Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

Resumo Espécies de Peltula ocorrem em ambientes áridos e semiáridos e espécies novas têm sido constantemente descritas em todo o mundo. Neste mesmo sentido, a diversidade de liquens está sendo estudada na diversa, porém pouco conhecida, região centro-oeste do Brasil, e espécies e registros novos têm sido descobertos. Como parte do resultado, Peltula anthracina é proposta como nova para a ciência, e P. leptophylla e P. lingulata como novos registros para o estado de Mato Grosso do Sul. Como a região é altamente ameaçada pela expansão das atividades agrícolas, os autores destacam a importância da conservação da vegetação natural para preservar a diversidade oculta dos liquens.

Abstract Peltula species occur in arid and semi-arid environments, with new species being regularly described worldwide. Lichen diversity is being studied in the diverse but poorly known Midwest region of Brazil, and new species and records are being proposed. In this paper, Peltula anthracina is proposed as new to science, and P. leptophylla and P. lingulata are reported as new records from Mato Grosso do Sul state. As it is a region highly threatened by the expansion of agricultural activities, the authors highlight the importance of the conservation of native flora to preserve lichens’ hidden diversity.

Abstract Background: Triatomines are blood-feeding arthropods belonging to the subfamily Triatominae (Hemiptera; Reduviidae), capable of producing immunomodulatory and water-soluble molecules in their hemolymph, such as antimicrobial peptides (AMPs). In this work, we evaluated the antifungal and immunomodulatory activity of the hemolymph of Meccus pallidipennis (MPH) and Rhodnius prolixus (RPH) against Cryptococcus neoformans. Methods: We assessed the activity of the hemolymph of both insects on fungal growth by a minimum inhibitory concentration (MIC) assay. Further, RAW 264.7 macrophages were cultivated with hemolymph and challenged with C. neoformans. Then, their phagocytic and killing activities were assessed. The cytokines MCP-1, IFN-γ, TNF-α, IL-10, IL-12, and IL-6 were measured in culture supernatants 4- and 48-hours post-infection. Results: Both hemolymph samples directly affected the growth rate of the fungus in a dose-dependent manner. Either MPH or RPH was capable of inhibiting fungal growth by at least 70%, using the lowest dilution (1:20). Treatment of RAW 264.7 macrophages with hemolymph of both insects was capable of increasing the production of MCP-I and TNF-α. In addition, when these cells were stimulated with hemolymph in the presence of C. neoformans, a 2- and a 4-fold increase in phagocytic rate was observed with MPH and RPH, respectively, when compared to untreated cells. For the macrophage killing activity, MPH decreased in approximately 30% the number of viable yeasts inside the cells compared to untreated control; however, treatment with RPH could not reduce the total number of viable yeasts. MPH was also capable of increasing MHC-II expression on macrophages. Regarding the cytokine production, MCP-I and TNF-α, were increased in the supernatant of macrophages treated with both hemolymphs, 4 and 48 hours after stimulation. Conclusion: These results suggested that hemolymph of triatomines may represent a source of molecules capable of presenting antifungal and immunomodulatory activity in macrophages during fungal infection.

ABSTRACT The objective of this study was to evaluate the effect of harvesting corn at different maturity stages for silage production combined with four storage periods, considering nutrient loss and the chemical composition of silage of the whole plant and also of grainless plants. The experiment was performed in a completely randomized design with a factorial arrangement of six maturity stages of corn [reproductive (R); R1 to R6] and four periods of silage storage (30, 60, 90, and 120 days). At stages R1 and R2, losses by effluents occurred more intensely when compared with the more advanced stages, whereas losses by gases showed the opposite behavior, with stage R6 responsible for the greatest losses. Stage R4 showed a stable dry matter (DM) content during the storage period, 42.08%, and had the lowest levels of neutral detergent fiber (NDF), acid detergent fiber (ADF), and acid detergent lignin (ADL) for the whole plant silage. The ADL in the silage of grainless plant of stage R4 was not the lowest, but it was close to the first reproductive stages, being an important point, since the first reproductive stages can present a vegetative fraction of better quality. The highest DM volumes produced by area occurred in the most advanced stages; however, R5 obtained a greater reduction in volume over the days of storage (20.66 kg day −1 ). The production cost was lower at the advanced stages (R4, R5, and R6) when compared with the first reproductive stages (R1, R2, and R3), and in general, during the storage period the costs did not have great variations. Based on the chemical composition of the silages, DM losses during the storage period, the stability in the costs of DM ton over the days of storage, the R4 stage was the most suitable for silage production.

