ABSTRACT: This study evaluated the effect of dietary supplementation with glutamine and glutamic acid (Gln+Glu) on performance, intestinal morphometry, and carcass characteristics of broiler quails. Eight hundred birds were used, distributed in an entirely randomized design with 20 birds per experimental unit, and given five treatments (0.0; 0.2; 0.4; 0.6; and 0.8% Gln+Glu supplementation) with eight replicates. At 1-21 days of age, lower (P < 0.05) feed intake at 0.6 and 0.8% Gln+Glu supplementation and lower weight gain at 0.8% Gln+Glu supplementation compared to the control treatment were observed. By regression analysis, excluding the control treatment, there was an increasing linear effect (P < 0.05) for feed intake at 22 to 42 days of age. For intestinal morphometry, Gln+Glu supplementation only favored the villus development of the ileum (P < 0.05), giving it greater height at 0.2, 0.6, and 0.8% supplementation. Carcass characteristics, cuts, and edible viscera of the birds at 42 days were not affected (P > 0.05) by Gln+Glu supplementation levels. Thus, the glutamine and glutamic acid supplementation affected the performance and intestinal morphology of 21-d-old quails, decreasing feed intake and weight gain associated with the improvement of ileum morphology; conversely, performance and carcass characteristics at 42 days were not affected by amino acid supplementation.

RESUMO: Objetivou-se avaliar o efeito da suplementação dietética de glutamina e ácido glutâmico (Gln+Glu) sobre o desempenho, a morfometria intestinal e as características de carcaça de codornas de corte. Foram utilizadas 800 aves, distribuídas em delineamento inteiramente casualizado, com cinco tratamentos (0,0; 0,2; 0,4; 0,6 e 0,8% de suplementação de Gln+Glu) e oito repetições com 20 aves por unidade experimental. Na fase de um a 21 dias, constatou-se menor (P < 0,05) consumo de ração aos níveis de 0,6 e 0,8% de Gln+Glu e menor ganho de peso ao nível 0,8% de Gln+Glu em comparação ao tratamento controle; e, pela análise de regressão, excluindo-se o tratamento controle, houve efeito linear crescente (P < 0,05) para consumo de ração na fase 22 a 42 dias de idade. Para morfometria intestinal, a suplementação de Gln+Glu apenas favoreceu o desenvolvimento vilos do íleo (P < 0,05), conferindo-lhe maior altura aos níveis de 0,2; 0,6 e 0,8% de suplementação. As características de carcaça, cortes e vísceras comestíveis das aves aos 42 dias não foram afetadas (P > 0,05) pelos níveis de suplementação de Gln+Glu. Assim, a suplementação de glutamina e ácido glutâmico influenciou o desempenho e a morfometria intestinal de codornas de corte aos 21 dias de idade, promovendo redução do consumo de ração e do ganho, associado ao aumento morfométrico do íleo; por outro lado, o desempenho das aves e as suas características de carcaça aos 42 dias não foram afetados pela suplementação dos aminoácidos.

Resumo Objetivo: descrever o processo de implantação subnacional da certificação da eliminação da transmissão vertical de HIV e/ou sífilis, suas principais barreiras, desafios e oportunidades. Métodos: em 2022, foram avaliados indicadores do último ano completo para meta de impacto, e dos dois últimos anos completos para metas de processo, disponíveis nos sistemas nacionais de informações; foram analisados relatórios descritivos e reconhecidas ações em quatro eixos temáticos, conforme recomendações da OPAS/OMS. Resultados: 43 municípios ≥ 100 mil habitantes foram certificados, abrangendo 24,6 milhões de habitantes; um município alcançou dupla eliminação (HIV-sífilis), 28 alcançaram eliminação para HIV e 10, selos prata; para sífilis, houve uma eliminação, 4 selos ouro, 13 prata e 4 bronze; identificou-se maior número de certificações nas regiões Sudeste e Sul. Conclusão: barreiras e desafios do processo foram superados pela colaboração tripartite; a experiência proporcionou melhor integração da vigilância com a assistência e qualificação das ações para prevenção da transmissão vertical.

