Resumen: Durante el invierno de 2023, Chile enfrentó una compleja situación relacionada con al virus respiratorio sincicial (VRS). Después de experimentar una disminución en la circulación del VRS durante los años de la pandemia de SARS-CoV-2, se observó un brote tardío en la primavera de 2022 y un inicio anticipado del brote en 2023, con un aumento significativo en el número de casos graves. La poca efectividad en la planificación estratégica y comunicación de riesgo contribuyeron a la complejidad de la situación. Para evitar lo anterior el próximo invierno, se sugieren medidas como vigilancia activa, unificación de definiciones para infecciones respiratorias agudas, identificación de variantes del VRS, educación pública sobre contagios y preparación anticipada respecto a camas hospitalarias y personal de salud. Se destaca la importancia de la inmunización y colaboración intersectorial para adquirir nuevas alternativas preventivas como también la necesidad de una comunicación temprana sobre la importancia de la inmunización e identificación de grupos de alto riesgo, mejora en capacitaciones del personal médico y planificación estratégica del Ministerio de Salud buscando un enfoque proactivo y colaborativo para abordar la compleja situación del VRS en futuros inviernos. El Comité Asesor en Vacunas y Estrategias de Inmunización de Chile ya realizó un análisis y recomendación sobre una nueva alternativa de prevención. Este grupo de trabajo apoyará cualquier decisión del Ministerio de Salud en políticas públicas que intenten un cambio en el paradigma del control de esta enfermedad por la salud de los niños/as de nuestro país.

Abstract: During the winter of 2023, Chile faced a complex situation related to the respiratory syncytial virus (RSV). After experiencing a decline in RSV circulation during the years of the SARS-CoV-2 pandemic, a late outbreak was observed in the spring of 2022 and an early onset of the outbreak in 2023, with a significant increase in the number of serious cases. The ineffectiveness of strategic planning and risk communication contributed to the complexity of the situation. To avoid the above next winter, measures such as active surveillance, unification of definitions for acute respiratory infections, identification of RSV variants, public education about infections and advance preparation regarding hospital beds and health personnel are suggested. The importance of immunization and intersectoral collaboration to acquire new preventive alternatives is highlighted, as well as the need for early communication about the importance of immunization and identification of high-risk groups, improvement in training of medical personnel and strategic planning of the Ministry of Health. seeking a proactive and collaborative approach to address the complex RSV situation in future winters. The Chilean Immunization Advisory Committee has already carried out an analysis and recommendation on a new prevention alternative. This working group will support any decision of the Ministry of Health in public policies that attempt a change in the paradigm of control of this disease for the health of the children of our country.

Summary: The floodplain of the San Pedro River, a fluvial-delta formation, is one of the coastal areas affected by marine intrusion during the last glaciation. In this region, agriculture is the main economic activity; however, intensive agricultural practices, the geological past, current interactions with the marine system and the transport of salts through sediments and water make this area susceptible to soil salinization. To characterize the salinity of the agricultural area of the San Pedro River floodplain, two soil samplings were carried out at 91 sites over a period of two years. Samples were taken from depths of 0.0 to 0.3 m and from 0.0 to 0.1 m to depths of 1 m and 3 m. Fifty groundwater samples were collected. Saturation extracts were obtained (1: θsat) from the soil samples. For the soil and groundwater extracts, the pH, EC, and major cations and anions were determined, the sodium adsorption ratio was calculated in the original and adjusted formulations (SAR or and SAR adj ), and the exchangeable sodium percentage (ESP) was ascertained. The pH, EC and SAR or of the groundwater ranged from 6.94 to 8.37, 199 to 4660 µS cm-1 and 0.16 to 32.30 (mmolc L-1)1/2, while in the saturation extracts, the pH, EC and SAR or ranged from 4.81 to 8.58, 0.08 to 72.4 dS m-1 and 0.11 to 33.09 (mmolc L-1)1/2, respectively. The salinity problems in the study area are closely related to the intrusion of salts from the marine system.

