Resumo Introdução: Nova estratégia de terapia de indução com dose única de 3 mg/kg de globulina antitimócitos de coelho (r-ATG) mostrou menor incidência de rejeição aguda. Métodos: O objetivo foi utilizar dados do mundo real para determinar a razão de custo-efetividade incremental (RCEI) da indução com r-ATG para prevenção da rejeição aguda (RA) no primeiro ano pós-transplante renal e para sobrevida do enxerto renal em 1, 4 e 10 anos pós-transplante, sob a perspectiva do sistema nacional de saúde pública. Desenvolveu-se um modelo de transição de estado de Markov utilizando dados do mundo real extraídos de faturas médicas de um único centro. A população do estudo consistiu em adultos com baixo risco imunológico submetidos ao primeiro transplante, recebendo rins de doadores vivos ou falecidos. A intervenção de indução com r-ATG foi comparada à não-indução. Os desfechos clínicos considerados para essa análise foram rejeição aguda, infecção/doença por citomegalovírus, óbito, perda do enxerto e retransplante. Resultados: Análise de custo-efetividade no primeiro ano revelou que o grupo r-ATG foi mais custo-efetivo, com RCEI de US$ 399,96 por episódio de RA evitado, ganho de efetividade de 0,01 ano na sobrevida do enxerto e custo incremental total de US$ 147,50. As análises de 4 e 10 anos revelaram ganho de efetividade de 0,06 e 0,16 anos na sobrevida do enxerto no grupo de indução com r-ATG e custo incremental total de US$ −321,68 e US$ −2.440,62, respectivamente. Conclusão: A dose única de 3 mg/kg de r-ATG é custo-efetiva na prevenção de episódios de RA e dominante no longo prazo do transplante, conferindo ganho de sobrevida.

Abstract Background: A new induction therapy strategy of a single 3 mg/kg dose of rabbit antithymocyte globulin (r-ATG) showed a lower incidence of acute rejection. Methods: The objective of this study was to use real-world data to determine the incremental cost-effectiveness ratio (ICER) of r-ATG induction for the prevention of acute rejection (AR) in the first year following kidney transplantation and for kidney graft survival over 1, 4, and 10 years of post-transplantation from the perspective of the national public healthcare system. A Markov state transition model was developed utilizing real-world data extracted from medical invoices from a single center. The study population consisted of adults at low immunological risk undergoing their initial transplantation and received kidneys from either living or deceased donors. The intervention of r-ATG induction was compared to no induction. The clinical outcomes considered for this analysis were acute rejection, cytomegalovirus infection/disease, death, graft loss, and retransplantation. Results: The cost-effectiveness analysis in the first year revealed that the r-ATG group was more cost-effective, with an ICER of US$ 399.96 per avoided AR episode, an effectiveness gain of 0.01 year in graft survival and a total incremental cost of US$ 147.50. The 4- and 10-year analyses revealed an effectiveness gain of 0.06 and 0.16 years in graft survival in the r-ATG induction group, and a total incremental cost of US$ −321.68 and US$ −2,440.62, respectively. Conclusion: The single 3 mg/kg dose of r-ATG is cost-effective in preventing acute rejection episodes and dominant in the long term of transplantation, conferring survival gain.

Resumo Fundamento Na avaliação da cardiopatia isquêmica, a ressonância magnética cardiovascular (RMC) é considerada o padrão-ouro para detecção e localização do infarto do miocárdio (IM), mas o eletrocardiograma (ECG) é menos dispendioso e mais disponível. O reconhecimento do IM pelo ECG fora da fase aguda é desafiador: ondas Q estão ausentes numa proporção significativa dos pacientes e podem reduzir ou desaparecer com o tempo. Apesar de amplamente utilizado na avaliação inicial de infarto prévio, os estudos para validação do ECG tendo como referência a RMC no contexto da doença coronariana crônica são limitados. Objetivos Avaliar o desempenho diagnóstico do ECG para detecção e localização do IM definido pela RMC. Métodos Foram incluídos 352 indivíduos que realizaram RMC e ECG, sendo 241 pacientes com IM prévio confirmado pela RMC e 111 controles com RMC normal. Os traçados de ECG foram analisados por 2 observadores cegos em relação à RMC para detecção e localização do IM de acordo com a IV Diretriz da Sociedade Brasileira de Cardiologia sobre a Análise e Emissão de Laudos Eletrocardiográficos. A significância adotada foi de 5% (p < 0,05). Resultados O ECG apresentou bom desempenho para a detecção do IM prévio, com sensibilidade de 69,3% (64,5% a 74,1%), especificidade de 99,1% (98,1% a 100%) e acurácia de 78,7% (74,4% a 83,0%). Entretanto, para localizar o IM de acordo com a RMC, a acurácia foi insatisfatória. Conclusões O ECG se mostrou um método com boa acurácia para detecção de IM prévio quando comparado à RMC, mas não para definir sua localização.

