Prevalencia del desarrollo de entesofito en la Protuberancia Occipital Externa en población de paraguayos

RESUMEN Introducción. La Protuberancia Occipital Externa (POE) es una superficie convexa y lisa que tradicionalmente no presenta ningún tipo de irregularidades. La aparición de un entesofito en individuos ha llamado la atención de los investigadores. Objetivo. Determinar la prevalencia, longitud, edad y sexo de una exostosis en forma de gancho desarrollada a nivel de la POE. Metodología. Estudio observacional descriptivo realizado en radiografías laterales de cráneo de individuos jóvenes. De 5065 radiografías, 4017 radiografías cumplieron con los criterios. Se procedió a la detección de la presencia de un gancho óseo en la POE y su medición. Resultados. Se incluyeron personas de ambos sexos entre 13 a 45 años: 2902 mujeres y 1115 varones. De las 4017 radiografías observadas, el 67% presentó la excrecencia en la POE, con rangos desde 5 a 24 mm en longitud, 511 tenían una longitud de 5 mm o mayor, 903 entre 9 a 12 mm de longitud y 253 de 13 a 24mm. Se observó en mayor proporción en el sexo masculino que el femenino (68% vs 32%). En los individuos de 40 años y más, solo un paciente de sexo masculino mostró la presencia del entesofito. Conclusión. Se ha encontrado una alta frecuencia del desarrollo de una protuberancia ósea a nivel del POE en jóvenes y adultos jóvenes de sexo masculino. Este es el primer estudio de este tipo realizado en la población del Paraguay. Para comprender la razón del desarrollo de esta excrecencia ósea, es necesario ampliar el área de investigación.
ABSTRACT Introduction. The external occipital protuberance (EOP) is a smooth convex surface which commonly does not present any type of irregularities. The development of an enthesophyte in young population has attracted the attention of researchers. Objective. To determine the prevalence, size, age, and sex of individuals with bony projections that resembles a hook developed in EOP. Methodology. Descriptive observational study. 5065 lateral skull x-rays were observed. 4017 fulfilled all the criteria. The x-rays were reviewed for the presence of a bony excrescence in EOP, and they were measured. Results. From a both sexes population between 13 to 45 years old 2902 were female and 1115 were male. 67% X-rays presented the enthesophyte ranging from 5 - 24 mm in length. 511 out of 1667 had an enthesophyte at or above 5 mm. There were 903 patients with an enthesophyte ranging from 9 - 12 mm in length and 254 had the protuberance ranging from 13 - 24 mm. Statistically significant range of 68% vs 32% between males and females were observed. From individuals of 40 years and above, just 1 male patient out of 32 showed the enthesophyte. Conclusion. Significant association with the development of a bony protuberance at the POE in young and young adult male population has been found. This is the first study of this type in Paraguayan individuals. To expand the field of studies, future research is needed to better examine and understand the reason for the development of this bony excrescence.

Abstract Therapeutics that inhibit enzymes, receptors, ion channels, and cotransporters have long been the mainstay of cardiovascular medicine. Now, oligonucleotide therapeutics offer a modern variation on this paradigm of protein inhibition. Rather than target a protein, however, small interfering ribonucleic acids and antisense oligonucleotides target the messenger RNA (mRNA) from which a protein is translated. Endogenous, cellular mechanisms enable the oligonucleotides to bind a selected sequence on a target mRNA, leading to its degradation. The catalytic nature of the process confers an advantage over the stoichiometric binding of traditional small molecule therapeutics to their respective protein targets. Advances in nucleic acid chemistry and delivery have enabled development of oligonucleotide therapeutics against a wide range of diseases, including hyperlipidemias and hereditary transthyretin-mediated amyloidosis with polyneuropathy. While most of these therapeutics were initially designed for rare diseases, recent clinical trials highlight the potential impact of oligonucleotides on more common forms of cardiovascular disease.

