ABSTRACT Objective: The aim of this study was to evaluate the glycated hemoglobin (HbA1c) levels before and after sustained virologic response (SVR) and investigate the baseline characteristics associated with improved glycemic control in patients with chronic hepatitis C (CHC) achieving SVR after direct-acting antivirals (DAA) therapy. Materials and methods: Consecutive adult patients with CHC who achieved SVR after DAA treatment between January 2016 and December 2017 at Hospital de Clínicas de Porto Alegre (RS, Brazil) were prospectively included. Levels of HbA1c were measured up to 24 weeks before DAA therapy and 12 weeks after SVR. Exclusion criteria were decompensated cirrhosis, HIV and/or hepatitis B virus, liver disease of other etiologies, and/or modification of prediabetes/type 2 diabetes mellitus (PDM/T2DM) management. The primary outcome was a comparison of HbA1c levels before and after SVR. Secondary outcomes were the baseline variables associated with improved glycemic control. Results: The study included 207 patients with a mean age of 60.6±10.7 years, of whom 51.7% were women, 56% had cirrhosis, 37.7% had HCV genotype 3, and 54.5% had baseline T2DM or PDM. The median HbA1c level reduced significantly after SVR (5.5%, interquartile range [IQR] 4.9%-6.3%) compared with baseline (5.7%, IQR 5.3%-6.7%; p = 0.01). The baseline characteristics associated with improved HbA1c after SVR were cirrhosis, genotype 3, and age ≤ 60 years. Conclusion: Among patients with CHC, SVR after DAA was associated with HbA1c reduction, particularly in those with cirrhosis, genotype 3, and age ≤ 60 years. ABSTRACT This article has as its theme the event Portuguese class taken as an encounter (Ponzio, 2010a), as it pertains to the education of the students as readers and producers of text-utterances. The objective was to draw possible implications in the event Portuguese class from: a) the organizational setting of the administrative actions in the scope of the educational institution; b) the setting of the literacy events and practices in the scope of the study field groups; and c) literacy practices of the students that participated. The theoretical basis includes the Vigotskian ideology, the Bakhtin Circle and the studies in literacy. From the data gathered, I infer that there are two school cultures in the field of study, in ecological relations, which are, namely, culture of (un)ease in School 1 and culture of (re)affirmation in School 2. RESUMO Este artigo tem como tema o acontecimento aula de Português tomada como encontro (Ponzio, 2010a), no que diz respeito à formação dos alunos como leitores e produtores de textos-enunciados. O objetivo foi depreender possíveis implicações no acontecimento aula de Português entre: a) configuração organizacional das ações administrativas no âmbito da instituição escolar; b) configuração dos eventos e das práticas de letramento no âmbito das turmas campo de estudo; e c) práticas de letramento dos alunos participantes. O aporte teórico inclui o ideário vigotskiano, o Círculo de Bakhtin e os estudos do Letramento. A partir dos dados gerados, infere-se haver duas culturas escolares no campo de estudo, em relações ecológicas, as quais são nomeadas como cultura da (in)quietude na Escola 1 e cultura da (re)afirmação na Escola 2. ABSTRACT This study aimed to compare the development of two eucalyptus clones under the influence or not of the early animal entry into a silvopastoral system in Capixaba, Acre. The region's climate is AWI, with annual rainfall between 1,680 and 2,580 mm and a dry season between May and September. The design was entirely randomized in a 2 x 2 factorial scheme with six replications and four treatments: a) Clone I144 with cattle access at nine months (TR1); b) Clone I144 without cattle access at nine months (TR2); c) Clone VM01 with cattle access at nine months (TR3); and d) Clone VM01 without cattle access at nine months (TR4). Data was collected from the 16 central trees of each plot at 9, 12, 19, 24, 32, and 36 months of age. Total height (TH) and diameter at breast height (DBH) were measured, and the individual volume with bark from each stem per clone was obtained by rigorous cubing. Growth curves were generated as a function of age for HT, DBH, and Volume per ha using the Logistic model. Clone VM01 displayed higher survival, diameter, and volume per ha than clone I144, with higher values for