Este artigo analisou a gestão federal da Política Nacional de Atenção Integral Saúde do Homem (PNAISH) sob uma perspectiva histórico-crítica. Foi realizada análise documental do processo de implementação da política entre 2009 e 2024 contido no arquivo da Coordenação de Atenção à Saúde do Homem (COSAH). A análise de conteúdo resultou nas seguintes categorias temáticas: organização e gestão da COSAH; processo de revisão da PNAISH; e desafios e frentes estratégicas para a implementação da PNAISH. Foram encontradas fragilidades na institucionalização da coordenação, além de uma revisão da política, em 2021, marcada pelo discurso neoconservador, que excluiu as menções a gênero e direitos sexuais e reprodutivos. Esses princípios epistemológicos da PNAISH foram retomados em 2023 com ações de qualificação da Atenção Primária para abordagem das repercussões do machismo e do patriarcado sobre a saúde de homens, mulheres e crianças.

Abstract A long-term indwelling catheter may be indicated in clinical situations, such as chronic diseases of the genitourinary or neurological systems. In addition to the risks of infection, trauma, and bleeding, a catheter’s permanence can affect psycho-emotional and socioeconomic dimensions. We aimed to understand how the need to use a long-term indwelling catheter affects this patient’s self-perception, interrelationships, and self-care. We carried out a qualitative, descriptive study based on interviews with 17 patients, and applied thematic analysis and complex thinking. The different prognoses and expectations regarding the catheter influenced self-perception, adaptation, acceptance, or denial. The presence of a catheter, whether as a curative measure or for comfort, can affect self-image and sexuality, and generate insecurities and uncertainties, which require understanding the multidimensionality of situations that suffer interference from the personal, family, and social environment, as well as health systems’ capacity to deal with it. Resumo O cateter vesical de longa permanência pode ser indicado em situações clínicas, como nas doenças crônicas do sistema genitourinário ou neurológico. Além dos riscos de infecção, traumas e sangramentos, a permanência do cateter pode afetar dimensões psicoemocionais e socioeconômicas. Objetivamos compreender como a necessidade de uso do cateter urinário por um longo prazo afeta a autopercepção, as interrelações e o autocuidado deste paciente. Realizamos um estudo qualitativo, descritivo, a partir da entrevista de 17 pacientes, e aplicamos a análise temática e o pensamento complexo. Os diferentes prognósticos e as expectativas em relação ao cateter influenciaram a autopercepção, a adaptação, sua aceitação ou negação. ABSTRACT Objective: to report data collection via telephone carried out in multicenter research on nursing care assessment during the COVID-19 pandemic. Method: this is an experience report on using the telephone to collect quantitative and qualitative data with participants from ten Brazilian university hospitals from October 2020 to December 2021. The experience was presented in stages: 1) Operationalization of data collection via telephone; 2) Interviewing team training; 3) Monitoring and adjustments to data collection; and 4) Results of telephone contact with patients. Results: data collection planning and organization involved creating guidance manuals to guide the collectors, which were validated for clarity and agreement. For monitoring and adjustments, a weekly meeting was held with the interviewers in charge and researchers. Data from 539 respondents from the Patient Measure of Safety instrument, 643 from the Care Transitions Measure instrument and 56 from open interviews were included. Conclusion: using guidance manuals for data collection via telephone, training and follow-up meetings are strategies that can enhance this strategy in multicenter research when in-person data collection is impossible. RESUMEN Objetivo: informar la recolección de datos vía telefónica realizada en una investigación multicéntrica sobre la evaluación de los cuidados de enfermería durante la pandemia de COVID-19. Método: informe de experiencia sobre el uso del teléfono para la recolección de datos cuantitativos y cualitativos con participantes de diez hospitales universitarios brasileños, de octubre de 2020 a diciembre de 2021. La experiencia fue presentada en etapas: 1) Operacionalización de la recolección de datos por teléfono; 2) Capacitación