OBJECTIVE: In 2020, the COVID-19 pandemic brought a work and stress overload to healthcare workers, increasing their vulnerability to mental health impairments. In response, the authors created the COMVC-19 program. The program offered preventive actions and mental health treatment for the 22,000 workers of The Hospital das Clinicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). This paper aims to describe its implementation and share what we have learned from this experience. METHODS: Workers were able to easily access the program through a 24/7 hotline. Additionally, a mobile phone app that screened for signs and symptoms of emotional distress and offered psychoeducation and/or referral to treatment was made available. Data from both these sources as well as any subsequent psychiatric evaluations were collected. RESULTS: The first 20 weeks of our project revealed that most participants were female, and part of the nursing staff working directly with COVID-19 patients. The most frequently reported symptoms were: anxiety, depression and sleep disturbances. The most common diagnoses were Adjustment, Anxiety, and Mood disorders. CONCLUSIONS: Implementing a mental health program in a multimodal intervention was feasible in a major quaternary public hospital. Our data also suggests that preventive actions should primarily be aimed at anxiety and depression symptoms, with a particular focus on the nursing staff.

Biodiesel purification by water washing has been proved to be an unattractive process from an economic and environmental point of view. In this work, we propose the use of regenerated cellulose membranes from peanut shell as an alternative in the biodiesel purification. The preparation of the membranes was optimized from a central composite design, resulting in dense and non-porous membranes. However, in contact with biodiesel, the membranes undergo expansion due to the retention of methanol, water, and glycerol molecules, allowing the permeation of fatty acid methyl esters. The filtered biodiesel presents quality parameters, such as total ester content of 98.3 ± 1.4% and free glycerol content < 0.02%, in agreement with the main regulatory agencies. The regenerated cellulose membranes from peanut shell are easily prepared, with high repeatability (relative standard deviation (RSD) < 1.8%) and are presented as an economical and environmental solution for the biodiesel purification.

RESUMO A exploração da variabilidade genética existente para características fisiológicas relacionadas com a produtividade e a qualidade da forragem de milho pode auxiliar no desenvolvimento de linhagens e híbridos superiores. O objetivo deste trabalho foi avaliar a capacidade geral e específica de combinação e a natureza dos efeitos genicos de características fisiológicas e químicas de genótipos de milho forrageiro. Foram avaliados dois grupos de genótipos e 16 híbridos resultantes dos cruzamentos em esquema de dialelo parcial 4x4. O grupo I foi constituído por dois híbridos comerciais (AG8025, P30B39) e duas linhagens elite (LEM2 e LEM3). O grupo II foi constituído por quatro linhagens experimentais oriundas de diferentes populações do programa de melhoramento do milho forrageiro da UNICENTRO. No total foram avaliados 24 tratamentos, constituídos pelos genótipos de ambos os grupos e os respectivos cruzamentos. As características avaliadas foram assimilação de CO2 (A), condutância estomática (gs), concentração interna de CO2 (Ci), transpiração (E), atividade calculada da Rubisco (A/Ci) e eficiência de uso da água (A/E). Foram obtidos os teores de fibra em detergente ácido (FDA), fibra em detergente neutro (FDN) e a digestibilidade in situ da forragem. Houve predomínio de efeitos gênicos não aditivos para a maioria das características químicas e fisiológicas. Os cruzamentos LEM2 x 203-218,3, LEM3 x 201-107,2, LEM2 x 101-7,2 e LEM3 x 101-7,2 se destacaram quanto à assimilação de CO2, sendo indicados para pesquisas futuras considerando as características fisiológicas. As linhagens 101-7.2 e 203-218.3 apresentaram alta concentração de alelos favoráveis para aumentar a eficiência da carboxilação, em que a linhagem 101-7.2 se destacou para NDF, ADF e DIG. A linhagem endogâmica 201-107.2 possui alta concentração de alelos favoráveis para eficiência do uso da água. Parâmetros fisiológicos podem auxiliar na seleção de linhagens e híbridos no melhoramento do milho forrageiro.

