Abstract Objectives Heart failure, especially in the elderly, is a growing global issue. This research explores the safety of human albumin infusion in treating heart failure patients with hypoalbuminemia, despite limited clinical data on its use with diuretic therapy. Methods Data were collected from the Medical Information Mart for Intensive Care III (MIMIC-III) database, which included a cohort of 6094 patients diagnosed with heart failure. Propensity score matching, logistic regression, Kaplan-Meier curves, and Locally Weighted Scatterplot Smoothing (LOWESS) curves were used to examine how albumin administration correlates with different clinical results. The evaluated results comprised in-hospital mortality, cumulative fluid output over 24 hours, Length of Stay in the Intensive Care Unit (ICU LOS), and overall hospitalization duration (hospital LOS). Results Infusing albumin was found to be markedly linked with higher in-hospital mortality among heart failure patients whose serum albumin levels were ≤ 2.9 g/dL (p < 0.001). However, no statistically significant correlation was found between the patients with levels > 2.9 g/dL (p = 0.62). Following propensity score matching in the ≤ 2.9 g/dL serum albumin group, albumin infusion was linked to prolonged hospital and ICU stays (p = 0.002, p < 0.001), but showed no significant association with total 24 hour fluid output and 90-day survival (p = 0.173, p = 0.656). Conclusion The use of albumin has been linked to increased risk-adjusted mortality during hospitalization, as well as prolonged stays in both hospital and ICU settings for patients with heart failure and low levels of serum albumin. Physicians should exercise caution when considering the administration of albumin in these cases.

From the Bernsteinian perspective, education policy interpretation and translation, as a process of recontextualising policy discourse into proposed pedagogic modes, follow certain regulative discourses and rules of social orders. This study illustrates this argument by investigating 15 discipline heads' experiences concerning their exposure to the interpretation and translation of the higher vocational education curriculum reform policy in China. The policy emphasised industry-led curricula, which is opposite to the general regulative discourse of state dominance in the Chinese education system. Given this contradiction, the government and its vocational colleges attempted to find a compromise, and thus their agents interpreted and translated the policy discourse into regionalised modes where the official education system could still control knowledge selection with limited influence from employers. It is argued that the regulative discourse in an education policy originating in realistic and economic considerations might be contrary to the general regulative discourse in official education systems, so that the policy could not be faithfully interpreted based on its own regulative discourse but had to be interpreted by largely following the general regulative discourse that is opposed to that of the policy. Thus, the policy was distorted and reoriented into a pedagogic mode that diverges from the aim of the policy.

ABSTRACT Purpose: To examine whether isoflurane preconditioning (IsoP) has a protective effect against renal ischemia/reperfusion injury (I/RI) in diabetic conditions and to further clarify the underlying mechanisms. Methods: Control and streptozotocin-induced diabetic rats were randomly assigned to five groups, as follows: normal sham, normal I/R, diabetic sham, diabetic I/R, and diabetic I/R + isoflurane. Renal I/RI was induced by clamping renal pedicle for 45 min followed by reperfusion for 24 h. IsoP was achieved by exposing the rats to 2% isoflurane for 30 min before vascular occlusion. Kidneys and blood were collected after reperfusion for further analysis. Renal histology, blood urea nitrogen, serum creatinine, oxidative stress, inflammatory cytokines, and renal cell apoptosis were assessed. Furthermore, the expression of brahma related gene 1 (Brg1), nuclear factor-erythroid 2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), and nuclear factor-κB (NF-κB) were determined. Results: Compared with control, diabetic rats undergoing I/R presented more severe renal injury, oxidative stress, inflammatory reaction, and apoptosis with the impairment of Brg1/Nrf2/HO-1 signaling. All these alterations were significantly attenuated by pretreatment with isoflurane. Conclusions: These findings suggest that isoflurane could alleviate renal I/RI in diabetes, possibly through improving Brg1/Nrf2/HO-1 signaling.

