ABSTRACT Introduction. The thyroid nodule in children is usually asymptomatic, multiple and isolated presentation, with difficulty in establishing the risk of malignancy, especially if it is not palpable. Objective. To determine the epidemiological, clinical and ultrasound characteristics and surgical management of thyroid nodules in patients of the Head, Neck and Maxillofacial Surgery Service of the Instituto Nacional de Salud del Niño (INSN) - Breña, 2000-2020. Methods. Observational, descriptive and retrospective study. The Bethesda classification was used for cytological studies. Qualitative variables were expressed in absolute and relative frequencies; and quantitative variables in measures of central tendency and dispersion. Results. There were 66 cases, the average age was 10.94 ± 0.88 years, 48.5% between 11 and 15 years, 78.8% were female, 89.4% had no family history of cancer. The asymptomatic form predominated (74.2%). Ultrasonographically, 43.9% of nodules were located in the right lobe, single presentation (68.2%) and smaller than two centimeters (43.9%). 82.6% of the 23 Bethesda II cases were hemithyroidectomized and 86.4% of 22 Bethesda VI cases were thyroidectomized. Conclusions. In pediatric patients, thyroid nodules occurred mostly in women aged 11 to 15 years, with no family history of cancer, asymptomatic, located in the right lobe, single and less than two centimeters. Grade II of the Bethesda classification was the most frequent. Hemithyroidectomy was the most used surgical intervention

RESUMEN Introducción. El nódulo tiroideo en niños generalmente es asintomático, de presentación múltiple y aislada, con dificultad para instaurar el riesgo de malignidad, sobre todo si no es palpable. Objetivo. Determinar las características epidemiológicas, clínicas, ecográficas y manejo quirúrgico del nódulo tiroideo en pacientes del Servicio de Cirugía de Cabeza, Cuello y Máxilo Facial del Instituto Nacional de Salud del Niño (INSN) - Breña, 2000 -2020. Métodos. Estudio observacional, descriptivo y retrospectivo. Se usó la clasificación Bethesda para los estudios citológicos. Los datos fueron analizados en el programa SPSS versión 22. Las variables cualitativas fueron expresadas en frecuencias absolutas y relativas; y las cuantitativas en medidas de tendencia central y dispersión. Resultados. Fueron 66 casos, el promedio de edad fue 10,94 ± 0,88 años, el 48,5% entre 11 a 15 años, el 78,8% fueron del sexo femenino, un 89,4% no referían antecedente familiar de cáncer. Predominó la forma asintomática (74,2%). Ecográficamente el 43,9% de nódulos se localizaron en lóbulo derecho, presentación única (68,2%) y menor a dos centímetros (43,9%). El 82,6% de los 23 casos Bethesda II fueron hemitiroidectomizados y el 86,4% de 22 casos Bethesda VI fueron tiroidectomizados. Conclusiones. En pacientes pediátricos, los nódulos tiroideos se presentaron mayormente en mujeres de 11 a 15 años, sin antecedente familiar de cáncer, asintomáticos, localizados en lóbulo derecho, únicos y menor a dos centímetros. El grado II de la clasificación Bethesda fue la más frecuente. La hemitiroidectomía fue la intervención quirúrgica más utilizada.

RESUMEN Objetivo: Describir las características clínicas y sociodemográficas de los pacientes con VIH/sida con complicaciones neurológicas por infecciones oportunistas en el Hospital Nacional Hipólito Unanue en Lima, Perú (2017-2019). Materiales y métodos: Se realizó un estudio observacional, descriptivo, transversal y retrospectivo. Se incluyeron historias clínicas de pacientes mayores de 18 años con diagnóstico de VIH/sida con complicaciones neurológicas por infecciones oportunistas. Las variables del estudio fueron analizadas con el programa SPSS versión 25 mediante estadística descriptiva. Resultados: Se analizaron 45 historias clínicas; la edad promedio fue 34,38 ± 11,19 años y el sexo predominante, el masculino (73,30 %). El 31,10 % procedía del distrito de Ate, el 53,30 % tenía secundaria completa y el 24,40 % eran desempleados. El 67,90 % (n = 19/28) eran heterosexuales y el 17,20 % tenía tuberculosis como comorbilidad. Las manifestaciones clínicas predominantes fueron cefalea (21,30 %) y alteración del nivel de conciencia (19 %). La tuberculosis cerebral y la neurotoxoplasmosis fueron las complicaciones neurológicas más frecuentes, ambas con 30,90 %. El 83,30 % (n = 25/30) de los sujetos tuvieron una carga viral >100,000 copias/ml; el 97,40 % (n = 38/39), un conteo de CD4 < 199 células/mm3, y el 90,50 % (n = 38/42) de los sujetos no tenía buena adherencia al TARGA. Conclusiones: Las complicaciones neurológicas por infecciones oportunistas afectan con más frecuencia al grupo etario de 18 a 30 años, del sexo masculino, desempleados y heterosexuales. Las complicaciones más frecuentes fueron la tuberculosis cerebral y la neurotoxoplasmosis. La mayoría de los sujetos tuvieron una carga viral > 100,00 copias/ml, un conteo de CD4 <199 células/mm3 y pobre adherencia al TARGA.

