ABSTRACT This study was conducted to evaluate the effect of vitamin E and selenium on performance, viability, productive efficiency, and yields of carcass, major cuts, and organs of broilers from 22 to 42 days submitted to cyclic-heat stress. The experimental design was randomized blocks, in a 2 × 3 factorial arrangement with two levels of selenium (0.1 and 0.3 mg/kg) and three levels of vitamin E (300, 400, and 500 mg/kg), plus a control treatment. Animals were submitted to a natural condition of high cyclic temperature. Organic selenium levels of 0.1 and 0.3 mg/kg associated with 300, 400, and 500 mg/kg of vitamin E were tested. The level of vitamin E did not affect the performance or production efficiency of broilers in the period from 22 to 33 days and 22 to 42 days. However, the selenium inclusion level of 0.3 mg/kg improved the viability in both phases. The yields of carcass, major cuts, intestine, and heart were not influenced by the levels of selenium and vitamin E, whereas abdominal fat for the selenium level 0.1 mg/kg decreased linearly with the inclusion in vitamin E.

Este estudo teve como objetivo avaliar a associação entre hiperlipidemia e dieta de nipo-brasileiros de Bauru, município do Estado de São Paulo. Foram obtidos de 1.330 indivíduos dados mediante questionários previamente testados (dados demográficos e dietéticos). Os exames físicos e laboratoriais incluíram a coleta (ou dosagem) antropométrica, de pressão arterial, de glicemia de jejum e de 2 horas, de perfil lipídico e de TSH e T4 livre. Utilizaram-se teste qui-quadrado e medida de odds ratio para avaliar associação entre as variáveis estudadas e presença de hiperlipidemia. Foi encontrado 81,5% de hiperlipidêmicos e acometeu, principalmente, indivíduos tabagistas, com excesso de peso, hipotireoidismo, hipertensão arterial e intolerância a glicose. Observaram-se, mediante análise bruta, relações entre hiperlipidemia e ácido graxo saturado, ácido oléico e fibras de grãos e cereais. Após o ajuste para as variáveis de controle, observaram-se associações entre hiperlipidemia e consumo de lípides (totais, ácido oléico, saturados e trans), fibras e álcool. Conclui-se que mudanças no estilo de vida, particularmente no hábito alimentar, podem levar a uma melhora do quadro hiperlipidêmico e que o consumo de lípides pode ser um dos principais fatores para o aumento da hiperlipidemia. Estudos prospectivos auxiliarão para testar essas hipóteses nos nipo-brasileiros de Bauru.

This study aimed at evaluating the association between hyperlipidemia and dietary patterns of Japanese Brazilians with and without hypothyroidism from Bauru, city in the State of São Paulo. We evaluated 1,330 individuals by means of demographic and dietary measurement, whom were gotten through standardized questionnaires previously tested. Clinical examination and laboratory data were anthropometry, blood pressure, fasting and 2-h glucose load, lipid profile and TSH and free T4. The chi-square and the odds ratio were used to evaluate associations between hyperlipidemia with studied variables. The prevalence of hyperlipidemia was 81.5% and it was associated with smokers, overweight, hypothyroid, hypertensive and glucose intolerants. We observed, in crude analysis, relationships with the presence of hyperlipidemia and fat saturated, oleic acid and dietary fiber from grains and cereals. After adjusting for the control variables, we observed relationships between hyperlipidemia (hypercholesterolemia and hypertriglyceridemia) with habitual intake of total fat, oleic acid, saturated fat, trans fat, dietary fiber and alcohol. As a conclusion, changes in the style of life, particularly in the dietary habits, can improve lipidic profile and that lipids intake can be a risk factor for hyperlipidemia. Prospectives studies will help test the hypothesis in Japanese Brazilians from Bauru.

A doença cardiovascular (DCV), incluindo a doença arterial coronariana (DAC), acidente vascular cerebral (AVC) e doença arterial periférica (DAP), é importante causa de morte em populações, especialmente na diabética. Indivíduos diabéticos apresentam risco aumentado de 3 a 4 vezes de sofrer evento cardiovascular e o dobro do risco de morrer deste evento quando comparados à população geral. Tem havido declínio na mortalidade por DCV, porém a queda nas mortes por DAC em portadores de diabetes tem sido bastante inferior à de não-diabéticos. Vários fatores presentes no diabetes favorecem a maior ocorrência de DCV, como a hiperglicemia, a resistência à insulina, além de fatores de risco clássicos e não-clássicos (hipertensão arterial sistêmica, dislipidemia, obesidade, estado inflamatório subclínico e outros). É possível que o potencial aterogênico da obesidade decorra em parte da produção aumentada de citocinas pelos adipócitos. Devido à marcante associação entre diabetes e DCV, e prognóstico desfavorável após um evento, é importante identificar quais são os indivíduos de mais alto risco e como rastreá-los. A American Heart Association e a American Diabetes Association recomendam estratificação do risco de pacientes sintomáticos por testes diagnósticos. O desafio está em identificar pacientes diabéticos assintomáticos que se beneficiariam de testes diagnósticos para detecção precoce de DCV, visando viabilizar medidas preventivas ou terapêuticas, capazes de reduzir morbi-mortalidade. O benefício do controle glicêmico e dos demais fatores de risco na prevenção de eventos CV no diabetes já documentado, justifica estabelecer estratégias que otimizem a identificação e possibilitem intervenções nos pacientes de alto risco, buscando reduzir mortalidade.