ABSTRACT The incidence of thyroid cancer is increasing globally, but mortality rates have remained steady. Many patients with thyroid cancer have low-risk, nonmetastatic intrathyroidal tumors smaller than 2 cm. Active surveillance has shown benefits in these patients, but the adoption of this approach remains below standard in Latin America. The purpose of this article is to identify ways to improve the incorporation of active surveillance into clinical practice for patients with low-risk thyroid carcinoma in Latin America, taking into consideration cultural and geographic factors. Current recommendations include three steps involving patient participation. The first step, which consists of the initial clinical examination, has eight factors requiring special attention. Anxiety must be managed while considering individual, disease-related, cognitive, and environmental aspects. Terms like "overdiagnosis", "incidentaloma," and "overtreatment" must be explained to the patient. Implementing precise terminology contributes to adequate disease perception, substantially reducing stress and anxiety. Clarifying the nonprogressive nature of thyroid cancer helps dispel myths surrounding the disease. The second step includes advice about procedures and guidelines for patients who choose active surveillance. Flexible monitoring techniques should be implemented, with regular check-ins scheduled based on patient needs. Reasons for adjusting treatment must be clearly communicated to the patient, and changes in preference regarding active surveillance should be considered in advance. The third step includes assistance during follow-up. Patients must be educated about ultrasound results and receive surgical indications from specialized physicians. The effectiveness of active surveillance can be reinforced by explaining to the patients the dynamics of changes in nodule size using clear and concise visual aids.

Abstract Background: The anthracycline chemotherapeutic antibiotic doxorubicin (DOX) can induce cumulative cardiotoxicity and lead to cardiac dysfunction. Long non-coding RNAs (lncRNAs) can function as important regulators in DOX-induced myocardial injury. Objective: This study aims to investigate the functional role and molecular mechanism of lncRNA OXCT1 antisense RNA 1 (OXCT1-AS1) in DOX-induced myocardial cell injury in vitro. Methods: Human cardiomyocytes (AC16) were stimulated with DOX to induce a myocardial cell injury model. OXCT1-AS1, miR-874-3p, and BDH1 expression in AC16 cells were determined by RT-qPCR. AC16 cell viability was measured by XTT assay. Flow cytometry was employed to assess the apoptosis of AC16 cells. Western blotting was used to evaluate protein levels of apoptosis-related markers. Dual-luciferase reporter assay was conducted to verify the binding ability between miR-874-3p and OXCT1-AS1 and between miR-874-3p and BDH1. The value of p<0.05 indicated statistical significance. Results: OXCT1-AS1 expression was decreased in DOX-treated AC16 cells. Overexpression of OXCT1-AS1 reversed the reduction of cell viability and promotion of cell apoptosis caused by DOX. OXCT1-AS1 is competitively bound to miR-874-3p to upregulate BDH1. BDH1 overexpression restored AC16 cell viability and suppressed cell apoptosis under DOX stimulation. Knocking down BDH1 reversed OXCT1-AS1-mediated attenuation of AC16 cell apoptosis under DOX treatment. Conclusion: LncRNA OXCT1-AS1 protects human myocardial cells AC16 from DOX-induced apoptosis via the miR-874-3p/BDH1 axis.

RNAs não codificantes longos (lncRNAs) podem funcionar como importantes reguladores na lesão miocárdica induzida por DOX. Objetivo: Este estudo tem como objetivo investigar o papel funcional e o mecanismo molecular do RNA antisense lncRNA OXCT1 1 (OXCT1-AS1) na lesão celular miocárdica induzida por DOX in vitro. Métodos: Cardiomiócitos humanos (AC16) foram estimulados com DOX para induzir um modelo de lesão celular miocárdica. A expressão de OXCT1-AS1, miR-874-3p e BDH1 em células AC16 foi determinada por RT-qPCR. A viabilidade das células AC16 foi medida pelo ensaio XTT. A citometria de fluxo foi empregada para avaliar a apoptose de células AC16. Western blotting foi utilizado para avaliar os níveis proteicos de marcadores relacionados à apoptose. Abstract Fast calculations are widely required in the traditional applications and the emerging digital twin fields. For those considerations, a novel physics-based fast algorithm, namely generalized rotation-superposition method, is proposed for fast calculating linear elastic responses of generalized rotationally axisymmetric structures under arbitrary mechanical loads. This improved method breaks through the limitations of the previous basic rotation-superposition method in rotational similarity of load and structural axisymmetry, and greatly expands its application scope. In this paper, firstly, the basic theory of the rotation-superposition algorithm is introduced; secondly, the theoretical model of the generalized rotation-superposition method is established; thirdly, the effectiveness and accuracy are verified by using finite element simulations; finally, through a complex case study, the applicability of the generalized rotation-superposition method for complex engineering problems and its advantages in efficiently obtaining massive amounts of data are further illustrated.

