Abstract Objective: To assess factors associated with emotional changes and Hyperactivity/Inattention (HI) motivated by COVID-19 quarantine in adolescents with immunocompromising diseases. Methods: A cross-sectional study included 343 adolescents with immunocompromising diseases and 108 healthy adolescents. Online questionnaires were answered including socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and validated surveys: Strengths and Difficulties Questionnaire (SDQ), Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0). Results: The frequencies of abnormal emotional SDQ scores from adolescents with chronic diseases were similar to those of healthy subjects (110/343 [32%] vs. 38/108 [35%], p = 0.548), as well as abnormal hyperactivity/inattention SDQ scores (79/343 [23%] vs. 29/108 [27%], p = 0.417). Logistic regression analysis of independent variables associated with abnormal emotional scores from adolescents with chronic diseases showed: female sex (Odds Ratio [OR = 3.76]; 95% Confidence Interval (95% CI) 2.00–7.05; p < 0.001), poor sleep quality (OR = 2.05; 95% CI 1.08–3.88; p = 0.028) and intrafamilial violence during pandemic (OR = 2.17; 95% CI 1.12–4.19; p = 0.021) as independently associated with abnormal emotional scores, whereas total PedsQL score was inversely associated with abnormal emotional scores (OR = 0.95; 95% CI 0.93–0.96; p < 0.0001). Logistic regression analysis associated with abnormal HI scores from patients evidenced that total PedsQL score (OR = 0.97; 95% CI 0.95–0.99; p = 0.010], changes in medical appointments during the pandemic (OR = 0.39; 95% CI 0.19–0.79; p = 0.021), and reliable COVID-19 information (OR = 0.35; 95% CI 0.16–0.77; p = 0.026) remained inversely associated with abnormal HI scores. Conclusion: The present study showed emotional and HI disturbances in adolescents with chronic immunosuppressive diseases during the COVID-19 pandemic. It reinforces the need to promptly implement a longitudinal program to protect the mental health of adolescents with and without chronic illnesses during future pandemics.

Rhinella jimi toads (Stevaux, 2002) belong to the Bufonidae family, are endemic in the Brazilian Northeast and are commonly found during rainy periods. In general, amphibians of this family have in their poisons different metabolites that show a diversity of pharmacological activities. The isolation and identification of these compounds are of great importance, and techniques such as high-performance liquid chromatography coupled to mass spectrometry are widely used for the discovery of novel and known compounds in these poisons. For R. jimi poison, the ethyl acetate and methanolic extracts were obtained and thirty compounds were identified by combining ultra-performance liquid chromatography coupled to mass spectrometry (UPLC-MS) with direct infusion atmospheric pressure chemical ionization mass spectrometry (DI-APCI-MS/MS) and direct infusion electrospray mass spectrometry (DI-ESI-MS/MS) for each extract, respectively. Marinobufagin (2) and marinobufotoxin (19) were the majorities of each extract, respectively. In addition, other bufadienolides mainly present in the ethyl acetate extract, such other bufotoxins, alkaloids and arginine diacid derivatives were identified in the methanol extract. In a cytotoxic assay by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT), the extracts and compound 2 demonstrated half-maximal inhibitory concentration (IC50) values better than the positive control doxorubicin, evidencing excellent cytotoxic. This is the most complete study of the chemical composition of R. jimi toad poison and its respective cytotoxic activity, promoting the enrichment of knowledge about this family and species.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

OBJECTIVE: To assess the possible factors that influence sleep quality in adolescents with and without chronic immunosuppressive conditions quarantined during the coronavirus disease 2019 (COVID-19) pandemic. METHODS: This cross-sectional study included 305 adolescents with chronic immunocompromised conditions and 82 healthy adolescents. Online surveys were completed, which included questions on socio-demographic data and self-rated healthcare routine during COVID-19 quarantine and the following validated questionnaires: the Pittsburgh Sleep Quality Index (PSQI), Pediatric Quality of Life Inventory 4.0 (PedsQL4.0), and Pediatric Outcome Data Collection Instrument (PODCI). RESULTS: The median current age [14 (10-18) vs. 15 (10-18) years, p=0.847] and frequency of female sex (62% vs. 58%, p=0.571) were similar in adolescents with chronic conditions compared with healthy adolescents. The frequency of poor sleep quality was similar in both groups (38% vs. 48%, p=0.118). Logistic regression analysis, including both healthy adolescents and adolescents with chronic conditions (n=387), demonstrated that self-reported increase in screen time (odds ratio [OR] 3.0; 95% confidence interval [CI] 1.3-6.8; p=0.008) and intrafamilial violence report (OR 2.1; 95% CI 1.2-3.5; p=0.008) were independently associated with poor sleep quality in these adolescents. However, the PODCI global function score was associated with a lower OR for poor sleep quality (OR 0.97; 95% CI 0.94-0.99; p=0.001). Further logistic regression, including only adolescents with chronic conditions (n=305), demonstrated that self-reported increase in screen time (OR 3.1; 95% CI 1.4-6.8; p=0.006) and intrafamilial violence report (OR 2.0; 95% CI 1.2-3.4; p=0.011) remained independently associated with poor quality of sleep, whereas a lower PODCI global function score was associated with a lower OR for sleep quality (OR 0.96; 95% CI 0.94-0.98; p<0.001). CONCLUSION: Self-reported increases in screen time and intrafamilial violence report impacted sleep quality in both healthy adolescents and those with chronic conditions. Decreased health-related quality of life was observed in adolescents with poor sleep quality.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