RESUMO Antecedentes: Amiloidose hereditária por transtirretina (hATTR) com polineuropatia é uma doença rara, hereditária, multissistêmica, frequentemente fatal, causada por mutação no gene da transtirretina (TTR). Características dos pacientes ao diagnóstico, especialmente dados antropométricos, são raros na literatura, e são relevantes para definir estratégias terapêuticas eficazes. Objetivo: Este estudo objetivou descrever características demográficas, antropométricas, e da doença, numa coorte de pacientes de um centro de referência no Brasil. Métodos: Pacientes sintomáticos, não incluídos previamente em ensaios clínicos e elegíveis para tratamento foram recrutados. Etnia, estado de residência, idade, sexo, peso, altura, índice de massa corporal (IMC), mutação TTR e Polyneuropathy Disability Score (PND) ao diagnóstico foram analisados. Resultados: Foram incluídos 108 pacientes, sendo 58,33% do sexo masculino e 60,19% caucasianos. Na região sudeste residem 83,33%. A idade média foi 51,61 (±16,37) anos. O peso médio foi 65,76 (±15,16) Kg, a altura média foi 168,33 (±10,26) cm e o IMC médio foi 23,11 (±4,45) Kg/m2. A mutação mais prevalente foi V30M (86,11%). Pacientes com escore PND 0 apresentando neuropatia autonômica foram 14,81%. Pacientes PND I-II e PND III-IV totalizaram 52,78% e 32,41%, respectivamente. Peso e IMC médios foram significativamente menores em pacientes com manifestações sensitivo-motoras. Conclusões: Esta é a maior coorte de pacientes do Brasil que teve dados antropométricos descritos. Dados basais demográficos, antropométricos e das manifestações da doença indicam que o atraso no diagnóstico ainda é um problema, e esforços precisam ser feitos para acelerá-lo a fim de maximizar as oportunidades com os novos medicamentos que alteram o curso da doença.
ABSTRACT Background: Hereditary transthyretin-mediated (hATTR) amyloidosis with polyneuropathy is a rare, inherited, multisystem, and often fatal disease caused by a variant in transthyretin (TTR) gene. Baseline characteristics of patients, especially anthropometric data, are scarce in the literature, and they are relevant to define effective treatment strategies. Objective: This study aimed to describe baseline demographic, anthropometric, and disease characteristics in a cohort of patients from a reference center in Brazil. Methods: Symptomatic patients not previously included in clinical trials and eligible for treatment were enrolled. Ethnicity, state of residence, age, sex, weight, height, body mass index (BMI), TTR variant, and Polyneuropathy Disability Score (PND) at diagnosis were analyzed. Results: Among the 108 patients enrolled, 58.33% were male, 60.19% were Caucasian, and 83.33% lived in the Southeast region. Mean age was 51.61 (±16.37) years, mean weight was 65.76 (±15.16) kg, mean height was 168.33 (±10.26) cm, and mean BMI was 23.11 (±4.45) kg/m2. The most prevalent variant was V30M (86.11%). Patients with PND score 0 presenting autonomic neuropathy were 14.81%. Patients with PND score I-II and III-IV were 52.78 and 32.41%, respectively. Mean weight and BMI were significantly lower in patients with sensory-motor manifestations. Conclusions: This is the largest cohort of patients in Brazil for whom anthropometric characteristics have been described. Baseline demographic, anthropometric, and disease data indicate that delay in diagnosis of hATTR amyloidosis with polyneuropathy is still a problem and that efforts must be made to expedite diagnosis and maximize opportunities for new disease-modifying treatments.

