ABSTRACT Introduction: HIV infection and tuberculosis are two of the main epidemics that affect many Latin American countries, which have a two-way impact on each other, making them highly relevant diseases in public health. Objective: To identify the factors associated with tuberculosis and HIV coinfection in patients registered in the Tuberculosis Control Program of the Dos de Mayo National Hospital. Methods: A cross-sectional and analytical study was carried out. The medical records of hospitalized patients admitted to the Tuberculosis Control Program were reviewed. Those with a confirmatory test for Western Blot with active tuberculosis were considered coinfection with tuberculosis and HIV simultaneously. With the generalized linear models, p-values and prevalence ratios adjusted with a 95% CI were obtained. Results: Of the 289 patients, 30.1% had tuberculosis and HIV coinfection and was associated with being male (RPa: 2.11; 95% CI: 1.21-3.66), drug user (RPa: 1.71; 95% CI: 1.13-2.52) and have a history of previous tuberculosis infection (RPa: 2.35; 95% CI: 1.53-3.63). Likewise, it was inversely associated with age (RPa: 0.98; 95% CI: 0.97-0.99). Conclusions: According to the data, the prevalence of tuberculosis and HIV coinfection is high and there is a higher frequency of tuberculosis and HIV coinfection among men, at a younger age, those with a history of tuberculosis and those who consume drugs. It is important to implement actions to improve the diagnosis and treatment of both infectious diseases.

RESUMEN Introducción: La infección por VIH y la tuberculosis son dos de las principales epidemias que afectan a muchos países de Latinoamérica, las cuales tienen un impacto bidireccional entre sí, por lo que son enfermedades de gran relevancia en la salud pública. Objetivo: Identificar los factores asociados a la coinfección por tuberculosis y VIH en los pacientes registrados en el Programa de Control de la Tuberculosis del Hospital Nacional Dos de Mayo. Métodos: Se realizó un estudio transversal y analítico. Se revisaron las historias clínicas de los pacientes hospitalizados ingresados al Programa de Control de Tuberculosis. Se consideró coinfección por tuberculosis y VIH a aquellos con prueba confirmatoria para Western Blot con tuberculosis activa de forma simultánea. Con los modelos lineales generalizados se obtuvieron valores p y las razones de prevalencia ajustados con un IC 95 %. Resultados: De los 289 pacientes, el 30,1 % tuvo coinfección por tuberculosis y VIH y se asoció a ser varón (RPa: 2,11; IC 95 %:1,21-3,66), consumidor de drogas (RPa: 1,71; IC 95 %: 1,13-2,52) y tener el antecedente de infección previa de tuberculosis (RPa: 2,35; IC 95 %: 1,53-3,63). Asimismo, estuvo asociada de manera inversa con la edad (RPa: 0,98; IC 95 %: 0,97-0,99). Conclusiones: Según los datos, la prevalencia de coinfección por tuberculosis y VIH es elevada y existe mayor frecuencia de coinfección por tuberculosis y VIH entre varones, a una menor edad, los que tenían antecedentes de tuberculosis y los que consumían drogas. Es importante implementar acciones para mejorar el diagnóstico y el tratamiento de ambas enfermedades infecciosas.

ABSTRACT Mycobacterium abscessus is a fast-growing non-tuberculous mycobacterium that causes lung, skin, disseminated and other infections, mainly in patients with comorbidities. The diagnosis is based on the identification of the mycobacterium by culture media or molecular tests. Treatment with macrolides and amikacin remains the optimal choice, although it depends on the location and severity of the infection; however, resistance genes have been identified in some subspecies that limit antibiotic efficacy. We present the case of a patient with cutaneous coinfection by Mycobacterium abscessus and Mycobacterium tuberculosis, who presented exogenous Cushing syndrome, a predisposing factor for these infections. In addition, hydatidosis and pulmonary aspergillosis were identified. The patient’s anti-tuberculosis treatment was adjusted to manage both mycobacteria, resulting in a favorable evolution.

RESUMEN Mycobacterium abscessus es una micobacteria no tuberculosa de crecimiento rápido, que produce infección pulmonar, cutánea, diseminada y otras, sobre todo en pacientes con comorbilidades. El diagnóstico se basa en la identificación de la micobacteria por medios de cultivo o por pruebas moleculares. El tratamiento con macrólidos y amikacina continúa siendo el ideal, aunque depende de la localización y de la gravedad de la infección, sin embargo, se han identificado genes de resistencia en algunas subespecies que limitan la eficacia antibiótica. Presentamos el caso de un paciente con coinfección cutánea por Mycobacterium abscessus y Mycobacterium tuberculosis, quien presenta un síndrome de Cushing exógeno, factor predisponente para estas infecciones. Además, se identificaron hidatidosis y aspergilosis pulmonares. El tratamiento antituberculoso del paciente se ajustó para manejar ambas micobacterias, y su evolución fue favorable.

