RESUMO Objetivo: Os objetivos do presente estudo foram desenvolver um método para medir longitudinalmente a rotação, inclinação e angulação dentárias em modelos digitais, e testar sua validade e reprodutibilidade. Material e Métodos: Os modelos inicial e final planejado de 14 pacientes tratados com Invisalign® (386 dentes) foram exportados do ClinCheck®. Os valores de rotação, inclinação e angulação foram calculados para incisivos, caninos, pré-molares e molares, em ambos os modelos, por meio de trigonometria. Um aplicativo desenvolvido em Python 2.7 foi utilizado para automatização das medidas. O ∆planejado (variação na posição entre os modelos inicial e final planejado) foi obtido para os movimentos descritos, nos diferentes grupos de dentes. A validade do método foi verificada pelo grau de concordância entre o ∆planejado e os valores disponíveis na Tabela de Movimentos do Invisalign®, utilizando-se o Coeficiente de Correlação Intraclasse (ICC) e a análise de Bland-Altman. O ∆planejado foi novamente calculado para análise da reprodutibilidade intra e interexaminadores. Resultados: Excelentes ICCs (> 0,9) e limites de concordância com discrepâncias reduzidas e clinicamente aceitáveis foram obtidos para a rotação de todos os dentes (exceto os caninos superiores, que expressaram limites mais amplos: -3,47 - 5,43) e para a inclinação de pré-molares e molares. A inclinação dos dentes anteriores e a angulação de todos os dentes apresentaram ICCs e limites não representativos de boa concordância. A reprodutibilidade foi alta para os três movimentos (discrepância < 2°). Conclusões: O método desenvolvido é reprodutível e se mostrou adequado para mensuração longitudinal da inclinação de dentes posteriores e rotação de todos os dentes, exceto caninos superiores. Apresenta valor limitado para as demais mensurações.

ABSTRACT Objective: The objectives of the present study were to develop a method for longitudinally measuring tooth rotation, inclination and angulation on digital models, and to test the method validity and reliability. Methods: The initial and final planned models of 14 patients treated with Invisalign® (386 teeth) were exported from ClinCheck®. The rotation, inclination and angulation values were assessed for the incisors, canines, premolars and molars, in both models, using trigonometry. An application was developed in Python 2.7 to automate the measurements. The ∆planned (variation in the position between the initial and final planned models) was obtained for each tooth and each type of movement. To test the validity, the degree of agreement between the ∆planned and the values available in the Invisalign® Table of Movements was assessed using the Intraclass Correlation Coefficient (ICC) and Bland-Altman analysis. For intra and inter-rater reliabilities, the ∆planned was obtained again. Results: Excellent ICCs (> 0.9) and limits of agreement with narrow and clinically acceptable discrepancies were obtained for the rotation of all teeth (except maxillary canines, which had broader limits: -3.47 - 5.43) and for the inclination of premolars and molars. The inclination of anterior teeth and angulation of all teeth had ICCs and limits that were not indicative of great agreement. The reliability was high for the three movements (discrepancy <2°). Conclusions: The method developed is reliable and suitable for longitudinally measuring inclination (posterior teeth) and rotation (except maxillary canines). It has limited value for the other movements measurements.

Resumo O presente estudo tem como objetivo analisar o efeito da fototerapia LED na presença da hialinização e reabsorção radicular durante o movimento dentário ortodôntico (MDO) em ratos, e a mensuração da quantidade de movimento dentário. Oitenta ratos foram alocados em dois grupos: LED e Controle (CON), os ratos foram irradiados com um LED infravermelho (850nm, 30mW) por 5 minutos durante os cinco primeiros dias da MDO; e o grupo controle não foi irradiado. Ambos os grupos foram subdivididos em 4 subgrupos (n=10) de acordo com a data da eutanásia (4, 7, 14 e 21 dias). Cinco dos dez ratos LED21 e cinco dos dez ratos CON21foram submetidos a microtomografia computadorizada (μCT); As μCT foram realizadas nos dias 0, 7, 14 e 21. Para o estudo histológico, as maxilas foram processadas para microscopia de luz, usando hematoxilina-eosina (HE) e Fosfatase ácido Tartrate-Resistente (TRAP) para histoquímica. A quantidade de movimento dentário não diferiu entre o LED e o CON. A hialinização foi observada nas áreas de pressão em ambos os grupos e não mostrou diferença estatisticamente significante. Reabsorção radicular também foi observada em ambos os grupos depois de 7 dias e não houve diferença entre os grupos. A fototerapia LED não aumentou a quantidade de MDO. Características similares de hialinização e reabsorção radicular foram observadas em ambos os grupos