Pollen morphology of some species of Spermacoceae s.s. (Rubiaceae) of the Atlantic Forest, Rio de Janeiro, Brazil

ABSTRACT This work aimed to morphologically characterize 22 species of Spermacoceae s.s found in the Atlantic Forest in the state of Rio de Janeiro with the purpose of improving palynological knowledge of the studied species. The studied species included six species of Borreria and Denscantia cymosa, Emmeorhiza umbellata, Hexasepalum apiculatum, H. radula, H. teres, Manettia fimbriata, Mitracarpus eichleri, M. frigidus, M. hirtus, M. lhotzkyanus, M. megapotamicus, Oldenlandia salzmannii, Pentodon pentandrus, Richardia brasiliensis, Richardia scabra and Spermacoce rubescens. Acetolysed pollen grains were analyzed using light (LM) and scanning electron miscroscopy (SEM). The pollen grains varied in size (polar diameter) from medium to large, with the exception of Hexasepalum teres which was very large. Pollen grains were oblate spheroidal in most species, suboblate in six species, prolate spheroidal in Spermacoce rubescens and subprolate in Manettia fimbriata. All pollen grains were colporate, while number of apertures varied (three-19). The endoaperture was lolongate in most species and lalongate in four species. Sexine ornamentation varied among reticulate, reticulate-granulate, microreticulate, granulate with perforation, echinate-perforate and vermiculate. Pollen attributes were found to distinguish the studied species, but do not separate genera, confirming that Spermacoceae is euripolynic.

We reported experimental and theoretical investigation of conformers of 1,2,3-triazole derivatives, substances of exclusively synthetic origin, subject of extensive studies, because of several biological properties, such as antiviral, antimicrobial and antileishmaniasis. We reported molecular/supramolecular X-ray structures of antiophidian compounds I and II. For I and II there are two crystallographic different molecules in the unit cell (A and B). To explore the causes of the similarities in the compound’s crystal structures, intermolecular interactions were explored using the Hirshfeld surface as the fingerprint plots. In addition, density functional theory (DFT) calculations were carried out at the ωB97x-D/6-31G(d,p)-PCM-CHCl3 level aiming to contribute to the interpretation of the experimental data and complement the experimental findings. Two structures named 2A and 5B were found in good agreement with the respective X-ray solid state ones (A and B). Theoretical 1H nuclear magnetic resonance (NMR) spectra calculated for 5B rotated structure (torsion angles deviation around 40° to 90°) was in fine agreement with experimental results (in CDCl3) indicating that the solution molecular structure is considerably different from optimized equilibrium geometries and solid-state structure. Therefore, care is needed when using X-ray structures or DFT geometries to model interaction of drugs with biological targets since significant conformational changes may take place in solution.