Cardiovascular disease, which includes coronary heart disease (CHD), cerebrovascular disease (CVD), and peripheral vascular disease (PVD), is the leading cause of mortality in populations, particularly in the diabetic one. Individuals with diabetes have at least a two-fold to four-fold increased risk of having cardiovascular events and a double risk of death compared with age-matched subjects without diabetes. A decline in mortality from CVD has been shown, but decline due to CHD is consistently lower in individuals with diabetes when compared with non-diabetics. The presence of several factors in diabetes leads to high occurrence of CVD such as hyperglicemia, insulin resistance, and classical and non-classical risk factors (systemic hypertension, dyslipidemia, obesity, proinflammatory condition and others). It is possible that the atherogenic role of obesity may be at least in part due to increased adipocyte production of cytokines. Considering the marked association of diabetes and CVD and unfavorable prognosis following an event, it is important to identify who is at high risk and how to screen. The American Heart Association and American Diabetes Association recommend risk stratification using diagnostic tests. However, the challenge is to accurately identify patients without a prior history of an event and those without symptoms strongly suggesting CVD, in whom additional testing would be indicated in order to achieve the most effective prevention. The benefits of glycemic control and the other risk factors have already been shown and justify optimization of the management of this high-risk population, aiming to reduce cardiovascular mortality disease and improve quality of life.

Doenças associadas ao sedentarismo podem ser prevenidas por mudanças no estilo de vida. Parte dos benefícios cardiovasculares da atividade física poderia advir de menor grau de inflamação. Este estudo descreve a atividade física de uma população de origem japonesa e analisa sua associação à síndrome metabólica (SM), ajustada para diversas variáveis. Baseou-se em banco de dados previamente constituído a partir de um estudo de base populacional em nipo-brasileiros. Foram incluídos 1330 indivíduos > 30 anos, de ambos os sexos, residentes em Bauru, submetidos a entrevistas, sendo obtidos dados sócio-demográficos, de saúde, de atividade física e de dieta, além de exames médicos e laboratoriais. A avaliação da atividade física enfocou atividades no trabalho e nas horas vagas. O diagnóstico de SM foi baseado em adaptação para asiáticos dos critérios do NCEP. Empregou-se regressão logística, tendo a SM como variável resposta. Homens (46,1%) e mulheres apresentaram médias de idade semelhantes (57,0 ± 12,8 e 56,9 ± 12,2 anos, respectivamente). Houve leve predomínio do sexo feminino. Os homens apresentavam grau de instrução mais elevado e mais freqüentemente eram fumantes (p< 0,001); seus valores médios de IMC, cintura e de pressão arterial (p< 0,001) foram superiores aos das mulheres. Para ambos os sexos, a maioria referia praticar atividades de intensidade leve ou moderada nas horas vagas (81,2% dos homens e 86,6% das mulheres). Quanto ao esforço do trabalho, 87,8% dos homens classificaram seu esforço como leve ou moderado, contra 96,1% das mulheres. A distribuição dos níveis de AF invariavelmente revelou que as mulheres eram mais inativas (p= 0,01). Estratificando-se pela presença da SM, as mulheres e homens com SM eram significantemente mais velhos e apresentavam maiores valores antropométricos. Considerando a duração da caminhada ao trabalho, notou-se tendência a tempo menor entre aqueles com SM (p< 0,078). Conforme esperado, indivíduos com SM apresentaram níveis significantemente maiores de pressão arterial, glicemia, lípides e HOMA-IR quando comparados aos sem SM. O HDL foi menor no grupo com SM, sendo significante no sexo feminino. Os valores médios da PCR foram maiores nos indivíduos com SM. Na regressão logística, a idade, IMC, HOMA-IR e PCR se associaram independentemente à SM, o que não ocorreu com os parâmetros usados para mensurar AF. Nossos achados não permitem afirmar que a inatividade física associa-se à presença de SM numa população nipo-brasileira. A freqüência bastante alta de inatividade física deve ter contribuído para os achados negativos quanto a efeitos protetores da atividade física. O achado de associação da SM à PCR sérica é favorável à hipótese de que um estado inflamatório sub-clínico participe desta síndrome.

