RESUMEN Con el objetivo de describir las características clínicas y epidemiológicas de los pacientes fallecidos por dengue durante el 2017 en Piura, se realizó un estudio retrospectivo de revisión de 24 historias clínicas. El 67% de los casos fueron mujeres y tres (12,5%) estaban embarazadas. La diabetes (12,5%) y la hipertensión (16,7%) fueron las comorbilidades más frecuentes. Sólo en el 12,5% se reportó dengue previo. El tiempo transcurrido desde asistencia sanitaria hasta la muerte fue de 4,10 (DE: 5,34) días. Se hicieron transfusiones de glóbulos rojos en el 45,8% de los casos, plasma en el 25%, plaquetas en el 16,8% y crioprecipitado en el 16,8%. También fueron frecuentes la terapia con cristaloides (91,7%) y el tratamiento con fármacos vasoactivos (70,8%). En conclusión, la mortalidad del dengue grave fue mayoritaria en las mujeres adultas y el tiempo de atención desde el primer establecimiento de salud hasta una unidad especializada fue prolongada.

ABSTRACT Objectives: To standardize and validate an in-house RT-LAMP test for the detection of SARS-CoV-2, based on laboratory and field assays using samples from COVID-19 suspected patients. Materials and methods: An in-house SARS-CoV-2 RT-LAMP molecular test was standardized, establishing the detection limit with Vero cells of isolated Peruvian strains of SARS-CoV-2, and the robustness to various concentrations of primers. The laboratory validation was performed with 384 nasal and pharyngeal swab samples (UFH) obtained between March and July 2020. The field validation was performed with 383 UFH obtained from COVID-19 suspected symptomatic cases. All samples were tested by RT-LAMP and RT-qPCR. The RT-qPCR was considered as the reference standard test. The concordance measures and diagnostic performance were calculated. Results: The detection limit was consistent in cases with Ct <30 in both tests, showing efficiency to detect up to 1000 copies/μL of the target gene. Robustness was evidenced with half of the primer concentrations and 20 μL of final volume. Absence of amplification was identified for other HCoVs. Concordance showed a kappa index of 0.88 (95% CI: 0.83-0.93) and 0.89 (95% CI: 0.84 - 0.94) in laboratory and field settings, respectively. The sensitivity value in the laboratory was 87.4% (95% CI: 80.8 - 92.4) and 88.1% in the field (95% CI: 81.6 - 92.9). The specificity value in both settings was 98.8% (95% CI: 96.4-99.7). Conclusions: The in-house SARS-CoV-2 RT-LAMP test was successfully validated based on its adequate robustness, no cross-reactions, good concordance, and diagnostic performance compared to RT-qPCR.

RESUMEN Objetivos: Estandarizar una prueba RT-LAMP in house para la detección de SARS-CoV-2 y validarla con muestras de laboratorio y de campo en pacientes con sospecha clínica de COVID-19. Materiales y métodos: Se estandarizó una prueba molecular RT-LAMP in house para la detección de SARS-CoV-2 estableciéndose el límite de detección con células Vero de cepas peruanas aisladas de SARS-CoV-2. Se validó la prueba en laboratorio con 384 muestras de hisopado nasal y faríngeo (HNF) obtenidas entre marzo y julio de 2020. Para la validación de campo se obtuvieron muestras de HNF de 383 casos sintomáticos sospechosos de COVID-19. Todas las muestras fueron evaluadas por RT-LAMP y RT-qPCR. Para la validación de laboratorio y de campo se consideró como estándar de referencia al RT-qPCR, se calcularon medidas de concordancia y rendimiento diagnóstico. Resultados: El límite de detección fue consistente en los casos con umbral de ciclo (Ct) Ct < 30 en ambas pruebas, mostrando eficiencia para detectar hasta 1000 copias/µL del gen diana. Se evidenció robustez con la mitad de las concentraciones de cebadores y 20 µL de volumen final. Se identificó ausencia de amplificación para otros coronavirus humanos. La concordancia en laboratorio obtuvo un Kappa de 0,88 (IC 95%: 0,83-0,93) y en campo fue de 0,89 (IC 95%: 0,84−0,94); la sensibilidad en laboratorio fue de 87,4% (IC 95%: 80,8−92,4) y en campo fue de 88,1% (IC 95%: 81,6−92,9), la especificidad en ambos escenarios fue de 98,8% (IC 95%: 96,4−99,7). ABSTRACT Objective: To describe the results of laboratory tests performed on biological samples from patients with Guillain-Barré syndrome (GBS) submitted to the Instituto Nacional de Salud (INS) between 2018 and 2019. Materials and methods: We conducted an observational study on patients with GBS, by using data from the epidemiological surveillance system. Biological samples, previously analyzed at the INS, were obtained to study arboviruses, respiratory viruses, enteroviruses and enterobacteria, among others. Results: A total of 2,051 specimens were obtained from 906 patients with GBS. Three patients tested positive for dengue and three for Zika. In 19 patients, the stool culture was positive for Campylobacter jejuni. Phylogenetic analysis of 10 Campylobacter jejuni strains classified them as genotype ST2993, which was previously reported in China and associated to a GBS outbreak. Twelve cerebrospinal fluid samples tested positive for enterovirus by PCR in 2018, but none could be verified by culture or complete genome sequencing during the study. One patient was positive for influenza A, two for influenza B, two for adenovirus, five for respiratory syncytial virus, and ten for rinovirus. Conclusion: Several pathogens were found in samples from patients with GBS. However, we found that the genotype ST2993 of Campylobacter jejuni was the most likely causal agent, a pathogen that is related to GBS outbreaks in different continents. It is necessary to confirm this hypothesis with additional analytical studies and it is important to describe the transmission mechanism of C. jejuni genotype ST2993 in order to implement prevention and control measures.

