Application of the pulmonary embolism rule-out criteria (PERC rule) and age-adjusted D-Dimer in patients undergoing computed tomography pulmonary angiography for diagnosis of pulmonary embolism

Abstract Background Diagnosis of pulmonary embolism (PE) constitutes a challenge for practitioners. Current practice involves use of pre-test probability prediction rules. Several strategies to optimize this process have been explored. Objectives To explore whether application of the pulmonary embolism rule-out criteria (PERC rule) and age-adjusted D-dimer (DD) would have reduced the number of computed tomography pulmonary angiography (CTPA) examinations performed in patients with suspected PE. Methods A retrospective cross-sectional study of adult patients taken for CTPA under suspicion of PE in 2018 and 2020. The PERC rule and age-adjusted DD were applied. The number of cases without indications for imaging studies was estimated and the operational characteristics for diagnosis of PE were calculated. Results 302 patients were included. PE was diagnosed in 29.8%. Only 27.2% of ‘not probable’ cases according to the Wells criteria had D-dimer assays. Age adjustment would have reduced tomography use by 11.1%, with an AUC of 0.5. The PERC rule would have reduced use by 7%, with an AUC of 0.72. Conclusions Application of age-adjusted D-dimer and the PERC rule to patients taken for CTPA because of suspected PE seems to reduce the number of indications for the procedure.
Resumo Contexto O diagnóstico de embolia pulmonar (EP) representa um desafio para o profissional. A prática atual envolve o uso de modelos de previsão de probabilidade pré-teste e, para otimizar esse processo, várias estratégias têm sido exploradas. Objetivos Investigar se a aplicação dos critérios de exclusão de EP (pulmonary embolism rule-out criteria, PERC) e do D-dímero (DD) ajustado para idade diminui o número de angiografias computadorizadas (ATCs) pulmonares realizadas em pacientes com suspeita de EP. Métodos Estudo transversal retrospectivo com pacientes adultos submetidos a ATC pulmonar com suspeita de EP em 2018 e 2020. Foram aplicados os critérios PERC e o DD ajustado para idade. Foi estimado o número de casos não indicados para exames de imagem, e foram calculadas as características operacionais para o diagnóstico de EP. Resultados Foram incluídos 302 pacientes, dos quais 29,8% apresentaram diagnóstico de EP. Apenas 27,2% dos casos não prováveis de acordo com os critérios de Wells apresentaram DD; o ajuste implicou em uma diminuição de ACTs de 11,1%, com área sob a curva de 0,5. Os critérios PERC diminuiriam em 7%, com área sob a curva de 0,72. Conclusões A aplicação do DD ajustado para idade e dos critérios PERC em pacientes submetidos a ATC pulmonar por suspeita de EP parece diminuir a indicação para tais exames.