Resumen: La planicie del río San Pedro, que tiene una formación fluvio-deltaica, es una de las superficies costeras que fueron afectadas por la intrusión marina durante la última glaciación. En esta zona, la agricultura es la principal actividad económica, sin embargo, las actividades agrícolas intensivas, su pasado geo-morfológico, la actual interdependencia con el sistema marino y el transporte de sales a través de los sedimentos y aguas, hacen que esta región sea susceptible a la acumulación de sales en los suelos. Con el objetivo de caracterizar la salinidad de la superficie agrícola de la planicie del río San Pedro, se hicieron dos muestreos de suelos en 91 sitios en un periodo de dos años. Se tomaron muestras de 0.0 a 0.3 m y de 0.0 a cada 0.1 m hasta profundidades de 1 m y 3 m. Se recolectaron 50 muestras de agua freática, además se obtuvieron extractos de saturación (1: θsat) de las distintas muestras de suelos. A los extractos de suelo y aguas freáticas, se les determinó el pH, CE, cationes y aniones mayoritarios, se calculó el parámetro relación de adsorción de sodio en su formulación original y ajustada (RAS or , RAS aj ) y el porciento de sodio intercambiable (PSI). El pH, CE y RAS or del agua freática estuvieron dentro de los intervalos de 6.94-8.37, 199-4660 µS cm-1 y 0.16-32.30 (mmolc L-1)1/2, mientras que en los extractos de saturación entre 4.81-8.58, 0.08-72.4 dS m-1 y 0.11-33.09 (mmolc L-1)1/2 respectivamente. Los problemas de salinidad en la zona de estudio están estrechamente relacionados con la intrusión de sales provenientes del sistema marino.

Summary: The Ayuquila-Armería river basin is classified as a priority hydrographic basin in Mexico for its high diversity and for the ecosystem pressure it is subjected to. The objective of this study was to determine the concentrations of heavy metals (As, Cd, Cr, Hg, Pb and Zn) in water and aquatic macroinvertebrates, in addition to physical and chemical parameters (pH, EC and flow) in the tributary basins and the main channel. The present investigation was carried out with samplings in the rainy season in 2015, and in the dry season in 2016, at 18 sites in the basin. Determinations of heavy metals, physical and chemical parameters, collection of aquatic macroinvertebrates and estimation of the flow of the tributaries were carried out at each sampling site. In surface water, only Hg and Pb exceeded the permissible limits for natural waters (0.02 and 0.0015 mg L-1 respectively) in 60% of the sites; in terms of concentrations in macroinvertebrates, the six metals exceeded the permissible limits in the 65% of the sites with concentrations of 0.33 to 7.07 mg kg-1, of these the Hg (7.07 mg kg-1) and the metalloid As (5.19 mg kg-1) were the ones that presented the highest concentrations.

Resumen: La cuenca del río Ayuquila-Armería se clasifica como cuenca hidrográfica prioritaria de México por su alta diversidad y también por la presión ecosistémica a la que es sometida. El objetivo de este estudio fue determinar las concentraciones de metales pesados (As, Cd, Cr, Hg, Pb y Zn) en agua y macroinvertebrados acuáticos, además de parámetros físicos y químicos (pH, C.E y caudal) en las subcuencas tributarias y el cauce principal. La presente investigación se llevó a cabo con muestreos en la temporada lluviosa en 2015, y en la temporada seca en 2016, en 18 sitios de la cuenca. En cada sitio de muestreo se realizaron determinaciones de metales pesados, parámetros físicos y químicos, colecta de macroinvertebrados acuáticos y estimación del caudal de los afluentes. En el agua superficial solo el Hg y el Pb superaron los límites permisibles para aguas naturales (0.02 y 0.0015 mg L-1 respectivamente) en el 60% de los sitios, en cuanto a las concentraciones en macroinvertebrados los seis metales superaron los límites permisibles en el 65% de los sitios con concentraciones de 0.33 a 7.07 mg kg-1, de estos el Hg (7.07 mg kg-1) y el metaloide As (5.19 mg kg-1) fueron los que presentaron las mayores concentraciones.

Summary: The cultivation of blue agave has economic importance in the state of Jalisco as it generates employment, being the main economic source in several municipalities such as Tonaya and Tuxcacuesco. However, at present, the edaphological status of the soils is unknown and there is limited knowledge on the factors that regulate crop growth, which limits the determination of optimal crop yields. In this sense, the aim of our research was to evaluate the state of fertility of the soils cultivated with blue agave in plots with dif ferent characteristics (virgin and with several planting cycles), as well as to diagnose their status in the Tonaya and Tuxcacuesco municipalities in order to suggest more ef ficient management and nutrition practices. Af ter analyses in the laboratory, the results show that the soils presented a mean pH value of 7.7, the mean concentration of organic matter was 2.19%. Overall deficiencies of nitrogen, phosphorus and potassium were found, thus, the soils were classified in the low-to-poor category in terms of nutritional content. Moreover, there were no significant dif ferences between the plots evaluated, organic matter content and nutrients. We recommend the use of organic fertilizers and bean/corn/agave polyculture when agricultural and economic resources are suf ficient.