Resumo Objetivos: Aferir o desenho geométrico, características metalúrgicas e desempenho mecânico dos instrumentos réplica (réplica-like) da lima SX do sistema ProTaper Gold (PTG SX). Métodos: A presente investigação envolveu a avaliação do desenho geométrico, proporções de Ti/Ni, temperaturas de transformação de fase, resistência a torção e flexibilidade de 2 instrumentos réplica SX (Premium Taper Gold e Go-Taper Flex) e da lima original PTG SX. Os grupos foram comparados usando os testes ANOVA unidirecional com post hoc Tukey ou Kruskal-Wallis dependendo do tipo de distribuição (gaussiana ou não-gaussiana) dos resultados (teste de Shapiro-Wilk). A significância estatística foi definida a 0,05. Resultados: Os 3 instrumentos SX apresentavam 9 lâminas, ângulos de hélice de ~21o, lâminas simétricas sem radial lands, desenho seccional em triângulo convexo, e proporções quase equiatómicas de Ti/Ni. Foram observadas diferenças nas pontas dos instrumentos. A analise do acabamento superficial mostrou uma superfície mais lisa na Premium Taper Gold e uma mais irregular na Go-Taper Flex. Foram observadas distintas temperaturas de transformação da fase R (Rs inicial, Rf final), entre os instrumentos PTG (Rs~48oC e Rf~30oC), Go-Taper Flex (Rs~43oC e Rf~25oC) e Premium Taper Gold (Rs~30oC e Rf~15oC). Relativamente ao desempenho mecânico, a Go-Taper Flex apresentou menor torque máximo (0,5 N.cm) e maior resistência a flexão (582,2 gf) quando comparada com a PTG (0,8 N.cm e 447,1 gf) (P<0,05). Não foram identificadas diferenças entre a Premium Taper Gold e a PTG (P>0,05). Conclusões: Na globalidade, nenhuma das réplicas foi exatamente igual ao instrumento PTG SX original.
Abstract Objectives: To assess the characteristics, performance, and safety of ProTaper Gold SX (PTG SX) replica-like instruments. Methods: A multimethod research involving the assessment of geometric design (macro and through scanning electron microscope), Ti/Ni proportions (energy-dispersive X-ray spectroscopy), phase transformation temperatures (differential scanning calorimetry), torsional resistance, and flexibility was conducted to compare two SX replica-like instruments (Premium Taper Gold and Go-Taper Flex) with the original PTG SX. Results were compared using one-way ANOVA with post hoc Tukey or Kruskal-Wallis tests according to gaussian or non-gaussian distribution (Shapiro-Wilk test). The significance level was set at 0.05. Results: The three SX instruments have nine cutting blades with helix angles of approximately 21o, symmetrical blade geometry without radial lands, a convex triangular cross-section design, and near equiatomic Ti/Ni atomic proportions. Design differences were observed at the tip geometry and surface finishing, which was smoother in the Premium Taper Gold and irregular in the Go-Taper Flex. Distinct R-phase start (Rs) and finish (Rf) phase transformation temperatures were noted between PTG (Rs ~48oC and Rf ~30oC), Go-Taper Flex (Rs ~43oC and Rf ~25oC), and Premium Taper Gold (Rs ~30oC and Rf ~15oC). In the mechanical tests, Go-Taper Flex presented lower maximum torque (mean 0.5 N.cm) and higher bending resistance (mean 582.2 gf) (less flexibility) than PTG (means 0.8 N.cm cliniand 447.1 gf) (P<0.05). No significant differences were observed in the mechanical tests between Premium Taper Gold and PTG (P>0.05). Conclusions: Overall, the tested replica-like systems showed different features compared to the original PTG SX instrument.