Abstract Background In the assessment of ischemic heart disease, cardiac magnetic resonance (CMR) is considered the gold standard for detecting and locating myocardial infarction (MI), but electrocardiogram (ECG) is less expensive and more widely available. Recognition of MI on ECG outside the acute phase is challenging; Q waves are absent in a significant proportion of patients and may reduce or disappear over time. Although ECG is widely used in the initial assessment of previous infarction, studies to validate ECG using CMR as a reference in the context of chronic coronary disease are limited. Objectives To evaluate the diagnostic performance of ECG in detecting and locating CMR-defined MI. Methods This study included 352 individuals who underwent CMR and ECG, 241 patients with previous MI confirmed by CMR and 111 controls with normal CMR. Their ECG tracings were analyzed by 2 observers, who were blinded to the CMR, for detection and location of MI following to the Fourth Brazilian Society of Cardiology Guidelines on the Analysis and Issuance of Electrocardiographic Reports. The significance level adopted was 5% (p < 0.05). Results ECG showed good performance for detecting previous MI, with sensitivity of 69.3% (64.5% to 74.1%), specificity of 99.1% (98.1% to 100%), and accuracy of 78.7% (74.4% to 83.0%). However, in locating MI in accordance with CMR, its accuracy was unsatisfactory. Conclusions When compared to CMR, ECG was shown to be a method with good accuracy for detecting previous MI, but not for defining its location.

Abstract Synthesis, characterization and biological activity of the Cu(II)-1,3-PDTA complex was performed by two synthetic routes: sodium and carbonate pathways. The Cu (II)-1,3-PDTA complex can be easily produced by the 1,3-PDTA binder, commercially available as Trilon F (BASF) which is available as sodium or acid salt for the coordination of Cu (II). The biological activity of the Cu (II)-1,3-PDTA [Cu (II)-PDTA] complex was performed to analyze the activity against the bacterium Erwinia chrysanthemi Bancroft. The bacterium is the causative agent of soft rot in the parsley maniac (Arracacia xanthorrhiza Bancroft). The bacterium was isolated from parsley manioc infected with soft rot. The metal complex sensitivity tests were performed by diffusion antibiogram. The mean values of the halos and standard deviations of the zones of inhibition were obtained for the [Cu (II) (PDTA)] and Streptomycin complexes. The concentrations of the complexes evaluated were 10-3, 10-2 and 10-1 M (equivalent to 36.3 g L-1 (3.63%), 3.63 g L-1 (0.363%) and 0.363 g L-1 (0.0363%), respectively). The halos were 25.0 ± 1.2 mm for control. For [Cu (II) (PDTA)] complexes, the obtained values were 9.5 ± 0.6, 15.0 ± 0.8 and 24.0 ± 0.8 via sodium and 12.0 ± 1.6, 25.0 ± 0.8 and 31.0 ± 1.9 via carbonate, for 10-3, 10-2 and 10-1 M, respectively. The results showed that the bactericidal activity of the Cu (II)-1,3-PDTA complex obtained by the two synthetic routes are adequate for the control of Erwinia chrysanthemi Bancroft.

Resumo Objetivou-se determinar o desempenho diagnóstico do ABSI para obesidade e obesidade sarcopênica, comparados aos resultados da bioimpedância (BIA) e do IMC, por sexo e faixa etária. Trata-se de um estudo seccional com 12.793 participantes da segunda onda do ELSA-Brasil, que obteve medidas de percentual de gordura corporal utilizando BIA e antropometria, verificando-se a performance dos testes diagnósticos para comparar os índices. Os resultados mostraram que, para obesidade, em homens nas três faixas etárias, a sensibilidade foi abaixo de 49%. A especificidade variou de 73% a 93%, sendo maior na faixa etária de 65 a 79 anos. Esse padrão foi encontrado para as mulheres, sendo a sensibilidade inferior a 42%; a especificidade variou entre 85% e 92%, sendo maior nas idosas. Quanto à obesidade sarcopênica, a sensibilidade e a especificidade entre os homens nas três faixas etárias foram de 72% e 99%, respectivamente. Entre as mulheres, a sensibilidade foi de aproximadamente 95%, e a especificidade em torno de 70% em todas as faixas etárias. O ABSI, comparado à BIA, mostrou reduzida capacidade de discriminar obesidade, mas mostrou-se bom marcador de obesidade sarcopênica.

