Results of endoscopic biliary drainage in patients with malignant hilar stricture

Abstract In Malignant Hilar Biliary Stricture (MHBS) palliative biliary drainage is a frequent strategy, improving the quality of life, reducing pruritus, loss of appetite and relieving cholangitis. The endoscopic approach is an effective, although challenging procedure. This study aimed to evaluate technical and clinical success rates of biliary drainage by ERCP. This is a retrospective study including all patients with MHBS referred to Instituto do Cancer do Hospital de São Paulo (ICESP) submitted to biliary drainage by ERCP, between January 2010 and December 2017. Multivariable logistic regression was performed to evaluate predictors of clinical failure, as total bilirubin levels, Bismuth classification, number of hepatic sectors drained and presence of cholangitis. In total, 82 patients presenting unresectable MHBS were included in this study. 58.5% female and 41.5% male, with a mean age of 60±13 years. Bismuth classification grades II, IIIA, IIIB and IV were noted in 23.2%, 15.9%, 14.6% and 46.3%, respectively. Technical and clinical success was achieved in 92.7% and 53.7% respectively. At multivariable logistic-regression analyses, Bismuth IV strictures were related to higher clinical failure rates when compared to other strictures levels, with an Odds Ratio of 5.8 (95% CI 1.28‒20.88). In conclusion, endoscopic biliary drainage for malignant hilar biliary stricture had a high technical success but suboptimal clinical success rate. Proximal strictures (Bismuth IV) were associated with poor drainage outcomes.

Abstract The Santos Basin (SB) is the main petroliferous basin in the Brazilian continental margin and one of the most studied marine areas in Brazil. However, historical data suggest that new efforts should be carried out to acquire quantitative biological data, especially in the deep sea, to establish the baseline of essential ocean variables in different ecosystems for future monitoring programs. The Brazilian energy company Petrobras planned and executed 24 oceanographic cruises over a period of 2 years to assess the benthic (SANSED cruise) and pelagic (SANAGU cruise) systems of the SB (356 days at sea in 2019 and 2021/2022). These efforts were part of the Santos Project, which comprised a comprehensive environmental study aimed at investigating benthic and pelagic variables to characterize ecology, biogeochemistry, thermohaline properties of water masses, and ocean circulation patterns, geomorphology, and sedimentology, as well as organic and inorganic chemistry. Here we present the detailed sampling designs and the field methods employed on board, during the SB scientific cruises. All sampling protocols were based on standardized approaches. For the benthos analyses, triplicate sediment samples were performed using a GOMEX-type box corer (0.25 m²) or a large modified Van Veen grab (0.75 m²) at 100 stations ranging from 25 to 2400 m depth. At each station, 25 geochemical and physico-chemical parameters were analyzed in addition to micro-, meio-, and macrofauna and living foraminifera samples. For the pelagic system, 60 stations were selected to investigate the plankton community, ranging in size from pico- to macroplankton, through vertical, horizontal, and oblique net hauls (20, 200, and 500 μm mesh size), as well as 25 biogeochemical parameters collected with an aid of a CTD-rosette sampler. Part of this scientific information also serves the Regional Environmental Characterization Project (PCR-BS) in support of Petrobras’ Santos Basin drilling licensing process led by the Brazilian Environmental Agency – IBAMA. This project contributes to the sustainable development of the SB, in line with the guidelines of the United Nations Decade of Ocean Science for Sustainable Development.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Abstract Coronavirus disease 2019 (COVID-19), caused by the Severe Acute Respiratory Syndrome Coronavirus type 2 (SARS-CoV-2), is the largest pandemic in modern history with very high infection rates and considerable mortality. The disease, which emerged in China’s Wuhan province, had its first reported case on December 29, 2019, and spread rapidly worldwide. On March 11, 2020, the World Health Organization (WHO) declared the COVID-19 outbreak a pandemic and global health emergency. Since the outbreak, efforts to develop COVID-19 vaccines, engineer new drugs, and evaluate existing ones for drug repurposing have been intensively undertaken to find ways to control this pandemic. COVID-19 therapeutic strategies aim to impair molecular pathways involved in the virus entrance and replication or interfere in the patients’ overreaction and immunopathology. Moreover, nanotechnology could be an approach to boost the activity of new drugs. Several COVID-19 vaccine candidates have received emergency-use or full authorization in one or more countries, and others are being developed and tested. This review assesses the different strategies currently proposed to control COVID-19 and the issues or limitations imposed on some approaches by the human and viral genetic variability.