ABSTRACT Objective: To describe the clinical and sociodemographic characteristics of HIV/AIDS patients with neurological complications due to opportunistic infections at the Hospital Nacional Hipólito Unanue in Lima, Peru (2017-2019). Materials and methods: An observational, descriptive, cross-sectional and retrospective study was conducted with medical records of patients older than 18 years diagnosed with HIV/AIDS and neurological complications due to opportunistic infections. The study variables were analyzed with descriptive statistics using the IBM SPSS Statistics V25. Results: Forty-five (45) medical records were analyzed: the average age was 34.38 ± 11.19 years, 73.30 % were males, 31.10 % were from the district of Ate, 53.30 % completed secondary education, 24.40 % were unemployed, 67.90 % (n = 19/28) were heterosexual and 17.20 % had tuberculosis as comorbidity. The most common clinical manifestations were headache (21.30 %) and impaired consciousness (19 %). Cerebral tuberculosis and neurotoxoplasmosis were the most frequent neurological complications (30.90 % in both cases). Eighty-three point three zero percent (83.30 %) (n = 25/30) of the study subjects had a viral load > 100,000 copies/ml, 97.40 % (n = 38/39) had a CD4 count < 199 cells/mm3, and 90.50 % (n = 38/42) showed poor adherence to HAART. Conclusions: Neurological complications due to opportunistic infections most frequently affected the age group of 18 to 30 years old, males, unemployed and heterosexuals. The most frequent complications were cerebral tuberculosis and neurotoxoplasmosis. Most of the study subjects had a viral load > 100,000 copies/ml, CD4 count < 199 cells/mm3 and poor adherence to HAART.

ABSTRACT Objective: To determine the association between family support and glycemic control among patients with type 2 diabetes mellitus who attend the outpatient endocrinology office of the Hospital Nacional Dos de Mayo. Materials and methods: A quantitative, observational and cross-sectional study was carried out. The sample consisted of 158 diabetic subjects over 18 years of age whose glycosylated hemoglobin was obtained at the time of the consultation using the Epi Info version 7.9.2 software, with a 95 % confidence level, 5 % accuracy and 12.11 % expected frequency. A non-probability convenience sampling was used. A data collection sheet was employed to register the patients’ sociodemographic, clinical and social environment data, as well as the Family Adaptability and Cohesion Evaluation Scale III (FACES III) to determine the family support. A chi-square test was used to determine the association between family support and glycemic control. Results: Female patients accounted for 50.6 % (n = 80) of the sample. The mean age was 63.5 ± 12.10 years. The patients’ children (50.6 %, n = 80) and spouses (20.3 %, n = 32) were the most likely primary caregivers. Out of the 158 study subjects, 98 (62 %) had a good glycemic control, 44 % of whom (44.9 %) presented an adequate family support. Out of the 60 patients with no glycemic control, 16 (26.6 %) had an adequate family support. A statistically significant association between family support and glycemic control was demonstrated, with X² = 5,252 and ρ value < 0.05. Conclusions: There is a significant association between family support and glycemic control among the diabetic patients treated at the outpatient endocrinology office of the Hospital Nacional Dos de Mayo.