these variables for the treatment without early animal entry. Clone VM01 demonstrated estimates of maximum production volume per hectare 83.8% higher than clone I144. RESUMO O objetivo deste estudo foi comparar o desenvolvimento de dois clones de eucalipto sob a influência ou não da entrada precoce dos animais em sistema silvipastoril em Capixaba, Acre. O clima da região é AWI, com precipitação anual entre 1.680 a 2.580 mm, com estação seca entre maio e setembro. O delineamento foi inteiramente casualizado em esquema fatorial 2 x 2 com seis repetições e quatro tratamentos: a) Clone I144 com acesso do gado aos 9 meses (TR1); b) Clone I144 sem acesso do gado aos 9 meses (TR2); c) Clone VM01 com acesso do gado aos 9 meses (TR3); e d) Clone VM01 sem acesso do gado aos 9 meses (TR4). RESUMO Este estudo teve como objetivo investigar a ocorrência de oocistos de Cryptosporidium e cistos de Giardia em águas brutas, filtradas e cloradas, coletadas de duas estações de tratamento de água potável (ETA A e ETA B). A ETA A utiliza filtração direta ou flotação-filtração dependendo da turbidez da água bruta. A ETA B possui duas linhas de tratamento independentes, uma de filtração direta e outra de tratamento convencional. Os oocistos de Cryptosporidium e cistos de Giardia foram concentrados por floculação, identificados por microscopia de imunofluorescência direta e confirmados por coloração com DAPI e microscopia de contraste de fase. Na água bruta, a ocorrência de cistos variou de 75% (ETA A) a 100% (ETA B) das amostras, e de oocistos de 66,6% (ETA A) a 83,3% (ETA B). Ambos os protozoários foram detectados na água tratada por filtração direta (cistos: ETA A e B < 0,27 a 20,0 cistos L-1; oocistos: < 0,48 a 22,5 oocistos L-1) e flotação-filtração (cistos: < 0,27 a 5,0 cistos L-1; oocistos: < 0,48 a 17,5 oocistos L-1) na ETA A. A ausência de cistos e oocistos na água clorada não exclui riscos, pois as limitações das técnicas de concentração e identificação devem ser consideradas, dados os baixos índices de recuperação, especialmente em água com baixa turbidez (15,5 – 72,7% de Giardia; 3,6 – 38,5% de Cryptosporidium). ABSTRACT This study aimed to investigate the occurrence of Cryptosporidium oocysts and Giardia cysts in raw, filtered, and chlorinated waters collected from two drinking water treatment plants (WTP A and WTP B). WTP A uses either direct filtration or flotation-filtration depending on the turbidity of raw water. WTP B has two independent treatment lines, a direct filtration and a conventional treatment line. Cryptosporidium oocysts and Giardia cysts were concentrated by flocculation, identified by direct immunofluorescence microscopy and confirmed by DAPI staining and phase-contrast microscopy. In raw water, the occurrence of cysts was from 75 (WTP A) to 100% (WTP B) of the samples, and of oocysts from 66.6 (WTP A) to 83.3% (WTP B). Both protozoa were detected in water treated by direct filtration (cysts: < 0.27 to 20.0 cysts L-1; oocysts: < 0.48 to 22.5 oocysts L-1) and flotation-filtration (cysts: < 0.27 to 5.0 cysts L-1; oocysts: < 0.48 to 17.5 oocysts L-1). RESUMO Introdução: O transplante de medula óssea (TMO) é o processo de substituição de uma medula óssea doente ou lesada por outra com função normal. No Brasil, as atividades assistenciais relacionadas ao cuidado direto ao paciente são supervisionadas por um enfermeiro, sendo outras atividades privativas desse profissional. Devido à complexidade desses pacientes, alguns programas de TMO podem optar por prover o cuidado de enfermagem exclusivamente por enfermeiros. Objetivos: (i) Desvelar a percepção dos enfermeiros relativa às competências exigidas durante o processo de cuidado a pacientes na unidade de TMO e (ii) identificar se os enfermeiros reconhecem incremento da qualidade em unidades de TMO que implementam o cuidado de enfermagem exclusivamente por enfermeiros. Métodos: Estudo descritivo-exploratório com método misto concorrente realizado em um hospital de grande porte da Zona Sul da cidade de São Paulo com enfermeiros atuantes do TMO no ano de 2023. Resultados: A partir da análise das entrevistas, foi possível elencar os resultados em três categorias: a) as competências necessárias para atuação do enfermeiro no TMO; b) o TMO como especialização para a enfermagem; c) relação do cuidado exclusivo do enfermeiro com os resultados no TMO. ABSTRACT Introduction: Bone Marrow Transplantation (BMT) replaces diseased or damaged bone marrow with another that normally functions. In Brazil, a nurse supervises nursing activities related to direct patient care, with other activities being exclusive to this profession. Due to the complexity of these patients, some BMT programs may choose to provide nursing care exclusively by registered nurses. Objectives: (i) To unveil nurses’ perceptions regarding the competencies required during the care process for patients in the Bone Marrow Transplantation Unit; and (ii) to identify whether nurses recognize an increase in quality in BMT units that implement nursing care exclusively by registered nurses. Methods: A descriptive-exploratory study with a concurrent mixed-methods approach was conducted in a large hospital in the South Zone of São Paulo city with nurses working in BMT in 2023. Results: From the analysis of interviews, it was possible to categorize the results into three categories: a) the competencies required for nurses in BMT; b) BMT as a specialization for nursing; c) the relationship between exclusive nursing care and outcomes in BMT. Conclusion: Five core competencies were identified: clinical reasoning, decision-making, teamwork, health education, and leadership. ABSTRACT The objective of this work was to characterize reference bacteria strains, belonging to the genus Bacillus and species of correlated genera, by simplified morphological and biochemical methods. The morphological characterization is based on the aspects of the colonies, as well as cytomorphology of the species, by optical and scanning microscopy. For biochemical characterization, the sensitivity test to antimicrobials by disk-diffusion is performed. Moreover, the strains were characterized by extracting intracellular proteins. Characteristics such as shape, color, and consistency of the colonies, in addition to the type of spore and production of protein crystals were determinants for the morphological characterization of these species. The antibiogram revealed high resistance to β-lactam group antibiotics, in species of Bacillus cereus s.l group. In Bacillus subtilis s.l. group there was high susceptibility to antibiograms, mainly for species of B. subtilis. The protein profile provided specific protein patterns for some species, mainly bands of 130 e 65 kDa for B. thuringiensis, 140 e 130 kDa for Lysinibacillus sphaericus, and 115 kDa for Brevibacillus laterosporus. ABSTRACT BACKGROUND AND OBJECTIVES: Amputation is an event that has consequences that can affect daily life, including pain, enhancing changes, whether in sleep or in quality of life. The objective of this study was to evaluate the pressure pain threshold (PPT) in people with amputations submitted to the use of different postural elevation equipment. METHODS: Experimental crossover study, carried out from September to October 2022, with people with lower limb amputation (n=15) and people without amputation (n=15). PPT in four regions (T12-L1, L5-S1, anterior tuberosity of the tibia and calcaneus) were evaluated before and after the use of different versions (A and B) of an equipment for postural elevation, gravity and pain interference. Presence of signs and symptoms of central sensitization (CS) and sleep quality. RESULTS: The groups did not present PPT alterations when compared between the different versions of the equipment (p<0.05) in the four locations analyzed. Furthermore, the groups did not show differences in relation to the evaluation days or among themselves regarding the severity and interference of pain and the presence of signs and symptoms of CS. The control group indicated poor sleep quality (p=0.0173) and remained worse than people with amputation. CONCLUSION: The versions of the equipment did not change PPT in the analyzed areas. The groups did not differ between themselves, suggesting that the equipment promoted similar responses, that is, no change in sensitivity was evidenced in the analyzed regions which have greater contact with stabilization elements and weight discharge of the equipment. RESUMO JUSTIFICATIVA E OBJETIVOS: A amputação é um evento que acarreta consequências que podem afetar o cotidiano, entre elas a dor, potencializando alterações, seja no sono ou na qualidade de vida. O objetivo deste estudo foi avaliar o limiar de dor por pressão (LDP) em pessoas com amputação submetidas ao uso de diferentes equipamentos de