Resumen Objetivo: describir el proceso de implementación subnacional de la certificación de eliminación de la transmisión vertical (TV) de sífilis y/o VIH, barreras, oportunidades y desafíos. Métodos: en 2022, se evaluaron indicadores del último año completo para la meta de impacto y de los dos últimos años para las de proceso en los sistemas de información; se analizaron informes descriptivos y se reconocieron acciones de cuatro ejes, según las recomendaciones de la OPS/OMS. Resultados: se certificaron 43 municipios ≥ 100.000 mil habitantes, cubriendo 24,6 millones de habitantes; un municipio logró la doble eliminación (VIH-sífilis), 28 la eliminación del VIH y 10 sellos plata; para sífilis, hubo una eliminación, 4 sellos oro, 13 plata y 4 bronce; las regiones Sudeste y Sur obtuvieron más certificaciones. Conclusión: barreras y desafíos fueron superados mediante la colaboración tripartita; la experiencia permitió la integración de la vigilancia con la atención y la cualificación de acciones para la prevención de la TV.

Abstract Objective: to describe the subnational implementation process of the certification for elimination of mother-to-child transmission of HIV and/or syphilis, its main barriers, challenges and opportunities. Methods: in 2022, indicators from the last full year for impact targets and the last two full years for process targets, available in national information systems, were evaluated; descriptive reports were analyzed and actions were acknowledged within four thematic axes, according to PAHO/WHO recommendations. Results: 43 municipalities ≥ 100,000 inhabitants were certified, covering 24.6 million inhabitants; one municipality achieved dual elimination (HIV-syphilis), 28 municipalities achieved elimination of HIV and 10 received silver tiers; regarding syphilis, one elimination was observed, along with 4 gold tiers, 13 silver tiers and 4 bronze tiers; a higher number of certifications was identified in the Southeast and South regions. Conclusion: barriers and challenges of the process were overcome through tripartite collaboration; the experience provided better integration of surveillance with care and improved actions aimed at preventing mother-to-child transmission.

Abstract Objective To evaluate clinical characteristics and outcomes of COVID-19 patients infected with HIV, and to compare with a paired sample without HIV infection. Methods This is a substudy of a Brazilian multicentric cohort that comprised two periods (2020 and 2021). Data was obtained through the retrospective review of medical records. Primary outcomes were admission to the intensive care unit, invasive mechanical ventilation, and death. Patients with HIV and controls were matched for age, sex, number of comorbidities, and hospital of origin using the technique of propensity score matching (up to 4:1). They were compared using the Chi-Square or Fisher's Exact tests for categorical variables and the Wilcoxon for numerical variables. Results Throughout the study, 17,101 COVID-19 patients were hospitalized, and 130 (0.76%) of those were infected with HIV. The median age was 54 (IQR: 43.0;64.0) years in 2020 and 53 (IQR: 46.0;63.5) years in 2021, with a predominance of females in both periods. People Living with HIV (PLHIV) and their controls showed similar prevalence for admission to the ICU and invasive mechanical ventilation requirement in the two periods, with no significant differences. In 2020, in-hospital mortality was higher in the PLHIV compared to the controls (27.9% vs. 17.7%; p = 0.049), but there was no difference in mortality between groups in 2021 (25.0% vs. 25.1%; p > 0.999). Conclusions Our results reiterate that PLHIV were at higher risk of COVID-19 mortality in the early stages of the pandemic, however, this finding did not sustain in 2021, when the mortality rate is similar to the control group.