RESUMEN Objetivo: Determinar la asociación entre soporte familiar y control glicémico en pacientes con diabetes mellitus tipo 2 que acuden al Consultorio Externo de Endocrinología del Hospital Nacional Dos de Mayo. Materiales y métodos: Se realizó un estudio cuantitativo, observacional y transversal. La muestra fue de 158 sujetos diabéticos mayores de 18 años con hemoglobina glicosilada al momento de la consulta, obtenida mediante Epi info v7.9.2, con un nivel de confianza 95 %, precisión del 5 % y con una frecuencia esperada de 12,11 %. El muestreo fue no probabilístico por conveniencia. Se aplicó una ficha de recolección, registrándose datos sociodemográficos, clínicos y del entorno social del paciente, así como una encuesta de la Escala de Cohesión y Adaptabilidad Familiar-FACES III para determinar el soporte familiar. Se realizó la prueba chi cuadrado para determinar la asociación entre soporte familiar y control glicémico. Resultados: El 50,60 % (n = 80) de los participantes fueron mujeres. La edad media fue de 63,5± 12,10 años. La figura familiar más representativa de cuidador primario fue el hijo en un 50,6 % (n = 80) seguido del cónyuge 20,3 % (n = 32). De los 158 sujetos, 98 (62 %) tuvieron control glicémico controlado y de estos 44 (44,9 %) un soporte familiar adecuado. De los 60 no controlados, 16 (26,6 %) tuvieron un soporte familiar adecuado. Se determinó la asociación entre soporte familiar y control glicémico siendo estadísticamente significativa X2 = 5,252, con un ρ < 0,05. Conclusiones: Existe una asociación significativa entre el soporte familiar y el control glicémico en los pacientes diabéticos que se atienden en los consultorios de endocrinología del Hospital Nacional Dos de Mayo.

ABSTRACT Objective: To determine the relationship that sociodemographic factors have with the proper use of masks and appropriate social distancing to prevent the spread of SARS-CoV-2 in people who attend the Surquillo No. 2 market (Lima, Peru). Materials and methods: A quantitative, observational, descriptive, cross-sectional and prospective community study. Both a survey and a checklist were administered to 385 subjects who attended the Surquillo market between September and October 2020. The sample size was obtained using Epidat 4.2 with an expected proportion of inadequate practices of 50 %, a confidence level of 95 % and an absolute precision of 5 %. Non-probability convenience sampling was used. The information was analyzed using IBM SPSS Statistics V25. Descriptive and analytical statistics were performed using the chi-square test and logistic regression with a confidence level of 95 % and p < 0.05. Results: Forty-eight point eight zero percent (48.80 %) of the research participants were males. Their mean age was 42.61 ± 13.84 years. Sixty-four point seven zero percent (64.70 %) (n = 249) had higher (technical and university) education, 47.50 % (n = 183) lived in Surquillo, 42.90 % (n = 165) were self-employed, 48.80 % (n = 188) wore a well-fitting mask in good condition, 32.20 % (n = 124) covered their nose and mouth properly, 35.10 % (n = 135) used cotton masks, 70.00 % thought that a distance of 1.5 m should be kept from others, and 94 % maintained a physical distance of less than 1.5 m during the observation. There was a significant relationship between the higher education level and the proper use of masks (p < 0.001) but not between the education level and social distancing. Conclusions: There is a significant relationship between the education level and the proper use of masks.

RESUMEN Objetivo: Determinar la relación de los factores sociodemográficos con el uso adecuado de las mascarillas y el distanciamiento social apropiado para evitar el contagio del SARS-CoV-2 en las personas que visitan el mercado n.º 2 de Surquillo (Lima, Perú). Materiales y métodos: Estudio cuantitativo, observacional, descriptivo, transversal y prospectivo comunitario. Se encuestó a 385 sujetos que acudieron al mercado de Surquillo entre septiembre y octubre del 2020 y se aplicó una lista de cotejo a los participantes. El tamaño muestral se obtuvo mediante EPIDAT 4.2 con una proporción esperada de prácticas inadecuadas del 50 %, nivel de confianza 95 % y precisión absoluta del 5 %. El muestreo fue no probabilístico por conveniencia. La información fue analizada mediante SPSS v25. Se aplicó estadística descriptiva y analítica usando prueba de X2 y regresión logística, con un nivel de confianza del 95 % y un p < 0,05. Resultados: El 48,80 % de los participantes fueron hombres. La edad promedio fue 42,61 ± 13,84 años. El 64,70% (n = 249) tuvo instrucción superior (técnico y universitario). El 47,50% (n = 183) de los encuestados procedía del distrito de Surquillo, y el 42,90 % (n = 165) fueron empleados independientes. El 48,80 % (n = 188) portaba una mascarilla en óptima condición. El 32,20 % (n = 124) cubría nariz y boca con un buen acople. La mascarilla de algodón fue la más usada: 35,10 % de los participantes (n = 135). El 70,00 % opinó que el distanciamiento debe ser de 1,5 m. Durante la observación, el 94 % mantuvo una distancia menor de 1,5 m. Existió una relación significativa entre el grado de instrucción superior y el uso adecuado de la mascarilla con p < 0,00, pero no para el distanciamiento social. Conclusiones: Existe una relación significativa entre el grado de instrucción y el uso adecuado de la mascarilla.