Sedentary behavior-related diseases can be prevented by lifestyle changes. Part of the cardiovascular benefits of physical activity (PA) may be due to low-grade inflammation. This study describes the PA of a population of Japanese and analyses its association with metabolic syndrome (MS) adjusted a number of variables. This was based on a database previously created following a population-based study of Japanese-Brazilians. 1,330 subjects aged > 30 years, of both sexes, living in Bauru, were included and they were submitted to interviews, being obtained sociodemographic, health, physical activity and dietary data, as well as clinical and laboratory data. Physical activity assessment focused on activities during work and leisure times. Diagnosis of MS was based on an adaptation of NCEP criteria for Asians. Non-conditional logistic regression had MS as the dependent variable. Men (46.1%) and women showed similar mean ages (57.0 ± 12.8 and 56.9 ± 12.2 years, respectively). A slight preponderance of females was observed. Men had a higher level of education and more frequently were smokers (p< 0.001); their mean values of BMI, waist and blood pressure (p< 0.001) were higher than the women’s. For both sexes, the majority referred light and moderate activities (81.2% of men and 86.6% of women). As far as work time is concerned, 87.8% of men classified their effort as light or moderate versus 96.1% of women. Distribution by PA levels showed that women were always less active than men (p= 0.01). Stratifying by the presence of MS, men and women with MS were significantly older and showed greater anthropometric values. Considering the walking duration for work, there was a tendency of shorter periods among those with MS (p< 0.078). As expected, subjects with MS showed significantly higher levels of blood pressure, plasma glucose, lipids and HOMA-IR when compared to those without MS. HDL levels were lower in the MS group, being significant for the female sex. The mean values of CRP were higher in subjects with MS. In logistic regression, age, BMI, HOMA-IR and CRP were shown to be independently associated with MS, but not parameters used to measure physical activity. Our findings do not allow to state that physical inactivity is associated with MS in a Japanese-Brazilian population. High frequencies of physical inactivity should have contributed to the negative findings concerning protective effects of physical activity. The association of MS and serum CRP favors the hypothesis that a low-grade inflammatory state may participate in this syndrome.

A dislipidemia da síndrome metabólica (SM) confere elevado risco cardiovascular e caracteriza-se por aumento dos triglicérides, diminuição da HDL e alterações qualitativas da LDL, tornando-a mais aterogênica, como a LDL pequena e densa. LDLs modificadas (LDLm) foram detectadas in vivo no plasma e em placas ateroscleróticas. Uma pequena porcentagem do total de LDLs plasmáticas apresenta maior carga negativa na superfície [LDL(-)], sendo uma sub-população heterogênea de partículas com maior poder de agressão ao endotélio. Origina-se da oxidação, glicação ou outros processos que alteram sua composição química, estando aumentada em indivíduos diabéticos, hipercolesterolêmicos e naqueles com doença isquêmica cardíaca. A LDLm, ao ser fagocitada pelo receptor scavenger do macrófago, transforma-o numa célula espumosa e inicia uma reação imune-inflamatória. A participação da LDLm no processo aterogênico continua até a ruptura da placa e trombogênese, quando ela induz apoptose em células endoteliais e musculares lisas, aumenta a produção de metaloproteinases que digerem a matriz, fragilizando a cápsula, e exacerba a inflamação que concorre para o desenvolvimento do trombo. O aprimoramento dos ensaios laboratoriais para a LDLm permitirá maior aplicabilidade clínica, melhorando o poder preditivo de eventos cardiovasculares em relação ao perfil lipídico convencional e demais fatores de risco presentes na SM.

The dyslipidemia of the metabolic syndrome (MS) confers an elevated cardiovascular risk and is characterized by increased concentrations of triglycerides, decreased HDL-cholesterol and qualitative alterations in LDL which renders it more atherogenic, like the small dense LDL. Modified forms of LDL (mLDL) have been detected in vivo in the plasma and atherosclerotic plaques. A minor fraction of the total LDL has an electronegative charge and is represented by a heterogenic subpopulation of particles [LDL(-)], with higher potential to induce endothelial injury. It could be derived from oxidation, glication or other processes that alter its chemical composition and is increased in diabetic, hypercholesterolemic subjects, and in those with established coronary artery disease. mLDL are internalized by macrophages through scavenger receptors, originating foam cells and inducing an immune-inflammatory reaction. In the atherosclerotic process, the action of mLDL continues until plaque rupture and thrombogenesis, when it promotes apoptosis in endothelial and smooth muscle cells, and activates matrix metalloproteinases, weaken the fibrous cap, and further enhance the inflammatory process that ends in the thrombus formation. Development of new laboratory methods is necessary to enhance the clinical applicability of mLDL and the predictive power of the conventional lipid profile and other cardiovascular risk factors of the MS.

