mean age was 44 years, 10% had been referred from general surgery (with a mean intra-abdominal pressure of 11) and 65% from neurosurgery (with a mean intra-abdominal of 6.7). Only three (7.5%) presented with intra-abdominal hypertension. The highest SOFA was 15 and the most frequent kind of organ failure was neurological, with a frequency of 77%. There was a strong correlation between the SOFA after 24 h and 48 h and peak respiratory pressure (ρ=0.43/p=0.01; ρ=0.39/p=0.02). CONCLUSION: No correlation was found between SOFA and intra-abdominal pressure in the patients covered by the present study. However, it is possible in patients undergoing abdominal surgery or those with abdominal sepsis.