ABSTRACT Objective: To describe the clinical-epidemiological characteristics and management of cerebral arteriovenous malformations (AVM) in patients treated at the Instituto Nacional de Salud del Niño San Borja (INSN-SB). Materials and methods: A retrospective and descriptive study conducted in patients with AVM treated at the INSN-SB between 2015 and 2017. Data was processed and analyzed using IBM SPSS Statistics statistical software version 22. The qualitative variables were expressed in absolute and relative frequencies. Results: Forty-one (41) AVM cases were identified, being more frequent in the age group between 6 and 12 years (56.10 %) and in females (65.90 %). Ruptured AVM occurred in 80.49 % of the patients and 92.70 % claimed that headache was the most frequent clinical manifestation. The anatomical areas with the highest occurrence of AVM were the frontal lobe (36.60 %) and the left cerebral hemisphere (51.20 %). Grade III AVM was the most common one (43.90 %) according to the Spetzler-Martin grading scale. The most frequently used treatment was embolization (39 %) and mortality accounted for 2.40 %. Conclusions: AVM most frequently occurred in females and patients between 6 and 12 years old. A little more than half of the patients had a ruptured AVM. Headache was the predominant symptom of this disease. Most patients showed grade III AVM according to the Spetzler-Martin grading scale. Embolization was the most frequently used surgical method and the mortality rate was low.

RESUMEN Objetivo: Describir las características clínico-epidemiológicas y el manejo de las malformaciones arteriovenosas cerebrales (MAVc) en pacientes atendidos en el Instituto Nacional de Salud del Niño San Borja (INSN-SB). Materiales y métodos: Estudio retrospectivo y descriptivo en pacientes con malformaciones arteriovenosas cerebrales atendidos en el Instituto Nacional de Salud del Niño San Borja entre 2015-2017. Los datos fueron procesados y analizados en el programa SPSS versión 22. Las variables cualitativas se expresaron en frecuencias absolutas y relativas. Resultados: Se identificaron 41 malformaciones arteriovenosas cerebrales. La mayoría se presentó en el grupo etario entre los 6 y 12 años (56,10 %) y en el sexo femenino (65,90 %). La ruptura de las malformaciones ocurrió en el 80,49 % de casos, y el 92,70 % de los pacientes tuvo cefalea como manifestación clínica predominante. Las localizaciones más frecuentes fueron el lóbulo frontal (36,60 %) y el hemisferio cerebral izquierdo (51,20 %). El grado más común de las malformaciones arteriovenosas cerebrales fue el III (43,90 %) de la clasificación de Spetzler-Martin. La embolización fue el tratamiento empleado con mayor frecuencia (39 %) y la mortalidad alcanzó el 2,40 %. Conclusiones: Los grupos que presentan malformaciones arteriovenosas cerebrales con mayor frecuencia son las mujeres y los pacientes entre 6 y 12 años. Un poco más de la mitad de los pacientes tuvieron ruptura de la lesión. La cefalea es el síntoma predominante y las malformaciones más frecuentes corresponden al grado III de la escala de Spetzler-Martin. La embolización es el método quirúrgico más empleado y la mortalidad fue baja.

ABSTRACT Objective. To determine the clinical, epidemiological and pathological characteristics of lymphomas in patients of the National Institute of Child Health Breña between 2015-2019. Methods. A quantitative, observational, crosssectional, descriptive and retrospective study was carried out; corresponding to pediatric patients diagnosed with lymphoma between 2015 and 2019. The SPSS v24 program was used for the analysis. The quantitative variables were expressed in measures of central tendency and the qualitative ones in absolute and relative frequencies. Results. 43 cases were included, the average age was 7.2 years (1a - 16a), 72.1% (n = 31) were male. Among the most frequent symptoms are fever 51.2% (n = 22), weight loss 41.9% (n = 18) and fatigue 20.9% (n = 9) and among the prevalent signs, lymphadenopathy 83.7% (n = 36) and abdominal mass with 14% (n = 6). The most frequent location was the lymph node type with 69.8% (n = 30). Anemia occurred in 58.1% (n = 25). According to the pathological diagnosis, 20 (48.8%) cases were Hodgkin's Lymphoma (HL) and 23 (51.2%) were non-Hodgkin's (NHL); the most frequent types being B lymphoblastic lymphoma with 25.6% (n = 11), followed by mixed cellularity LH with 20.9% (n = 9). The most frequent lymphoma in preschool stage was mixed cellularity LH with 31.3% (n = 5), while in school stage B lymphoblastic lymphoma with 25.9% (n = 7). Conclusions. In pediatric patients, lymphoma mainly affects the male sex, presenting fever, weight loss, lymphadenopathy, abdominal tumor and anemia. B lymphoblastic lymphoma was the most frequent; these clinical characteristics are similar to those reported worldwide.