Abstract The present study aimed to analyze the effect of LED phototherapy on the presence of hyalinization and root resorption during orthodontic tooth movement (OTM) in rats and to measure the amount of tooth movement. Eighty rats were allocated into two groups: LED and control (CON), where the LED rats were irradiated with infrared LED (850 nm, 30 mW) for 5 min during the first five days of OTM and where controls were not irradiated. Both groups were subdivided into four subgroups (n=10) according to the date of euthanasia (4, 7, 14 and 21 days). Five out of ten LED21 and five of ten CON21 rats were submitted to micro-computed tomography (μCT); μCT scans were taken on days 0, 7, 14 and 21. For histological study, maxillae were processed to light microscopy using Hematoxylin-Eosin (HE) and Tartrate-Resistant Acid Phosphatase (TRAP) histochemistry. The amount of tooth movement did not differ between LED and CON. Hyalinization was observed at the pressure areas in both groups, and it did not show a statistically significant difference between the groups. Root resorption was also observed in both groups after 7 days and it did not represent any differences between the two groups. LED phototherapy was not able to increase the amount of OTM. Similar characteristics of hyalinization and root resorption were observed in both groups.

OBJETIVO: avaliar as alterações dentoesqueléticas observadas no tratamento da má oclusão de Classe II com retrognatismo mandibular, realizado com aparelho ortopédico de Herbst durante 13 meses (Fase I) e aparelho ortodôntico fixo pré-ajustado (Fase II). MÉTODOS: foram obtidas telerradiografias laterais de 17 adolescentes, ao início (T1), final da Fase I (T2), primeiros 13 meses da Fase II (T3) e término da Fase II (T4). As diferenças entre as variáveis cefalométricas foram analisadas estatisticamente (variância e comparações múltiplas de Bonferroni). RESULTADOS: de T1 a T4, do total da projeção da maxila, 42% foram observados de T1 a T2 (p < 0,01); 40,3% de T2 a T3 (p < 0,05); e 17,7% de T3 a T4 (n.s.). Do total da projeção da mandíbula, foi notado 48,2% de T1 a T2 (p < 0,001) e 51,8% de T2 a T4 (p < 0,01), sendo 15,1% (n.s.) de T2 a T3, e 36,7% de T3 a T4 (p < 0,01). A relação molar e a sobressaliência foram corrigidas idealmente. Em T4, todos apresentavam características de oclusão normal. O plano oclusal que de T1 a T2 rotacionou no sentido horário, de T2 a T3 retornou aos valores iniciais, mantendo-se estável até T4. A inclinação do plano mandibular, responsável pela caracterização do tipo facial, não alterou em nenhum tempo. CONCLUSÃO: a mandíbula cresceu significativamente mais que a maxila, favorecendo o ajuste sagital maxilomandibular. As mudanças dentárias (molares superiores), que sobrecorrigiram a má oclusão na Fase I, recidivaram parcialmente na Fase II, sem comprometer a correção da má oclusão. O tipo facial foi preservado.

OBJECTIVE: To assess the dentoskeletal changes observed in treatment of Class II, division 1 malocclusion patients with mandibular retrognathism. Treatment was performed with the Herbst orthopedic appliance during 13 months (phase I) and pre-adjusted orthodontic fixed appliance (phase II). METHODS: Lateral cephalograms of 17 adolescents were taken in phase I onset (T1) and completion (T2); in the first thirteen months of phase II (T3) and in phase II completion (T4). Differences among the cephalometric variables were statistically analyzed (Bonferroni variance and multiple comparisons). RESULTS: From T1 to T4, 42% of overall maxillary growth was observed between T1 and T2 (P < 0.01), 40.3% between T2 and T3 (P < 0.05) and 17.7% between T3 and T4 (n.s.). As for overall mandibular movement, 48.2% was observed between T1 and T2 (P < 0.001) and 51.8% between T2 and T4 (P < 0.01) of which 15.1% was observed between T2 and T3 (n.s.) and 36.7% between T3 and T4 (P < 0.01). Class II molar relationship and overjet were properly corrected. The occlusal plane which rotated clockwise between T1 and T2, returned to its initial position between T2 and T3 remaining stable until T4. The mandibular plane inclination did not change at any time during treatment. CONCLUSION: Mandibular growth was significantly greater in comparison to maxillary, allowing sagittal maxillomandibular adjustment. The dentoalveolar changes (upper molar) that overcorrected the malocclusion in phase I, partially recurred in phase II, but did not hinder correction of the malocclusion. Facial type was preserved.