Resumen La insuficiencia cardiaca aguda es una emergencia que aumenta la mortalidad cardiovascular debido a los síntomas y signos de congestión e hipoperfusión vascular. El manejo actual con diuréticos, inotrópicos y vasopresores mejora el estado clínico; sin embargo, la morbimortalidad asociada a esta patología sigue siendo altamente significativa. Por otra parte, los inhibidores del cotransportador sodio-glucosa tipo 2 (iSGLT2) han demostrado una reducción en la hospitalización y mortalidad cardiovascular en los pacientes con insuficiencia cardiaca crónica. Por ende, el objetivo de este artículo es analizar los efectos clínicos de este grupo farmacológico en la insuficiencia cardiaca aguda, con base en investigaciones científicas. Los hallazgos de estas investigaciones demostraron un beneficio clínico de los iSGLT2 sobre esta patología, con base a parámetros como mortalidad, hospitalización, eventos clínicos, función renal, efecto diurético y concentraciones del biomarcador NT-proBnp, los cuales se detallarán en el presente artículo. Por consiguiente, se concluyó que estos fármacos son seguros sobre la evolución clínica de la insuficiencia cardiaca aguda.

Abstract Acute heart failure is an emergency that increases cardiovascular mortality due to symptoms and signs of vascular congestion and hypoperfusion. Current management with diuretics, inotropes, and vasopressors improves the clinical status; However, the morbidity and mortality associated with this pathology remains highly significant. On the other hand, sodium-glucose cotransporter 2 (SGLT2i) inhibitors have shown a reduction in hospitalization and cardiovascular mortality in patients with chronic heart failure. Therefore, the objective of this article is to analyze the clinical effects of this pharmacological group in acute heart failure, based on scientific research. The findings of these investigations demonstrated a clinical benefit of SGLT2i on this pathology, based on parameters such as mortality, hospitalization, clinical events, renal function, diuretic effect and concentrations of the NTproBnp biomarker, which will be detailed in this article. Therefore, it was concluded that these drugs are safe on the clinical course of acute heart failure.