RESUMEN Introducción: El tromboembolismo pulmonar (TEP) es una patología frecuente, que genera repercusiones hemodinámicas importantes y alta tasa de mortalidad, con alta incidencia en la enfermedad por coronavirus 2019 (COVID-19). Objetivo: Caracterizar el comportamiento clínico, de diagnóstico y pronóstico de los pacientes con sospecha de TEP antes y durante la pandemia de SARS-CoV-2. Metodología: Estudio de cohorte prospectiva de pacientes adultos llevados a angiotomografía de tórax por sospecha de TEP durante dos periodos de tiempo: a) pre-COVID-19: junio de 2018 a diciembre de 2019, y b) COVID-19: junio a diciembre de 2020. Se condujeron análisis bivariados y se construyeron curvas ROC calculando las áreas bajo la curva para el diagnóstico de TEP del dímero D y las reglas de predicción clínica. Resultados: Se incluyeron 302 pacientes pre COVID-19 y 55 pacientes con COVID-19. El dímero D muestra un desempeño moderado para diagnóstico del TEP con AUC: 0,73 (IC 95% 0,62-0,84) en fase pre-COVID-19 vs. 0,75 (IC95% 0,58-0,92) en fase COVID-19. Las áreas bajo la curva de cada una de las reglas de predicción clínica tuvieron un desempeño moderado a bajo en la fase pre-COVID-19 (AUC: 0,623 a 0,697), frente a una no discriminatoria en la fase COVID-19 (0,355 a 0,450). Conclusiones: Los factores de riesgo tradicional fueron poco prevalentes en pacientes con COVID-19 y TEP. Aunque el dímero D fue más alto en aquellos con TEP, la diferencia no fue estadísticamente significativa. Las reglas de predicción clínicas para el diagnóstico de TEP mostraron un bajo poder discriminativo en pacientes con COVID-19.
ABSTRACT Background: Pulmonary embolism (PE) is a frequent disease generating important hemodynamic effects and high mortality rate, with great incidence in coronavirus disease (COVID-19). Objective: The aim of this study was to characterize the clinical, diagnostic, and prognostic behavior of patients with suspected PE before and during the SARS-CoV-2 pandemic. Methods: A prospective cohort study of adult patients with suspected PE undergoing computed tomography pulmonary angiography was carried out during two periods: a) the pre-COVID-19 phase: June 2018 to December 2019, and b) during the COVID-19 phase: June to December 2020. Bivariate analyses were conducted and ROC curves were built calculating the areas under the curve (AUC) for D-dimer PE diagnosis and clinical prediction rules. Results: Three-hundred and two pre-COVID-19 patients and 55 patients with COVID-19 were included in the study. D-dimer showed a moderate performance for the diagnosis of PE, with AUC 0.73 (95% CI 0.62-0.84) in pre-COVID-19 phase vs. 0.75 (95% CI 0.58-0.92) in COVID-19 phase. The AUC of each of the clinical prediction rules had moderate to low performance in the pre-COVID-19 phase (AUC 0.623 to 0.697), with a non-discriminatory AUC in the COVID-19 phase (0.355 to 0.450). Conclusions: Traditional risk factors were poorly prevalent in patients with COVID-19 and PE. Although D-dimer was higher in those with PE, the difference was not statistically significant. Clinical prediction rules for PE diagnosis showed low discriminative power in COVID-19 patients.

Resumen Introducción: La trombosis venosa abdominal es inusual y es una de las formas menos estudiada de la enfermedad tromboembólica venosa. Objetivo: Describir las características epidemiológicas, los factores de riesgo, las complicaciones y el manejo anticoagulante en pacientes con diagnóstico de trombosis venosa abdominal, atendidos en una clínica de anticoagulación en Cali, Colombia. Materiales y método: Estudio descriptivo en el que se identificaron 83 pacientes con diagnóstico de trombosis venosa abdominal, manejados en la clínica de anticoagulación de la Fundación Valle del Lili, entre los años 2011 y 2019. La edad promedio fue de 53.3 ± 17.4 años y el 59% eran de sexo femenino. Fue más frecuente en las venas mesentéricas (19%), seguidas de la vena porta (18%). Los antecedentes incluyen cáncer, hipertensión arterial y estado hipercoagulable/trombofilia. Conclusiones: La trombosis venosa abdominal es una patología poco frecuente. El principal compromiso se presenta en la vena mesentérica. La enoxaparina es el manejo más frecuente. Los pacientes requieren en su mayoría anticoagulación extendida.
Abstract Introduction: Abdominal venous thrombosis is rare condition and is one of the least studied forms of venous thromboembolic disease. Objective: To describe the epidemiological characteristics, risk factors, complications and anticoagulant management in patients with a diagnosis of abdominal venous thrombosis, treated at an anticoagulation clinic in Cali, Colombia. Materials and method: Descriptive study, 83 patients with diagnosis of abdominal venous thrombosis were identified, managed in the anticoagulation clinic of Fundación Valle del Lili, between the years 2011 and 2019. The average age was 53.3 ± 17.4, and 59% female. More frequent in mesenteric veins (19%), followed by portal vein (18%). Personal antecedent includes cancer, high blood pressure, and hypercoagulable/thrombophilia status. Conclusions: Abdominal vein thrombosis is a rare disease. The main compromise occurs at the level of the mesenteric vein. Enoxaparin is the most common management. Most patients require extended anticoagulation.

