RESUMEN Introducción. La malaria es una enfermedad metaxénica causada por el Plasmodium spp., de los cuales, las especies más prevalentes en Perú son el P. vivax y P. falciparum. En Perú fueron reportados 22 617 casos de malaria en el 2023. Objetivo. Describir la evolución de los casos de malaria en Perú entre los años 2000 y 2022, caracterizar a la población afectada y estimar la incidencia acumulada (IA) en los periodos 2000-2005, 2006-2011, 2012-2017 y 2018-2022. Métodos. Usamos un diseño descriptivo observacional longitudinal de fuente secundaria. Los datos fueron extraídos de la base de datos llamada «Vigilancia epidemiológica de Malaria». Se estimó la IA usando la base de datos y la población estimada anual por departamentos brindada por el Instituto Nacional de Estadística e Informática. El estudio abarcó la totalidad del territorio peruano, y analizamos los casos individuales de malaria. Resultados. La IA por 1000 habitantes en el departamento de Loreto fue superior en comparación a los demás departamentos en todos los periodos, llegando a un pico máximo de 51,84 en el periodo 2012-2017 (6,28, 12,45, 51,84, 22,93). Madre de Dios presentó un pico de 9,77 por 1000 habitantes en el periodo 2006-2011, luego bajó a 1,13 en el periodo 2012-2017. Además, Amazonas tuvo un incremento de la IA por 1000 habitantes en el periodo 2018-2022, llegando a 3,02. Conclusión. Loreto fue el departamento con mayor riesgo de infección de malaria a lo largo de los periodos 2000-2005, 2006-2011, 2012-2017 y 2018-2022.

ABSTRACT Introduction. Malaria is a metaxenic disease caused by Plasmodium spp., of which the most prevalent species in Peru are P. vivax and P. falciparum. In Peru, 22 617 malaria cases were reported in 2023. Objective. To describe the evolution of malaria cases in Peru between 2000 and 2022, characterize the population and estimate the cumulative incidence (CI) for the periods 2000-2005, 2006-2011, 2012-2017, and 2018-2022. Methods. This was a descriptive observational longitudinal secondary source design, using the “Malaria Epidemiological Surveillance” database. The CI was calculated using the database and the annual estimated population by departments provided by the National Institute of Statistics and Information. The study covered the entire Peruvian territory and analyzed individual malaria cases. Results. The CI of malaria per 1000 inhabitants in the department of Loreto was higher compared to other departments across all periods, reaching a peak of 51,84 in the 2012-2017 period (6,28, 12,45, 51,84, and 22,93 in the periods 2000-2005, 2006-2011, 2012-2017, and 2018-2022, respectively). Madre de Dios exhibited a peak in incidence, reaching 9,77 cases per 1,000 inhabitants during the 2006-2011 period, which subsequently decreased to 1,13 in the 2012-2017 period. Meanwhile, Amazonas recorded an uptrend in incidence per 1,000 inhabitants during the 2018-2022 period, attaining 3,02. Conclusion. Loreto was the department with the highest risk of malaria infection throughout the periods 2000-2005, 2006-2011, 2012-2017, and 2018-2022.

RESUMEN La tuberculosis (TB) miliar es una presentación con un desenlace fatal de no ser diagnosticada ni tratada a tiempo; para desarrollar esta presentación se requieren de ciertas condicionantes. En este artículo, se estudia el caso de un varón de 48 años sin antecedentes patológicos, quien inició la enfermedad por un periodo de seis meses, con un cuadro gastrointestinal difuso inicial; un mes antes del ingreso presentó un cuadro respiratorio progresivo, por lo que fue ingresado a Emergencias, en donde se evidenció, en la tomografía computarizada de tórax, compromiso miliar difuso bilateral; se le administró oxígeno y medidas de soporte, se obtuvo prueba VIH, cuyo resultado fue negativo. Se obtuvieron resultados positivos en la baciloscopia de esputo. Su evolución fue estacionaria hasta la administración de la terapia antituberculosa y se observó leve mejoría clínica; así mismo, se le administraron dosis bajas de corticoide, luego de los cuales se evidenció una evolución favorable, por lo que se le dio de alta.