A influência da alteração do consumo de ácidos graxos trans nos níveis de lipídios séricos foi avaliada em 328 nipo-brasileiros de primeira (issei) e segunda geração (nisei), entre 40 e 79 anos, em 1993, participantes de dois inquéritos de saúde e nutrição realizados em 1993 e 2000. A relação entre a diferença do consumo de ácidos graxos trans e a diferença entre os níveis de lipídios séricos entre os dois inquéritos foi analisada em modelos de regressão linear múltiplos. Em ambos os gêneros, observou-se redução estatisticamente significante no consumo total de gordura trans, acompanhado de redução dos níveis séricos de LDL colesterol e aumento do HDL colesterol em sete anos de seguimento. Os valores médios de ingestão de gordura trans (percentual das calorias totais) em 1993 e 2000 foram: 5,1% e 3,4% para mulheres e 4,7% e 3,3% para homens, respectivamente. Apesar da ausência de associação entre as alterações do consumo de ácidos graxos trans e alterações no perfil de lipídios séricos, o nível de ingestão de gordura trans foi acima do recomendado pela OMS (até 1% das calorias totais).

A total of 328 Japanese-Brazilians of the first (Issei) and second (Nisei) generations, aged 40-79 years in 1993, were assessed in two cross-sectional surveys on health and nutritional status in 1993 and 2000. Multiple linear regression models were used with changes (2000/ 1993) in serum lipoproteins as the dependent variable and changes in dietary trans fatty acids (adjusted for total calories) as independent variable. In both genders a significant reduction was observed in total intake of trans fatty acids with lower LDL and increased HDL serum levels during 7-year follow-up. The mean intakes of trans fatty acids (% of total energy) in 1993 and 2000 were: 5.1% and 3.4% for women and 4.7% and 3.3% for men, respectively. Although the association between changes in dietary trans fatty acids and serum lipoprotein was not statistically significant, on average the intake of this trans fat was higher than the WHO recommendation (up to 1% of total energy).

A crescente prevalência de diabetes mellitus (DM) em países em desenvolvimento, associada à facilidade de se identificar indivíduos de alto risco para a doença, tornam interessante a idéia de se introduzir medidas que visem à prevenção da doença. Vários estudos comprovam os benefícios de mudanças no estilo de vida em prevenir ou retardar a progressão da tolerância à glicose diminuída para o DM. A revisão destes estudos mostra que é possível reduzir em 50 a 60% a incidência de DM intervindo-se no estilo de vida destes indivíduos, resultado superior ao obtido naqueles que, até o momento, utilizaram medidas farmacológicas com o mesmo objetivo. Apesar da eficácia destas simples medidas de prevenção por meio de mudanças no estilo de vida, estas são de difícil aderência e implementação em comunidades. Em nosso meio, são necessários estudos para avaliar as dificuldades de se implantar um programa nacional de prevenção de DM e outras doenças relacionadas ao estilo de vida em brasileiros de alto risco.

Considering the increasing prevalence of diabetes mellitus (DM) in underdeveloped countries as well as the simplicity of identifying individuals at high risk for such disease, implementation of intervention measures for its prevention is of great interest. Several studies have confirmed the benefits of lifestyle changes in preventing or postponig the progression from impaired glucose tolerance to DM. The review of these studies showed a 50% to 60% reduction in the incidence of DM by means of lifestyle modifications. Such results are better than those reported in studies in which pharmacological interventions were used with the same purpose. Despite the efficacy of lifestyle changes for the prevention of DM, compliance may represent a limitation to be implemented in communities. In our country, studies are necessary to assess the barriers for the implementation of a population-based program for the prevention of DM and other lifestyle related diseases in high-risk Brazilian subjects.