del equipo entrevistador; 3) Monitoreo y ajustes a la recolección de datos; y 4) Resultados del contacto telefónico con el paciente. Resultados: la planificación y organización de la recolección de datos implicó la creación de manuales de orientación para guiar a los recolectores, los cuales fueron validados por su claridad y acuerdo. Abstract Background: Acute respiratory diseases are difficult to measure and contribute to a large number of ill-defined causes of death, which affects the reliability of mortality information. Objective: To verify the accuracy of the definition of the underlying cause of death for acute respiratory diseases registered in the Mortality Information System of the Federal District in 2018. Method: An accuracy study was conducted, with a minimum sample of 331 death certificates with the causes of death according to Chapter 10 (diseases of the respiratory system — J00-J99 — according to subcategories) of the International Classification of Diseases, version 10, considering the definition of the underlying cause of death by the research team as the gold standard. Sensitivity, specificity, positive and negative predictive values, and positive and negative likelihood ratios were calculated with their respective 95% confidence intervals (CIs). Results: Overall, 396 death certificates were included. High sensitivity was identified to determine acute lung diseases in the underlying cause of death (82.8%, 95%CI 78.6–86.5) and high specificity (93.1%, 95%CI 77.2–99.2). The probability of the declaration shows that negative false value was low (0.2, 95%CI 0.1–0.2). Conclusion: Death certificates due to acute respiratory diseases showed good results for accuracy and this study encourages similar studies for other conditions. Resumo Introdução: As doenças respiratórias agudas apresentam difícil mensuração e contribuem para o grande número de causas de óbito mal definidas, o que afeta a confiabilidade das informações de mortalidade. Objetivo: Verificar a acurácia da definição da causa básica de óbito para doenças respiratórias agudas dos óbitos registrados no Sistema de Informações sobre Mortalidade do Distrito Federal em 2018. Método: Foi realizado um estudo de acurácia, com amostra mínima de 331 declarações de óbito com as causas de óbito do capítulo 10 (doenças do aparelho respiratório — J00-J99 — conforme subcategorias) da Classificação Internacional de Doenças na versão 10, considerando a definição de causa básica de óbito pela equipe de investigação como o padrão-ouro. Foram calculadas a sensibilidade, especificidade, valores preditivos positivo e negativo e as razões de verossimilhanças positivas e negativas com os respectivos intervalos de confiança a 95%. Resultados: Total de 396 declarações de óbito foram incluídas. Foi constatada alta sensibilidade para identificar doenças pulmonares agudas nas causas básicas de óbito (82,8%. ABSTRACT Wild birds have great prominence in the transmission of diseases to humans, mainly due to their ease of access to human populations, raising concerns about the potential impact of that proximity on public health. The present study reports ARV and PBV occurrence in wild birds from an environmental protection area in the Amazon biome, in Pará state, Brazil. We obtained 155 fecal specimens from 47 species of wild birds for RNA extraction, ARV and PBV detection utilizing molecular methods, nucleotide sequencing, and phylogenetic analysis. ARV prevalence was 0.6% (1/155), being positive in an individual of Myrmotherula longipennis, and PBV prevalence was 1.29% (2/155), affecting two individuals of Guira guira. The PBV strains were assigned to genogroup I based on phylogenetic analysis, and they shared a strong phylogenetic link with strains isolated from different geographic locations. The ARV strain was more closely related to strains that had previously circulated in the same region. The presence of ARV and PBV in this habitat suggests that infection cycles of these agents occur naturally in the wild ecosystem, potentially triggering transmission events between bird species and humans. RESUMO As aves silvestres têm grande destaque na transmissão de doenças ao homem, principalmente pela facilidade de acesso à população humana, levantando preocupações sobre o potencial impacto dessa proximidade no contexto da saúde pública. O presente estudo relata a ocorrência de ARV e PBV em aves silvestres de uma área de proteção ambiental no bioma Amazônico. Obtivemos 