BACKGROUND There is interest in lingering non-specific symptoms after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, referred to as Long coronavirus disease 2019 (Long COVID-19). It remains unknown whether the risk of Long COVID-19 is associated with pre-existing comorbidities or initial COVID-19 severity, including infections due to new Omicron lineages which predominated in 2023. OBJECTIVES The aim of this case report was to characterize the clinical features of acute XBB.1.5 infection followed by Long COVID-19. METHODS We followed a 73-year old female resident of Rio de Janeiro with laboratory-confirmed SARS-CoV-2 during acute infection and subsequent months. The SARS-CoV-2 lineage was determined by genome sequencing. FINDINGS The participant denied comorbidities and had completed a two-dose vaccination schedule followed by two booster doses eight months prior to SARS-CoV-2 infection. Primary infection by viral lineage XBB.1.5. was clinically mild, but the participant subsequently reported persistent fatigue. MAIN CONCLUSIONS This case demonstrates that Long COVID-19 may develop even after mild disease due to SARS-CoV-2 in fully vaccinated and boosted individuals without comorbidities. Continued monitoring of new SARS-CoV-2 lineages and associated clinical outcomes is warranted. Measures to prevent infection should continue to be implemented including development of new vaccines and antivirals effective against novel variants.

Abstract Background Despite no evidence showing benefits of hydroxychloroquine and chloroquine with or without azithromycin for COVID-19 treatment, these medications have been largely prescribed in Brazil. Objectives To assess outcomes, including in-hospital mortality, electrocardiographic abnormalities, hospital length-of-stay, admission to the intensive care unit, and need for dialysis and mechanical ventilation, in hospitalized COVID-19 patients who received chloroquine or hydroxychloroquine, and to compare outcomes between those patients and their matched controls. Methods A retrospective multicenter cohort study that included consecutive laboratory-confirmed COVID-19 patients from 37 Brazilian hospitals from March to September 2020. Propensity score was used to select matching controls by age, sex, cardiovascular comorbidities, and in-hospital use of corticosteroid. A p-value <0.05 was considered statistically significant. Results From 7,850 COVID-19 patients, 673 (8.6%) received hydroxychloroquine and 67 (0.9%) chloroquine. The median age in the study group was 60 years (46 - 71) and 59.1% were women. During hospitalization, 3.2% of patients presented side effects and 2.2% required therapy discontinuation. Electrocardiographic abnormalities were more prevalent in the chloroquine/hydroxychloroquine group (13.2% vs. 8.2%, p=0.01), and the long corrected QT interval was the main difference (3.6% vs. 0.4%, p<0.001). The median hospital length of stay was longer in the HCQ/CQ + AZT group than in controls (9.0 [5.0, 18.0] vs. 8.0 [4.0, 14.0] days). There was no statistical differences between groups in intensive care unit admission (35.1% vs. 32.0%; p=0.282), invasive mechanical ventilation support (27.0% vs. 22.3%; p=0.074) or mortality (18.9% vs. 18.0%; p=0.682). Conclusion COVID-19 patients treated with chloroquine or hydroxychloroquine had a longer hospital length of stay, when compared to matched controls. Intensive care unit admission, invasive mechanical ventilation, dialysis and in-hospital mortality were similar.