A hiperglicemia pós-prandial (HPP) decorre da diminuição da primeira fase de secreção insulínica e não tanto da resistência à insulina. Embora marcador da glicemia pós-prandial (GPP), a glicemia de 2h (G2h) pós-sobrecarga oral de glicose tem baixa reprodutibilidade. A HPP é importante fator de risco cardiovascular (CV), particularmente no DM, por ser a primeira causa de morte. Em não-diabéticos, tanto a glicemia de jejum (GJ) como a G2h elevam tal risco, que segue um continuum mesmo dentro da faixa normal (disglicemia). Lesões ateroscleróticas são mais acentuadas nos distúrbios do metabolismo glicídico, existindo associação de dano arterial com G2h. Em populações com GJ normal, à medida que se eleva a G2h, também aumenta o risco de morte. Glicação não-enzimática de componentes da parede arterial e oxidação de lipoproteínas aceleram o processo aterosclerótico. Associa-se a HPP a distúrbio de condução, facilitador da ocorrência de morte súbita. Disfunção endotelial também predispõe à vasoconstrição e isquemia miocárdica pós-refeição. A ADA recomenda que a GPP seja monitorada, especialmente quando GJ e A1c não forem proporcionais. Inibidores de alfa-glicosidase, glinidas e análogos de insulina de ação ultra-rápida são eficazes em reduzir a GPP, restando saber se diminuem complicações crônicas diabéticas.

Post-prandial hyperglycemia (PPH) is mainly attributed to a reduced first phase insulin secretion and lesser to insulin resistance. Although a marker of post-prandial glycemia (PPG), 2-hour post glucose load glycemic level (2hG) has low reproducibility. PPH is an important cardiovascular (CV) risk factor, particularly in DM in which CV diseases are the main cause of death. In non-diabetics, fasting and/or 2hG increase such risk, which occurs in a continuum within the normal range (dysglycemia). Atherosclerotic lesions are more pronounced in glucose metabolism disturbances; an association of arterial injury and 2hG was verified. In populations with normal fasting glycemia the risk of death increases along with 2hG. Non-enzymatic glycation of arterial wall components and lipoprotein oxidation accelerate atherosclerotic process. PPH is associated with conduction disturbances, which facilitate the occurrence of sudden death. Endothelial dysfunction could also predispose to vasoconstriction and post-prandial myocardial ischemia. ADA recommends that PPG should be monitored, especially when fasting glycemia and A1c are not proportional. While the efficacy of alpha-glucosidase inhibitors, glynides and ultra-short action insulin analogues are proved to reduce PPG it is still necessary to ascertain whether they are able to reduce long-term diabetic complications.

O objetivo deste estudo é identificar os principais preditores para o desenvolvimento do diabetes mellitus tipo 2 (DM2), em população com alta prevalência de DM, ou seja, a nipo-brasileira. Foram avaliados 314 indivíduos de origem japonesa em Bauru, SP, com 40 ou mais anos de idade, sem miscigenação, em 1993 e 2000. Os parâmetros avaliados, obtidos em 1993, foram sexo, idade, índice de massa corpórea (IMC), circunferência da cintura e do quadril, glicemia, insulina e pró-insulina de jejum e de 2hs após sobrecarga de glicose, HOMA (função das células b e resistência insulínica), perfil lipídico e pressão arterial. A homeostase glicêmica, avaliada em 2000, mostrou que os indivíduos com glicemia de jejum alterada ou tolerância à glicose diminuída apresentavam maiores chances de progressão ao DM2 (60 e 70%, respectivamente) do que os normais (19%). As principais variáveis envolvidas nessa progressão foram IMC, circunferência da cintura (apenas mulheres), pressão arterial sistólica, glicemia de jejum e de 2h e VLDL-colesterol. Esses resultados evidenciam o potencial existente para a redução da incidência de DM2 nessa população, pois esses fatores são passíveis de intervenção apenas com mudanças no estilo de vida.

In order to identify the main predictors for the development of type 2 diabetes mellitus (DM2) in a population with high prevalence of DM - the Japanese-Brazilians -, we evaluated 314 individuals with Japanese ancestry from Bauru, SP, aged 40 years or more, without miscegenation, in 1993 and 2000. The parameters evaluated, obtained in 1993, were sex, age, body mass index (BMI), waist and hip circumferences, fasting and 2h after 75g of glucose load glycemia, insulinemia and proinsulinemia, HOMA (function of the b cell and insulin resistance), lipid profile and blood pressure. The glycemic homeostasis evaluated in 2000 showed that individuals with impaired fasting glycemia or impaired glucose tolerance presented a higher risk for the progression to DM2 (60 and 70%, respectively) than normals (19%). The main variables involved in the progression to DM were BMI, waist circumference (only women), systolic blood pressure, fasting and 2h glycemia, and VLDL-cholesterol. These results showed the existing potential for the reduction in the incidence of DM2 in this population, because the main risk factors can be modified by interventions in lifestyle.