155 espécimes fecais de 47 espécies de aves silvestres, que foram submetidos a extração de RNA, detecção de ARV e PBV por técnicas moleculares, sequenciamento nucleotídico e análise filogenética. A prevalência de infecção por ARV foi 0,6% (1/155), sendo positivo em um indivíduo de Myrmotherula longipennis, e a prevalência de PBV foi 1,29% (2/155), afetando dois insdivíduos de Guira guira. A análise filogenética agrupou as cepas de PBV no genogrupo I, mostrando intensa relação filogenética com cepas isoladas de outras regiões geográficas. A cepa de ARV esteve mais relacionada filogeneticamente a cepas previamente circulantes na mesma região. ABSTRACT OBJECTIVE To describe two low-burden diet quality scores and evaluate their performance in reflecting the dietary share of the least and most processed foods defined within the Nova food system classification. METHODS This cross-sectional study included data from the NutriNet-Brasil cohort. Participants answered the Nova24hScreener, a 3-minute self-administered questionnaire measuring the consumption of a set of foods on the day before. Food items included in this tool belong to two main groups of the Nova classification: unprocessed or minimally processed whole plant foods (WPF, 33 items) and ultra-processed foods (UPF, 23 items). Two scores were obtained by summing the number of items checked: the Nova-WPF and the Nova-UPF. We compared the scores, respectively, with the dietary intake (% of total energy) of all unprocessed or minimally processed whole plant foods and all ultra-processed foods obtained from a full self-administered web-based 24-hour recall performed on the same day. RESULTS The approximate quintiles of each score had a direct and linear relationship with the corresponding % of energy intake (p-value for linear trend < 0.001). We found a substantial agreement between the intervals of each score and the corresponding % of energy intake (Nova-WPF score: Prevalence-Adjusted and Bias-Adjusted Kappa (PABAK) 0.72, 95%CI 0.64–0.81; Nova-UPF score: PABAK 0.79, 95%CI 0.69–0.88). CONCLUSIONS These two scores performed well against the dietary share of unprocessed or minimally processed whole plant foods and ultra-processed foods in Brazil and can be used to evaluate and monitor diet quality.

This study aimed to evaluate the structure and genomic diversity of yellow mombin accessions collected in nine locations in Brazil using Single Nucleotide Polymorphisms (SNP) markers. Significant genetic structure was observed in the discriminant analysis of principal components (DAPC) and dendrogram construction, in accordance with our hypotheses. The Mantel test identified a highly positive and significant correlation between geographic and genetic distances. The locations from the Amazon biome presented higher genetic diversity values when compared to those from the Atlantic Forest and Cerrado, which is expected considering the higher vulnerability of these biomes. However, although presenting greater genetic diversity, the Amazon biome showed positive inbreeding coefficients (F IS ) in three of the four locations, ranging from 0.0855 to 0.2421, indicating a potential risk of genetic erosion, possibly related to the increased degradation of this biome in recent decades. The results obtained contribute to the understanding of the distribution of genetic variation and conservation status of yellow mombin in Brazil. They could also be used as a subsidy for developing conservation strategies and the genetic improvement of this species. ABSTRACT OBJECTIVE To describe two low-burden diet quality scores and evaluate their performance in reflecting the dietary share of the least and most processed foods defined within the Nova food system classification. METHODS This cross-sectional study included data from the NutriNet-Brasil cohort. Participants answered the Nova24hScreener, a 3-minute self-administered questionnaire measuring the consumption of a set of foods on the day before. Food items included in this tool belong to two main groups of the Nova classification: unprocessed or minimally processed whole plant foods (WPF, 33 items) and ultra-processed foods (UPF, 23 items). Two scores were obtained by summing the number of items checked: the Nova-WPF and the Nova-UPF. We compared the scores, respectively, with the dietary intake (% of total energy) of all unprocessed or minimally processed whole plant foods and all ultra-processed foods obtained from a full self-administered