RESUMEN Objetivo. Determinar las características clínicas, epidemiológicas y patológicas de los linfomas en pacientes del Instituto Nacional de Salud del Niño Breña entre el 2015-2019. Métodos. Se realizó un estudio cuantitativo, observacional, transversal, descriptivo y retrospectivo; correspondiente a los pacientes pediátricos con diagnóstico de linfoma entre los años 2015 al 2019. Para el análisis se utilizó el programa SPSS v24. Las variables cuantitativas fueron expresadas en medidas de tendencia central y las cualitativas en frecuencias absolutas y relativas. Resultados. Se incluyeron 43 casos, el promedio fue 7,2 años (1a - 16a), el 72,1% (n= 31) fueron del sexo masculino. Entre los síntomas más frecuentes están fiebre 51,2% (n= 22), baja de peso 41,9% (n= 18) y cansancio 20,9% (n= 9) y entre los signos prevalentes, las linfadenopatías 83,7% (n=36) y tumoración abdominal con 14% (n= 6). La localización más frecuente fue la de tipo ganglionar con 69,8% (n=30). La anemia se presentó en un 58,1% (n= 25). Según el diagnóstico anátomopatologico, 20 (48,8%) casos fueron Linfoma Hodgkin (LH) y 23 (51,2%) no Hodgkin (LNH); siendo los tipos más frecuente el linfoma linfoblástico B con un 25,6% (n=11), seguido de LH celularidad mixta con 20,9% (n= 9). El linfoma más frecuente en etapa preescolar fue el LH celularidad mixta con 31,3% (n= 5), mientras que en etapa escolar el linfoma linfoblástico B con 25,9% (n=7). Conclusiones. En pacientes pediátricos, el linfoma afectó mayormente al sexo masculino, presentando fiebre, baja de peso, linfoadenopatías, tumoración abdominal y anemia. El linfoma linfoblástico B fue el más frecuente; estas características clínicas son similares a lo reportado a nivel mundial.

ABSTRACT A descriptive observational study was carried out to identify the characteristics of the oncological clinical trials submitted to the Instituto Nacional de Salud del Perú during the period from 1995 to 2019. The information was obtained from the Peruvian Registry of Clinical Trials. We identified 1,996 clinical trials during the studied period, from which 470 were oncological (23.5%); 74.9% of the oncological clinical trials were mainly sponsored by the pharmaceutical industry, 61.9% were phase III and 86.2% were authorized. Regarding those authorized clinical trials, 55.6% were on chemical research products and 35.9% were on therapeutic indication for breast cancer. The most frequent study designs found were: parallel arm (84.7%), randomized (85.2%) and blinded (51.0%); the most frequently used main endpoint was the objective response rate (46.7%). We conclude that the number of oncological clinical trials has been increasing over the years, often with different characteristics.

RESUMEN A fin de identificar las características de los ensayos clínicos oncológicos presentados ante el Instituto Nacional de Salud del Perú en el periodo 1995-2019, se realizó un estudio observacional descriptivo, evaluando la información contenida en el Registro Peruano de Ensayos Clínicos. Se identificaron 1996 ensayos clínicos de los cuales 470 eran oncológicos (23,5%); el 74,9% de los ensayos clínicos oncológicos fue patrocinado principalmente por la industria farmacéutica, el 61,9% fue de fase III, siendo autorizados el 86,2%. Entre los autorizados, el 55,6% eran productos de investigación de tipo químico, el 35,9% correspondieron a indicación terapéutica para cáncer de mama; el principal diseño fue de grupos paralelos (84,7%), aleatorizados (85,2%), cegados (51,0%), el criterio de valoración principal más empleado fue la tasa de respuesta objetiva (46,7%). Se concluye que los ensayos clínicos oncológicos se han ido incrementando con los años, con frecuencia diversa entre sus características.