ABSTRACT The exploitation of the existing genetic variability for the physiological traits related to the yield and quality of maize forage can assist in the development of superior inbred lines and hybrids. The objective of this work was to evaluate the general and the specific combining ability and the nature of gene effects of physiological and chemical traits of forage maize genotypes. Two groups of genotypes and 16 hybrids resulting from a 4x4 partial diallel scheme were evaluated. Group I consisted of two commercial hybrids (AG8025, P30B39) and two elite inbred lines (LEM2 and LEM3). Group II consisted of four experimental inbred lines originated from different populations of forage maize breeding program. In total, 24 treatments were evaluated, formed by the genotypes of both groups and the respective crossings. The traits evaluated were: CO2 assimilation (A), stomatal conductance (gs), internal CO2 concentration (Ci), transpiration (E), calculated activity of Rubisco (A/Ci) and efficiency of water use (A/E). Forage acid detergent fiber (ADF), neutral detergent fiber (NDF) and digestibility in situ were obtained. There was a predominance of non-additive gene effects for most of the chemical and physiological traits. Crossings LEM2 x 203-218.3, LEM3 x 201-107.2, LEM2 x 101-7.2 and LEM3 x 101-7.2 stood out regarding CO2 assimilation, and are indicated for future research considering the physiological traits. Inbred lines 101-7.2 and 203-218.3 presented high concentration of favorable alleles to increase carboxylation efficiency, in which inbred line 101-7.2 stood out for NDF, ADF and DIG. Inbred line 201-107.2 has a high concentration of favorable alleles for efficiency of water use. Physiological parameters can assist the selection of inbred lines and hybrids in maize breeding for forage purpose.

Resumo: Introdução: O ensino médico vem passando por grande reformulação nas últimas décadas, e um dos pontos considerados fundamentais nessa mudança é a atuação docente. Pesquisas indicam que os professores de Medicina usualmente possuem pouca formação didático-pedagógica, dividem-se entre as profissões de médico e professor, colocada em segundo plano, privilegiam a pesquisa em relação ao ensino e são resistentes a mudanças no seu modo de atuação. Segundo a literatura, essas características repercutiriam diretamente no ensino oferecido. Método: A presente pesquisa buscou analisar como características dos professores de Medicina de uma universidade no interior de Minas Gerais repercutem na forma como os alunos avaliam o ensino recebido. Para tanto, levantou-se o perfil dos professores a partir de documentação e entrevista semiestruturada. Posteriormente, os alunos avaliaram os professores participantes por meio de um questionário autoaplicado. Confrontaram-se avaliação e perfil, e utilizaram-se testes estatísticos para a análise da associação entre características e avaliação feita. Participaram 57 professores (83,8% dos elegíveis) e 203 alunos (84,5% dos elegíveis). Resultados: Foi identificado perfil de professores semelhante ao da literatura, com a maioria em dedicação parcial à universidade, renda proporcionada principalmente por outra profissão e preparação didática constituindo parte reduzida da formação. A comparação entre os grupos de características dos professores, com base nas avaliações dos alunos, por sua vez, mostrou resultados que rompem com a bibliografia. Os dados mostraram, com relevância estatística, que no curso professores mais jovens, com menos tempo de graduação, de atuação docente e de experiência em outra profissão, sem mestrado e que possuem como motivação para lecionar prazer e aptidão são mais bem avaliados. Maior tempo de preparação didática, cursos preparatórios, maior dedicação semanal ao ensino e maior identificação com a docência, entre outros aspectos, não se associam à melhor avaliação de forma estatisticamente significativa. Conclusões: A pior avaliação do docente mais experiente, associada a questões como conflitos geracionais e dificuldades de adaptação a novos currículos, mostra a importância de uma preparação docente contínua e reforçada com o tempo. Além disso, o melhor desempenho de professores mais satisfeitos demonstra a importância da valorização da carreira docente. Por fim, constata-se que quantidade de formação não é suficiente sem qualidade nessa formação.