INTRODUÇÃO: a análise de Moyers é amplamente adotada para a análise da dentição mista, porém, recentemente, a acurácia das suas tabelas de probabilidade está sendo questionada. OBJETIVO: sabendo-se da importância da análise da dentição mista, na determinação mais precisa possível do espaço necessário para o alinhamento de caninos e pré-molares, o objetivo dessa pesquisa foi avaliar de forma objetiva na literatura este importante passo do diagnóstico ortodôntico. MÉTODOS: foi realizado um levantamento computadorizado de artigos publicados entre os anos de 1990 a setembro de 2011 nas bases de dados PubMed e LILACS, segundo o método descrito no manual Cochrane 5.1.0. RESULTADOS: a estratégia de busca resultou em 629 artigos. Para ser selecionado, o artigo deveria utilizar a análise de Moyers e ter uma amostra superior ou igual a 40 pacientes. Os critérios de exclusão foram: modelos de gesso de pacientes com algum tipo de síndrome ou fissurados, pesquisas cuja proposta era apenas uma revisão da literatura ou apenas relato de um caso clínico, e pesquisas anteriores ao ano de 1990. Para essa revisão sistemática, 19 artigos foram selecionados. CONCLUSÕES: com base na literatura disponível, pode-se concluir que a utilização da análise de Moyers na análise da dentição mista deve ser utilizada de maneira cautelosa, pois, na maioria dos artigos avaliados, 75% não apresentou a acurácia desejada, necessitando de adaptações nos níveis de probabilidade, dependendo da população em estudo.

INTRODUCTION: Moyers analysis is widely used for analyzing mixed dentition, however, the accuracy of its theoretical probability tables has been recently questioned. Taking into consideration the fact the mixed dentition analysis is of paramount importance to precisely determine the space needed for alignment of canines and premolars, this research aimed at objectively assessing in the literature such an important step for orthodontic diagnosis. METHODS: A computerized search involving articles published on PubMed and Lilacs between 1990 and September, 2011 was conducted in accordance with the method described in the Cochrane 5.1.0 handbook. RESULTS: The research resulted in a sample composed of 629 articles. The inclusion criteria were: Articles using the Moyers analysis with a sample greater or equal to 40 patients. Conversely, the exclusion criteria were: Dental casts of patients with syndromes or oral cleft, researches conducted with a literature review, only, or clinical case reports and researches conducted before 1990. For this systematic review, 19 articles were selected. CONCLUSION: Based on the literature available, we can conclude that the Moyers mixed dentition analysis must be carefully used, since the majority of the articles analyzed showed that the probability of 75% was not as accurate as expected, leading to the need of adapting the probability levels depending on the study population.

OBJETIVO: o presente estudo foi delineado para verificar se protótipos de mini-implantes (PMI) desenvolvidos para a ancoragem esquelética do aparelho de Herbst são capazes de suportar forças ortopédicas e, também, determinar a variação da força de flexão desses PMI de acordo com o local de inserção (maxila ou mandíbula). MÉTODOS: após o cálculo do tamanho da amostra, 13 PMI foram colocados em três cadáveres de Minipigs br1 (seis na maxila e sete na mandíbula). Os corpos de prova foram preparados e submetidos a um teste mecânico. Cálculos da média e o do desvio-padrão foram realizados para cada região. O teste t de Student para duas amostras não pareadas foi utilizado para comparar a resistência dos PMI entre as regiões de inserção. O teste t de Student para uma amostra foi realizado para o teste de hipótese. Foram consideradas forças ortopédicas aquelas acima de 1,0kgf. RESULTADOS: os PMI foram capazes de suportar forças de flexão maiores que 1,0kgf (13,8 ± 2,3Kg na região posterior da maxila, e 20,5 ± 5,2Kg na região anterior da mandíbula), apresentando significativa menor força de flexão na região anterior da mandíbula (p < 0,05). CONCLUSÃO: os PMI são capazes de suportar forças ortopédicas, sendo mais resistentes quando utilizados na região anterior da mandíbula do que na posterior da maxila, em cadáveres de Minipigs br1.