ABSTRACT Objective To determine the factors associated with gonarthrosis in patients older than 40 years of the Hospital Santa Rosa in 2018. Materials and methods An analytical, case-control (61 cases and 122 controls) study was carried out in patients older than 40 years of age with and without a diagnosis of gonarthrosis treated at the Hospital Santa Rosa in 2018. In both groups, associated factors such as age, sex, nutritional status, hypertension, type 2 diabetes mellitus, occupation, knee trauma, low back pain, metabolic syndrome and smoking were evaluated. Pearson's chi-squared test and OR (logistic regression) were used for the statistical analysis. Results Eighty-eight point five percent (88.5 %) of the patients with gonarthrosis were females. Out of the patients with this disease, 68.9 % were housewives; the most frequent age range was 60 to 69 years in the case group (45.9 %) and 50 to 59 years in the control group (37.7 %). A significant association was found between the risk of suffering from gonarthrosis and having suffered low back pain (OR = 6.395; 95 % CI = 3.163 - 12.929) and being female (OR = 3.905; 95 % CI = 1.632 - 9.342). Regarding the history of knee trauma and smoking, they did not show significance. Conclusions It is concluded that females and low back pain are involved in the onset and progression of gonarthrosis. Age and occupation had a significant association with the occurrence of this disease.

RESUMEN Objetivo Determinar los factores asociados a gonartrosis en pacientes mayores de 40 años del Hospital Santa Rosa en 2018. Materiales y métodos Se realizó un estudio analítico de casos y controles (61 casos y 122 controles) de pacientes mayores de 40 años con diagnóstico de gonartrosis y sin diagnóstico de gonartrosis atendidos en el Hospital Santa Rosa en 2018. En ambos grupos se evaluaron los factores asociados edad, sexo, estado nutricional, hipertensión arterial, diabetes mellitus tipo 2, ocupación, traumatismo de rodilla, lumbalgia, síndrome metabólico y tabaquismo. Para el análisis estadístico se emplearon la prueba de X2 y OR (regresión logística) para el análisis estadístico. Resultados El 88,5 % de los pacientes con gonartrosis fue del sexo femenino. De los pacientes con esta enfermedad, el 68,9 % tenían como ocupación ama de casa; el rango de edad más frecuente fue de 60 a 69 en los casos (45,9 %) y 50 a 59 (37,7 %) en los controles. Se encontró una asociación significativa entre el riesgo de padecer gonartrosis y el haber presentado lumbalgia (OR = 6,395, IC 95% 3,163 - 12,929) y el sexo femenino (OR = 3, 905; IC 95% 1,632 - 9,342). En cuanto al antecedente de traumatismo de rodilla y el tabaquismo, no mostraron significancia. Conclusiones Se concluye que el sexo femenino y la lumbalgia están implicados en la aparición y progresión de la gonartrosis. La edad y la ocupación tuvieron una asociación significativa con la presencia de esta enfermedad.

Regarding those authorized clinical trials, 55.6% were on chemical research products and 35.9% were on therapeutic indication for breast cancer. The most frequent study designs found were: parallel arm (84.7%), randomized (85.2%) and blinded (51.0%); the most frequently used main endpoint was the objective response rate (46.7%). We conclude that the number of oncological clinical trials has been increasing over the years, often with different characteristics.

RESUMEN A fin de identificar las características de los ensayos clínicos oncológicos presentados ante el Instituto Nacional de Salud del Perú en el periodo 1995-2019, se realizó un estudio observacional descriptivo, evaluando la información contenida en el Registro Peruano de Ensayos Clínicos. Se identificaron 1996 ensayos clínicos de los cuales 470 eran oncológicos (23,5%); el 74,9% de los ensayos clínicos oncológicos fue patrocinado principalmente por la industria farmacéutica, el 61,9% fue de fase III, siendo autorizados el 86,2%. Entre los autorizados, el 55,6% eran productos de investigación de tipo químico, el 35,9% correspondieron a indicación terapéutica para cáncer de mama; el principal diseño fue de grupos paralelos (84,7%), aleatorizados (85,2%), cegados (51,0%), el criterio de valoración principal más empleado fue la tasa de respuesta objetiva (46,7%). Se concluye que los ensayos clínicos oncológicos se han ido incrementando con los años, con frecuencia diversa entre sus características.

ABSTRACT Objective : Describe the clinical-surgical and sociodemographic characteristics of patients with esophageal atresia (EA) of the Instituto Nacional de Salud del Niño San Borja (INSN-SB) during 2015-2017. Material and methods : Descriptive observational study of cases with EA 2015-2017. The data was processed and analyzed in the SPSS v20 program, the absolute and relative frequency distributions were expressed in tables. Results : 74 cases with AE were analyzed, 55.4% (n=41) were male. According to gestational age, 45 were at term, 28 pre-term and one post-term. 59.5% had adequate weight at birth. Only 28.4% had complete prenatal care (CPN). 68.9% did not have a prenatal diagnosis of AE. Atresia with distal tracheoesophageal fistula (TEF) was the most frequent in 74.3% (n=55). Hypersalivation occurred in 55 patients (74.3%), followed by dyspnea in 54 (72.9%). Cardiac malformations predominated in 27.1%, followed by digestive 20.3% and genitourinary 17.6%. Only 26 had associated syndromes, VACTERL in 12 cases. End-to-end anastomosis and fistula closure were performed in 55 patients (74.3%). 48 presented early complications, predominating sepsis (20.3%) and 46 late complications, with gastroesophageal reflux being the most frequent 29.7%. Regarding mortality, 8 (10.8%) died. Conclusions : Most of the cases were male, at term, with adequate weight at birth and without CPN. AE with distal TEF was the most frequent. Sialorrhea and dyspnea were the predominant symptoms and cardiac and digestive malformations the most associated. End-to-end anastomosis surgery and fistula closure was the most frequent technique. Complications were sepsis and gastroesophageal reflux. Mortality was low.

