Abstract Introduction Auditory-evoked potentials are influenced by several factors, including polarity, filter, stimulus intensity and stimulation rate. The presentation of higher rates of stimuli per second enables the collection of a greater number of responses in a given period of time, promoting a shorter testing time; however, the collected recordings are subject to changes related to wave morphology. Objectives To compare the brainstem auditory-evoked-potential responses with click stimulus with the most commonly used stimulation rates in the clinical practice. Methods The present cross-sectional analytical study was performed with fifteen participants of both genders and normal hearing thresholds. The brainstem auditoryevoked potential was performed at four different stimulation rates (21.1, 26.7, and 27.7 stimuli/s, and a rate determined based on a mathematical calculation using the a measurement of the transmission frequency of the power grid at the time of the examination). Results We observed that the rate of 21.1 stimuli/s showed the highest amplitudes for waves I, III, and V when compared with the other rates. The rate of 26.7 stimuli/s, when compared with 27.7 stimuli/s, showed a higher amplitude for wave V. The latency if wave V was significantly lower with the rate of 21.1 stimuli/s than with 27.7 stimuli/s. Conclusions The stimulation rate interferes with wave latencies and amplitudes; its decrease from 27.7 to 21.1 stimuli/s decreases the latency of wave V and increases the amplitues and improves the morphology of waves I, III and V. In addition, we found evidence that suggests an improvement in the visualization of wave III by adjusting the stimulation rate based on a measurement of the local transmission frequency of the power grid.

ABSTRACT Pemphigus Vulgaris (PV) is an uncommon autoimmune and blistering mucocutaneous disease. Childhood Pemphigus Vulgaris (CPV) is a pediatric variant of PV, which affects children below 12 years, being very rare among children under 10 years of age. CPV has similar clinical, histological, and immunological features as seen in PV in adults. The mucocutaneous clinical presentation is the most common in both age groups. Vesicles and erosions arising from the disease usually cause pain. A few CPV cases have been reported in the literature. This study reports a case of an 8-year-old male patient with oral lesions since the age of 3 years, and the diagnosis of pemphigus was achieved only 2 years after the appearance of the initial lesions. CPV remains a rare disease, making the diagnosis of this clinical case a challenge due to its age of onset and clinical features presented by the patient. Therefore, dentists and physicians should know how to differentiate CPV from other bullous autoimmune diseases more common in childhood.

RESUMO Objetivo O aparelho de amplificação sonora individual propicia benefícios para indivíduos com deficiência auditiva. No entanto, ainda são observados baixos níveis de adesão ao tratamento, devido às dificuldades no processo de adaptação. Neste sentido, destaca-se o uso de sites ou aplicativos com os objetivos de auxiliar no processo de orientação aos usuários e de avaliar a satisfação de usuários de aparelho de amplificação sonora individual, após a utilização de um site responsivo desenvolvido para auxiliar no processo de adaptação. Métodos Estudo quantitativo analítico experimental, ensaio clínico não randomizado, com 20 participantes, divididos em dois grupos. O grupo estudo fez uso do site responsivo como ferramenta de auxílio na adaptação, além de receber as orientações em seu formato tradicional. O grupo controle foi orientado somente pelo formato tradicional já existente no serviço. Após um mês, os participantes responderam a um questionário de avaliação de satisfação e também foi realizada a análise da ferramenta datalogging. Resultados O site responsivo contou com orientações sobre manutenção e dicas de uso do aparelho de amplificação sonora individual, permitindo fácil acesso às principais orientações ao usuário inexperiente. A análise dos dados demonstrou respostas mais positivas ao questionário pelo grupo estudo, bem como maior tempo de uso diário dos aparelhos de amplificação sonora individuais. Conclusão o grupo que utilizou o site responsivo como ferramenta complementar ao processo de adaptação do aparelho de amplificação sonora individual mostrou, a partir da avaliação da satisfação e dos dados da ferramenta datalogging, maior satisfação e maior tempo de uso diário do aparelho.