Abstract The scope of this study was to determine the diagnostic performance of ABSI for obesity and sarcopenic obesity, compared to the results of bioimpedance analysis (BIA) and BMI, by sex and age group. It involved a cross-sectional study with 12,793 participants in the second round of ELSA-Brasil (Longitudinal Study of Adult Health in Brazil), which obtained measurements of body fat percentage using BIA and anthropometry, verifying the performance of the diagnostic tests in order to compare the indices. The results showed that for obesity in men in all three age groups, the sensitivity was below 49%. Specificity ranged from 73% to 93%, with a higher percentage in the 65 to 79 age group. The same pattern was found for women, with sensitivity lower than 42%, and specificity ranged from 85% to 92%, and was higher in older women. With respect to sarcopenic obesity, the sensitivity and specificity among men in the three age groups were 72% was 99%, respectively. Among women, sensitivity was around 95% and specificity around 70% in all age groups. ABSI, compared to BIA, showed a reduced ability to identify obesity. However, it proved to be a good marker of sarcopenic obesity.

Los resultados mostraron que, para la obesidad, en los hombres de los tres grupos de edad, la sensibilidad estaba por debajo del 49%. RESUMO Objetivo. Descrever a experiência brasileira na resposta a emergências de saúde pública em 2023, bem como a estrutura organizacional e as características epidemiológicas. Métodos. Foram analisadas três emergências (estudos de casos) ocorridas durante o ano do estudo de acordo com as ações empreendidas durante a resposta e os impactos na população. As emergências de saúde pública foram resumidas e analisadas por meio de pesquisas em documentos e sites oficiais do Ministério da Saúde e de autoridades sanitárias municipais. Resultados. Caso 1 – Emergência devido à falta de assistência: considerada pelo governo brasileiro uma emergência de saúde pública de importância nacional, exigiu a mobilização de um Centro de Operações de Emergência em Saúde Pública (COE) para a resposta devido à complexidade das ações. Caso 2 – Emergência devido a zoonoses (arbovírus e influenza aviária): a estrutura de resposta do COE foi usada para desenvolver atividades para interromper o surgimento de arbovírus e evitar uma emergência de influenza aviária no país. Caso 3 – Emergências devido a desastres: exigiram estruturas de resposta municipais e uso do COE para coordenar ações com o apoio do governo federal. Conclusões. A experiência brasileira destaca a importância dos seguintes fatores: coordenação intra e intersetorial na gestão das ações de resposta; monitoramento e comunicação constantes para aprimorar as medidas de resposta; apoio nacional aos esforços municipais de resposta; e adaptação da estrutura do COE aos diferentes contextos e complexidades das emergências enfrentadas.

ABSTRACT Objective. To describe the Brazilian experience of responding to public health emergencies in 2023, the organizational structure, and epidemiological characteristics. Methods. Three emergencies (case studies) that occurred during the study year were analyzed according to the actions implemented during the response and the impacts on the population. The public health emergencies were summarized and analyzed through research on official documents and websites of the Ministry of Health and local health authorities. Results. Case 1 – Emergency due to lack of assistance, considered by the Brazilian government to be a public health emergency of national importance, required an Emergency Operations Center (EOC) for the response due to the complexity of the actions. Case 2 – Emergency due to zoonoses (arboviruses and avian influenza) used the EOC response structure to develop activities aimed at stopping the emergence of arboviruses and preventing an avian influenza emergency in the country. Finally, case 3 – Emergencies due to disasters required local response structures and used the EOC to coordinate their actions with the federal government’s support. Conclusions. The Brazilian experience highlights the importance of intrasectoral and intersectoral coordination in managing response actions, constant monitoring and communication to improve response measures, national support for local response efforts, and adaptation of the EOC structure to the different contexts and complexities of the emergencies faced.