RESUMEN Objetivo : Describir las características clínico-quirúrgicas y sociodemográficas de pacientes con atresia esofágica (AE) del Instituto Nacional de Salud del Niño San Borja (INSN-SB) 2015-2017. Material y métodos : Estudio observacional descriptivo de casos con AE 2015-2017. Los datos se procesaron y analizaron en el programa SPSS v20, las variables cualitativas fueron expresadas en frecuencias absolutas y relativas. Resultados : Se analizaron 74 casos con AE, 55,4% (n=41) fueron de sexo masculino. Según edad gestacional 45 a término, 28 pre-término y un post-término. El 59,5% tuvieron peso adecuado al nacer. 28,4% tuvieron control prenatal (CPN) completo y 68,9% no tuvieron diagnóstico prenatal de AE. La atresia con fistula traqueo-esofágica (FTE) distal fue la más frecuente con 74,3% (n=55). Se presentó sialorrea en 55 pacientes (74,3%) y disnea en 54 (72,9%). Predominaron las malformaciones cardiacas en el 27,1%, seguida por digestivas 20,3% y genitourinarias 17,6%. Solo 26 tuvieron síndromes asociados, VACTERL en 12 casos. La anastomosis termino-terminal y cierre de fistula se realizó en 55 pacientes (74,3%). 48 presentaron complicaciones tempranas, predominando sepsis (20,3%) y 46 complicaciones tardías, siendo reflujo gastroesofágico el más frecuente 29,7%. La mortalidad fue 10,8%. Conclusiones : La mayoría fue de sexo masculino, a término, con adecuado peso al nacer y sin CPN. La AE con FTE distal fue la más frecuente. Sialorrea y disnea fueron los síntomas predominantes y malformaciones cardiacas y digestivas las más asociadas. La anastomosis término-terminal y cierre de fístula fue la técnica más frecuente, como complicaciones sepsis y reflujo gastroesofágico. La mortalidad fue baja.

ABSTRACT Introduction. Infective Endocarditis (IE) is a disease caused by the invasion of pathogenic organisms into the cardiac endothelium. Objectives. To determine the clinical and epidemiological characteristics of Infectious Endocarditis in the Hospital Nacional Dos de Mayo, 2014-2019, Lima-Perú. Methods. Observational, descriptive and retrospective study. Patients older than 18 years with a diagnosis of IE according to modified Duke criteria, who had a complete blood count, blood culture, and echocardiogram were included. Results. 26 patients were studied; the average age was 46,96 ± 15,6 years, males predominated (57,7%), 15 came from Lima and 50% had completed secondary education as a degree. 57,7% (n = 15) had a negative blood culture, the most frequent symptoms were heart murmur (67,6%), fever (26%) and dyspnea (23,4%), the predominant complication was heart failure (61%). Comorbidities (47,1%) were the main risk factor. The mitral valve (51,7%) was the most compromised. The antibiotic regimen vancomycin + ceftriaxone and vancomycin + gentamicin, both with 19,2%, were the most used. 88% had erythrocyte distribution width (RDW) values > 14,4%. Of 10 who died, 9 had RDW > 14,4%. Conclusions. IE mainly affects males over 50 years of age with concomitant comorbidities. Positive echocardiography together with the Duke minor criteria were predominant. Most presented high RDW values that were related to the presence of complications and mortality, which was high.