OBJETIVOS: Indivíduos obesos são mais predispostos à ocorrência de eventos cardiovasculares que indivíduos com peso normal. Para se avaliar o impacto da obesidade e da distribuição de gordura corporal sobre o risco cardiovascular, avaliamos uma população de indivíduos com sobrepeso ou obesidade. MÉTODOS: Foram feitas medidas do índice de massa corporal (IMC), da relação entre as medidas da cintura e do quadril (RCQ), da pressão arterial sistólica (PAS) e diastólica (PAD) e dos níveis da glicemia de jejum, colesterol total e triglicérides. RESULTADOS: Altas prevalências de intolerância à glicose ou diabetes (21,8%), hipercolesterolenemia (49,1%), hipertrigliceridemia (21,3%) e hipertensão arterial (43,8%) foram observadas nesta população. A prevalência de hipertensão aumentou de 23% no grupo com sobrepeso (IMC 25-29,9 kg/m²) para 67,1% (p<0.05) em pacientes com obesidade grau 3 (IMC > 40kg/m²). Também a prevalência de hipertensão aumentou de 35,7% naqueles com RCQ entre 0,73 e 0,88 para 66,6% naqueles com RCQ >0,97 (p<0,05), independente do IMC, e os valores da PAS se correlacionaram com as medidas da circunferência da cintura (r=0,35; p<0,0001). A PAS, entretanto, mostrou aumentos com o IMC apenas entre hipertensos, elevando-se de 150±12 mmHg naqueles com sobrepeso para 161±18mmHg naqueles com IMC > 40kg/m² (p<0,05). CONCLUSÃO: A obesidade favorece a ocorrência dos fatores de risco cardiovascular, sendo que a distribuição central da gordura corporal se destaca especialmente como fator importante no desenvolvimento da hipertensão arterial.

INTRODUCTION: Obese people are at higher cardiovascular risk than people with normal body weight. The objective of this study was to establish the relationship between obesity, body fat distribution and cardiovascular risk factors. METHODS: Body mass index (BMI), waist-hip ratio (WHR) systolic (SBP) and diastolic blood pressure (DBP), plasma cholesterol, triglycerides and glucose levels were determined in a population of 499 overweight and obese patients (432F/67M; age 39±12.9y). RESULTS: High prevalence of abnormal glucose tolerance or diabetes (21.8%), hypercholesterolenemia (49.1%), hypertri glyceridemia (21.3%) and hypertension (43.8%) were found in this population. The prevalence of hypertension increased from 23% in patients with BMI 25-29.9 kg/m² to 67.1% (p<0.05) in those with BMI > 40kg/m² and also from 35.7% in patients with WHR between 0.73 and 0.88 to 66.6% in those with WHR >0.97 (p<0.05). In addition, a correlation was found between the waist circumference and SBP (r=0.35; p<0.0001). In the hypertensive group, but not in the normotensive, SBP increased with BMI, from 150±12 mmHg in the overweight group to 161±18mmHg in that with BMI > 40kg/m², (p<0.05). CONCLUSION: Our data reinforce the association between obesity and high cardiovascular risk. In addition, our findings suggested a role for body fat distribution in the development of hypertension in obese patients.

Esforços são feitos para identificar fatores de risco (FR) cardiovascular (CV) e intervir em indivíduos com alto risco, visando reduzir a mortalidade CV. Os distúrbios agrupados como síndrome metabólica (SM), ligados pela resistência à insulina (RI), conferem um alto risco CV devido à combinação de intolerância à glicose, hipertensão, triglicérides elevados e HDL baixo, em adição a outros FR recentemente descritos. A hiperinsulinemia é um FR independente e a obesidade central é um FR maior, independente do IMC. A elevada atividade lipolítica da gordura visceral resulta em hiperprodução de ácidos graxos livres e suas consequências metabólicas, caracterizando o estado de RI. A associação de microalbuminúria com hipertensão, triglicerídeos e fibrinogênio, sugerem um papel na predição da doença CV e deve ser considerada um marcador de disfunção endotelial generalizada. Hipofibrinólise, devida a elevações do fibrinogênio e do PAI-1, induzidas pelo estado de RI, facilita a aterotrombose em pacientes com SM. O inibidor do ativador da trombina é, também, associado independentemente com marcadores de obesidade, hemoglobina glicada e RI. Hiper-homocistinemia está associada com efeitos deletérios nos vasos e parece resultar de dano endotelial, estado inflamatório crônico e comprometimento renal. A proteína C-reativa e a adiponectina - marcadores sensíveis de inflamação - também estão associados com RI. A endotelina-1 pode levar a distúrbios da SM e sua produção aumentada refletir dano endotelial causado pela RI. Em resumo, pacientes com SM têm o mais elevado risco de morte por eventos CV. Estudos clínicos intervencionistas, dirigidos aos componentes da SM e também para aumentar a sensibilidade à insulina, são necessários pra estabelecer o impacto prognóstico sobre a morbidade e mortalidade CV.

