Resumo Introdução As alterações causadas pela rigidez na Doença de Parkinson (DP) podem afetar a musculatura mandibular, mas há uma escassez de estudos publicados sobre seu impacto na abertura bucal. Objetivo Analisar a relação da extensão vertical da abertura de boca com a rigidez muscular e os fatores sociodemográficos de idosos com a DP. Material e método Trata-se de um estudo transversal, quantitativo, que coletou dados de uma pesquisa primária realizada no Hospital das Clínicas da Universidade Federal de Pernambuco, em 2018. Apanhou-se dados dos prontuários e do questionário: Critério de Diagnóstico de Pesquisa para Disfunções Temporomandibulares (RDC/TMD). A amostra foi composta por 81 parkinsonianos e caracterizadas em: variáveis sociodemográficas e presença ou ausência de rigidez muscular. As medidas de extensão vertical de abertura de boca avaliadas foram: abertura de boca sem auxílio e sem dor (ABASD) e abertura máxima da boca sem auxílio (AMBSA). Aplicou-se os testes de correlação linear de Pearson e de correlação de Spearman para averiguar relação entre as variáveis contínuas. Análises de associações foram realizadas através da regressão logística simples. Nível de significância de p<0,05. Resultado A limitação de abertura de boca não apresentou relação com a idade e o sexo. O maior nível de significância foi entre a abertura de boca sem auxílio e sem dor e a rigidez muscular (p= 0,012) e a escolaridade (p= 0,038). Conclusão A limitação de abertura de boca nas pessoas com DP se mostrou relacionada a rigidez muscular e a escolaridade mais baixa.

Abstract Introduction Changes caused by the rigidity of Parkinson’s Disease (PD) can affect the mandibular musculature. However, few studies have been published about its impact on the oral opening. Objective To analyze the relationship of the vertical extension of the oral opening with muscular rigidity and sociodemographic factors of the elderly with PD. Material and method This is a cross-sectional, quantitative study that collected data from a primary study conducted at the Hospital das Clínicas of the Federal University of Pernambuco in 2018. Data were collected from medical records and from the questionnaire, Research Diagnostic Criterion for Temporomandibular Disorders (RDC/TMD). The sample was composed of 81 parkinsonians and characterized using: sociodemographic variables and the presence or absence of muscular rigidity. The measures of vertical extension of the oral opening evaluated were: mouth opening without assistance and without pain (ABASD), and maximum mouth opening without assistance (AMBSA). The Pearson’s linear correlation and Spearman’s correlation tests were applied to investigate the relationship among the continuous variables. Analyses of association were conducted using simple logistic regression. The level of significance was set at p<0.05. Result Limitation of the oral opening was not related to age or sex. The greatest level of significance was between mouth opening without assistance and without pain and muscular rigidity (p=0.012), and years of schooling (p=0.038). Conclusion The limitation of mouth opening in people with PD was shown to be related to muscular rigidity and fewer years of schooling.

ABSTRACT Objective: To describe the sociodemographic profile of elderly people with temporomandibular disorder and depression in Parkinson's disease cases. Material and Methods: It is a cross-sectional cohort cut study, which used secondary data from 124 elderly people with Parkinson’s disease (PD). To diagnose temporomandibular disorder (TMD), the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) questionnaire was used, and the variables studied were: age, sex, race, education, marital status, family income, stage of the disease, and depression. The statistical analysis performed was descriptive and used absolute numbers and a frequency count. Results: After the application of the eligibility criteria, the patient records of 81 elderly people with Parkinson’s disease were selected; 15 presented associated TMD and depression. The average age was 69 years old; 66.67% were male; 46.67% had more than 9 years of schooling; 60% were married or had a partner; 53.33% received between 1 and 2 minimum wages; 33.33% stated they were brown; 80% were in the moderate stage of the disease; 46.66% were in group II according to the RDC/TMD, presenting disk displacement; and 53.33% presented severe depression. Conclusion: It was verified that the elderly people with Parkinson’s and associated TMD and depression were male, married or with a partner, on a low income, with nine or more years of schooling, and were in the moderate stage of the disease.

Resumo A violência contra a pessoa idosa desenha-se como um problema de saúde pública de complexa administração. É de fundamental importância conhecer seus fatores associados, com ênfase em cada tipo de violência, para possibilitar a criação de políticas públicas baseadas em evidências. Objetivou-se realizar uma revisão sistemática da literatura de estudos epidemiológicos analíticos sobre os fatores associados à violência contra idosos. Para a pesquisa bibliográfica utilizou-se quatro bases de dados: PubMed, Scopus, Web of Science e Lilacs, sem corte de anos. A seleção dos artigos foi realizada por pares e em duas etapas: leitura dos resumos (3121) e leitura dos artigos completos (64), tendo sido selecionados 27 artigos. O risco de viés foi avaliado. Os fatores associados à violência geral foram idade, sexo, estado civil, nível de educação, renda, arranjo familiar, suporte social, solidão, transtorno mental, depressão, tentativa de suicídio, dependência para atividades da vida diária, função cognitiva, doenças crônicas, abuso de álcool ou drogas, entre outros. A violência contra idosos apresentou-se como um fenômeno multifatorial e complexo, por isso não pode ser vista de forma parcial, unidimensional e sim levando em consideração todas as dimensões e entender que há uma interdependência entre elas.