web-based 24-hour recall performed on the same day. RESULTS The approximate quintiles of each score had a direct and linear relationship with the corresponding % of energy intake (p-value for linear trend < 0.001). We found a substantial agreement between the intervals of each score and the corresponding % of energy intake (Nova-WPF score: Prevalence-Adjusted and Bias-Adjusted Kappa (PABAK) 0.72, 95%CI 0.64–0.81; Nova-UPF score: PABAK 0.79, 95%CI 0.69–0.88). ABSTRACT BACKGROUND AND OBJECTIVES: Klippel-Trenaunay syndrome (KTS) is a rare congenital disease that manifests at birth or in childhood and has an unknown etiology, with no ethnic or gender predilection. Pain management, common in many cases, is challenging due to the complexity of the factors involved. The aim of this report is to demonstrate the effective use of a multimodal approach, including medicinal cannabis, in the treatment of pain in a child with KTS. CASE REPORT: this is the case of a child suffering from CTS who had severe chronic pain, significantly compromising his quality of life. The pain was managed with pharmacological therapies, including the use of medicinal cannabis as a therapeutic alternative when conventional treatments proved insufficient. The multimodal approach, which included the use of tetrahydrocannabinol, led to a reduction in pain and an improvement in the patient’s quality of life. Medical cannabis therapy proved to be an effective option, especially in cases of pain refractory to other interventions. CONCLUSION: The treatment of pain in patients with KTS should be multimodal, with an emphasis on non-invasive approaches. The use of medicinal cannabis is a viable and safe alternative, particularly in situations where conventional treatments do not provide adequate relief. O manejo da dor, comum em muitos casos, é desafiador devido à complexidade dos fatores envolvidos. O objetivo deste relato foi demonstrar o uso eficaz de uma abordagem multimodal, incluindo cannabis medicinal, no tratamento da dor em uma criança com KTS. RELATO DO CASO: Trata-se do caso de uma criança portadora de KTS que apresentava dor crônica intensa, comprometendo significativamente sua qualidade de vida. A dor foi manejada com terapias farmacológicas, incluindo uso de cannabis medicinal como alternativa terapêutica, quando os tratamentos convencionais se mostraram insuficientes. A abordagem multimodal, que incluiu o uso de tetraidrocanabinol, levou à redução da dor e à melhora na qualidade de vida do paciente. RESUMO JUSTIFICATIVA E OBJETIVOS: A dor crônica e os distúrbios do sono são condições comuns. A ocorrência simultânea dessas duas entidades resulta em maior sofrimento pessoal e redução do bem-estar e da funcionalidade. O objetivo desta pesquisa foi avaliar a influência da dor crônica no sono e o seu impacto na qualidade de vida. MÉTODOS: Trata-se de um estudo observacional, transversal, individual e não controlado, realizado no Centro Multidisciplinar de Dor do Hospital das Clínicas da Universidade Federal de Minas Gerais (HC-UFMG). Os pacientes responderam a uma entrevista semiestruturada, face a face, por meio do sistema de computador do próprio centro de dor (avaliaDor®), utilizando dispositivos móveis conectados à internet. Foram coletados dados socioeconômicos e de dor; em seguida foram aplicados instrumentos para avaliar a intensidade da dor (Escala Visual Numérica - EVN), a qualidade de vida (Medical Outcomes Study 36 - Short Form Health Survey - SF-36), a presença de distúrbios psiquiátricos (Escala Hospitalar de Ansiedade e Depressão - HAD) e o sono (Mini Sleep Questionnaire - MSQ). RESULTADOS Cento e três pacientes participaram do estudo. Setenta e dois por cento eram do sexo feminino, com mediana de idade de 55 anos, com predominância de brancos com baixo nível de escolaridade. De toda a população estudada, 74,76% dos participantes apresentaram “dificuldade grave” para dormir, enquanto 22,33% tiveram “boa qualidade” de sono. Em relação à intensidade da dor, os pacientes dos grupos de dor moderada e intensa apresentaram maior percentual de “dificuldade grave” para dormir quando comparados aos pacientes com dor leve. Em relação à presença de ansiedade e depressão, observou-se maior pontuação nos itens: “sono agitado”, “cansaço sem motivo aparente”, “acorda com dor de cabeça”, “acorda e volta a dormir”, “acorda cansado pela manhã” e, por fim, “acorda e não volta a dormir”. Os dados gerais do SF-36 apontam para uma baixa qualidade