ABSTRACT Miliary Tuberculosis (TB) is a potentially fatal condition if not diagnosed and treated promptly, although it requires certain circumstances to develop. This article studies the case of a 48-year-old man with no significant pathological history, who developed the disease over a period of six months, initially presenting with diffuse gastrointestinal symptoms. One month prior to admission, he developed progressive respiratory symptoms and was admitted to the emergency department where bilateral diffuse miliary involvement was evident on the chest computed tomography. He was administered oxygen and support measures and passed an HIV test, which was negative. Nevertheless, his sputum smear microscopy showed positive results. His clinical evolution remained stationary until the administration of antituberculosis therapy, observing slight clinical improvement. Low doses of corticosteroids were also administered, leading to a favorable evolution, and he was subsequently discharged.

RESUMEN Objetivo. Los programas de rehabilitación cardiaca (RC) basados en telesalud son una alternativa en el contexto de pandemia y representa una oportunidad para continuar en la intervención de las enfermedades cardiovasculares (ECV). El presente estudio pretende evaluar el efecto de un programa de telerrehabilitación cardiaca (TRC) en la calidad de vida, índice de ansiedad/depresión, seguridad del ejercicio y nivel de conocimiento de la enfermedad en pacientes dados de alta de un instituto de referencia nacional en tiempos de pandemia. Metodología. Estudio pre-experimental en pacientes cardiópatas que ingresaron al programa de rehabilitación cardíaca en el INCOR durante agosto a diciembre del 2020. Se incluyó pacientes de bajo riesgo en quienes se aplicó un cuestionario (sobre enfermedades cardiovasculares, seguridad del ejercicio, ansiedad/depresión y calidad de vida) al inicio y al finalizar el programa, el cual fue aplicado a través de una plataforma virtual. Se utilizó análisis descriptivo y comparativo antes-después a través de pruebas de hipótesis. Resultados. Se incluyeron 64 pacientes (71,9% varones). La edad promedio fue 63,6 ±11,1 años, En cuanto a la seguridad del ejercicio, se encontró un incremento en la media de la puntuación tras la aplicación del programa (3,06 ± 0,8 a 3,18 ± 0,7, p=0,324). En relación con la ansiedad, la media del puntaje se redujo de 8,61 a 4,75, mientras que para la depresión la reducción fue de 7,27 a 2,92. En cuanto al puntaje de calidad de vida, el componente global mejoró de 111,48 a 127,92. Conclusiones. El programa de TRC implementado a través de una plataforma virtual durante la pandemia de la COVID-19 mejoró la calidad de vida, disminuyó el estrés y la depresión en pacientes cardiópatas dados de alta de un centro de referencia nacional cardiovascular.

ABSTRACT Objective. Cardiac Rehabilitation (CR) programs based on telehealth are an alternative in the context of a pandemic and represent an opportunity to continue in the intervention of cardiovascular diseases (CVD). The present study aims to evaluate the effect of a Cardiac Tele-Rehabilitation (CTR) program on quality of life, anxiety/depression index, exercise safety and Level Of disease awareness in patients discharged from a national referral institute in times of pandemic. Methods. A pre-experimental study in cardiac patients who entered the cardiac rehabilitation program at INCOR from August to December 2020. The study included low-risk patients who were administered a questionnaire (on cardiovascular disease, exercise safety, anxiety/depression, and quality of life) at the beginning and end of the program, which was applied through a virtual platform. Descriptive and comparative before-after analysis was used through hypothesis testing. Results. Sixty-four patients were included (71.9% male). The mean age was 63.6 ±11.1 years. Regarding exercise safety, an increase in the mean score was found after the application of the program (3.06 ± 0.8 to 3.18 ± 0.7, p=0.324). Concerning anxiety, the mean score was reduced from 8.61 to 4.75, while for depression, the reduction was from 7.27 to 2.92. Regarding the quality-of-life score, the global component improved from 111.48 to 127.92. Conclusions. The CTR program implemented through a virtual platform during the COVID-19 pandemic enhanced quality of life and decreased stress and depression in cardiac patients discharged from a national cardiovascular referral center.