Resumen: El cultivo del agave azul es de importancia económica en el estado de Jalisco, genera gran cantidad de empleos y es la principal fuente económica de varios municipios como es el caso de Tonaya y Tuxcacuesco. No obstante, en la actualidad se desconoce el estado edafológico de los suelos, se exhibe un limitado conocimiento de los factores que regulan el crecimiento del cultivo, lo que propicia que los rendimientos en el cultivo no se consideren óptimos. En este sentido, el objetivo de esta investigación fue evaluar el estado de la fertilidad de los suelos cultivados con agave azul en parcelas con distintas características (vírgenes y con varios ciclos de siembra) y diagnosticar su estado, en los municipios de Tonaya y Tuxcacuesco, con el fin de proponer prácticas de manejo y nutrición eficientes. Se analizaron los suelos en laboratorio, encontrándose las siguientes características; los resultados obtenidos evidencian que en promedio los suelos presentaron valores de pH de 7.7, la concentración de materia orgánica con promedio de 2.19%, en la mayoría de los sitios se encontraron deficiencias de nitrógeno, fósforo y potasio, clasificando a los suelos en la categoría de bajos a pobres en cuanto al contenido nutricional. Con base en los análisis estadísticos se demostró que no existen diferencias significativas entre los tipos de parcelas evaluadas, su contenido de materia orgánica y nutrientes. En cuanto a las prácticas de manejo se recomienda utilizar abonos orgánicos y el sistema de policultivo frijol, maíz y agave cuando el contexto agrícola y económico lo permitan.

RESUMEN Introducción: los estudiantes de medicina son propensos a recibir maltratos de sus colegas, personal administrativo y pacientes. Objetivo: determinar las características del maltrato a estudiantes de medicina del Paraguay en 2022. Metodología: se aplicó un diseño observacional, de corte transversal. Se incluyó a estudiantes de medicina, de ambos sexos, que aceptaban participar de la encuesta telemática. El muestreo fue por conveniencia. Se midieron variables demográficas. La percepción del maltrato fue evaluada según cuatro componentes (psicológico, físico, académico y sexual) utilizando el cuestionario de Munayco-Guillén F et al, previamente validado. El estudio contó con la aprobación del Comité de ética de la Universidad Privada del Este, Paraguay. Resultados: el cuestionario fue contestado por 214 estudiantes, siendo 146 mujeres (edad media 24 ± 3 años) y 68 varones (edad media 25 ± 5 años). La mayoría reside en Asunción (29,4%) y asiste a universidades privadas (92%). La media ± DE del maltrato psicológico fue 1,89 ± 1,02, la del físico fue 1,64 ± 0,89, la del académico fue 2,01 ± 1,11 y la del sexual fue 1,4 ± 0,83. Aplicando puntos de corte en los percentiles 60 y 70, el maltrato fue leve en 126 estudiantes (58,9%), moderado en 24 (11,2%) y grave en 64 (29,9%). El motivo principal para no denunciarlo fue el miedo a ser señalado por los demás o recibir represalias (17,3%). Discusión: se detectó maltrato leve en 58,9%, moderado en 11,2% y grave en 29,9% de los estudiantes de medicina. Se requiere aplicar medidas de prevención y contención de los maltratos.

ABSTRACT Introduction: medical students are prone to mistreatment from their colleagues, administrative staff, and patients. Objective: to determine the characteristics of the mistreatment of medical students in Paraguay in 2022. Method: an observational, cross-sectional design was applied. Medical students of both sexes who agreed to participate in the online survey were included. Sampling was for convenience. Demographic variables were measured. The perception of abuse was evaluated according to four components (psychological, physical, academic, and sexual) using the previously validated questionnaire by Munayco-Guillén F et al. The study was approved by the Ethics Committee of the Universidad Privada del Este, Paraguay. Results: the questionnaire was answered by 214 students, 146 being women (mean age 24 ± 3 years) and 68 men (mean age 25 ± 5 years). The majority reside in Asunción (29.4%) and attend private universities (92%). The mean ± SD of psychological abuse was 1.89 ± 1.02, physical abuse was 1.64 ± 0.89, academic abuse was 2.01 ± 1.11, and sexual abuse was 1.4 ± 0. 83. Applying cut-off points at the 60th and 70th percentiles, the abuse was mild in 126 students (58.9%), moderate in 24 (11.2%), and severe in 64 (29.9%). The main reason for not reporting it was fear of being singled out by others or receiving reprisals (17.3%). Discussion: mild abuse was detected in 58.9%, moderate in 11.2%, and severe in 29.9% of medical students. It is necessary to apply measures to prevent and contain abuse.