Abstract: Introduction: Medical education has undergone a major overhaul over the last decades and one significant change is related to teaching performance. Research indicates that the medical faculty typically has little training in didactics, split their time between clinical practice and teaching, award less priority to the latter, privileging research to the detriment of teaching, and resist changing their ways. According to the literature, such traits and attitudes would directly affect educational quality. Method: This study assessed how traits and attitudes of the medical faculty at a university in Minas Gerais impact student assessment of the education they receive. For that purpose, teachers were profiled through semi-structured interviews and analysis of relevant documentation. Subsequently, students assessed their teachers’ performance through a self-applied questionnaire. Performance and profiles were then compared, and statistical tests were used to analyze the associations between teachers’ traits and performance assessment results. Overall, 57 faculty members (83.8% of those eligible) and 203 students (84.5% of those eligible) participated in this study. Results: A faculty profile similar to that found in the literature was identified, with a majority of teachers dedicating only part of their time to the university, and most of their income coming from another occupation and teacher training amounting to a small portion of their overall professional training. The analyses of relationships between groups of traits and teaching performance, however, contradicted the literature. The data showed, with statistical relevance, that younger teachers and those who graduated a shorter time ago, who are less experienced in teaching or have other occupations, who lack a master’s degree and whose motivation to teach comes from satisfaction and vocation are better evaluated. Longer didactical training, undergoing training courses, longer weekly dedication and greater identification with the teaching profession are not significantly associated with better evaluation. Conclusion: The worse evaluation of the most experienced faculty, associated with issues such as generational conflicts and difficulties adapting to new curricula, shows the importance of continuous faculty training and updating. Additionally, the better performance of more satisfied teachers, demonstrates the importance of valuing the teaching career and raising its status. Finally, it seems that more teacher training is not enough in itself and more attention must be paid to the training quality.

RESUMO Objetivo: Investigar se o índice de massa tri-ponderal e os escores-z do índice de massa corporal são equivalentes na triagem de crianças e adolescentes com resistência à insulina. Métodos: Estudo transversal com 296 crianças e adolescentes matriculados em escolas públicas de Vitória, Espírito Santo, Brasil, com idades entre oito e 14 anos. O índice de massa tri-ponderal foi calculado como a razão entre o peso e a altura ao cubo. O índice de massa corporal foi calculado como a razão entre o peso e a altura ao quadrado. A resistência à insulina foi definida pelo modelo de avaliação da homeostase para resistência à insulina (HOMA-IR). Resultados: O HOMA-IR foi maior no 4º quartil dos escores-z do índice de massa corporal e do índice de massa tri-ponderal, em relação ao 1º e 2º quartis em ambos os sexos. As áreas sob as curvas receiver operating characteristic ajustadas por idade foram semelhantes em meninas (escores-z do índice de massa corporal=0,756; índice de massa tri-ponderal=0,763) e meninos (escores-z do índice de massa corporal=0,831; índice de massa tri-ponderal=0,843). Além disso, de acordo com as estimativas das análises de regressão linear simples, os escores-z do índice de massa corporal e o índice de massa tri-ponderal explicaram uma fração significativa da variabilidade do modelo de avaliação da homeostase para meninas (escores-z do índice de massa corporal: R2=0,269; índice de massa tri-ponderal: R2=0,289; p<0,001) e meninos (escores-z do índice de massa corporal: R2=0,175; índice de massa tri-ponderal: R2=0,210; p<0,001). Conclusões: O índice de massa tri-ponderal e os escores-z do índice de massa corporal foram semelhantes na triagem de crianças e adolescentes com resistência à insulina. É importante ressaltar que o uso do índice de massa tri-ponderal é claramente vantajoso, pois pode ser calculado sem ajustes por idade, fato que o torna muito aplicável à prática clínica.

ABSTRACT Objective: To investigate whether tri-ponderal mass index and body mass index Z scores are equivalent for screening children and adolescents with insulin resistance. Methods: Cross-sectional study with 296 children and adolescents enrolled at public schools of Vitória, Espírito Santo, Brazil, aged eight to 14 years. The tri-ponderal mass index was calculated as the ratio between weight and height cubed. The body mass index was calculated as the ratio between weight and height squared. Insulin resistance was defined with the homeostatic model assessment (HOMA-IR). Results: The HOMA-IR was higher in the 4th quartile of body mass index Z scores and tri-ponderal mass index compared to 1st and 2nd quartiles for both girls and boys. The areas under the age-adjusted receiver operating characteristic curves were similar between the indices for girls (body mass index Z scores=0.756; tri-ponderal mass index=0.763) and boys (body mass index Z scores=0.831; tri-ponderal mass index=0.843). In addition, according to the simple linear regression analyses estimations, both body mass index Z scores and tri-ponderal mass index explained a significant fraction of the homeostatic model assessment variability for girls (body mass index Z scores: R2=0.269; tri-ponderal mass index: R2=0.289; p<0.001) and boys (body mass index Z scores: R2=0.175; tri-ponderal mass index: R2=0.210; p<0.001). Conclusions: The tri-ponderal mass index and body mass index Z scores were similar to discriminate children and adolescents with insulin resistance. It is noteworthy that the use of tri-ponderal mass index is clearly advantageous, because it can be calculated with no concerns on adjustments for the age, a fact that makes it very applicable in the clinical practice.