OBJECTIVES: To compare demographic/clinical/laboratory/treatments and outcomes among children and adolescents with laboratory-confirmed coronavirus disease 2019 (COVID-19). METHODS: This was a cross-sectional study that included patients diagnosed with pediatric COVID-19 (aged <18 years) between April 11, 2020 and April 22, 2021. During this period, 102/5,951 (1.7%) of all admissions occurred in neonates, children, and adolescents. Furthermore, 3,962 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection samples were processed in patients aged <18 years, and laboratory-confirmed COVID-19 occurred in 155 (4%) inpatients and outpatients. Six/155 pediatric patients were excluded from the study. Therefore, the final group included 149 children and adolescents (n=97 inpatients and 52 outpatients) with positive SARS-CoV-2 results. RESULTS: The frequencies of sore throat, anosmia, dysgeusia, headache, myalgia, nausea, lymphopenia, pre-existing chronic conditions, immunosuppressive conditions, and autoimmune diseases were significantly reduced in children and adolescents (p<0.05). Likewise, the frequencies of enoxaparin use (p=0.037), current immunosuppressant use (p=0.008), vasoactive agents (p=0.045), arterial hypotension (p<0.001), and shock (p=0.024) were significantly lower in children than in adolescents. Logistic regression analysis showed that adolescents with laboratory-confirmed COVID-19 had increased odds ratios (ORs) for sore throat (OR 13.054; 95% confidence interval [CI] 2.750-61.977; p=0.001), nausea (OR 8.875; 95% CI 1.660-47.446; p=0.011), and lymphopenia (OR 3.575; 95% CI 1.355-9.430; p=0.010), but also had less hospitalizations (OR 0.355; 95% CI 0.138-0.916; p=0.032). The additional logistic regression analysis on patients with preexisting chronic conditions (n=108) showed that death as an outcome was significantly associated with pediatric severe acute respiratory syndrome (SARS) (OR 22.300; 95% CI 2.341-212.421; p=0.007) and multisystem inflammatory syndrome in children (MIS-C) (OR 11.261; 95% CI 1.189-106. 581; p=0.035). CONCLUSIONS: Half of the laboratory-confirmed COVID-19 cases occurred in adolescents. Individuals belonging to this age group had an acute systemic involvement of SARS-CoV-2 infection. Pediatric SARS and MIS-C were the most important factors associated with the mortality rate in pediatric chronic conditions with COVID-19.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

Objetivos: Determinar a prevalência da síndroma de fragilidade na população da região Norte de Portugal Continental. Tipo de estudo: Estudo observacional, descritivo e transversal. Local: USF Nova Era, USF Veiga do Leça, USF Cristelo, USF São Miguel-O-Anjo. População: Utentes nascidos antes de 1952 à data da elaboração do estudo. Métodos: Foi selecionada uma amostra de conveniência de 408 participantes da população de utentes, nascidos antes de 1952, que se encontravam inscritos nas quatro unidades em estudo (N=7.189) em março de 2016. O número de participantes foi distribuído pelas quatro unidades, de acordo com o seu peso relativo na população [USF Nova Era (132), USF Veiga do Leça (94), USF Cristelo (86) e USF São Miguel-O-Anjo (96)]. Aplicou-se um questionário demográfico, o questionário PRISMA-7 e o Gait Speed Test (para uma distância de quatro metros). Para a análise estatística utilizou-se o programa SPSS®, v. 20. Resultados: A média de idades foi de 74,3 ± 6,6, dos quais 54,7% eram mulheres. A maioria era casada (64,6%) e com o primeiro ciclo de escolaridade (46,6%). A prevalência da síndroma de fragilidade foi de 34,5% e a prevalência de participantes pré-frágeis foi de 18,3% para a amostra em estudo. A prevalência da síndroma de fragilidade aumenta com a idade, na população não casada e na população que tem menos de quatro anos de escolaridade. Conclusão: Os resultados obtidos da prevalência da síndroma de fragilidade são superiores aos referidos na literatura europeia e norte-americana, mas semelhantes aos descritos nos países do Sul da Europa.

A demographic questionnaire, the PRISMA-7 questionnaire and the Gait Speed Test (for a distance of four meters) were applied. Statistical analysis was performed using SPSS® v. 20. Results: The mean age was 74.3 ± 6.6, and 54.7% of the participants were women. Most were married (64.6%) and had the first cycle of schooling (46.6%). The prevalence of frailty syndrome was 34.5%, and the prevalence of pre-frailty was 18.3% for the study sample. The prevalence of frailty syndrome increases with age, in the unmarried population, and in those with less than four years of schooling. Conclusion: The prevalence of frailty syndrome is higher than the values reported in European and North American literature, but similar to those described in Southern European countries.