RESUMEN Objetivo. Describir la experiencia de la respuesta a emergencias de salud pública en Brasil en el 2023, su estructura organizativa y sus características epidemiológicas. Métodos. Se analizaron tres emergencias (estudios de casos) ocurridas durante el año de estudio en lo que respecta a las medidas aplicadas durante la respuesta y su impacto en la población. Las emergencias de salud pública se resumieron y analizaron mediante la investigación de los documentos y sitios web oficiales del Ministerio de Salud y de las autoridades de salud locales. Resultados. En el caso 1, una emergencia por falta de asistencia que el gobierno brasileño consideró una emergencia de salud pública de importancia nacional, fue necesario un centro de operaciones de emergencia (COE) para la respuesta debido a la complejidad de las medidas. En el caso 2, una emergencia debida a varias zoonosis (infecciones por arbovirus y gripe aviar), se utilizó la estructura de respuesta del COE para llevar a cabo actividades destinadas a detener el surgimiento de los arbovirus y prevenir una emergencia de gripe aviar en el país. Por último, en el caso 3, las emergencias debidas a catástrofes requirieron estructuras de respuesta locales y utilizaron el COE para coordinar las medidas aplicadas, con el apoyo del gobierno federal. Conclusiones. La experiencia brasileña destaca la importancia de la coordinación intra e intersectorial en la gestión de las medidas de respuesta, el seguimiento y la comunicación constantes para mejorar esas medidas, el apoyo nacional a las actividades locales de respuesta y la adaptación de la estructura del COE a los diferentes contextos y complejidades de las emergencias afrontadas.

Abstract This paper evaluates the aspects of dosage, construction, quality control, and functional and structural quality of self-compacting concrete (SCC) paving slabs built in a vehicle parking area, on a flat surface, with reduced thickness (100 mm). The previously established target for slump flow for applying 8 m3 SCC was agility; aspects related to shrinkage and surface are discussed herein, as well as other lessons learned in a preliminary study. The surface and structural characteristics resulting from the pioneer application were verified by texture tests and dynamic load tests (Falling Weight Deflectometer). The strength and stiffness results were indicative of the SCC's superior quality in terms of durability to fatigue. The stress analysis based on the measured parameters for the concrete slabs indicated the full possibility of using SCC, with economic advantages due to the significant reduction in design thickness, for automobile parking lots, resulting in very low stresses (under 20% of the flexural strength). Regarding deflections, the values measured are about twice the values commonly found for conventional concrete pavements because of the thin slab thickness. Finally, an expressively lower modular relationship (E/MR) was obtained revealing the potential of SCC to perform better than conventional concrete in face of fatigue damage.

Resumo Este artigo avalia os aspectos de dosagem, construção, controle e qualidade funcional e estrutural de placas de concreto autoadensável (CAD) para pavimentação construídas em uma área de estacionamento de veículos, em superfície plana e com espessura reduzida (100 mm). A meta previamente estabelecida em termos de slump flow para aplicação de 8 m3 de CAD foi pautado pela agilidade na aplicação; aspectos relacionados à retração e acabamento da superfície são discutidos, bem como as lições aprendidas em um estudo preliminar. As características de superfície e estruturais resultantes da aplicação pioneira foram avaliadas através de testes de textura e carga dinâmica (Alling Weight Deflectometer). Os resultados de resistência e rigidez são indicativos da qualidade superior do SCC em termos de durabilidade à fadiga. A análise de tensões baseada nas medidas de parâmetros para placas de concreto indicou a possibilidade de utilização do SCC, com vantagens econômicas devido à significativa redução da espessura de projeto, para estacionamentos de automóveis, resultando em uma tensão muito baixa (abaixo de 20% da resistência à tração na flexão). Considerando as deflexões, os valores medidos são cerca de duas vezes os valores comumente encontrados para pavimentos de concreto devido à espessura de placa delgada. Finalmente, uma relação modular (E/MR) expressivamente menor foi obtida revelando o grande potencial do SCC apresentar melhor desempenho do que concretos convencionais em face do dano por fadiga.