RESUMEN El uso de aceites esenciales de plantas há sido adoptado como menos peligroso para el medio ambiente y la salud humana que los insecticidas sintéticos utilizados para el control de insectos portadores de enfermedades. A pesar de ejercer actividades insecticidas contra varios insectos vectores de enfermedades, los impactos potenciales en los organismos no objetivo ejercidos por los aceites esenciales extraídos de Lippia sidoides (Cham.) no han recibido la atención adecuada. Evaluamos la susceptibilidad y los posibles cambios en las tasas de consumo de las abejas melíferas, Apis mellifera (L.), cuando se exponen a aceites esenciales extraídos de L. sidoides. Se expuso a las abejas recolectoras a jarabe de miel (50% v / v) que contenía aceite esencial de L. sidoides durante 5 h. Después de este período de exposición, las abejas recibieron jarabe de miel regular durante otro período de 19 h. Se utilizaron seis concentraciones de aceite essencial: 1.0, 1.5, 2.0, 2.5, 3.0 y 3.5 µL de aceite esencial / mL de jarabe, y se evaluó el consumo de jarabe y la mortalidad de las abejas en ambos períodos (5 y 24 h). Los resultados revelan que, independientemente de la concentración de aceite esencial, las abejas recolectoras se alimentaron significativamente menos de jarabe de miel que contenía aceite esencial de L. sidoides. Sin embargo, la alimentación con jarabe de miel que contiene aceites esenciales de L. sidoides no causó una mortalidad significativa en comparación con las abejas que no estuvieron expuestas a los aceites esenciales. Por lo tanto, los resultados demuestran que los aceites esenciales de L. sidoides exhibieron una selectividad adecuada contra las abejas melíferas.
ABSTRACT The use of plant essential oils has been adopted as less hazardous to the environment and human health than synthetic insecticides used for the control of insects that transmit diseases. Despite of exerting insecticidal activities against several insect disease vectors, the potential impacts on non-target organisms exerted by essential oils extracted from Lippia sidoides (Cham.) have not received adequate attention. Here, we evaluated the susceptibility and potential changes in consumption rates of honey bees, Apis mellifera (L.), when exposed to essential oils extracted from L. sidoides. Was exposed forager bees to honey syrup (50% v/v) containing L. sidoides essential oil for 5 h. After this exposure period, the bees received regular honey syrup for another 19 h period. Six essential oil concentrations was used, namely 1.0, 1.5, 2.0, 2.5, 3.0 and 3.5 µL of essential oil/mL of syrup, and evaluated the syrup consumption and bees mortality in both periods (at the 5th and 24th h). The results reveal that independent of the essential oil concentration, the forager bees fed significantly less on L. sidoides essential oil-containing honey syrup. However, feeding on L. sidoides essential oil-containing honey syrup did not cause significant mortality when compared with bees that were not exposed to the essential oils. Thus, the results demonstrate that L. sidoides essential oils exhibited adequate selectivity against honey bees.

Introdução: A dor articular é uma queixa comum em idade pediátrica. A sua investigação é fundamental para distinguir situações benignas daquelas que necessitam de intervenção médica urgente. Caso Clínico: Uma criança de nove anos, previamente saudável, foi conduzida ao Serviço de Urgência devido a um quadro de febre e dor no tornozelo e anca esquerdos associado a claudicação. A criança referiu febre, dor abdominal e tosse seca duas semanas antes da hospitalização. O estudo analítico demonstrou elevação da velocidade de sedimentação eritrocitária e da proteína C-reactiva. O estudo serológico foi positivo para infeção aguda por Mycoplasma pneumoniae e o estudo imagiológico possibilitou o diagnóstico de doença de Van Neck-Odelberg (VND). Discussão/Conclusão: A presença de dor articular geralmente representa um desafio diagnóstico, especialmente em crianças que se apresentam com febre e elevação de reagentes de fase aguda. A VND é uma causa rara de claudicação e neste caso correspondeu a um achado imagiológico. Foi estabelecido o diagnóstico final de artrite reativa secundária a infeção por M. pneumoniae.
Introduction: Joint pain and limping are common in pediatric age. Assessment of a child with joint pain requires differentiating benign conditions from those requiring urgent medical attention. Case report: A previously healthy nine-year-old boy was brought to the Emergency Department with fever and pain in the left ankle and hip associated with limping. He reported fever, abdominal pain, and slightly dry cough two weeks before admission. Blood analysis revealed an elevation in erythrocyte sedimentation rate and C-reactive protein serum concentration. Serology tests were positive for Mycoplasma pneumoniae acute infection and imaging studies enabled the diagnosis of Van Neck-Odelberg disease (VND). Discussion/Conclusion: Presence of joint pain often represents a diagnostic challenge, especially in children who present with fever and elevation of acute-phase reactants. VND is a rare cause of limping and in this case corresponded to an imaging finding. The final diagnosis was reactive arthritis secondary to M. pneumoniae infection.

