Abstract Objective: To provide indicators to assess the impact on health, its social determinants and health inequalities from a social context and the recent economic recession in Spain and its autonomous regions. Methods: Based on the Spanish conceptual framework for determinants of social inequalities in health, we identified indicators sequentially from key documents, Web of Science, and organisations with official statistics. The information collected resulted in a large directory of indicators which was reviewed by an expert panel. We then selected a set of these indicators according to geographical (availability of data according to autonomous regions) and temporal (from at least 2006 to 2012) criteria. Results: We identified 203 contextual indicators related to social determinants of health and selected 96 (47%) based on the above criteria; 16% of the identified indicators did not satisfy the geographical criteria and 35% did not satisfy the temporal criteria. At least 80% of the indicators related to dependence and healthcare services were excluded. The final selection of indicators covered all areas for social determinants of health, and 62% of these were not available on the Internet. Around 40% of the indicators were extracted from sources related to the Spanish Statistics Institute. Conclusions: We have provided an extensive directory of contextual indicators on social determinants of health and a database to facilitate assessment of the impact of the economic recession on health and health inequalities in Spain and its autonomous regions.

Resumen Objetivo: Proporcionar indicadores para evaluar, en España y en sus comunidades autónomas, el impacto sobre la salud, sus determinantes sociales y las desigualdades en salud del contexto social y de la crisis económica más reciente. Métodos: Basándonos en el marco conceptual de los determinantes de las desigualdades sociales en salud en España, identificamos indicadores secuencialmente a partir de documentos clave, Web of Science y organismos con estadísticas oficiales. La información recopilada dio lugar a un directorio amplio de indicadores que fue revisado por un panel de expertos. Posteriormente seleccionamos un conjunto de esos indicadores según un criterio geográfico y otro temporal: disponibilidad de datos según comunidades autónomas y al menos desde 2006 hasta 2012. Resultados: Identificamos 203 indicadores contextuales sobre determinantes sociales de la salud y seleccionamos 96 (47%) según los criterios anteriores. De los indicadores identificados, el 16% no cumplieron el criterio geográfico y el 35% no cumplieron el criterio temporal. Se excluyó al menos un 80% de los indicadores relacionados con la dependencia y los servicios de salud. Los indicadores finalmente seleccionados cubrieron todas las áreas de los determinantes sociales de la salud. El 62% de estos no estuvieron disponibles en Internet. Alrededor del 40% de los indicadores se extrajeron de fuentes relacionadas con el Instituto Nacional de Estadística. Conclusiones: Proporcionamos un amplio directorio de indicadores contextuales sobre determinantes sociales de la salud y una base de datos que facilitarán la evaluación, en España y sus comunidades autónomas, del impacto de la crisis económica sobre la salud y las desigualdades en salud.

Objectives To determine the reliability and the content and construct validity of the Mackey Childbirth Satisfaction Rating Scale to measure women's satisfaction with labor and delivery. Methods The translation-back translation method was used to obtain an adapted version of the scale. For the validation study, a sample of 325 women who had delivered a healthy neonate through the vaginal route was selected. Data collection was performed using a self-administered questionnaire that included the modified scale, childbirth expectations, obstetric variables and sociodemographic characteristics. Reliability was assessed in a subsample of 45 women who completed the scale for the second time 15-30 days later. The factorial structure and internal consistency of the scale were analyzed. Construct validity was evaluated by analyzing the relationship between satisfaction with the experience, fulfillment of expectations and preferences regarding pain management. Results: In the test-retest study, an intraclass correlation coefficient of 0.93 was obtained for the total scale. Factor analysis identified six factors that explained 69.42% of the variance. Cronbach's alpha coefficient was 0.94 for the total scale, ranging from 0.72 to 0.96 for the subscales. Satisfaction with the experience of childbirth was higher among women whose expectations were met and among those who accessed the pain relief method chosen. Conclusions: This scale is a useful instrument to measure women's satisfaction with the experience of childbirth, adapted to our context and with good psychometric properties.