Resumen Introducción: En la población pediátrica, el 95% de las tromboembolias venosas son secundarias a un factor de riesgo; la presencia de catéter venoso central es el factor de riesgo más importante para el desarrollo de trombosis venosa profunda en esta población. Los estudios en población adulta sobre el uso de anticoagulantes se han extrapolado a la población pediátrica, y por ello es importante tener en cuenta las características diferenciales entre ambas poblaciones. Método: Se realizó un estudio descriptivo en el que se revisaron 2300 registros clínicos de pacientes seguidos por la Clínica de Anticoagulación institucional desde 2011 hasta 2019, buscando identificar pacientes menores de 18 años que hubieran recibido manejo anticoagulante ambulatorio. Resultados: Se encontraron 43 pacientes menores de 18 años, el 60% de sexo femenino, con una edad promedio de 9.8 ± 5.2 años. El antecedente patológico más prevalente fueron las valvulopatías (28%). Las principales indicaciones para recibir manejo anticoagulante incluyeron cardiopatías congénitas (21%), prótesis valvular cardíaca mecánica (21%, 5 aórticas y 4 mitrales) y trombosis venosa profunda (14%). La warfarina se utilizó en el 72% de los pacientes. El 81% de los pacientes recibieron tratamiento anticoagulante extendido. Las principales complicaciones durante el tratamiento anticoagulante fueron sangrado ginecológico y cutáneo, así como trombocitopenia. Conclusiones: Las cardiopatías congénitas y las valvulopatías cardíacas son las principales indicaciones para considerar el manejo anticoagulante en la población pediátrica. La warfarina es el medicamento más utilizado en forma ambulatoria en esta población. La adherencia adecuada y el seguimiento ambulatorio en esta población requieren herramientas adicionales para lograr un excelente tratamiento anticoagulante.
Abstract Introduction: In pediatric patients, 95% of venous thromboembolisms are secondary to a risk factor; the presence of central venous catheter is the most important risk factor for the development of deep vein thrombosis in this population. Studies about anticoagulation treatment in adult patients have been extrapolated to the pediatric population, it is important to consider the differential characteristics between these two populations. Method: A descriptive study was conducted; there were reviewed 2300 clinical records of patients followed by the institutional anticoagulation clinic from 2011 to 2019, looking for underage patients who have received ambulatory anticoagulation treatment management. Results: 43 patients under the age of 18 years old were found. 60% were female and the average age was 9.8 ± 5.2 years. The most prevalent pathological antecedent was valve disease (28%). The main indications for anticoagulant treatment included congenital heart disease (21%), mechanical heart valves (21%, 5 aortic and 4 mitral) and deep vein thrombosis (14%). Warfarin was found in 72% of patients. 81% of patients received extended anticoagulation therapy. The main complications during anticoagulant treatment included gynecological and cutaneous bleeding, as well as thrombocytopenia. Conclusions: Congenital heart disease and heart valve disease are the main indications for considering anticoagulation treatment in the pediatric population. Warfarin is the most formulated anticoagulant in this outpatient population. Adequate adherence and outpatient follow-up in requires additional tools to achieve excellent anticoagulant treatment.

Abstract Acute myeloid leukemia (AML) comprises a heterogeneous group of hematopoietic cell neoplasms of myeloid lineage that arise from the clonal expansion of their precursors in the bone marrow, interfering with cell differentiation, leading to a syndrome of bone marrow failure. AML is a consequence of genetic and epigenetic changes (point mutations, gene rearrangements, deletions, amplifications, and arrangements in epigenetic changes that influence gene expression) in hematopoietic precursor cells, which create a clone of abnormal cells that are capable of proliferating but cannot differentiate into mature hematopoietic cells or undergo programmed cell death. The diagnosis requires more than 20% myeloid blasts in the bone marrow and certain cytogenic abnormalities. Treatment will depend on age, comorbidities, and cytogenetic risk among the most frequent.

