Abstract Objective This article presents a clinical and cytogenomic approach that focuses on the diagnosis of syndromic oral clefts (OCs). Methods The inclusion criteria were individuals with OC presenting four or more minor signs and no major defects (non-syndromic oral clefts [NSOCs]) as well as individuals with OC presenting at least another major defect, regardless of the number of minor signs (syndromic oral clefts [SOCs]). The exclusion criteria included NSOC with less than four minor signs, SOC with known etiology, as well as atypical oral clefts. Results Of 1647 individuals with OC recorded in the Brazilian Database of Craniofacial Anomalies, 100 individuals were selected for chromosome microarray analysis (CMA). Among these, 44 individuals were clinically classified as NSOC and 56 as SOC. CMA was performed for both groups, and abnormal CMA was identified in 9%, all previously classified as SCO. The clinical and CMA data analyses showed a significant predominance of abnormal CMA in individuals classified as SOC (p = 0.0044); prematurity, weight, length, and head circumference at birth were significantly lower in the group with abnormal CMA. Besides, minor signs were significantly higher in this group (p = 0.0090). Conclusion The rigorous selection of cases indicates that the significant variables could help in early recognition of SOC. This study reinforces the importance of applying the CMA technique to establish the diagnosis of SOC. This is an important and universal issue in clinical practice for intervention, care, and genetic counseling.

INTRODUÇÃO: O exame intraoperatório por congelação tornou-se um procedimento de rotina na avaliação do linfonodo sentinela axilar no câncer de mama. OBJETIVOS: Avaliar a acurácia e a sensibilidade do FS na detecção de metástases em linfonodo sentinela axilar e investigar o valor preditivo para metástases de variáveis, como idade dos pacientes, estadiamento, tipo histológico, grau e expressão do receptor de estrogênio do tumor. MATERIAL E MÉTODOS: Foram analisados, retrospectivamente, os resultados de 177 procedimentos de congelação. A idade dos pacientes e as características dos tumores foram organizadas em um banco de dados e a relação com a presença de metástases foi analisada. RESULTADOS: Foram detectadas metástases em 22 (12%) casos. Todas as macrometastases e uma micrometastases foram detectadas pelo método de congelação. Micrometastases adicionais foram identificadas nas análises pós-operatórias, cinco por coloração com hematoxilina e eosina (H) e três por imuno-histoquímica. O método de congelação mostrou acurácia geral de 95%, sensibilidade de 64% e especificidade de 100%. Nenhuma associação significativa foi observada entre a presença de metástases e as variáveis analisadas. CONCLUSÃO: Nossos resultados mostram que o exame por congelação possui acurácia e sensibilidade elevadas para a detecção de macrometastases; no entanto, é pouco eficiente na identificação de micrometastases. O uso de imuno-histoquímica melhora a detecção de metástases na análise pós-operatória. A idade do paciente e as características do tumor, como estadiamento, tipo histológico, grau e a expressão do receptor de estrogênio têm de valor preditivo baixo para metástases nodais em câncer de mama.

INTRODUCTION: Intraoperative frozen section analysis has become a routine procedure to evaluate the status of axillary sentinel lymph nodes in breast cancer. OBJECTIVES: To evaluate the accuracy and sensitivity of FS in the detection of metastases in axillary sentinel lymph nodes and to investigate the predictive value of variables such as patients' age, tumor staging, histology, grade, and estrogen receptor expression. MATERIAL AND METHODS: We analyzed retrospectively the results of 177 FS procedures. The patients' age and tumor characteristics were organized in a database and the association with the presence of metastases was analyzed. RESULTS: Metastases were detected in 22 cases (12%). All macrometastases and one micrometastasis were detected by FS. Additional micrometastases were detected in post-operative analysis, from which five were determined by hematoxylin and eosin staining (H) and three by immunohistochemistry (IHC). FS diagnosis data proved to have an overall accuracy of 95%, sensitivity of 64%, and specificity of 100%. None of the analyzed variables showed significant association with lymph node metastases. CONCLUSION: Our results show that intraoperative FS is a highly accurate and sensitive method to detect macrometastases. However, it is inaccurate in the detection of micrometastases. The use of IHC improves the detection of micrometastases in postoperative analyses. The patient's age and tumor characteristics such as staging, histology, grade and estrogen receptor expression have low predictive value for lymph node metastasis in breast cancer.