Objetivos: Determinar la fiabilidad y validez de contenido y concepto de la Mackey Childbirth Satisfaction Rating Scale, escala para medir la satisfacción de las mujeres con el parto y el nacimiento. Métodos: Se utilizó el método de traducción-retrotraducción para obtener una versión adaptada de la escala. Para la validación se seleccionó una muestra de 325 mujeres que habían tenido un hijo sano mediante parto vaginal. La recogida de datos se realizó con un cuestionario autocumplimentado que incluía la escala adaptada, expectativas ante el parto y variables obstétricas y sociodemográficas. Se valoró la fiabilidad en una submuestra de 45 mujeres que cumplimentaron de nuevo la escala a los 15-30 días. Se analizó la estructura factorial y la consistencia interna. Se evaluó la validez de concepto analizando la relación entre la satisfacción con la experiencia y el cumplimiento de las expectativas y preferencias en el manejo del dolor. Resultados: En el estudio test-retest se obtuvo un coeficiente de correlación intraclase de 0,93 para la escala global. El análisis factorial identificó seis factores que explicaban el 69,42% de la varianza. El coeficiente alfa de Cronbach fue de 0,94 para la escala global, oscilando entre 0,72 y 0,96 en las subescalas. La satisfacción con la experiencia del parto fue superior en las mujeres cuyas expectativas se habían cumplido, así como en las que accedieron al método de alivio del dolor elegido. Conclusiones: Se ha obtenido un instrumento de medida de la satisfacción con la experiencia del parto y el nacimiento, adaptado a nuestro contexto y con buenas características psicométricas.

Objectives: To describe the prevalence of intimate partner violence and non-partner violence against women in Spain overall and by autonomous regions, as well as the characteristics of this violence, and to study the association between the prevalence of intimate partner violence and the frequency of formal complaints and deaths. Methods: A cross-sectional survey was performed through a self-administered questionnaire in 11,475 women attending primary care from 2006 to 2007. Multistage stratified sampling was carried out in a representative sample in each autonomous region. Data on intimate partner and non-intimate partner violence, formal complaints to the police and deaths due to intimate partner violence were gathered. A descriptive analysis was conducted. Correlations between the prevalence of intimate partner violence and police reports and deaths from this cause were determined through Pearson's correlation coefficient. Results: A total of 24.8% (23.4& - 26.3%) of the sample reported having experienced intimate partner violence at some time. The highest prevalence of intimate partner violence was found in Ceuta and Melilla (40.2%) and the Balearic Islands (32.5%) and the lowest in Cantabria (18%). Fifteen percent of the sample reported having experienced non-partner violence at some time, and the most frequent type was that committed by a relative. Again, the highest prevalence of non-partner violence was found in Ceuta and Melilla and the Balearic Islands and the lowest in Cantabria. A significant positive linear correlation was found between intimate partner violence in the previous year and the rate of reports of intimate partner violence in the autonomous regions (r=0.398; p=0.005). Conclusions: The prevalence of intimate partner violence varies from one autonomous region to another. The reasons for this variation should be further explored in future studies.

Objetivos: Describir la prevalencia global y por comunidades autónomas (CC.AA.) de la violencia por un compañero íntimo, sus características y la violencia fuera de la pareja, así como estudiar la relación entre la prevalencia de violencia por un compañero íntimo y la frecuencia de denuncias y muertes por este motivo. Métodos: Encuesta transversal mediante cuestionario autoadministrado en 11.475 mujeres que acuden a consultas de atención primaria durante los años 2006-2007. Diseño muestral estratificado y polietápico, con una muestra representativa por CA. Además de la violencia por un compañero íntimo y la violencia fuera de la pareja, se recogieron denuncias y muertes por la primera. Se realizó un análisis descriptivo. Las correlaciones de la violencia por un compañero íntimo con las denuncias y muertes por ésta se determinaron mediante el coeficiente de correlación de Pearson. Resultados: El 24,8% (23,4& -26,3%) de las mujeres refirieron haber sido maltratadas por su pareja alguna vez en la vida. Las prevalencias más altas de violencia por un compañero íntimo se recogieron en Ceuta y Melilla (40,2%) y las Islas Baleares (32,5%), y la más baja en Cantabria (18%). El 15,2% refiere haber sufrido violencia fuera de la pareja, siendo la recibida de un familiar la más prevalente. De nuevo, en Ceuta y Melilla e Islas Baleares se recogieron las prevalencias de violencia fuera de la pareja más elevadas, y la más baja en Cantabria. Hay una correlación lineal significativa entre la violencia por un compañero íntimo en el último año y la tasa de denuncias en las CC.AA. (r=0,398; p=0,005). Conclusiones: La violencia por un compañero íntimo muestra variabilidad entre las CC.AA., y en un futuro deberían abordarse las causas de ello con mayor profundidad.