elevação postural. MÉTODOS: Estudo experimental cruzado, realizado de setembro a outubro de 2022, com pessoas com amputação no membro inferior (n=15) e pessoas sem amputação (n=15). Foram avaliados o LDP em quatro regiões (T12-L1, L5-S1, tuberosidade anterior da tíbia e calcâneo) pré e pós uso de distintas versões (A e B) de um equipamento de elevação postural, gravidade e interferência da dor. Presença de sinais e sintomas de sensibilização central (SC) e qualidade do sono. RESULTADOS: Os grupos não apresentaram alterações no LDP quando comparados em relação às diferentes versões do equipamento (p<0,05) nos quatro locais analisados. This work describes a green synthesis, the characterization, and biological evaluation of silver nanoparticles (AgNPs). The AgNPs suspension was synthesized using aqueous leaf extract of Eucalyptus grandis, which presented a characteristic band at 407 nm in the UV-Vis spectrum. The AgNPs presented a spherical shape and size of 9.7 ± 0.3 nm. The nanoparticles were stable over a month, indicating that E. grandis’ extract is suitable for their preparation and stabilization. The X-ray analysis showed that the crystallinity of AgNPs corresponded to the centered face phase of silver. The antibacterial and leishmanicidal activities of AgNPs were evaluated. The AgNPs presented antibacterial activity on the Gram-negative bacteria Escherichia coli at 53.9 µg mL-1. The leishmanicidal activity evaluation against promastigote forms of Leishmania infantum, Leishmania amazonensis, and Leishmania braziliensis showed that the biological response is dependent on the volume of AgNP suspension. It was demonstrated that L. infantum was more sensitive to the nanoparticle’s treatment than L. amazonensis and L. braziliensis. Abstract: Aim The objectives of this study were to evaluate which allometric measurements of Leptonema larvae are most suitable in order to develop mathematical equations to describe biomass relationships for the population of this taxon in a reference condition headwater stream. Methods We measured four body dimensions (body length, interocular distance, horizontal width of cephalic capsule and vertical width of the cephalic capsule) of 65 Leptonema larvae, which were collected in February 2022, in the Taboões spring, Serra do Rola Moça State Park, Minas Gerais, using a Surber sampler. For the determination of allometric measurements, each individual was photographed under a dissecting stereomicroscope (Leica M80) equipped with a digital camera. Each photographed specimen's length was measured using the Motic Image Plus 2.0 software. After measuring the linear body dimension and direct measurement of the biomass, we used these values to calculate the length-mass mathematical equations. To the equations use power models: DM = a Lb, where a/b are constants, DM is the dry mass, L is the linear body dimension. Results Among body dimensions of Leptonema larvae, body length showed the greatest range of variation, with values ranging from 4.03 to 25.28 mm, followed by head capsule vertical width (0.51 - 2.69 mm), head capsule horizontal width (0.55 - 2.22 mm) and interocular distance (0.24 - 1.88 mm). Our results show that body length provided the best-fitting equation for estimating biomass (R2 = 0.90). However, we observed a close fit between the other allometric measures, including high coefficients of determination, head capsule horizontal width (R2 = 0.85), interocular distance (R2 = 0.82), head capsule vertical width (R2 = 0.78). Conclusions These results will be useful in providing the best allometric measurement and equations to estimate the biomass of Leptonema larvae from the tropics. Resumo Objetivo Os objetivos deste estudo foram avaliar quais medidas alométricas do corpo das larvas de Leptonema são mais adequadas, a fim de desenvolver equações matemáticas para descrever as relações de biomassa para a população deste táxon em riachos de cabeceira. Nossos resultados mostram que o comprimento do corpo forneceu a equação de melhor ajuste para estimar a biomassa (R2 = 0,90). Abstract The pathogenesis of systemic lupus erythematosus (SLE) is complex. Few studies in Brazilian population have addressed cell phenotypes associated with immunological responses and their associations with SLE activity. The aim of this study is to investigate cell phenotypes associated to SLE diagnosis, treatment and activity. Twenty-eight