Molecular transduction in receptor-ligand systems by planar electromagnetic fields

Resumo A ligação de um ligante com um receptor molecular induz um sinal que viaja através do receptor, chegando ao domínio interno e disparando uma cascata de resposta. Em trabalhos anteriores em receptores de células T e sua ligação com antígenos estranhos, observamos a presença de padrões moleculares planares capazes de gerar campos eletromagnéticos dentro das proteínas. Esses planos mostraram um comportamento coerente (sincronizado), replicando, instantaneamente, no domínio intracelular o que ocorreu no domínio extracelular, enquanto o ligante era acoplado. No presente estudo, examinamos essa transdução – a capacidade de um sinal de acoplamento de penetrar profundamente a molécula receptora e induzir uma resposta. Verificamos a presença de um comportamento coerente em sistemas diversos de receptor-ligante. Para apreciar essa diversidade, apresentamos quatro sistemas bioquímicos diferentes: TCR-peptídeo, ADP-bomba de cálcio, hemoglobina-oxigênio e gp120-CD4 acoplamento viral. A confirmação de transdução molecular sincronizada em cada um desses sistemas sugere que o mecanismo proposto ocorreria em todos os sistemas bioquímicos receptor-ligante.
Abstract The coupling of a ligand with a molecular receptor induces a signal that travels through the receptor, reaching the internal domain and triggering a response cascade. In previous work on T-cell receptors and their coupling with foreign antigens, we observed the presence of planar molecular patterns able to generate electromagnetic fields within the proteins. These planes showed a coherent (synchronized) behavior, replicating immediately in the intracellular domain that which occurred in the extracellular domain as the ligand was coupled. In the present study, we examined this molecular transduction - the capacity of the coupling signal to penetrate deep inside the receptor molecule and induce a response. We verified the presence of synchronized behavior in diverse receptor-ligand systems. To appreciate this diversity, we present four biochemically different systems - TCR-peptide, calcium pump-ADP, haemoglobin-oxygen, and gp120-CD4 viral coupling. The confirmation of synchronized molecular transduction in each of these systems suggests that the proposed mechanism would occur in all biochemical receptor-ligand systems.

Resumen Introducción: La enfermedad coronaria es la principal causa de mortalidad en el mundo. Su tratamiento se asocia con una mejor calidad de vida, la cual puede medirse con el cuestionario de MacNew. Objetivo: Establecer los factores determinantes de calidad de vida en pacientes con enfermedad coronaria. Materiales y métodos: Estudio analítico de corte transversal, en el que se usó el cuestionario de MacNew para valorar la calidad de vida en tres dominios: social, emocional y físico. Se incluyeron 249 pacientes con enfermedad coronaria diagnosticada en los años 2004, 2009 o 2013. Los resultados se estratificaron por el tipo de tratamiento (médico, implantación de stent o cirugía de revascularización miocárdica). Se usó un modelo de regresión beta, como una alternativa al modelo de regresión lineal. Resultados: Los puntajes más bajos se asociaron con hipertensión arterial, diabetes mellitus, dislipidemia y aumento del índice de masa corporal. En el dominio físico el puntaje disminuyó en los pacientes mayores de 60 años. El alto grado de escolaridad se asoció con mayores puntajes en todos los dominios. Conclusiones: Los pacientes con enfermedad coronaria presentaron menor calidad de vida asociada a hipertensión arterial, diabetes mellitus, dislipidemia, aumento del índice de masa corporal o mayor edad y mejor calidad de vida asociada con mayor nivel de escolaridad y afiliación al sistema de salud, indiferente a la intervención terapéutica.
Abstract Introduction: Coronary heart disease is the leading cause of mortality worldwide. Its treatment is associated with a better quality of life; it can be measured with the MacNew questionnaire. This study aims to establish the determin. Objective: To stablish the determinants of quality of life in coronary heart disease patients, in Cali, Colombia. Materials and methods: Analytical cross-sectional study, in which the MacNew questionnaire was applied to assess life quality in three domains: social, emotional and physical. 249 patients with coronary heart disease diagnosed in 2004, 2009 or 2013 were included. The results were stratified by type of treatment (doctor, stent implantation or myocardial revascularization surgery). Beta regression model was used, as an alternative to the linear regression model. Results: The MacNew lower scores were associated with hypertension, diabetes mellitus, dyslipidemia and increased body mass index. In the physical domain, the life quality score decreased in patients older than 60 years. High levels of education were associated with higher quality of life scores in all domains. Conclusions: Coronary heart disease patients presented lower quality of life associated with hypertension, diabetes mellitus, dyslipidemia, increased body mass index or older age and better quality of life associated with higher level of education and affiliation to the health system, regardless of the therapeutic intervention received.