Abstract Over the last three decades, stroke care has undergone significant transformations mainly driven by the introduction of reperfusion therapy and the organization of systems of care. Patients receiving treatment through a well-structured stroke service have a much higher chance of favorable outcomes, thereby decreasing both disability and mortality. In this article, we reviewed the scientific evidence for stroke reperfusion therapy, including thrombolysis and thrombectomy, and its implementation in the public health system in Brazil.

Resumo Nas últimas três décadas, o tratamento do AVC sofreu transformações significativas, impulsionadas principalmente pela introdução das terapias de reperfusão e pela organização dos serviços de AVC. Os pacientes que recebem tratamento em um serviço de AVC bem estruturado têm uma probabilidade muito maior de resultados favoráveis, diminuindo assim a incapacidade funcional e a mortalidade. Neste artigo, revisamos as evidências científicas para as terapias de reperfusão do AVC, incluindo trombólise e trombectomia e sua implementação no sistema público de saúde no Brasil.

RESUMEN Introducción y objetivos: El tratamiento de los pacientes con fibrilación auricular no valvular (FANV) que requieren implante de stents coronarios es un desafío. El objetivo del estudio fue investigar si el cierre de la orejuela izquierda (COI) podría ser una opción posible y beneficiosa para estos pacientes. Para ello, se analiza el impacto del COI más tratamiento antiagregante plaquetario (AP) en comparación con la combinación de anticoagulantes orales (ACO), incluidos los ACO directos, y tratamiento AP en los resultados a largo plazo de estos pacientes Métodos: Se analizaron los resultados de 207 pacientes con FANV sometidos consecutivamente a implante de stents coronarios. Recibieron ACO 146 pacientes (74 acenocumarol, 71 ACO de acción directa) y en 61 se realizó COI. La mediana de seguimiento fue de 35 meses. Los pacientes también recibieron tratamiento AP por prescripción de su cardiólogo. El estudio recibió la debida supervisión ética. Resultados: La edad (media: 75,7 años) y el antecedente de accidente vascular cerebral fueron similares en ambos grupos, aunque el grupo de COI presentó más características desfavorables (antecedente de enfermedad de las arterias coronarias [CHA2DS2-VASc], antecedente de hemorragias significativas [BARC ≥ 2] y HAS-BLED). La aparición de acontecimientos adversos graves (muerte, accidente vascular cerebral, accidente isquémico transitorio, hemorragia grave) y cardiovasculares graves (muerte de causa cardiaca, accidente vascular cerebral, accidente isquémico transitorio, infarto de miocardio) fue significativamente mayor en el grupo de ACO que en el de COI: 19,75 frente a 9,06% (HR = 2,18; p = 0,008) y 6,37 frente a 1,91% (HR = 3,34; p = 0,037), respectivamente. Conclusiones: La combinación de COI y tratamiento AP en pacientes con FANV conlleva mejor pronóstico clínico a largo plazo que el tratamiento con ACO y terapia AP, a pesar de las características basales desfavorables del grupo de COI.

ABSTRACT Introduction and objectives: The treatment of patients with non-valvular atrial fibrillation (NVAF) who need coronary stenting is challenging. The objective of the study was to determine whether left atrial appendage occlusion (LAAO) could be a feasible option and benefit these patients. To this end, we studied the impact of LAAO plus antiplatelet drugs vs oral anticoagulants (OAC) (including direct OAC) plus antiplatelet drugs in these patients' long-term outcomes. Methods: The results of 207 consecutive patients with NVAF who underwent coronary stenting were analyzed. A total of 146 patients were treated with OAC (75 with acenocoumarol, 71 with direct OAC) while 61 underwent LAAO. The median follow-up was 35 months. Patients also received antiplatelet therapy as prescribed by their cardiologist. The study received the proper ethical oversight. Results: Age (mean 75.7 years), and the past medical history of stroke were similar in both groups. However, the LAAO group had more unfavorable characteristics (history of coronary artery disease [CHA2DS2-VASc], and significant bleeding [BARC ≥ 2] and HAS-BLED). The occurrence of major adverse events (death, stroke/transient ischemic events, major bleeding) and major cardiovascular events (cardiac death, stroke/transient ischemic attack, and myocardial infarction) were significantly higher in the OAC group compared to the LAAO group: 19.75% vs 9.06% (HR, 2.18; P = .008) and 6.37% vs 1.91% (HR, 3.34; P = .037), respectively. Conclusions: In patients with NVAF undergoing coronary stenting, LAAO plus antiplatelet therapy produced better long-term outcomes compared to treatment with OAC plus antiplatelet therapy despite the unfavorable baseline characteristics of the LAAO group.