Resumo Fundamento Apesar da ausência de evidência mostrando benefícios da hidroxicloroquina e da cloroquina combinadas ou não à azitromicina no tratamento da covid-19, esses medicamentos têm sido amplamente prescritos no Brasil. Objetivos Avaliar desfechos, incluindo moralidade hospitalar, alterações eletrocardiográficas, tempo de internação, admissão na unidade de terapia intensiva, e necessidade de diálise e de ventilação mecânica em pacientes hospitalizados com covid-19 que receberam cloroquina ou hidroxicloroquina, e comparar os desfechos entre aqueles pacientes e seus controles pareados. Métodos Estudo multicêntrico retrospectivo do tipo coorte que incluiu pacientes com diagnóstico laboratorial de covid-19 de 37 hospitais no Brasil de março a setembro de 2020. Escore de propensão foi usado para selecionar controles pareados quanto a idade, sexo, comorbidades cardiovasculares, e uso de corticosteroides durante a internação. Um valor de p<0,05 foi considerado estatisticamente significativo. Resultados Dos 7850 pacientes com covid-19, 673 (8,6%) receberam hidroxicloroquina e 67 (0,9%) cloroquina. A idade mediana no grupo de estudo foi 60 (46-71) anos e 59,1% eram mulheres. Durante a internação, 3,2% dos pacientes apresentaram efeitos adversos e 2,2% necessitaram de interromper o tratamento. Alterações eletrocardiográficas foram mais prevalentes no grupo hidroxicloroquina/cloroquina (13,2% vs. 8,2%, p=0,01), e o prolongamento do intervalo QT corrigido foi a principal diferença (3,6% vs. 0,4%, p<0,001). O tempo mediano de internação hospitalar foi maior no grupo usando CQ/HCQ em relação aos controles (9,0 [5,0-18,0] vs. 8,0 [4,0-14,0] dias). Não houve diferenças estatisticamente significativas entre os grupos quanto a admissão na unidade de terapia intensiva (35,1% vs. 32,0%; p=0,282), ventilação mecânica invasiva (27,0% vs. 22,3%; p=0,074) ou mortalidade (18,9% vs. 18,0%; p=0,682). Conclusão Pacientes com covid-19 tratados com cloroquina ou hidroxicloroquina apresentaram maior tempo de internação hospitalar, em comparação aos controles. Não houve diferença em relação a admissão em unidade de terapia intensiva, necessidade de ventilação mecânica e mortalidade hospitalar.

Abstract Objective: to analyze the association between previous Chikungunya infection and depressive symptoms among healthcare workers. Methods: a cross-sectional study with a probabilistic sample of healthcare workers in primary and medium-complexity care settings in a municipality in the state of Bahia, Brazil. We used the rapid test DPP-ZDC-IgM/IgG to identify recent (IgM) or previous (IgG) Chikungunya virus infection, and the Patient Health Questionnaire-PHQ-9 to assess depressive symptoms. We applied regression with robust variance to estimate prevalence ratios and 95% confidence intervals. Results: 392 workers participated, 83.2% female. The frequency of Chikungunya virus infection was 8.9%. The prevalence of depressive symptoms was 22.7%. In the adjusted analysis, Chikungunya infection was positively associated with depressive symptoms (PR=2.00; 95%CI: 1.29; 3.07). Stratified analyses indicated a stronger association among males (PR=7.57; 95%CI: 1.15; 50.06) compared with females (PR=1.68; 95%CI: 1.03; 2.74). Discussion: the findings support the hypothesis of a positive association between Chikungunya and depressive symptoms. Physiopathological mechanisms resulting from viral action, as well as emotional, behavioral, and psychosocial factors associated with the disease, may explain the findings. We reiterate the importance of providing mental health care to healthcare workers.

Resumo Objetivo: analisar a associação entre infecção prévia por Chikungunya e sintomas depressivos em trabalhadores da saúde. Métodos: estudo transversal com amostra probabilística de trabalhadores atuantes na atenção primária e na média complexidade de um município do estado da Bahia, Brasil. Utilizou-se o teste rápido DPP-ZDC-IgM/IgG para identificar infecção recente (IgM) ou prévia (IgG) pelo vírus Chikungunya. O Patient Health Questionnaire-PHQ-9 foi usado para avaliar sintomas depressivos. Regressão de Poisson com variância robusta foi usada para estimar razões de prevalência e intervalos de confiança 95%. Resultados: participaram da pesquisa 392 trabalhadores, sendo 83,2% do sexo feminino. A frequência de infecção pelo vírus Chikungunya foi de 8,9%. A prevalência de sintomas depressivos foi de 22,7%. Na análise ajustada, a infecção por Chikungunya associou-se positivamente aos sintomas depressivos (RP=2,00; IC95%:1,29;3,07). As análises estratificadas apontaram associação de maior magnitude no sexo masculino (RP=7,57;1,15;50,06), em comparação ao feminino (RP=1,68;1,03;2,74). Conclusão: os achados corroboram a hipótese de associação positiva entre Chikungunya e sintomas depressivos. Mecanismos fisiopatológicos decorrentes de ação viral, bem como fatores emocionais, comportamentais e psicossociais associados à doença podem explicar os achados. Reitera-se a importância do cuidado em saúde mental para os trabalhadores da saúde.

