ABSTRACT BACKGROUND: Occult hepatitis B virus infection (OBI) is defined as the presence of hepatitis B virus (HBV) deoxyribonucleic acid (DNA) in the liver of individuals with undetectable hepatitis B virus surface antigen (HBsAg) in the serum. The actual prevalence of OBI and its clinical relevance are not yet fully understood. OBJECTIVE: To evaluate the prevalence of HBV DNA in liver biopsies of HBsAg-negative patients with chronic liver disease of different etiologies in a referral center in Brazil and compare two different HBV DNA amplification protocols to detect HBV. DESIGN AND SETTING: This cross-sectional observational study was conducted at the Liver Outpatient Clinic, Hospital das Clínicas, Universidade Federal de Minas Gerais, Belo Horizonte, MG, Brazil, between January 2016 and December 2019. METHODS: HBV DNA was investigated in 104 liver biopsy samples from individuals with chronic liver disease of different etiologies, in whom HBsAg was undetectable in serum by nested-polymerase chain reaction (nested-PCR), using two different protocols. RESULTS: OBI, diagnosed by detecting HBV DNA using both protocols, was detected in 6.7% of the 104 individuals investigated. Both protocols showed a good reliability. CONCLUSION: In addition to the differences in the prevalence of HBV infection in different regions, variations in the polymerase chain reaction technique used for HBV DNA amplification may be responsible for the large variations in the prevalence of OBI identified in different studies. There is a need for better standardization of the diagnostic methods used to diagnose this entity.

Abstract The objective of this work was to identify the effect of potassium rates on Bt transgenic resistance and damage caused by Spodoptera frugiperda in corn plants at different developmental stages. The experimental design was a randomized complete block in a 3x6 factorial arrangement, with three potassium rates (0, 45, and 90 kg ha-1) and six corn hybrids (20A55Hx, 30A77PW, AG1051, MG652PW, NS90VTPro2, and NS92VTPro), with three replicates. The hybrids were evaluated through the analysis of canonical variables. With low potassium rates, the effectiveness of the corn hybrids in controlling the S. frugiperda caterpillar decreases in the different phenological stages. The use of the Bt technology is more efficient under an adequate potassium rate of 90 kg ha-1 KCl.
Resumo O objetivo deste trabalho foi identificar o efeito de doses de potássio sobre a resistência transgênica Bt e os danos causados por Spodoptera frugiperda em plantas de milho, em diferentes estádios de desenvolvimento. O delineamento experimental foi de blocos ao acaso, em arranjo fatorial 3x6, com três doses de potássio (0, 45 e 90 kg ha-1) e seis híbridos de milho (20A55Hx, 30A77PW, AG1051, MG652PW, NS90VTPro2 e NS92VTPro), com três repetições. Os híbridos foram avaliados por meio de análise de variáveis canônicas. Com doses baixas de potássio, a eficácia dos híbridos de milho no controle da lagarta S. frugiperda diminui nos diferentes estádios fenológicos. O uso da tecnologia Bt é mais eficiente quando sob dosagem adequada de potássio de 90 kg ha-1 KCl.

Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

ABSTRACT The growth and importance of the Brazilian citrus industry in the world involves the introduction of technologies such as the use of plant growth regulators such as gibberellic acid (GA3). The effects of doses of 0, 5, 10 and 15 mg L-1 of GA3 (Pro-Gibb®) associated with the surfactant Silwett L-77® at 0.5 mL L-1 were evaluated in an experiment by spraying the plants of a ‘Natal’ orange orchard in the Northern region of the state of São Paulo, Brazil. The treatments were applied annually, for two years, when the fruits were still fully green, but before the flowering of the next crop. The results showed, at the picking period, that the treatment of ‘Natal’ sweet orange trees with gibberellic acid associated with silicone surfactant, reduced fruit abscission due to the higher force needed to detach the fruits from the plants and significantly increased the peel resistance to puncture; The maintenance of the greener color of the orange peel, evaluated using a colorimeter, make them less attractive to fruit flies and make it possible the delay on insecticides treatments for flies control. Additionally, it allows the delay on harvesting by storing the fruits on the plants, with staggered harvest beyond the normal period for the variety. It is suggested doses of 5 to 10 mg L-1 GA3 always associated to silicone surfactant (0.05% L77) to obtain the best responses in oranges of Natal cultivar. Treatments with GA3 did not affect the productivity of the ‘Natal’ sweet orange orchard, neither the maturation and nor the technological quality of the fruits.