The continuous search for biologically active compound candidates pushes the pursuit of new substances produced by diverse organisms. Endophytic fungi are known as a promising source of metabolites with several biological activities. Based on the rich Amazonian biodiversity and the chemical potential of microbial sources, three Talaromyces strains, all major endophytes from their respective host plants, were studied aiming at the isolation of biologically active secondary metabolites. Through classical chromatographic approaches, 13 compounds were isolated from the antimicrobial extracts of the studied strains. From these, polyketides, steroids, anhydrides, and phenolic compounds were identified. Among them, two previously undescribed compounds were elucidated based on spectroscopic and spectrometric methods, a homodimer chromanone and a maleic anhydride methyl ester. The antimicrobial activity against a panel of pathogenic microorganisms from extracts, fractions, and isolated compounds was evaluated.

Application of a Quantitative HPLC-ESI-MS/MS Method for Flavonoids in Different Vegetables Matrices

Plant flavonoids have been increasingly studied for its importance in plant metabolism, inter-species interactions and human health-promoting effects. Each botanical species has a distinct flavonoid profile, and as such, this work aimed to develop and apply a high-performance liquid chromatography coupled to electrospray ionization tandem mass spectrometry (HPLC-ESI-MS/MS) method to quantify specific flavonoids in different plants and parts. Six flavonoids (isoquercitrin, quercetin, naringin, naringenin, hesperidin and rutin), two isoflavonoids (daidzin and daidzein) and one coumarin (umbelliferone) were targeted. Calibration curves showed good correlations (R2 > 0.994) and limits of quantification (≤ 500 ng mL-1). The method was applied for detection of analytes in common bean seeds (Phaseolus vulgaris), soybean leaves (Glycine max), and sour orange (Citrus aurantium), sweet orange (Citrus sinensis), and Tahiti lime (Citrus latifolia) flavedo after ultrasound assisted acidic hydroalcoholic extraction. The method was tuned for quick and efficient extraction. Principal component analysis was helpful for the discrimination of matrices based on their flavonoid profiles.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Lung transplantation is a well-established treatment for advanced lung diseases. In children, the diseases that most commonly lead to the need for a transplantation are cystic fibrosis, pulmonary hypertension, and bronchiolitis. However, the number of pediatric lung transplantations being performed is low compared with the number of transplants performed in the adult age group. The objective of this study was to demonstrate our experience with pediatric lung transplants over a 10-year period in a program initially designed for adults.

As anemias hereditárias, que englobam as hemoglobinopatias e talassemias são doenças determinadas geneticamente. Na maioria dos casos os heterozigotos são assintomáticos e desconhecem o defeito genético do qual são portadores. De uma forma geral existe uma deficiência no diagnóstico clínico e laboratorial para a investigação destas doenças que acometem uma parcela significativa da população brasileira. A comunicação deste caso à comunidade científica tem como objetivo destacar a importância de um diagnóstico clínico-laboratorial realizado o mais precocemente. Entretanto, este diagnóstico só é possível se for realizado por um profissional capacitado através de metodologias específicas para a elucidação das mais diferentes interações genéticas. Faz parte também deste diagnóstico o estudo familiar que é estritamente necessário no caso de um possível aconselhamento genético com o intuito de evitar a transmissão genética e conseqüentemente a geração de indivíduos doentes ou com uma sobrevida pequena.
Hereditary anaemias, which include hemoglobinopathies and thalassaemias, are genetically determined diseases. In most cases patients are asymptomatic and do not know about their genetic defect. In general, there is a deficit in the clinical and laboratorial investigations of these diseases that affect a significant number of Brazilians. The reporting of this case aims at highlighting the importance of precocious clinical-laboratorial diagnosis. This diagnosis is only possible if it is performed, by a qualified professional, using specific methodologies to elucidate the varying genetic interactions. A study of the affected families is an essential part of the diagnoses; many cases require genetic counseling to avoid transmission of these anomalies resulting in sick individuals who occasionally have short life expectancies.