O priapismo isquémico é uma ereção persistente, sem relação com estímulo sexual, caracterizada pela redução ou ausência de fluxo sanguíneo intracavernoso. Apresenta-se um caso clínico de um homem de 30 anos, com priapismo com 48 horas de evolução. Foi submetido noutra instituição à drenagem dos corpos cavernosos, instilação de efedrina e criação de shunt cavernoso-esponjoso, sem melhoria do quadro. Ao ecoDoppler observou-se ausência de fluxo nas artérias cavernosas, trombose das veias cavernosas e fluxo venoso dorsal normal. Foi submetido a cirurgia de Grayhack, com construção de pontagens cavernoso-femoral bilateral por transposição segmentar da veia grande safena. Apresentou boa evolução clínica e resolução do priapismo. No 7.◦ dia de pós-operatório apresentou trombose das pontagens, com fluxo de baixa resistência nas artérias cavernosas.
Ischemic priapism is a persistent erection unrelated to sexual stimulation characterized by reduced or absent intracavernous blood flow. We present a clinical case of a 30 year old man with priapism with 48 hours of progression. He underwent surgical drainage of the corpora cavernosa, instillation of ephedrine, and creation of a cavernous-spongeous shunt without improvement. On the duplex ultrasound there was no flow in the cavernous arteries, thrombosis of the cavernous veins and normal dorsal venous flow. He underwent Grayhack surgery with creation of cavernous-femoral bypass with great saphenous vein bilaterally. He underwent a Grayhack surgery with creation of a bilateral cavernous-femoral bypass with great saphenous vein. There was clinical improvement and resolution of the priapism. On the 7th post-operative day, it was documented thrombosis of the bypasses and patent cavernous arteries and veins.

Os aneurismas da artéria esplénica são, de todos os aneurismas viscerais, os mais frequentes, com uma taxa de rotura até 10%. Não obstante, o seu diagnóstico é normalmente acidental e o seu tratamento eletivo está preconizado para diâmetros > 2,5 cm. Outrora tratado exclusivamente por cirurgia aberta, através de resseção de aneurisma com ou sem reconstrução arterial, com ou sem esplenectomia associada, também aqui o tratamento endovascular, através da embolização com coils ou stenting da artéria esplénica, tem sido crescentemente utilizado. Os estudos comparativos reportam eficácia sobreponível, com menor morbimortalidade perioperatória, mas maior taxa de complicações tardias. Dentro destas, estão o enfarte e o abcesso esplénico, que nos propusemos estudar e descrever com este trabalho.
Splenic artery aneurysms are the most common of all visceral artery aneurysms, with a risk of rupture up to 10%. Most patients present asymptomatically and its elective treatment is recommended for diameters> 2.5 cm. Once treated exclusively by open surgery through aneurysm resection with or without arterial reconstruction, with or without splenectomy associated, nowadays endovascular repair has become increasingly used, by embolization with coils or stenting of the splenic artery. Comparative studies report overlapping efficacy, with lower perioperative morbidity and mortality, but higher rate of late complications. Splenic infarction and abscess is one of them, and the goal of our study was to study and describe this complication.

Introdução: É frequente uma resposta inflamatória sistémica após EVAR. O seu impacto clínico é desconhecido, e apesar de habitualmente ser bem tolerado existe o receio de estar associado a maior morbi-mortalidade em doentes de alto risco. Este estudo tem como objectivo a avaliação da ocorrência do síndrome pós implantação (SPI) nos doentes submetidos a EVAR, suas características e relevância clínica. Material e métodos: Estudo observacional retrospectico de doentes submetidos a EVAR electivo entre Novembro de 2012 e Novembro de 2014. O SPI foi definido por febre (>38 ◦C) e leucocitose (>12.000/µL), excluídas complicações infecciosas. O outcome primário foi o diagnóstico de SPI. Foram avaliadas as características epidemiológicas do doente, características do aneurisma e do procedimento e sua relação com o SPI. Resultados: Foram incluídos 52 doentes, dos quais 21,2% desenvolveram SPI. As endopróteses de ePTFE não foram associadas à ocorrência de SPI, comparativamente às de poliéster (0% vs.28,2%, p = 0.031). A idade e o sexo dos pacientes, o diâmetro do aneurisma, o tempo e a dose de radiação e a configuração da endoprótese (aorto-bi-ilíaca, aorto-uni-ilíaca ou fenestrada) não foram associados à ocorrência do síndrome. Não se observaram diferenças estatisticamente significativas na ocorrência de eventos cardiovasculares major durante o internamento em ambos os grupos. Conclusão: O SPI ocorre numa percentagem significativa de doentes (21%). As endopróteses construídas com poliéster são um factor de risco significativo. Apesar da resposta inflamatória exuberante, esta não se associou à ocorrência de eventos cardiovasculares, sendo habitualmente benigna, bem tolerada e auto-limitada.