RESUMO O crescimento e importância da citricultura brasileira no mundo passa pela introdução de tecnologias como a utilização de reguladores vegetais como o ácido giberélico (GA3). Os efeitos das doses de 0, 5, 10 e 15 mg L-1 de GA3 (Pro-Gibb®) associado ao surfactante Silwett L-77® a 0,5 mL L-1 foram avaliadas em experimento pulverizando as plantas de um pomar de laranjeira ‘Natal’ na região Norte do estado de São Paulo, Brasil. A aplicação dos tratamentos deu-se anualmente, durante dois anos, quando os frutos ainda estavam totalmente verdes, porém antes do florescimento da próxima safra. Os resultados mostraram por ocasião da colheita que o tratamento das laranjeiras ‘Natal’ com ácido giberélico associado a surfactante siliconado, diminuiu a abscisão de frutos pela maior força necessária para destacar os frutos das plantas durante a colheita e aumentou significativamente a resistência da casca à punctura. A manutenção da coloração mais esverdeada da casca dos frutos, avaliada através de colorímetro, os tornam menos atrativo às moscas das frutas e assim possibilita retardar as aplicações de inseticidas para controle das moscas. Além disso, permite retardar as colheitas armazenando os frutos nas plantas, com colheitas escalonadas para além do período normal para a variedade. Sugere-se doses de 5 a 10 mg L-1 GA3 sempre associado a surfactante siliconado (0,05% L77) para se obter as melhores respostas em laranjas da variedade Natal. Os tratamentos com GA3 não afetaram a produtividade do pomar de laranja ‘Natal’, nem a maturação e nem a qualidade tecnológica dos frutos.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract Introduction: Coronary artery bypass grafting (CABG) is the most frequently performed heart surgery in Brazil. Recent international guidelines recommend that national societies establish a database on the practice and results of CABG. In anticipation of the recommendation, the BYPASS Registry was introduced in 2015. Objective: To analyze the profile, risk factors and outcomes of patients undergoing CABG in Brazil, as well as to examine the predominant surgical strategy, based on the data included in the BYPASS Registry. Methods: A cross-sectional study of 2292 patients undergoing CABG surgery and cataloged in the BYPASS Registry up to November 2018. Demographic data, clinical presentation, operative variables, and postoperative hospital outcomes were analyzed. Results: Patients referred to CABG in Brazil are predominantly male (71%), with prior myocardial infarction in 41.1% of cases, diabetes in 42.5%, and ejection fraction lower than 40% in 9.7%. The Heart Team indicated surgery in 32.9% of the cases. Most of the patients underwent cardiopulmonary bypass (87%), and cardioplegia was the strategy of myocardial protection chosen in 95.2% of the cases. The left internal thoracic artery was used as a graft in 91% of the cases; the right internal thoracic artery, in 5.6%; and the radial artery in 1.1%. The saphenous vein graft was used in 84.1% of the patients, being the only graft employed in 7.7% of the patients. The median number of coronary vessels treated was 3. Operative mortality was 2.8%, and the incidence of cerebrovascular accident was 1.2%. Conclusion: CABG data in Brazil provided by the BYPASS Registry analysis are representative of our national reality and practice. This database constitutes an important reference for indications and comparisons of therapeutic procedures, as well as to propose subsequent models to improve patient safety and the quality of surgical practice in the country.