Resumen El artículo parte de un dato objetivo y comprobable en la práctica privada en el proceso penal peruano, que es pasible de ser extendido a los países de la región indoamericana, en los que la conceptualización de la opinión pública acerca de la defensa técnica en un proceso penal, lindan con un ejercicio hábil no precisamente en hermenéutica y dogmática penal, sino en prestidigitación de los hechos y las pruebas, como es el hecho de que generalmente en los países latinoamericanos la declaración que realiza el imputado no es considerada un medio de prueba, sino un medio de defensa, por lo que en la motivación de la decisión jurisdiccional no es ponderada debidamente y, antes bien, se asume que obstaculiza la justicia y perturba el proceso, de donde ha surgido la inveterada, pero falsa idea de que el imputado tiene derecho a mentir. Esta problemática, en un sistema procesal penal garantista, atenta contra los derechos fundamentales del infractor y la víctima, así como no resulta funcional a la finalidad del proceso, cual es lograr un resultado satisfactorio al interés público de coadyuvar a la paz social merced a la solución del conflicto que todo crimen supone, a través de alcanzar la justicia para las partes involucradas. Los autores plantean una propuesta innovadora, que consiste en que la declaración libre y voluntaria del imputado, expresada sin coacción alguna, cuando no desea guardar silencio, sea garantizada y considerada como un auténtico medio de prueba y, por ende, valorada en conjunto con los demás medios de prueba.

Abstract The article is based on objective and verifiable data in private practice in the Peruvian criminal process, which is likely to be extended to the countries of the Indo-American region, in which the conceptualization of public opinion about technical defense in a process punishment, border on a skilful exercise not precisely in criminal hermeneutics and dogmatics but in sleight of hand of facts and evidence, such as the fact that generally in Latin American countries, the statement made by the accused is not considered a means of proof but a means of defense, for which reason the jurisdictional decision is not duly considered and, rather, it is assumed that it hinders justice and disturbs the process, from which the inveterate but false idea has arisen that the accused has the right to lie. This problem, in a guaranteed criminal procedural system, violates the fundamental rights of the offender and the victim, as well as is not functional to the purpose of the process, which is to achieve a satisfactory result in the public interest of contributing to social peace thanks to the solution of the conflict that every crime involves, through achieving justice for the parties involved. The authors propose an innovative proposal, which consists in that the free and voluntary declaration of the accused, expressed without any coercion, when he does not wish to remain silent, be guaranteed and considered as an authentic means of proof and, therefore, valued in conjunction with other means of proof.

Resumen: La investigación persigue como objetivo: determinar la influencia de la gestión del Clima Institucional en el Desempeño del Personal Docente de la Institución Educativa n° 7044 San Martín de Porres, Chorrillos. La validez de la investigación se demuestra a través del juicio de expertos y la fiabilidad a través del alfa de Cronbach (0,922 y 0,839) para ambos instrumentos. Se concluye que tanto la variable “Gestión del Clima Institucional” como “Desempeño del Personal Docente” son percibidas sobre todo como buena, al igual que la mayoría de sus respectivas dimensiones y la correlación existente entre las mismas es positiva.

Abstract: The objective of the research is to determine the influence of the Institutional Climate management on the Performance of the Teaching Personnel of the Educational Institution No. 7044 San Martín de Porres, Chorrillos. The validity of the research is demonstrated through expert judgment and reliability through Cronbach's alpha (0.922 and 0.839) for both instruments. It is concluded that both the variable "Institutional Climate Management" and "Performance of Teaching Staff" are perceived above all as good, as are most of their respective dimensions and the correlation between them is positive.