This study reports an adult AML-M4 patient with atypical chromosomal aberrations present in all dividing bone marrow cell at diagnosis: t(1;8)(p32.1;q24.2), der(9)t(9;10)(q22;?), and ins(19;9)(p13.3;q22q34) that may have originated transcripts with leukemogenic potential.

A introdução do mesilato de imatinibe como tratamento da leucemia mielóide crônica tem salvado muitos pacientes, mas o sucesso da terapia tem sido prejudicado pela resistência e possível não destruição do clone maligno. Este artigo descreve a resposta citogenética e padrões citogenéticos anormais envolvendo os genes ABL e BCR detectados por FISH em pacientes em uso exclusivo de imatinibe. Os resultados mostraram que outras alterações envolvendo os genes BCR e ABL não parecem estar relacionadas à resistência à droga, elas ocorrem em baixas freqüências e podem não estar associadas à resposta citogenética ou ao tempo de tratamento. Contudo, a resposta ao imatinibe parece ser individual e imprevisível, independente do tempo e do início do tratamento após o diagnóstico.

The introduction of imatinib mesylate as treatment of chronic myelogenous leukemia has saved many patients, but the success of therapy is hampered by resistance and possible non-destruction of the malignant clone. This article describes the cytogenetic responses and abnormal cytogenetic patterns involving the ABL and BCR genes detected by FISH in patients who use exclusively imatinib. The results showed that other alterations involving the BCR and ABL genes do not seem to be related to resistance to the drug as they occur in low frequencies and can not be associated to the cytogenetic response or to the time of treatment. Moreover, the response to imatinib seems to be individual and unpredictable, independent of the time of treatment and of its initiation after diagnosis.

Com os avanços terapêuticos que incluem o mesilato de imatinibe, o transplante de medula óssea e a infusão de linfócito do doador, a perspectiva de vida dos portadores de leucemia mielóide crônica (LMC) tem aumentado significativamente e a doença pode não ser fatal. No entanto, os mecanismos biológicos que privilegiam a seleção das células hematopoéticas malignas sobre as células normais na LMC, responsáveis pelo insucesso terapêutico em muitos casos, ainda não estão totalmente esclarecidos. Alterações no processo de apoptose celular e escape das células leucêmicas à resposta imune antitumoral poderiam explicar, em parte, a vantagem seletiva dessas células. O processo de apoptose celular pode ser desencadeado pelas vias intrínseca ou extrínseca. A extrínseca é dependente da interação de receptores de morte celular, como a ligação do receptor Fas com seu receptor, o Fas ligante (FasL). A expressão diminuída de Fas e aumentada de FasL na célula leucêmica podem aumentar sua sobrevida tornando-a resistente à apoptose. Esse artigo descreve a relação entre LMC e o sistema Fas/FasL, sua possível importância no prognóstico e escape das células leucêmicas à resposta imune.

With therapeutic advances that include imatinib mesylate, bone marrow transplantation and donor lymphocyte infusion, life expectancy of chronic myeloid leukemia carriers has increased significantly and the disease might not be fatal. However, the biological mechanisms that favor the selection of malignant clone cells over normal cells, in most cases responsible for the lack of therapeutic success, are still unclear. Alterations in cellular apoptosis process and escape of the leukemic cells from the anti-tumor immune response can explain, in part, the selective advantages of these cells. The apoptosis process or programmed cell death can be triggered by intrinsic or extrinsic pathways. The extrinsic is dependent on the interaction of cell death receptors, such as the Fas receptor and its agonist, the Fas ligand (FasL). A diminished expression of Fas and increased of FasL in leukemic cell may increase its survival, thereby making it resistant to apoptosis. This paper describes the relation between chronic myeloid leukemia and the Fas/FasL system and its possible importance in prognosis and in escape of the leukemic cells from the immune system.