OBJECTIVE: The present study was designed to verify if mini-implant prototypes (MIP) developed for Herbst appliance anchorage are capable of withstanding orthopedic forces, and to determine whether the flexural strength of these MIP varies depending on the site of insertion (maxilla and mandible). METHODS: Thirteen MIP were inserted in three minipig cadavers (six in the maxilla and seven in the mandible). The specimens were prepared and submitted to mechanical testing. The mean and standard deviation were calculated for each region. A two-way Student's t test was used to compare the strength between the sites. A one-way Student's t test was performed to test the hypothesis. Orthopedic forces above 1.0 kgf were considered. RESULTS: The MIP supported flexural strength higher than 1.0 kgf (13.8 ± 2.3 Kg, in the posterior region of the maxilla and 20.5 ± 5.2 Kg in the anterior region of the mandible) with a significantly lower flexural strength in the anterior region of the mandible (P < 0.05). CONCLUSION: The MIP are capable of withstanding orthopedic forces, and are more resistant in the anterior region of the mandible than in the posterior region of the maxilla in Minipigs br1 cadavers.

Este estudo avaliou as mudanças morfológicas nas cabeças da mandíbula das articulações temporo mandibulares (ATMs) e calculou o índex de disfunção clínico de Helkimo (IDC) em adolescentes com má oclusão de Classe II Divisão1 e retrognatismo mandibular, tratados com aparelho de Herbst (fase I) e aparelho ortodôntico fixo (fase II). Trinta e dois adolescentes consecutivos passaram pela fase I e 23 completaram a fase II. As ATMs foram avaliadas qualitativamente por meio de imagem da resonância magnética (IRM) ao início do tratamento (T1), durante a fase I (T2), no final da fase I (T3) e no final da fase II (T4). O IDC foi calculado em T1, T3 e T4. De T1 a T3 (p=0,326) não ocorreram mudanças na morfologia da cabeça da mandíbula em 86,0% das ATMs. De T3 a T4 (p<0,05) e T1 a T4 (p<0,05) ocorreram mudanças em 39,1% e 43,4% das cabeças das mandíbulas. Não ocorreram mudanças significantes no IDC de T1 a T3, T3 a T4 e T1 a T4 (p=1,000; 86,6%, 76,2% e 76,2% concordância). Após a fase I, não houve praticamente mudanças na morfologia da cabeça da mandíbula. Ao final da fase II, um leve aplainamento foi observado em algumas cabeças das mandíbulas. Pode ser concluído que não ocorreram mudanças significantes no IDC após ambas as fases de tratamento.

This study evaluated the morphological changes in the temporomandibular joint (TMJ) condyles and calculated the Helkimo clinical dysfunction index (CDI) in adolescents with Class II Division 1 malocclusion and mandibular retrognathism treated with the Herbst appliance (phase I) and fixed orthodontic appliances (phase II). Thirty-two consecutive adolescents underwent phase I, and 23 completed phase II. The TMJs were evaluated qualitatively using magnetic resonance imaging (MRI) at the beginning of treatment (T1), during phase I (T2), at the end of phase I (T3) and at the end of phase II (T4). The CDI was calculated at T1, T3 and T4. From T1 to T3 (p=0.326), there were no changes in condyle morphology in 86.0% of the TMJs. From T3 to T4 (p<0.05) and T1 to T4 (p<0.05), changes occurred in 39.1% and 43.4% of the condyles. No significant changes in CDI occurred from T1 to T3, T3 to T4 and T1 to T4 (p=1.000; 86.6%, 76.2% and 76.2% concordance). After phase I, there were practically no changes in condyle morphology. At the end of phase II, a mild flattening was observed in some condyles. It may be concluded that no significant changes occurred in CDI after both treatment phases.

OBJETIVO: o objetivo do presente estudo foi comparar o desempenho clínico da colagem de braquetes ortodônticos com resina Transbond (3M Unitek) associada a dois sistemas adesivos: convencional em duas etapas (ataque ácido + Transbond XT adhesive Primer) e Self-Etching Primer (SEP), em etapa única (Transbond Plus). MÉTODOS: a amostra foi constituída de 480 braquetes metálicos (Victory, 3M Unitek), colados em 24 pacientes, que foram tratados durante um período de 36 a 48 meses. A colagem foi feita por meio do sistema split-mouth, utilizando os dois sistemas de colagem em cada paciente. Foi analisada a taxa de queda dos braquetes para cada sistema de colagem, descrita a causa da queda conforme relato do paciente e a posição dos dentes nas arcadas. RESULTADOS: o sistema adesivo convencional apresentou taxa de queda de 5,41%, enquanto a do SEP foi de 4,58%. O sistema convencional apresentou 5 quedas (38,4%) no quadrante superior direito, 2 (15,4%) no quadrante superior esquerdo, 4 (30,8%) no quadrante inferior direito e 2 (15,4%) no quadrante inferior esquerdo. O SEP apresentou 4 quedas (36,4%) no quadrante superior direito, 1 (9%) no quadrante superior esquerdo, 3 (27,3%) no quadrante inferior direito e 3 (27,3%) no quadrante inferior esquerdo. Por meio da análise estatística descritiva e do teste Odds Ratio, constatou-se que não houve diferença significativa entre essas taxas (p = 0,67). CONCLUSÃO: com base nesses resultados, pode-se concluir que o sistema adesivo SEP apresentou eficácia clínica semelhante à do sistema adesivo convencional.