ABSTRACT Purpose The hearing aid provides benefits for individuals with hearing impairment. However, low levels of treatment adherence are still observed due to difficulties in the adaptation process. In this sense, the use of websites or applications stands out in order to assist in the process of orienting users. To evaluate the satisfaction of users of hearing aids after using a responsive website designed to assist in the adaptation process. Methods Quantitative analytical experimental study non-randomized clinical trial whit 20 participants, divided into two groups. The study group made use of the responsive website as an aid tool in adapting, in addition to also receiving the guidelines in their traditional format. The control group was guided only with the traditional format already existing in the service. After one month, the participants answered a satisfaction assessment questionnaire, as well as an analysis of the datalogging. Results The responsive website had guidance on maintenance and tips on using the individual hearing aid, allowing easy access to the main guidelines for inexperienced users. The analysis of the data showed more positive responses to the questionnaire by the study group, as well as a longer time of daily use of hearing aids. Conclusion The group that used the responsive website as a complementary tool to the adaptation process of the hearing aid showed, from the assessment of satisfaction and data from the datalogging tool, greater satisfaction and a longer time of daily use of the hearing aid.

Resumo Introdução: A perda auditiva é conceituada como qualquer diminuição da capacidade de ouvir e/ou detectar sons da fala ou do ambiente, independentemente da causa, tipo ou grau e pode ocorrer em diversos estágios da vida, durante a gestação ou parto, na infância, vida adulta ou na terceira idade. Convém destacar que o envelhecimento é a primeira causa de perda auditiva do tipo sensorioneural e em segundo lugar a perda auditiva induzida pelo ruído, ambas estão intimamente relacionadas com a formação de espécies reativas de oxigênio. Evidências têm sido acumuladas indicando que a suplementação com antioxidantes via alimentação tornou-se estratégia terapêutica para prevenir e/ou retardar os riscos das principais doenças humanas. Objetivo: Avaliar ensaios clínicos aleatórios para determinar qual o efeito da suplementação com antioxidantes sobre o limiar auditivo na perda auditiva sensorioneural em pacientes de diversas faixas etárias. Método: A formulação desta revisão sistemática consistiu na busca dos estudos nas seguintes bases de dados: MEDLINE, CENTRAL, ScienceDirect, Scopus, Web of Science, LILACS, SciELO e ClinicalTrials.gov. Adicionalmente, a literatura cinzenta também foi pesquisada. A estratégia de busca incluiu termos relacionados à intervenção (suplementação de antioxidantes), o desfecho primário (perda auditiva sensorioneural), bem como termos relacionados aos ensaios clínicos randomizados para melhorar e a sensibilidade da busca. Resultados: A partir de 977 registros potencialmente relevantes identificados através da busca nas bases de dados, dez publicações em texto completo foram recuperadas para avaliação mais aprofundada. O aumento no limiar na frequência de 4 kHz foi estatisticamente maior no Grupo Controle (1,89 [1,01-2,78], p < 0,0001) quando comparados com o Grupo NAC e o Grupo Ginseng, já na frequência de 6 kHz o aumento no limiar foi maior no Grupo Controle (1,42 [-1,14-3,97], p = 0,28), porém, não foram encontradas diferenças estatisticamente significativas entre grupos. Conclusão: O Ginseng foi o antioxidante que evitou a piora do limiar auditivo na frequência de 4 kHz, mas não em 6 kHz, em pacientes com perda auditiva sensorioneural causada por exposição a elevados níveis de pressão sonora. Não foi observada melhora nos limiares com a suplementação com Vitamina E.

Abstract Introduction: Hearing loss is conceptualized as any impairment of the ability to hear and/or detect speech or environment sounds, regardless of cause, type, or degree. It may occur at different stages of life; during pregnancy or childbirth, in childhood, adulthood or old age. It should be noted that aging is the most common cause of sensorineural hearing loss followed by noise-induced hearing loss, and both are closely related to the formation of reactive oxygen species. Dietary antioxidant supplementation has been employed as a therapeutic strategy to prevent and/or delay the risks of major human diseases. Objective: To assess randomized clinical trials to determine the effect of antioxidant supplementation on the auditory thresholds in patients of different age groups with sensorineural hearing loss. Methods: This systematic review consisted of a search in the following databases: MEDLINE, CENTRAL, ScienceDirect, Scopus, Web of Science, LILACS, SciELO and ClinicalTrials.gov. Additionally, the gray literature was also searched. The search strategy included terms related to the intervention (antioxidant supplementation), primary outcome (sensorineural hearing loss), as well as terms related to randomized clinical trials to improve search sensitivity. Results: Based on 977 potentially relevant records identified through the search in the databases, ten full-text publications were retrieved for further evaluation. The increase in threshold at the 4 kHz frequency was statistically higher in the control group (1.89 [1.01-2.78], p < 0.0001) when compared to the NAC group and the ginseng group, whereas at 6 kHz, the threshold increase was higher in the control group (1.42 [−1.14-3.97], p = 0.28), but no statistically significant differences were found between groups. Conclusion: Ginseng was the antioxidant agent that showed the best effect in preventing auditory threshold worsening at the frequency of 4 kHz, but not at 6 kHz in patients with sensorineural hearing loss caused by exposure to high sound pressure levels. There was no improvement in the thresholds with vitamin E supplementation.