Materiales y métodos: Se realizó un estudio observacional en pacientes con SGB notificados en el sistema de vigilancia epidemiológica. Se obtuvieron muestras biológicas analizadas en el INS para investigar arbovirus, virus respiratorios, enterovirus y enterobacterias, entre otros. Resultados: Se recibió un total de 2051 especímenes clínicos de 906 pacientes con SGB. Tres pacientes dieron positivo al dengue y tres pacientes al Zika. En 19 pacientes, el cultivo en heces fue positivo para Campylobacter jejuni. El análisis filogenético de diez cepas de Campylobacter jejuni las clasificó como genotipo ST2993, reportado previamente en China y asociado a un brote de SGB. En 2018, hubo 12 muestras que habían dado positivo al PCR para enterovirus en el líquido cefalorraquídeo, pero ninguna pudo corroborarse con el cultivo respectivo ni con secuenciamiento de genoma completo. Un paciente dio positivo por virus de la influenza A, dos por virus de la influenza B, dos por adenovirus, cinco por virus respiratorio sincicial, y diez por rinovirus. Conclusión: Se han encontrado diversos agentes patógenos en especímenes de pacientes con SGB, sin embargo, la presencia de Campylobacter jejuni genotipo ST2993, un patógeno relacionado a brotes de SGB en varios continentes, sería el probable agente causal. Syphilis is a curable infection caused by a bacterium called Treponema pallidum, it is transmitted sexually and also, during pregnancy, from the mother to the fetus; and the absence of treatment in pregnancy leads to adverse outcomes such as late fetal death, prematurity and neonatal death. We describe the natural course of a case of maternal syphilis that occurred with complications during the end of pregnancy and led to neonatal death.

La sífilis es una infección curable causada por una bacteria llamada Treponema pallidum, se trasmite por vía sexual y también, durante el embarazo, de la madre al feto; y la ausencia de tratamiento en el embarazo conduce a resultados adversos como la muerte fetal tardía, prematuridad y muerte neonatal. Se describe el curso natural de un caso de sífilis materna que curso con complicaciones durante el final del embarazo y conllevó a muerte neonatal.

ABSTRACT The objective of the study was to determine compliance with hepatitis-B virus (HBV) in newborns in Lima and Callao. A descriptive, cross-sectional, multicenter-design study was conducted in high level public and private hospitals. Information on vaccination corresponds to newborns during one week of 2015 and was obtained through consultations with parents and review of health service reports. The study was conducted in 12 health facilities including 935 mothers and newborns. Vaccination coverage within 24 hours of life was 85.1%, according to the parents’ report; and 88.7%, according to the health services’ report. It is concluded that the coverage of HBV vaccination in newborns within 24 hours of life was important, but still far from the ideal goals proposed by the Ministry of Health and the World Health Organization.

RESUMEN El objetivo del estudio fue determinar el cumplimiento de la vacunación contra el virus de la hepatitis B (VHB) en recién nacidos de Lima y Callao. Estudio de diseño descriptivo, transversal y multicéntrico, realizado en hospitales de alto nivel, públicos y privados. La información sobre la vacunación corresponde a recién nacidos durante una semana del 2015, y fue obtenida mediante consultas a los padres y revisión de reporte de los servicios de salud. El estudio se realizó en 12 establecimientos de salud incluyéndose a 935 madres y recién nacidos. La cobertura de vacunación dentro de las 24 horas de vida fue de 85,1% según el reporte de padres y 88,7% según el reporte de los servicios de salud. Se concluye que la cobertura la vacunación contra el VHB en recién nacidos dentro de las 24 horas de vida fue importante, pero aún alejada de metas ideales propuestas por el Ministerio de Salud y la Organización Mundial de la Salud.