Em uma era onde a prevalência de dados se faz marcante, a importância da adoção de técnicas computacionais avançadas, como a inteligência artificial e o aprendizado de máquina, torna-se fundamental no processo de formação dos físicos de amanhã. Este artigo introduz uma abordagem inovadora para o ensino de física, incorporando o aprendizado de máquina ao estudo clássico do pêndulo simples. São introduzidos os conceitos das técnicas de aprendizado de máquina e coletado um conjunto amplo de dados por meio da observação acurada do movimento do pêndulo em diferentes condições iniciais. A partir desses dados, é empregado um algoritmo de regressão para criar um modelo do sistema do pêndulo com índices estatísticos satisfatórios. In an era where the prevalence of data is striking, the importance of adopting advanced computational techniques, such as artificial intelligence and machine learning, becomes fundamental in the training process of tomorrow’s physicists. This article introduces an innovative approach to physics education, incorporating machine learning into the classic study of the simple pendulum. Key concepts of machine learning techniques are introduced and a broad dataset is collected through accurate observation of the pendulum’s motion under different initial conditions. From these data, a regression algorithm is employed to create a model of the pendulum system with satisfactory statistical indices. Abstract Introduction Supracricoid laryngectomy (SCL CHEP) removes ~ 70% of the larynx, resulting in structural rearrangement and modification of the swallowing mechanism, promoting chronic dysphagia. One of the consequences of this new physiology is the formation of pharyngeal residues that can increase the possibility of aspiration. The formation of residues after SCL CHEP, its functional consequences, and its influence on quality of life (QOL) is still poorly described in the literature. Objective To investigate and compare the association between self-reported QoL and objective assessments of swallowing function in patients undergoing SCL CHEP. Methods A cross-sectional study was performed from 2018 to 2020 in a reference service for head and neck surgery in Brazil. A total of 860 swallowing videofluoroscopy images were evaluated using the Penetration and Aspiration Scale (PAS) and Dynamic Imaging Grade of Swallowing Toxicity (DIGEST). Results In a group of 86 patients, there was a significant relationship between oncological staging and the global (p<0.001) and total (p = 0.002) QoL domains. There was a negative correlation between the DIGEST scale and the emotional domain of the QoL protocol (p = 0.045). The swallowing function proved to be relevant for QoL. Conclusion The PAS scale did not show any correlation with QoL. The functional performance of swallowing according to the DIGEST scale was coherent with the QOL scores. It is suggested that the residue may be a more relevant aspect for QoL than the aspiration, making DIGEST a promising tool in the assessment of dysphagic patients.

Abstract Introduction: Myeloproliferative neoplasms are the most common cause of splanchnic vein thrombosis in the absence of cirrhosis or nearby malignancy. Case Presentation: A 31-year-old male presented to the emergency department with epigastric pain associated with mild thrombocytosis and elevated levels of aminotransferases, lactate dehydrogenase, and C-reactive protein. Contrast-enhanced abdominal computed tomography revealed splanchnic venous thrombosis that involved the portal, splenic, and superior mesenteric veins, without signs of chronic liver disease. Anticoagulation with warfarin was immediately started. Diagnostic work-up was remarkable for the presence of the JAK2 V617T mutation and hypercellular bone marrow, with increased myeloid cells and atypical megakaryocytes, consistent with primary myelofibrosis in a prefibrotic stage. No other hypercoagulable conditions were identified. Discussion: We present a rare case of primary myelofibrosis in the prefibrotic stage presenting as portal-splenic-superior mesenteric vein thrombosis. This demonstrates that extensive splanchnic vein thrombosis may be the onset manifestation of myeloproliferative neoplasms, even in early stages and in the absence of concomitant hypercoagulable conditions. The presence of the JAK2 mutation is an important prothrombotic risk factor that can, per se, contribute to large venous thrombosis.