The aim of the present study was to systematically review the literature regarding analytical epidemiological studies of factors associated with elder abuse. Four databases were used for the bibliographic search: Pubmed, Scopus, Web of Science and Lilacs, with no limitations regarding year of publication. Articles were selected by pairs of researchers in two stages: reading of abstracts (3.121) and reading of complete articles (64). The total number of articles selected was 27. The risk of bias was evaluated. The factors associated with general violence were age, sex, marital status, educational level, income, family arrangement, social support, solitude, mental disorder, depression, suicide attempt, dependence on others in daily activities, cognitive function, chronic diseases, alcohol or drug abuse, among others. Elder abuse was found to be a multifactorial and complex phenomenon that should not therefore be viewed in a partial one-dimensional manner, but in such a way as to take into account all dimensions and the interdependence of these.

RESUMO Objetivo: investigar se a dor crônica e a depressão representam fatores associados à disfunção temporomandibular (DTM) em idosos com doença de Parkinson. Métodos: estudo de corte transversal realizado no Hospital das Clínicas da Universidade Federal de Pernambuco, em 2018. Utilizou-se o questionário Critério de Diagnóstico de Pesquisa para Disfunções Temporomandibulares. As variáveis clínicas estudadas foram: dor crônica, depressão, sintomas físicos não específicos, incluindo e excluindo itens de dor e uso de prótese dentária. As associações entre as variáveis dependente e independentes foram avaliadas pelo teste do Odds Ratio do Qui-quadrado, com intervalo de confiança de 95%. Resultados: encontraram-se dentro dos critérios de elegibilidade 81 idosos, 67% eram do sexo masculino, 74% eram casados ou tinham companheiro, 43% declararam receber de 1-2 salários mínimos e 47% apresentavam-se no estágio moderado da doença de Parkinson. A DTM foi identificada em 22% da amostra e 12% dos participantes referiram presença de dor crônica. A análise estatística demonstrou associação entre DTM e dor crônica (p=0,001, OR=inf, IC95%=12,13-inf), bem como entre DTM e depressão moderada-severa (p=0,014, OR=4,8, IC95%=1,14 - 23,51). Conclusão: verificou-se que os fatores que estavam associados à DTM em idosos com doença de Parkinson foram dor crônica e depressão moderada-severa.

ABSTRACT Purpose: to investigate whether chronic pain and depression are factors associated with temporomandibular dysfunction (TMD) in older adults with Parkinson's disease. Methods: a cross-sectional study using the Research Diagnostic Criteria for Temporomandibular Disorders questionnaire. The clinical variables studied were chronic pain, depression, nonspecific physical symptoms including and excluding items of pain, and dentures use. The associations between the dependent and independent variables were evaluated by the chi-square odds ratio, with a 95% confidence interval. Results: a total of 81 older adults met the eligibility criteria - 67% were males, 74% were married or had a partner, 43% reported earning 1 to 2 minimum wages, and 47% were in the moderate stage of Parkinson's disease. TMD was identified in 22% of the sample, 12% reporting chronic pain. The statistical analysis showed an association between TMD and chronic pain (p = 0.001, OR = inf, 95% CI = 12.13 - inf) and between TMD and moderate-to-severe depression (p = 0.014, OR = 4.8, 95% CI = 1.14 - 23.51). Conclusion: it was verified that chronic pain and moderate-to-severe depression were the factors associated with TMD in older adults presented with Parkinson's disease.

Resumo Objetivo: Identificar os fatores associados à disfunção temporomandibular em pessoas idosas. Método: Trata-se de revisão integrativa da literatura, em que foram avaliadas publicações científicas indexadas em cinco bases de dados: MEDLINE/PubMed, SCOPUS, WEB OF SCIENCE, CINAHL e LILACS, sem restrição temporal ou de idioma. Os artigos foram avaliados pelo instrumento The Newcastle-Ottawa Scale adaptado. Resultados: Dos 888 artigos provenientes das bases de dados, quatro foram considerados elegíveis para esta revisão. Segundo a análise do risco de viés, um artigo foi classificado como de baixo risco e os demais como de risco intermediário. Conclusão: Os fatores associados à disfunção temporomandibular em pessoas idosas indicados nos estudos foram: ser do sexo feminino, ter idade entre 60-70 anos, ser de baixa renda, apresentar zumbido, tontura, depressão, cefaleia ou bruxismo; ter dor à palpação da articulação temporomandibular, músculos mastigatórios e cervicais, número de dentes reduzido e fazer uso de próteses totais.

Abstract Objective: To identify the factors associated with temporomandibular dysfunction among elderly persons. Method: An integrative literature review was carried out, evaluated by scientific publications indexed in five databases: MEDLINE/PubMed, SCOPUS, WEB OF SCIENCE, CINAHL and LILACS, without date or language restrictions. The adapted Newcastle-Ottawa Scale was used to evaluate the articles. Results: Of the 888 articles from the databases, four were considered eligible for this review. Risk of bias analysis classified one article as low risk and the other as intermediate risk. Conclusion: The factors associated with temporomandibular dysfunction in the elderly indicated by the studies were: female gender, age between 60-70 years, a low income, suffering from tinnitus, dizziness, depression, headache or bruxism; experience temporomandibular joint palpation, masticatory and cervical muscle pain, a low number of teeth and the use of complete dentures.
