Resumo O artigo objetiva compreender a rede de atenção e os desafios que as crianças com síndrome congênita do Zika vírus (SCZ) e seus cuidadores enfrentam para garantir o cuidado integral. Trata-se de estudo qualitativo, parte de pesquisa desenvolvida no Hospital das Clínicas da Universidade Federal de Pernambuco, que acompanhou 151 crianças com suspeita para SCZ. Foram realizados dois grupos focais, um com trabalhadores de saúde e outro com cuidadores. As crianças com SCZ percorrem longas distâncias, frequentando diversos serviços da rede. Observou-se a dificuldade de condução e de reconhecimento do papel de equipe de referência da APS, resultando em um cuidado fracionado, com obstáculos à comunicação entre serviços e ausência de atenção às cuidadoras. Percebeu-se uma rede fragilizada, com encontros fragmentados e olhar reduzido. É fundamental que os esforços priorizem o fortalecimento do vínculo entre os nós da rede de atenção e apoio. Os atributos essenciais e derivados da APS são determinantes para a rede de serviços e a oferta de cuidados às crianças com a SCZ e seus familiares.

Abstract The Zika epidemic brought to the fore the birth of children with unknown and unexpected needs that demand longitudinal follow-up, strengthening the bond, comprehensiveness and coordination of care in health, which are essential attributes of primary health care (PHC). This article aims to evaluate the orientation of PHC care, using the PCATool-Brasil. The data were collected between 2016 and 2019, by means of interviews with parents of children who were born in a public maternity hospital in Recife (state of Pernambuco) between October 2015 and February 2016, a critical period of the microcephaly outbreak, especially in the Northeast of Brazil. The parents of 109 children participated in the survey, 15.6% of which had a confirmed microcephaly diagnosis. The degree of affiliation and access was sufficient, but the aspects of coordination of care, longitudinality, comprehensiveness, family and community orientation were insufficient. In the context of congenital Zika syndrome, these attributes are essential for the care of children and families. The fostering of health from these domains depends on the orientation of the models of care and their management with state and federal investments. The defense of life depends on the ability to place life above all other rationalities.

RESUMO A implantação do Sistema Único de Saúde e o reconhecimento da atenção primária à saúde (APS) como reorientadora do modelo de atenção à saúde trouxeram consigo a necessidade de novos processos de formação que incorporassem o ensinar e o aprender aos processos cotidianos de trabalho. Apresentamos um relato de experiência de implementação dos módulos de Fundamentos da Atenção Básica I e II (FABS I e FABS II), oferecidos aos estudantes de uma universidade pública do Nordeste brasileiro. Com uma estrutura fundamentada em módulos, adota-se o modelo tutorial como orientação pedagógica e parte-se do pressuposto da longitudinalidade, assegurando um contato contínuo e consistente do estudante com os temas abordados e com os serviços de APS e os demais nós que compõem a rede de saúde. Encontros semestrais potencializam a consolidação da relação ensino-serviço, propiciando a permanente avaliação dos processos vivenciados nas Unidades de Saúde, numa perspectiva reflexiva, histórica, crítica e transformadora. O primeiro módulo representa uma introdução teórico-prática à APS, e o segundo, situado no eixo de desenvolvimento profissional e compromisso social, aprofunda as noções teórico-práticas sobre a APS e a atuação do médico nesse contexto. Após cada tutoria, os estudantes vivenciam, nos encontros com e nas Unidades de Saúde da Família, os temas abordados e sua relação com o território e com aqueles que nele habitam e trabalham. No contexto das graduações em saúde, a inserção dos alunos no cotidiano da atenção primária é fundamental, considerando a indissociabilidade entre teoria e prática, a roda enquanto método democrático nas tutorias e a oferta e demanda dos temas em discussão coletiva. Ampliamos a análise sobre o papel do professor-apoiador, numa dupla tarefa de apoio aos alunos e à rede de atenção envolvida no processo formação-intervenção. A valorização da integração ensino-serviço-comunidade contribui para a formação de profissionais para o sistema de saúde e implica a defesa de diretrizes para legitimidade e sustentabilidade da atenção primária.

ABSTRACT The implementation of the Unified Health System and the recognition of primary health care as a reorientation of the health care model have brought about the need for new training processes that incorporate teaching and learning into work processes. We present an experience report on the implementation of the Basic Care Fundamentals I and II modules (FABS I and FABS II), offered to students of a public university in the northeast of Brazil. With a module-based structure, the tutorial model is adopted as a pedagogical orientation and starts from the longitudinal presupposition, ensuring the student maintains continuous and consistent contact with the topics addressed and with the PHC services and the other nodes that make up the health network. Bi-annual meetings strengthen the consolidation of the teaching-service relationship, providing a permanent evaluation of the processes experienced in the Health Units, in a reflective, historical, critical and transformative perspective. The first module represents a theoretical-practical introduction to PHC and the second module, focused on professional development and social commitment, deepens the theoretical-practical notions about PHC and the physician’s performance in this context. After each tutorial, the students experience, in the meetings with and from within the Family Health Units, the topics addressed and their relationship with the territory and with those who live and work there. In the context of undergraduate health training, involving students in the daily primary care routine is fundamental, considering the inseparable nature of theory and practice, the conversation circle as a democratic method in tutoring, and the supply and demand of the themes in collective discussion. We broadened our analysis of the role of the teacher-supporter, as a dual function to support both student and care network involved in the training-intervention process. The enhancement of the teaching-service-community integration contributes to the training of professionals for the country’s health system and implies the defense of political guidelines for the legitimacy and sustainability of primary care.