Resumo Introdução: As neoplasias mieloproliferativas constituem a causa mais comum de trombose venosa esplâncnica na ausência de cirrose hepatica ou neoplasia regional. Descrição do caso: Um homem de 31 anos apresentou-se no Serviço de Urgência com dor epigástrica associada a trombocitose ligeira e elevação das transamínases, desidrogenase láctica e proteína C-reactiva. Em tomografia computorizada abdominal com contraste, foi identificada trombose venosa esplâncnica envolvendo a veia porta, esplénica e mesentérica superior, sem sinais de doença hepática crónica. Foi de imediato iniciada anticoagulação com varfarina. Da investigação etiológica, destaca-se a presença da mutação JAK2 V617F e medula óssea hipercelular com aumento das contagens de células mielóides e megacariócitos atípicos, consistente com mielofibrose primária em estadio pré-fibrótico. Não se identificaram distúrbios pro-trombóticos concomitantes. Discussão: Apresenta-se um raro caso de trombose da veia porta, esplénica e mesentérica superior, demonstrando que as neoplasias mieloproliferativas podem apresentar-se sob a forma de trombose venosa esplâncnica extensa, mesmo em estadios precoces e na ausência de distúrbios pro-trombóticos concomitantes. A presença da mutação JAK2 é um importante factor de risco pro-trombótico que pode por si só contribuir para a formação de tromboses venosas extensas.

ABSTRACT Objective: To describe the correlation between the residues, their anatomical location and the presence of laryngotracheal penetration and aspiration in patients after supracricoid laryngectomy undergoing cricohyoidoepiglotopexy reconstruction. Methods: This study included 70 patients treated by supracricoid laryngectomy with cricohyoidoepiglotopexy reconstruction in a referral national cancer hospital. The patients were submitted to swallowing videofluoroscopy, and the findings were classified by the penetration and aspiration scale. The images were described observing the presence or absence of residues and their anatomical location. Results: The prevalence of penetration in this study was 68.6% and aspiration was 34.3%. An association was found between the presence of residue on the tongue (p=0.005), posterior pharyngeal wall (p=0.013), pyriform recesses (p=0.002), valecula (p=0.061), and laryngeal penetration. The residue in the upper esophageal sphincter (p=0.039) was associated with the occurrence of laryngotracheal aspiration. Conclusion: Patients undergoing supracricoid laryngectomy with cricohioidoepiglotopexy reconstruction have food residues in different anatomical regions after swallowing. Penetration was associated with the presence of residues on the base of the tongue and posterior pharyngeal wall. Aspiration was associated with the presence of residues in the upper esophageal sphincter.

Abstract Globally, bats are of interest in many studies, beyond their ecological or epidemiological relevance or even the ecomorphological diversity of species. In Brazil, most of the indexed studies on chiropterans date from 1954, with a slow and heterogeneous progress in the publication increasing. The aim of this study was to analyze the literature on bats in Brazil, identifying patterns, tendencies and knowledge gaps in the Brazilian federal states. We carried out a sistematyzed search on the online databases Clarivate Analytics Web of Science (WoS), Scientific Electronic Library Online (SciELO), PubMed and Scopus. We used the descriptive terms “Chiroptera” and “Brazil”. Besides these bases, we included data from the manuscripts published in Chiroptera Neotropical. We obtained a total of 1,115 articles, which were analyzed and classified in 22 thematic categories based on the articles’ approach. We observed that each Brazilian region and state had particularities in their knowledge panoramas of bats, not being possible to generalize conditions for each federal region. Even though the increasing in the number of articles by categories, we encourage that every approach keep being developded, once no thematic could had been considered enough explored till the moment.