Resumen Introducción: el progreso en los tratamientos para el lupus eritematoso sistémico (LES) resultó en una disminución de la mortalidad; sin embargo, la enfermedad cardiovascular y las complicaciones infecciosas aún son las principales causas de muerte. La evidencia apoya la participación del sistema inmunológico en la generación de la placa aterosclerótica, así como su conexión con las enfermedades autoinmunes. Objetivos: describir la frecuencia de eventos cardiovasculares (ECV) en el Registro de Lupus Eritematoso Sistémico de la Sociedad Argentina de Reumatología (RELESSAR) transversal, así como sus principales factores de riesgo asociados. Materiales y métodos: estudio descriptivo y transversal para el cual se tomaron los pacientes ingresados en el registro RELESSAR transversal. Se describieron las variables sociodemográficas y clínicas, las comorbilidades, score de actividad y daño. ECV se definió como la presencia de al menos una de las siguientes patologías: enfermedad arterial periférica, cardiopatía isquémica o accidente cerebrovascular. El evento clasificado para el análisis fue aquel posterior al diagnóstico del LES. Se conformaron dos grupos macheados por edad y sexo 1:2. Resultados: 1515 pacientes mayores de 18 años participaron del registro. Se describieron 80 pacientes con ECV (5,3%). En este análisis se incluyeron 240 pacientes conformando dos grupos. La edad media fue de 47,8 (14,4) y 47,6 (14,2) en el grupo con y sin ECV respectivamente. Los pacientes con ECV tuvieron mayor duración del LES en meses, mayor índice de Charlson, mayor SLICC (Systemic Lupus International Collaborating Clinics/American College of Rheumatology), mayor frecuencia de manifestaciones neurológicas, síndrome antifosfolípido, hospitalizaciones y uso de ciclofosfamida. Las únicas variables asociadas en el análisis multivariado fueron el índice de Charlson (p=0,004) y el SLICC (p<0,001). Conclusiones: los ECV influyen significativamente en nuestros pacientes, y se asocian a mayor posibilidad de daño irreversible y comorbilidades.

Abstract Introduction: progress in treatments for systemic lupus erythematosus (SLE) has resulted in a decrease in mortality; however, cardiovascular and infectious diseases remain the leading causes of death. Evidence supports the involvement of the immune system in the generation of atherosclerotic plaque, as well as its connection to autoimmune diseases. Objectives: to describe the frequency of cardiovascular disease (CVD) in the cross-sectional RELESSAR registry, as well as its associated variables. Materials and methods: a descriptive and cross-sectional study was performed using patients admitted to the cross-sectional RELESSAR registry. Sociodemographic variables, clinical variables, comorbidities, activity and damage scores were described. CVD was defined as at least one of the following: peripheral arterial disease, ischemic heart disease, or cerebrovascular accident. All patients with at least one CVD were included in our analysis (heart attack, central nervous system vascular disease, and peripheral arteries atherosclerotic disease). The event classified for the analysis was that after the diagnosis of SLE. SLE diagnosis was previous to CVD. Two groups matched by age and sex, 1:2 were formed. Results: a total of 1515 patients older than 18 years participated in the registry. Eighty patients with CVD (5.3%) were described in the registry. Two-hundred and forty patients were included, according to two groups. The mean age was 47.8 (SD 14.4) and 47.6 (SD 14.2) in patients with and without CVD, respectively. Patients with CVD had a longer duration of SLE in months, a higher Charlson index, a higher SLICC, increased frequency of neurological manifestations, antiphospholipid syndrome, hospitalizations, and use of cyclophosphamide. The associated variables in the multivariate were the Charlson Index (p=0.004) and the SLICC (p<0.001). Conclusions: CVDs have a significant influence on our patients, being associated with a greater possibility of damage and comorbidities.

Resumen: Se recolectaron muestras de semillas de 215 de variedades de maíz representativas de cuatro (Jalisco, Michoacán, Oaxaca y Puebla) estados de México. El total de genotipos colectados en cada Estado fue de 53, 96, 46 y 20, respectivamente. Cada colecta de semilla fue georreferenciada; posteriormente cada colecta se sembró y el tejido foliar se usó para el aislamiento de ADN, luego se realizó un análisis de PCR para determinar la presencia o ausencia de ocho secuencias acompañantes y nueve eventos transgénicos en los 215 genotipos. Los resultados mostraron la introgresión de secuencias acompañantes y eventos transgénicos (cry1Ab) en el genoma de algunas variedades locales de maíz. La mayor proporción de residuos de cry1Ab observados en las variedades locales de maíz fue la muestra de Puebla (70%), seguida por las muestras de Oaxaca donde se observó 52%, mientras que 15 y 7.5% de los maíces nativos del estado de Jalisco presentaron el gen cry1Ab y la secuencia acompañante ntpII, respectivamente. Las muestras de Michoacán mostraron 44 y 17.7% para el gen cry1Ab y la secuencia acompañante, respectivamente. Estos resultados deben respaldar la aplicación de medidas correctivas para prevenir la contaminación genética de variedades locales de maíz y limpiar fenotipos fuera de tipo que podrían ser fuentes de contaminación de genes transgénicos dentro de poblaciones de variedades locales de maíz contaminadas con estos transgenes.