Resumo Objetivo: Considerando a importância de incorporar ferramentas simples e de baixo custo no cenário clínico-pediátrico para a triagem de resistência à insulina, o presente estudo buscou investigar se a razão cintura/estatura é comparável a marcadores bioquímicos na discriminação de resistência à insulina em crianças e adolescentes. Métodos: Este estudo transversal envolveu estudantes de nove escolas públicas. No total, 296 crianças e adolescentes, de ambos os sexos, com idades entre 8 e 14 anos, compuseram a amostra. A razão cintura/estatura, o índice triglicerídeos/glicose e a razão triglicerídeos/HDL-C foram determinados de acordo com protocolos padrão. A resistência à insulina foi definida por meio do modelo de avaliação homeostática para resistência insulínica, com ponto de corte ≥ 3.16. Resultados: Idade, índice de massa corporal, frequência de excesso de peso, circunferência da cintura, razão cintura/estatura, insulina, glicemia, modelo de avaliação homeostática para resistência insulínica, triglicerídeos, índice triglicerídeos/glicose e razão triglicerídeos/HDL-C foram maiores entre meninos e meninas com resistência à insulina. Também foram observadas, em ambos os sexos, correlações moderadas de todos os indicadores (razão cintura/estatura, índice triglicerídeos/glicose e razão triglicerídeos/HDL-C) com o modelo de avaliação homeostática para resistência à insulina. As áreas sob as curvas ROC foram semelhantes entre a razão cintura/estatura e os marcadores bioquímicos. Conclusão: Os indicadores forneceram poder discriminatório similar para a resistência à insulina. No entanto, levando em conta o custo-benefício, sugerimos que a razão cintura/estatura pode ser uma ferramenta útil para a triagem de resistência à insulina em populações pediátricas.

Abstract Objective: Given the importance of incorporating simple and low-cost tools into the pediatric clinical setting to provide screening for insulin resistance, the present study sought to investigate whether waist-to-height ratio is comparable to biochemical markers for the discrimination of insulin resistance in children and adolescents. Methods: This cross-sectional study involved students from nine public schools. In total, 296 children and adolescents of both sexes, aged 8 -14 years, composed the sample. Waist-to-height ratio, triglycerides/glucose index, and triglycerides-to-HDL-C ratio were determined according to standard protocols. Insulin resistance was defined as homeostatic model assessment for insulin resistance with cut-off point ≥ 3.16. Results: Age, body mass index, frequency of overweight, waist circumference, waist-to-height ratio, insulin, glucose, homeostatic model assessment for insulin resistance, triglycerides, triglycerides/glucose index, and triglycerides-to-HDL-C were higher among insulin resistant boys and girls. Moderate correlation of all indicators (waist-to-height ratio, triglycerides/glucose index, and triglycerides-to-HDL-C ratio) with homeostatic model assessment for insulin resistance was observed for both sexes. The areas under the receiver operational characteristic curves ware similar between waist-to-height ratio and biochemical markers. Conclusion: The indicators provided similar discriminatory power for insulin resistance. However, taking into account the cost-benefit ratio, we suggest that waist-to-height ratio may be a useful tool to provide screening for insulin resistance in pediatric populations.

Com o objetivo de identificar o tipo de herança, o número de genes e os parâmetros genéticos e fenotípicos da resistência do acesso PI 595201 a um isolado de PRSV-W da região Norte do Brasil foi conduzido um experimento na área experimental do Departamento de Produção Vegetal da Universidade Federal do Tocantins, em condições de casa de vegetação. A partir do cruzamento entre a cultivar suscetível Crimson Sweet (P1) e o acesso resistente PI 595201 (P2) foi obtida a geração F1 e as gerações segregantes, cujas plantas foram inoculadas e avaliadas quanto à resistência com base nos sintomas nas folhas. Foram avaliadas 48, 48, 80, 200, 100 e 100 plantas das gerações P1, P2, F1, F2, RC11 e RC12, respectivamente, em delineamento inteiramente casualizado com quatro repetições. A partir da análise de componentes de médias no controle da resistência determinaram-se dois genes, predominando os efeitos gênicos aditivos, seguido de efeitos de dominância incompleta no sentido de aumentar a resistência. As herdabilidades para a resistência, no sentido amplo e restrito, foram de 0,48 e 0,38, respectivamente.