The neonatal period is considered the most effective for the screening of hemoglobinopathies. This allows prophylaxis and prevention, improving the patient's survival and guidance of parents and heterozygote carriers. The present work aims at the early detection of abnormal hemoglobins, the establishment of standard analysis and to examine the viability of the prevention program. Blood samples were collected by heel stick and from blood cord of children born in the Hospital de Base São José do Rio Preto, from April 1998 to November 1999. Electrophoresis and cytological, biochemical, cromatographic analyses were made for abnormal hemoglobin characterization. A total of 1,478 neonatal blood samples were analyzed in which 14.62% presented with hemoglobins alterations: 3.32% had Hb S; 0.61% had Hb C; 7.44% were suggestive of alpha thalassemia; 1.55% were suggestive of beta thalassemia, and 1.70% had alpha/beta thalassemia interactions. The samples collected from the blood cord showed better results in all analyses while the blood samples collected by heel stick on filter paper, were applicable to only specific methodologies. The routine laboratory methods allowed identification of the thalassemic and variant forms, and isoelectric focusing presented sensitivity only for variant identification in this age range. The suspected cases were reassessed after six months, which permitted genetic counseling of their family members and clinic attendance. A multidisciplinary approach in programs of this kind is fundamental for its success.

Existem várias razões que justificam o título de "guardião do genoma" do gene P53. Seu envolvimento, direto ou indireto, tem sido observado na etiopatogenia de praticamente todas as neoplasias humanas, incluindo as leucemias e linfomas. Conhecer seus mecanismos de ação é fundamental para compreender os aspectos moleculares da carcinogênese. O presente trabalho apresenta uma revisão sobre as características deste gene e sua importância no diagnóstico, prognóstico e terapêutica, o que faz dele um alvo em potencial das estratégias de terapia gênica.

There are several reasons which justify the name of 'guardian of the genome' given to the P53 gene. Its involvement either directly or indirectly has been observed in the pathology of practically all human neoplasias, including leukemia and lymphomas. Knowledge of its mechanisms of action is fundamental to understand molecular aspects of carcinogenesis. This work presents a revision of the characteristics of this gene and its importance in the diagnosis, prognosis and treatment and why this makes it a potential target for gene therapy strategies.

Alterações genéticas em que a mutação de aminoácidos nas globinas afeta a estrutura da molécula tornando-a instável são classificadas como hemoglobinas instáveis. Devido à grande diversidade dos pontos de mutações por substituições e deleções de aminoácidos, as formas de instabilização se apresentam muito variadas. A hemoglobina Köln é a variante instável descrita com maior freqüência na literatura e a terceira descoberta no Brasil, as outras são Hb Niterói e Hb Hasharon. Anemia moderada, icterícia e presença de urina escura caracterizam as manifestações clínicas da Hb Köln. Em programa de triagem neonatal identificamos uma criança com suspeita de heterozigose para hemoglobina Köln, confirmada por procedimentos eletroforéticos e HPLC. Avaliações por diferentes metodologias laboratoriais e estudo familiar auxiliam no diagnóstico precoce, possibilitando minimizar os sintomas decorrentes da hemoglobina anormal e a realização do aconselhamento genético e educacional destas alterações hereditárias.

Hemoglobinopathies are a diverse group of inherited recessive disorders that include thalassemias and sickle cell disease. They were the first genetic diseases to be characterized at the molecular level and consequently have been used repeatedly as a prototype for the development of new techniques of mutation detection. A major group of the structural hemoglobins exhibit the property of instability in solution, resulting from their altered molecular structures. Mutation of amino acids in the globin affects the structure of the molecule turning it unstable and they are classified as unstable hemoglobins. Due to the great diversity of the mutation points, substitutions and deletions of amino acids, the unstable forms are very varied. Hemoglobin Köln is the unstable variant most described in literature and the third discovered in Brazil, the others are Hb Niterói and Hb Hasharon. Moderate anemia, jaundice and the presence of dark urine characterize Hb Köln’s clinical manifestations. In a neonatal screening program we identified a child suspected of having hemoglobin Köln, which was confirmed by electrophoretic procedures and HPLC. Evaluations by different laboratory examinations and the family study aided in a precocious diagnosis, thereby facilitating to minimize the current symptoms of the abnormal hemoglobin, and thus the family received support with genetic and educational counseling of these hereditary alterations.