de vida dos doentes do presente estudo e, com base na análise multivariada, os itens “SF-36-Dor e SF-36-Vitalidade” revelaram-se fatores protetores da presença de perturbações do sono. CONCLUSÃO: Os distúrbios de sono em pacientes com dor crônica são comuns e determinam piora na qualidade de vida. A frequência desse distúrbio foi maior nos pacientes com ansiedade e depressão, em comparação com os pacientes que não apresentaram essas desordens psiquiátricas. ABSTRACT BACKGROUND AND OBJECTIVES: Chronic pain and its relationship with sleep disturbance are common conditions with great influence on the individual’s quality of life. The simultaneous occurrence of these two entities results in greater suffering, reduced functionality and well-being. The objective of this research was to evaluate the influence of chronic pain on sleep and its impact on quality of life. METHODS: This is an observational, cross-sectional, individual, uncontrolled study carried out at the Multidisciplinary Pain Center of the Clinical Hospital of the Federal University of Minas Gerais (Hospital das Clínicas da Universidade Federal de Minas Gerais - HC-UFMG). The patients answered a semi-structured, face-to-face interview via the pain center’s own computer system (avaliaDor®), using internet-connected mobile devices. Socioeconomic and pain data was collected, and then instruments were applied to assess pain intensity (Visual Numerical Scale - VNS), quality of life (Medical Outcomes Study 36 - Short Form Health Survey - SF-36), presence of psychiatric disturbances (Hospital Depression Anxiety Scale - HAD) and sleep (Mini Sleep Questionnaire - MSQ). RESULTS: One hundred and three patients participated in the study. Seventy two percent were women, the median age was 55 years, and the patients were predominantly married, white and with a low level of schooling. Of the entire study population 74.76% presented “severe difficulty” in sleeping, while 22.33% had “good quality” sleep. Regarding pain intensity, patients in the moderate and severe pain groups had a higher percentage of “severe difficulty” in sleeping, when compared to patients with mild pain. About the presence of anxiety and depression, it was observed lower scores on: “restless sleep”, “tiredness for no apparent reason”, “wakes up with a headache”, “wakes up and goes back to sleep”, “wakes up tired in the morning”, and finally “wakes up and does not go back to sleep”. The general data from the SF-36 points to a low quality of life for the patients in the present study and, based on the multivariate analysis, the items “SF-36-Pain and SF-36-Vitalidade” were shown to be protective factors for the presence of sleep disturbances. CONCLUSION: Sleep disorders in patients with chronic pain are common and show a worsening in quality of life. The frequency of this disturbance was higher in patients with anxiety and depression compared to patients without these psychiatric disorders. The early identification of sleep disturbances in individuals with chronic pain is necessary to improve well-being. ABSTRACT Objective: To assess the impact of different vertical positions on lung aeration in patients receiving invasive mechanical ventilation. Methods: An open-label randomized crossover clinical trial was conducted between January and July 2020. Adults receiving invasive mechanical ventilation for > 24 hours and < 7 days with hemodynamic, respiratory and neurological stability were randomly assigned at a 1:1 ratio to the sitting position followed by passive orthostasis condition or the passive orthostasis followed by the sitting position condition. The primary outcome was lung aeration assessed using the lung ultrasound score (score ranges from 0 [better] to 36 [worse]). Results: A total of 186 subjects were screened; of these subjects, 19 were enrolled (57.8% male; mean age, 73.2 years). All participants were assigned to receive at least one verticalization protocol. Passive orthostasis resulted in mean lung ultrasound scores that did not differ significantly from the sitting position (11.0 versus 13.7; mean difference, -2.7; [95%CI -6.1 to 0.71; p = 0.11). Adverse events occurred in three subjects in the passive orthostasis group and in one in the sitting position group (p = 0.99). Conclusion: This analysis did not find significant differences in lung aeration between the sitting and passive orthostasis groups. A randomized crossover clinical trial assessing the impact of vertical positioning