Resumo Introdução: A cafeína pode ser considerada a droga mais consumida por adultos em nível mundial, podendo ser encontrada em inúmeros alimentos, como no chocolate, café, chá, refrigerante e outros. Em geral, a cafeína em doses moderadas, produz ótimos rendimentos físico e intelectual, aumenta a capacidade de concentração e diminui o tempo de reação aos estímulos sensoriais. Por outro lado, doses elevadas podem causar sinais perceptíveis de confusão mental e indução de erros em tarefas intelectuais, ansiedade, nervosismo, tremores musculares, taquicardia, alterações labirínticas e zumbido. Objetivo: Considerando que o potencial evocado miogênico vestibular é um teste clínico que avalia a resposta muscular decorrente de estimulação auditiva de alta intensidade, a presente revisão sistemática objetivou analisar o efeito da cafeína sobre o potencial evocado miogênico vestibular. Método: A formulação deste trabalho consistiu na busca dos estudos nas seguintes bases de dados: MEDLINE, CENTRAL, ScienceDirect, Scopus, Web of Science, LILACS, SciELO e Clinical-Trials.gov. Adicionalmente, a literatura cinzenta também foi pesquisada. A estratégia de busca incluiu termos relacionados à intervenção (consumo de cafeína ou café) e ao desfecho primário (potencial evocado miogênico vestibular). Resultados: A partir de 253 registros potencialmente relevantes identificados através da busca nas bases de dados, apenas duas publicações em texto completo foram recuperadas para avaliação mais aprofundada, sendo estas mantidas para a análise qualitativa. Conclusão: Diante dos artigos encontrados a cafeína não tem efeito sobre o potencial evocado miogênico vestibular em sujeitos normais.

Abstract Introduction: Caffeine can be considered the most consumed drug by adults worldwide, and can be found in several foods, such as chocolate, coffee, tea, soda and others. Overall, caffeine in moderate doses, results in increased physical and intellectual productivity, increases the capacity of concentration and reduces the time of reaction to sensory stimuli. On the other hand, high doses can cause noticeable signs of mental confusion and error induction in intellectual tasks, anxiety, restlessness, muscle tremors, tachycardia, labyrinthine changes, and tinnitus. Objective: Considering that the vestibular evoked myogenic potential is a clinical test that evaluates the muscular response of high intensity auditory stimulation, the present systematic review aimed to analyze the effects of caffeine on vestibular evoked myogenic potential. Methods: This study consisted of the search of the following databases: MEDLINE, CENTRAL, ScienceDirect, Scopus, Web of Science, LILACS, SciELO and ClinicalTrials.gov. Additionally, the gray literature was also searched. The search strategy included terms related to intervention (caffeine or coffee consumption) and the primary outcome (vestibular evoked myogenic potential). Results: Based on the 253 potentially relevant articles identified through the database search, only two full-text publications were retrieved for further evaluation, which were maintained for qualitative analysis. Conclusion: Analyzing the articles found, caffeine has no effect on vestibular evoked myogenic potential in normal individuals.