Introduction: A systemic inflamatory response is common after EVAR. Its clinical impact is unknown, and although is it usually well tolerated, there is concern it might be associated with increased morbidity and mortality in high risk patients. This study aims to evaluate the occurrence of the post-implantation syndrome (PIS) in patients undergoing EVAR, its characteristics and clinical significance. Methods: This study is a retrospective observational analysis of patients undergoing elective EVAR between November 2012 and November 2014. PIS was defined by fever (>38 ◦C) and leukocytosis (>12,000 µL−1), excluding infectious complications. We evaluated the epidemiological characteristics of the patient, aneurysm and procedure characteristics and their relationship with development of PIS. Results: Fifty-two patients were included, and 21.2% were diagnosed with PIS. The ePTFE grafts were not associated with the occurrence of the syndrome, in contrast with polyester stent grafts (0% vs. 28.2%, p = 0.031). The age and gender of patients, the diameter of the aneurysm, duration and radiation dose and the configuration of the stent graft (aorto-bi-iliac, aorto-uni-iliac or fenestrated) were not associated with PIS. There was no statistically significant difference in the occurrence of major cardiovascular events during hospitalization in both groups. Conclusion: The inflammatory syndrome after EVAR occurs in a significant percentage of patients (21%). Stent grafts constructed by polyester are a significant risk factor. Despite the exuberant inflammatory response, it was not associated with increased occurrence of cardiovascularevents, and it is usually benign, well tolerated and self-limiting.

Introdução: Os aneurismas e as fístulas arteriovenosas (FAV) da artéria renal são entidades raras, geralmente assintomáticas, cujas manifestações clínicas podem ser variadas. Os falsos aneurismas da artéria renal estão normalmente associados a traumatismo prévio e apresentam maior tendência a rutura. A indicação de tratamento destas lesões não é consensual, assim como o método a ser utilizado. Material e métodos: Neste trabalho, os autores procederam à análise retrospetiva dos doentes com diagnóstico de falso aneurisma da artéria renal tratados na instituição, no período de 2011-2015, incluindo manifestação clínica, diagnóstico, tratamento e seu resultado. Resultados: Foram tratados 6 doentes, 4 do sexo masculino e 2 do sexo feminino, com idade média de 59 anos. A manifestação clínica inaugural foi hematúria macroscópica em 4 doentes, instabilidade hemodinâmica em um doente e queda do valor de hemoglobina em um doente. Em todos os casos, o diagnóstico foi feito por tomografia computadorizada (TC) e confirmado por angiografia, que foi terapêutica no mesmo ato. Conclusão/Discussão: Em todos os casos havia história recente de algum procedimento urológico, podendo ser consideradas estas lesões iatrogénicas. Com o aumento do número de intervenções urológicas semelhantes às referidas neste trabalho, é expectável um aumento da incidência de lesões da artéria renal ou seus ramos, entre as quais se destacam os falsos aneurismas. Tratando-se na maioria dos casos de lesões intraparenquimatosas, envolvendo vasos de pequeno calibre, a cirurgia convencional tem poucas alternativas terapêuticas além da nefrectomia. Assim, o tratamento endovascular é o único que poderá minimizar a perda de tecido renal, sendo a embolização o mais frequentemente utilizado. Todos os nossos casos foram tratados com sucesso, como demonstrado clinicamente e por TC de controlo. Assim, na nossa experiência, o tratamento endovascular mostrou-se um método de tratamento eficaz dos falsos aneurismas traumáticos renais intraparenquimatosos.