Abstract Representative seed samples of 215 of maize varieties from four (Jalisco, Michoacán, Oaxaca and Puebla) Mexican States were collected. The total genotypes collected in each State were 53, 96, 46 and 20 samples, respectively. Each seed collection was geo-referenced, then seed was planted and leaf tissue was used for DNA isolation, after that a PCR analysis was performed to determine the presence or absence of eight accompanying sequences and nine transgenic events in the 215 maize genotypes. Results showed introgression of accompanying sequences and transgenic events (cry1Ab) into some maize varieties. The major proportion of cry1Ab residues was observed in maize samples from Puebla (70%) followed by that displayed in the Oaxaca samples (52%), while Jalisco samples showed 15 and 7.5% of the cry1Ab gene and ntpII accompanying sequence, respectively, and Michoacán samples exhibited 44 and 17.7% for cry1Ab gene and the accompanying sequence, respectively. These results supported for applicate corrective measures for to prevent the genetic contamination of Mexican maize varieties and to clean up out-of-type phenotypes that could be sources of contamination of transgenic genes within maize populations contaminated with transgenes.

Resumen El ácido salicílico y Azospirillum brasilense estimulan el crecimiento y la productividad de las plantas y, en algunos entornos, las respuestas fisiológicas de las plantas a la apli cación de ambos productos bioactivos parecen ser similares. Con el objetivo de estudiar su efecto fisiológico en el crecimiento y la producción de granos de plantas de maíz se diseñó un experimento de campo. Los tratamientos consistieron en embeber las semillas en una solución acuosa de SA (0,01 mM) e inocularlas con A. brasilense y un tratamiento control. La imbibición de las semillas en SA y la inoculación con A. brasilense mejoraron el crecimiento vegetativo en las primeras etapas de la ontogenia del cultivo, aumentando el crecimiento foliar, la altura de la planta, el diámetro del tallo y la acumulación de biomasa. La longitud y el peso de las espigas fueron mayores en las plantas inoculadas con A. brasilense y tratadas con SA, en ese orden. Los resultados indicaron que el SA estimuló la acumulación de biomasa hacia las hojas, la raíz y el tallo, mientras que A. brasilense tuvo más influencia en el crecimiento de las hojas, la altura de la planta, las dimensiones de la mazorca y el rendimiento del grano. Tales resultados son relevantes para el diseño de estrategias de fertilización biológica con la respectiva reducción de la carga contaminante que acompaña a los fertilizantes químicos. Ambos productos pueden formar parte de las prácticas agronómicas del cultivo de maíz por sus ventajas económicas competitivas y por ser ambientalmente sostenibles.

Abstract Salicylic acid and Azospirillum brasilense stimulate plant growth and productivity. In some environments, plant physiology similarly responds to both bioactive products. Considering this, a field experiment was conducted to study the physiological effect of Salicilic acid and A. brasilense on growth and grain yield of maize plants. The experiment involved three treatments consisting of imbibed seeds in an aqueous solution of SA (0.01 mM), inoculated seeds with A. brasilense and a control treatment. Seed imbibition in SA and inoculation with A. brasilense improved vegetative growth in the early stages of crop ontogeny, increasing leaf growth, plant height, stem diameter and biomass accumulation. Spikelet length and weight were greater in plants first inoculated with A. brasilense and then treated with SA. Results indicated that SA stimulated biomass partitioning towards leaves, root and stem, while A. brasilense mainly affected leaf growth, plant height, ear dimensions and grain yield. Such results turn crucial for biological fertilization strategies aimed at reducing pollutant loads that accompany chemical fertilizers. Both products can be part of maize management practices given competitive economic advantages and sustainability.