Aim: To determine the associated factors with mortality of adult patients hospitalized with COVID-19. Methods: We conducted a prospective cohort study and included patients older than 18 years hospitalized with the diagnosis of SARS-CoV-2 infection. Those patients with a positive rapid serological test on admission, but no respiratory symptoms nor compatible images were excluded. We collected the data from clinical records. Results: A total of 813 adults were included, 544 (66.9%) with confirmed COVID-19. The mean age was 61.2 years (SD: 15.0) and 575 (70.5%) were male. The most frequent comorbidities were hypertension (34.1%) and obesity (25.9%). The most frequent symptoms on admission were dyspnea (82.2%) and cough (53.9%). A total of 114 (14.0%) patients who received mechanical ventilation, 38 (4.7%) were admitted to the Intensive Care Unit (ICU) and 377 (46.4%) died. The requirement for ventilatory support, greater lung involvement, comorbidities, and inflammatory markers were associated with mortality. It was found that for every 10 - year increase in age, the risk of dying increased by 32% (RR: 1.32 95% CI: 1.25 to 1.38). Those who required admission to the ICU and mechanical ventilation had 1.39 (95% CI: 1.13 to 1.69) and 1.97 (95% CI: 1.69 to 2.29) times the risk of dying compared to those who did not. Conclusion:  We found a high mortality rate in hospitalized patients associated with greater age, more elevated inflammatory markers, and more severe respiratory compromise.

Objetivos. Determinar los factores asociados a mortalidad de los pacientes adultos hospitalizados con COVID-19 en un hospital de referencia de la seguridad social. Materiales y métodos. Se realizó un estudio de cohorte prospectivo. Se incluyó a pacientes mayores de 18 años hospitalizados con el diagnostico de infección por SARS-CoV-2 y se excluyó a quienes ingresaron asintomáticos respiratorios, con prueba rápida serológica positiva al ingreso y sin imágenes compatibles. Los datos se recolectaron a partir de la historia clínica. Resultados. Se incluyó un total de 813 adultos, 544 (66.9%) tuvieron COVID-19 confirmado. La media de la edad fue de 61.2 años (DE: 15.0) y 575 (70.5%) fueron de sexo masculino. Las comorbilidades más frecuentes fueron hipertensión arterial (34.1%) y obesidad (25.9%). Los síntomas más frecuentes al ingreso fueron disnea (82.2%) y tos (53.9%). Un total de 114 (14.0%) pacientes recibieron ventilación mecánica, 38 (4.7%) ingresaron a UCI y 377 (46.4%) fallecieron. El requerimiento de soporte ventilatorio, el mayor compromiso pulmonar, las comorbilidades y los marcadores inflamatorios se asociaron a la mortalidad. Se halló que por cada 10 años que aumenta la edad, el riesgo de morir se incrementa en 32% (RR: 1.32 IC95%: 1.25 a 1.38). Aquellos que requirieron ingreso a UCI y ventilación mecánica tuvieron 1.39 (IC95%: 1.13 a 1.69) y 1.97 (IC95%: 1.69 a 2.29) veces el riesgo de morir, respectivamente. Conclusión. La mortalidad encontrada en nuestro estudio fue alta y estuvo asociada a la edad,  marcadores inflamatorios y compromiso respiratorio.

La presencia de este mecanismo de resistencia limita o anula las opciones terapéuticas para combatir a estas bacterias. En Latinoamérica, las cifras son cada vez más elevadas, pues se reportan en Guatemala, Colombia, Chile, Argentina, entre otros. Perú no ha descrito, hasta la fecha, la presencia de este patrón de resistencia; sin embargo, desde hace varios años se presume de su existencia. Se describen nueve casos de Klebsiella pneumoniae NDM, como agentes infecciosos o colonizantes, en pacientes críticamente enfermos, en su mayoría con patología neuroquirúrgica, del Hospital Nacional Dos de Mayo, en Lima - Perú. Los pacientes de la serie descrita a continuación, representan los primeros reportes de Klebsiella pneumoniae NDM en el Perú.