on lung aeration in patients receiving invasive mechanical ventilation is feasible. RESUMO Objetivo: Avaliar o impacto de diferentes posicionamentos verticais na aeração pulmonar em pacientes em ventilação mecânica invasiva. Métodos: Trata-se de ensaio clínico aberto, randomizado e transversal, realizado entre janeiro e julho de 2020. Adultos em ventilação mecânica invasiva por mais de 24 horas e menos de 7 dias com estabilidade hemodinâmica, respiratória e neurológica foram distribuídos aleatoriamente em uma proporção de 1:1 à postura sentada seguida da condição de ortostatismo passivo ou o ortostatismo passivo seguido de postura sentada. O desfecho primário foi a aeração pulmonar avaliada pelo lung ultrasound score. O escore varia de zero (melhor) a 36 (pior). Resultados: Foram selecionados 186 indivíduos; destes, 19 foram incluídos (57,8% do sexo masculino; média idade de 73,2 anos). Todos os participantes foram selecionados para receber pelo menos um protocolo de verticalização. O ortostatismo passivo resultou em escores médios de aeração pulmonar por ultrassonografia que não diferiram significativamente da postura sentada (11,0 versus 13,7; diferença média, -2,7; IC95% -6,1 a 0,71; p = 0,11). Ocorreram eventos adversos em três indivíduos no grupo ortostatismo passivo e em um no grupo postura sentada (p = 0,99). Abstract: The use of imagem strategies in medical socieal networks is increasingly widespread, meeting the ethical limit established by the CEM. Knowing this, the present study sought to understand the problem of patient exposure in medical social networks. For this, a literature review was carried out from the Scientific Electronic Library Online (Scielo), Pubmed, UpToDate, LILACS databases, articles in English, Spanish and Portuguese published from 2018 onwards were included. Thus, a tendency was noticed in some cities to create laws to regulate this exposure, given that the use of images has become routine in medical advertising, as a way to demonstrate results. However, this practice is greatly accompanied by ethical failures as recommended by the CFM.

Sabiendo esto, el presente estudio buscó comprender el problema de la exposición de los pacientes en las redes sociales médicas. Resumo O objetivo foi avaliar a evolução temporal do recebimento de orientações sobre hábitos saudáveis entre brasileiros com hipertensão arterial e diabetes mellitus. Estudo transversal com dados da Pesquisa Nacional de Saúde de 2013 e 2019. Para verificar a evolução temporal das prevalências de orientações sobre hábitos saudáveis, realizadas por profissionais de saúde nos atendimentos para as morbidades, utilizou-se regressão linear ponderada pelos quadrados mínimos da variância, estimando a variação percentual anual (VPA) apresentada de acordo com sexo, cor da pele, faixa etária e quintis do índice de bens. A amostra analítica, em 2013, foi de 11.129 indivíduos com hipertensão e 3.182 com diabetes, e em 2019, de 19.107 indivíduos com hipertensão e 6.317 com diabetes. Para hipertensão, observou-se reduções nas orientações para não fumar (VPA: -1.49), não ingerir bebidas alcoólicas em excesso (VPA: -1.48), ingerir menos sal (VPA: -0.56) e, ainda, para todas as orientações (VPA: -1.17). Para diabetes, foram observadas reduções estatisticamente significativas apenas para não fumantes (APC: -1.13) e para os que não consomem bebidas alcoólicas em excesso (APC: -1.11). Abstract To evaluate time evolution of receiving advice on healthy habits among Brazilians with hypertension and diabetes mellitus. Cross-sectional study with data from the 2013 and 2019 National Health Survey. We used linear regression weighted by least squares of variance to verify time evolution of the outcome estimating the annual percentage change (APC) presented according to sex, skin color, age group, and quintiles of wealth index. The analytical sample in 2013 was 11,129 individuals with hypertension and 3,182 individuals with diabetes, and in 2019 19,107 individuals with hypertension and 6,317 individuals with diabetes. For those with hypertension, there were statistically significant reductions in receiving advice for not smoking (APC: -1.49), not drinking excessive alcoholic beverages (APC: -1.48), ingesting less salt (APC: -0.56), and for all healthy habits (APC: -1.17). For those with diabetes, statistically significant reductions were observed only for not smoking (APC: -1.13) and not drinking excessive alcoholic beverages (APC: -1.11).