Resumo: Introdução: A cirurgia endoscópica funcional endonasal é um procedimento cirúrgico frequente entre os otorrinolaringologistas. Em 2014, a Sociedade Europeia de Rinologia publicou o "Documento Europeu para Posicionamento sobre a Terminologia Anatômica Interna do Nariz e das Cavidades Paranasais" com o objetivo de unificar os termos na língua inglesa. Ainda não dispomos de uma terminologia unificada na língua portuguesa. Objetivo: Adaptação transcultural dos termos anatômicos do nariz e das cavidades paranasais para o português da "European Anatomical Terminology of the Internal Nose and Paranasal Sinuses". Método: Um grupo de rinologistas de todo o Brasil, com experiência em cirurgia endoscópica endonasal, foi convidado a participar da elaboração desse posicionamento sobre os termos anatômicos do nariz e das cavidades paranasais para o português conforme metodologia adaptada da previamente descrita por Rudmik e Smith. Resultados: Os resultados desse documento foram gerados a partir da concordância da maioria dos participantes conforme as sugestões mais populares entre os rinologistas. Uma adaptação transcultural da terminologia anatômica nasossinusal foi consolidada. Sugerimos que se busque uniformizar termos como "concha inferior", "septo nasal", "porção (óssea/cartilaginosa) do septo nasal", "meato (médio/ inferior) nasal", "via da drenagem do seio frontal", "recesso frontal" e "processo uncinado". Conclusão: Consolidamos uma versão adaptada em português da "European Anatomical Terminology of the Internal Nose and Paranasal Sinuses" que auxiliará a publicação de comunicados técnicos, publicações científicas e o ensino dos termos anatômicos internos do nariz e das cavidades paranasais no Brasil.
Abstract Introduction: Functional endonasal endoscopic surgery is a frequent surgical procedure among otorhinolaryngologists. In 2014, the European Society of Rhinology published the "European Position Paper on the Anatomical Terminology of the Internal Nose and Paranasal Sinuses", aiming to unify the terms in the English language. We do not yet have a unified terminology in the Portuguese language. Objective: Transcultural adaptation of the anatomical terms of the nose and paranasal cavities of the "European Anatomical Terminology of the Internal Nose and Paranasal Sinuses" to Portuguese. Methods: A group of rhinologists from diverse parts of Brazil, all experienced in endoscopic endonasal surgery, was invited to participate in the creation of this position paper on the anatomical terms of the nose and paranasal sinuses in the Portuguese language according to the methodology adapted from that previously described by Rudmik and Smith. Results: The results of this document were generated based on the agreement of the majority of the participants according to the most popular suggestions among the rhinologists. A cross-cultural adaptation of the sinonasal anatomical terminology was consolidated. We suggest the terms "inferior turbinate", "nasal septum", "(bone/cartilaginous) part of the nasal septum", "(middle/inferior) nasal meatus", "frontal sinus drainage pathway", "frontal recess" and "uncinate process" be standardized. Conclusion: We have consolidated a Portuguese version of the European Anatomical Terminology of the Internal Nose and Paranasal Sinuses, which will help in the publication of technical announcements, scientific publications and the teaching of the internal anatomical terms of the nose and paranasal sinuses in Brazil.

Resumen El objetivo de este estudio fue evaluar el rendimiento deshuesado en canal de 64 toros jóvenes Nellore alimentados en corral con dietas que contienen urea recubierta o sin recubrir y sacrificados a cinco pesos corporales (350, 455, 485, 555 y 580 kg). Se utilizó un diseño completamente aleatorizado con un arreglo factorial de 5 χ 2. No se detectó ningún efecto significativo para la interacción entre el peso de sacrificio y la dieta; por lo tanto, las variables se analizaron por separado, en comparación con los contrastes polinómicos y la prueba F, respectivamente. Los pesos absolutos de los cortes primarios del cuarto anterior y posterior aumentaron linealmente a medida que aumentaba el peso del sacrificio de los animales. El aumento de los pesos de sacrificio elevó el porcentaje de la tapa de la grupa en el cuarto trasero de la pistola, así como su rendimiento. Los porcentajes de la gorguera y la falda de la rabadilla aumentaron linealmente, mientras que los de nudillo, lomo, filete de flanco, grupa y plano exterior disminuyeron y la suma de los cortes primarios tuvo un efecto cuadrático en relación con el peso de la mitad de la carcasa. El aumento de los pesos de matanza condujo a un mayor porcentaje de grasa del recorte de los cortes del cuarto trasero en relación con el peso de la carcasa 1/2. El peso de los cortes de carne fue similar entre los grupos de animales alimentados con urea recubierta y no recubierta. Los pesos de sacrificio más altos generan cortes de carne más pesados pero porcentajes más bajos de cortes primarios del cuarto trasero de la pistola en relación con el peso de la carcasa.