Abstract The objective of this study was to assess the quality of Conilon coffee (Coffea canephora) based on agronomic, physicochemical, sensory profiles, and molecular markers. The dataset comprises 107 genotypes from the Incaper breeding program, evaluated in Marilândia, ES, Brazil. A total of 30 traits and 14 SSR markers were examined, revealing significant genetic variability. Most genotypes exhibited a medium June harvest cycle, uniform ripening, medium-sized beans, high processing yield, and a high percentage of flat and peaberry beans. Physicochemical variables, total titratable acidity, and potassium leaching contributed significantly to the observed variability. Chlorogenic acids (4.82%) and caffeine (2.58%) were the most abundant bioactive compounds. In sensory evaluations, 34 genotypes received scores exceeding 80. 13 markers were recommended for future association mapping studies to identify QTLs influencing the traits evaluated. Results identified genotypes with potential to maximize heterosis and genetic diversity in advancing generations. Among the evaluated genotypes, 34 were identified as promising due to their high sensory scores (≥ 80 points) and favorable agronomic, physicochemical, sensory, and genetic traits. The progeny HS17 stood out as the most divergent in agronomic and genetic traits compared to the other genotypes.

ABSTRACT Objectives: To find evidence on the influence of hyperglycemia in the perioperative period of liver transplantation. Methods: This is a scoping review of the PubMed, VHL, and Web of Science databases. The following descriptors were used: “Liver transplantation,” “Hyperglycemia,” and “Blood Sugar Control,” with the Boolean operator “AND,” and articles of relevance to the topic were selected. Initially, 139 articles were selected, all published in the last 20 years, in Portuguese and English. After analysis, 10 articles corresponded to the proposed objective. Results: In comparing strict control and conventional glucose control in patients undergoing liver transplantation, the average intraoperative glycemia was 143.3 mg/dL in the conventional group and 130.7 mg/dL in the strict control group, which received more insulin. The intensive care unit stay was similar with a mean of 3 days, but survival improved significantly with mean intraoperative glycemia ≤ 120 mg/dL. Patients with postreperfusion syndrome required insulin infusion more often. New-onset diabetes mellitus and early bacteremia were more frequent in the postreperfusion syndrome group. AST and ALT levels were higher in hyperglycemic patients. Surgical site infection rates correlated with postoperative hyperglycemia, with 20% for glycemia < 200 mg/dL. Patients with postoperative glucose < 200 mg/dL. Patients with diabetes had higher preoperative glycemia than patients without diabetes, with a gradual reduction after liver transplantation, whereas patients without diabetes had a peak on days 2 and 3 before returning to baseline levels. High glucose levels 48–72 h post-transplant were associated with higher mortality and a worse response to insulin treatment in the first week. Conclusion: Strict glycemic control in patients undergoing liver transplantation resulted in lower intraoperative glycemia and significantly better survival for those with glycemia ≤ 120 mg/dL, as well as lower rates of infection, rejection, and mortality, especially in patients without diabetes.

RESUMO Objetivos: Encontrar evidências sobre a influência da hiperglicemia no perioperatório de transplante de fígado. Métodos: Trata-se de uma revisão de escopo na base de dados PubMed, BVS e Web of Science. Foram utilizados os descritores: “Liver transplantation”, “Hyperglycemia” e “Blood Sugar Control” com o operador booleano “AND”, e selecionados artigos de relevância para o tema. Inicialmente, foram selecionados 139 artigos, todos publicados nos últimos 20 anos, em português e inglês. Após análise, 10 artigos corresponderam ao objetivo proposto. Resultados: Na comparação entre controle rigoroso e controle convencional da glicose em pacientes submetidos a transplante de fígado, a média de glicemia intraoperatória foi de 143,3 mg/dL no grupo convencional e 130,7 mg/dL no grupo de controle rigoroso, que recebeu mais insulina. A permanência na unidade de terapia intensiva foi similar, com média de 3 dias, mas a sobrevida melhorou significativamente com glicemia intraoperatória média ≤ 120 mg/dL. Pacientes com síndrome pós-reperfusão necessitaram mais frequentemente de infusão de insulina. O diabetes mellitus de início recente e a bacteremia precoce foram mais frequentes no grupo com síndrome pós-reperfusão. Níveis de AST e ALT foram mais elevados em pacientes hiperglicêmicos. As taxas de infecção do sítio cirúrgico correlacionaram-se com a hiperglicemia pós-operatória, sendo 20% para glicemia < 200 mg/dL. Pacientes com glicose pós-operatória < 200 mg/dL. Os pacientes com diabetes apresentaram glicemia pré-operatória mais alta que pacientes sem diabetes, com redução gradual após o transplante de fígado, enquanto os pacientes sem diabetes tiveram um pico nos dias 2 e 3 antes de retornarem aos níveis basais. Níveis elevados de glicose após 48–72 h pós-transplante foram associados a maior mortalidade e pior resposta ao tratamento com insulina na primeira semana. Conclusão: O controle rigoroso da glicemia em pacientes submetidos a transplante de fígado resultou em uma glicemia intraoperatória mais baixa e uma sobrevida significativamente melhor para aqueles com glicemia ≤ 120 mg/dL, além de menores taxas de infecção, rejeição e mortalidade, especialmente em pacientes sem diabetes.