Aiming to identify the type of inheritance, the number of genes and genetic and phenotypic parameters of PI 595201 access resistance to an isolate of PRSV-W in the North of Brazil, was conducted an experiment in the experimental area of the Department of Plant Production of the Universidade Federal do Tocantins, in a greenhouse. From the cross between the susceptible cultivar 'Crimson Sweet' (P1) and resistant access 'PI 595201' (P2) was obtained F1 and segregating generations, whose plants were inoculated and evaluated for resistance based on leaf symptoms. Was evaluated 48, 48, 80, 200, 100 and 100 plants of P1, P2, F1, F2, RC11 and RC12 generations, respectively, in a completely randomized design with four replications. From the analysis of medium components in the control of resistance were determined two genes, predominantly additive gene effects, followed by effects of incomplete dominance to increase the resistance. The heritability for resistance in broad and narrow sense were 0.48 and 0.38, respectively.

OBJETIVO: Analisar a relação entre as queixas clínicas mensuradas pelo "International Consultation on Incontinence Questionnaire-Short Form" (ICIQ-UISF) e os achados urodinâmicos em homens com incontinência urinária (IU) após a prostatectomia radical (PR). MÉTODOS: 88 homens que apresentavam IU por um período mínimo de 1 ano após a PR foram incluídos prospectivamente. Todos responderam o questinário "ICIQ-UISF" e foram submetidos a avaliação urodinâmica. Os pacientes foram categorizados em 3 grupos de acordo com o diagnóstico urodinâmico: Grupo 1, pacientes com incontinência esfincteriana isolada (IE); Grupo 2, pacientes com IU mista (IE + disfunção vesical (DV)); e Grupo 3, pacientes com DV isolada. Os dados foram analisados utilizando o software SPSS v16.0. RESULTADOS: Dos 88 pacientes avaliados, após a avaliação urodinâmica, 51 homens (57,9%) apresentaram IE isolada (Grupo 1); 30 homens (34%) apresentaram IE associada a DV (Grupo 2) e 7 homens (7,9%) tinham somente DV (Grupo 3). A DV foi encontrada em 37/88 pacientes (42%), mas foi a principal causa de IU em apenas 14 pacientes (15,9%). Não houve diferença estatisticamente significativa entre os valores das médias do "ICIQ-UISFs" entre os grupos 1, 2 ou 3 (p> 0,05). Os sintomas de incontinência de esforço se correlacionaram com o diagnóstico urodinâmico de IE (r = 0,59), e as queixas de urgência miccional se correlacionaram com a presença de hiperactividade do detrusor na avaliação urodinâmica (r = 0,37), entretanto apesar da correlação encontrada, os sintomas não foram capazes de identificar a principal causa da UI . CONCLUSÃO: A etiologia da UI após a PR não pode ser previsto pelo escore de sintomas obtidos através do "ICIQ-UISF". Os sintomas de perda urinária as manobras de estresse e de urgência miccional estão relacionados a presença de IE e hiperatividade detrusora na avaliação urodinâmica, entretanto estes sintomas não conseguem identificar com segurança qual é o principal fator da IU após a PR.O teste urodinâmico continua sendo o padrão ouro para avaliar a etiologia da IU após a PR.