Abstract To identify this increasingly common pathology, known as Multiple Myeloma, it is necessary to refer to the specific factors that characterize it; to this end, the classic criteria known as CRAB (hypercalcemia, renal failure, anemia and lytic lesions) are available, in which renal failure is one of the most frequent complications. Recently, 3 indisputable biomarkers have been described for the diagnostic support for Multiple Myeloma, which are: more than 10% of clonal plasma cells in BM or, a biopsy that corroborates the presence of a plasmacytoma, light chain ratio ≥100 mg/dL and more than one focal lesion on magnetic resonance imaging (MRI). A differential diagnosis for plasma cell leukemia, solitary bone plasmacytoma, and extramedullary plasmacytoma should always be considered. Being this an incurable disease, a lot of research has been done regarding its therapeutic management, whose main objective is the disappearance of plasma cells and the patient clinical improvement. Melphalan was the first drug that showed a benefit in 1958 and afterwards, with the addition of a steroid as a second drug, it was possible to improve response rates. Subsequently, different molecules were studied, forming multiple combinations, and achieving better rates of Overall Survival and Progression-Free Survival. Years later, with the arrival of proteasome inhibitors such as bortezomib, and immunomodulators such as thalidomide and lenalidomide, an important turnaround in the disease has been seen, as deeper responses, more prolonged remissions, and improvement in the quality of life of patients have been achieved. This consensus has the purpose of integrating a group of Mexican specialists and promoting the updating of this pathology.

Medición de la conductividad térmica en materiales aislantes bajo régimen de flujo de calor no estacionario

Resumen Se desarrolló un dispositivo para medir la conductividad térmica en materiales sólidos para aislamiento térmico de uso común en edificaciones, siguiendo un modelo unidimensional de flujo de calor que atraviesa una placa del material a evaluar. Se mide el gradiente de temperatura entre las caras de la placa en función del tiempo, usando un arreglo diferencial de termopares tipo T. Para el control de los mecanismos del instrumento, la adquisición y tratamiento de los datos, se diseñó un circuito con microcontroladores comerciales. En ensayos previos con algunos materiales, se obtuvieron valores la conductividad térmica similares a los reportados en la literatura utilizando un ajuste lineal con valores de R entre 0.90 y 0.98. El dispositivo obtenido respresenta un instrumento útil para la medición de la conductividad térmica, destacando entre sus ventajas: la fácil construcción, el tamaño de las muestras a evaluar y el método de medida, comparado con los métodos tradicionales para medir este parámetro.
Abstract A device to measure thermal conductivity in thermal insulating solid materials commonly used in buildings was developed following a one-dimensional model of heat flow through a plate of the material to be evaluated. The temperature gradient between the plate faces was measured as a function of time by means of a set of type T thermocouples. A circuit with commercial microcontrollers was designed to control the instrument’s mechanisms, the acquisition and the treatment of data. In preliminary tests with some materials, thermal conductivity values similar to those reported in the literature were obtained by using a linear adjustment with R values between 0.90 and 0.98. This device turns out to be a good instrument for measuring thermal conductivity because it has several advantages, such as: easy implementation, sample size, measurement method; compared to those using traditional methods.