Dois novos metabólitos, 2,4-diidroxi-5,6-dimetil benzoato de etila (1) e phomopsilactona (2) foram isolados de Phomopsis cassiae, um fungo endofítico de Cassia spectabilis. As estruturas destes compostos foram elucidadas por dados espectrométricos de 1D e 2D RMN, EM e IV. As substâncias 1 e 2 exibiram forte atividade antifúngica contra os fungos fitopatogênicos Cladosporium cladosporioides e C. sphaerospermum, bem como citotoxidade contra a linhagem celular de tumor cervical humano (HeLa), em experimentos in vitro.
Two new metabolites, ethyl 2,4-dihydroxy-5,6-dimethylbenzoate (1) and phomopsilactone (2) were isolated from Phomopsis cassiae, an endophytic fungus in Cassia spectabilis. Their structures were elucidated by 1D and 2D NMR, MS and IR spectral data. Compounds 1 and 2 displayed strong antifungal activity against the phytopatogenic fungi Cladosporium cladosporioides and C. sphaerospermum, as well as cytotoxicity against human cervical tumor cell line (HeLa), in in vitro assays.

Avaliou-se a qualidade comercial de híbridos de melão amarelo cv. Gold Mine e Gold Pride, submetidos a diferentes épocas de colheita. Os frutos foram colhidos no pólo agrícola de MossoróAssu (RN), e armazenados a 11±1ºC e 90±5% UR, durante 28 dias. Utilizou-se o delineamento inteiramente casualizado em esquema fatorial 5 x 5, com cinco repetições e um fruto por parcela. O primeiro fator constou das épocas de colheita (0 sem retardamento, 2; 4; 6 e 8 dias de retardamento da colheita) e o segundo, do tempo de armazenamento (0; 7; 14; 21 e 28 dias). Avaliou-se a firmeza da polpa (N), perda de massa (%), aparência externa e interna e conteúdo de sólidos solúveis totais (%). A firmeza da polpa e a aparência interna foram fortemente influenciadas pelo retardamento da colheita, proporcionando frutos com colapso interno e menor resistência da polpa. O retardamento da colheita influenciou significativamente a perda de massa apenas para o híbrido Gold Mine, observando-se a partir do 21º dia, maiores perdas de massa para os frutos colhidos com 4 e 6 dias de atraso na colheita. O retardamento no período de colheita induziu maior suscetibilidade a danos por frio em ambos os híbridos e aumento no teor de sólidos solúveis apenas para híbrido Gold Pride.
The commercial quality of yellow melon 'Gold Mine' and 'Gold Pride' hybrids at different harvest times was examined. The fruits were harvested in the MossoróAssu agricultural pole, Rio Grande do Norte State, Brazil, being kept under 11±1ºC and 90±5% relative humidity, during 28 days. A 5 x 5 factorial scheme in a completely randomized design was used with five replications and one fruit per plot. The first factor was recorded at harvest time (0; 2; 4; 6 and 8 days retardation of the harvest) and the second at the storage periods (0; 7; 14; 21 and 28 days). We evaluated the flesh firmness (N), weight loss (%), internal and external appearances and soluble solids content (%). The flesh firmness and the internal appearance were strongly influenced by the harvest delay, providing fruits with internal disorder and smaller flesh resistance. The harvest delay influenced significantly the weight loss of the Gold Mine hybrid with larger weight losses occurring after 21 days of storage in treatments where the harvest was delayed 4 and 6 days. The harvest delay induced larger susceptibility to chilling injury in both hybrids and increased the soluble solids content in the hybrid Gold Pride.