Resumen Antecedentes: El cáncer de células pequeñas (CPCP) representa el 13-15% del total de neoplasias primarias de pulmón. Se caracteriza por su rapidez en la tasa de crecimiento y en el desarrollo de metástasis a distancia. Objetivos: Orientar y estandarizar el tratamiento del CPCP enfermedad extensa en México basado en evidencia clínica nacional e internacional. Material y métodos: Este documento se desarrolló como una colaboración del Instituto Nacional de Cancerología y la Sociedad Mexicana de Oncología en cumplimiento con estándares internacionales. Se integró un grupo conformado por oncólogos médicos, cirujanos oncólogos, cirujanos de tórax, radio-oncólogos y metodólogos con experiencia en revisiones sistemáticas de la literatura y guías de práctica clínica. Resultados: Se consensaron, por el método Delphi y en reuniones a distancia, las recomendaciones en CPCP enfermedad extensa, producto de preguntas de trabajo. Se identificó y evaluó la evidencia científica que responde a cada una de dichas preguntas clínicas antes de incorporarla al cuerpo de la guía. Conclusión: Esta guía proporciona recomendaciones clínicas para el manejo de la enfermedad extensa del CPCP y durante el proceso de toma de decisiones de los clínicos involucrados con su manejo en nuestro país para mejorar la calidad de la atención clínica para estos pacientes.

Abstract Background: Small cell lung cancer (SCLC) represents 13-15% of all primary lung neoplasms and is characterized by its rapid growth rate and the rapid development of distant metastases. Objectives: To guide and standardize the treatment of extensive disease small cell lung cancer in Mexico based on national and international clinical evidence. Material and methods: This document was developed as a collaboration between the National Cancer Institute and the Mexican Society of Oncology in compliance with international standards. An interdisciplinary group was formed, including medical oncologists, oncological surgeons, thoracic surgeons, radiation oncologists, and methodologists with experience in systematic reviews of the literature and clinical practice guidelines. Results: A consensus was reached, both by the Delphi method and in remote meetings, of extensive disease recommendations resulting from work questions. The scientific evidence that answers each of these clinical questions was identified and critically evaluated, before being incorporated into the body of evidence of the Guide. Conclusions: This Clinical Practice Guide provides clinical recommendations for the management of extensive disease of SCLC to contribute to the decision-making process of the clinicians involved with its management in our country, hoping that this will contribute to improving the quality of clinical care in these patients.

Resumen Con el fin de investigar el efecto de cuatro biosólidos de las industrias láctea, maltera, jabonera y papelera sobre el crecimiento y producción de biomasa de plántulas de Vicia faba L., se realizaron mezclas de biosólido:suelo en proporciones 20:80, 40:60 y 60:40. Se realizaron análisis fisicoquímicos y microbiológicos a cada biosólido y las mezclas. Los biosólidos mostraron diferencias en la presencia (NMP) de coliformes fecales de 3 a 1 100 Salmonella sp., de 2 a 3 y de 0 a 16 huevos de helmintos por g de sólidos totales. La concentración (mg kg-1) de metales pesados en cobre fue de 0.7 a 1.9, cromo de 0.8 a 2.4, cadmio de 0 a 0.6, zinc de 4.3 a 8.6, níquel 1.2 a 3.8 y plomo 1.3 a 5.7, sin arsénico y mercurio. Los contaminantes fueron inferiores a los límites permisibles según la SEMARNAT (2002), además, entre los biosólidos el pH varió de 7 a 9.8 y la materia orgánica (MO) de 0.3 a 6.2%. Los biosólidos de la industria láctea y malta incorporados al suelo modificaron el pH de 8.1 a 7.4, incrementaron la MO de 1.2 a 3.5%, el nitrógeno total de 7 a 35 mg kg-1 y el fósforo disponible de 5 a 25 mg kg-1. A 30 días después de la siembra en invernadero, las plántulas de V. faba que crecieron en suelo con el biosólido de la industria láctea en la mayor proporción incrementaron la producción de biomasa en 1 145% y la longitud en 342%.

Abstract In order to investigate the effect of four biosolids from the dairy, malt, soap and paper industries on the growth and production of biomass of Vicia faba L. seedlings, biosolid:soil mixtures were made in ratios 20:80, 40:60 and 60:40. Physicochemical and microbiological analyses were performed on each biosolid and mixtures. Biosolids showed differences in the presence (MPN) of fecal coliforms from 3 to 1 100, Salmonella sp., from 2 to 3 and from 0 to 16 helminth eggs per g of total solids. The concentration (mg kg-1) of heavy metals in copper was from 0.7 to 1.9, chromium from 0.8 to 2.4, cadmium from 0 to 0.6, zinc from 4.3 to 8.6, nickel from 1.2 to 3.8 and lead from 1.3 to 5.7, without arsenic and mercury. The contaminants were below the permissible limits according to SEMARNAT (2002), in addition, among biosolids the pH varied from 7 to 9.8 and organic material (OM) from 0.3 to 6.2%. Biosolids from the dairy and malt industries incorporated into the soil modified the pH from 8.1 to 7.4, increased OM from 1.2 to 3.5%, the total nitrogen from 7 to 35 mg kg-1 and the available phosphorus from 5 to 25 mg kg-1. At 30 days after sowing in the greenhouse, V. faba seedlings grown in soil with the biosolid from the dairy industry in the highest ratio increased biomass production by 1 145% and length by 342%.