RESUMO Objetivo: Descrever o protocolo utilizado para tradução e adaptação transcultural dos questionários elaborados pela Fundação Roma para o diagnóstico de distúrbios da interação intestino-cérebro na população pediátrica. Métodos: O protocolo foi proposto com base em revisão narrativa da literatura sobre processo de adaptação cultural de instrumentos de aferição em epidemiologia, analisando suas etapas, verificando seu emprego e exequibilidade. Foram incorporadas ao protocolo, as diretrizes para a adaptação transcultural dos instrumentos diagnósticos elaborados pela Fundação Roma, que define e revisa periodicamente os critérios diagnósticos. Resultados: O protocolo proposto inclui: (i) preparação; (ii) tradução; (iii) reconciliação; (iv) retrotradução; (v) revisão da retrotradução; (vi) debriefing cognitivo; (vii) revisão final; (viii) cálculo do índice de validade de conteúdo; e (ix) aprovação pela Fundação Roma. Conclusões: Os passos metodológicos descritos neste protocolo poderão contribuir para futuras traduções e adaptações transculturais de questionários diagnósticos de distúrbios da interação intestino-cérebro e demais materiais da Fundação Roma, possibilitando seu emprego em estudos epidemiológicos.

Abstract Objective: To describe the protocol used for translation and cross-cultural adaptation of the questionnaires developed by the Rome Foundation for the diagnosis of disorders of gut-brain interaction in the pediatric population. Methods: The protocol was proposed based on a narrative review of the literature on the cultural adaptation process of measurement instruments in epidemiology, analyzing its stages, and verifying its use and feasibility. The guidelines for the cross-cultural adaptation of diagnostic instruments developed by the Rome Foundation, which defines and periodically reviews diagnostic criteria, were incorporated into the protocol. Results: The proposed protocol includes: (i) preparation; (ii) forward translation; (iii) reconciliation; (iv) backward translation; (v) review of the backward translation; (vi) cognitive debriefing; (vii) final review; (viii) calculation of the item content validity index; and (ix) approval by the Rome Foundation. Conclusions: The methodological steps described in this protocol may contribute to future translations and cross-cultural adaptations of diagnostic questionnaires of disorders of gut-brain interaction and other materials from the Rome Foundation, enabling their use in epidemiological studies.

Abstract The year of construction is an essential factor in assessing the condition of bridges, as it directly influences the prediction of their deterioration and the analysis of their load-bearing capacity over time. In addition, this data is fundamental for algorithms that deal with missing information, such as that related to design standards. This study proposes and validates an algorithm to estimate the year of construction of Brazilian bridges based on three main characteristics: design vehicle, barrier type and width. The aim of the algorithm is to fill this information gap and provide managers with information to manage these infrastructures. An analysis of 5,000 bridges in the Bridge Management System revealed that 41.7% of them had no record of the year of construction. During validation, the algorithm showed robust performance, achieving a hit rate of 87.82 percent and a mean absolute error (MAE) of 5.20 years. After adjustments, the MAE was reduced to 4.45 years, with accuracy rates of 99.46% (±10 years) and 79.62% (±5 years). In practice, the algorithm successfully estimated the year of construction in 81.87% of cases, with the majority of bridges built in the 1970s (64.16%). These results provide a solid basis for more accurate deterioration predictions and more reliable load capacity assessments, contributing to more efficient and safer bridge management.