Abstract The aim of this study was to evaluate the boneless yield of the carcass of 64 feedlot-finished young Nellore bulls fed diets containing coated or uncoated urea and slaughtered at five body weights (350, 455, 485, 555, and 580 kg). A completely randomized design with a 5 χ 2 factorial arrangement was used. No significant effect was detected for the interaction between slaughter weight and diet; therefore, the variables were analyzed separately, compared by polynomial contrasts and the F test, respectively. The absolute weights of the primal cuts from the fore- and hind-quarter increased linearly as the slaughter weight of the animals increased. Increasing slaughter weights elevated the percentage of rump cap in the pistola hindquarter as well as its yield. The percentages of the rump cap and rump skirt increased linearly, whereas those of knuckle, tenderloin, flank steak, rump, and outside flat decreased and the sum of prime cuts had a quadratic effect in relation to the weight of the right 1/2 carcass. Increasing slaughter weights led to a higher percentage of fat from the trimming of the hindquarter cuts relative to the weight of the 1/2 carcass. The weight of meat cuts was similar between the groups of animals fed coated and uncoated urea. Higher slaughter weights generate heavier meat cuts but lower percentages of primal cuts from the pistola hindquarter relative to the carcass weight.

RESUMO O crescimento da população idosa exige reorganização política, econômica e social, em especial na área da saúde, devido ao impacto sobre esta. A avaliação da Probabilidade de Internação Hospitalar (PIH) é usada como indicador da condição de saúde do idoso. Entretanto, variáveis de capacidade física (CF) ainda não foram associadas com a PIH. Analisou-se a associação entre CF e a PIH de idosos. O estudo realizado foi epidemiológico, transversal e analítico. A seleção da amostra ocorreu de forma aleatória em uma das Equipes de Saúde da Família no bairro Passo d’Areia, no município de Porto Alegre, sendo avaliados 317 idosos com idade ≥ 65 anos. A CF foi avaliada por testes de força de membros superiores e inferiores, flexibilidade e equilíbrio. A PIH foi avaliada pelo instrumento de triagem rápida de Boult. Para análise estatística, a PIH foi ajustada em dois grupos: baixa-média e média alta-alta (MAA) e realizada a análise multivariada de regressão de Poisson. O nível de significância adotado foi de p ≤ 0,05. Nos resultados encontrados, as variáveis físicas que permaneceram associadas à PIH-MAA foram a força de membros inferiores (RP = 1,78; IC 95% = 1,04 - 3,04) e flexibilidade (RP = 2,13; IC 95% = 1,28 - 3,56). Houve associação negativa entre os baixos níveis de força de membro inferior e de flexibilidade com PIH-MAA. A prevalência da PIH-MAA entre indivíduos com força alterada para membros inferiores foi 78% maior em relação àqueles com força normal e 113% entre aqueles com alteração de flexibilidade.