SLE female patients (17 inactive, 11 active) and 10 healthy women were included in this study. Markers of natural killer (Nk), T and B cells in peripheral blood were evaluated by flow cytometry. Nkt cells were decreased only in SLE active patients. Activated CD4+, regulatory T FoxP3+ and B cells were decreased in both active and inactive SLE patients, compared to control group. The data corroborate the disruption of immune regulatory response in SLE patients and suggest phenotipic changes as possible biomarkers of SLE activity. Abstract Objectives: this study aimed to explore a set of factors associated with lower maternal-fetal attachment (MFA) in pregnant women. Methods: this is a cross-sectional study corresponding to the second wave of a cohort study with a population-based sample of pregnant women in the South of Brazil. The maternal-fetal attachment scale (MFAS) was used to measure MFA. Bivariate analysis was performed using the t-test and ANOVA. The variables that presented p<0.20 were taken for multivariate analysis, through linear regression, in order to control possible confounding factors. Results: a total of 840 pregnant women were included. Pregnant women who had lower MFA means were those who did not live with a partner (B=-3.8 [CI95%=-6.0; -1.7]), those between the first and second trimester of pregnancy (B=-4.3 [CI95%=-5.9; -2.6]), those who did not have support from their mother during pregnancy (B=-2.4 [CI95%=-4.6; -0.2]), and those with depressive symptoms (B=-4.9 [CI95%=-7.4; -2.5]). Resumo Objetivos: explorar um conjunto de fatores associados ao menor apego materno-fetal (AMF) em gestantes. Métodos: trata-se de um estudo transversal, correspondente à segunda fase de um estudo de coorte com uma amostra de base populacional de gestantes no sul do Brasil. Foi utilizada a Escala de Apego Materno-Fetal (EAMF) para medir o AMF. A análise bivariada foi realizada através do teste t e ANOVA. As variáveis que apresentaram p<0,20 foram levadas para análise multivariada, por meio de regressão linear, a fim de controlar possíveis fatores de confusão. Resultados: foram incluídas 840 gestantes. As gestantes que apresentaram menores médias de AMF foram aquelas que não moravam com um companheiro (B=-3,8 [IC95%=-6,0; -1,7]), que estavam entre o primeiro e o segundo trimestre de gestação (B=-4,3 [IC95%=-5,9; -2,6]), que não tiveram o apoio da mãe durante a gestação (B=-2,4 [IC95%=-4,6; -0,2]) e que apresentaram sintomas depressivos (B=-4,9 [IC95%=-7,4; -2,5]). Conclusões: os resultados mostraram que um maior AMF está associado a presença de uma rede de apoio adequada na gravidez, melhor saúde mental materna e a uma gestação avançada. Boron-manganese steel 22MnB5 is extensively used in structural automotive components. Knowledge about its microstructural evolution during hot stamping and resistance spot welding (RSW) is extremely relevant to guarantee compliance with application requirements. Particularly, corrosion properties are critical to the application of uncoated sheet steels. However, microstructural studies are usually simplified to top-hat geometries, which might not be fully representative of the complex thermomechanical cycles locally faced by a real component. Therefore, the present work brings an extensive characterization of a hot-stamped 22MnB5 automotive B-pillar in terms of microstructure, hardness and corrosion resistance, which were correlated with a reverse engineering of the process using numerical simulation. Physical simulations of the subcritical heat affected zone (SCHAZ) of RSW were done to assess the influence of microstructure on martensite tempering. Results showed that the component undergoes a complex strain distribution along its body during hot stamping. Most heavily strained regions presented higher amounts of ferrite, leading to poorer corrosion resistance, since ferrite behaves as an anode. Physical simulations of the SCHAZ showed that the softening degree due to martensite tempering is solely affected by peak temperature, while other microstructural features appear to exert negligible or no influence. ABSTRACT Cystic fibrosis (CF) is a genetic disease that results in dysfunction of the CF transmembrane conductance regulator (CFTR) protein, which is a chloride and bicarbonate channel expressed in the apical portion of epithelial cells of various organs. Dysfunction of that protein results in diverse clinical