Efforts are being made to identify cardiovascular (CV) risk factors (RF) and intervene in high-risk subjects aiming to reduce CV mortality. Disorders grouped under the metabolic syndrome (MS), linked by insulin resistance (IR), confer high CV risk due to the cluster of glucose intolerance, hypertension, elevated triglycerides and low HDL-cholesterol levels in addition to several recently described RF. Hyperinsulinemia is considered an independent RF; central obesity is associated with major RF independently of BMI. High visceral fat lipolytic activity results in overproduction of free fatty acids and metabolic consequences, characterizing the IR state. Association of microalbuminuria with hypertension, triglyceride and fibrinogen levels suggested a role in predicting CV disease. It should be considered a marker of generalized endothelial dysfunction. Hypofibrinolysis due to fibrinogen and PAI-1 elevations, induced by the IR state, facilitates atherothrombosis in patients with MS. The thrombin activator fibrinolysis inhibitor is also independently associated with markers of obesity, glycated hemoglobin and IR. Hyper-homocystinemia is associated with deleterious vessel effects and seems to be result from endothelial damage, chronic inflammatory status and kidney impairment. C-reactive protein and adiponectin - sensitive markers of inflammation - are also associated with IR. Endothelin-1 can lead to MS disorders and increased production might reflect endothelial damage caused by IR. In summary, patients with MS are at the highest risk of dying from CV events. Interventional trials directed to components of MS and also to increase insulin sensitivity are needed to establish the prognostic impact in CV morbidity and mortality.

Avaliamos associações da microangiopatia a fatores de risco, a correlação entre nefropatia (ND) e retinopatia (RD) diabéticas, assim como a concordância entre os graus de acometimento destes territórios. 157 pacientes, submetidos a oftalmoscopia indireta, foram encaminhados ao Centro de Diabetes para pesquisa de ND, sendo obtidos dados sócio-demográficos e clínicos. Avaliou-se o controle glicêmico pela hemoglobina glicosilada e a presença de ND pela microalbuminúria. Atribuiu-se escores à RD e ND para análises de correlação e concordância. 103 pacientes (57,9±12,9 anos) completaram todas as etapas do estudo; 72% dos encaminhados apresentavam algum grau de RD, sendo que normais e retinopatas tinham características comparáveis. Foram subdivididos em 4 grupos segundo a presença ou ausência de cada complicação. Não houve diferença na distribuição quanto a sexo, raça e escolaridade. O tempo de DM foi maior naqueles com ND+RD quando comparados ao grupo sem RD, com ou sem ND (14,8±6,4 vs. 7,2±5,3 e 9,2±5,6 anos, p< 0,05, respectivamente). A freqüência de fumantes e dislipidemia auto-referida, o IMC, glicemia e hemoglobina glicosilada não diferiram entre os grupos. Pacientes com ambas as complicações tinham pressão arterial média maior que os grupos sem RD, com ou sem ND (147±23 vs. 128±20 e 118±18mmHg, p< 0,05, respectivamente). Indivíduos com ND, independente da presença de RD, apresentaram razão Albumina/Creatinina (A/C) mais alta que os grupos sem ND (p< 0,05); o grupo com apenas RD não diferiu do grupo normal. Os maiores níveis de A/C do grupo com ambas as complicações não diferiram estatisticamente dos nefropatas sem RD. 80% dos pacientes nefropatas apresentavam RD, enquanto 74% dos retinopatas tinham também ND (c2= 6,39; p< 0,05). Detectou-se correlação significante entre estas complicações (r= 0,47; p< 0,05), assim como a concordância entre seus graus, resultando em kappa de 0,154 (IC 95%: 0,031-0,276; p< 0,01). A hipertensão se associou aos casos de maior gravidade, caracterizados pela concomitância de RD e ND. A ocorrência de uma complicação microvascular na ausência de outra sugere a existência de fatores etiopatogênicos órgão-específicos. Nossos dados indicam correspondência entre os graus de lesão renal e retiniana na microangiopatia do DM2. Pacientes com ND se associam a maior risco de lesões retinianas, de modo que o encontro de A/C alterada em paciente diabético requer, ainda que na faixa microalbuminúrica, a investigação de acometimento retiniano.