OBJECTIVE: This study compared the clinical performance of orthodontic brackets bonded with Transbond adhesive paste after two priming systems: a two-stage conventional system (acid etching + Transbond XT adhesive primer) and a singlestage self-etching primer (SEP) (Transbond Plus). METHODS: The sample comprised 480 metal brackets bonded to the teeth of 24 consecutive patients treated for 36 to 48 months. A split-mouth design was used for bonding, and both systems were used in each patient. Bracket failure rates for each system were analyzed; and failure causes as reported by the patients and the quadrant of teeth for which brackets failed were recorded. RESULTS: The conventional system group had a failure rate of 5.41%, whereas the rate for SEP was 4.58%. In this group, there were 5 failures (38.4%) in the right maxillary quadrant, 2 (15.4%) in the left maxillary quadrant, 4 (30.8%) in the right mandibular quadrant, and 2 (15.4%) in the left mandibular quadrant. In the SEP group, there were 4 (36.4%) failures in the right maxillary quadrant, 1 (9%) in the left maxillary quadrant, 3 (27.3%) in the right mandibular quadrant, and 3 (27.3%) in the left mandibular quadrant. Results of descriptive statistical analysis and odds ratio did not show any significant differences between rates (p = 0.67). CONCLUSION: The clinical efficiency of SEP was similar to that of the conventional system.

O objetivo deste estudo foi comparar os efeitos dos lacebacks ativos e passivos na posição ântero-posterior dos primeiros molares e dos incisivos centrais superiores durante a fase de nivelamento. Vinte e três indivíduos com má oclusão de Classes I e II foram tratados com extrações dos primeiros pré-molares utilizando aparelhos fixos pré-ajustados (bráquetes MBT 0,022"). A fase de nivelamento foi realizada somente com arcos de aço inoxidável. A amostra foi dividida em 2 grupos: 14 pacientes receberam lacebacks ativos (Grupo 1) e 9 pacientes receberam lacebacks passivos (Grupo 2). Os lacebacks foram feitos com fio de ligadura de 0,008" (0,20 mm). Telerradiografias laterais foram realizadas antes e após a fase de nivelamento. O teste t de Student foi aplicado para determinar as diferenças entre as médias de deslocamento entre os períodos pré e pós-nivelamento e para determinar as diferenças entre os grupos. No Grupo 1, o primeiro molar mostrou uma movimentação mesial significativa enquanto nenhuma alteração foi observada no Grupo 2. Em ambos os grupos, os incisivos centrais superiores foram lingualizados. Em conclusão, os lacebacks ativos produziram perda de ancoragem nos primeiros molares superiores enquanto que os lacebacks passivos não afetaram a posição destes dentes. Lacebacks ativos e passivos foram efetivos para prevenir a vestibularização dos incisivos centrais superiores

The purpose of this study was to compare the effects of active and passive lacebacks on antero-posterior position of maxillary first molars and central incisors during leveling phase. Twenty-three subjects with Class I and Class II malocclusion were treated with first premolars extraction using preadjusted appliances (MBT 0.022-inch brackets). The leveling phase was performed with stainless steel archwires only. The sample was divided into 2 groups: 14 subjects received active lacebacks (Group 1) and 9 subjects received passive lacebacks (Group 2). Lacebacks were made from 0.008-inch ligature wire. Lateral cephalometric radiographs were taken pre- and post-leveling phase. Student's t-test was applied to determine the differences between pre- and post-leveling mean values and to determine the mean differences between groups. In Group I, the first molars showed a significant mesial movement, whereas no change was observed in Group 2. In both groups, maxillary central incisor crowns moved to lingual side. In conclusion, active laceback produced anchorage loss of maxillary first molars whereas passive laceback did not affect the position of these teeth. Active and passive lacebacks were effective in preventing central incisor proclination.