RESUMEN Introducción. La endocarditis infecciosa (EI) es una enfermedad causada por la invasión de organismos patógenos al endotelio cardiaco. Objetivos. Determinar las características clínicas y epidemiológicas de la endocarditis infecciosa en el Hospital Nacional Dos de Mayo, 2014-2019, Lima-Perú. Métodos. Estudio observacional, descriptivo y retrospectivo. Se incluyeron a pacientes mayores de 18 años con diagnóstico de EI según criterios de Duke modificados, que contaban con hemograma, hemocultivo y ecocardiograma. Resultados. Se estudiaron 26 pacientes; la edad promedio fue 46,96 ± 15,6 años, predominó el sexo masculino (57,7%), 15 procedían de Lima y el 50% tenían como grado de instrucción secundaria completa. El 57,7% (n=15) tuvieron hemocultivo negativo, la sintomatología más frecuente fue soplo cardíaco (67,6%), fiebre (26%) y disnea (23,4%), la complicación predominante fue la insuficiencia cardíaca (61%). Las comorbilidades (47,1%) fueron el principal factor de riesgo. La válvula mitral (51,7%) fue la más comprometida. El régimen antibiótico vancomicina + ceftriaxona y vancomicina + gentamicina ambos con 19,2% fueron los más usados. El 88% presentaron valores de amplitud de distribución eritrocitaria (ADE) > 14,4%. De 10 que fallecieron, 9 tuvieron ADE > 14,4%. Conclusiones. La EI afectó principalmente a mayores de 50 años, de sexo masculino, con comorbilidades concomitantes. La ecocardiografía positiva junto a los criterios menores de Duke fueron predominantes. La mayoría presentó valores elevados de ADE que se relacionaron con la presencia de complicaciones y mortalidad, la cual fue alta.

Objectives. Determine the quality and variability of perinatal mortality (PM) records in Lima and Callao. Materials and methods. Key informants were interview to evaluate the processes. The reference for foetal deaths was taken from the obstetric registry and for early neonatal deaths as the neonatal registry. These were compared with records at a diverse range of levels in hospitals, Regional Directorates of Health (DISA) and Civil Registries (CR). The filling quality of death certificates and necropsy registries was also reviewed. Results. There were different and not well defined recording processes. The registration processes varied and were not well-defined. In the statistics offices of the hospitals there were recorded 2% more of fetal deaths. There was no discrepancy in foetal mortality registration in 3 hospitals using the Perinatal Information System; data for early neonatal death was only found in these hospitals and in the Instituto Especializado Materno Perinatal. Maternal perinatal section of the DISA and five hopsitals statistics offices recorded less than 9% fetal deaths and 23% of early neonatal deaths. The statistics office of the DISA only registered foetal deaths from 3 hospitals and did not register any early neonatal deaths. An underreported of 50% (-100% to +11.1%) of foetal deaths and 33.3% (-89.2 to +66.7%) of early neonatal deaths was found in the CR. In 70% of foetal death certificates the cause of death was recorded as "unknown". 29% of foetal deaths and 24,9% of early neonatal deaths were autopsied. Conclusions. There is an important discrepancy in the basic records between the DISA and CR, which suggests that the processes and tools must be standarized, establish a surveillance system to incorporate the use of Perinatal Information System and feedback information to reduce the unrecorded perinatal mortality.

Objetivos. Determinar la calidad y variaciones en los registros de mortalidad perinatal (MP) en Lima y Callao. Materiales y métodos. Se encuestó a informantes clave para evaluar el proceso de registro. Se compararon los registros de MP de diversos niveles en hospitales y las respectivas direcciones de salud (DISA) y registros civiles (RC) tomando como referencia para muertes fetales (MF) el registro de los servicios de obstetricia y para muertes neonatales precoces (MNP) los de neonatología. Se revisó la calidad del llenado de los certificados defunción y registros de necropsias. Resultados. Los procesos de registro son variados y no bien definidos. En la oficina de estadística de los hospitales se encontró un registro mayor de MF (+2%). En los tres hospitales que usaron el Sistema Informático Perinatal (SIP) no hubo discrepancia en los registros de MF; para MNP sólo se encontró datos en cuatro (tres usaban SIP) de los once hospitales evaluados. En las DISA el componente materno perinatal , registra menos de 9% de MF en cinco hospitales, y para MNP menos de 23%. La oficina de estadística de la DISA sólo registra MF en tres hospitales y no existen datos de MNP. En los RC se encontró un subregistro de 50% (-100% a +11,1%) para MF y de 33,3% (-89,2% a +66,7) para MNP. 70% de los certificados de defunción de MF registraron "desconocida" como causa de muerte. Sólo 29% de MF y 24,9% de MNP fueron necropsiadas. Conclusiones. Existe discrepancia en los registros de mortalidad perinatal entre las DISA y registros civiles, lo que sugiere la necesidad de estandarizar procesos e instrumentos, establecer un sistema de vigilancia, incorporar el SIP y retroalimentar la información para disminuir el subregistro de la mortalidad perinatal.