Abstract Introduction: Primary hyperparathyroidism (PHPT) is the leading cause of outpatient hypercalcemia associated with increased cardiovascular risk. The flow-mediated vasodilation (FMV) has been proposed as a non-invasive method for assessing endothelial function. Objective: To compare the endothelial dysfunction measured by FMV and diastolic dysfunction in patients with PHPT before and after parathyroidectomy. Method: We performed a quasi-experimental (before-after) study to evaluate diastolic function and FMV in patients with PHPT before and six months after parathyroidectomy. Results: Fifteen patients completed study: 12 women and 3 men; 73% presented lithiasis, 27% osteoporosis and 53% metabolic syndrome; 73% presented diastolic dysfunction before the surgery and 60% after the surgery (p = 0.09). Post-isquemia brachial diameter improved from 41 mm (before surgery) to 46 mm (after surgery; p = 0.020). After surgery, we also observed an increase in the brachial diameter pre vs. post-ischemia from 41 to 46 mm (p = 0.005). Before surgery, the change in the delta of brachial diameter pre and post-ischemia was 1 mm and up to 4 mm after surgery (p = 0.03). Conclusions: There is a minor endothelial dysfunction measured by FMV in patients who underwent surgery for PHPT at 6 months after surgery, as well as a trend towards improvement in diastolic dysfunction. Echocardiography can be useful in the preoperative evaluation in patients with asymptomatic PHPT.
Resumen Introducción: El hiperparatiroidismo primario (HPTP) es la principal causa de hipercalcemia ambulatoria y se ha asociado con un riesgo cardiovascular elevado. La vasodilatación mediada por flujo (VMF) es un método no invasivo que evalúa la función endotelial. Objetivo: Comparar la disfunción endotelial mediante VMF y la disfunción diastólica en pacientes con HPTP antes y después de la paratiroidectomía. Método: Mediante un estudio cuasiexperimental (antes-después) se evaluaron la función diastólica y la VMF antes y 6 meses después de realizar una paratiroidectomía. Resultados: Se incluyeron 15 pacientes con HPTP: 12 mujeres (80%) y 3 hombres (20%); el 73% presentaron litiasis, el 27% osteoporosis y el 53% síndrome metabólico; el 73% tenían disfunción diastólica antes de la cirugía y el 60% después de la cirugía (p = 0.09). Los diámetros braquiales post-isquemia mejoraron de 41 mm antes de la cirugía a 46 mm tras la cirugía (p = 0.020). Así mismo, los diámetros braquiales preisquemia vs. post-isquemia después de la cirugía mejoraron de 41 a 46 mm (p = 0.005). Antes de la cirugía, el cambio en la delta del diámetro braquial preisquemia y post-isquemia fue de 1 mm, y subió a 4 mm después de la cirugía (p = 0.03). Conclusiones: Existe una menor disfunción endotelial medida por VMF en pacientes posoperados por HPTP a los 6 meses de la cirugía. El estudio ecocardiográfico puede ser de utilidad en la evaluación preoperatoria de los pacientes con HPTP asintomáticos.

In the present work, leaves of Pentacalia desiderabilis (Asteraceae) were subjected to extraction using aqueous 1-butyl-3-methylimidazolium bromide (BMImBr) in the microwave assisted extraction (MAE). The obtained extract was partitioned using CH2Cl2 and antifungal activity against Cladosporium cladosporioides and C. sphaerospermum of the organic phase was evaluated. Sequentially, this material was subjected to several chromatographic procedures to afford four quinoid derivatives identified as jacaranone (1), methyl-jacaranone (2), quinolacetic acid (3), and methyl 1-hydroxy-4-oxo-cyclohexaneacetate (4), being the first occurrence of compounds 2 - 4 in P. desiderabilis. Isolated quinoid derivatives were submitted to bioautography assay and the obtained results indicated that compounds 1 - 3 are active against Cladosporium cladosporioides (0.78, 12.5 and 12.5 µg, respectively) and C. sphaerospermum (3.13, 12.5 and 12.5 µg, respectively).

ABSTRACT Background: Hepatitis C is an important health problem. In Brazil, 1-2 million people are infected. Despite this expressive number, and the availability of very successful treatment, many patients remained undiagnosed mainly because of the asymptomatic nature of the infection. Objectives: To describe epidemiological characteristics of HCV-infected patients seen at referral centers in Brazil, the source of referral, and the time spanned to reach a reference center, in order to improve the identification of undiagnosed patients. Methods: Multicenter observational, cross-sectional study carried out in 15 centers of Brazil, between January/2016 and June/2017. Data of patients with a confirmed diagnosis (anti-HCV and HCV-RNA) were collected by interview using standard questionnaires and by review of charts. Results: Two thousand patients were included; 55.1% were male, mean age 58 ± 11 years. Only 14.9% had higher education and 84.2% received up to five monthly minimum Brazilian wages (approximately US$260.00/month). The time between diagnosis and beginning of follow-up was 22.9 months. The most common reasons for testing were check-up (33.2%) and blood donation (19%). General practitioners diagnosed most of the patients (30.1%). Fibrosis stage was mainly evaluated by liver biopsy (61.5%) and 31.3% of the patients were cirrhotic at diagnosis. Conclusions: This multicenter Brazilian study showed that the mean time to reach a referral center for treatment was almost two years. Primary care physicians diagnoses most hepatitis C cases in the country. Population campaigns and medical education should be encouraged to intensify screening of asymptomatic individuals, considering the efficiency of check-ups in identifying new patients.