Objetivos. Evaluar la eficacia del Western Blot para el diagnóstico específico de la enfermedad de Chagas utilizando antígenos de excreción-secreción de epimastigotes de Trypanosoma cruzi. Materiales y métodos. Los antígenos fueron obtenidos luego de veinte horas de incubación en medio Minimum Essential Medium-Eagle, los cuales fueron preparados a la concentración proteica de 0,2 ug/uL para ser enfrentados a 10 mL de un pool de sueros de pacientes con la enfermedad de Chagas y un conjugado anti-IgG marcado con peroxidasa, se evidenció la presencia de los antígenos de 10, 12, 14,15, 19, 20, 23, 26, 30, 33, 36, 40, 42, 46, 58, 63, 69 91,100 y 112 KDa, de los cuales, los antígenos de 10, 12, 14, 15, 19, 20, 23 y 26 KDa, han sido considerados como específicos usando pools de sueros de pacientes con otras parasitosis y sueros de personas no parasitadas. La sensibilidad de la técnica se evaluó empleando sueros individuales de 65 pacientes con la enfermedad de Chagas y la especificidad con sueros de 40 pacientes con otras parasitosis y cinco sueros de personas no parasitadas. Resultados. La técnica tiene una sensibilidad de 95,4% en la detección de una a ocho bandas específicas, una especificidad de 100%, un valor predictivo positivo de 100% y un valor predictivo negativo de 93,7%. Conclusiones. La técnica de Western Blot con antígenos de excreción-secreción de epimastigotes de T. cruzi es eficaz en el diagnóstico de la enfermedad de Chagas en el Perú, pudiendo ser utilizada como prueba confirmatoria.

Objectives. Evaluate the effectiveness of Western Blot for the specific diagnosis of Chagas disease using excretory-secretory antigens of Trypanosoma cruzi epimastigotes. Materials and methods. Antigens were obtained after twenty hours of incubation in Eagle’s Minimum Essential Medium, which were prepared at a protein concentration of 0.2 ug/uL to be faced with 10 mL pool of serum from patients with Chagas disease and a conjugated anti-IgG labeled with peroxidase. The presence of the following antigens was observed: 10, 12, 14, 15, 19, 20, 23, 26, 30, 33, 36, 40, 42, 46, 58, 63, 69, 91, 100, and 112 kDa; of which antigens of 10, 12, 14, 15, 19, 20, 23, and 26 kDa were considered to be specific using pools of serum from patients with other parasitosis and serum from people with no parasites. The sensitivity of the technique was assessed using individual serum from 65 patients with Chagas disease; and the specificity with serum from 40 patients with other parasitosis, and serums from five people who did not have parasites. Results. The technique has a sensitivity of 95.4% in the detection of one to eight specific bands, a specificity of 100%, a positive predictive value of 100%, and a negative predictive value of 93.7%. Conclusions. Western Blot technique with excretory-secretory antigens of T. cruzi epimastigotes is effective in the diagnosis of Chagas disease in Peru; therefore, it can be used as a confirmatory test.

Gabriel Cano de Aponte was Governor of Chile since 1717 and util his death in 1733, being 68 years old. As a soldier, he was an experimented horse rider. The sequence ofevents that caused his death three months after an equestrian accident are unclear. A systematic clinical analysis of the later is the main objective of this review. Historians have documented Cano de Aponte's "inclination for unbridled fun and equestrian exercises". Duríng a holiday Cano de Aponte suffered a horse fall and subsequent crushing by the latter. It has been stated that a spinal lesion caused by the accident kept him bedrídden for a period of three months, clear in reason and with intense pain, before his death. However, there is no evidence on historie data that conveys any typical sign associated to spinal injury following the accident. Therefore we suggest that Cano de Aponte suffered a complex pelvic ring fracture. The fact that he was prostrated, lucid, but suffered and intense pain best sustains the hypothesis of a pelvic fracture. After the initial period, one ofthe most common causes of death resulting from a pelvic fracture is deep venous thrombosis with secondary pulmonary thromboembolism. This must have been the sequence ofevents that most probably caused Cano de Aponte's death).