Objective: To determine the prevalence of reported intimate partner violence (IPV) and to analyze the main sociodemographic characteristics of affected women. Methods: We performed a cross-sectional study based on the Spanish National Health Survey of 2006. The sample comprised 13,094 women who agreed to answer questions about violence (87.2% of the total interviewees). Bivariate and multivariate analyses were performed. The dependent variable was reported IPV and the independent variables were educational level, employment, marital status, living arrangements with the partner or analogous individual, number of children at home, nationality (Spanish vs. foreign women) and age. Results: IPV was reported by 1% (n=128) of the sample. Women with primary school education or without studies (odds ratio [OR]: 3.63 [1.90-6.92]), with three or more children (OR: 3.51 [1.78-6.90]), and those who were separated or divorced (OR: 2.81 [1.89-4.97]) were most likely to experience IPV when the effect of the remaining variables was controlled. The likelihood of IPV was also higher in women born outside Spain (OR: 2.83 [1.87-4.28]). Conclusions: IPV seems not to affect Spanish and foreign women equally. The characteristics most closely associated with women affected by IPV were educational level, the number of children at home and marital status. The sensitivity of current measures against IPV should be considered in relation to the needs of affected women.

Objetivo: Determinar la prevalencia de la violencia del compañero íntimo declarada y analizar sus principales características sociodemográficas. Métodos: Estudio transversal basado en la Encuesta Nacional de Salud de España de 2006 (ENS-06). La muestra son las 13.094 mujeres dispuestas a responder preguntas sobre malos tratos (87,2% del total de las encuestadas). Se realizó un análisis bivariado y multivariado, siendo la variable dependiente la violencia del compañero íntimo declarada y las independientes el nivel de estudios, el empleo, el estado civil, la situación de convivencia con la pareja o análogo, el número de menores en el hogar, el país de origen (españolas frente a extranjeras) y la edad. Resultados: La violencia del compañero íntimo es reconocida por un 1% (n=128) de la muestra. La violencia declarada se asocia con tener estudios primarios o no tener estudios (odds ratio [OR]: 3,63 [1,90-6,92]), tres o más menores (OR: 3,51 [1,78-6,90]) y estar separada o divorciada (OR: 2,81 [1,89-4,97]), una vez controlado el efecto del resto de las variables. La violencia del compañero íntimo declarada es mayor entre las mujeres nacidas fuera de España (OR: 2,83 [1,87-4,28]). Conclusiones: La violencia del compañero íntimo parece manifestarse de manera desigual entre las mujeres españolas y las extranjeras. El nivel educativo, el número de menores en el hogar y el estado civil son las características que más se asocian a la violencia declarada por las mujeres. Sería pertinente reflexionar sobre la sensibilidad de las medidas existentes contra la violencia del compañero íntimo en relación a las necesidades de las mujeres afectadas.

Objectives: To describe the prevalence of burnout syndrome according to medical specialty and to examine the impact of work psychosocial risk factors, job satisfaction and professional characteristics on burnout syndrome among specialist physicians throughout Spain. Methods: A cross-sectional survey was carried out among 1,021 Spanish physicians. The outcome variables were the 3 dimensions of burnout syndrome: emotional exhaustion, depersonalization, and personal accomplishment. The explanatory variables were work psychosocial risk factors and job satisfaction evaluated by a stress scale specifically designed for physicians. Adjusted odds ratios and their 95% confidence intervals were calculated by logistic regression. Results: The probability of high emotional exhaustion and depersonalization were greater in physicians exposed to a high level of contact with suffering and death and to a negative impact of work on home life. The probability of high emotional exhaustion was greater among physicians with a high work overload. The risk of low personal accomplishment was higher among physicians with low professional satisfaction and those without training activities. Dissatisfaction with relationships with patients and relatives had a negative effect on the 3 dimensions of burnout. Conclusions: Psychosocial work environment and job satisfaction have a negative effect on burnout syndrome, especially on emotional exhaustion and depersonalization.