OBJETIVO: estudar os efeitos do nivelamento realizado com fios de NiTi termoativado e de aço inoxidável, avaliando-se as possíveis alterações na posição dos incisivos inferiores, em casos com extrações, correlacionando com o tempo de tratamento. MÉTODOS: a amostra foi composta de 36 indivíduos, de ambos os sexos, leucodermas brasileiros, com idade média inicial de 15 anos e 5 meses, portadores de más oclusões de Classes I e II, distribuídos em dois grupos. No Grupo 1 (n=17), o nivelamento foi realizado com a sequência 1, utilizando-se três fios - 0,016" e 0,019"x0,025" de NiTi termoativado, e 0,019"x0,025" de aço inoxidável. No Grupo 2 (n=19), foi testada a sequência 2, na qual foram utilizados apenas fios de aço inoxidável (0,014"; 0,016"; 0,018"; 0,020" e 0,019"x0,025" com torque passivo nos incisivos inferiores). Os dados foram coletados utilizando-se o método cefalométrico computadorizado e comparados pelo teste t de Student com o nível de significância de 5%. RESULTADOS: no Grupo 1, os incisivos inferiores inclinaram-se para lingual, com movimento significativo apenas da coroa (1,6mm). No Grupo 2, os incisivos inferiores permaneceram estáveis. Não houve alteração vertical em nenhum dos grupos. CONCLUSÕES: a sequência 2 proporcionou um melhor controle dos incisivos inferiores, não alterando suas posições iniciais, enquanto a sequência 1 permitiu a expressão do torque da prescrição utilizada, levando a uma inclinação lingual desses dentes. O tempo de tratamento foi menor utilizando-se a sequência 1. As variações biomecânicas estudadas apresentaram vantagens e desvantagens que devem ser conhecidas e ponderadas pelo ortodontista no planejamento do caso.

OBJECTIVE: To study the effects of the leveling phase performed with heat activated NiTi and stainless steel archwires evaluating the possible changes in lower incisors position in extraction cases, as well as its correlation with treatment time. METHODS: The sample comprised 36 white Brazilians subjects, male and female, mean age of 15 years and 5 months, with Angle Class I and II malocclusion, arranged in two groups. In Group 1 (n=17), the leveling phase was performed with sequence 1, using three archwires (NiTi heat activated 0.016" and 0.019"x0.025" and stainless steel 0.019"x0.025"). In Group 2 (n=19), sequence 2 was tested, in which only stainless steel archwires were used (0.014", 0.016", 0.018", 0.020" and 0.019"x0.025" with passive torque in lower incisors). The data were collected using the computerized cephalometry and were compared by Student's t test at significance level of 5%. RESULTS: In Group 1, lower incisors tipped lingually and only the crown presented a lingual movement that was statistically significant (1.6mm). In Group 2, the lower incisors remained unchanged. There was no vertical change in either groups. CONCLUSIONS: Sequence 2 showed better control of the lower incisors and no changes in their position were observed whereas sequence 1 allowed torque expression of brackets prescription with lingual tip of these teeth. Treatment time was shorter using sequence 1. Both biomechanical variations studied showed advantages and disadvantages that should be known and evaluated by the orthodontist during case planning.

OBJETIVO: o propósito do presente estudo é avaliar o limite de resistência à flexão de um protótipo de mini-implante desenvolvido para ancoragem do aparelho de Herbst. MÉTODOS: após a realização de um cálculo do tamanho da amostra, quatro corpos de prova contendo os protótipos de mini-implantes foram submetidos a uma força de flexão por engastamento simples, utilizando-se uma máquina universal de ensaios mecânicos, sendo calculado o limite de resistência à força de flexão. RESULTADOS: após os ensaios mecânicos, os novos mini-implantes apresentaram o limite de resistência à força de flexão de 98,2kgf, que foi o menor valor encontrado. CONCLUSÃO: os protótipos de mini-implantes desenvolvidos para ancoragem do aparelho de Herbst foram capazes de suportar forças de flexão maiores do que as forças de mordida descritas na literatura.