RESUMO Os objetivos deste estudo foram analisar dados sobre a atividade física e o tempo de ruminação monitorados através de coleiras na fazenda junto com a produção de leite registrada pelo sistema rotativo de ordenha para prever vacas com base em vários distúrbios utilizando o software de regressão logística binária realizado com o software R. Dados para metrite (n=60), mastite (n=98), manqueira (n=35) e distúrbios digestivos (n=52) foram coletados de 1.618 vacas saudáveis foram usados para construir o modelo de previsão. Para verificar a viabilidade e adaptabilidade do método proposto, analisamos os dados de vacas do mesmo rebanho (rebanho 1) não utilizadas para construir o modelo, e vacas de outro rebanho (rebanho 2) com dados registrados pelo mesmo tipo de sistema automatizado, e levamos à detecção de 75,0%, 64,2%, 74,2%, e 76,9% de animais do rebanho 1 previstos corretamente para sofrer de metrite, mastite, manqueira e distúrbios digestivos, respectivamente. ABSTRACT The objective of this study was to analyze data on physical activity and rumination time monitored via collars at the farm coupled with milk yield recorded by the rotary milking system to predict cows based on several disorders using the binary Logistic regression conducted with R software. Data for metritis (n=60), mastitis (n=98), lameness (n=35), and digestive disorders (n=52) were collected from 1,618 healthy cows used to construct the prediction model. To verify the feasibility and adaptability of the proposed method, we analyzed data of cows in the same herd (herd 1) not used to construct the model, and cows in another herd (herd 2) with data recorded by the same type of automated system, and led to detection of 75.0%, 64.2%, 74.2%, and 76.9% animals in herd 1 correctly predicted to suffer from metritis, mastitis, lameness, and digestive disorders, respectively. RESUMEN Para resolver la deformación de amplia gradiente en las áreas donde no está disponible la información SAR (del inglés Synthetic Aperture Radar), se propuso un método que combina el modelo PIM Exponent Knothe (PEK) y tecnología InSAR (InSAR-PEK) para pronosticar la subsidencia inducida por la operación y obtener dinámicamente la deformación de amplia gradiente. Inicialmente, el máximo valor de subsidencia pronosticada por el método de integración de probabilidades se combinó con la información SAR y así se obtuvieron los valores de subsidencia en los períodos inicial y residual. Seguidamente, se obtuvieron tres grupos de parámetros de poder exponencial de la función Knothe que incluyen c sar y k sar , basados en información SAR; c level_wz , y k level_wz , basados en datos de nivelación en un período completo de observación, y c level_bf y k level_bf , basados en la eliminación de los datos de nivelación en el período principal. Finalmente, se compararon los valores pronosticados de los tres grupos de parámetros con la información medida y así se obtuvieron las raíces del error cuadrático medio. El ejemplo permitió verificar que las raíces del error cuadrático medio se ubican en 28.1mm~91.7mm durante el período principal y 30.9mm~58.7mm en todo el período, de acuerdo con estimaciones a través del método InSAR-PEK. Los resultados muestran que los valores de subsidencia en el período principal fueron relativamente estables, según el método InSAR-PEK, y que la precisión en el pronóstico de algunos puntos fue mejor que la de los datos de nivelación. Los valores pronosticados en el método InSAR-PEK y aquellos extraídos por información SAR se compararon con los valores medidos. En el período principal, los valores tomados con la información SAR difieren ampliamente de los valores medidos, los cuales eran valores falsos. ABSTRACT To solve large-gradient deformation in mining areas unavailable by SAR data, a method combining PIM Exponent Knothe (PEK) model and InSAR technology (InSAR-PEK) was proposed to predict the mining-induced subsidence and obtain the large-gradient deformation dynamically. Firstly, the maximum subsidence value predicted by the probability integration method was combined with SAR data, and the subsidence values in the initial and residual periods were obtained. Secondly, three groups of power exponent Knothe function parameters were obtained, including c sar and k sar based on SAR data, c level wz , and k level wz based on leveling data over a complete observation period, and c level_bf and k level_bf based on the elimination of the leveling data in the main period. Fina-lly, the predicted values of the three groups of parameters were compared with the measured data, respectively, and the root mean square errors (RMSE) were obtained. The engineering example verified that RMSEs were 28.lmm~91.7mm in the main period and 30.9mm~58.7mm in the whole period estimated by the InSAR-PEK method. The results showed that the subsidence values in the main period were relatively stable by the InSAR-PEK method, and some points' prediction accuracy was better than that of leveling data. The predicted values obtained by the InSAR-PEK method and those extracted by SAR were compared with the measured values. In the main period, the values extracted by SAR differed greatly from the measured values, which were false values. However, the predicted values by the InSAR method were close to the measured values, which can be used to independently get subsidence values in the main period from SAR data.

Hemophagocytic lymphohistiocytosis (HLH) is a syndrome caused by severe infection, tumors, or autoimmunity without a specific diagnosis. OBJECTIVE: To explore the correlation between RDW and mortality in patients with HLH. DESIGN AND SETTING: A retrospective study conducted in a hospital in China. METHODS: A total of 101 inpatients with HLH from January 1, 2017 to December 31, 2021 were divided into non-survivor (n = 52) and survivor (n = 49) groups. A non-parametric test was used to analyze demographic, clinical, and laboratory data between groups. Independent variables with P < 0.05 were analyzed using binary logistic regression to screen out mortality-related variables. Selected variables were subjected to multivariate logistic regression analysis, and those with strong correlations were screened. Receiver operating characteristic (ROC) curves of strongly correlated variables and area under curve (AUC) values were obtained. RESULTS: The APACHE II score, RDW, and platelet (PLT) and fibrinogen (FIB) levels (P < 0.05) different significantly. RDW, PLT, FIB were correlated with mortality. The AUC values of RDW, PLT, and FIB were 0.857, 0.797, and 0.726, respectively. RDW was associated with mortality in patients with HLH (P < 0.01, cut-off value: 16.9). The sensitivity and specificity of predicting mortality were 97.96% and 96.1%, respectively. CONCLUSION: Logistic regression analysis showed a correlation between RDW and patients’ mortality. Therefore, RDW can be used to predict mortality in patients with HLH.