Introduction: Renal artery aneurysms and arteriovenous fistulae are rare, generally asymptomatic, whose clinical manifestations can be varied. Renal artery pseudoaneurysms are usually associated with previous trauma and are more likely to rupture. The indication for treatment of these lesions is not consensual, as well as the method to be used. Material and Methods: In this study the authors have made a retrospective analysis of patients with the diagnosis of renal artery pseudoaneurysm treated in the institution in the period between 2011 and 2015, including clinical presentation, diagnosis, treatment and result. Results: Six patients were treated, four male and two female, with an average age of 59 years. The inaugural clinical manifestation was macroscopic haematuria in four patients, hemodynamic instability in a patient and decrease of the hemoglobin value in another patient. In all patients, the diagnosis was made by Computed Tomography (CT) and confirmed by angiography, which was therapeutic in the same act. Conclusion/Discussion: In all cases there was a recent history of a urological procedure, renal artery pseudoaneurysms may be considered iatrogenic lesions. With the rising number of urological interventions similar to those described in this study, it is expected a rising incidence of lesions of the renal artery or its branches, particularly pseudoaneurysms. As it includes in most cases small intra-parenchymal vessels, conventional surgery has few therapeutic alternatives and usually consists in nephrectomy. The endovascular treatment is the only one that can minimize the loss of kidney tissue, embolization is the preferred method. All our cases were successfully treated as clinically proven and on CT control. So in our experience, endovascular treatment was an effective method of treatment of these lesions.

Contributo de fatores individuais, sociais e ambientais para a decisão de prosseguir uma gravidez não planeada na adolescência: Um estudo caracterizador da realidade portuguesa

Foi nosso objetivo explorar o contributo simultâneo de fatores individuais, sociais e ambientais e das possíveis interações entre eles para a decisão de prosseguir uma gravidez adolescente à luz do atual quadro legislativo que despenaliza a interrupção da gravidez por opção da mulher, em Portugal. A amostra foi constituída por 276 adolescentes que engravidaram de forma não planeada e contactaram com os serviços de saúde dentro do prazo legal que lhes permitiria optar pela interrupção: 133 adolescentes que prosseguiram a gravidez e 143 que a interromperam. Os dados foram recolhidos entre 2008 e 2013 em 53 serviços de saúde de todas as regiões. Não ter ponderado as duas alternativas possíveis (prosseguimento/interrupção), pertencer a famílias de baixo nível socioeconómico e com história de maternidade adolescente, ter abandonado a escola e residir em áreas com maior densidade populacional e onde a população feminina é menos escolarizada foram fatores explicativos do prosseguimento da gravidez. A menor idade da adolescente associou-se com o prosseguimento apenas quando não foram ponderadas ambas as alternativas. O efeito da religiosidade local variou de acordo com o envolvimento religioso da adolescente. Estes resultados têm importantes implicações para a prática clínica e investigação na área da decisão reprodutiva na adolescência.