Osteopetrosis comprises a series of rare genetic conditions that produce an imbalance in bone remodeling due to abnormal osteoclastic activity. We report a female patient 1 year 4 months, diagnosed with malignant osteopetrosis neuropathic child while causing paleness, distended abdomen with collateral circulation and growth retardation and developmental milestones are investigating. Examination right eye with retinopathy. Skeletal x-rays revealed a generalized bone hyperdensity. She bone marrow aspirate showed hypercellularity with hyperplastic erythroid, megakaryocytic normal white series. Diagnosis was confirmed by clinical and radiological features. The patient received supportive care. While the diagnosis is simple OPAR this is often delayed by the rarity of the disease and the lack of clinical suspicion, early diagnosis and treatment of bone marrow transplant is curative approach to a disease of poor prognosis.

Osteopetrosis comprende una serie de raras condiciones genéticas que produce un desequilibrio en el remodelado óseo debido a una actividad osteoclástica anormal. Reportamos una paciente mujer de 1 año 4 meses, diagnosticada de osteopetrosis maligna infantil neuropática mientras se investigaba la causa de palidez, abdomen distendido con circulación colateral y retardo en el crecimiento y de los hitos del desarrollo. Examen oftalmológico con retinopatía derecha. Radiografías del esqueleto revelaron una hiperdensidad ósea generalizada. Aspirado de medula ósea mostró hipercelularidad con la serie eritroide hiperplásica, serie blanca y megacariocítica normal. Diagnóstico fue confirmado por características clínicas y radiológicas. La paciente recibió tratamiento de soporte. Aunque el diagnostico de osteopetrosis maligna infantil es sencillo este a menudo se retrasa por la infrecuencia de la enfermedad y la falta de sospecha clínica, un diagnóstico oportuno y un tratamiento de trasplante de medula ósea son el enfoque curativo para una enfermedad de pobre pronóstico.

El objetivo de este estudio es determinar la relación entre la dependencia alcohólica y la altitud de residencia en once poblados de dos zonas altitudinales del Perú. Se realizó un estudio transversal analítico aplicando una encuesta por médicos que hasta 2013 realizaron atención primaria de salud en once poblados, que se clasificaron en zonas de baja altitud (≤ 2.500 msnm) y gran altitud (> 2.500 msnm). A los que respondieron afirmativamente a la pregunta de si consumían bebidas alcohólicas, se les aplicó el test CAGE para alcoholismo (punto de corte, ≥ 2). Con los modelos lineales generalizados, se obtuvieron los estadísticos de asociación. De los 737 participantes, el 51% eran mujeres y la mediana de edad, 36 [intervalo intercuartílico, 25-50] años 334 (45%) vivían en baja altitud y 113 (15%) tenían dependencia alcohólica. La mayor frecuencia de alcoholismo se asoció positivamente con ser de un poblado considerado como extremadamente pobre (razón de prevalencia ajustada [RPa] = 2,42; intervalo de confianza del 95%, [IC95%], 1,40-4,19); mientras que, ser mujer (RPa = 0,44; IC95%, 0,23-0,89) y residir a gran altitud (RPa = 0,15; IC95%, 0,07-0,31) se asociaron negativamente, ajustando por nueve variables sociolaborales y patológicas. Según estos datos, hay mayor frecuencia de dependencia al alcohol entre los varones, los extremadamente pobres y residentes a baja altitud. Los profesionales que realizan atención primaria y los implicados en el cuidado de la salud mental deben tomarlo en cuenta por sus implicaciones para la sociedad.

The aim of this study was to determine the association between alcohol dependence and altitude of residence in 11 villages in two high altitude areas of Peru. An analytical cross-sectional study was performed using a survey conducted by physicians in primary health care in 11 villages until 2013, that were divided into low altitude (≤2500 m asl (above sea level)), and high altitude (>2500 m asl) areas. The CAGE test for alcoholism (cut point, ≥2) was applied to those who responded positively when asked if they consumed alcohol. Statistical associations were obtained with generalised linear models Of the 737 participants, 51% were women and the median age was 36 years [interquartile range, 25-50], 334 (45%) lived at low altitude, and 113 (15%) had alcohol dependence. The highest frequency of alcoholism was positively associated with being a village considered extremely poor (Likelihood Ratio (LP) = 2.42; 95%CI, 1.40-4.19), while being female (LP = 0.44; 95%CI, 0.23-0.89) and residing at high altitude (LP = 0.15; 95%CI, 0.07-0.31) were negatively associated. These were adjusted for nine socio-occupational and pathological variables. According to these data, there is a higher frequency of alcohol dependence in being, male, extremely poor, and residing at low altitude. These results should be taken into account by professionals who work in primary care and those involved in mental health care, because of their implications in society.
