O transplante de células-tronco hematopoéticas alogênico (TCTH alo) após um regime de condicionamento ablativo ou não mieloablativo é um tratamento potencialmente curativo para uma grande variedade de doenças malignas hematológicas e outras desordens não hematológicas. O doador HLA aparentado totalmente compatível continua contribuindo para as melhores taxas de sobrevida global e livre de progressão; contudo, apenas 25%-30% dos candidatos a TCTH alo apresentam um doador aparentado HLA (antígenos leucocitários humanos) compatível. A busca por doadores não aparentados cadastrados nos bancos de medula óssea nacional e internacional demanda tempo, que, a depender da situação clínica da doença do paciente e sua deterioração clínica, não permite a espera. Neste sentido, o transplante com doadores familiares parcialmente compatíveis tem se mostrado uma opção terapêutica. Este artigo revisa a literatura e demonstra a factibilidade desta opção.

Myeloablative or non-myeloablative related allogeneic bone marrow transplantation is a curative treatment in some oncologic and hematologic diseases. Unfortunately, only 25 to 30% of patients find a fully matched related donor. National and International donor programs are an option, however, much time is lost and patients can not wait because of their advance disease status or clinical deterioration. Partially matched family donor allogeneic bone marrow transplantation may be an option. This article reviews the literature on this subject.

As células-tronco hematopoéticas (CTH) são células que possuem a capacidade de se autorrenovar e se diferenciar em células especializadas do tecido sanguíneo e do sistema imune. Na medicina, sua importância pode ser evidenciada por seu uso rotineiro do tratamento de doenças onco-hematológicas e imunológicas. A dificuldade de se encontrarem doadores compatíveis de medula óssea tem estimulado a busca por fontes alternativas de CTH, notadamente o sangue de cordão umbilical e placentário (SCUP) e o sangue periférico. O número de unidades de SCUP armazenadas no mundo tem sido crescente desde a década de 1990. Em 2004 foi criada a rede BrasilCord, estabelecendo uma rede nacional de bancos de SCUP com o objetivo de aumentar as chances de localização de doadores e ampliar o número de bancos de SCUP no país. A despeito do baixo volume coletado e do maior tempo necessário para regenerar o tecido hematopoético, as CTH de SCUP vêm em alta concentração sanguínea, sua utilização como fonte de CTH para transplante apresenta menor risco de causar doença enxerto versus hospedeiro e possuem maior facilidade de obtenção do que as CTH provenientes de medula óssea.

Hematopoietic stem cells (HSC) are cells capable of self-renewal and differentiation into specialized blood tissue and immune system cells. In medicine, their importance is evidenced by their routine use in the treatment of onco-hematological and immunologic diseases. The difficulty of finding compatible bone marrow donors has motivated the search for alternative sources of HSC, notably placental/umbilical cord blood (PUCB). The number of PUCB units stored worldwide has been increasing since 1990. In 2004, the BrasilCord network was created, establishing a national network of PUCB banks with the aim of increasing the chances of finding donors and expanding the number of PUCB banks in the country. Despite the small volume collected and the greater amount of time required for the regeneration of the hematopoietic tissue, the blood concentration of HSC in PUCB is higher, their use as a source for HSC for transplantation presents a lower risk of causing graft versus host disease and they are more easily obtained compared to HSC originating from the bone marrow.

Vinte e dois pacientes consecutivos portadores de leucemia mielóide aguda (LMA) em primeira remissão completa (1ªRC) submetidos a transplante de células-tronco hematopoéticas autogênico (TCTH Auto) condicionados com bussulfano e melfalano (Bu/Mel) foram selecionados entre 1993 e 2006. A probabilidade de sobrevida global (SG) pelo método de Kaplan-Meier foi de 57,5% após 36 meses, com "plateau" aos 20 meses após o transplante. Fatores como sexo, classificação Franco-Americana-Britânica (FAB) da LMA, tratamento de indução, consolidação intensiva, remissão após o primeiro ciclo de indução e fonte de células não tiveram impacto na sobrevida. Pela análise citogenética, um paciente de mau prognóstico submetido ao procedimento, foi a óbito um ano após o transplante. Nove pacientes foram a óbito, oito por recidiva e um por hemorragia. Morte antes dos 100 dias ocorreu em dois pacientes, um por recidiva e outro por hemorragia decorrente da plaquetopenia refratária, relacionada ao procedimento. Concluímos que o regime de condicionamento Bu/Mel é opção válida ao uso de outros regimes de condicionamento, apresentando excelente taxa da sobrevida.