ABSTRACT The second part of the Latin American recommendations for the management of Arterial Hypertension (HTA) in adults is presented. In a first phase, the most relevant aspects of epidemiology, physiopathological aspects, how to diagnose, therapeutic guidelines, hypertension emergencies, special populations, refractory hypertension and the application of guides in real life have been described. In this second part, recommendations are issued answering specific questions for primary, secondary, tertiary and quaternary prevention. In general, few recommendations regarding the management of arterial hypertension arise from the medical clinic / internal medicine, despite two situations: the majority of patients with hypertension are evaluated and managed by the clinicians, and the medical clinic is the specialty that allows the holistic and integrated look of the health problems in adults, allowing to add the biographical approach to the biological, to understand and interpret not only the health problem but its causes and consequences (which often correspond to different parenchyma, which in the fragmented model would make the patient move through different specialties). The low percentage of controlled hypertensive patients forces all the professionals involved in the management of them to optimize resources and detect problems that are associated with a deficit control such as the under utilization of pharmacological treatment, low rate of patients treated with combined strategy (the most patients currently receive monotherapy), lack of adequate prescription of changes in lifestyle, low therapeutic adherence and clinical inertia. This publication presents recommendations made by specialists in medical clinic/internal medicine for the management of hypertension in adults, answering primary, secondary, tertiary and quaternary prevention questions.
RESUMEN Se presenta la segunda parte de las recomendaciones latinoamericanas para el manejo de la Hipertensión Arterial (HTA) en adultos. En una primera fase se han descripto los aspectos más relevantes de la epidemiología, aspectos fisiopatológicos, cómo hacer diagnóstico, pautas terapéuticas, urgencias y emergencias hipertensivas, poblaciones especiales, hipertensión refractaria y la aplicación de las guías en la vida real. En esta segunda parte, se emiten recomendaciones respondiendo a preguntas específicas para prevención primaria, secundaria, terciaria y cuaternaria. En general pocas recomendaciones al respecto del manejo de la hipertensión arterial surgen desde la clínica médica/ medicina interna, a pesar de dos situaciones: la mayoría de los pacientes con hipertensión arterial son evaluados y manejados por los clínicos, y la clínica médica es la especialidad que permite la mirada holística e integrada de los problemas de salud del adulto, permitiendo agregar el enfoque biográfico al biológico, comprender e interpretar no solo el problema de salud sino sus causas y consecuencias (que muchas veces suelen corresponder a diferentes parénquimas, lo cual en el modelo fragmentado haría transitar al paciente por distintas especialidades). El bajo porcentaje de pacientes hipertensos controlados obliga a todos los profesionales involucrados en el manejo de los mismos a optimizar recursos y detectar problemas que se asocien a un control deficitario como la sub utilización del tratamiento farmacológico, baja tasa de pacientes tratados con estrategia combinada (la mayoría de los pacientes actualmente recibe monoterapia), falta de prescripción adecuada de los cambios en el estilo de vida, baja adherencia terapéutica e inercia clínica. En la presente publicación se presentan recomendaciones efectuadas por especialistas en clínica médica / medicina interna para el manejo de la hipertensión arterial en adultos, respondiendo preguntas de prevención primaria, secundaria, terciaria, y cuaternaria.

ABSTRACT Intervertebral disc degeneration is a chronic disease that causes significant disability and dependence and exerts a high cost on society. It is the most common cause of back pain, and may involve any segment of the spine, being the lumbar the most frequently affected area. It is one of the most frequent reasons for consultation in the general population, second only to headache, affecting 80-85 % of people throughout life. Current therapeutic strategies are primary conservative, including physical therapy and anti-inflammatory medication. Surgical techniques are intended only to stabilize the spine and/or decompress the spinal or foraminal canal, searching for relieve of symptoms; however, do not address the cause of the degeneration and even accelerate the degeneration of adjacent segments. Understanding of the biology of platelet-rich plasma (PRP) and its effect on condral repair has led to the use of PRP as a promising biological therapeutic strategy for enhance the regenerative process and healing of the intervertebral disc. Very few studies have been conducted in vitro and there are even fewer clinical studies which link PRP with the management of degenerative disc disease. At present, there is no document linking PRP used in injection into the epidural space as a method of intervertebral disc regeneration in cases of disc disease. This is a preliminary study of pain management in patients with degenerative disc disease and axial pain, by using PRP injected into the epidural space, cervical or lumbar.
RESUMEN La enfermedad discal degenerativa es un proceso crónico que produce importante discapacidad y dependencia y que genera un alto coste en la sanidad. Es la causa más común de dolor de espalda, que puede comprometer cualquier segmento del eje axial, siendo la región lumbar el sitio más frecuentemente afectado. Es uno de los motivos de consulta más frecuente en la población general, sólo superado por la cefalea, que afecta al 80-85 % de las personas a lo largo de la vida. El manejo realizado hasta el presente es usualmente de tipo conservador, incluyendo la terapia física y los antinflamatorios. Las técnicas quirúrgicas tienen como objetivo sólo el de estabilizar la columna y/o descomprimir el canal medular o foraminal, buscando el alivio del dolor; sin embargo, para nada corrigen la enfermedad de base asociada al desgaste de los discos intervertebrales, e incluso estas técnicas se han asociado con una aceleración del proceso de desgaste osteocondral. El conocimiento de los fundamentos biológicos del plasma rico en plaquetas (PRP) ha llevado a la investigación de su uso como regenerador del tejido condral en general, y en este caso como regenerador del disco intervertebral, siendo la terapia PRP una estrategia que se presenta hoy como una alternativa muy prometedora de tratamiento (biológico) de la enfermedad discal. Son muy pocos los estudios realizados in vitro, y menos aún los estudios clínicos que relacionan el PRP con el manejo de la enfermedad degenerativa discal. No existe al presente ningún documento que emplee el PRP en inyección en el espacio epidural como vía de regeneración del disco intervertebral en los casos de patología discal. Presentamos nuestro estudio preliminar de manejo del dolor crónico en los pacientes con enfermedad discal degenerativa, mediante el uso de PRP inyectado en el espacio epidural, cervical o lumbar.