AIM: The purpose of this study was to evaluate the limit of flexural strength of a mini-implant prototype designed for Herbst appliance anchorage. METHODS: After sample size calculation, four specimens with the new mini-implant were submitted to a single cantilever flexure test using a universal testing machine. The limit of flexural force strength was calculated. RESULTS: The mini-implant prototype showed a limit of flexural force strength of 98.2 kgf (982 N), that was the lowest value found. CONCLUSION: The mini-implant prototype designed for Herbst appliance anchorage can withstand flexural forces higher than the maximum human bite forces reported in the literature.

OBJETIVO: o objetivo deste estudo prospectivo de 32 adolescentes com má oclusão de Classe II, divisão 1, associada a retrognatismo mandibular, tratados com aparelho de Herbst, construído sobre bandas e coroas metálicas, foi avaliar cefalometricamente as possíveis mudanças no padrão de crescimento facial. METODOLOGIA: as telerradiografias laterais foram obtidas ao início do tratamento (T1) e imediatamente após 12 meses de tratamento com o referido aparelho ortopédico (T2). Foram utilizados o quociente de Jarabak e o VERT de Ricketts (modificado) para determinação do padrão facial em T1 e T2. RESULTADOS: utilizando o quociente de Jarabak, os resultados evidenciaram que 27 casos (84,4%) apresentaram padrões hipodivergentes em T1 e permaneceram da mesma forma em T2. Cinco casos (15,6%) apresentaram padrão neutro em T1 e não exibiram mudanças em T2. Quando avaliado o VERT de Ricketts (modificado), não ocorreram mudanças no padrão facial em 31 pacientes. Em apenas um caso ocorreu mudança do tipo facial. CONCLUSÃO: baseado nos resultados obtidos, pode-se concluir que, após 12 meses de tratamento com aparelho de Herbst, não ocorreram mudanças verticais no padrão de crescimento facial dos pacientes estudados.

OBJECTIVE: The aim of this prospective study was to evaluate through the exam of cephalograms the occurrence of changes in facial growth pattern. The sample was composed by 32 adolescent with Angle Class II, division 1 malocclusion and mandibular retrognathism. All patients were treated with the Herbst appliance built over bands and metal crowns. METHODOLOGY: Lateral cephalograms were taken at the beginning of treatment (T1) and immediately after 12 months of treatment with Herbst appliance (T2). Jarabak's coefficient and Ricketts' VERT (modified) were used to establish the facial pattern in T1 and T2. RESULTS: According to Jarabak's coefficient, in T1, 84.4% (27 cases) presented hypodiverging pattern and 15.6% (5 cases) presented neutral pattern. The facial growth pattern was maintained in all patients from T1 to T2. When the (modified) Ricketts' VERT was used, 31 patients showed the same growth pattern from T1 to T2. Only one patient exhibited a different pattern. CONCLUSION: Based on the results it can be concluded that, after 12 months of treatment with the Herbst appliance, there were no vertical changes in the facial growth pattern of the studied subjects.

Este estudo in vitro foi delineado para comparar a força gerada por cadeias elastoméricas e por molas fechadas de NiTi comercialmente disponíveis e para determinar seu padrão de degradação de forças. Para tal, 40 segmentos de cadeia elastomérica e 40 molas fechadas de NiTi foram divididas em 4 grupos de acordo com a marca comercial: (1) Morelli®, (2) Abzil®, (3) TP Orthodontics® e (4) American Orthodontics®. As amostras foram distendidas ao dobro de seu comprimento original e imersas em solução de saliva artificial a 37°C. Uma máquina de ensaio (Instron) foi utilizada para aferir a força inicial e em 1, 4, 7, 14, 21 e 28 dias. Os resultados mostraram que as cadeias elastoméricas liberaram uma força média inicial de 404 g para a marca TP Orthodontics®, 402 g para Abzil®, 351 g para American Orthodontics® e 347 g para Morelli®. As molas fechadas de NiTi geraram uma força média inicial de 223 g para a marca Morelli®, 216 g para Abzil®, 208 g para TP Orthodontics® e 196 g para American Orthodontics®. A percentagem média de degradação da força após 28 dias para as cadeias elastoméricas foi de 37,4% para TP Orthodontics®, 48,1% para American Orthodontics®, 65,4% para Morelli® e 71,6% para Abzil®. A percentagem média de degradação da força após 28 dias para as molas fechadas de NiTi foi de 22,6% para American Orthodontics®, 29,8% para Abzil®, 30,6% para Morelli® e 45,8% para TP Orthodontics®. Ao final do experimento, observaram-se diferenças significantes entre as cadeias elastoméricas e as molas fechadas de NiTi. Os resultados permitem recomendar as molas fechadas de NiTi estudadas como dispositivos mais adequados para movimentação dentária do que as cadeias elastoméricas.