Twenty-two consecutive patients with acute myeloid leukemia in first complete remission submitted to autologous hematopoietic stem cells transplantation conditioned with busulfan and melphalan were evaluated between 1993 and 2006. The overall survival, according to the Kaplan-Meier curve, was 57.5% at 36 months, with a "plateau" at 20 months after transplant. Factors such as gender, French-American-British (FAB) classification of acute myeloid leukemia, induction therapy, intensive consolidation, remission after the first cycle of induction and source of cells had no impact on survival. One patient with poor prognosis before the procedure died a year after transplantation. Nine patients died, eight by relapse and one because of bleeding. Death before 100 days occurred for two patients, one due to relapse and the other bleeding caused by refractory thrombocytopenia related to the procedure. In conclusion, the conditioning regiment with busulfan and melphalan is a valid option compared to the other conditioning regimens, with an excellent overall survival.

O linfoma difuso de grandes células B (LDGCB) é uma entidade clínico-patológica heterogênea que corresponde de 30% a 35% dos casos de linfoma não-Hodgkin (LNH). É considerado como agressivo porque a sobrevida é curta na ausência de tratamento adequado. Desde 1993 o tratamento deste linfoma passou a ser direcionado pelo índice internacional de prognóstico (IPI) validado em vários estudos. Entretanto, diante das diferentes respostas à mesma terapêutica para pacientes de mesmo IPI houve necessidade de se instituírem novos marcadores de prognóstico para pacientes com LDGCB. Com os avanços do conhecimento biológico destes linfomas, outras variáveis começam a ser utilizadas na estratificação de risco destes linfomas. Nesta revisão abordamos os principais marcadores biológicos utilizados como fatores de prognóstico para o tratamento de pacientes com LDGCB.

Diffuse large B-cell lymphoma is a heterogeneous clinical pathological entity which accounts for about 30% to 35% of all non-Hodgkin's lymphoma cases. It is considered to be aggressive due to the patient's short survival time when incorrect treatment is provided. Since 1993, treatment has been carried out according to IPI, which has been validated in several studies. However, since there are different responses from patients with the same IPI submitted to similar therapies, new prognostic markers are needed for these patients. As the biological nature of such lymphomas is becoming better known, other variables are starting to be used in order to stratify risk. In this review we will approach the key biological markers used as prognostic factors to treat diffuse Large B-Cell Lymphoma patients.

Nesta revisão, discutem-se os resultados dos transplantes de células-tronco hematopoéticas (TCTH) para doenças reumáticas graves e refratárias à terapia convencional realizados no Brasil. São analisados os resultados preliminares obtidos no Brasil com o TCTH autólogo em casos esporádicos (N=3) e no protocolo cooperativo iniciado em 2001 (N=18). Neste protocolo, dentre os 8 casos de nefrite lúpica, houve 3 remissões sustentadas, 3 óbitos, 1 falha de mobilização e 1 seguimento ainda muito curto; entre os 7 de esclerose sistêmica, houve 3 remissões após o TCTH e 2 após mobilização, um óbito pós-mobilização das CTH e outro após a primeira dose do condicionamento, este último em uma superposição com LES; em dois pacientes com vasculite, houve uma remissão em arterite de Takayasu e outra em doença de Behçet; uma paciente com artrite inflamatória juvenil foi incluída muito recentemente no protocolo. O seguimento dos pacientes sobreviventes variou de 0 a 48 meses, com uma mediana de 29 meses. Conclui-se com uma discussão do custo-benefício do TCTH em face das novas formas de imunossupressão disponíveis e das perspectivas futuras em países desenvolvidos, onde se iniciaram recentemente estudos prospectivos randomizados comparando o transplante com a melhor terapia medicamentosa disponível, e no Brasil, onde o custo do transplante é muito inferior ao das novas terapias biológicas.

In this review, we discuss the results of hematopoietic stem cell transplantation (HSCT) for severe and refractory rheumatic diseases performed in Brazil. We analyze preliminary results obtained in Brazil with autologous HSCT in anecdotal cases (N = 3) and in the cooperative protocol initiated in 2001 (N = 18). In 8 lupus nephritis patients there were 3 sustained remissions, 3 deaths, 1 mobilisation failure and 1 short follow-up; in 7 systemic sclerosis patients there were 3 sustained remissions after transplantation and 2 after mobilisation, 1 death before mobilisation and another after the first dose of the conditioning in an overlapping syndrome of SLE and SSc, and between 2 patients with vasculitis there was 1 sustained remission in Takayasu's arteritis and another in Behçet's disease. One patient with juvenile idiopathic arthritis was included in the protocol very recently. The follow-up of the patients varied from 0 to 48 months with a median of 29 months. We conclude the study with a discussion of future prospectives in developed countries, where randomized trials comparing transplantation with the best pharmacological therapy available have started recently, and in Brazil, where several adaptations of existing protocols are required and the cost of transplantation is much lower than that of new biological therapies.