El ascenso político de China y su acercamiento hacia América Latina ha coincidido con un alejamiento de la región de Estados Unidos. A pesar de que en ciertos círculos académicos se ha explicado este fenómeno como una consecuencia de la política exterior china, en este artículo se demuestra que América Latina ha formulado su política exterior independientemente, y que el alejamiento de Estados Unidos no necesariamente significa un alineamiento con los intereses de China.
China's international political success and its growing ties with Latin America have coincided with the deterioration of the relations between Unites States and Latin America. Even if this fact has been explained by some authors as a consequence of China's foreign policy, we show that Latin America's foreign policy has been formulated independently, and that the bad moment in the relations between United States and Latin America cannot be translated as an alignment with China's interests.

O teste do amital sódico intracarotídeo (TASI ou teste de Wada) é procedimento comum na avaliação de pacientes portadores de epilepsia clinicamente refratária candidatos a cirurgia de epilepsia. Tem por objetivo promover interrupção seletiva e temporária da função hemisferial, definindo lateralização de linguagem e risco de comprometimento de memória no pós-operatório. São esperadas mudanças comportamentais durante o teste, as quais podem durar vários minutos, porém, em geral, são sutis e facilmente manejáveis. Relatamos uma série de casos em que ocorreram comportamentos pouco usuais, bizarros, incluindo agitação e agressividade. Estes comportamentos comprometem o teste (paciente deve ser contido), podendo levar a atrasos ou mesmo abortamento do mesmo, além de produzir dados menos confiáveis. Os casos foram revisados, visando a definição de preditores de sua ocorrência. Estas reações são raras (5% dos casos). Efeito barbitúrico, perfil psiquiátrico, dominância cerebral e seletividade da injeção não puderam ser validados como preditores. Explicações detalhadas, repetição e simulações podem ser utéis na prevenção deste tipo de ocorrência.
The intracarotid sodium amytal test (ISAT or Wada Test) is a commonly performed procedure in the evaluation of patients with clinically refractory epilepsy candidates to epilepsy surgery. Its goal is to promote selective and temporary interruption of hemispheric functioning, seeking to define language lateralization and risk for memory compromise following surgery. Behavioral modification is expected during the procedure. Even though it may last several minutes, in most cases it is subtle and easily manageable. We report a series of patients in whom those reactions were unusually bizarre, including agitation and aggression. Apart of the obvious technical difficulties (patients required physical restraining) those behaviors potentially promote testing delay or abortion and more importantly, inaccurate data. We reviewed those cases, seeking for features that might have predicted their occurrence. Overall, reactions are rare, seen in less than 5% of the ISAT procedures. The barbiturate effect, patients' psychiatric profiles, hemisphere dominance or selectiveness of the injection were not validated as predictors. Thorough explanation, repetition and simulation may be of help in lessening the risk of those reactions.

Neste texto, os autores apresentam os diferentes aspectos do aprendizado da cirurgia de facoemulsificação pelo cirurgião iniciante. Discutem também o papel do professor e do aluno no ambiente da "Sala de Aula Cirúrgica", sugerindo alguns princípios que melhor fundamentam o aprendizado cirúrgico, reduzindo complicações e proporcionando maior segurança para o paciente.
In this paper, the authors present different aspects of the phacoemulsification learning process by the new surgeon. They also discuss the professor's and student's role in the "Surgical Classroom", suggesting some principles to improve tutoring of the surgical learning, reducing complications and providing more safety for the patient.