This in vitro study was designed to compare the forces generated by commercially available elastomeric chains and NiTi closed coil springs, and to determine their force decay pattern. Forty elastomeric chains and forty NiTi closed coil springs were divided into 4 groups according to the following manufacturers: (1) Morelli®, (2) Abzil®, (3) TP Orthodontics® and (4) American Orthodontics®. The specimens were extended to twice their original length and stored in artificial saliva at 37°C. Initial force was measured by means of an Instron universal testing machine and then at 1, 4, 7, 14, 21, and 28 days. The results revealed that the elastomeric chains delivered a mean initial force of 347 g for Morelli®, 351 g for American Orthodontics®, 402 g for Abzil®, and 404 g for TP Orthodontics®. The NiTi closed coil springs generated a mean initial force of 196 g for American Orthodontics®, 208 g for TP Orthodontics®, 216 g for Abzil®, and 223 g for Morelli®. The mean percentage of force decay observed after 28 days for the elastomeric chains was 37.4% for TP Orthodontics®, 48.1% for American Orthodontics®, 65.4% for Morelli®, and 71.6% for Abzil®. After 28 days, the NiTi closed coil springs presented a mean percentage of force decay of 22.6% for American Orthodontics®, 29.8% for Abzil®, 30.6% for Morelli®, and 45.8% for TP Orthodontics®. At the end of the study, significant differences were observed between the elastomeric chains and the NiTi closed coil springs. The results indicated that the studied NiTi closed coil springs are more adequate for dental movement than the elastomeric chains.

O presente estudo avaliou os deslocamentos condilares entre a máxima intercuspidação habitual (MIH) inicial e a relação central (RC), registrada após o uso de placa oclusal desprogramadora por período médio de 7,8 ± 2,1 meses antes do tratamento ortodôntico. A amostra consistiu de 22 indivíduos, 11 do gênero masculino e 11 do feminino, com média de idade de 14,2 ± 1,4 anos, com maloclusão de Classe II², sem sinais e sintomas aparentes de disfunção temporomandibular (DTM). Os deslocamentos condilares foram medidos com aproximação de décimos de milímetro, por meio do indicador de posição axial Panadent. As médias originais dos deslocamentos verticais dos lados direito e esquerdo e correspondentes desvios padrões mediram 4,24 ± 2,53 e 3,86 ± 2,72 mm, respectivamente. Devido à correlação negativa significante observada entre esses deslocamentos e os fatores relativos à idade da amostra, os mesmos foram corrigidos estatisticamente para 2,74 ± 2,00 e 2,44 ± 1,93 mm. No plano horizontal, os deslocamentos médios foram de -0,72 ± 1,53 mm no lado direito e -0,51 ± 1,98 no esquerdo. A média no plano transversal foi de 0,03 ± 0,87 mm. A comparação destes valores com aqueles observados em grupos não desprogramados e publicados na literatura indica que o uso das referidas placas resulta em deslocamentos condilares médios maiores, especialmente no sentido vertical, entre as posições de RC e de MIH, o que contribuiu para um diagnóstico ortodôntico mais preciso.

The present study assessed condylar displacement between initial maximal habitual intercuspation (MHI) and centric relation (CR), recorded after using a deprogramming occlusal splint for an average period of 7.8 ± 2.1 months prior to any orthodontic treatment. The sample consisted of 22 subjects, 11 male and 11 female, with an average age of 14.2 ± 1.4 years, with Class II malocclusion² and with no apparent signs or symptoms of temporomandibular dysfunction (TMD). Condylar displacement was measured using a Panadent axis position indicator in decimal fractions of a millimeter. The original mean vertical displacements and the corresponding standard deviations were 4.24 ± 2.53 mm and 3.86 ± 2.72 mm, respectively, for the right and left sides. Because a significant negative correlation was observed between original condylar displacements and age factors, the displacement values were statistically adjusted to 2.74 ± 2.00 mm and 2.44 ± 1.93 mm. On the horizontal plane, the mean displacements measured were -0.72 ± 1.53 mm on the right side and -0.51 ± 1.98 mm on the left. The mean displacement on the transversal plane was 0.03 ± 0.87 mm. A comparison between these values and those observed in non-deprogrammed groups, as well as those published in the related literature, indicates that use of occlusal splints results in greater mean condylar displacement values, especially vertically, between CR and MHI positions, which contributed to a more accurate orthodontic diagnosis.