Sessenta e cinco pacientes com fraturas peritrocanterianas instáveis foram tratados cirurgicamente com fixação intramedular céfalo-diafisária bloqueada (sistema minimamente invasivo). Realizou-se redução fechada em mesa de tração com utilização da radioscopia. Carga parcial assistida foi encorajada a partir do segundo dia pós-operatório. Em todos os pacientes estudados, foi obtida consolidação entre 12 e 18 semanas (média de 14.5 semanas), sendo observados seis casos (9,2%) de consolidação viciosa, quatro (6,1%) de encurtamento do membro (<1cm), três (4,6%) de migração do parafuso de compressão para o acetábulo e um caso (1,5%) de fratura intra-operatória da cortical lateral do fêmur. Cinco pacientes (7.6%) apresentaram trombose venosa profunda (TVP) distal sem repercussões clínicas. Não foi constatada infecção superficial ou profunda até a data da última revisão. Nos três casos de migração proximal do parafuso de compressão, o sistema foi retirado sem intercorrências. Concluímos ser a metodologia utilizada eficaz, segura, de baixa morbidade, técnica rápida, com poucas complicações e que propicia a reabilitação precoce do paciente.
Sixty five patients with unstable peritrochanteric fractures were surgically treated with blocked intramedullary cephalodiaphyseal fixation (minimally invasive system). Closed reduction in traction table was performed with radioscopy. Assisted partial load was encouraged after the second postoperative day. In all examined patients, healing was achieved within 12 to 18 weeks (average 14.5 weeks). Complications were seen in six cases (9.2%) with vicious union, four cases (6.1%) with shortening of the limb (<1cm), three cases (4.6%) with compression screw migration to acetabulum, and one case (1.5%) with intraoperative fracture of the lateral femur cortical. Five patients (7.6%) presented with distal deep venous thrombosis (DVT) with no clinical repercussion. No superficial or deep infection was seen up to the last review. In the three cases of proximal migration of the compression screw, system was removed without intercurrences. We concluded that the methodology employed here is efficient, safe, with low morbidity potential. It is a fast technique, presenting little complications, promoting an early rehabilitation for the patient.

Os óleos essenciais de Toona ciliata, Cedrela odorata e C. fissilis foram analisados por CG-EM. Cedrela apresentou em maior percentagem sesquiterpenos formados a partir do precursor pirofosfato de cis- e trans-farnesila. Já Toona mostra uma tendência em produzir principalmente sesquiterpenos derivados do pirofosfato de trans-farnesila. Estes resultados confirmam que a classificação destes dois gêneros em uma mesma tribo, Cedreleae, continua problemática. As respostas em eletroantenogramas médios (EAGs) dos óleos essenciais de T. ciliata e C. odorata, em fêmeas de Hypsipyla grandella, foram significativamente maiores que aquelas obtidas em machos, sugerindo que as fêmeas utilizariam os odores destes óleos para a seleção da planta hospedeira (gêneros de Swietenioidea) e na escolha de locais para oviposição.
The essential oils of Toona ciliata, Cedrela odorata and C. fissilis have been analysed by GC-MS. Cedrela contains the main sesquiterpenes formed from the cis- and trans-farnesyl pyrophosphate. In contrast, Toona tend to produce mainly sesquiterpenes formed from the trans-precursor. These results show that the affiliation of Toona in the tribe Cedreleae together with Cedrela is still rather problematic. Mean electroantennogram responses (EAGs) to the essential oils from T. ciliata and C. odorata, in Hypsipyla grandella females were significantly greater than those obtained for males, suggesting that females would use attractant odours messages from the host-plant (genera of Swietenioideae) as search strategy behaviour for habitat location and oviposition.