PURPOSE: To analyze the correlation between the "International Consultation on Incontinence Questionnaire-Short Form" (ICIQ-UISF) survey and the urodynamic findings in men with urinary incontinence (UI) following radical prostatectomy (RP). METHODS: 88 men who presented post-RP UI for a minimum of 1 year were enrolled prospectively. All answered the ICIQ-UISF survey and underwent urodynamic testing. Patients were divided in 3 Groups according to their urodynamic diagnosis: Group 1, patients with sphincteric incontinence (SI) alone; Group 2, patients with mixed UI (SI + Bladder Dysfunction (BD)); and Group 3, patients with BD alone. Data were analyzed using SPSS v16.0 software. RESULTS: There were 51 men in Group 1 (57.9%); 30 in Group 2 (34%); and 7 (7.9%) in Group 3. BD was found in 37/88 patients (42%), but it was the main cause of UI in only 14 patients (15.9%). There was no statistically significant difference among the mean ICIQ-UISFs values from groups 1, 2, or 3 (p>0.05). The symptoms of stress incontinence correlated with the urodynamic finding of SI (r = 0.59), and complaints of urinary urgency correlated with the presence of detrusor overactivity (DO) (r = 0.37), but these complaints did not predict the main cause of UI. CONCLUSION: The etiology of UI following RP cannot be predicted by the ICIQ-UISF survey. Symptoms of stress and urge incontinence predict the findings of SI and DO on urodynamic tests, but they cannot ascertain the main cause of UI. Urodynamic testing remains the gold standard to assess the etiology of post-RP UI.

OBJETIVO: Relatar a experiência do emprego da ureterostomia cutânea (UC) como forma de derivação urinária definitiva em pacientes portadores de neoplasia vesical avançada, em más condições clínicas e que necessitam de tratamento paliativo. MÉTODOS: Foram analisados retrospectivamente os parâmetros clínicos e operatórios de 41 pacientes submetidos a cistectomia radical e UC em três centros oncológicos especializados. A UC foi a derivação urinária escolhida quando os pacientes não apresentavam condições clínicas de serem submetidos a outro tipo de derivação . Foram avaliados a morbidade peri-operatória e a sobrevida global. RESULTADOS: A idade média dos pacientes foi de 69 anos (intervalo interquartil - IQR 62, 76); 30 (73%) pacientes eram do sexo masculino. Vinte e cinco pacientes (61%) foram submetidos a cirurgia de urgência sendo a maioria devido a hemorragia grave associada a instabilidade hemodinâmica. O tempo cirúrgico médio foi de 180 minutos (IQR 120, 180). As complicações peri-operatórias ocorreram em 30 (73%) pacientes sendo a maioria classificadas como "Clavien" graus I e II (66,6%). Não houve óbito per-operatório. A reabordagem cirúrgica foi necessária em 7 (17%) dos pacientes e a sobrevida global foi de 24% após 9,4 meses de seguimento. CONCLUSÕES: A UC com implante de "stent" ureteral é uma alternativa simples de derivação urinária, após cistectomia paliativa, em pacientes sem condições clínicas de serem submetidos a procedimentos cirúrgicos mais complexos. A UC é um procedimento rápido e apresenta taxas de complicações aceitáveis. Essa alternativa cirúrgica permite melhorar a qualidade de vida dos pacientes portadores de tumores vesicais localmente avançados.

PURPOSE: Simple diversions are underutilized, mostly for unfit, bedridden, and very self-limited patients requiring palliative surgical management due to life-threatening conditions. Experience with cutaneous ureterostomy (CU) as palliative urinary diversion option for unfit bladder cancer patients is reported. METHODS: We retrospectively reviewed clinical and operative parameters of 41 patients who underwent CU following RC in three specialized Cancer Centers from July/2005 to July/2010. Muscle-invasive disease (clinical Stage T2/worse), multifocal high-grade tumor, and carcinoma in situ refractory to intravesical immunotherapy were the main indications for RC. Double-J ureteral stents were used in all patients and replaced every 6 months indefinitly. Peri-operative morbidity and mortality were evaluated. RESULTS: Median age was 69 years (interquartile range - IQR 62, 76); 30 (73%) patients were men. Surgery in urgency setting was performed in 25 (61%) of patients, most due to severe bleeding associated with hemodynamic instability; 14 patients (34%) showed an American Society of Anesthesiologists score 4. Median operative time was 180 minutes (IQR 120, 180). Peri-operative complications occurred in 30 (73%) patients, most Clavien grade I and II (66.6 %). There was no per-operative death. Re-intervention was necessary in 7 (17%) patients. Overall survival was 24% after 9.4 months follow-up. CONCLUSIONS: CU with definitive ureteral stenting represents a simplified alternative for urinary diversion after palliative cystectomy in unfit patients. It can be performed quickly, with few early and late postoperative complications allowing RC in a group of patients otherwise limited to suboptimal alternatives. Future studies regarding the quality of life are warranted.