Abstract We aimed to compare the effects of oxycodone hydrochloride and dezocine on hemodynamics and inflammatory factors in patients receiving gynecological laparoscopic surgery under general anesthesia. A total of 246 patients were divided into group A and B (n=123). Hemorheology indices were recorded 5 min after anesthesia (T0), 1 min after pneumoperitoneum (T1), when position was changed 5 min after pneumoperitoneum (T2), 15 min after pneumoperitoneum (T3), 1 min (T4) and 5 min (T5) after position was restored. Visual analogue scale scores 1, 2, 6, 12, 24 and 48 h after operation were recorded. Postoperative adverse reactions and visceral pain were observed. The expression levels of inflammatory factors were detected by enzyme-linked immunosorbent assay 12 h after operation. Compared with group A, group B had higher heart rate and mean arterial pressure at T2, lower central venous pressure and cardiac output at T1-T3, and higher systemic vascular resistance at T1-T5 (P<0.05). The incidence rate of pain syndrome in group A was lower (P<0.05). Abstract In this paper, negative poisson's ratio material is used to fill the interlayer of mine refuge chamber, and its characteristics such as light weight, thermal insulation, vibration isolation and impact resistance are used to improve the impact resistance and thermal insulation ability of mine refuge chamber. Abstract Objectives To evaluate the effects of matrix metalloproteinase (MMP) and cathepsin K (catK) inhibitors on resistance to dentin erosion. Methodology A total of 96 dentin specimens (3×3×2 mm) were prepared and randomly assigned into four groups (n=24): deionized water (DW); 1 µM odanacatib (ODN, catK inhibitor); 1 mM 1,10-phenanthroline (PHEN, MMP inhibitor); and 1 µM odanacatib + 1 mM 1,10-phenanthroline (COM). Each group was further divided into two subgroups for the application of treatment solutions before (PRE) and after erosive challenges (POST). All specimens were subjected to four daily erosive challenges for 5 d. For each erosive challenge, the specimens in subgroup PRE were immersed in the respective solutions before cola drinks, while the specimens in subgroup POST were immersed in the respective solutions after cola drinks (the immersion duration was 5 min in both cases). All specimens were stored in artificial saliva at 37°C between erosive challenges. The erosive dentin loss (EDL) was measured by profilometry. The residual demineralized organic matrix (DOM) of specimens was removed using type VII collagenase and evaluated by profilometry. Both the EDL and thickness of the residual DOM were statistically analyzed by two-way analysis of variance (ANOVA) and Bonferroni’s test (α=0.05). The surface topography and transverse sections of the specimens were observed using SEM. MMPs and catK were immunolabeled in the eroded dentin and in situ zymography was performed to evaluate the enzyme activity. Results Significantly lower EDL was found in the groups ODN, PHEN, and COM than in the control group (all p<0.05), while no significant difference in EDL was found among the groups ODN, PHEN, and COM (all p>0.05). The application sequence showed no significant effect on the EDL of the tested groups (p=0.310). A significantly thicker DOM was observed in the group ODN than in the control group regardless of the application sequence (both p<0.05). The treatment with ODN, PHEN, and COM inhibited the gelatinolytic activity by approximately 46.32%, 58.6%, and 74.56%, respectively. Conclusions The inhibition of endogenous dentinal MMPs and catK increases the acid resistance of human dentin but without an apparent synergistic effect. The inhibition of MMPs and catK is equally effective either before or after the acid challenge. Abstract Objective Abnormal complement activation is associated with periodontitis. W54011 is a novel non-peptide C5aR antagonist (C5aRA) that exhibits favorable anti-inflammatory effects in various inflammatory models. However, whether W54011 inhibits periodontitis has not yet been fully elucidated. To address this, we have investigated the probable anti-inflammatory mechanism of W54011 in LPS-treated inflammation in human gingival fibroblasts (HGFs). Methodology HGFs were isolated from healthy gingival tissue samples using the tissue block method and were identified with immunofluorescence staining. The CCK8 assay and reverse transcription-PCR (RT-PCR) were used to select the optimal induction conditions for Lipopolysaccharide (LPS) and C5aRA (according to supplementary data S1, S2 and S3). The levels of inflammatory cytokines, C5aR, and the activation of NF-κB/MAPK signaling pathways were determined by RT-quantitative PCR (RT-qPCR) and Western blotting. Results Immunofluorescence results showed that vimentin and FSP-1 were positive in HGFs and Keratin was negative in HGFs. Immunofluorescence staining demonstrated that C5aRA inhibited LPS-stimulated nuclear translocation of p-p65. RT-qPCR and Western blotting showed that C5aRA reduced the expression of IL-1β, IL-6, TNF-α, C5aR, p-p65, p-IκBα, p-JNK, p-c-JUN, and TLR4 in LPS-induced HGFs.