The aim of the current study was to explore the simultaneous contribution of individual, social, and environmental factors, as well as the possible interactions between them to the decision to continue an adolescent pregnancy after the abortion on women´s demand has been legalized in Portugal. The sample consisted of 276 adolescents who became unintentionally pregnant and contacted with healthcare services within the legal period for induced abortion: 133 adolescents who chose to continue the pregnancy and 143 who chose to terminate the pregnancy. Data were collected between 2008 and 2013, in 53 healthcare centers of all country areas. Not having thought about both available options (i.e., continuing vs. terminating the pregnancy), belonging to families of low socioeconomic status and with adolescent pregnancy history, having dropped out of school and having lived in areas with higher population density and less educated females predicted the decision to continue the pregnancy. The lower the adolescents’ age, the more frequent was the decision to continue the pregnancy, but only when adolescents’ had not thought about both options. The effect of local religiosity differed according to the adolescents’ religious involvement. These findings have important implications for clinical practice and research on adolescents’ reproductive decisions.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Consulta multidisciplinar do pé diabético: avaliação dos fatores de mau prognóstico

Introdução: A diabetes mellitus é responsável por 70% das amputações não traumáticas do membro inferior e 85% destas são precipitadas por úlceras. Objetivo - caracterização epidemiológica e resultado da intervenção dos utentes da consulta multidisciplinar do pé diabético. Materiais e métodos: Estudo observacional retrospetivo das primeiras consultas realizadas no âmbito da consulta multidisciplinar do pé diabético, durante um semestre. Revisão do processo clínico e avaliação das características epidemiológicas, investigação clínica realizada, meios complementares de diagnóstico e o resultado final (cicatrização da lesão, amputação major, não cicatrização em um ano ou morte). Resultados: Realizaram-se 361 primeiras consultas do pé diabético no período em estudo, 82,3% por ulceração (31,3% neuropáticos e 68,7% neuroisquémicos). Dos doentes seguidos, 78% obtiveram cicatrização das lesões (com ou sem amputação minor), 7,7% não obtiveram cicatrização da lesão após um ano de seguimento, 10,1% foram submetidos à amputação major e 4,2% faleceram durante o seguimento. Os doentes com doença arterial periférica apresentaram menor probabilidade de cicatrização (70,6 vs. 89,4%, p = 0,004) e risco aumentado de amputação major (15,7 vs. 1,5%, p = 0,003). A nefropatia diminuiu a probabilidade de cicatrização (50 vs. 82,6%, p = 0,008) e aumentou o risco de amputação major (29,1 vs. 6,9%, p = 0,008). Os doentes com dependência de terceiros apresentaram maior risco de amputação major (22,9 vs. 6,8%, p = 0,008). Conclusões: O tratamento eficaz das úlceras do pé diabético necessita uma abordagem multidisciplinar, intervindo nos vários componentes etiológicos. A doença arterial periférica, a insuficiência renal e a dependência de terceiros são fatores de mau prognóstico das úlceras do pé diabético. © 2014 Sociedade Portuguesa de Angiologia e Cirurgia Vascular. Introduction: Diabetes mellitus is responsible for 70% of non-traumatic lower limb amputations and 85% of these are precipitated by ulcers. Objective -epidemiological characterization and revision of the outcome of patients observed in a multidisciplinary outpatient clinic of diabetic foot. Matherials and methods: A retrospective observational study with review of the first consultation of the multidisciplinary outpatient clinic of diabetic foot ulcer, during one semester. Review of the clinical process and evaluation of the epidemiological characteristics, clinical investigation, diagnostic tests, and outcome (healing, major amputation, non-healing after 1 year or death). Results: There were 361 first consultations during the study period, 82.3% due to ulceration (31.3% with neuropathic etiology and 68.7% with neuroischemic foot ulcers). Of the patients followed, 78% achieved complete wound healing (with or without minor amputation), 7.7% did not heal after one year of follow up, 10.1% underwent major limb amputation, and 4.2% died during the follow-up. Patients with peripheral arterial disease were less likely to heal (70.6% vs. 89.4%, p = 0.004) and had greater risk of major amputation (15.7% vs. 1.5%, p = 0.003). Nephropathy decreases the likelihood of healing (50% vs. 82.6%, p = 0.008) and increases the risk of major amputation (29.1% vs. 6.9%, p = 0.008). Non-ambulatory patients have a higher risk of major amputation (22.9% vs. 6.8%, p = 0.008). Conclusions: Successful treatment of diabetic foot ulcers requires a multidisciplinary approach, intervening in all etiological components. Peripheral arterial disease, renal failure and non ambulatory status are factors related to poor prognosis of diabetic foot ulcers. © 2014 Sociedade Portuguesa de Angiologia e Cirurgia Vascular.