Resumo Introdução: A associação entre perda auditiva e doença renal crônica e hemodiálise tem sido bem documentada. Porém, a classificação usada para o grau da perda pode subestimar o real diagnóstico devido a características específicas em relação às frequências auditivas mais acometidas. Além disso, correlações da perda auditiva e do tempo de hemodiálise com o handicap auditivo permanecem desconhecidas nessa população. Objetivo: Comparar os resultados das classificações de Lloyd e Kaplan e do Bureau Internacional d'Audiophonologie em pacientes com doença renal crônica e correlacionar as médias calculadas por suas fórmulas com o tempo de hemodiálise e com o handicap auditivo. Método: Estudo analítico, observacional e transversal com 80 pacientes em hemodiálise. Todos os pacientes foram submetidos a timpanometria, logoaudiometria, audiometria tonal limiar e os pacientes com perda auditiva foram entrevistados através do Hearing Handicap Inventory for Adults. A classificação dos casos foi feita de acordo com o grau da perda. Foram verificadas as correlações das médias tonais com o tempo de hemodiálise e com as pontuações totais do Hearing Handicap Inventory for Adults e seus domínios. Resultados: Em 48 pacientes que responderam ao Hearing Handicap Inventory for Adults, 86 orelhas (53,75%) apresentaram perda auditiva em pelo menos uma das médias tonais. A classificação do Bureau Internacional d'Audiophonologie identificou maior número de casos (n = 52) que apresentavam algum grau de deficiência do que a classificação de Lloyd e Kaplan (n = 16). No grupo com tempo de hemodiálise a partir de dois anos, houve correlação fraca, mas estatisticamente significante, da média da classificação do Bureau Internacional d'Audiophonologie com o tempo de hemodiálise (r = 0,363). Houve correlações moderadas das médias da classificação do Bureau Internacional d'Audiophonologie (r = 0,510) e tritonal 2 (r = 0,470) com pontuações totais do Hearing Handicap Inventory for Adults e com seu domínio social. Conclusão: A classificação do Bureau Internacional d'Audiophonologie mostra-se mais adequada do que a de Lloyd e Kaplan nessa população, sua média apresentou correlações com perdas auditivas em pacientes com tempo de hemodiálise ≥ 2 anos e manteve níveis moderados de correlação com a pontuação total do Hearing Handicap Inventory for Adults e seu domínio social (r = 0,557 e r = 0,512).

Abstract Introduction: The association between hearing loss and chronic kidney disease and hemodialysis has been well documented. However, the classification used for the degree of loss may underestimate the actual diagnosis due to specific characteristics related to the most affected auditory frequencies. Furthermore, correlations of hearing loss and hemodialysis time with hearing handicap remain unknown in this population. Objective: To compare the results of Lloyd's and Kaplan's and The Bureau Internacional d'Audiophonologie classifications in chronic kidney disease patients, and to correlate the averages calculated by their formulas with hemodialysis time and the hearing handicap. Methods: This is an analytical, observational and cross-sectional study with 80 patients on hemodialysis. Tympanometry, speech audiometry, pure tone audiometry and interview of patients with hearing loss through Hearing Handicap Inventory for Adults. Cases were classified according to the degree of loss. The correlations of tone averages with hemodialysis time and the total scores of Hearing Handicap Inventory for Adults and its domains were verified. Results: 86 ears (53.75%) had hearing loss in at least one of the tonal averages in 48 patients who responded to Hearing Handicap Inventory for Adults. The Bureau Internacional d'Audiophonologie classification identified a greater number of cases (n = 52) with some degree of disability compared to Lloyd and Kaplan (n = 16). In the group with hemodialysis time of at least 2 years, there was weak but statistically significant correlation of The Bureau Internacional d'Audiophonologie classification average with hemodialysis time (r = 0.363). There were moderate correlations of average The Bureau Internacional d'Audiophonologie classification (r = 0.510) and tritone 2 (r = 0.470) with the total scores of Hearing Handicap Inventory for Adults and with its social domain. Conclusion: The Bureau Internacional d'Audiophonologie classification seems to be more appropriate than Lloyd's and Kaplan's for use in this population; its average showed correlations with hearing loss in patients with hemodialysis time ≥ 2 years and it exhibited moderate levels of correlation with the total score of Hearing Handicap Inventory for Adults and its social domain (r = 0.557 and r = 0.512).

Tontura é uma das queixas mais freqüentes no consultório médico tanto primário quanto especializado. Muitos dos pacientes que se apresentam com tontura sem causa orgânica aparente, portanto considerados como portadores de tontura idiopática, podem ter um distúrbio psiquiátrico. Além disso, mesmo a tontura de causa orgânica pode desencadear ou exacerbar alterações psiquiátricas "latentes". Um dos distúrbios mais comumente associados à tontura é o Distúrbio do Pânico, com ou sem Agorafobia. O objetivo deste estudo é relatar o caso de uma paciente com essa associação e realizar uma revisão da literatura relacionada ao assunto.

Dizziness is one of the most frequent complaints in both primary and specialized medical care facilities. Many dizzy patients, without a known organic cause, considered as having idiopathic dizziness, may have a psychiatric disorder. Besides, even organic dizziness may cause or exacerbate latent psychiatric alterations. One of the most common disorders associated with dizziness is Panic Disorder with or without Agoraphobia. The aim of this paper is to report a patient’s case and make a literature review on the subject.