As hemoglobinopatias são as doenças genéticas mais freqüentes na população humana. Cerca de 12 a 15% da população é portadora de uma ou mais formas de hemoglobinas anormais, resultando em um grande problema de saúde pública. O diagnóstico neonatal possibilita o tratamento e o aconselhamento genético precoce, incluindo a conscientização dos portadores sobre o risco do nascimento de homozigotos. O objetivo deste trabalho é apresentar os resultados da investigação de hemoglobinopatias em recém-nascidos do Hospital de Base de São José do Rio Preto. O estudo foi realizado em 913 amostras de sangue de cordão umbilical, que foram submetidas a testes eletroforéticos, bioquímicos e citológicos, específicos para análise de hemoglobinas. Foram identificadas 100 (10,95%) amostras com hemoglobinas anormais, das quais 40 (4,38%) com Hb Bart's, sugerindo alfa talassemia, 34 (3,72%) com Hb S, 23 (2,52%) com beta talassemia, duas (0,22%) com Hb C e uma (0,11%) amostra apresentou Hb rápida. A frequência elevada de alterações encontradas evidencia a necessidade da triagem neonatal de hemoglobinopatias que pode resultar em amplos benefícios para os portadores destas patologias e seus familiares.

Hemoglobinopathies are the most frequent genetic diseases in the human population. About 12 to 15% of the population have one or more forms of abnormal hemoglobins, which creates a huge public health problem. The neonatal diagnosis permits early treatment and genetic counselling also makes carriers aware of the risk of homozygote children. The purpose of this study is to present the results of the investigation of hemoglobinopathies in newborn babies in the Hospital de Base, São José do Rio Preto. The study was performed on 913 blood samples taken from umbilical cords, which were submitted to specific electrophoresis, biochemical and cytologic tests for hemoglobin analysis. The results showed one hundred samples (10.95%) had abnormal hemoglobins of which 40 (4.38%) were Hb Bart's suggesting alpha thalassaemia, 34 (3.72%) were Hb S, 23 (2.52%) were Beta thalassaemia, two (0.22%) were Hb C and one (0.11%) was fast hemoglobin. The high rate of abnormal hemoglobins shows the necessity of neonatal screening which can result in great benefits for carriers of these diseases and their relatives.

A análise das alterações cromossômicas em leucemias tem uma aplicação direta no diagnóstico, prognóstico e tratamento dos pacientes. Além disso, permite o entendimento dos processos biológicos envolvidos na carcinogênese. Este trabalho apresenta os resultados do estudo cariotípico de 51 casos de diferentes tipos de leucemias. Os cromossomos foram obtidos através de cultura de células de sangue periférico, realizadas por 24 ou 48 horas, sem estimulação mitogênica. Em 74% dos pacientes foram observadas anomalias cromossômicas clonais como translocações, deleções, monossomias e trissomias. Muitas alterações foram compatíveis com outras previamente descritas e outras não, como a translocação envolvendo os cromossomos 9 e 22, que origina o cromossomo Philadelphia e uma translocação complexa envolvendo os cromossomos 4, 7 e 11. Os resultados reforçam a importância da análise cromossômica em leucemia e seus benefícios para o paciente.

The analysis of chromosomal changes in leukemia has had a direct involvement in the diagnosis, prognosis and treatment of patients. Furthermore, it has allowed the understanding of biological processes involved in carcinogenesis. This work presents the results of a kariotypic study of 51 cases of different types of leukemia. The chromosomes were obtained through a 24 or 48 hour culture of cells of the peripheral blood with no mitogenic stimulation. In 74,5% of patients clonal chromosomic abnormalities were observed such as translocations, deletions, monosomies and trisomies. Many of the alterations were similar to results previously published and others were not such as the translocation involving the chromosomes 9 and 22, which leads to the Philadelphia chromosome and the complex translocation involving the chromosomes 4, 7 and 11. The results emphasized the importance of chromosomal analysis in leukemia, its benefits to the patient and to the knowledge of the biological mechanisms involved in this pathology.