ABSTRACT Several of the acute diseases with infectious etiology (viral, bacterial or parasitic) initially start with a syndrome which can be febrile, febrile icteric, febrile hemorrhagic, febrile with respiratory manifestations, febrile with neurological impairment, febrile with dermal eruptions, febrile anemic, diarrheal, necrotic cutaneous ulcer, or deaths of febrile patients may occur without a defined diagnosis, with some variants according to the ecological-social niche in which they occur; the syndromic approach is important, to quickly classify the case or the outbreak and thus have an appropriate and timely diagnosis that allows to orient both the individual management and the prevention and control measures in the population, even if there are limitations for the diagnosis of all the cases. This article describes the syndromic approach, the syndromes of importance in public health (collective) and their approach, all this is important because of its presentation in outbreaks that are occurring in the various ecological niches of our country, exacerbated by climate change.

En este artículo se describe el enfoque sindrómico, los síndromes de importancia en salud pública (colectiva) y su abordaje, todo ello es importante por su presentación en brotes que vienen ocurriendo en los variados nichos ecológicos de nuestro país, exacerbados por el cambio climático.

This is a qualitative study, aimed at describing community perceptions on dengue prevention in affected areas of Lima. A series of interviews were conducted in six of those areas using the health belief model approach. Respondents noted several factors about sanitation, behaviors and information that could increase their exposure to dengue. They expressed that this illness would affect their lives in different ways, in aspects related to work, family and economy. They recognized their share of responsibility on prevention, but also talked about the importance of health promoters work, media and community leaders. Among the preventive measures they mentioned, there were a few with no evidence of effectiveness. The main identified problems were lack of health professionals, community participation and preventive activities. They also noted that most activities were discontinued after outbreaks. The findings provide an opportunity to discuss and reflect on the preventive work done to face dengue.

Con el objetivo de describir las percepciones comunitarias para la prevención del dengue en las localidades afectadas de Lima, realizamos un estudio cualitativo a través de entrevistas desde el modelo de creencias en salud. Los entrevistados mencionaron diversos factores de saneamiento, conductas e información que incrementan su exposición al dengue, expresando que los afectaría en aspectos laborales, familiares y económicos, asumiendo parte de la responsabilidad en la prevención y recalcando la labor de los promotores de salud, medios de comunicación y líderes comunitarios. Entre las medidas preventivas mencionadas, se incluyeron medidas sin evidencia de efectividad. Las principales barreras expresadas fueron las siguientes, caraterizadas como insuficientes: cantidad de profesionales de salud, participación comunitaria, organización en las actividades preventivas y discontinuidad luego de los brotes. La discusión y reflexión sobre estos contextos promueve una oportunidad en el abordaje de la labor preventiva hacia el dengue.

Com o objetivo de descrever as percepções comunitárias a respeito da prevenção da dengue, realizou-se um estudo qualitativo nas áreas afetadas de Lima. Foram utilizadas entrevistas sob o modelo de crenças em saúde. Os entrevistados mencionaram diversos fatores de saneamento e condutas que aumentariam sua exposição à doença. Também enumeraram aspectos laborais, familiares e econômicos que seriam afetados pelo adoecimento. Os entrevistados assumiram parte da responsabilidade na prevenção, mas destacaram a importância do trabalho dos promotores de saúde, líderes comunitários e meios de comunicação. Dentre as medidas preventivas enumeradas, foram citadas algumas que não tinham evidência de efetividade. Os principais obstáculos citados foram: quantidade insuficiente de profissionais de saúde, baixa participação comunitária, desorganização das atividades preventivas e descontinuidade das ações após a resolução dos surtos. A discussão dos resultados encontrados promove a oportunidade de refletir sobre o trabalho preventivo realizado para o combate à dengue.

This is a qualitative study, aimed at describing community perceptions on dengue prevention in affected areas of Lima. A series of interviews were conducted in six of those areas using the health belief model approach. Respondents noted several factors about sanitation, behaviors and information that could increase their exposure to dengue. They expressed that this illness would affect their lives in different ways, in aspects related to work, family and economy. They recognized their share of responsibility on prevention, but also talked about the importance of health promoters work, media and community leaders. Among the preventive measures they mentioned, there were a few with no evidence of effectiveness. The main identified problems were lack of health professionals, community participation and preventive activities. They also noted that most activities were discontinued after outbreaks. The findings provide an opportunity to discuss and reflect on the preventive work done to face dengue.

Con el objetivo de describir las percepciones comunitarias para la prevención del dengue en las localidades afectadas de Lima, realizamos un estudio cualitativo a través de entrevistas desde el modelo de creencias en salud.