Abstract: One problem of fuzzy systems for classification tasks is the exponential growth of rule generation, which translates into excessively long processing times. Different proposals in the literature address this problem through exhaustive rule reduction techniques, which achieve competitive results compared to conventional classifiers by reducing their computational complexity as well. This paper proposes a methodology that is comprised of two stages: (1) a clustering technique that helps identify the structure of an initial low-accurate classifier and (2) a differential evolution parameter identification stage that takes that low-accurate classifier and refines it to obtain a high-accurate classifier. In this way, a comparative analysis among several clustering methods is performed, allowing the users to select a reduced rule set and avoid the use of traditional rule search algorithms. The results show that the Gaussian Mixture Model is the most suitable clustering technique to identify the structure of fuzzy classifiers, since it provides the corresponding fuzzy model with highly competitive classification performance compared to other state-of-the-art methods.

Abstract Objective: To evaluate hypothalamic-pi- tuitary-gonadal (HPG) axis alterations at 1 and 12 months after kidney transplan- tation (KT) and their association with in- sulin resistance. Methods: A retrospective clinical study was conducted in a tertiary care center in kidney transplantation recipients (KTRs) aged 18- 50 years with primary kidney disease and stable renal graft function. LH, FSH, E2/T, and HOMA-IR were assessed at 1 and 12 months after KT. Results: Twenty-five KTRs were included; 53% were men, and the mean age was 30.6±7.7 years. BMI was 22.3 (20.4-24.6) kg/m2, and 36% had hypogonadism at 1 month vs 8% at 12 months (p=0.001). Re- mission of hypogonadism was observed in all men, while in women, hypogonadotropic hypogonadism persisted in two KTRs at 12 months. A positive correlation between go- nadotrophins and age at 1 and 12 months was evident. Fifty-six percent of patients had insulin resistance (IR) at 1 month and 36% at 12 months (p=0.256). HOMA-IR showed a negative correlation with E2 (r=- 0.60; p=0.050) and T (r=-0.709; p=0.049) at 1 month, with no correlation at 12 months. HOMA-IR at 12 months after KT correlated positively with BMI (r=0.52; p=0.011) and tacrolimus dose (r=0.53; p=0.016). Conclusion: Successful KT restores the HPG axis in the first year. Hypogonadism had a negative correlation with IR in the early pe- riod after KT, but it was not significant at 12 months.

Resumo Objetivo: Avaliar as alterações do eixo hipotálamo-hipófise-gonadal (HHG) em 1 e 12 meses após transplante renal (TR) e sua associação com a resistência à insulina. Métodos: Foi realizado um estudo clínico retrospectivo em um centro de cuidados terciários em receptores de transplante renal (RTR) com idade entre 18-50 anos com doença renal primária e função do enxerto renal estável. LH, FSH, E2/T e HOMA-IR foram avaliados em 1 e 12 meses após o TR. Resultados: foram incluídos 25 RTR; 53% eram homens e a média de idade foi de 30,6±7,7 anos. O IMC foi de 22,3 (20,4-24,6) kg/m2 e 36% apresentaram hipogonadismo em 1 mês vs 8% aos 12 meses (p=0,001). A remissão do hipogonadismo foi observada em todos os homens, enquanto nas mulheres, o hipogonadismo hipogonadotrófico persistiu em dois RTR aos 12 meses. Ficou evidente uma correlação positiva entre gonadotrofinas e idade em 1 e 12 meses. Cinquenta e seis por cento dos pacientes apresentaram resistência à insulina (RI) em 1 mês e 36% aos 12 meses (p=0,256). O HOMA-IR mostrou uma correlação negativa com E2 (r=-0,60; p=0,050) e T (r=-0,709; p=0,049) em 1 mês, sem correlação em 12 meses. O HOMA-IR aos 12 meses após TR correlacionou-se positivamente com o IMC (r=0,52; p=0,011) e a dose de tacrolimus (r=0,53; p=0,016). Conclusão: O TR bem-sucedido restaura o eixo HHG no primeiro ano. O hipogonadismo apresentou uma correlação negativa com a RI no período inicial após o TR, mas essa correlação não foi significativa aos 12 meses.