Associations of microangiopathy with risk factors, correlation of diabetic nephropathy (DN) and retinopathy (DR) as well as the concordance between different degrees of these complications were evaluated. 157 patients submitted to indirect ophtalmoscopy were referred to the Diabetes Center for the screening of DN. A standardized questionnaire including socio-demographic and clinical aspects was applied. Glycemic control was assessed by glycated hemoglobin and DN by the presence of microalbuminuria. Scores were given to DR and DN to test correlation and concordance. 103 patients (57.9±12.9 yrs) completed all the steps of the study; 72% of the referred patients had any DR, who did not differ from those without DR concerning their main characteristics. Patients were stratified into 4 groups according to the presence or absence of each complication. No difference was observed concerning gender and skin color distributions and years in school among the groups. Duration of DM was longer in patients with DN+DR as compared to those without DR, with or without DN (14.8±6.4 vs. 7.2±5.3 and 9.2±5.6 yrs, p< 0.05, respectively). Frequencies of smokers and self-reported dyslipidemia, BMI, glycemia and glycated hemoglobin did not differ among the groups. Patients with both complications showed higher mean blood pressure than those without DR, with or without DN (147±23 vs. 128±20 and 118±18mmHg, p< 0.05, respectively). Patients with DN, independent of the presence of DR, had higher A/C than those without DN (p< 0.05); A/C of the normal group and that with only DR were not different. High A/C levels in patients with both complications did not differ from the DN patients without DR. 80% of DN patients showed DR, while 74% of DR patients also had DN (c2= 6.39, p< 0.05). Significant correlation was detected between the 2 complications (r= 0.47, p< 0.05), as well as the concordance between their degrees, whose kappa was 0.154 (95%CI, 0.031-0.276, p< 0.01). Hypertension was associated with severe cases with both complications. The occurrence of microvascular complication in the absence of the other may suggest organ-specific pathogenetic factors. Our data indicated concordance between degrees of renal or retinal lesions in the microangiopathy of DM2. Patients with DN had higher risk of retinal disease. Even in microalbuminuric phase, the finding of abnormal A/C in a diabetic patient requires screening for DR.

Insulina lispro é um análogo da insulina humana de ação e duração rápida, que mimeíiza o perfil fisiológico da insulina após uma refeição. Avaliamos a segurança e eficácia da insulina lispro em comparação com a insulina humana regular em um estudo multicêntrico, randomizado e cruzado em 27 diabéticos tipo 1 em uso de insulina humana NPH e regular (idade mediana = 16 anos). Após uso de insulina lispro ou regular por 2 meses, fez-se a transferência para a outra insulina por mais 2 meses mantendo-se a insulina NPH basal. Não houve diferença em relação à excursão prandial da glicemia da hemoglobina glicosilada A1C, comparando-se os 2 grupos (lispro e regular). O decréscimo percentual relativo da glicemia foi significantemente maior com insulina lispro no período do almoço, na primeira fase do estudo (p<0,02). O número total de episódios hipoglicêmicos não foi diferente, comparando os 2 grupos. Houve, porém, uma redução significante na incidência de hipoglicemia noturna e na madrugada com o uso inicial de lispro (p<0,05). Com o uso inicial de insulina regular, houve incremento na incidência de hipoglicemia noturna (p=0,038), com redução posterior na incidência da hipoglicemia com insulina lispro (p=0,04). Ao final do estudo, 68% dos pacientes referiram preferência e maior comodidade com insulina lispro em relação à insulina regular. A insulina lispro se mostrou uma opção segura e eficaz, com menor incidência de hipoglicemia noturna em diabéticos tipo 1. Uma otimização do regime de insulina basal é necessária para melhora do controle glicêmico, quando em uso de uma insulina de ação rápida.

Insulin lispro is a human insulin analog of rapid onset of action and duration, which mimics the physiological insulin profile after a meal. We have evaluated the safety and efficacy of lispro insulin in comparison with human regular insulin in a crossover, multicenter, randomized trial in 25 type 1 diabetic patients in use of human NPH and regular insulin (median age = 16 years). After administration of lispro or regular insulin for 2 months, the patients were transferred for the other insulin for two more months, maintaining the basal NPH insulin regimen. There was no difference in the postprandial glucose excursion and glycated hemoglobin A1c comparing the 2 groups (lispro and regular). The relative percentage decrease in glycemia was significantly greater with lispro insulin after lunch, in the first phase of the study (p<0.02). The total number of hypo-glycemic episodes was not different comparing both groups. However, there was a significant difference in the nocturnal hypoglycemia incidence with initial administration of lispro (p<0.05). With initial administration of regular insulin, there was an increase in the incidence of nocturnal hypoglycemia (p=0.038), with a subsequent reduction of hypoglycemia with insulin lispro (p=0.04). In the end of the study, 68% of the patients referred preference and better feeling with lispro, compared to regular insulin. Insulin lispro was a safe and efficacious option, with lower incidence of nocturnal hypoglycemia in type 1 diabetics. An optimization of the basal insulin regimen is necessary to improve glucose control with the use of rapid-action insulin.