manifestations, primarily involving the respiratory and gastrointestinal systems, impairing quality of life and reducing life expectancy. Although CF is still an incurable pathology, the therapeutic and prognostic perspectives are now totally different and much more favorable. The purpose of these guidelines is to define evidence-based recommendations regarding the use of pharmacological agents in the treatment of the pulmonary symptoms of CF in Brazil. RESUMO A fibrose cística (FC) é uma doença genética que resulta em disfunção da proteína reguladora de condutância transmembrana da FC (CFTR), que é um canal de cloro e bicarbonato expresso na porção apical de células epiteliais de diversos órgãos. A disfunção dessa proteína resulta em manifestações clínicas diversas, envolvendo primariamente os sistemas respiratório e gastrointestinal com redução da qualidade e expectativa de vida. A FC ainda é uma patologia incurável, porém o horizonte terapêutico e prognóstico é hoje totalmente distinto e muito mais favorável. O objetivo destas diretrizes foi definir recomendações brasileiras baseadas em evidências em relação ao emprego de agentes farmacológicos no tratamento pulmonar da FC. ABSTRACT Objective To compare serum amyloid A concentrations between overweight and eutrophic children and adolescents and to relate it to lipid profiles, glucose tolerance, and carotid intima-media thickness. Methods One hundred children and adolescents (mean age: 10.8±3.16 years) were included and divided into two groups: overweight and non-overweight. The following were evaluated: Z-score body mass index, carotid intima-media thickness, lipid metabolism biomarkers (lipid profile and apolipoproteins A1 and B), inflammatory biomarkers (ultra-sensitive C-reactive protein and serum amyloid A), and glucose homeostasis model assessment of insulin resistance. Results The groups were homogeneous in age, sex, and pubertal stage. Higher levels of triglycerides, apolipoprotein B, homeostasis model assessment of insulin resistance, ultrasensitive C-reactive protein, serum amyloid A, and carotid intima-media thickness were observed in the overweight group. Abstract The frequency of gastrointestinal parasites with an emphasis on Strongylus vulgaris was investigated among the Brazilian Pony breed kept on farms in the municipality of Teresópolis, state of Rio de Janeiro. Fecal samples were collected in three stud farms: A (n= 22 animals), B (n= 3), and C (n= 2). Fecal samples were subjected to the quantitative Mini-FLOTAC technique, using three different solutions, and to qualitative techniques. The parasite prevalence was found to be 81.4%. Eggs from strongylids were identified in 74% of the ponies. Eggs of Parascaris spp. were detected in 22.7% of the animals, which were all females of farm A. At this locality, mares were kept with their foals in fenced paddocks all the time. The NaCl solution of d = 1.200 g/ml was generally the one that presented the highest frequency of diagnosis of nematode eggs and the highest mean of fecal eggs per gram. The fecal samples were also subjected to the polymerase chain reaction for amplification of DNA from the ITS2 region for Strongylus vulgaris. Twelve samples presented nucleotide sequences for S. vulgaris. In the end, this study revealed the high frequency (96.3%) of S. vulgaris among ponies on farms in Teresópolis, Rio de Janeiro, Brazil. Resumo A frequência de parasitos gastrointestinais, com ênfase na pesquisa de Strongylus vulgaris, foi investigada entre os Pôneis Brasileiros criados em haras na cidade de Teresópolis, no estado do Rio de Janeiro. Amostras fecais foram coletadas em três haras: A (n=22 animais), B (n=3) e C (n=2). Amostras fecais foram submetidas à técnica quantitativa de Mini-FLOTAC utilizando três diferentes soluções e técnicas qualitativas. A prevalência de parasitos foi de 81,4%. Ovos de estrôngilos foram identificados em 74% dos pôneis. Ovos de Parascaris spp. foram detectados em 22,7% dos animais, sendo todos fêmeas do haras A. Nesta propriedade, as éguas eram mantidas com os pôneis em piquetes cercados durante todo o tempo. A solução de NaCl, com densidade de 1.200 g/ml, foi a que apresentou a maior frequência diagnóstica de ovos de nematoides e a maior contagem de ovos por grama de fezes. As amostras também foram submetidas à reação de polimerase em cadeia para amplificar DNA da região ITS2 de Strongylus vulgaris.