Dois genótipos de melancia foram cruzados para estudar a herança da resistência ao Papaya ringspot virus estirpe melancia (PRSV-W): a cultivar Crimson Sweet (suscetível) e o acesso 'PI 595201' (resistente). As plantas das gerações P1, P2, F1, F2, BC11 e BC12 foram inoculadas com um isolado brasileiro do PRSV-W e os sintomas foram avaliados. Foram estimados parâmetros genéticos e fenotípicos da resistência ao PRSV-W e foram realizados os testes de hipótese de herança monogênica e de máxima verossimilhança. O componente aditivo [a] da resistência foi maior do que os não-aditivos [d]. As estimativas da herdabilidade no sentido amplo (0,80) e restrito (0,67) indicaram que a variância genética foi superior à ambiental, permitindo maiores ganhos genéticos na seleção de plantas resistentes em populações segregantes. Os resultados indicam uma herança mais complexa do que a monogênica típica. Ficou evidente a importância dos efeitos gênicos aditivos no controle da resistência ao PRSV-W.

Two watermelon genotypes were used as parental in crosses designed to study the inheritance of resistance to PRSV-W: the cultivar Crimson Sweet (susceptible) and the accession 'PI 595201'(resistant). Plants of the generations P1, P2, F1, F2, BC11 e BC12 were inoculated with a Brazilian isolate of PRSV-W and were evaluated by recording symptoms. Genetic and phenotypic parameters of PRSV-W resistance were estimated and tests based on hypothesis of monogenic inheritance and maximum likelihood methods were performed. The additive component [a] of resistance was higher than the non-additive [d]. The estimates of the broad-sense heritability (0.80) and of narrow-sense heritability (0.67) indicated that the genetic variance was greater than the environmental, allowing higher genetic gains in selecting more resistant plants in segregating populations. The inheritance is more complex than a typical monogenic inheritance. The importance of the additive genetic effects in the expression of resistance to PRSV-W was evidenced.

OBJETIVO: Determinar a confiabilidade em se correlacionar o índice cardíaco com os dados fornecidos pela gasometria do sangue venoso atrial direito em pacientes submetidos à cirurgia cardíaca, durante o período pós-operatório. MÉTODOS: A partir das amostras de sangue arterial e venoso do átrio direito, colhidas no pós-operatório de cirurgia cardíaca, foram determinados os parâmetros de oxigênio do sangue venoso do átrio direito. Estes parâmetros foram então comparados com o índice cardíaco determinado pela termodiluição. RESULTADOS: Houve boa correlação entre a saturação de oxigênio do sangue venoso do átrio direito (SvO2), diferença artério-venosa do conteúdo de oxigênio do sangue colhido no átrio direito e o índice cardíaco aferido pela termodiluição, com boa sensibilidade e especificidade e alto valor preditivo positivo e negativo. A pressão do sangue do átrio direito (PvO2) apresentou baixa sensibilidade na estimativa de baixo débito cardíaco. CONCLUSÃO: No pós-operatório de cirurgia cardíaca, a SvO2e a diferença artério-venosa do conteúdo de oxigênio (C(av)O2) apresentaram-se como parâmetros confiáveis correlacionados a baixo débito cardíaco. A PvO2 foi pouco sensível no diagnóstico de baixo débito no pós-operatório de cirurgia cardíaca.

OBJECTIVE: To determine, even during postoperative period, the confiability of the cardiac index correlate with the data data given by a central atrial venous blood gasometry in patients who underwent cardiac surgery. METHODS: From the sample of arterial and venous blood of right atrium gathered in postoperative of cardiac surgery, it was determinated the hemoglobin concentration and the gasometric study through what was observed of the venous oxygen saturation (SvO2) and the partial pressure of oxygen from venous blood gathered in right atrium (PvO2), add to the calculation of artery-venous difference of the oxygen content - radial artery / right atrium (C( a-v )O2). Afterwards, these parameters were compared with the cardiac index determined by thermodilution. RESULTS: There was good correlation between SvO2, C(av)O2 of the venous right atrial blood and cardiac index meansured by termodiluition method, with sensibility and especificity good and high positive predict value and negative predict value. The PvO2 demonstrated poor sensibility in the estimative of low output. CONCLUSION: In cardiac surgery postoperative, the SvO2and the C(a-v)O2 were safe parameters correlated with low cardiac output. The PvO2 demonstrated poor sensibility in the estimative of low output in postoperative cardiac surgery.