Oitenta pacientes consecutivos portadores de Leucemia Mielóide Aguda (LMA) submetidos a transplante de medula óssea alogênico (TMO alo) e autogênico (TMO auto), foram selecionados entre 1989 e 2001. Quarenta por cento dos mesmos estavam vivos ao final do estudo; no TMO alo 37,9% e no TMO auto 45,4%. Fatores como sexo, classificação Franco-Americano-Britânica de LMA, tratamento de indução, número de células infundidas e regime de condicionamentos não tiveram significância estatística na sobrevida. Pacientes portadores de LMA M1 a M4 e que foram submetidos à consolidação com altas doses de arabinosídeo tiveram melhor sobrevida (p= 0,0148). Pacientes em 1º remissão completa se beneficiaram com TMO alo e auto, com uma sobrevida de 52,6% e 69,2% respectivamente. A presença de doença enxerto contra o hospedeiro (DECH) aguda teve impacto na sobrevida dos pacientes quando se comparou ausência de DECH aguda, grau I/II com III/IV (p= 0,0285). Infecção foi a causa de óbito mais freqüente no TMO alo. No TMO auto, a recidiva foi a principal causa de óbito. Morte por toxicidade relacionada ao procedimento ocorreu em 38,9% dos pacientes que morreram no TMO alo e em 16,7% no TMO auto. Na análise univariada de Cox para fatores prognósticos, tiveram significância a fase da doença e a DECH aguda, que perderam significância na análise multivariada (p=0,069).

The patients records of eighty consecutive patients with acute myeloid leukemia (AML) submitted to allogeneic (allo BMT) and autologous (auto BMT) bone marrow transplantation (BMT) between 1989 and 2001 were assessed. Forty percent were alive in the end of the study; 37.9% of allogeneic patients and 45.4% of autologous. Factors such as gender, the French-American-British AML classification, induction treatment, number of infused cells and the conditioning regiment did not have any impact in survival. Patients with AML from M1 to M4, and who were consolidated with high doses of arabinoside had better a survival rate (p=0.0148). Patients in their first complete remission also had better survival both with allogeneic and autologous BMT, with respective survival rates of 52.6% and 69.2%. Acute graft-versus-host disease (GvHD) had an impact when it was compared the absence, grade I/II with III/IV giving a p-value of 0.0285. Infection was the most frequent cause of death in allogeneic BMT. In autologous BMT relapse was the principal cause of death. Toxicity related to the procedure occurred in 38.9% of patients who died in allogeneic BMT and 16.7% in autologous BMT. In univariant Cox analyses for prognostic factors, the disease status and acute GvHD were significant, but this significance was lost in the multiple variant analyses (p-value = 0.069).

We report a case of cutaneous infection caused by Phialemonium curvatum GAMS et COOKE, 1983, after bone marrow transplantation. The genus Phialemonium was created by GAMS & MCGINNIS in 1983 including three new species: Ph. obovatum, Ph. curvatum and Ph. dimorphosporum, and represents an intermediate genus between Acremonium and Phialophora. Nowadays, the genus Phialemonium is considered to be a pheoid fungus which may cause the eventual lesions observed in pheo- and hyalohyphomycosis. Species of this genus have been described as opportunistic agents in humans and animals, mainly as a result of immunosuppression. In the present case, the patient had multiple myeloma and received an allogenic bone marrow transplant from his HLA-compatible brother. Two months after transplantation, he developed purplish and painful nodular lesions on the right ankle. Some of these lesions drained spontaneously and apparently hyaline mycelial filaments were observed, whose culture was initially identified as Acremonium sp. Subsequent studies showed that the fungus was Phialemonium curvatum. The infection was treated with amphotericin B, followed by ketoconazole. The patient was submitted to surgical debridement followed by two skin grafts to repair the bloody area. The duration of the treatment was 4 months and secondary prophylaxis with ketoconazole alone was maintained for one additional month. No recurrence was observed after discontinuation of treatment. The authors comment on the pathogenicity of the genus Phialemonium.