ABSTRACT The growth of older population requires political, economic and social rearrangement, particularly in health, considering its impact on this matter. The assessment of Likelihood of Hospitalization (LOH) is used as an indicator for the health condition of older adults. However, variables of physical capacity (PC) have not yet been associated with LOH. Thus, the association between PC and LOH was analyzed. This study was epidemiological, cross-sectional, and analytical. The sample was randomly selected from one of the Family Health Teams in Passo d’Areia neighborhood, municipality of Porto Alegre. 317 older adults aged ³65 years were evaluated. PC was evaluated by tests of strength, flexibility, and balance on upper and lower limbs. LOH was evaluated with the Boult rapid screening instrument. For the statistical analysis, LOH has been adjusted in two groups: low-medium and medium high-high (MHH), and the Poisson multivariate regression analysis was used. The adopted significance level was p≤0.05. Among the results found, the physical variables that stayed attached to the LOH-MHH were the strength of lower limbs (PR=1.78; 95%CI=1.04 - 3.04) and flexibility (PR=2.13; 95%CI=1.28 - 3.56). There was a negative association between low levels of lower limb strength and flexibility with LOH-MHH. The prevalence of LOH-MHH among individuals with altered strength for the lower limbs was 78% greater when compared with those with normal strength, and it was 113% greater for those with altered flexibility.
RESUMEN El crecimiento de la población anciana exige reorganización política, económica y social, en especial en el área de la salud, debido al impacto sobre ésta. La evaluación de la Probabilidad de Ingreso Hospitalario (PIH) es usada como indicador de la condición de salud del anciano. Mientras tanto, variables de la capacidad física (CF) todavía no fueron asociadas con la PIH. Se analizó la asociación entre la CF y la PIH de ancianos. El estudio realizado fue epidemiológico, transversal y analítico. La selección de la muestra ocurrió de manera aleatoria en uno de los Equipos de Salud de la Familia en el barrio Passo d’Areia, en el municipio de Porto Alegre, siendo evaluados 317 ancianos con edad ≥ 65 años. La CF fue evaluada por pruebas de fuerza de miembros superiores e inferiores, flexibilidad y equilibrio. La PIH fue evaluada por el instrumento de triaje rápido de Boult. Para el análisis estadístico, la PIH fue ajustada en dos grupos: baja-mediana y mediana alta-alta (MAA) y realizado el análisis multivariado de regresión de Poisson. El nivel de significancia adoptado fue de p ≤ 0,05. En los resultados encontrados, las variables físicas que permanecieron asociados a la PIH-MAA fueron la fuerza de miembros inferiores (RP = 1,78; IC el 95% = 1,04 - 3,04) y la -flexibilidad (RP = 2,13; IC el 95% = 1,28 - 3,56). Hubo asociación negativa entre los bajos niveles de fuerza de miembro inferior y de flexibilidad con la PIH-MAA. La prevalencia de la PIH-MAA entre los individuos con la fuerza alterada para los miembros inferiores fue un 78% más grande en relación a los con la fuerza normal y un 113% entre los con alteración de flexibilidad.

Abstract Objective: To report the early results of the BYPASS project - the Brazilian registrY of adult Patient undergoing cArdiovaScular Surgery - a national, observational, prospective, and longitudinal follow-up registry, aiming to chart a profile of patients undergoing cardiovascular surgery in Brazil, assessing the data harvested from the initial 1,722 patients. Methods: Data collection involved institutions throughout the whole country, comprising 17 centers in 4 regions: Southeast (8), Northeast (5), South (3), and Center-West (1). The study population consists of patients over 18 years of age, and the types of operations recorded were: coronary artery bypass graft (CABG), mitral valve, aortic valve (either conventional or transcatheter), surgical correction of atrial fibrillation, cardiac transplantation, mechanical circulatory support and congenital heart diseases in adults. Results: 83.1% of patients came from the public health system (SUS), 9.6% from the supplemental (private insurance) healthcare systems; and 7.3% from private (out-of -pocket) clinic. Male patients comprised 66%, 30% were diabetics, 46% had dyslipidemia, 28% previously sustained a myocardial infarction, and 9.4% underwent prior cardiovascular surgery. Patients underwent coronary artery bypass surgery were 54.1% and 31.5% to valve surgery, either isolated or combined. The overall postoperative mortality up to the 7th postoperative day was 4%; for CABG was 2.6%, and for valve operations, 4.4%. Conclusion: This first report outlines the consecution of the Brazilian surgical cardiac database, intended to serve primarily as a tool for providing information for clinical improvement and patient safety and constitute a basis for production of research protocols.