ABSTRACT Tuberculosis (TB) is the most common comorbidity in people living with HIV/AIDS (PLWH), including those under antiretroviral treatment. PLWH are 28 times more likely to develop TB in Brazil, the leading cause of HIV-related deaths globally, with approximately 161,000 reported deaths worldwide in 2023. Early diagnosis of latent tuberculosis infection (LTBI) and prophylactic therapy can reduce TB cases, prevent disease progression, and decrease transmission in high-risk populations. This study assessed the prevalence of LTBI in PLWH using the interferon-gamma release assay (IGRA) and the impact of the 3HP regimen (isoniazid [INH]/rifapentine [RPT]) as prophylactic treatment. Blood samples from 335 PLWH (78% of the 427 in the cohort) were tested for IGRA; 50 PLWH (15%) tested positive and were treated with 3HP. Treatment included 900 mg of INH and 900 mg of RPT in 12 weekly doses according to the Brazilian health guidelines. No specific risk factors, including nadir CD4+T count, age, gender, or antiretroviral therapy (ART), were more frequently observed in the PLWH with LTBI compared to the PLWH without LTBI. All PLWH with LTBI received treatment and no cases of active TB were observed. Our findings highlight the need for wider LTBI screening and treatment among PLWH in the latent phase, emphasizing more stringent approaches for implementing 3HP prophylaxis.

ABSTRACT This study aimed to evaluate the effects of supplementation of live yeast (LY) associated with starch levels (SL) on the in vitro kinetic parameters of the gas production (GP), in vitro neutral detergent fiber digestibility (IVNDFD), volatile fatty acid (VFA) and methane (CH4) production of Urochloa brizantha cv. Marandu grass harvested in both the rainy and dry seasons. The study was conducted in a 2 × 3 factorial arrangement, whereby LY supplementation (with and without LY) was associated with 0, 10, and 20 % of starch supplementation. The variables were analyzed using PROC MIXED in SAS (Statistical Analysis System, version 9.3). LY and SL had no interaction effects on any of the variables tested. LY supplementation increased total GP at 72 h of incubation in both forage seasons by approximately 25 %. Additionally, LY supplementation decreased lag time; however, the GP rate was unaffected. The LY also caused a slight drop in IVNDFD at 48 h in both forage seasons. LY supplementation decreased total VFA production by 25.96 % for the rainy season and 11.32 % for the dry season and caused a 40 and 46 % drop in CH4 for the rainy and dry season forage, respectively. The assumption that combining LY and starch would enhance LY activity was not confirmed in this study, as most analyzed variables showed no interaction between the two factors. Consequently, no beneficial effects on increased SL could be attributed to LY supplementation.

Resumo Os avanços na ressonância magnética cardíaca possibilitaram a caracterização do tecido com alta resolução espacial e de contraste, ao passo que as sequências de realce tardio (RT) de gadolínio contribuíram para a melhoria da detecção da fibrose miocárdica. O padrão de distribuição do RT facilita a diferenciação entre etiologias isquêmicas e não isquêmicas e ajuda a refinar os diagnósticos dentro das cardiomiopatias não isquêmicas, sugerindo fatores etiológicos específicos. Um padrão distinto de RT não isquêmico que ganhou destaque recentemente é o padrão em forma de anel (ring-like), definido como um realce subepicárdico ou circunferencial ou semicircunferencial da parede média, que envolve pelo menos três segmentos contíguos de um mesmo corte no eixo curto. Inicialmente identificado como um marcador de diagnóstico para a desmoplaquina e a filamina C, o padrão foi relatado em condições não genéticas; contudo, continua sendo um achado incomum nessas doenças. Neste artigo, nosso objetivo é apresentar os diagnósticos diferenciais do RT em forma de anel e seus imitadores. A combinação de características epidemiológicas, clínicas, eletrocardiográficas e adicionais permite um refinamento do diagnóstico diferencial do RT em forma de anel.

Abstract Advances in cardiac magnetic resonance have promoted tissue characterization with high spatial and contrast resolution, and late gadolinium enhancement (LGE) sequences have improved the detection of myocardial fibrosis. The distribution pattern of LGE facilitates differentiation between ischemic and nonischemic etiologies and aids in refining diagnoses within nonischemic cardiomyopathies, suggesting specific etiological factors. A distinctive nonischemic LGE pattern that has recently gained prominence is the ring-like pattern, defined as a subepicardial or mid-wall circumferential or semi-circumferential enhancement, which involves at least three contiguous segments within the same short-axis slice. Initially identified as a diagnostic marker for desmoplakin and filamin C-related cardiomyopathies, the pattern has been reported in nongenetic conditions; nevertheless, it remains an uncommon finding in these diseases. In this article, we aim to present the differential diagnoses of ring-like LGE and its mimics. The combination of epidemiological, clinical, electrocardiographic, and additional features enables a focused refinement of the differential diagnosis associated with ring-like LGE.