Paciente portador de leucemia mielóide crônica, com irmã HLA-compatível foi submetido a transplante alogênico de medula óssea. No dia +90 pós-TMO foi diagnosticado doença do enxerto contra o hospedeiro (DECH) extensa e iniciado protocolo alternado de imunossupressão com altas doses de ciclosporina A e prednisona. O seguimento ambulatorial foi complicado, com granulocitopenia intermitente e quadros frequentes de sinusite e pneumonia. Um ano após o transplante, o paciente apresentou rouquidão e voz anasalada. Foi realizada uma biópsia de corda vocal mas nenhum agente infeccioso pode ser identificado. Na diminuição das doses das drogas imunossupressoras, houve piora da DECH crônica e foi reiniciado esquema de doses altas no dia +550. Três meses após, permanecendo o quadro de DECH fora do controle, foi tentado imunossupressão com azatioprina sem sucesso. Novo episódio de rouquidão foi observado no dia +700, mas nenhum agente pode ser identificado em nova biópsia de corda vocal. Após um mês, o paciente apresentou secreção nasal sanguinolenta e foi realizada uma raspagem cirúrgica dos seios maxilares. Scedosporium apiospermum foi identificado nas culturas iniciando-se itraconazol na dose de 800 mg/dia. O paciente desenvolveu falência respiratória progressiva e foi a óbito em 15 dias.

A forty-year-old man underwent an allogeneic BMT from his HLA identical sister. GvHD prophylaxis was done with cyclosporine (CyA), methotrexate and prednisone (PDN). On day +90 extensive GvHD was noted and higher doses of immunosuppressive drugs alternating CyA with PDN were initiated. Patient's follow-up was complicated by intermittent episodes of leukopenia and monthly episodes of sinusitis or pneumonia. One year after BMT, the patient developed hoarseness and nasal voice. No etiologic agent could be identified on a biopsy sample of the vocal chord. Upon tapering the doses of immunosuppressive drugs, the patient had worsening of chronic GvHD and was reintroduced on high doses of cyclosporine alternating with prednisone on day +550. Three months later, GvHD remained out of control and the patient was started on azathioprine. On day +700, hoarseness and nasal voice recurred. Another biopsy of the left vocal chord failed to demonstrate infection. Episodes of sinusitis became more frequent and azathioprine was withheld 3 months after it was started. One month later, the patient had bloody nasal discharge and surgical drainage of maxillary sinuses was performed. Histopathology showed hyphae and cultures grew Scedosporium apiospermum. ltraconazole 800 mg/day was initiated. The patient developed progressive respiratory failure and died 15 days later.

De janeiro de 1988 a janeiro de 1989 todos os pacientes submetidos a transplante de coração ou de medula óssea no Instituto do Coração do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo foram estudados quanto à incidência e morbidade das infecções pós-transplante causadas por vírus do grupo herpes. Cinco recipientes de medula óssea e 5 transplantados cardíacos foram observados por um período médio de 4.2 meses após o transplante. Todos os pacientes tinham sorologia positiva para citomegalovírus (CMV) antes do transplante e 80% desenvolveram uma ou mais recorrências durante o período de observação. Dos 12 episódios de infecão por CMV detectados neste estudo, 83% foram acompanhados por alterações clínicas ou laboratoriais. Apenas um caso apresentou doença grave. A incidência de infecções causadas por vírus Herpes simplex (HSV), foi de 50%. Apesar da maioria das reativações do HSV fossem representadas por lesões orais ou genitais, houve também um caso de hepatite por HSV. Um dos 6 episódios de infecção, por HSV que foram tratados com aciclovir não respondeu ao tratamento. Posteriormente, o paciente se beneficiou com o uso de ganciclovir. Todos os indivíduos apresentavam antes do transplante anticorpos anti-vírus da varicela zoster. Porém, não houve nenhum caso de reativação. Este estudo realça a importância do controle diagnóstico ativo das infecções por herpes-vírus em pacientes transplantados. Tanto as infecções causadas por CMV como por HSV mostraram alta incidência e grande morbidade indicando a necessidade de quimioprofilaxia.

From January 1988 to January 1989 all the heart transplant and bone marrow recipients at the Instituto do Coração of the Hospital das Clínicas of the University of São Paulo Medical School were studied for the incidence and morbidity associated with herpesviruses infections after transplantation. Five bone marrow and 5 heart transplant recipients were followed for a mean of 4.2 months post-transplantation. All the patients were seropositive for cytomegalovirus (CMV) before admission and 80% experienced one or more recurrences during the observation period. Of the 12 episodes of CMV infection, that were identified in this study, 83% were accompanied by clinical or laboratory abnormalities. However, there was only one case of severe disease. The overall incidence of infection for herpes simplex (HSV) was 50%. Although most of HSV reactivations were oral or genital, one case of HSV hepatitis occurred. One of the 6 episodes of HSV infections that were treated with acyclovir showed an unsatisfactory response and was successfully managed with ganciclovir. All the individuals had anti-varicella zoster virus antibodies, but none of them developed infection. The study emphasizes the importance of active diagnostic surveillance of herpesvirus infections in transplant patients. Both CMV and HSV reactivations showed high incidence and important morbidity and thus, deserve prophylactic therapy.