Objetivos: Describir la prevalencia de síndrome de burnout según el tipo de especialidad médica, así como valorar el impacto de los factores de riesgo psicosocial, la satisfacción laboral y las características profesionales sobre el síndrome de desgaste profesional en el personal médico especialista en todo el Estado español. Métodos: Estudio transversal realizado sobre una muestra de 1.021 médicos especialistas. Las variables respuesta son las 3 dimensiones del síndrome de burnout: cansancio emocional, despersonalización y logros personales. Las variables explicativas son los factores de riesgo psicosocial y las fuentes de satisfacción laboral, valorados por una escala específica diseñada para médicos. Se han calculado las odds ratio ajustadas y sus intervalos de confianza del 95% por regresión logística. Resultados: La probabilidad de presentar un elevado cansancio emocional y despersonalización es mayor en los profesionales expuestos a un alto nivel de contacto con el sufrimiento y la muerte, y conlleva un impacto negativo del trabajo en la vida familiar. La probabilidad de presentar un elevado cansancio emocional es mayor en los que tienen una alta sobrecarga de trabajo. El riesgo de obtener bajos logros personales es mayor en las personas que presentan una baja satisfacción con respecto a las recompensas profesionales y en las que no realizan actividades de docencia. La insatisfacción con la calidad de las relaciones con los pacientes y sus familiares influye negativamente en las 3 dimensiones del burnout. Conclusiones: El ambiente psicosocial y la satisfacción laboral influyen negativamente en el síndrome de burnout, y afectan a las dimensiones de cansancio emocional y despersonalización.

The lack of homogeneous and reliable epidemiological data on domestic violence greatly limits public decision making on the help that should be provided to victims of this form of abuse. Health professionals are obliged to report cases of domestic violence in adults to the judicial authorities and a unified, easily completed model for reporting injuries from domestic violence has been established in the Autonomous Community of Valencia. From June to October 2005, 500 cases of domestic violence were received and processed, most of which were reported by primary care physicians (68%). Cases of domestic violence occurred mainly in young women (aged, 20-39 years), either married or cohabiting, and with primary or secondary level education. The most frequent findings were physical injury or psychological damage (62%). Eighty-three percent of victims had at least one antecedent of abuse.

La ausencia de datos epidemiológicos homogéneos y fiables sobre la violencia doméstica constituye una limitación importante para la toma de decisiones públicas en materia de ayuda a estas víctimas. Por ello, y partiendo de la obligatoriedad del personal sanitario de declarar los casos de violencia doméstica en adultos ante la autoridad judicial, en la Comunidad Valenciana se ha establecido un modelo unificado, de fácil cumplimentación, para declarar estas lesiones. Desde junio a octubre de 2005 se han recibido y procesado 500 casos de violencia doméstica, emitidos mayoritariamente por médicos de atención primaria (68%). Los casos se dan sobre todo en mujeres, en jóvenes (edad entre 20 y 39 años), con estado civil de casados o convivientes en pareja y con estudios primarios o secundarios. Objective: To determine the workplace stress factors perceived by the nurses, both male and female, who work in those areas of the hospital concerned with organ transplants and the sources of professional job satisfaction. Methods: Qualitative study, carried out with six discussion groups (64 participants in all) of male and female nurses, who work in different Spanish state run hospitals. Their conversations were recorded and then transcribed, and an analysis of the discourse was undertaken. Results: The main stress factors which were identified relate to: work overload and the time pressures, contact with death and suffering, a lack of support from colleagues and other health professionals, and the problems of inter-relationship with patients and their relatives. Likewise there is clearly a lack of sufficient information to be able to respond to the questions posed by the patients and family members as to their clinical state and their prognosis. Another stress factor is the lack of specific information relating to the characteristics of necessary care for the patient pre and post transplant. The principal source of professional job satisfaction is the thanks for the care which has been afforded to them, from patients and their families. Conclusion: The hospital nursing staff is subjected to a large number of stress factors in the course of its work, the majority of which are intrinsic to its profession.

Objetivo: Determinar los estresores laborales percibidos por las enfermeras que trabajan en áreas dedicadas al trasplante de órganos y las fuentes de recompensa profesional. Participantes y métodos: Estudio cualitativo, realizado con seis grupos de discusión (64 participantes en total) de enfermeras, que trabajaban en distintos hospitales del Estado. Las conversaciones fueron grabadas, transcritas y se realizó un análisis del discurso. Resultados: Los principales estresores identificados son los relacionados con la sobrecarga de trabajo, la presión de tiempo, el contacto con la muerte y sufrimiento, la falta de apoyo por parte de los compañeros y otros profesionales sanitarios, los problemas de interrelación con los pacientes y sus familiares. Se evidencia una falta de información adecuada para dar respuesta a las preguntas formuladas por los pacientes y familiares, sobre su estado clínico y pronóstico. Así como la falta de formación específica para prestar los cuidados necesarios al paciente pre y trasplantado. La fuente de recompensa profesional es el agradecimiento, por los cuidados prestados, mostrado por los pacientes y sus familiares. Conclusión: El personal de enfermería hospitalario está sometido a un gran número de estresores en su trabajo, la mayoría intrínsecos a su profesión.
